Objetivo: Descrever de forma crítica todo o processo de aquisição e desenvolvimento de competências comuns e específicas do enfermeiro especialista em EMC-PSC. Metodologia: Método descritivo e critico-reflexivo com recurso à pesquisa bibliográfica, bases de dados científicas, descrição e análise das atividades realizadas e desenvolvidas ao longo do Estágio Final. Recorremos ainda à Metodologia de Projeto para desenvolver o Projeto de Intervenção Profissional sustentado pelo Modelo de Aquisição de Competências de Patricia Benner. Resultados: O processo de aquisição de competências refletiu todo o percurso realizado ao longo do Estágio Final no Serviço de urgência de um HP. O desenvolvimento do Projeto de Intervenção Profissional possibilitou promover a segurança e qualidade dos cuidados prestados à Pessoa em Situação Crítica através da elaboração de um programa de integração para os novos enfermeiros. Conclusão: Foram descritas e analisadas as atividades desenvolvidas com enfoque na aquisição e desenvolvimento das competências. A implementação de programas de integração facilita a adaptação do enfermeiro ao seu novo contexto profissional com repercussões na melhoria na qualidade e segurança dos cuidados.

What does composing music mean today? What is the role of the composer in today’s musical world? Can we continue to talk about “composing music” in every situation of musical creation? Or should we consider recourse to other expressions? What is the composition process in the case of museum exhibitions, or installations? Can a sound artist be considered a composer of music? Is the creation of sound art a form of musical composition? Furthermore, can real-time coding or free improvisation be considered forms of instantaneous musical composition? In a world where technological means make musical creation accessible to all, what is the role of the “traditional composer”? Music is today more diverse than ever before. The range of genres, practices, techniques and technologies, forms of dissemination and reception have changed the way in which music is composed. Music is now almost omnipresent in our society spanning concert halls, museums, digital media, as well as public and private spaces. For each of these listening situations, someone conceives and composes the music, creating the sound and organizing the musical discourse through means of a diversity of approaches, knowledge and technologies. A local drug delivery system that attempts to find a suitable balance between antimicrobial and regenerative actions was developed for osteomyelitis treatment (OM). This system combines the angiogenic and immunomodulatory peptide LLKKK18 (LL18) and vancomycin hydrochloride (VH), loaded into an injectable oxidized dextrin (ODEX)-based hydrogel (HG). In vitro cytotoxicity was analyzed in MC3T3-E1 pre-osteoblasts and erythrocytes. The kinetics of LL18 release was studied. Antimicrobial activity was assessed in vitro against a clinical Methicillin-Resistant Staphylococcus aureus (MRSA) strain. A rat model of acute OM was developed by direct inoculation into a tibia defect, concomitantly with the implantation of the drug-loaded HG. The local bioburden was quantified and damage in surrounding tissues was examined histologically. In vitro, ODEX-based HG displayed a safe hemolytic profile. Half of LL18 (53%) is released during the swelling phase at physiological pH, then being gradually released until complete HG degradation. LL18-loaded HG at 300 μM was the most effective peptide formulation in decreasing in vivo infection among concentrations ranging from 86 to 429 μM. The histopathological scores observed in vivo varied with the LL18 concentration in a dose-dependent manner. VH at 28 mM completely eradicated bacteria, although with substantial tissue injury. We have found that sub-millimolar doses of VH combined with LL18 at 300 μM may suffice to eradicate the infection, with reduced tissue damage. What does the expression “old is new” mean in music studies? The discovery of novelty in past music? Or the recovery of old practices in new styles? In reading this volume, it is fascinating to find out the extraordinary variety of formulations that this expression can produce. But these answers above already show a subtle connection. What is the use of an old practice in a new piece, if not the acknowledgment of a newness in an old style? Can we say then that “old is new” may open to any historical connection between musical activities? Certainly, but not only this. If with “old” we mean “obsolete” or “abandoned”, we could think of an act of past recovery, but if “old” suggests attributes such “stereotyped” or “conventional”, we are possibly considering a socio-cultural perspective instead of an historical one. Development of chronic CSC is an indication to treat, and ICGA-guided verteporfin photodynamic therapy has been shown to be very effective. Our objective was to evaluate the 13-year follow-up of chronic CSC patients treated with standard-fluence PDT and to explore the long-term microstructural and vascular choroidal changes related to the disease and treatment. Methods: Retrospective, interventional case-series analysis was conducted on 18 patients with cCSC, treated with standard PDT. Evaluations were performed every 3 months in the first year, every 6 months during the second year, and annually thereafter. All participants underwent a comprehensive ophthalmic examination. Retinal and choroidal imaging was performed with SD-OCT, SS-OCT, Optomap and OCTA. Results: Twenty-three eyes of eighteen patients were included, with