101. Photo- and thermally induced interdiffusion in Se/As2S3 nanomultilayers prepared by pulsed laser deposition and thermal evaporation

Abstract: The effects of light- and thermally stimulated interdiffusion in Se/As2S3 nanomultilayers (NML) with modulation period of 6–9 nm fabricated by pulsed laser deposition (PLD) and by the method of cyclic thermal evaporation (CTE) of the Se and As2S3 components were investigated by means of direct low-angle X-ray diffraction (LAXRD) measurements and by indirect methods of optical measurements. More pronounced nano-granular structure is characteristic for PLD samples but at the same time the interfaces are essentially sharpened, the periodicity and the initial blue-shift of the optical absorption edge are better in comparison with CTE samples. Somewhat less efficient interdiffusion and changes of optical parameters i.e. amplitude-phase recording in PLD samples in comparison with CTE ones may be caused by different deposition conditions, ratio of sub-layer's thickness and their structure, eventually by changes in chemical composition.

Abstract: The main aim was to gain structured insight into the use of musculoskeletal ultrasonography (MSUS) in routine rheumatology practices in Central and Eastern European (CEE) countries. In a cross-sectional, observational, international, multicenter survey, a questionnaire was sent to investigational sites in CEE countries. Data on all subsequent routine MSUS examinations, site characteristics, MSUS equipment, and investigators were collected over 6 months or up to 100 examinations per center. A total of 95 physicians at 44 sites in 9 countries provided information on a total of 2810 MSUS examinations. The most frequent diagnoses were rheumatoid arthritis (RA) and spondyloarthritis (34.8 and 14.9 % of cases, respectively). Mean number of joints examined was 6.8. MSUS was most frequently performed for diagnostic purposes (58 %), particularly in patients with undifferentiated arthritis, suspected soft tissue disorders, or osteoarthritis (73.0–85.3 %). In RA patients, 56.3 % of examinations were conducted to monitor disease activity. Nearly all investigations (99 %) had clinical implications, while the results of 78.6 % of examinations (51.6–99.0 %) were deemed useful for patient education. This first standardized multicountry survey performed in CEEs provided a structured documentation of the routine MSUS use in participating countries. The majority of MSUS examinations were performed for diagnostic purposes, whereas one-third was conducted to monitor disease activity in RA. A majority of examinations had an impact on clinical decision making and were also found to be useful for patient education. Abstract: Objectives: The systemic inflammatory response syndrome (SIRS) after cardiac surgery with cardiopulmonary bypass (CPB) exacerbates organ dysfunction and increases postoperative mortality. The aim of this study was to reduce SIRS after CPB in a pig model by profoundly decreasing all blood defence factors (complement, coagulation and fibrinolytic and contact systems, leukocytes and thrombocytes) using pre‑operative aphaeresis. Methods: Thirty-three pigs underwent 3 h of hypothermic CPB with 2 h of cardioplegic arrest, followed by 4 days of observation. One half of the sample underwent prebypass plasma-thrombo-leukocyte aphaeresis with the adjuvant leukofiltration. Results: In the control group, there were classical signs of SIRS (tachycardia, tachypnea and leukocytosis) postoperatively. There was also myocardial ischaemia and the need for inotropic support in 90% of the control animals. Neutrophils showed an increase in superoxide anion production (P < 0.001), and surface neutral protease activity (P < 0.001) and blood endotoxin levels increased (P < 0.01) compared with preoperative levels. In contrast, in the aphaeretic group, there were no classical signs of SIRS; no myocardial ischaemia; minimum neutrophil production of the superoxide anion and protease activity were recorded (P < 0.001); and endotoxin levels were also decreased (P < 0.05) compared with the controls. In the control group, the haemodynamic problems associated with disconnecting from CPB correlated with the histologic findings in the myocardium (leukocyte endothelial adhesion and leukodiapedesis). Conclusions: Pre‑operative plasma‑thrombo-leukocyte aphaeresis significantly reduces the major symptoms of SIRS and organ dysfunction after 3 h of CPB without adverse effects, such as bleeding and infection, during the postoperative course.

105. Amorphous Ge-As-Te Thin Films Prepared by Pulsed Laser Deposition - A Photostability Study

Abstract: Pulsed laser deposition was used for the fabrication of amorphous thin films from Ge-As-Te system with the aim to study their intrinsic photostability. Photostability of prepared layers was studied using spectroscopic ellipsometry within as-deposited as well as relaxed layers. For irradiation, laser sources operating at three energies (1.17, 0.92 and 0.8 eV) in band gap region of the studied materials were employed. The lowest values of photorefraction (refractive index changes) accompanied with lowest changes of band gap values present Ge20As20Te60 thin films, which are therefore considered as the layers with highest photostability, especially in relaxed state.

