Using a cross section of commercial and merchant banks, the study showed that the Nigerian banking industry was not efficient both in the pre-and-post-liberalization era. The study further revealed that market share was the strongest determinant of technical efficiency in the Nigerian banking Industry. Thus, appropriate macroeconomic policy, institutional development and structural reforms must accompany financial liberalization to create the stable environment required for it to succeed. Hence, the present bank consolidation and reforms by the Central Bank of Nigeria, which started with Soludo and continued with Sanusi, are considered necessary, especially in the areas of e banking and reorganizing the management of banks.

This study investigated the quantitative polyphenolic constituents and gastroprotective effects of methanol extract of Vernonia amygdalina leaf (MEVA) against aspirin-induced gastric ulcer in rats. Ulceration was induced by 3 days’ oral administration of aspirin (150 mg/kg body weight). Wistar rats were pretreated with cimetidine (reference drug) at a dose of 100 mg/kg body weight and MEVA at 200, 300, and 400 mg/kg body weight once daily for 28 days prior to ulcer induction. At the end of the experiment, gastric secretions, antioxidant status, and histopathological alteration were evaluated. We observed that the significantly increased ulcer index, gastric volume, free and total acidity, malondialdehyde level, and pepsin activity were effectively reduced following treatment with 200 and 300 mg/kg MEVA. The extract also markedly attenuated the reduced activity of superoxide dismutase and reduced glutathione level as well as pH and mucin content in the ulcerated rats. Administration of the extract also significantly attenuates necrosis of the stomach tissue of the ulcerated rats. The results suggested that the MEVA leaf, preferably at 200 and 300 mg/kg body weight, ameliorated aspirin-induced gastric ulceration via antioxidative and H2receptor antagonist.

Background: This study assessed the responses of Blood Pressure (BP) and Heart Rate (HR) to change of posture, Sustained Handgrip (SH),deep breathing(DB) and Valsalva manoeuvre (VM) in order to determining the normal limits and pattern of Autonomic Cardiovascular Indices (ACI) among healthy young adults. Methods: Two hundred and four volunteer (98 men and 106 women) aged 18- 40 years (mean, 22.45 ± 4.86 years) participated in the study. After excluding systemic diseases by clinical evaluation, each participant performed five non-invasive cardiac autonomic function tests in series. Differences in supine and erect systolic BPs were assessed by sphygmomanometry, diastolic BP responses to SH were determined by dynamometry while HR variability in response to VM, DB and postural change were assessed by continuous electrocardiogram. Normal limits of ACI were determined at 5th and 95th percentiles. Results: The mean (normal limits) for SBP response to change of posture (mmHg); DBP response to sustained handgrip (mmHg); Valsalva ratio; heart rate response to deep breathing (beats per minute); PTI; tachycardia ratio; bradycardia ratio and RHR(beats per minute) respectively were: -4.26 ± 9.17(-19.00-11.00); 20.14 ± 17.35(0.00-54.00); 1.55 ± 0.40(1.10-2.40); 31.82 ± 10.81(13.0048.75); 1.41 ± 0.31(1.08-1.84); 0.80 ± 0.12(0.58-1.00); 1.21 ± 0.20(0.95-1.60) and 70.61 ± 12.23(53.00-93.00). During Valsalva maneouvre, Post-Strain Pause (PSP) which ranged from 1.2812.76s was observed in 14(6.86%) of the participants. PSP occurred more frequently in women (9.43%) than men (4.08%). No significant gender difference in the duration of post-strain pause (t=-1.628, p=0.105). In majority of the participants (67.6%), an elevation of SBP occurred within 2minutes of rising from supine to erect position. Immediate Post-Standing Pause (IPSP) occurred among 139 (68.14%) participants. Conclusion: Non-invasive assessment of cardiac autonomic function is feasible in Nigeria. Increase rather than decrease in systolic blood pressure response occurred more frequently upon a change from supine to erect posture. IPSP was a unique pattern of cardiac autonomic functions in study population.

