Variations in practice often lead to adverse maternity outcomes following PPH. Our objective was to assess the current practice in managing PPH in the UK.Methods: We performed a national multicenter prospective service evaluation study over one calendar month and compared the current performance to national standards for managing PPH. We used a standardized data collection tool and collected data on patients’ demographics, incidence of PPH, estimated blood loss (EBL), prophylactic and treatment measures, onset of labor, and mode of delivery.Results: We collected data from 98 obstetric units, including 3663 cases of primary PPH. Fifty percent of cases were minor PPH (EBL 500–1000 mL, n=1900/3613, 52.6%) and the remaining were moderate PPH (EBL >1000 to 2000 mL, n=289/3613, 8%). The majority of women received active management of the third stage of labor (3504/3613, 97%) most commonly with Syntometrine intramuscular (1479/3613, 40.9%). More than half required one additional uterotonic agent (2364/3613, 65.4%) most commonly with Syntocinon intravenous infusion (1155/2364, 48.8%). There was a poor involvement of consultant obstetricians and anesthetists in managing PPH cases, which was more prevalent when managing major PPH (p=0.0001).Conclusion: There are still variations in managing PPH in the UK against national guidelines. More senior doctor involvement and regular service evaluation are needed to improve maternal outcomes following PPH. Abstract

Sulfonated multiblock copolymers composed of Polysulfone (PSU) and Polyphenylsulfone (PPSU) poly(ether sulfone) segments (SPSU/SPPSU) are synthesized for the first time by polycondensation in a “one-pot two-step synthesis” of commercial monomers, followed by sulfonation reaction with trimethylsilyl chlorosulfonate (TMSCS). Both segments are responsible for proton conductivity, although the PSU block has greater affinity to be sulfonated. Even though no microphase separation is detected, the resulting ionomers exhibit good mechanical properties due to the non-sulfonated blocks remaining and to the high molecular weights of the ionomers. The chemical structure is confirmed by H-NMR, F-NMR and FTIR analysis. The degree of sulfonation (0.93–1.58) is determined from the IEC values and H-NMR spectra. In situ through-plane proton conductivity measured on the MEAs is 34.1 mS cm at 70 °C. A maximum power density of 400 mW cm , a current density of 1100 mA cm and outstanding thermo-mechanical stability, these proton-conducting membranes can therefore be implemented in PEMFC.

Abstract

BACKGROUND: Common variable immunodeficiency disorders (CVID) are a group of rare innate disorders characterized by specific antibody deficiency and increased rates of infections, comorbidities and mortality. The burden of CVID in Europe has not been previously estimated. We performed a retrospective analysis of the European Society for Immunodeficiencies (ESID) registry data on the subset of patients classified by their immunologist as CVID and treated between 2004 and 2014. The registered deaths and comorbidities were used to calculate the annual average age-standardized rates of Years of Life Lost to premature death (YLL), Years Lost to Disability (YLD) and Disability Adjusted Life Years (DALY=YLL + YLD). These outcomes were expressed as a rate per 105 of the CVID cohort (the individual disease burden), and of the general population (the societal disease burden). RESULTS: Data of 2700 patients from 23 countries were analysed. Annual comorbidity rates: bronchiectasis, 21.9%; autoimmunity, 23.2%; digestive disorders, 15.6%; solid cancers, 5.5%; lymphoma, 3.8%, exceeded the prevalence in the general population by a factor of 34.0, 7.6, 8.1, 2.4 and 32.6, respectively. The comorbidities of CVID caused 8722 (6069; 12,363) YLD/105 in this cohort, whereas 44% of disability burden was attributable to infections and bronchiectasis. The total individual burden of CVID was 36,785 (33,078, 41,380) DALY/105. With estimated CVID prevalence of ~ 1/ 25,000, the societal burden of CVID ensued 1.5 (1.3, 1.7) DALY/105 of the general population. Abstract