107. Kinetics of Photosensitivity in Ge-Sb-Se Thin Films

Abstract: Chalcogenide (GeSe2)100−x(Sb2Se3)x thin films obtained using pulsed laser deposition are exposed to light with energy close to band gap energy, in order to investigate kinetics of photoinduced phenomena. It appears that a reversible transient photodarkening is observed. The metastable part of photodarkening, which seems to be slower, is followed by photobleaching. A modelling of the evolution of transmission during illumination suggests that each process has an independent effective time constant, and that magnitude of photoinduced phenomena depends on various parameters, such as laser's fluency, absorption coefficient and composition.

Abstract: Background Rupture of a papillary muscle is an infrequent but often fatal mechanical complication of acute myocardial infarction. Aim The aim of this paper is to present a case report of a 65-year old women with acute severe mitral regurgitation with cardiogenic shock caused by two-step complete anterior papillary muscle rupture during acute myocardial infarction. The transthoracic echocardiography was obtained at the bedside and showed a posterior mitral valve prolapse with a severe mitral regurgitation. During this examination the patient developed acute pulmonary oedema and a consequent cardiogenic shock. Transthoracic echocardiography was then reevaluated and completed. New findings of bi-leaflet mitral flail and progression of massive mitral regurgitation were documented. The complete rupture of a papilary muscle was then considered as a cause of an acute clinical deterioration. Intraoperative findings showed a complete transection of both heads of anterolateral and necrotic regions of basis of posteromedial papillary muscle. Conclusions This case confirms the importance of immediate echocardiography in confirming a diagnosis of acute mechanical complications of acute coronary syndromes and this examination is important for the management of a hemodynamically unstable patient. Echocardiography should be done immediately on any patient in whom the diagnosis of mechanical complication of acute coronary syndromes is suspected.

Abstract: Amorphous films of Ag x (Sb 0.33 S 0.67 ) 100− x composition, where x was between 0 and 25 at.% Ag, were prepared by pulsed laser deposition and optically induced silver dissolution into the binary Sb-S chalcogenides deposited by vacuum evaporation techniques. We have studied the effect of silver content in the host materials and Ar + ion laser exposure on the optical properties and structure of the films. Optically induced crystallization was demonstrated in films and the kinetics of the crystallization were studied. This work is anticipated to be applicable to optical memories.

Abstract: Principles, advantages, applications and drawbacks of pulsed laser deposition (PLD) technique for thin films preparation are reviewed. The PLD method is promising for preparation of thin films of complex composition, including rare-earth and Ag-doped chalcogenide films; all components of the target can be evaporated at once. Low volatility and refractory materials can be also deposited. The deposition in vacuum, inert or reactive atmosphere is possible. Results obtained in a study of chalcogenide films are discussed and the current state-of-the-art is reviewed. The composition and structure of PLD films can be different from thermally evaporated films and new materials or materials with new properties applicable in optics and optoelectronics can be prepared. The method can be used for fabrication of different chalcogenide-based sensors and memory materials of complex composition. Photoinduced changes of structure and properties of PLD films and chalcogenides exposed to intense laser pulses are also discussed. The intense laser pulses can change the properties of the materials prepared and can be used for fabrication of chalcogenide-based waveguides, diffractive elements and high-density optical data storage media.

111. Newly described anatomical opening on forelimb tendon in the artiodactyls and its relation to knee clicks

Abstract: Abstract To understand which morphological/anatomical parts may be responsible in artiodactyl ungulates for the clicking sound made when moving, this research focuses on the forelimb tendon apparatus where an undescribed opening in the fibrous cuff (manica flexoria), called hereafter for its shape as an “oval window” in the manica flexoria (OWMF), was detected. This oval window was found in 24 of the 25 species of four families (Camelidae, Giraffidae, Cervidae, and Bovidae) evaluated; the exception being in Bos taurus taurus (Domestic cattle). The length and width of the OWMF enabled correct species discrimination between the majority of species, but remained conservative intraspecifically, as it did not differ between the left and right side of the forelimb, third and fourth digits, or between sexes. When evaluating the shape of OWMF in individual species, and measuring its length and width, 18 out of the 24 species investigated had this window as an oval shape, the remaining 25% of species exhibited more oval-oblong shapes with either proximal or distal asymmetry. The function of the OWMF in the thoracic autopodium of most ruminant even-toed ungulates is not yet fully understood. Its most likely function is to help balance the pressure inside the ligament cuff and reduce the friction of the touching surfaces of the muscle tendons—thus facilitating the movement of the digits when walking. None of the absolute or relative OWMF parameters fit exclusively with the occurrence and distribution of knee-clicks produced by some bovids and cervids during movement, so the mechanism responsible for this sound remains cryptic from the present anatomical perspective.