The population of students all over the world is growing without a proportionate increase in teaching/learning resources/infrastructure. There is also much quest for learning in an environment that provides equal opportunities to all learners. The need to provide an equal opportunity learning environment that will hitherto improve the system of education globally has therefore become imperative. Based on our findings, a mathematical model Web-based Virtual Classroom system (WebVCS) was developed to provide a viable medium through which sound education can be offered in tertiary institutions that can carter for varieties of learners irrespective of their abilities, dispositions and geographical locations. Our system model is developed based on active learning approach that adopts blended learning theory (Constructivist-Cognivist learning approach), incorporating e-pedagogy that supports collaboration among participants in the web-based Virtual learning environment. The key objects used in creating the WebVCS model are: Courses, Students, Instructors and Learning performances. Our system model sets a framework for developers of virtual classrooms and successful implementation of the model leads to students learning by interacting with their peers resulting in the construction of knowledge.

To investigate the impact and possible mechanism of action of the rodent malarial parasite on reproduction. Male albino mice were infected with 15, 30 and 45% Plasmodium berghei berghei through inoculation with 107 parasitized red blood cells. Each experiment had its own control that was not infected with P. berghei berghei. Mice infected with 15% P. berghei berghei were killed on days 0, 5, 10 and 15; those infected with 30% P. berghei berghei were killed on days 0, 3, 6 and 10; and those infected with 45% P. berghei berghei were killed on days 1–7 after infection. Caudal epididymal sperm motility counts and morphology body and wet organ weights and hematological indices were determined. The results showed a progressive duration dependent decrease in sperm motility sperm count and viability (P < 0.01) in parasitized mice. There were significant decreases in serum testosterone and increases in cortisol levels (P < 0.05) in the infected mice compared with the controls. There was also a progressive decrease (P < 0.05) in red blood cell count and packed cell volume. However, there was a progressive increase (P < 0.01) in white blood cell count and weight of the spleen and liver. There was no significant change in weight of the testis and epididymides. The results suggest that the malaria parasite could depress male fertility indices.

Cocos nucifera L. is known as a tree of life because of its economic, domestic, and nutritional usefulness. Coconut oil (CO), which is derived from Cocos nucifera L., has received considerable attention because of its reported folkloric, nutritional, biological, and pharmacological properties. We previously reported the outstanding physicochemical properties of CO; therefore, we analyzed its antioxidant activity and chemical composition in this study. CO was extracted using a fermentation method with and without applying heat. Its antioxidant activity was investigated using the DPPH free radical scavenging method, the metal chelation capacity, the reduction of antioxidant power, and the nitric oxide scavenging capacity index. The compounds were identified using GC-MS. The data were expressed as a mean ± SEM and analyzed by one-way analysis of variance (ANOVA) using SPSS 21.0 with data being considered significant at P < 0.05. The results showed that CO demonstrated a concentration-dependent DPPH radical scavenging activity and nitric oxide scavenging capacity index, with the highest activity in the heat-extracted virgin CO (HEVCO). The ferric-reducing antioxidant power and metal chelation capacity significantly varied (P < 0.05) between the HEVCO and the cold extracted virgin CO (CEVCO). The GCMS analysis of virgin CO identified important active compounds. The results revealed a higher content of phenolic compounds in HEVCO compared to CEVCO. In conclusion, applying heat favored incorporating phenolic compounds into CO and consequently improved the antioxidant potential of HEVCO compared to CEVCO. [ABSTRACT FROM AUTHOR]

116. Botryosphaeriaceae causing branch dieback and tree death of macadamia in Australia.

- Abstract
Incidence of branch dieback of macadamia with characteristic symptoms including dark necrotic lesions on the wood and blackening of the vascular tissue is increasing in commercial macadamia orchards in the south-eastern production regions in Australia. In many cases, disease progresses from the branch to the main trunk resulting in total tree death and reduced orchard productivity. Previously, only Botryosphaeria ribis was associated with branch dieback of macadamia, however, recent observations suggest other species in the Botryosphaeriaceae may be involved. This study aimed to identify the causal agents of branch dieback of macadamia and examine their pathogenicity on a main macadamia cultivar (HAES 246). Thirty-four representative Botryosphaeriaceae isolates, obtained from over 150 samples of branch dieback symptoms on macadamia trees, were identified using the partial sequence of the internal transcribed spacer of the rDNA and partial sequences of β-tubulin and elongation factor gene regions. Six species in the Botryosphaeriaceae were identified. Lasiodiplodia pseudotheobromae (n = 18; 53%) was the most prevalent species, followed by Neofusiccocum parvum (n = 5; 14%), L. iraniensis (n = 4; 12%), N. luteum and L. theobromae (n = 3; 9% each) and N. australe (n = 1; 3%). Using an in planta assay, pathogenicity tests showed that all six species caused dieback and necrotic lesion symptoms on macadamia plants. Inoculated plants died within 4 weeks of inoculation, showing the characteristic symptoms (blackening of the wood tissue). 117. Influence of carrier water pH and hardness on imazapic efficacy for sicklepod (Senna obtusifoliaL.) control in peanut

