The challenges associated with reliably observing and simulating hazardous hailstorms call for new approaches that combine information from different available sources, such as remote sensing instruments, observations, or numerical modelling, to improve understanding of where and when severe hail most often occurs. In this work, a proxy for hail frequency is developed by combining overshooting cloud top (OT) detections from the Meteosat Second Generation (MSG) weather satellite with convection-permitting High rEsolution ReAnalysis over Italy (SPHERA) reanalysis predictors describing hail-favourable environmental conditions. Atmospheric properties associated with ground-based reports from the European Severe Weather Database (ESWD) are considered to define specific criteria for data filtering. Five convection-related parameters from reanalysis data quantifying key ingredients for hailstorm occurrence enter the filter: most unstable convective available potential energy (CAPE), K index, surface lifted index, deep-layer shear, and freezing-level height. A hail frequency estimate over the extended summer season (April–October) in south-central Europe is presented for a test period of 5 years (2016–2020). OT-derived hail frequency peaks at around 15:00 UTC in June–July over the pre-Alpine regions and the northern Adriatic Sea. The hail proxy statistically matches with ∼63 % of confirmed ESWD reports, which is roughly 23 % more than the previous estimate over Europe coupling deterministic satellite detections with coarser global reanalysis ambient conditions. The separation of hail events according to their severity highlights the enhanced appropriateness of the method for large-hail-producing hailstorms (with hailstone diameters ≥ 3 cm). Nutritional therapy (NT) based on a controlled protein intake represents a cornerstone in managing chronic kidney disease (CKD). However, if a CKD patient is at the same time affected by cancer, oncologists and nutritionists tend to suggest a dietary regimen based on high protein intake to avoid catabolism and malnutrition. International guidelines are not clear when we consider onco-nephrological patients and, as a consequence, no clinical shared strategy is currently applied in clinical practice. In particular, no precise nutritional management is established in nephrectomized patients for renal cell carcinoma (RCC), a specific oncological cohort of patients whose sudden kidney removal forces the remnant one to start a compensatory mechanism of adaptive hyperfiltration. Our study aimed to investigate the efficacy of a low–normal-protein high-calorie (LNPHC) diet based on a Mediterranean model in a consecutive cohort of nephrectomized RCC patients using an integrated nephrologist and nutritionist approach. A consecutive cohort of 40 nephrectomized RCC adult (age > 18) patients who were screened for malnutrition (malnutrition screening tool, MST < 2) were enrolled in a tertiary institution between 2020 and 2022 after signing a specific informed consent form. Each patient underwent an initial nephrological and nutritional evaluation and was subsequently subjected to a conventional CKD LNPHC diet integrated with aproteic foods (0.8 g/Kg/die: calories: 30–35 kcal per kg body weight/die) for a period of 6 months (±2 months). The diet was structured after considering eGFR (CKD-EPI 2021 creatinine formula), comorbidities, and nutritional status. MST, body mass index (BMI), phase angle (PA), fat mass percentage (FM%), fat-free mass index (FFMI), body cell mass index (BCMI), extracellular/intracellular water ratio (ECW/ICW), extracellular matrix/body cell mass ratio (ECM/BCM), waist/hip circumference ratio (WHC), lab test exams, and clinical variables were examined at baseline and after the study period. Our results clearly highlighted that the LNPHC diet was able to significantly improve several nutritional parameters, avoiding malnutrition and catabolism. In particular, the LNPHC diet preserved the BCM index (delta on median, ΔM + 0.3 kg/m2) and reduced the ECM/BCM ratio (ΔM − 0.03 *), with a significant reduction in the ECW/ICW ratio (ΔM − 0.02 *), all while increasing TBW (ΔM + 2.3% *). The LNPHC diet was able to preserve FFM while simultaneously depleting FM and, moreover, it led to a significant reduction in urea (ΔM − 11 mg/dL **). In conclusion, the LNPHC diet represents a new important therapeutic strategy that should be considered when treating onco-nephrological patients with solitary kidney due to renal cancer. The transformations of intimacy represent an important field of study for contemporary sociology. Among these, consensual non-monogamies are forms of relationship in which all partners give explicit consent to engage in romantic, intimate, and/or sexual relationships with multiple people. These new forms of relationship reflect some characteristics of the contemporary world: the flexibility, the reversibility of choices, the redefinition of gender relations, the centrality of open communication among partners. In this study, we applied the well-known Sternberg Triangular Love Scale to measure intimacy, passion and commitment in an international sample consisting of 558 people from 33 countries, who practice forms of consensual non-monogamy and who had at least two simultaneous emotional or romantic relationships at the time of the research. Results seems partially similar to that found in previous studies applying the same scale to monogamous relationships. In particular, it is partially confirmed that the commitment dimension increases with the duration of the relationship, and that the intimacy dimension is substantially stable. Contrary to previous studies that described a progressive waning of passion as the relationship progressed, in our sample this dimension proved to be substantially stable over time. Furthermore, the three dimensions in one relationship are all positively and significantly correlated to the three dimensions in the other relationship. A possible interpretation of this finding is that there is a "virtuous" effect among multiple relationships experienced by the same person: the liveliness, the duration and depth of a relationship do not necessarily subtract resources from the other (or from the others). BackgroundThe prognosis for adults with relapsed acute lymphoblastic leukemia is poor. We sought to determine whether inotuzumab ozogamicin, an anti-CD22 antibody conjugated to calicheamicin, results in better outcomes in patients with relapsed or refractory acute lymphoblastic leukemia than does standard therapy.MethodsIn this phase 3 trial, we randomly assigned adults with relapsed or refractory acute lymphoblastic leukemia to receive either inotuzumab ozogamicin (inotuzumab ozogamicin group) or standard intensive chemotherapy (standard-therapy group). The primary end points were complete remission (including complete remission with incomplete hematologic recovery) and overall survival.ResultsOf the 326 patients who underwent randomization, the first 218 (109 in each group) were included in the primary intention-to-treat analysis of complete remission. The article presents the results of a study that mapped academics’ teaching excellences, needs and areas for improvement to tailor educational staff development initiatives. Academics from seven Italian universities (Bari, Camerino, Catania, Florence, Foggia, Genoa, Turin) completed a survey that included three open-ended questions, aimed at exploring innovative practices, critical aspects and types of desired support. The study involved 7.278 professors who received the survey; 4.289 completed the main questionnaire, with a response rate of about 59%. Academics’ answers were analyzed by content analysis through the software Atlas.ti. For each question, results offer a wide picture of main elements mentioned by respondents. This experience represents an integrated systemic initiative using research to inform practices, promoting a shared culture of teaching and learning in higher education, that is critical for the vitality of the academic environment, as well as a scholarly approach aimed at valuing teaching and identifying tailored strategies to improve it. In fact, content analysis allowed researchers to design a multi-level action plan, taking into account three different stages: an institutional level, an inter-institutional one, and a national one.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection results in a wide range of outcomes characterized by a high heterogeneity in both symptomatology and susceptibility to the disease. In such a perspective, COVID-19 may be considered as a multifactorial disease featured by the interaction between the environment, which is the virus itself, and the genetic profile of the host. Our analysis aimed at investigating the transmission dynamics of SARS-CoV-2 within families whose members responded in different ways to the infection, although the exposure was common to the entire group and occurred before the availability of any vaccine. The goal was to understand how the genetic background of each subject can influence the viral infection outcome and hence the above-mentioned clinical variability. We performed a segregation analysis in 19 Italian families with a designed custom panel of 42 genes involved in immunity and virus entry and which have also been shown to be related to SARS-CoV-2 host response. We carried out both a familial segregation analysis and a global statistical analysis. In the former we identified eighteen risk variants co-segregating with a COVID-positive status and six variants with a possible protective effect. In addition, sixteen variants showed a trend of association to a severe phenotype. Together with common SNPs, we detected private rare variants that may also provide insight into the observed clinical COVID-19 heterogeneity. The global statistical analysis confirmed statistically significant positive associations between SARS-CoV-2 individual response and some specific gene variants identified in familial analysis. In conclusion our data confirm that the clinical expression of COVID-19 is markedly influenced by the host genetic profile both with a mendelian transmission pattern and a polygenic architecture.