Abstract: Thin amorphous films of Ag-As-S and Ag-Sb-S systems have been prepared by pulsed laser deposition (PLD) at five different conditions, i.e. at different pulse energies and pulse repetition intervals of the KrF laser. The obtained films were analysed by RBS, ERDA (elastic recoil detection analysis) spectral analysis and also EDXA elemental analysis. The results of RBS and EDXA analysis were compared to the composition of the source bulk glass materials. Film compositions varied compared to the source material according to the deposition condition with film composition close to the stoichiometric one, i.e. AgAsS 2 and AgSbS 2 could be prepared by the PLD technique. RBS spectroscopy is known as an important tool for establishing depth distribution of the elements within the prepared films. The thickness of the films was chosen so that depth profiling of the entire layer is possible. ERDA allowed us to find, apart from all obvious atoms (Ag, As, Sb, S), also H atoms present in the films. The structure of the films has been studied by Raman spectroscopy as well. Vibration bands characteristic of As(Sb)S 2 -Ag-As(Sb)S 2 structural units present in prepared films have been observed. The films are potentially applicable for optical memories (e.g. digital video discs).

Abstract: International audience; The 3-15 μm range is a key region for a large number of applications in diverse areas such as biology and medicine, molecular spectroscopy, ground based and space borne environmental monitoring but also an important issue of instrumentation for astronomy and astrophysics to achieve complex but very reliable instruments. Going to the mid IR region is crucial for the observation of warm objects around 300 K and the characterization of their spectral features, considered as signatures for biological activity (e.g. water, ozone, carbon dioxide). Most infrared signatures or "fingerprints" (fundamental transitions) of organic species and biomolecules are essentially located in this spectral window. Thus, infrared spectroscopy is a powerful tool for detecting and determining the composition of complex samples; it is simple, reliable, fast, cost-efficient and non-destructive method. Then, it is crucial to be able to work at the extension toward thermal IR waveguides, where chalcogenide glasses provide promising properties, specifically their extended optical transmission window from the visible to the far infrared (up to 20 μm)

Abstract: Background Repolarization processes in female and male are different. This study provided pilot data on automatic measurements of QT intervals in heart transplant (HT) recipients stratified according to the sex of the recipient and the donor. Methods and results The following groups were analyzed: Group A—20 males with male heart, group B—14 females with male heart, group C—13 females with female heart, group D—11 males with female heart, group E—20 healthy males, and group F—20 healthy females. Twelve-lead electrocardiograms were digitally captured during autonomic provocative test of five postural 8-minute stages—supine, unsupported sitting, supine, unsupported standing, and supine. Fridericia formula was used for heart rate correction together with a generic correction for QT/RR hysteresis. Neither female nor male HT recipients exhibit any differences in QTc interval duration related to the sex of the donor. There was, however, a trend towards longer QTc intervals in female HT recipients compared to male HT recipients irrespective of the sex of the donor. The QTc differences between healthy control females and males were highly statistically significant proving the assay sensitivity of the study. Conclusion The available pilot data suggest that in HT patients, the sex of the donor has little influence on the QTc interval of the transplanted heart.

Abstract: International audience; Amorphous chalcogenide thin films are of high current interest for technol. applications as optical storage media or waveguides for photonic integrated circuits. As part of a larger project including fs, ps and ns pulsed laser deposition regimes, Er- and Pr-doped GLS thin films were deposited by ns PLD, and their structural, chem. and optical properties were analyzed by optical and electronic microscopy, stylus profilometry, X-ray diffraction, Raman spectroscopy, time-of-flight secondary ion mass spectrometry (TOF-SIMS), energy-dispersive X-ray spectroscopy, variable-angle spectroscopic ellipsometry and optical transmission. Films deposited at moderate fluence (∼4 J/cm2) in UV (266 nm) presented a good surface quality, while exhibiting acceptable compn. uniformity and deviations from stoichiometry in line with the literature. Compn. and optical properties dependences on the deposition conditions were investigated and discussed with respect to previous studies on similar systems.

Observed phenomena can be used for the one-step fabrication of micro-optical elements.

