BackgroundInterleukin 10 (IL-10) and heart fatty acid-binding protein (H-FABP) have gained interest as diagnostic biomarkers of traumatic brain injury (TBI), but factors affecting their blood levels in patients with moderate-to-severe TBI are largely unknown.ObjectiveTo investigate the trajectories of IL-10 and H-FABP between TBI patients with and without extracranial injuries (ECI); to investigate if there is a correlation between the levels of IL-10 and H-FABP with the levels of inflammation/infection markers C-reactive protein (CRP) and leukocytes; and to investigate if there is a correlation between the admission level of H-FABP with admission levels of cardiac injury markers, troponin (TnT), creatine kinase (CK), and creatine kinase MB isoenzyme mass (CK-MBm).Materials and methodsThe admission levels of IL-10, H-FABP, CRP, and leukocytes were measured within 24 h post-TBI and on days 1, 2, 3, and 7 after TBI. The admission levels of TnT, CK, and CK-MBm were measured within 24 h post-TBI.ResultsThere was a significant difference in the concentration of H-FABP between TBI patients with and without ECI on day 0 (48.2 ± 20.5 and 12.4 ± 14.7 ng/ml, p = 0.02, respectively). Abstract

Weissella strains are often detected in spontaneously fermented foods. Because of their abilities to produce lactic acid and functional exopolysaccharides as well as their probiotic traits, Weissella spp. improve not only the sensorial properties but also nutritional values of the fermented food products. However, some Weissella species have been associated with human and animal diseases. In the era of vast genomic sequencing, new genomic/genome data are becoming available to the public on daily pace. Detailed genomic analyses are due to provide a full understanding of individual Weissella species. In this study, the genomes of six Weissella paramesenteroides strains were de novo sequenced. The genomes of 42 W. paramesenteroides strains were compared to discover their metabolic and functional potentials in food fermentation. Comparative genomics and metabolic pathway reconstructions revealed that W. paramesenteroides is a compact group of heterofermentative bacteria with good capacity of producing secondary metabolites and vitamin Bs. Since the strains rarely harbored plasmid DNA, they did not commonly possess the genes associated with bacteriocin production. All 42 strains were shown to bear vanT gene from the glycopeptide resistance gene cluster vanG. Yet none of the strains carried virulence genes.

112. Towards inclusive teaching: Finnish teachers' justifications for co-teaching

Abstract

The Finnish school system is associated with the provision of inclusive values, such as equality and equal opportunity for all. In recent years, the pupils attending regular schools have become more heterogeneous and the number of pupils in need of support has grown. This study examines teachers’ perceptions of when the implementation of co-teaching is justified and when it is not justified. Co-teaching, which means teaching together with another teacher, is one way to organise pedagogical support in the general education group. Although several studies have reported the benefits and challenges of co-teaching, researchers have not reached an agreement on the situations where teachers see co-teaching as justified. Most Finnish teachers have a positive attitude towards co-teaching, yet its degree of implementation is low. The data have been collected from primary education teachers (N = 432) in Finland and analysed in a data-driven manner adopting a grounded theory approach. The results of the research show that the teachers’ arguments for implementing co-teaching are contradictory. Moreover, their attitudes towards inclusion are reflected in their perceptions of co-teaching. The justifications for co-teaching are the benefits gained through collaboration and the support for education. Most differences in opinions occur when pupils in need of various kinds of special support are integrated into the general education group.

Abstract

Abstract Background Poor treatment compliance is a common problem in the treatment of asthma. To our knowledge, no previous long-term follow-up studies exist on how scheduled asthma follow-up contacts occur in primary health care (PHC) versus secondary care and how these contacts relate to adherence to medication and in participation to further scheduled asthma contacts. The aim of this study was to evaluate occurrence of scheduled asthma contacts and treatment compliance in PHC versus secondary care, and to identify the factors associated with non-participation to scheduled contacts. Methods Patients with new adult-onset asthma (n = 203) were followed for 12 years in a real-life asthma cohort of the Seinäjoki Adult Asthma Study (SAAS). The first contacts were mainly carried out in secondary care and therefore the actual follow-up time including PHC visits was 10 years. Results A majority (71%) of the patients had ≥ 2 scheduled asthma contacts during 10-year follow-up and most of them (79%) mainly in PHC. Patients with follow-up contacts mainly in PHC had better adherence to inhaled corticosteroid (ICS) medication during the whole 12-year period compared to patients in secondary care. In the study population, 29% of the patients had only 0–1 scheduled asthma contacts during the follow-up. Heavy alcohol consumption predicted poor participation in scheduled contacts. Conclusions Patients with mainly PHC scheduled asthma contacts were more adherent to ICS medication than patients in the secondary care. Based on our results it is necessary to pay more attention to actualization of asthma follow-up visits and systematic assessment of asthma patients including evaluation of alcohol consumption. This study aims at advancing mathematical and computational techniques for reconstructing the interior structure of a near earth asteroid (NEA) via Computed Radar Tomography (CRT). We introduce a far-field model for full-wave CRT and validate it numerically for sparse and limited-angle data and an orbiting distance of about 5 km. Abstract

