It is argued that the provision of fundamental nursing care is the very essence of what it means to be a nurse. However, in contemporary health care contexts the pursuit of increased efficiencies has seen the concern with care diminish. The literature indicates that in these circumstances, the key providers of care to patients in hospitals, general medical nurses, are less than happy with the situation. In part this is reflected in the problems currently facing health care agencies with recruiting and retaining nurses. In a climate where recruitment and retention of experienced nurses is a major political issue, it is interesting to note that within the nursing literature, there is a virtual absence of research which addresses the issues and concerns of nurses who choose to work in a general medical ward setting. In order to begin the process of addressing this hiatus, this study utilized a critical research. method to explore the issues and concerns of five nurses who work as general medical nurses. Consistent with research that has a critical intent, the nurses engaged in 'critical dialogues' which were analysed and developed into a discussion paper. The discussion paper was then returned to the nurses, and the issues raised were subjected to considerable debate when the participants subsequently met as a group. The findings of the research provide keen insights into the experiences, issues and concerns of this group of general medical nurses. They highlight the operation of an array of conflicting interests which negatively impact on the capacity of general medical nurses to provide quality nursing care. Significantly, the perceived lowering of standards of patient care has caused the participants of this study to question their integrity and identity as nurses. However, given the opportunity to engage in a dialogue and to critically reflect on the issues raised, the nurses developed new insights into their situation which affirmed their worth as general medical nurses.

The professional identity of nursing students involves student nurses perceiving themselves as developing the attributes of nursing professionals. The aim of this study was to explore the influence of social media usage on the development of Saudi undergraduate nursing students' professional identity using an exploratory sequential mixed methods design. It is concluded that the more students purposefully engaged with social media, the more they developed their nursing professional identity. Social media is a powerful tool that can benefit nursing students in many ways, including opening the gate on their path to becoming nurses.

During clinical placement nursing students may observe clinical practices that breach safety standards. Students are often perceived as inferior and struggle to belong as a health care team member. This creates uncertainty for students about situations where they feel they need to speak up to prevent patient harm. The difficulties are amplified in rural contexts due to the lack of structured support, a lack of education resources, and the absence of the multidisciplinary team at the site. Through an Interpretive Description lens this study aimed to create practice-based solutions that enable students to speak up without fear. This study began with a rigorous review and analysis of the concept of speaking up for students. A two-phased sequential data collection process involved twelve in-depth interviews and six focus groups with students to examine their perceptions, experiences and influences on speaking up during rural placement. The participants were recruited from two universities who completed a placement in rural and regional clinical placement settings. The findings revealed that students encounter a complex alienating culture that undermines their psychological safety and compromises their ability to speak up. Students are conflicted when witnessing breaches in policy and practice and are confused when health care staff justify their unsafe practices. Students speaking up behaviours correlate with experiences they have during placement. Learning to speak up is complex and the trajectory to becoming confident and competent is not clear-cut. The ability and willingness to speak up is influenced by underlying intricacies relating to people and the workplace culture. Students become aware of potential risks associated with speaking up, gaining a sense of agency and developing strategies to mitigate risk. The complex nature of the placement setting and diversity in patient safety curricula creates challenges for students to speak up in practice. Nursing students are the future healthcare workforce who need to be valued and included in the safety culture to feel psychologically safe and have a sense of agency, enabling them to voice their concerns and contribute to preventing patient harm.

Clinical reasoning incorporates the use of critical thinking, discipline-specific knowledge, and experiential understanding in a non-linear process. Registered nurses use clinical reasoning to understand situations, identify issues, and make professional judgements and decisions involving nursing practice; however, concerns surrounding the preparedness of graduate registered nurses to use clinical reasoning are evident in research (del Bueno, 2005; Gillespie & Peterson, 2009; Levett-Jones et al., 2010; Purling & King, 2012). Underdeveloped clinical reasoning has been linked with adverse events and failure-to-rescue scenarios, with research now examining the capability of tertiary institutions to develop the clinical reasoning of registered nurses to a safe level prior to completion of their studies (Kavanagh & Szweda, 2017; Liao et al., 2016; Missen et al., 2016). Currently, there are no discipline-specific tools to assess the clinical reasoning of undergraduate registered nurses throughout their Bachelor of Nursing program and only sparse research on what clinical reasoning looks like and how it is demonstrated in undergraduate registered nurses. To determine the standard of clinical reasoning required by undergraduate registered nurses upon completion of their preregistration studies, the clinical reasoning of registered nurses working in clinical practice was examined. This information formed the groundwork for the development of a professionally contextualised assessment framework for use in undergraduate registered nursing programs. A multi-phase study featuring a scoping review, benchmarking against practice standards, a literature review and a modified Delphi study was used to develop the Clinical Reasoning Achievement-Based (CRAB) framework. A further confirmation study was undertaken to test the useability, reliability, and validity of the developed framework. In the scoping review, registered nurses were found to employ numerous interrelated characteristics and processes of clinical reasoning while working in practice with a multitude of factors influencing their clinical reasoning. The majority of the identified characteristics and processes aligned with the Nursing and Midwifery Board of Australia's Registered Nurse Standards for Practice (2016). The expert panel from the modified Delphi study determined which characteristics and processes of clinical reasoning should be included in the framework, as well as the level at which undergraduate registered nurses should be demonstrating these. The expert panel also agreed a clinical case scenario was the most appropriate method to assess the clinical reasoning of undergraduate registered nurses with the CRAB framework eventually comprising three components: a CRAB Assessment Tool, CRAB Explanatory Notes and CRAB Clinical Case Scenario requirements. The CRAB framework was found to assess clinical reasoning, with the format of the CRAB Assessment Tool identified by the assessor as easy to use; however, further testing is required. The CRAB Clinical Case Scenario requirements were found to strengthen the development of clinical case scenarios, which supported the CRAB Assessment Tool.