a mean age of 63.9��8.5 years. The mean follow-up was 15.8��1.4 years. The mean number of sPDT treatments was 1.2��0.4. At baseline, subretinal fluid was found in all eyes, RPE proliferation in 9.1% and atrophy in 9.1% of the eyes. O nosso objectivo foi avaliar a eficácia e segurança da TFD standard com verteporfina, durante um período de seguimento de 13 anos, descrevendo as alterações microestruturais e vasculares a longo prazo. Métodos: Procedeu-se a um estudo retrospetivo de doentes com CSC crónica, tratados com TFD standard. As avaliações periódicas foram realizadas a cada 3 meses no primeiro ano, cada 6 meses no segundo e, posteriormente, anualmente. Todos os participantes realizaram um exame oftalmológico completo. A imagiologia da retina e da coróide foi obtida recorrendo a abordagem multimodal com SD-OCT, SS-OCT, Optomap e OCTA. Resultados: Foram incluídos 23 olhos de 18 doentes, com uma idade média de 63,9±8,5 anos. O período médio de follow-up foi 15,8±1,4 anos. O número médio de tratamentos de TFDs foi de 1,2±1,4. No início do estudo, o fluído subretiniano estava presente em todos os olhos, a proliferação do EPR em 9,1% e atrofia em 9,1% dos olhos. O nacionalismo catalão pode ser entendido como expressão de uma das três nacionalidades identificadas em Espanha (Catalunha, País Basco, Galiza). A evolução do nacionalismo catalão sofreu mutações políticas que o transformaram de um nacionalismo cultural e/ou linguístico para um nacionalismo mais claramente independentista ou separatista. Este trabalho pretende (1) analisar os conceitos de nacionalismo cultural e nacionalismo separatista/independentista e como se adequam ao caso espanhol e catalão, (2) enumerar os principais fatores políticos e institucionais que provocaram alterações na natureza do nacionalismo catalão, nomeadamente a Constituição de 1978, o processo de revisão do Estatuto de Autonomia, iniciado em 2006, e a consequência prática da decisão do Tribunal Constitucional de 2010 e (3) perceber os resultados das alterações no nacionalismo catalão para o cenário político do referendo de 1 de outubro de 2017 na Catalunha e consequente polarização política. The catalan nationalism can be understood as an expression of one of the three nationalities identified in Spain (Catalunya, Basque Country and Galiza). The evolution of the Catalan nationalism suffered political mutations that transformed it from a cultural and/or linguistic nationalism towards a more clearly independentist or separatist nationalism. This thesis intends to (1) analyse the concepts of cultural and/or linguistic nationalism and separatist/independentist nationalism and how they apply to the Spanish and Catalan case, (2) enumerate the main political and institutional factors that caused changes in the nature of the Catalan nationalism, namely the Constitution of 1978, the process of revision of the statute of autonomy that began in 2006 and the practical consequences of the decision of the Constitutional Court in 2010 and(3) understand the results of the changes in Catalan nationalism under the political scenario of the referendum of October 1 of 2017 in Catalunya and consequent political polarization

Importance: Polypoidal choroidal vasculopathy (PCV) is far less common and studied in a Caucasian population than in an Asian population, and the optimal treatment approach remains to be confirmed. Methods: A 52-week, double-masked, sham-controlled, phase 4, investigator-initiated randomized clinical trial (RCT) in naive symptomatic Caucasian patients with PCV treated with aflibercept in a treat-and-extend regimen (T&E) (intravitreal aflibercept injection [IVAI] T&E). Patients were randomized at week 16 to receive IVAI T&E plus either sham photodynamic therapy (PDT) or standard fluence PDT with verteporfin. The main outcome measures were changes in best-corrected visual acuity (BCVA) from baseline to 52 weeks and polyp occlusion at week 52. Data are presented as median (interquartile range [IQR]) for BCVA, number of IVAI, and change in central retinal thickness (CRT). Results: Of the 50 patients included in the study, 48 patients completed the 52 weeks of follow-up. During this period, a significant median (IQR) BCVA gain of 6 [2-12] Early Treatment Diabetic Retinopathy Study letters was observed for all patients (p < 0.001), after 8 (7-9) injections, with a significant reduction of -93.0 [-154.0, -44.0] mu m in central macular thickness (p < 0.001). Using indocyanine green angiography, a complete occlusion of polypoidal lesions was documented in 72% of the cases. Still, no significant difference was detected between the sham PDT and the aflibercept PDT arms, at week 52, for BCVA change (6.5 [2-11] vs. 5 [2-13] letters (p = 0.98)), number of IVAIs (8.5 [7-9] vs. 8 [7-9] (p = 0.21)), change in CRT (-143 [-184; -47] vs. -89 [-123; -41.5] mu m [p = 0.23]), and rates of complete polyp occlusion: 77 versus 68% (p = 0.53) or presence of fluid: 68 versus 57% (p = 0.56). No serious ocular adverse events were registered in the 2 arms. Conclusions and Relevance: To our knowledge, this is the first RCT to compare aflibercept T&E monotherapy with aflibercept T&E plus verteporfin PDT in a Caucasian population with PCV. Aflibercept monotherapy in a T&E showed to be effective and safe with a significant median BCVA improvement of 6 letters and a complete occlusion of polypoidal lesions in near 3 quarters of the eyes, at 1 year. As only 22% of the eyes underwent PDT treatment, the benefit of combined treatment for PCV in Caucasian patients could not be definitively elucidated from this study.

