105. Vesico-Adnexal Fistula Treated with Transurethral Embolization Under Fluoroscopic Guidance.

Objective: Rare disease Background: Vesico-adnexal fistulae are rare. Potential causes of such fistulae include infection, endometriosis, and iatrogenic causes following pelvic surgeries. To the best of our knowledge, only 3 cases of vesico-adnexal fistulae have been reported, and all these patients were treated surgically by removing the involved adnexa, excising the fistulous duct, and suturing the bladder. We describe the first case of vesico-adnexal fistula that developed after pelvic surgery, and it was successfully treated by transurethral embolization under fluoroscopic guidance. Case Report: Our patient was a 27-year-old woman with a history of hysterectomy. She presented to our institution with urethral discharge and a recurrent urinary tract infection. The cystogram showed a fistula tract connecting the urinary bladder and left adnexal cystic cavity. She was treated conservatively with antibiotics and prolonged Foley catheterization to allow for spontaneous closure of the fistula; however, conservative management failed. The patient was successfully treated with transurethral embolization of the tract under fluoroscopic guidance. Conclusions: In such a rare scenario with limited treatment options, interventional radiology offers an alternative minimally invasive treatment strategy. [ABSTRACT FROM AUTHOR]

This paper includes a simulation-based analysis of non-stop traffic at isolated signalised intersections. A Min-Max traffic control method is proposed to replace conventional traffic signals and maintain a continuous traffic flow. The method applicability is tested by means of Discrete Event Simulation (DES) using AutoMod software package. A set of quantitative Measures of Performance (MOP) is used to demonstrate the benefits of the proposed method compared to conventional traffic signals. In addition to eliminating vehicles stoppages, simulation results of the proposed method showed improvement in the overall traffic flow by reducing vehicle average travel time, average delay time, and the average number of vehicles at the intersection. Brain metastases from cervical cancer are extremely rare. We report on two patients who developed cerebellous metastases following uterine cervical cancer. The interval between diagnosis of the primary cancer and diagnosis of brain metastasis was 8 months. The main complaint was symptoms of increased intracranial pressure and cerebellous syndrome. Surgical excision of the brain lesion followed by radiation therapy was performed in the first case. The second patient received palliative radiation therapy. The first patient died 8 months after diagnosis. The second patient is alive 2 months after diagnosis. [Copyright &y& Elsevier]

The primary non hodgkin’s lymphoma of the uterus is rare. This rarity explains of one part certain difficulties of the histological diagnosis and on the other hand the absence of a therapeutic strategy clearly established. We report 4 cases of primary non-hodgkin lymphoma of the uterus. Two patients had a cervical location, the two other had corpus location. The average age of our patients is of 59 years (extremes: 54–68). Histological diagnosis was confirmed by biopsy for the cervical location. For the corpus location, it is study of the uterus after hysterectomy which retained the diagnosis of lymphoma. The type of the lymphoma was low grade in two cases and high grade in the two other cases. The disease was limited to the pelvis for all our patients (stage IE according to Ann-Arbor’s classification). The treatment consisted of an association of chemotherapy and radiotherapy in both cases of lymphoma of the cervix and in a radical hysterectomy followed by chemotherapy for the two cases of lymphoma of the corpus. Our patients are regularly followed, with an average follow-up of 56 months. Two patients are in disease free, the third patient presented a dissemination of the disease and the fourth patient presented a squamous cell carcinoma of the lung. [Copyright &y& Elsevier]

126. Roles of driver and vehicle characteristics in speed choice along rural highways

- Abstract
A total of 2510 vehicle speed observations were measured from a straight section of a rural two-lane two way highway in Jordan. The mean speeds were above the maximum allowable speed (acceptable limit for citations) for passenger cars, vehicles with engine sizes of 1800 cc or more, new vehicles (1-10 years), vehicles with 1-3 passengers and drivers wearing seat belts. They were above the speed limit (posted limit on the traffic sign) and below the maximum allowable speed for young adult and adult drivers, male drivers, non-seat belt users, pickups, busses, vans, medium age vehicles (11-15 years), vehicles with four passengers and vehicles with engine sizes of less than 1800 cc. However, they were below the speed limit for older drivers, female drivers, trucks and older vehicles (>15 years). Loaded pickups and trucks had lower speeds than unloaded ones.