AbstractCarrier water quality is an important consideration for herbicide efficacy. Field and greenhouse studies were conducted from 2021 to 2023 to evaluate the effect of carrier water pH and hardness on imazapic efficacy for sicklepod control in peanut crops. In separate field experiments imazapic was applied postemergence at 0.071 kg ai ha−1with carrier water pH levels of 5, 6, 7, 8, or 9; and hardness levels of 0 (deionized water), 100, 200, 400, or 500 mg L−1of CaCO3equivalent. In greenhouse experiments, imazapic was applied to sicklepod that was either 10 cm, 15 cm, or 20 cm tall at similar carrier water pH levels and hardness levels of 0, 100, 200, 400, or 800 mg L−1of CaCO3. In the field study, sicklepod control, density, and biomass reductions were lower with carrier water pH 5 or 9 compared with pH 7. In the greenhouse study, control was not different among carrier water pH levels when imazapic was applied to 10-cm-tall sicklepod; however, when applied to 15- or 20-cm-tall sicklepod, control was at least 25% greater with acidic (pH 5) compared to alkaline (pH 9) carrier water. Results from the field study showed that carrier water hardness ≤500 ppm did not reduce the efficacy of imazapic to control sicklepod. In the greenhouse study, regardless of sicklepod height, carrier water hardness of 800 mg L−1reduced sicklepod control by 15% and biomass reduction by 17% compared with deionized water (pH 7). The effects of carrier water pH and hardness on imazapic efficacy did not compromise peanut yield in the field study. However, this study indicates that both acidic and alkaline carrier water pH and hardness (800 mg L−1CaCO3L−1) have the potential to reduce imazapic efficacy on sicklepod, and appropriate spray solution amendments maybe be needed to maintain optimum efficacy.

- Abstract
Incidence of dry flower disease of macadamia (Macadamia integrifolia), expressed as blight of the flowers, necrosis and dieback of the rachis, is increasing in Australia. In the 2012/13 production season, incidence of dry flower disease resulted in 10% to 30% yield loss in the affected orchards. Etiology of the disease has not been established. This study was established to characterise the disease and identify the causal pathogen. A survey of the major macadamia producing regions in Australia revealed dry flower disease symptoms, regardless of cultivar or location at all stages of raceme development. Based on colony and conidial morphology, the majority (41%) of fungal isolates obtained from tissue samples were identified as Pestalotiopsis and Neopestalotiopsis spp. The phylogeny of the combined partial sequence of the internal transcribed spacer, beta-tubulin and translation elongation factor 1-alpha gene loci, segregated the isolates into two well supported clades, independent of location or part of the inflorescence affected. Further morphological examination supported the establishment of two new species, which are formally described as Neopestalotiopsis macadamiae sp. nov. and Pestalotiopsis macadamiae sp. nov. Using spore suspensions of isolates of both species, Koch?s postulates were fulfilled on three macadamia cultivars at all stages of raceme development. To our knowledge, this is the first report of species of Neopestalotiopsis and Pestalotiopsis as causal agents of inflorescence disease in macadamia.