Abstract: Aim: An ageing population and increase in patient co-morbidities are forcing cardiac surgeons to meticulously consider the benefi ts and risks of respective conduits and their harvesting techniques. Case reports: Two cases of simultaneous endoscopic radial artery and great saphenous vein harvesting, for redo coronary artery bypass grafting, are presented. A shortage of venous conduits after previous bypass grafting, as well as the presentation of several risk factors of wound-healing complications, favoured simultaneous utilisation of both endoscopic techniques. Conclusion: Endoscopic vessel harvesting together with the pre-harvesting duplex study is able to gain not only high-quality conduits but also minimize the risk of wound-healing and neurological disturbances associated the saphenous vein and radial artery harvesting.

Abstract: High purity (GeS2)8−x (Sb2S3)20(NdCl3)x x=0, 0.01, 0.1, 0.5, glasses were prepared and their optical properties determined. The Ge-Sb-S system dissolves up to 0.5 mol.% of NdCl3 and still forms stable glasses. The structure of these glasses is formed by interconnected GeS4 tetrahedra and SbS3 pyramids as it follows from the Raman spectra. The glasses are optically well transparent in the range from 15400 cm−1 to 1000 cm−1. Doping with Nd creates new absorption bands which can be assigned to electron transfer from the 4 I 9/2 level to 2 G 5/2, 2 G 7/2, 2 H 11/2, 4 F 9/2, 4 F 7/2, 4 S 3/2, 2 H 9/2, 4 F 5/2, 4 F 3/2, 4 I 13/2 and 4 I 11/2 levels. The oscillator strengths and Judd-Ofelt parameters were evaluated. Their values are close to the values of those for Nd3+1 in another chalcogenide hosts. The long-wavelength absorption edge was found near 1000 cm−1 and is due to multiphonon Ge-S and Sb-S vibrations. In doped glasses, several broad luminescence bands near 890, 1080, 1370, and 1540 nm were found. They can be attributed to the transitions from 4 F 3/2 to 4 I 9/2, to 4 I 11/2, to 4 I 13/2 and 4 I 15/2 electron levels. The first luminescence band was excited also by 1064-nm line and represents probably the upconversion of light.

124. Comparison of bicuspidization and Ross procedure in the treatment of unicuspid aortic valve disease in adults – Insight from the AVIATOR registry

Abstract: BackgroundUnicuspid aortic valve (UAV) is the second most common underlying cause of aortic valve dysfunction in young adults after the bicuspid valve. The valve may be replaced (for example by pulmonary autograft) or repaired using the bicuspidization technique. The aim of our study was to compare short- and mid-term outcomes of Ross procedure with bicuspidization in patients with severe UAV dysfunction.MethodsThis was a multi-center retrospective observational cohort study comparing data from two dedicated Ross centers in the Czech Republic with bicuspidization outcomes provided by AVIATOR registry. As for the Ross group, only the patients with UAV were included. Primary endpoint was mid-term freedom from reintervention. Secondary endpoints were mid-term freedom from major adverse events, endocarditis and pacemaker implantation.ResultsThroughout the study period, 114 patients underwent the Ross procedure (years 2009-2020) and 126 patients underwent bicuspidization (years 2006-2019). The bicuspidization group was significantly younger and presented with a higher degree of dyspnea, a lower degree of aortic valve stenosis and more often with pure regurgitation. The primary endpoint occurred more frequently in the bicuspidization group than in the Ross group – 77.9 vs. 97.9 % at 5 years and 68.4 vs. 75.2 % at 10 years (p < 0.001). There was no difference in secondary endpoints.ConclusionRoss procedure might offer a significantly lower mid-term risk of reintervention than bicuspidization in patients with UAV. Both procedures have comparable survival and risk of other short- and mid-term complications postoperatively.

Abstract: International audience; The aim of this work is to characterize the ejection plume obtained by laser ablation of GaLaS (GLS) samples in order to better understand the ablation phenomena for optimizing the pulsed laser deposition of chalcogenide thin films. The dynamics of the plasma between target and substrate was investigated through time- and space-resolved optical emission spectroscopy. High-resolution optical spectra have been recorded in the UV-VIS range using a 500-mm focal length monochromator and a fast gate ICCD camera. From the space-time evolution of the optical signals, the velocities of various species (including neutrals and ions) have been derived. Using the relative intensity method, the space- and time-evolution of the excitation temperature and electronic density have been determined. A complex behavior of the laser ablation plasma has been revealed.

L'objectif de ce travail est de réaliser des microcavités actives en dopant la couche séparatrice par des ions Erbium émettant à 1,53 et à 4,65 µm.