In recent years, the molecular subtyping of gastric cancer has led to the identification of novel clinically relevant biomarkers as well as promising therapeutic targets. In parallel, the advent of checkpoint inhibitors has expanded treatment options beyond conventional chemotherapy. Compelling evidence has shown unprecedented efficacy results for anti-PD1-based therapies in the molecular subgroups of dMMR/MSI-h, EBV+ and PD-L1 CPS+ patients, to the point that these are granted approval for gastric cancer adenocarcinoma (AGC) in several countries. Despite this, cytotoxic chemotherapy remains the only treatment choice for the considerable proportion of biomarkers-negative patients. In this context, little is known about the association between subtypes-defining biomarkers (HER2, MMR/MSI, PD-L1, and EBV) and the efficacy of standard chemotherapy in non-Asian AGC. Here, we aimed to investigate the prevalence, the clinic-pathologic features, and the impact on treatment outcome of clinical molecular subtypes in a new-diagnosed Western cohort of AGC.

The blood ammonia (NH3) level is one of the most important hepatic biomarkers for the diagnosis and monitoring of liver pathologies and infections. In this work, we developed an optimized optical biosensing method to extract and quantify the ammonia contained in complex-matrix samples emulating the blood serum. First, the approach was tested with solutions of phosphate-buffered saline (PBS) and ammonia chloride. Then, further trials were carried out with solutions of fetal bovine serum (FBS). The ammonia was extracted from the tested samples through a customized cell, and it was optically quantified by exploiting the indophenol reaction. The extraction cell included a cation-exchange membrane in Nafion, which was chemically pre-treated through cleaning procedures of sulfuric acid and hydrogen peroxide to keep a basic pH in the ammonia solution and to avoid contaminants in the membrane. From the NH3 solution, the indophenol reaction produced light-reactive indophenol dye molecules, which were used as colorimetric indicators. Through absorbance measurements of the indophenol dye solution at 670 nm wavelength, we were able to detect and quantify the ammonia level in the samples both with a spectrophotometer and a customized miniaturized read-out system, obtaining a detection limit of 0.029 µmol/mL.