The average prime and maximum lending rates were as high as 16.96 per cent 24.65 per cent in 2012. Conversely, the weighted average savings and term deposit rates stabilized at 5.30 per cent between July and October 2012.The level of interest rate is important in the mobilization and allocation of funds in an economy. Therefore, it is not surprising why the current high lending interest rate regime in Nigeria remains a source of concern to policy makers. This paper articulates the factors explaining the present high cost of funds in the Nigerian banking system and policy options useful in achieving low and sustainable interest rate objectives in Nigeria. Using descriptive analysis covering the 1990s, when the interest rate was fully deregulated in Nigeria, up until 2012; the paper notes that high interest rate is largely driven by high sunk costs due to dearth of infrastructure. It recommends a strong legal framework for addressing loan defaults. In this direction, review of the legal clause which gives undue advantage to borrowers is critical. A time frame for the disposal of loan default cases need to be set as prolonged litigations at the instance of loan defaulters usually stall the process of take over and the consequent auctioning of borrowers’ collateral by lenders to recover their loans. For over four decades, the Nigerian economy has mostly depended on proceeds from the sale of crude oil. This is at the expense of other sectors such as solid minerals and agriculture that hitherto, contributed significantly to the economy of Nigeria. Employing both qualitative and quantitative analysis, this paper shows that the solid mineral sector in Nigeria has the potential to contribute immensely to the economy of Nigeria. Specifically, the paper reveals that the development of the solid mineral sector could help to combat poverty in Nigeria via job creation; especially, given its forward linkage with other sectors of the economy. Most importantly, it could help alleviate some of the problems associated with “enclave” nature of the Nigerian economy that has for too long being vulnerable to fluctuations in global oil prices. To realize these potentials, the paper highlights the need for a strengthening of Nigeria’s existing solid mineral development policy. Abstract: Bambara groundnut is an indigenous African vegetable grown mainly for human food and animal feed due to its high protein content. Different factors like varieties and origin can influence the chemical composition of Bambara groundnut cultivars. Therefore, the aims of this study are to produce defatted flour and protein concentrate from newly developed Bambara groundnut cultivars [Accessions No: TVSU 5 – Bambara Groundnut White (BGW) and TVSU 146 – Bambara Groundnut Brown (BGB)] and compare their nutritional, physicochemical, and functional properties with market sample [Bambara groundnut commercial (BGC)]. Higher protein content was observed in BGW (20.73%) and BGB (20.14%) as compared to BGC (18.50%). Also, the fat and ash contents of BGB and BGW were higher than that of BGC. Also, the new varieties were found to contain higher levels of some essential fatty acids such as linoleic and linolenic acids. The concentration of thiamine, riboflavin, niacin, pantothenic, ascorbic acids, pyrodoxine, alpha tocopherol, and vitamin K were also significantly higher in the two new varieties. The new varieties were good sources of magnesium, calcium, iron, manganese, sodium, and potassium. The oil and water absorption and swelling capacities of whole, defatted, and protein concentrate flour of the new varieties increase with increase in temperature. The defatted flour and protein concentrate of brown Bambara groundnut was found to exhibit high emulsifying activity and stability at different pH's and salt concentrations. The new varieties possess significantly higher foaming capacity and stability than the commercial variety. The results obtained from this study have shown the potential for the industrial and household use of the new Bambara groundnut cultivars into shelf stable protein products and could be a useful ingredient in food formulations. Pawpaw (Carica papaya Linn) seeds, peels and leaves are well known for their medicinal and potent pharmacological values. The plant belongs to the family Caricaceae and its fruit is commonly known for its food and nutritional values. Though the roots, bark, peel, pulp and the seeds had been shown to have medicinal properties, little is known about their nutritional properties. During the last few years, there had been series of research to explore further pharmacological and medicinal properties as well as it nutritional qualities of these plants parts, thus giving rise to it nutraceutical abilities. Scientific attempts have also been made over the decades to authenticate its nutraceutical properties. While nutritional constituents such as oil, proteins, and minerals haven been characterised from its seeds, little is known about other parts of the plant. Though the nutraceutical properties of papaya as drug or quasi-food product are not well understood or value, still it is an important gift of nature that is readily available in our eco-world and worth exploring. This review focuses on previous studies by various researchers on the medicinal, nutraceutical properties as well as the antioxidant potential of these plant parts and suggestions for further studies as regarding the optimisation of the potential of this plant.

Aim: To investigate the effect of Morinda lucida Benth (Rubiaceae) on the reproductive activity of male albino rats. Methods: Two groups of rats were treated with 400 mg/(kg·d) of Morinda lucida leaf extract for 4 and 13 weeks, respectively. The control rats received the vehicle. All the treated rats had corresponding recovery groups. At the end of each experimental period, animals were killed and organ weights, sperm characteristics, serum testosterone levels, histology of the testes and fertility were assessed. Results: Morinda lucida leaf extract did not cause any changes in body and somatic organ weights, but significantly increased the testis weight (P < 0.05). The sperm motility and viability, and the epididymal sperm counts of rats treated for 13 weeks were significantly reduced (P < 0.05). Sperm morphological abnormalities and serum testosterone levels were significantly increased (P < 0.05). There were various degrees of damage to the seminiferous tubules. The extract reduced the fertility of the treated rats by reducing the litter size. Reversal of these changes, however, occurred after a period of time. Conclusion: The extract of Morinda lucida has reversible antispermatogenic properties. (Asian J Androl 2005 Dec; 7: 405–410)