Like most things in today's world, health technology has undergone many changes over the past few decades. Medical and technological advances have changed many health practices. The subject of this study is perioperative personnels' knowledge and understanding of radiation in the perioperative environment. The concern of this study is whether perioperative personnel who are exposed to radiation during their daily work routines are adapting their work practices in line with this changing technology, specifically regarding radiation in the Operating Suite. A focus group study was undertaken to determine the knowledge and understanding of radiation exposure and safety held by perioperative personnel during their daily work routines within the Operating Suite. The personnel studied came from diverse backgrounds with varying education levels and experiences. They consisted of preoperative nurses, theatre assistants, surgeons and anaesthetists. A total of 23 personnel from the population of 82 participated in four homogeneous focus groups. The discussions were tape-recorded and copies of the transcripts and preliminary analysis were returned to participants for correction, comment and verification. Following the processes of data collection and ongoing data analysis five intra and inter related categories were identified. These were the dangers of radiation exposure; the lack of knowledge and education on radiation exposure and safety; the radiation environment; protective devices and apparel; and, the radiographer's role and responsibilities. The study identified self -determined deficits in the knowledge and understanding of radiation exposure and safe radiation practices by the participants. The results demonstrate that, at this point in time, safe radiation practices by perioperative personnel are not optimised within this environment. The recommendations - education on radiation exposure and safety, the appropriate quantity and quality of protective apparel, attention given to signage and warning systems, the roles and responsibilities of radiographers identified and enacted upon; and, future research involving the monitoring of perioperative personnel to determine whether safe exposure levels were not exceeded - could indeed provide a safer perioperative environment. It has also been recommended that policies and procedures and an education package covering the use of radiation within the perioperative environment are required

Idiopathic Pulmonary Fibrosis (IPF) is a rare terminal respiratory disease caused by scarring of the terminal airways resulting in eventual respiratory failure (King et al. 2011). IPF has a median life expectancy of three to five years, however the disease trajectory is often unpredictable (Martinez et al. 2005 & Lindell et al. 2010). While both nonpharmacological and pharmacological interventions are available, lung transplantation is the only treatment option associated with reduced mortality rates (Sampson et al. 2015). However, only a minority of individuals diagnosed with IPF receive a lung transplant (Irish Thoracic Society 2018), therefore symptom management is imperative. In correlation with the National Patient Charter for IPF, palliative care implementation at diagnosis is recommended for optimum symptom management (Irish Lung Fibrosis Association 2015). In contrast, current literature appears to be inconsistent with these guidelines and portrays evidence of poor palliative care implementation practice in IPF at present. As nurses predominantly spend the greatest amount of time with patients (Delucia et al. 2009), the nurses’ experiences on the provision of palliative care in IPF including the impact on the nursing professional appears vital to enhance future care for those with IPF. Thus, the aim of this study is to explore nurses’ experiences in relation to the delivery of palliative care in IPF to improve future care for both patients and nursing professionals. To address this research question, a qualitative description (QD) approach was used to explore nurses’ experiences. Semi structured interviews facilitated data collection while data analysis was conducted through a thematic analysis (TA) strategy. Four themes emerged from the data collected, hope, symptom burden, nurse impact and palliative care implementation measures to name some. In conclusion, IPF is associated with high symptom burden thus palliative care is required for better patient outcomes. However, as a result of stigma, palliative care discussions often result in a sense of hopelessness for those with IPF. Overall improvement is required in palliative care implementation in current practice; nevertheless, implementation timing should be assessed on an individual level. Strategies such as disease awareness, palliative care education and improved communication are required to enhance IPF management in the future.