Abstract: International audience; In this work, the fabrication of amorphous chalcogenide thin films (As2S3 and Ge25Sb10Se65) by radio-frequency sputtering was studied. The morphology, chemical composition, optical properties and structure of fabricated layers as well as bonding arrangement at the surface of the films were investigated by scanning electron microscope with an energy-dispersive X ray analyzer, atomic force microscopy, transmittance measurements, variable angle spectroscopic ellipsometry, prism coupling technique, X-ray reflectometry, profilometry, conductivity measurements, Raman scattering spectroscopy and X-ray photoelectron spectroscopy. As2S3 thin films show some crystals formation on the surface indicating the ageing (oxidation) of the films. Ge25Sb10Se65 layers are more stable against oxidation and the morphology of the layers seems to be influenced by the sputtering pressure. Moreover, gold nanoparticles were deposited onto Ge25Sb10Se65 thin films by direct-current sputtering to assess surface enhanced infrared absorption spectroscopy of prepared structures. The morphology and thickness of gold films were also investigated. The infrared transmission spectra of a self-assembled monolayer of 4-nitrothiophenol deposited on gold are enhanced according to surface selection rules

Abstract: OBJECTIVES: The aim of the work was to find biomarkers identifying patients at high risk of adverse clinical outcomes after TAVI and SAVR in addition to currently used predictive model (EuroSCORE). BACKGROUND: There is limited data about the role of biomarkers in predicting prognosis, especially when TAVI is available. METHODS: The multi-biomarker sub-study included 42 consecutive high-risk patients (average age 82.0 years; logistic EuroSCORE 21.0%) allocated to TAVI transfemoral and transapical using the Edwards-Sapien valve (n = 29), or SAVR with the Edwards Perimount bioprosthesis (n = 13). Standardized endpoints were prospectively followed during the 12-month follow-up. RESULTS: The clinical outcomes after both TAVI and SAVR were comparable. Abstract: Background Rheumatoid arthritis (RA) is a disease characterized by chronic inflammation of synovial tissue, and progressive destruction of the joints. In RA patients a high prevalence of serum vitamin D deficiency/insufficiency was described. Vitamin D has a central role in the immune system and also correlates with increased levels of estrogen receptor α. Objectives The study analyzed the relationships between polymorphisms in the gene for vitamin D receptor (VDR) and the development and/or progression of the disease. Methods A total of 156 patients were enrolled with a diagnosis of RA (median age 55.6 years; 78% of them women; control group of 50 subjects). Development of the disease was scored by positive antibodies against cyclic citrullinated peptide (anti-CCP), rheumatoid factor (RF), CRP. Disease progression was assessed by the modified Sharp-van der Heijdeovou method Total Sharp Score (TSS). Polymorphisms were detected by PCR and subsequent restriction analysis (rs4516035 EcoRV; rs7975232: ApaI; rs731236: TaqI). Results The results show the relation of polymorphism VDR rs4516035 to seropositivity for anti-CCP in the distribution of genotypes (P=0.03), with a higher prevalence of the TT genotype when compared to the CC + CT variants (p=0.007, OR =3.2). The relationship was found also in the frequency of alleles (p=0.04). Further, a borderline relationship of this polymorphism to RF IgA levels (P=0.06) was detected. A relationship of polymorphism VDR rs731236 with the disease progression evaluated by TSS (p=0.02) was demonstrated, with the highest TSS in the patients carrying the GG variant. Conclusions The results show that the GG variant of VDR rs731236 polymorphism was associated with a worse prognosis of RA. Moreover, the VDR rs4516035 polymorphism was related to the seropositivity for anti-CCP, with a significant prevalence of TT genotype in positive RA patients. Disclosure of Interest None declared

Abstract: The aim of study was to examine relationship among levels of cytokines (IL-6, IL-13, IL-15, TNF-α) and chemokine (IL-8), production of autoantibodies, radiographic progression, and factors describing rheumatoid arthritis (RA). A total of 156 RA patients according to ACR criteria, and 55 control subjects were recruited into study. We observed higher levels of IL-15 within RA patients compared to healthy controls. Correlations among cytokine levels and the measures of rheumatoid factors, anti-CCP, measures of disease activity, and radiographic progression were observed. We conclude that IL-15 level in circulation could serve as one of the biomarkers for RA detection.

Abstract: Aim: The aim of our study was to assess the course of immediate post-exercise pulmonary capillary wedge pressure (PCWP) changes to identify the optimal time window for the noninvasive diagnostics of exercise-induced PCWP elevation. Methods and Results: Seventy-one patients at risk of heart failure with normal left ventricular ejection fraction underwent simultaneous exercise echocardiography and right heart catheterization. The ratio of early left ventricular filling velocity (E) to early diastolic mitral annular velocity (e') was used to predict noninvasively exercise-induced PCWP elevation. Fifty-one patients had exercise-induced PCWP elevation ≥ 8 mmHg and reached peak exercise PCWP ≥ 20 mmHg. Rapid post-exercise recovery of PCWP within 2 min was achieved in 18 (35.3%) patients. Intermediate post-exercise PCWP recovery at 3 and 4 min was found in 16 (31.4%) patients while late post-exercise PCWP recovery (≥ 5 min) was achieved in 17 (33.3%) patients. Conclusion: The course of post-exercise PCWP recovery is highly variable, and a significant proportion of patients have only a brief period (≤ 2 min) of exercise-induced PCWP elevation. This fact should be taken into account in noninvasive assessment of exercise-induced PCWP.