This study concentrates on advancing mathematical and computational methodology for radar tomography imaging in which the unknown volumetric velocity distribution of a wave within a bounded domain is to be reconstructed. Our goal is to enable effective simulation and inversion of a large amount of full-wave data within a realistic 2D or 3D geometry. For propagating and inverting the wave, we present a rigorous multigrid-based forward approach which utilizes the finite-difference time-domain method and a nested finite element grid structure. Based on the multigrid approach, we introduce and validate a multiresolution algorithm which allows regularization of the unknown distribution through a coarse-to-fine inversion scheme. In this approach, sparse signals can be effectively inverted, as the coarse fluctuations are reconstructed before the finer ones. Furthermore, the number of nonzero entries in the system matrix can be compressed and thus the inversion procedure can be speeded up. As a test scenario we investigate satellite-based asteroid interior reconstruction. We use both full-wave and projected wave data and estimate the accuracy of the inversion under different error sources: noise and positioning inaccuracies. The results suggest that the present full-wave inversion approach allows recovering the interior with a single satellite recording backscattering data. It seems that robust results can be achieved, when the peak-to-peak signal-to-noise ratio is above 10 dB. Abstract

This study aims to advance hardware-level computations for travel-time tomography applications in which the wavelength is close to the diameter of the information that has to be recovered. Such can be the case, for example, in the imaging applications of (1) biomedical physics, (2) astro-geophysics and (3) civil engineering. Our aim is to shed light on the effect of that preprocessing the digital waveform signal has on the inversion results and to find computational solutions that guarantee robust inversion when there are incomplete and/or noisy measurements. We describe a hardware-level implementation for integrated and thresholded travel-time computation (ITT and TTT). We compare the ITT and TTT approaches in inversion analysis with experimental acoustic travel-time data recorded using a ring geometry for the transmission and measurement points. The results obtained suggest that ITT is essential for maintaining the robustness of the inversion with imperfect signal digitization and sparsity. In order to ensure the relevance of the results, the specifications of the test setup were related to those of applications (1)-(3).

117. A Character-Word Compositional Neural Language Model for Finnish

Abstract

Inspired by recent research, we explore ways to model the highly morphological Finnish language at the level of characters while maintaining the performance of word-level models. We propose a new Character-to-Word-to-Character (C2W2C) compositional language model that uses characters as input and output while still internally processing word level embeddings. Our preliminary experiments, using the Finnish Europarl V7 corpus, indicate that C2W2C can respond well to the challenges of morphologically rich languages such as high out of vocabulary rates, the prediction of novel words, and growing vocabulary size. Notably, the model is able to correctly score inflectional forms that are not present in the training data and sample grammatically and semantically correct Finnish sentences character by character.

118. pocl: A Performance-Portable OpenCL Implementation

Abstract

OpenCL is a standard for parallel programming of heterogeneous systems. The benefits of a common programming standard are clear; multiple vendors can provide support for application descriptions written according to the standard, thus reducing the program porting effort. While the standard brings the obvious benefits of platform portability, the performance portability aspects are largely left to the programmer. The situation is made worse due to multiple proprietary vendor implementations with different characteristics, and, thus, required optimization strategies. In this paper, we propose an OpenCL implementation that is both portable and performance portable. At its core is a kernel compiler that can be used to exploit the data parallelism of OpenCL programs on multiple platforms with different parallel hardware styles. The kernel compiler is modularized to perform target-independent parallel region formation separately from the target-specific parallel mapping of the regions to enable support for various styles of fine-grained parallel resources such as subword SIMD extensions, SIMD datapaths and static multi-issue. Unlike previous similar techniques that work on the source level, the parallel region formation retains the information of the data parallelism using the LLVM IR and its metadata infrastructure. This data can be exploited by the later generic compiler passes for efficient parallelization. The proposed open source implementation of OpenCL is also platform portable, enabling OpenCL on a wide range of architectures, both already commercialized and on those that are still under research. Abstract

This qualitative research explored the rationales of open-source development in cheminformatics. The objective was to promote open science by mapping out and categorizing the reasons why open-source development is being carried out. This topic is important because cheminformatics has an industrial background and open-source is the key solution in promoting the growth of cheminformatics as an independent academic field. The data consisted of 87 research articles that were analyzed using qualitative content analysis. The analysis produced six rationale categories: (1) Develop New Software, (2) Update Current Features, Tools, or Processes, (3) Improve Usability, (4) Support Open-source Development and Open Science, (5) Fulfill Chemical Information Needs, and (6) Support Chemistry Learning and Teaching. This classification can be used in designing rationales for future software development projects, which is one of the largest research areas in cheminformatics. In particular, there is a need to develop cheminformatics education for which software development can serve as an interesting multidisciplinary framework.