Portugal tem assistido, particularmente desde 2015, a um fluxo migratório oriundo de países como a Síria e a Eritreia, o qual tem exigido um aprofundamento do conhecimento sobre o processo de acolhimento, contribuindo assim para que ele seja cada vez mais eficaz. Neste processo, a língua apresenta-se como componente fundamental para a integração e subsequente autonomização de pessoas que chegam ao país e que requerem/beneficiam de proteção internacional. Com efeito, o recente estudo do Observatório das Migrações recomenda que o Plano Nacional de Acolhimento e Integração determine a especificidade de um programa de língua portuguesa, conducente à aprendizagem e à formação. Os cursos de Português para Falantes de Outras Línguas (PFOL), criados pela Portaria n.º 1262/2009, de 15 de outubro, mostravam-se desajustados, impedindo uma resposta eficaz às necessidades individuais dos destinatários. O caráter desenquadrado daqueles cursos levou a que a tutela apresentasse alterações ao quadro normativo, criando os cursos de Português Língua de Acolhimento através da publicação da Portaria n.º 183/2020, de 5 de agosto. Esta comunicação descreve um estudo de caso que incidiu num curso PFOL (150 horas) de uma escola transmontana, durante o período de transição (novembro de 2020 a maio de 2021) entre os quadros normativos referidos, para a obtenção da certificação de nível A2 do Quadro Comum Europeu de Referência. Os sujeitos envolvidos neste estudo constituíram uma turma heterogénea não só quanto aos países de origem (Eritreia, Paquistão, Croácia, Moldávia), como também pelas diferenças culturais, religiosas, linguísticas e pelos seus diferentes níveis de competências digitais. Neste âmbito, poderá o design thinking aliado às tecnologias digitais contribuir para a aprendizagem da língua? Para responder a esta questão desenvolveu-se uma investigação empírica que recorreu a várias técnicas de pesquisa de dados: pesquisa documental e observação, incluindo as atividades de codiagnóstico. Destaca-se que a análise do diário do investigador apontou para propostas de atividades de ensino do português L2 e de didática da língua, baseadas na utilização de técnicas de Design Thinking. O Design Thinking apresenta-se simultaneamente como um método e uma ferramenta capaz de contribuir para a resolução de problemas, numa prática participativa e colaborativa, aplicados ao contexto e conteúdos educativos, com o recurso a tecnologias digitais. Portugal has observed, particularly since 2015, a migratory flow from countries such as Syria and Eritrea, which has required a deepening of knowledge about the reception process, contributing to make it increasingly effective. In this process, language presents itself as a fundamental component for the integration and subsequent empowerment of people who arrive in the country and who require/benefit from international protection. Indeed, the recent study by the Migration Observatory recommends that the National Plan for Reception and Integration determines the specificity of a Portuguese language program conducive to learning and training. The Portuguese for Speakers of Other Languages (PSOL), created by Ordinance No. 1262/2009, of 15 October, proved to be inadequate, preventing an effective response to the individual needs of the beneficiaries. The non-compliant nature of those courses led the tutelage to present changes to the normative framework, creating the courses of Portuguese Host Language through the publication of Ordinance No. 183/2020, of 5 August. This communication describes a case study that focused on a PSOL course (150 hours) at a school in Trásos- Montes, during the transition period (November 2020 to May 2021) between the aforementioned normative frameworks, in order to obtain the A2 level certification of the Common European Framework of Reference. The subjects involved in this study constituted a heterogeneous class not only in terms of their countries of origin (Eritrea, Pakistan, Croatia, Moldova), but also because of cultural, religious, linguistic differences and their different levels of digital skills. In this context, can design thinking combined with information and communication technologies contribute to language learning? To answer this question, an empirical investigation was developed that used several data search techniques: documentary research and observation, including co-diagnostic activities. It is noteworthy that the analysis of the researcher's diary pointed to proposals for Portuguese L2 teaching activities and language didactics, based on the use of Design Thinking techniques. Design Thinking presents itself simultaneously as a method and a tool capable of contributing to the resolution of problems, in a participatory and collaborative practice, applied to the context and educational content, using digital technologies. info:eu-repo/semantics/publishedVersion