127. Osteoblastoma Is a Metabolically Active Benign Bone Tumor on 18F-FDG PET Imaging.

We describe a case of a 9-y-old girl who on
18 F-FDG PET imaging was found to have a highly metabolically active sacral tumor with an average standarized uptake value of 6.2. The tumor was proven to be osteoblastoma by pathologic examination. Osteoblastoma is a relatively rare benign primary bone tumor and occurs predominantly in patients younger than 20 y. The most common area of involvement is the spine. Osteoblastoma has been reported to be metabolically active on18 F-FDG PET imaging, with an average standarized uptake value of 3.2, which renders18 F-FDG PET imaging unable to differentiate benign from malignant primary bone tumors. To our knowledge, only 5 cases of osteoblastoma evaluated by18 F-FDG PET imaging have been reported in the literature; all were metabolically active on18 F-FDG PET imaging. Fallopian tube obstruction is a major cause of female infertility. We aimed to evaluate the factors potentially affecting pregnancy rate following fallopian tube recanalization (FTR) in infertile women with proximal fallopian tube obstruction. Data was retrospectively collected for 61 women (25, primary infertility; 36, secondary infertility) who underwent FTR at our institution. Bivariable and multivariable analyses of clinical pregnancy rates in relation to the following factors were performed: primary vs. secondary infertility, duration of infertility, age at the time of FTR, unilateral vs. bilateral obstruction, and previous pelvic interventions. All women who underwent fluoroscopically guided transcervical FTR of one or both proximally obstructed tubes were successfully recanalized (technical success rate, 100%). Within a year after FTR, 41% of women had conceived. None of the studied variables was significantly associated with pregnancy rate on bivariable analysis. Nevertheless, on multivariable analysis, the type and duration of infertility were significantly associated with pregnancy among women aged <35 years at the time of FTR. Our findings regarding pregnancy rates following FTR reflect the diversity of the patient population and suggest the presence of multiple contributing factors. Younger women with secondary infertility for <5 years are highly likely to achieve conception following FTR. [ABSTRACT FROM AUTHOR]

The polycrystalline layered perovskite sample LaBiCaMn2O7 was synthesized by the conventional solid-state reaction process. By the X-ray powder diffraction, we have investigated the structural properties. The diffraction patterns are consistent with tetragonal lattice parameters a = 3.824 Å and c = 19.183 Å with I4/mmm as a space group. The magnetic measurements have demonstrated that their properties are relevant two magnetic transitions: first for a spin-glass (SG) transition at blocking temperature TB = 76 K and a second for the ferromagnetic–paramagnetic transition is found at Curie temperature TC = 225 K. The analysis of the Arrot diagram has shown the appearance of two transitions to be of the second-order type. The magnetic entropy variation (− ∆SM), induced by the magnetic field, was presumed on the basis of Maxwell relation. Under a change in a magnetic field of 2 T and around TC, the value of (− Δ S M max ) is found to be 4.17 J kg−1 K−1, and the value of RC (refrigeration capacity) is of 397 J.kg−1. It should be noted that LaCaBiMn2O7 displays a relatively prominent magnetocaloric effect at 225 K. [ABSTRACT FROM AUTHOR]