120. I Still Miss Human Contact, but This Is More Flexible--Paradoxes in Virtual Learning Interaction and Multidisciplinary Collaboration

Abstract

Teaching and learning in universities have shifted towards online education, and there is not enough scientific knowledge about suitable pedagogical design principles to guide the construction of hybrid learning spaces for today's university students. In this design-based research, we studied the benefits and challenges in reaching the expected learning outcomes of an e-course, "Collaboration and Professional Interaction Skills." The practical purpose was developing the pedagogical design of this e-course. The scientific objective was to build on the research about design principles that guide teachers in constructing e-courses, enabling in-depth learning in hybrid learning spaces. The pedagogical design was guided by six design principles. Empirical data (21 students' assignments and reflections, teacher's observations, data from the digital learning management system) were collected in two research cycles. Students were from different degree programmes of educational sciences. Thematic analysis of students' individually and collaboratively authored assignments was carried out in both research cycles. The most emphasised benefits and challenges concerned the combination of working and creating knowledge together, virtually, in a multidisciplinary group. Based on the results, practical recommendations were made for university-level pedagogical designs that facilitate effective and active collaboration and interaction, and learning of such competence.

121. Supporting Pupils in Finnish and Swedish Schools--Teachers' Views

Abstract

The purpose of this study was to investigate the different support systems used by teachers in compulsory education. Class and subject teachers and special educators from Finland (N = 57) and Sweden (N = 57) participated in the study, in which both qualitative and quantitative methods were used. Participants completed an electronic questionnaire to identify the supports and methods they use when working with pupils who have special educational needs. The findings indicate both similarities and differences between the two countries. One of the most common forms of support was individualisation, including pedagogical modifications. Methods of supporting academic skills such as reading differed from those used to support behavioral issues. Positive pedagogy and structuring the environment were used ways of supporting pupils with behavioral challenges. Results for both countries are compared and support needs are discussed.

Abstract

Abstract Background RIFINs and STEVORs are variant surface antigens expressed by P. falciparum that play roles in severe malaria pathogenesis and immune evasion. These two highly diverse multigene families feature multiple paralogs, making their classification challenging using traditional bioinformatic methods. Results STRIDE (STevor and RIfin iDEntifier) is an HMM-based, command-line program that automates the identification and classification of RIFIN and STEVOR protein sequences in the malaria parasite Plasmodium falciparum. STRIDE is more sensitive in detecting RIFINs and STEVORs than available PFAM and TIGRFAM tools and reports RIFIN subtypes and the number of sequences with a FHEYDER amino acid motif, which has been associated with severe malaria pathogenesis. Conclusions STRIDE will be beneficial to malaria research groups analyzing genome sequences and transcripts of clinical field isolates, providing insight into parasite biology and virulence.

Abstract

Background: The morbidity and mortality of acute subdural hematoma (aSDH) remains high. Several factors have been reported to affect the outcome and survival of these patients. In this study, we explored factors potentially associated with the outcome and survival of surgically treated acute subdural hematoma (aSDH), including postcraniotomy hematomas (PCHs). Methods: This retrospective cohort study was conducted in a single tertiary university hospital between 2008 and 2012 and all aSDH patients that underwent surgical intervention were included. A total of 132 cases were identified for collection of demographics, clinical, laboratory, and imaging data. Univariate and multivariable analyses were performed to assess factors associated with three-month Glasgow Outcome Scale (GOS) and survival at one- and five-year. Results: In this study, PCH (n = 14, 10.6%) was not associated with a worse outcome according to the 3- month GOS (p = 0.37) or one (p = 0.34) and five-year (p = 0.37) survival. The multivariable analysis showed that the volume of initial hematoma (p = 0.009) and Abbreviated Injury Scale score (p = 0.016) were independent predictors of the three-month GOS. We describe the MalariaGEN Pf7 data resource, the seventh release of Plasmodium falciparum genome variation data from the MalariaGEN network. It comprises over 20,000 samples from 82 partner studies in 33 countries, including several malaria endemic regions that were previously underrepresented. For the first time we include dried blood spot samples that were sequenced after selective whole genome amplification, necessitating new methods to genotype copy number variations. We identify a large number of newly emerging crt mutations in parts of Southeast Asia, and show examples of heterogeneities in patterns of drug resistance within Africa and within the Indian subcontinent. We describe the profile of variations in the C-terminal of the csp gene and relate this to the sequence used in the RTS,S and R21 malaria vaccines. Pf7 provides high-quality data on genotype calls for 6 million SNPs and short indels, analysis of large deletions that cause failure of rapid diagnostic tests, and systematic characterisation of six major drug resistance loci, all of which can be freely downloaded from the MalariaGEN website.