Abstract: Aims. To determine the impact of HLA compatibility measured by the Compatibility Index, on survival, rate of rejections, malignancies and infections in patients after heart transplantation (HTx). Methods. We carried out a retrospective analysis of 182 consecutive patients who underwent heart transplantation in our center from January 2001 to April 2010. According to degree of HLA-A, B and DR matching (Compatibility Index, CI) the patients were divided in two groups, Group A (n=83) with an IC 0-17 and group B (n=99) with an IC 18-26. There was no significant difference in demographic parameters between recipients and donors. Results. We found no difference in rates of rejections or infections between Group A and Group B (AR: 22 (26.5%) vs. 34 (34.3%), P=0.2539; infections: 21 (25.3%) vs. 27 (27%) P=0.7637). The distribution of infections in terms of type (bacterial, viral, fungal, including Aspergillus) was similar in both groups. The incidence of malignant tumours was infrequent (3 (3.6%) vs. 4 (4.0%), P=0.8817). We found trend toward lower level of tacrolimus in Group A. Long term survival was similar in both groups. Conclusions. Based on the results of our single-center trial, we found no impact of higher degree of HLA-A,-B, and -DR matching on survival, rejection episodes or infection. Further large studies are necessary to confirm our hypothesis that subjects with better HLA compatibility could require lower dose immunosuppression.

Abstract: The aim of the study was to examine the relation between polymorphisms and serum levels of selected cytokines (IL-6, IL-13 and IL-15), production of autoantibodies and factors describing rheumatoid arthritis (RA), such as DAS28 and Total Sharp Score. A total of 156 patients with RA according to the ACR criteria, and 200 control subjects were recruited into the study. The measurements of CRP, anti-CCP, the presence of rheumatoid factors (RFs), radiographs of both hands with calculation of Total Sharp Score (TSS) and DAS28 were obtained from all patients with RA. In total, five polymorphisms in genes coding cytokines (IL-6, IL-13 and IL-15) were detected. The levels of these selected cytokines were measured in serum using ELISA method. A significant difference in allele frequencies between patients with RA and controls was observed for IL-15 -267C/T polymorphism. A higher prevalence of heterozygote variants of IL-15 polymorphisms (14035A/T and -267C/T) in the RF IgG- and RF IgA-negative subgroups was observed. Furthermore, the association of polymorphisms in gene for IL-15 with circulating level of IL-15 (14035A/T and 367G/A) and with total RF and Ig-specific RFs (-267C/T) was found. The relation of IL-15 to RFs IgA, IgM, IgG and the measure of DAS28 was proved. The frequency of the T allele of the IL-13 polymorphism -1112C/T was higher in subgroup with faster progression of the disease (TSS/month ≥ 0.1). In conclusion, we present an association of IL-15 gene polymorphisms with the RFs including subtypes (RF, IgG, IgA) underlined by the relation of increased IL-15 levels in circulation to RFs.

Abstract: International audience; Electronic nonlinearities can lead to ultra-fast refractive index switching. This dynamic refractive index change can be used to shift wavelengths as well as to mix pulses of different center wavelengths. Due to its high refractive index silicon is suitable for tightly focusing light and generating high intensities required for such nonlinear effects, however high nonlinear losses in silicon (two photon absorption and absorption by free carriers generated via two photon absorption) limit transmission of high power pulses in silicon. Polymers and chalcogenide glasses have an improved nonlinear figure of merit (ration of nonlinear effect to nonlinear losses) and also don't show free carrier absorption. Due to acceptable levels of losses from generated free carriers, silicon organic hybrid or silicon glass heterogeneous structures offer to achieve high conversion efficiencies and large net gain in micro photonic devices, which can be used for wavelength conversion, parametric amplification and parametric oscillators, or for the generation of entangled photon pairs. We show both theoretical estimates and experimental results for four wave mixing conversion efficiencies in silicon hybrid and silicon heterogeneous structures.