Security of networking systems has been an issue since computer networks became prevalent, most especially now that Internet is changing the facie computing. Intrusions pose significant threats to the integrity, confidentiality and availability of information for the internet users. In this paper, a new approach to real-time network anomaly intrusion detection via Fuzzy-Bayesian is proposed to detect malicious activity against computer network; the framework is described to demonstrate the effectiveness of the technique. The combination of fuzzy with Bayesian classifier will improve the overall performance of Bayes based intrusion detection system (IDS). This paper presents an architectural approach to detect Distributed Denial of Service (DDoS) in Virtual Knowledge Community (VKC). A Denial-of-Service (DoS) attack is a malicious attempt by a single person or a group of people to cause the victim, site, or node to deny service to its customers. Virtual knowledge community is a virtual place where agents can meet, communicate and interact among themselves. The emergence of DDoS attack has become more frequent in today's network environment. In this paper, we develop a DDoS attack models for detecting attack traffics in VKC and propose measures to deal with such an attack. Mathematical models are generated to illustrate our approach. Introduction Dextrocardia with situs inversus is a rare congenital disease. In patients with this condition, the heart is presented as a mirror image of itself with its apex pointing to the right. The pulmonary and abdominal anatomies are reversed. Dextrocardia with situs inversus occurs at birth but its diagnosis may be in adulthood. This case advances knowledge by graphically describing the unusual electrocardiographic features of dextrocardia in a young adult. Case presentation We report a case of a 22-year-old Nigerian man of Yoruba ethnicity who presented himself for preadmission medical test. He had a standard 12-lead electrocardiogram which revealed uncommon features: inversion of P waves in leads I, aVL and aVR; dominantly negative QRS waves in leads I, V1 to V6; reverse R wave progression in chest leads; low voltage in V4 to V6; extreme QRS axis; flattened T waves in V4 to V6 and aVR; and inverted T waves in lead I and aVL. An electrocardiogram diagnosis of dextrocardia was made. The differential diagnosis considered was right ventricular hypertrophy. A cardiovascular examination showed pulse rate of 70 beats per minute, blood pressure of 119/62mmHg, visible cardiac impulse at right precordium, apex beat was located at his fifth right intercostal space mid-clavicular line. A chest X-ray (posterior anterior view) including upper abdomen showed dextrocardia; his aortic arch was located on the right. His stomach bubble was located below his right hemidiaphragm. His trachea was slightly deviated to the left. The findings in his lung fields were not remarkable. Abdominopelvic ultrasonography showed that right-sided intra-abdominal organs (liver, gallbladder) were located on the left while left-sided organs (stomach, spleen) were located on the right. His abdominal aorta was on the right while his inferior vena cava was located on the left. A diagnosis of dextrocardia with situs inversus was made ultrasonographically. Conclusions A properly interpreted electrocardiogram was useful in suspecting the diagnosis of dextrocardia with situs inversus. So, an analysis of a relatively simple and non-invasive diagnostic tool such as an electrocardiogram allows for suspicion of a cardiovascular anomaly in a setting of scarce diagnostic resources.