Abstract

Plasmodium parasites caused 241 million cases of malaria and over 600,000 deaths in 2020. Both P. falciparum and P. ovale are endemic to Mali and cause clinical malaria, with P. falciparum infections typically being more severe. Here, we sequenced RNA from nine pediatric blood samples collected during infections with either P. falciparum or P. ovale, and characterized the host and parasite gene expression profiles. We found that human gene expression varies more between individuals than according to the parasite species causing the infection, while parasite gene expression profiles cluster by species. Additionally, we characterized DNA polymorphisms of the parasites directly from the RNA-seq reads and found comparable levels of genetic diversity in both species, despite dramatic differences in prevalence. Our results provide unique insights into host-pathogen interactions during malaria infections and their variations according to the infecting Plasmodium species, which will be critical to develop better elimination strategies against all human Plasmodium parasites.

Abstract

Variant surface antigens (VSAs) play a critical role in severe malaria pathogenesis. Defining gaps, or "lacunae", in immunity to these Plasmodium falciparum antigens in children with severe malaria would improve our understanding of vulnerability to severe malaria and how protective immunity develops. Using a protein microarray with 179 antigen variants from three VSA families as well as more than 300 variants of three other blood stage P. falciparum antigens, reactivity was measured in sera from Malian children with cerebral malaria or severe malarial anaemia and age-matched controls. Sera from children with severe malaria recognized fewer extracellular PfEMP1 fragments and were less reactive to specific fragments compared to controls. Following recovery from severe malaria, convalescent sera had increased reactivity to certain non-CD36 binding PfEMP1s, but not other malaria antigens. Sera from children with severe malarial anaemia reacted to fewer VSAs than did sera from children with cerebral malaria, and both of these groups had lacunae in their seroreactivity profiles in common with children who had both cerebral malaria and severe malarial anaemia. This microarray-based approach may identify a subset of VSAs that could inform the development of a vaccine to prevent severe disease or a diagnostic test to predict at-risk children.

Abstract

OBJECTIVE: This study aimed to examine duration of sickness absence due to knee osteoarthritis (OA) and sustained return to work (RTW) among municipal employees, who had at least one compensated sickness absence period due to knee OA. The contribution of sociodemographic characteristics, diabetes and previous sickness absence were assessed. We differentiated between participants with and without total knee arthroplasty (TKA). METHODS: Data from 123 506 employees in the Finnish Public Sector Study were linked with national health and mortality register information. There were 3 231 sickness absence periods (2372 participants) due to knee OA in 2005−2011. Kaplan-Meier curves for sustained RTW were obtained and median time with inter-quartile range (IQR) calculated for those with and without TKA. Cox regression analyses were carried out in multivariable analyses. RESULTS: The median time to RTW from the beginning of sickness absence was 21–28 days when TKA was not related to sickness absence and 92–145 days when it was. Among participants with no TKA, age 60−64, non-sedentary work, diabetes, and previous sickness absences predicted longer time to RTW, while pain medication predicted a shorter time. Among participants with TKA, non-sedentary work and previous sickness absences predicted a longer time to RTW. CONCLUSIONS: The clinical relevance of the difference in time to RTW between employees with or without TKA was substantial. Employees with knee OA working in physically demanding jobs need work modifications after TKA, and this calls for a dialog between occupational health care professionals and workplaces.

Abstract Knowledge of the Plasmodium falciparum antigens that comprise the human liver stage immunoproteome is important for pre-erythrocytic vaccine development, but, compared with the erythrocytic stage immunoproteome, more challenging to classify. Previous studies of P. falciparum antibody responses report IgG and rarely IgA responses. We assessed IgG and IgA antibody responses in adult sera collected during two controlled human malaria infection (CHMI) studies in malaria-naïve volunteers and in 1- to 6-year-old malaria-exposed Malian children on a 251 P. falciparum antigen protein microarray. IgG profiles in the two CHMI groups were equivalent and differed from Malian children. IgA profiles were robust in the CHMI groups and a subset of Malian children. We describe immunoproteome differences in naïve vs. exposed individuals and report pre-erythrocytic proteins recognized by the immune system. IgA responses detected in this study expand the list of pre-erythrocytic antigens for further characterization as potential vaccine candidates.