Abstract: Aim. The aim of this study was to compare the quality of life after transcatheter aortic valve implantation (TAVI) and surgical replacement (SAVR) at one year. Methods. The study included 45 consecutive high-risk patients (average age 82.0 years; logistic Euroscore 22.3%) with symptomatic severe aortic stenosis allocated to TAVI transfemoral, TAVI transapical using the Edwards-Sapien valve or SAVR with the Edwards Perimount bioprosthesis (n=15 in each). The pre-operative characteristics were similar except for more myocardial infarctions in TAVI. The quality of life was assessed using the standardized EQ-5D questionnaire at baseline and on days 30, 90 and 360. The protocol was approved by the local ethics committee and an informed consent was signed. A total of 7 patients (15.5%) died during follow-up. Results. At baseline no significant differences in any of the quality-of-life parameters were found except for usual activities described as “best” (46.7% in SAVR vs. 10.0% in TAVI; P=0.002). At 30 and 90 days surviving patients were similar and at 360 days only the anxiety/depression score was “best” in 83.3% SAVR vs. 59.1% (P=0.046). Functional status improved in all patients (NYHA class I-II in 13.3% at baseline vs. 78.9% at 360-days) and the general health median significantly improved in TAVI patients (from 50 to 67; P=0.001) with a positive trend in SAVR patients (P=0.060). Conclusions. At one year, the general quality of life of high-risk patients had significantly improved after transcatheter aortic valve implantation with a positive trend in surgically treated patients.

Abstract: Aims. To determine the effect of pre-existing pulmonary hypertension (PHT) on early hemodynamics, morbidity and mortality after heart transplantation (HTx). Methods. Data were prospectively collected from 149 patients, who underwent HTx between January 2000 and December 2007. The subjects were divided into 3 groups: Group A (n=84) without PTH, group B (n=50) with mild to moderate PTH and group C (n=15) with severe PTH. We studied hemodynamic profile, tricuspid valve regurgitation (TR), incidence of acute cellular rejections (AR), infections, duration of hospitalization, 30-day mortality and a longterm survival. Results. Baseline characteristics were similar in all groups. Using vasodilator treatment PVR was successfully brought down to normal range 2.5 ± 0.6 Wood’ units (WU) on the day 1 following the surgery in all groups. Over 80% of patients were treated in Group C, 32% in Group A and 46% in Group B. There was no significant difference in the severity of TR among the 3 groups early after HTx (severe TR was observed in 46%, 54%, 33%, respectively). There was no significant difference in incidence of AR (G ≥ 2 Banff classification) (23%, 23%, 33%, respectively), infections (28%, 32%, 33%, respectively) or duration of hospitalization (30, 30, 28 days, respectively). There was no correlation between pre-transplant PHT and 30-day mortality or a long-term survival. Conclusions. In our cohort, PHT dropped very quickly after HTx, and was not associated with acute right heart failure following the surgery. Reversible PTH does not have a negative impact on short- or long-term survival after HTx.

143. Laser desorption ionization time-of-flight mass spectrometric study of binary As-Se glasses

Abstract: Binary chalcogenide As-Se glasses and their thin films are important for optics, computers, materials science and technological applications. To increase understanding of the properties of thin films fabricated by plasma deposition techniques, more information concerning the physics of plasma plume is needed. In this study the formation of clusters in plasma plume from different As-Se glasses by laser desorption ionization (LDI) or laser ablation (LA) was studied by time-of-flight mass spectrometry (TOF MS) in positive and negative ion modes. Formation of a number of As(p)Se(q) singly charged clusters As(3)Se(q)(+) (q = 1-5), AsSe(q)(-) (q = 1-3), As(2)Se(q)(-) (q = 2-4), and As(3)Se(q)(-) (q = 2-5) was found from As-Se glasses with the molar ratio As:Se in the range from 1:2 to 7:3. The stoichiometry of the As(p)Se(q) clusters was determined via isotopic envelope analysis and computer modeling. The structure of the clusters is proposed and the relationship to the structure of the parent glasses, as also suggested by Raman scattering spectra, is discussed.

Abstract: Topic: Electronics, Optics and Optoelectronics - WOS; International audience; Amorphous chalcogenide films play a motivating role in the development of integrated planar optical circuits due to their potential functionality in near infrared (IR) and mid-IR spectral regions. More specifically, the photoluminescence of rare earth ions in amorphous chalcogenide films can be used in laser and amplifier devices in the IR spectral domain. The aim of the present investigation was to optimize the deposition conditions for the fabrication of undoped and Er3+ doped sulphide and selenide thin films with nominal composition Ga5Ge20Sb10S(Se)65 or Ga5Ge23Sb5S67 by pulsed laser deposition (PLD). The study of compositional, morphological and structural characteristics of the layers was realized by scanning electron microscopy-energy dispersive spectroscopy, atomic force microscopy and Raman spectroscopy analyses, respectively. Some optical properties (transmittance, index of refraction, optical band gap, etc.) of prepared chalcogenide films and optical losses were investigated as well. The clear identification of near-IR photoluminescence of Er3+ ions was obtained for both selenide and sulphide films. The decay of the 4I13/2 → 4I15/2 transition at 1.54 µm in Er3+ doped Ga5Ge20Sb10S65 PLD sulphide films was studied to assess the effects of film thickness, rare earth concentration and multilayer PLD deposition on their spectroscopic properties.