The central Sierra Madre Oriental in Mexico developed primarily in the Late Cretaceous to early Palaeogene as a thin-skinned fold belt above a décollement hosted in Jurassic evaporite. In some regions shortening transitioned to sub-décollement structural levels, resulting in thick-skinned uplifts such as the Potosí uplift in Nuevo León. Thick-skinned deformation in the Potosí uplift involved folding of sub-décollement strata into an NNW-trending anticlinorium, cleavage development, thrust and conjugate strike-slip faulting, and extension fractures associated with barite mineralization. These structures consistently record subhorizontal shortening, directed ~52–65° in the southern uplift, ~69–72° in the northern uplift. Palaeocene to mid-Eocene zircon (U-Th)/He cooling dates record the timing of exhumation associated with thick-skinned uplift and suggest a continuation of shortening rather than a separate tectonic event. Zircon (U-Th)/He dates across the southern Potosí uplift and the Aramberri uplift, ~50 km to the south, range from ~66–53 Ma, whereas dates in the northern part of the Potosí uplift range from ~49–44 Ma. We attribute the transition to thick-skinned shortening to the mechanical strengthening of a planar décollement as rheologically weak evaporite was evacuated beneath synclinal keels of detachment folds. Along-strike differences in timing of exhumation and shortening directions may relate to differences in mechanical stratigraphy. Thicker intervals of evaporite in the northern uplift allowed thin-skinned shortening to continue while the southern uplift transitioned to thick-skinned shortening as the evaporite décollement was exhausted. As a result, stress-strain trajectories in the northern uplift refracted clockwise during continued deformation. Our findings provide new insight into the structural evolution of the Potosí uplift and may provide a framework for studying other thick-skinned uplifts in the orogen, and more generally orogenic belts that record a transition in deformation styles during progressive shortening. Abstract

Language disorder (LD) and social-emotional and behavioural (SEB) difficulties are common childhood problems that often co-occur. While there is clear evidence of these associations from clinical samples, less is known about community samples. This paper examines these associations in children aged 4-7 years from a community-based longitudinal study. 771 families provided questionnaire and assessment data at 4, 5 and 7 years. Parent-reported SEB difficulties were measured at each point (SDQ). Child language was directly assessed at 4 (CELF-P2), 5 and 7 years (CELF-4). Linear regression analysis was used to compare cross-sectional differences in mean SDQ scores between children with and without LD at each time point. Linear regression was then used to examine how patterns of language development (language disordered at three time points; never disordered; disordered at one or two time points, i.e. 'unstable' group) related to SEB difficulties at each age, adjusted for potential confounders, as in the previous analyses. Higher hyperactivity/inattention scores were associated with LD at each age. In fully adjusted models, there was little difference in mean emotional symptoms scores between children with and without LD. The 'never' LD group had lower mean SDQ scores at each time point than the 'unstable' group. Findings highlight that children with persistent LD from preschool to early primary school may be more likely to have concomitant SEB difficulties, particularly behavioural difficulties. Those with unstable LD may also have co-occurring SEB difficulties, showing a need for education and health professionals to monitor early language and SEB development.

Abstract

The ocean is nearly transparent to low frequency sound permitting the observation of distant events such as earthquakes or explosions at fully basin scales. For very low frequency the ocean acts as a shallow-water waveguide and lateral variability in bathymetry can lead to out-of-plane effects. In this paper, data from the International Monitoring System of the Comprehensive Test Ban Treaty Organization (CTBTO) is used to present two cases where robustly localized seismic events in locations clearly within the two-dimensional (2-D) shadow of a continent or large island generate T-phase signals that are received on a hydro-acoustic station. A fully three- dimensional parabolic equation model is used to demonstrate that lateral variability of the bathymetry can lead to diffraction, explaining both observations. The implications of this are that the CTBTO network has greater coverage than predicted by 2-D models and that inclusion of diffraction in future processing can improve the automatic global association of hydroacoustic events. © 2017 Acoustical Society of America.

Abstract

1,4-Dinitro-2-methylpyrrole (NMP), resulting from the reaction between sorbic acid and nitrite, has mutagenic properties and its alkylating capacity is examined. The calculated energy barrier for the alkylation of 4-(p-nitrobenzyl)pyridine (NBP) for NMP and the value of the fraction of alkylating agent forming the adduct are consistent with mutagenicity of NMP.

Abstract

This paper is the review of the Consensus Document on Intermittent Claudication of the Central European Vascular Forum (CEVF), published in 2008, and and shared with the North Africa and Middle East Chapter of International Union of Angiology and the Mediterranean League of Angiology and Vascular Surgery. The Document presents suggestions for general practitioners and vascular specialists for more precise and appropriate management of PAD, particularly of intermittent claudication, and underlines the investigations that should be required by GPs and what the GP should expect from the vascular specialist (angiologist, vascular surgeon). The idea of the Faculty is to produce a short document, which is an easy reference in daily clinical practice, both for the GPs and vascular specialists.

Abstract

The reactivity of amino acids with an -NH2 group in nitrosation reactions and its relationship with the alkylating potential of their products is investigated. It was observed that the species resulting from the nitrosation of these amino acids are the corresponding lactones. The investigation was carried out to gain insight into the formation of gamma-hydroxy butyric acid (GHB) in the hydrolysis of gamma-butyrolactone.