This study investigated the quantitative polyphenolic constituents and gastroprotective effects of methanol extract of Vernonia amygdalina leaf (MEVA) against aspirin-induced gastric ulcer in rats. Ulceration was induced by 3 days' oral administration of aspirin (150 mg/kg body weight). Wistar rats were pretreated with cimetidine (reference drug) at a dose of 100 mg/kg body weight and MEVA at 200, 300, and 400 mg/kg body weight once daily for 28 days prior to ulcer induction. At the end of the experiment, gastric secretions, antioxidant status, and histopathological alteration were evaluated. We observed that the significantly increased ulcer index, gastric volume, free and total acidity, malondialdehyde level, and pepsin activity were effectively reduced following treatment with 200 and 300 mg/kg MEVA. The extract also markedly attenuated the reduced activity of superoxide dismutase and reduced glutathione level as well as pH and mucin content in the ulcerated rats. Administration of the extract also significantly attenuates necrosis of the stomach tissue of the ulcerated rats. The results suggested that the MEVA leaf, preferably at 200 and 300 mg/kg body weight, ameliorated aspirin-induced gastric ulceration via antioxidative and H2 receptor antagonist. Background The correct management of immunocompromised patients with pneumonia is debated. We evaluated the prevalence, risk factors, and characteristics of immunocompromised patients coming from the community with pneumonia. Methods We conducted a secondary analysis of an international, multicenter study enrolling adult patients coming from the community with pneumonia and hospitalized in 222 hospitals in 54 countries worldwide. Risk factors for immunocompromise included AIDS, aplastic anemia, asplenia, hematological cancer, chemotherapy, neutropenia, biological drug use, lung transplantation, chronic steroid use, and solid tumor. Results At least 1 risk factor for immunocompromise was recorded in 18% of the 3702 patients enrolled. The prevalences of risk factors significantly differed across continents and countries, with chronic steroid use (45%), hematological cancer (25%), and chemotherapy (22%) the most common. Among immunocompromised patients, community-acquired pneumonia (CAP) pathogens were the most frequently identified, and prevalences did not differ from those in immunocompetent patients. Risk factors for immunocompromise were independently associated with neither Pseudomonas aeruginosa nor non–community-acquired bacteria. Specific risk factors were independently associated with fungal infections (odds ratio for AIDS and hematological cancer, 15.10 and 4.65, respectively; both P = .001), mycobacterial infections (AIDS; P = .006), and viral infections other than influenza (hematological cancer, 5.49; P < .001). Conclusions Our findings could be considered by clinicians in prescribing empiric antibiotic therapy for CAP in immunocompromised patients. Patients with AIDS and hematological cancer admitted with CAP may have higher prevalences of fungi, mycobacteria, and noninfluenza viruses.

Background: Aspiration community-acquired pneumonia (ACAP) and community-acquired pneumonia (CAP) in patients with aspiration risk factors (AspRFs) are infections associated with anaerobes, but limited evidence suggests their pathogenic role. Research Question: What are the aspiration risk factors, microbiology patterns, and empiric anti-anaerobic use in patients hospitalized with CAP? Study Design and Methods: This is a secondary analysis of GLIMP, an international, multicenter, point-prevalence study of adults hospitalized with CAP. Patients were stratified into three groups: (1) ACAP, (2) CAP/AspRF+ (CAP with AspRF), and (3) CAP/AspRF- (CAP without AspRF). Data on demographics, comorbidities, microbiological results, and anti-anaerobic antibiotics were analyzed in all groups. Patients were further stratified in severe and nonsevere CAP groups. Results: We enrolled 2,606 patients with CAP, of which 193 (7.4%) had ACAP. Risk factors independently associated with ACAP were male, bedridden, underweight, a nursing home resident, and having a history of stroke, dementia, mental illness, and enteral tube feeding. Among non-ACAP patients, 1,709 (70.8%) had CAP/AspRF+ and 704 (29.2%) had CAP/AspRF-. Microbiology patterns including anaerobes were similar between CAP/AspRF-, CAP/AspRF+ and ACAP (0.0% vs 1.03% vs 1.64%). Patients with severe ACAP had higher rates of total gram-negative bacteria (64.3% vs 44.3% vs 33.3%, P =.021) and lower rates of total gram-positive bacteria (7.1% vs 38.1% vs 50.0%, P 50% in all groups) independent of AspRFs or ACAP received specific or broad-spectrum anti-anaerobic coverage antibiotics. Interpretation: Hospitalized patients with ACAP or CAP/AspRF+ had similar anaerobic flora compared with patients without aspiration risk factors. Gram-negative bacteria were more prevalent in patients with severe ACAP. Despite having similar microbiological flora between groups, a large proportion of CAP patients received anti-anaerobic antibiotic coverage.

BACKGROUND: Antibiotic resistance is a major global health problem and pathogens such as meticillin-resistant Staphylococcus aureus (MRSA) have become of particular concern in the management of lower respiratory tract infections. However, few data are available on the worldwide prevalence and risk factors for MRSA pneumonia. We aimed to determine the point prevalence of MRSA pneumonia and identify specific MRSA risk factors in community-dwelling patients hospitalised with pneumonia.METHODS: We did an international, multicentre study of community-dwelling, adult patients admitted to hospital with pneumonia who had microbiological tests taken within 24 h of presentation. We recruited investigators from 222 hospitals in 54 countries to gather point-prevalence data for all patients admitted with these characteristics during 4 days randomly selected during the months of March, April, May, and June in 2015. We assessed prevalence of MRSA pneumonia and associated risk factors through logistic regression analysis.FINDINGS: 3702 patients hospitalised with pneumonia were enrolled, with 3193 patients receiving microbiological tests within 24 h of admission, forming the patient population. 1173 (37%) had at least one pathogen isolated (culture-positive population). The overall prevalence of confirmed MRSA pneumonia was 3·0% (n=95), with differing prevalence between continents and countries. Three risk factors were independently associated with MRSA pneumonia: previous MRSA infection or colonisation (odds ratio 6·21, 95% CI 3·25-11·85), recurrent skin infections (2·87, 1·10-7·45), and severe pneumonia disease (2·39, 1·55-3·68).INTERPRETATION: This multicountry study shows low prevalence of MRSA pneumonia and specific MRSA risk factors among community-dwelling patients hospitalised with pneumonia.FUNDING: None.

