Abstract

No robust evidence is provided by literature regarding the management of intracranial hypertension following severe traumatic brain injury (TBI). This is mostly due to the lack of prospective randomized controlled trials (RCTs), the presence of studies containing extreme heterogeneously collected populations and controversial considerations about chosen outcome. A scientific society should provide guidelines for care management and scientific support for those areas for which evidence-based medicine has not been identified. However, RCTs in severe TBI have failed to establish intervention effectiveness, arising the need to make greater use of tools such as Consensus Conferences between experts, which have the advantage of providing recommendations based on experience, on the analysis of updated literature data and on the direct comparison of different logistic realities. The Italian scientific societies should provide guidelines following the national laws ruling the best medical practice. However, many limitations do not allow the collection of data supporting high levels of evidence for intracranial pressure (ICP) monitoring and decompressive craniectomy (DC) in patients with severe TBI. This intersociety document proposes best practice guidelines for this subsetting of patients to be adopted on a national Italian level, along with joint statements from “TBI Section” of the Italian Society of Neurosurgery (SINch) endorsed by the Neuroanesthesia and Neurocritical Care Study Group of the Italian Society of Anesthesia, Analgesia, Resuscitation and Intensive Care (SIAARTI). Presented here is a recap of recommendations on management of ICP and DC supported a high level of available evidence and rate of agreement expressed by the assemblies during the more recent consensus conferences, where members of both groups have had a role of active participants and supporters. Abstract

No robust evidence is provided by literature regarding the management of intracranial hypertension following severe traumatic brain injury (TBI). This is mostly due to the lack of prospective randomized controlled trials (RCTs), the presence of studies containing extreme heterogeneously collected populations and controversial considerations about chosen outcome. A scientific society should provide guidelines for care management and scientific support for those areas for which evidence-based medicine has not been identified. However, RCTs in severe TBI have failed to establish intervention effectiveness, arising the need to make greater use of tools such as Consensus Conferences between experts, which have the advantage of providing recommendations based on experience, on the analysis of updated literature data and on the direct comparison of different logistic realities. The Italian scientific societies should provide guidelines following the national laws ruling the best medical practice. However, many limitations do not allow the collection of data supporting high levels of evidence for intracranial pressure (ICP) monitoring and decompressive craniectomy (DC) in patients with severe TBI. This intersociety document proposes best practice guidelines for this subsetting of patients to be adopted on a national Italian level, along with joint statements from “TBI Section” of the Italian Society of Neurosurgery (SINch) endorsed by the Neuroanesthesia and Neurocritical Care Study Group of the Italian Society of Anesthesia, Analgesia, Resuscitation and Intensive Care (SIAARTI). Presented here is a recap of recommendations on management of ICP and DC supported a high level of available evidence and rate of agreement expressed by the assemblies during the more recent consensus conferences, where members of both groups have had a role of active participants and supporters. 127. 11177 Uterine Inflammatory Characteristics Following Cesarean Delivery.

Abstract

It has been suggested that inflammatory process at the area of abnormal cesarean scar (niche) harms the endometrial environment and cause infertility, however, data is limited. The aim of this study was to compare the inflammatory characteristics of women with cesarean scar to those without. A prospective study. Hysteroscopic ambulatory clinic. Study population included women that underwent diagnostic hysteroscopy with cesarean uterine scar (study group) that were compared to those without (controls). Women six months from delivery, breastfeeding, with abnormal finding on hysteroscopy or uterine scar other than cesarean were excluded. A syringe was attached to the outlet of the diagnostic hysteroscope and the first 5 cc of 0.9% normal saline fluid used for hydro-dissection were collected. The samples were analyzed for inflammatory factors levels, including: GM-CSF, IFN-gamma, IL-1, IL-2, IL-5, IL-6, IL-7. IL-13, IL-15, IL-17, IL-17F, IL-22, IL-23, IL-31, IL-36 and TNF-alpha. Transvaginal ultrasound was performed after hysteroscopy for women in the study group to characterize the uterine scar. Primary outcome was defined as the difference in the inflammatory factors level. 80 women met inclusion criteria, of them 29(36%) had history of CD and 51(64%) had no uterine scar. Women's demographic and clinical characteristics were comparable between groups. No difference was found in the level of any of the 17 factors collected. Sub-analysis by infertility indication to perform the procedure to all other indications, found IL 33 significantly higher in the infertility group (p=0.02). While comparing only women in the CD group, IL 33 was found to be 25 times higher in the infertility group [61.23(19.84-169.44) vs. 4.61(0-33.72) pg/ml; p=0.004]. This difference was not demonstrated in women without CD (p=0.29). Women undergoing evaluation due to infertility with history of uterine scar have significantly higher level of inflammatory marker IL33. [ABSTRACT FROM AUTHOR]

Abstract

• Quadricuspid aortic valve is a rare congenital heart defect. • Echocardiography is a useful tool for quadricuspid aortic valve diagnosis. • Prevalence of this congenital heart defect remains unknown in the horse. A 16-year-old horse, 400 kg, male, Quarter horse gelding was examined for a cardiac murmur that had not been previously heard. Physical examination revealed a regularly irregular pulse and a grade III/VI, decrescendo, diastolic murmur with a point of maximum intensity over the left heart base. Base-apex standard electrocardiographic examination at rest showed sinus rhythm with second-degree atrio-ventricular blocks. Echocardiography identified the presence of four aortic valve cusps, two equal larger and two unequal smaller cusps. Color flow Doppler examination showed a diastolic regurgitant jet emerging from the central region of closed aortic valve. Based on clinical and echocardiographic findings a diagnosis of mild to moderate aortic valve insufficiency secondary to quadricuspid aortic valve was made.