Resistance to antimalarial drugs, and in particular to the artemisinin derivatives and their partner drugs, threatens recent progress toward regional malaria elimination and eventual global malaria eradication. Population-level studies utilizing whole-genome sequencing approaches have facilitated the identification of regions of the parasite genome associated with both clinical and in vitro drug-resistance phenotypes. However, the biological relevance of genes identified in these analyses and the establishment of a causal relationship between genotype and phenotype requires functional characterization. Here we examined data from population genomic and transcriptomic studies in the context of data generated from recent functional studies, using a new population genetic approach designed to identify potential favored mutations within the region of a selective sweep (iSAFE). We identified several genes functioning in pathways now known to be associated with artemisinin resistance that were supported in early population genomic studies, as well as potential new drug targets/pathways for further validation and consideration for treatment of artemisinin-resistant Plasmodium falciparum. In addition, we establish the utility of iSAFE in identifying positively-selected mutations in population genomic studies, potentially accelerating the time to functional validation of candidate genes.

Abstract

Elective language courses, particularly those starting from the beginner level, constitute their own special group within the communication and language course offerings of universities. The elementary courses of less commonly taught languages (LCTL), such as Japanese, provide students with the opportunity to acquire, among other benefits, a proficiency that distinguishes them from other job applicants. Ordinary language skills, commonly limited to English only, are today regarded as the default and not as any unique international asset. Even partial knowledge of a less commonly taught language and culture broadens one's worldview and increases cultural understanding. The many years of experience in teaching Japanese have evoked in me the desire to analyse my students' backgrounds, studies and employment. The survey described in this article constitutes the basis for a broader study to be conducted in the future. The purpose of the survey was to collect and analyse data on the faculties and major subjects of students who attend elementary Japanese courses, as well as their language repertoires besides Japanese. Their initial motivation, reasons for the choice of Japanese and plans for the future related to Japan and Japanese studies were analysed. Abstract

ObjectiveSpine injury is highly prevalent in patients with poly-trauma, but data on the co-occurrence of spine trauma in patients with traumatic brain injury (TBI) are scarce. In this study, we used the Collaborative European NeuroTrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) database to assess the prevalence, characteristics, and outcomes of patients with TBI and a concurrent traumatic spinal injury (TSI).MethodsData from the European multi-center CENTER-TBI study were analyzed. Adult patients with TBI (≥18 years) presenting with a concomitant, isolated TSI of at least serious severity (Abbreviated Injury Scale; AIS ≥3) were included. For outcome analysis, comparison groups of TBI patients with TSI and systemic injuries (non-isolated TSI) and without TSI were created using propensity score matching. Rates of mortality, unfavorable outcomes (Glasgow Outcome Scale Extended; GOSe < 5), and full recovery (GOSe 7–8) of all patients and separately for patients with only mild TBI (mTBI) were compared between groups at 6-month follow-up.ResultsA total of 164 (4%) of the 4,254 CENTER-TBI core study patients suffered from a concomitant isolated TSI. The median age was 53 [interquartile range (IQR): 37–66] years and 71% of patients were men. mTBI was documented in 62% of cases, followed by severe TBI (26%), and spine injuries were mostly cervical (63%) or thoracic (31%). Surgical spine stabilization was performed in 19% of cases and 57% of patients were admitted to the ICU. Mortality at 6 months was 11% and only 36% of patients regained full recovery. There were no significant differences in the 6-month rates of mortality, unfavorable outcomes, or full recovery between TBI patients with and without concomitant isolated TSI. However, concomitant non-isolated TSI was associated with an unfavorable outcome and a higher mortality. In patients with mTBI, a negative association with full recovery could be observed for both concomitant isolated and non-isolated TSI.ConclusionRates of mortality, unfavorable outcomes, and full recovery in TBI patients with and without concomitant, isolated TSIs were comparable after 6 months. However, in patients with mTBI, concomitant TSI was a negative predictor for a full recovery. These findings might indicate that patients with moderate to severe TBI do not necessarily exhibit worse outcomes when having a concomitant TSI, whereas patients with mTBI might be more affected.