Head and neck squamous cell carcinoma (HNSCC) is the sixth most common cancer. In locally advanced (LA) HNSCC, a multidisciplinary approach consisting of surgery followed by chemoradiation (CRT) or definitive CRT is the mainstay of treatment. In recurrent metastatic (R/M), HNSCC immune checkpoint inhibitors (ICIs) with or without chemotherapy represent the new first-line option. However, cancer will recur in about two out of five patients with LA HNSCC. If progression occurs within six months from platin-radiotherapy treatment, anti-programmed cell death-1 (PD-1) may be prescribed. Otherwise, immunotherapy with or without chemotherapy might be considered if PD-L1 is expressed. Despite several improvements in the outcome of patients with R/M HNSCC, overall survival (OS) remains dismal, equaling a median of 14 months. In-depth knowledge of the tumor microenvironment (TME) would be required to change the course of this complex disease. In recent years, many predictive and prognostic biomarkers have been studied in the HNSCC TME, but none of them alone can select the best candidates for response to ICIs or targeted therapy (e.g., Cetuximab). The presence of cytokines indicates an immune response that might occur, among other things, after tumor antigen recognition, viral and bacterial infection, and physic damage. An immune response against HNSCC results in the production of some cytokines that induce a pro-inflammatory response and attract cells, such as neutrophils, macrophages, and T cell effectors, to enhance the innate and adaptive anti-tumor response. We revised the role of a group of cytokines as biomarkers for treatment response in HNSCC.

Abstract At the national level, in Italy, observational and forecast data are collected by various public bodies and are often kept in various small, heterogeneous and non‐interoperable repositories, released under different licenses, thus limiting the usability for external users. In this context, MISTRAL (the Meteo Italian SupercompuTing PoRtAL) was launched as the first Italian meteorological open data portal, with the aim of promoting the reuse of meteorological data sets available at national level coverage. The MISTRAL portal provides (and archives) meteorological data from various observation networks, both public and private, and forecast data that are generated and post‐processed within the Consortium for Small‐scale Modeling‐Limited Area Model Italia (COSMO‐LAMI) agreement using high performance computing (HPC) facilities. Also incorporated is the Italy Flash Flood use case, implemented with the collaboration of European Centre for Medium‐Range Weather Forecasts (ECMWF), which exploits cutting edge advances in HPC‐based post‐processing of ensemble precipitation forecasts, for different model resolutions, and applies those to deliver novel blended‐resolution forecasts specifically for Italy. Finally, in addition to providing architectures for the acquisition and display of observational data, MISTRAL also delivers an interactive system for visualizing forecast data of different resolutions as superimposed multi‐layer maps.

Program evaluations often investigate complex or multi-dimensional constructs, such as individual opinions or attitudes, by means of ratings. A different interpretation of the same question may affect cross-country comparability, leading to the Differential Item Functioning problem. Anchoring vignettes were introduced in the literature as a way to adjust self-evaluations from this interpersonal incomparability. In this paper, we first introduce a new nonparametric solution to analyse anchoring vignette data, recoding a variable based on a rating scale to a new corrected- variable that guarantees comparability in any cross-country analysis. Then, we exploit the flexibility of a mixture model introduced to account for uncertainty in the response process (the CUP model) to test if the proposed solution is effectively able to remove this reported heterogeneity. This solution is easy to construct and has important advantages compared with the original nonparametric solution adopted with anchoring vignette data. The novel indicator is applied to investigate self-reported depression in an old population. Data that will be analysed come from the second wave of the Survey of Health, Ageing and Retirement in Europe, collected in 2006/2007. Results highlight the need of correcting for reported heterogeneity comparing individual self-evaluations. Once interpersonal incomparability resulting from the different uses of response scales is removed from the self-assessments, some estimates are reversed in magnitude and signs with respect to the analysis of the collected data. Abstract The value of cross-country comparisons is at the heart of large-scale international surveys. Yet the validity of such comparisons is often challenged, particularly in the case of latent traits whose estimates are based on self-reported answers to a small number of questionnaire items. Many believe self-reports to be unreliable and not comparable, and indeed, formal statistical procedures very often reject the assumption that the questions are understood and answered in the same way in different countries (measurement invariance). A methodological conference on the comparability of questionnaire scales was hosted by the OECD on 8 and 9 November 2018. This meeting report summarises the discussions held at the conference about measurement invariance testing and instrument design. The report first provides a brief introduction to the measurement models and the accompanying invariance analyses typically used in the industry of large-scale international surveys and points to the main limitations of these current standard approaches. It then presents classical and novel ways to deal with imperfect comparability of measurements when scaling and reporting on continuous traits and on categorical latent variables. It finally discusses the extent to which item design can improve the cross-country comparability of the measured constructs (e.g. by adopting innovative item formats such as anchoring vignettes and situational judgement test items). It concludes with some general considerations for survey design and reporting on invariance analyses and survey results.