Abstract: Malignant plasma cells in multiple myeloma (MM) are frequently characterized by complex karyotypes and chromosome instability. These cytogenetic changes are considered important prognostic indicators in MM patients. We have studied samples from 68 patients with newly diagnosed MM who were treated with high-dose chemotherapy and autologous stem cell transplantation. G-banding revealed abnormal karyotypes in 14 of 55 patients (25%) who had informative conventional cytogenetics. The combination of cytoplasmic immunoglobulin light chain labeling and interphase fluorescent in situ hybridization (cIg-FISH) revealed the presence of genetic aberrations in 53 of 68 patients (78%). Chromosome 13 abnormalities were found in 33 patients (50%) and IgH rearrangements in 36 patients (56.25%). In IgH positive patients we performed subsequent examinations of IgH affecting translocations t(4;14) and t(11;14) and we found translocation t(11;14) in 8 patients (12.5%) and t(4;14) in 10 patients (15.5%). The occurrences of others chromosomal abnormalities with known prognostic impact in MM were as follows: del(17)(p13) was present in 5 patients (9.8%) and gain 1q21 in 14 patients (36%). Analysis of survival of patients with different cytogenetic abnormalities revealed shorter overall survival (OS) in patients with IgH rearrangements (p=0.020) and trend to shorter OS in patients with gain 1q21 (p=0.064), respectively. Remarkably, patients with two or more aberrations had significantly shorter overall survival (p=0.001), time to progression (p=0.036) and progression free survival (p=0.008). Our results show a high incidence of chromosomal abnormalities in MM patients and confirm the prognostic impact of selected chromosomal aberrations as well as cumulative effect of multiple cytogenetic changes occurring simultaneously.

146. Successful re-use of the transplanted heart

Abstract: The case of re-use of a previously transplanted heart after brain death of the first recipient is reported. The second recipient was a 60-year-old man who suffered end-stage ischemic heart disease. The operative and postoperative course was uneventful, with the exception of secondary diabetes. The patient is post-transplantation by more than 10 months and is now in New York Heart Association functional class I. This case confirms the possibility of using this procedure, particularly in the situation when there is a shortage of organ donors.

Abstract: International audience; Electronic transitions of Pr3+ ions in Ga-Ge-Sb-Se glasses corresponding to emissions in the infrared region were studied by means of absorption and emission spectroscopies and fluorescence lifetimes measurements. Transition probabilities, radiative lifetimes, branching ratios, and quantum efficiencies of most of the emission transitions including the infrared ones occurring around 1.3, 1.7, and 2.4 μm were estimated based on a standard Judd-Ofelt analysis.

Abstract: conference 7366 " Photonic Materials, Devices, and Applications ", Session " Posters-Tuesday " [7366-65], http://spie.org/microtechnologies-new-millennium.xml; International audience; Due to remarkable properties of the chalcogenide glasses (Chgs), especially sulphide glasses, amorphous chalcogenide films should play a motivating role in the development of integrated planar optical circuits and their components. This paper describes the fabrication and properties of optical waveguides of undoped and erbium doped sulphide films obtained by RF magnetron sputtering and laser ablation (PLD). The deposition parameters were adjusted to obtain, from sulphide glass targets with a careful control of their purity, layers with appropriate compositional, morphological, structural characteristics and optical properties. A transmission loss of 0.8 dB/cm can be obtained for rib waveguides produced by dry etching under CF4 plasma (4-300 μm wide, 5.5 μm film thickness, 1.5 μm etched thickness). The photo-luminescence of erbium doped Ge20Ga5Sb10S65 films were clearly observed in the n-IR and mid-IR spectral domain. The study of their decay lifetime with a well adapted annealing treatment controlling the roughness variation reached value of the bulk counterpart. Amplification tests were carried out leading to a complete characterisation of the Erbium doped waveguide. Gain on/off of 4.4 dB (3.4 dB/cm) were achieved for a signal at 1.54 μm in multiple modes sulphide:Er waveguides. The first demonstration of photoluminescence in mid-IR in an Er3+- doped Chg waveguide could potentially be employed to produce sources or amplifiers operating in the mid-IR.