Abstract

Abstract Plasmodium falciparum erythrocyte membrane protein-1s (PfEMP1s), diverse malaria proteins expressed on the infected erythrocyte surface, play an important role in pathogenesis, mediating adhesion to host vascular endothelium. Antibodies to particular non-CD36-binding PfEMP1s are associated with protection against severe disease. We hypothesized that given lifelong P. falciparum exposure, Malian adults would have broad PfEMP1 serorecognition and high seroreactivity levels during follow-up, particularly to non-CD36-binding PfEMP1s such as those that attach to endothelial protein C receptor (EPCR) and intercellular adhesion molecule-1 (ICAM-1). Using a protein microarray, we determined serologic responses to 166 reference PfEMP1 fragments during a dry and subsequent malaria transmission season in Malian adults. Malian adult sera had PfEMP1 serologic responses throughout the year, with decreased reactivity to a small subset of PfEMP1 fragments during the dry season and increases in reactivity to a different subset of PfEMP1 fragments during the subsequent peak malaria transmission season, especially for intracellular PfEMP1 domains. For some individuals, PfEMP1 serologic responses increased after the dry season, suggesting antigenic switching during asymptomatic infection. Adults were more likely to experience variable serorecognition of CD36-binding PfEMP1s than non-CD36-binding PfEMP1s that bind EPCR or ICAM-1, which remained serorecognized throughout the year. Sustained seroreactivity to non-CD36-binding PfEMP1s throughout adulthood amid seasonal fluctuation patterns may reflect underlying protective severe malaria immunity and merits further investigation.

Snow water equivalent (SWE) is an important variable in describing global seasonal snow cover. Traditionally, SWE has been measured manually at snow transects or using observations from weather stations. However, these measurements have a poor spatial coverage, and a good alternative to in situ measurements is to use spaceborne passive microwave observations, which can provide global coverage at daily timescales. The reliability and accuracy of SWE estimates made using spaceborne microwave radiometer data can be improved by assimilating radiometer observations with weather station snow depth observations as done in the GlobSnow SWE retrieval methodology. However, one possible source of uncertainty in the GlobSnow SWE retrieval approach is the constant snow density used in modelling emission of snow. In this paper, three versions of spatially and temporally varying snow density fields were implemented using snow transect data from Eurasia and Canada and automated snow observations from the United States. Snow density fields were used to post-process the baseline GlobSnow v.3.0 SWE product. Decadal snow density information, i.e. fields where snow density for each day of the year was taken as the mean calculated for the corresponding day over 10 years, was found to produce the best results. Overall, post-processing GlobSnow SWE retrieval with dynamic snow density information improved overestimation of small SWE values and underestimation of large SWE values, though underestimation of SWE values larger than 175 mm was still significant.

Abstract

We determine the annual timing of spring recovery from space-borne microwave radiometer observations across northern hemisphere boreal evergreen forests for 1979-2014. We find a trend of advanced spring recovery of carbon uptake for this period, with a total average shift of 8.1 d (2.3 d/decade). We use this trend to estimate the corresponding changes in gross primary production (GPP) by applying in situ carbon flux observations. Micrometeorological CO2 measurements at four sites in northern Europe and North America indicate that such an advance in spring recovery would have increased the January-June GPP sum by 29 g⋅C⋅m-2 [8.4 g⋅C⋅m-2 (3.7%)/decade]. We find this sensitivity of the measured springtime GPP to the spring recovery to be in accordance with the corresponding sensitivity derived from simulations with a land ecosystem model coupled to a global circulation model. The model-predicted increase in springtime cumulative GPP was 0.035 Pg/decade [15.5 g⋅C⋅m-2 (6.8%)/decade] for Eurasian forests and 0.017 Pg/decade for forests in North America [9.8 g⋅C⋅m-2 (4.4%)/decade]. This change in the springtime sum of GPP related to the timing of spring snowmelt is quantified here for boreal evergreen forests.

Abstract

Summary: Background: Early death after cancer surgery is higher in low-income and middle-income countries (LMICs) compared with in high-income countries, yet the impact of facility characteristics on early postoperative outcomes is unknown. The aim of this study was to examine the association between hospital infrastructure, resource availability, and processes on early outcomes after cancer surgery worldwide. Methods: A multimethods analysis was performed as part of the GlobalSurg 3 study—a multicentre, international, prospective cohort study of patients who had surgery for breast, colorectal, or gastric cancer. The primary outcomes were 30-day mortality and 30-day major complication rates. Potentially beneficial hospital facilities were identified by variable selection to select those associated with 30-day mortality. Adjusted outcomes were determined using generalised estimating equations to account for patient characteristics and country-income group, with population stratification by hospital. Findings: Between April 1, 2018, and April 23, 2019, facility-level data were collected for 9685 patients across 238 hospitals in 66 countries (91 hospitals in 20 high-income countries; 57 hospitals in 19 upper-middle-income countries; and 90 hospitals in 27 low-income to lower-middle-income countries). The availability of five hospital facilities was inversely associated with mortality: ultrasound, CT scanner, critical care unit, opioid analgesia, and oncologist. Abstract