Fe2−xRuxO3 (0 ≤ x≤ 0.08) powders were prepared by solid-state method. X-ray diffraction patterns revealed that the Fe2O3 powders crystallize in a hexagonal structure typical of α-Fe2O3 phase (hematite). The crystallite size was found to decrease from 141 to 84 nm with increasing Ru content from 0 to 0.08, respectively. UV–visible absorption study was performed at room temperature. The direct energy band gap of un-doped α-Fe2O3 was 2.22 eV and found to increase with increasing Ru concentration to reach 2.45 eV for x = 0.08. This increase in band gap was attributed to the decrease in oxygen vacancies with increasing Ru as indicated by calculations of the oxygen occupancy rate using Rietveld refinement. Other properties such as the absorption coefficient, refractive index, the extinction coefficient, real and imaginary dielectric constant, real and imaginary electrical conductivity were calculated using spectrophotometric measurements. Contains fulltext : 177987.pdf (Publisher’s version ) (Open Access) BACKGROUND: Antimicrobial Stewardship Programs (ASPs) have been promoted to optimize antimicrobial usage and patient outcomes, and to reduce the emergence of antimicrobial-resistant organisms. However, the best strategies for an ASP are not definitively established and are likely to vary based on local culture, policy, and routine clinical practice, and probably limited resources in middle-income countries. The aim of this study is to evaluate structures and resources of antimicrobial stewardship teams (ASTs) in surgical departments from different regions of the world. METHODS: A cross-sectional web-based survey was conducted in 2016 on 173 physicians who participated in the AGORA (Antimicrobials: A Global Alliance for Optimizing their Rational Use in Intra-Abdominal Infections) project and on 658 international experts in the fields of ASPs, infection control, and infections in surgery. RESULTS: The response rate was 19.4%. One hundred fifty-six (98.7%) participants stated their hospital had a multidisciplinary AST. The median number of physicians working inside the team was five [interquartile range 4-6]. An infectious disease specialist, a microbiologist and an infection control specialist were, respectively, present in 80.1, 76.3, and 67.9% of the ASTs. A surgeon was a component in 59.0% of cases and was significantly more likely to be present in university hospitals (89.5%, p < 0.05) compared to community teaching (83.3%) and community hospitals (66.7%). Protocols for pre-operative prophylaxis and for antimicrobial treatment of surgical infections were respectively implemented in 96.2 and 82.3% of the hospitals. The majority of the surgical departments implemented both persuasive and restrictive interventions (72.8%). The most common types of interventions in surgical departments were dissemination of educational materials (62.5%), expert approval (61.0%), audit and feedback (55.1%), educational outreach (53.7%), and compulsory order forms (51.5%). CONCLUSION: The survey showed a heterogeneous organization of ASPs worldwide, demonstrating the necessity of a multidisciplinary and collaborative approach in the battle against antimicrobial resistance in surgical infections, and the importance of educational efforts towards this goal.

Primary squamous cell cercinoma of the breast is a rare neoplasma included in metaplastic breast cancer. The histogenesis remains unknown. Clinical and radiological appearances are not specific. Nodal involvment is rare and hormons receptors are negative. The treatment is based on surgery associated to radiation therapy and chemotherapy. Prognosis seems to be similar to others breast carcinoma. We report three cases of primary squamous cell carcinoma of the breast recruited at National Institute of Oncology with review of the literature. [Copyright &y& Elsevier]

International audience; This declaration, signed by an interdisciplinary task force of 234 experts from 83 different countries with different backgrounds, highlights the threat posed by antimicrobial resistance and the need for appropriate use of antibiotic agents and antifungal agents in hospitals worldwide especially focusing on surgical infections. As such, it is our intent to raise awareness among healthcare workers and improve antimicrobial prescribing. To facilitate its dissemination, the declaration was translated in different languages.

Intra-abdominal infections (IAI) are an important cause of morbidity and are frequently associated with poor prognosis, particularly in high-risk patients. The cornerstones in the management of complicated IAIs are timely effective source control with appropriate antimicrobial therapy. Empiric antimicrobial therapy is important in the management of intra-abdominal infections and must be broad enough to cover all likely organisms because inappropriate initial antimicrobial therapy is associated with poor patient outcomes and the development of bacterial resistance. The overuse of antimicrobials is widely accepted as a major driver of some emerging infections (such as C. difficile), the selection of resistant pathogens in individual patients, and for the continued development of antimicrobial resistance globally. The growing emergence of multi-drug resistant organisms and the limited development of new agents available to counteract them have caused an impending crisis with alarming implications, especially with regards to Gram-negative bacteria. An international task force from 79 different countries has joined this project by sharing a document on the rational use of antimicrobials for patients with IAIs. The project has been termed AGORA (Antimicrobials: A Global Alliance for Optimizing their Rational Use in Intra-Abdominal Infections).