Pawpaw (Carica papaya Linn) seeds, peels and leaves are well known for their medicinal and potent pharmacological values. The plant belongs to the family Caricaceae and its fruit is commonly known for its food and nutritional values. Though the roots, bark, peel, pulp and the seeds had been shown to have medicinal properties, little is known about their nutritional properties. During the last few years, there had been series of research to explore further pharmacological and medicinal properties as well as it nutritional qualities of these plants parts, thus giving rise to it nutraceutical abilities. Scientific attempts have also been made over the decades to authenticate its nutraceutical properties. While nutritional constituents such as oil, proteins, and minerals haven been characterised from its seeds, little is known about other parts of the plant. Though the nutraceutical properties of papaya as drug or quasi-food product are not well understood or value, still it is an important gift of nature that is readily available in our eco-world and worth exploring. This review focuses on previous studies by various researchers on the medicinal, nutraceutical properties as well as the antioxidant potential of these plant parts and suggestions for further studies as regarding the optimisation of the potential of this plant.

This study aimed to describe real-life microbiological testing of adults hospitalised with community-acquired pneumonia (CAP) and to assess concordance with the 2007 Infectious Diseases Society of America (IDSA)/American Thoracic Society (ATS) and 2011 European Respiratory Society (ERS) CAP guidelines. This was a cohort study based on the Global Initiative for Methicillin-resistant Staphylococcus aureus Pneumonia (GLIMP) database, which contains point-prevalence data on adults hospitalised with CAP across 54 countries during 2015. In total, 3702 patients were included. Testing was performed in 3217 patients, and included blood culture (71.1%), sputum culture (61.8%), Legionella urinary antigen test (30.1%), pneumococcal urinary antigen test (30.0%), viral testing (14.9%), acute-phase serology (8.8%), bronchoalveolar lavage culture (8.4%) and pleural fluid culture (3.2%). A pathogen was detected in 1173 (36.5%) patients. Testing attitudes varied significantly according to geography and disease severity. Testing was concordant with IDSA/ATS and ERS guidelines in 16.7% and 23.9% of patients, respectively. - Abstract
Objective: To evaluate the reliability of obstetric handheld smartphone-based point-of-care ultrasound (POCUS) in a resource-limited high-volume setting., Methods: A single-center prospective observational study among women requiring urgent diagnosis and admitted to a maternity referral hospital in Sierra Leone from March to April 2019. Pre-specified ultrasound findings were obtained with a handheld POCUS device; a comprehensive ultrasound examination was then performed by an experienced operator using conventional full-feature apparatus. Agreement was assessed by diagnostic accuracy and Cohen κ-statistics., Results: Overall, there were 307 participants. The mean aggregated diagnostic accuracy was 95.5% (κ-statistic, 0.90; 95% confidence interval [CI], 0.89-0.93; P < 0.001). Highest accuracy was reported for detecting free fluid collection in the abdominal cavity (100%; κ-statistic, 1.00; 95% CI, 1.00-1.00; P < 0.001). Ultrasound findings obtained with the handheld device for intrauterine pregnancy, fetal heartbeat, cephalic presentation, multifetal pregnancy, and assessment of gestational age based on bi-parietal diameter were highly reliable (agreement, >90%; κ-statistic, >0.80). Detection of low-lying placenta or placenta previa was the least reliable (κ-statistic, 0.53; 95% CI, 0.13-0.93; P < 0.001)., Conclusion: Handheld POCUS findings were found to be reliable for detecting pre-specified urgent obstetric findings in a high-volume resource-limited referral hospital., (© 2020 International Federation of Gynecology and Obstetrics.)