Diffuse axonal injury (DAI) is a common neuropathological manifestation of traumatic brain injury (TBI), presenting as traumatic alterations in the cerebral white matter (WM) microstructure and often leading to long-term neurocognitive impairment. These WM alterations can be assessed using diffusion tensor imaging (DTI). Cerebral microbleeds (CMBs) are a common finding on head imaging in TBI and are often considered a visible sign of DAI, although they represent diffuse vascular injury. It is poorly known how they associate with long-term white matter integrity. This study included 20 patients with TBI and CMBs, 34 patients with TBI without CMBs, and 11 controls with orthopedic injuries. DTI was used to assess microstructural WM alterations. CMBs were detected using susceptibility-weighted imaging (SWI) and graded according to their location in the WM and total lesion load was counted. Patients underwent SWI within 2 months after injury. DTI and clinical outcome assessment were performed at an average of eight months after injury. Outcome was assessed using the extended Glasgow Outcome Scale (GOSe). The Glasgow Coma Scale (GCS) and length of post-traumatic amnesia (PTA) were used to assess clinical severity of the injury. We found that CMB grading and total lesion load were negatively associated with fractional anisotropy (FA) and positively associated with mean diffusivity (MD). Patients with TBI and CMBs had decreased FA and increased MD compared with patients with TBI without CMBs. CMBs were also associated with worse clinical outcome. When adjusting for the clinical severity of the injury, none of the mentioned associations were found. Thus, the difference in FA and MD is explained by patients with TBI and CMBs having more severe injuries. Our results suggest that CMBs are not associated with greater WM alterations when adjusting for the clinical severity of TBI. Thus, CMBs and WM alterations may not be strongly associated pathologies in TBI.

Abstract

Writing rap lyrics requires both creativity to construct a meaningful, interesting story and lyrical skills to produce complex rhyme patterns, which form the cornerstone of good flow. We present a rap lyrics generation method that captures both of these aspects. First, we develop a prediction model to identify the next line of existing lyrics from a set of candidate next lines. This model is based on two machine-learning techniques: the RankSVM algorithm and a deep neural network model with a novel structure. Results show that the prediction model can identify the true next line among 299 randomly selected lines with an accuracy of 17%, i.e., over 50 times more likely than by random. Second, we employ the prediction model to combine lines from existing songs, producing lyrics with rhyme and a meaning. An evaluation of the produced lyrics shows that in terms of quantitative rhyme density, the method outperforms the best human rappers by 21%. The rap lyrics generator has been deployed as an online tool called DeepBeat, and the performance of the tool has been assessed by analyzing its usage logs. Consequently, appropriate inhaler choice is important to maximize clinical benefit. The primary objective of this study was to characterize inspiratory flow parameters across two Easyhaler® inhalers and the HandiHaler® inhaler in patients with COPD and healthy volunteers.Methods: In this randomized, open-label, crossover study, subjects (100 patients with COPD; 100 healthy volunteers) were trained to perform inhalations of placebo powder via two variants of Easyhaler and placebo capsules via the HandiHaler inhalers. Subjects then performed three placebo inhalations through each inhaler in a random sequence. Inspiratory flow parameters were assessed, including peak inspiratory flow (PIF), for each inhaler. A parallel sub-study was conducted in patients with COPD from the main study to assess correct use of the inhalers, patient’s preference, ability to learn to use the inhalers, and the feasibility of the In-Check Dial device to measure PIF values.Results: Mean PIF rates and inspiratory volumes through the three inhalers were similar between patients with COPD and healthy volunteers, and all subjects achieved the 30 L/min PIF required for effective use of Easyhaler. Almost 70% of the 88 patients enrolled in the sub-study used the Easyhaler and HandiHaler inhalers without errors. The Easyhaler was preferred by 51% of patients, while 25% favored the HandiHaler. Teaching the use of both inhalers to almost 70% of patients was very easy. Abstract The Guttmacher-Lancet Commission report on Sexual and Reproductive Health and Rights called for the acceleration of progress to achieve SRHR that is essential for sustainable development. To integrate the essential services defined in this report into universal health coverage in the 11 sovereign nations in the Pacific, quality data is required to ensure needs are met efficiently and equitably. However, there are no comprehensive reports for Pacific Island countries that provide insight into all areas of SRHR. We collated the latest literature to identify the most up-to-date relevant data from United Nations and Guttmacher Institute reports to discern gaps in SRHR information and services relating to contraception, abortion and reproductive coercion. Investment is urgently required to strengthen health information systems for SRHR in the Pacific.

145. A Decade of Finnish Engineering Education for Sustainable Development

Abstract

Purpose: The paper aims to examine the current status and development of sustainable development in Finnish engineering education. Design/methodology/approach: The study consists of interviews with key stakeholders supplemented with the analysis of documented material. Development is discussed in relation to the findings of collaborative strategy process in the year 2009. Findings: The paper observes that the Finnish universities providing engineering education are committed to sustainable development in their strategies. However, a lot of work remains to be done before the strategies are implemented and sustainable development is integrated to all degree programs. Explicit knowledge and individual learning in clearly defined disciplinary boundaries have been the main focus of engineering education. Practical implications: The paper suggests that engineers need to be provided with mental tools to cope with uncertainty, complexity and ambiguity. Key competencies include holistic understanding, communication and collaboration skills, ability and willingness for critical and reflective thinking, creativity, innovativeness and entrepreneurship. Thus, collaborative learning, open dialogue and innovation are at the heart of education for sustainable development. Originality/value: This paper has a relatively wide approach as it analyses sustainable development in the context of Finnish engineering education both on institutional and societal levels and is based on a national project.

146. Collaborative Case-Based Virtual Learning in Higher Education: Consultation Case in Special Education

Abstract

Collaborative case-based virtual learning was used with special teacher education students from Finland (N = 94) and Sweden (N = 59). The case was about consultation and was tailor-made to fit their studies. The case was used as a bridge between theoretical and practical studies, and consisted of videos of an imaginary school, which proceeded in the form of a narrative. The goals for the students were practicing challenging situations and critical thinking in small groups--a kind of artificial experience building. After completing the case, the students responded to a questionnaire about it. They evaluated the case positively as a new, inspiring method. Only a few critical comments were received, such as that there was too little time for deep discussion. The method of virtual learning seems to be a good, inspiring way to study in higher education, as an alternative to lectures. Other topics these students would like to study in a similar way were issues related to interaction and new professional skills, such as co-teaching or discussions with parents. The different aspects of this kind of virtual collaborative learning are discussed.

Abstract

Tunable coupling of superconducting qubits has been widely studied due to its importance for isolated gate operations in scalable quantum processor architectures. Here, we demonstrate a tunable qubit-qubit coupler based on a floating transmon device, which allows us to place qubits at least 2 mm apart from each other while maintaining over 50-MHz coupling between the coupler and the qubits. In the introduced tunable-coupler design, both the qubit-qubit and the qubit-coupler couplings are mediated by two waveguides instead of relying on direct capacitive couplings between the components, reducing the impact of the qubit-qubit distance on the couplings. This leaves space for each qubit to have an individual readout resonator and a Purcell filter, which is needed for fast high-fidelity readout. In addition, simulations show that the large qubit-qubit distance significantly lowers unwanted non-nearest-neighbor coupling and allows multiple control lines to cross over the structure with minimal crosstalk. Using the proposed flexible and scalable architecture, we demonstrate a controlled-Z gate with (99.81±0.02)% fidelity.

Abstract

Background Regardless of its benefit in promoting maternal health, contributing to a healthy pregnancy, little is known concerning the prevalence of utilization of preconception care and its determinant in southern Ethiopia. Hence, this study designed to determine the prevalence of utilization of preconception care and contributing factors among pregnant women in West Guji Zone, Southern Ethiopia, 2021. Methods A community-based cross-sectional study was conducted among systematically selected 660 pregnant women in West Guji from June 15 to July 30, 2021. A pretested interviewer-administered structured questionnaire was used to collect the data. Data entry was done in Epidata version3.1 and exported to SPSS version 25 for analysis. Descriptive statistics were used to summarize the data. To identify the factors associated with the utilization of preconception care binary and multivariable logistic regression analysis was performed. Adjusted odds ratios (AOR) with 95% CI were estimated to assess the strength of associations and statistical significance was declared at a p-value < 0.05. Results One hundred-forty seven, 22.3% [95% CI (19.2, 25.4)] of mothers utilized preconception care. Being college and above [(AOR = 5.51 95%CI 91.43-21.19)] and secondary [(AOR = 4.46 95%CI (1.38-14.39)] in educational status, rich [(AOR = 4.23 95%CI (1.32-13.55)], having good knowledge about preconception care [AOR = 2.34 95%CI (1.05-5.28)], having a positive attitude towards preconception care [(AOR = 9.99 95%CI (4.25-23.48)] and deciding with her husband regarding maternal health services [(AOR = 4.71 95%CI (1.91-11.56)] were factors positively affecting utilization of preconception care. Conclusions The utilization of preconception care in the study area is low. Being college and above and secondary in educational status, rich, good knowledge, positive attitude towards preconception care, and deciding with her husband regarding maternal health services were independent factors promoting the utilization of preconception care. Information, education, and communication activities should be strengthened to increase awareness of mothers about preconception care.
