A study of the pharmacokinetics and biological actions of C-type natriuretic peptide (CNP) in sheep through an analysis of the renal, hemodynamic and hormonal effects of two doses, dose 1 and dose 10, of intravenous CNP-22, in normal conscious sheep reveals the natriuretic activity of CNP in sheep and that cardiac output is reduced by CNP without any effects on arterial pressure. Sodium excretion, following CNP administration, exhibits a delayed increase. Plasma CNP levels go up from a basal value to a plateau with both doses.

Some studies have shown an increase in mortality, as well as poorer outcomes, among stroke patients who had either higher blood sugar levels on admission to the hospital, or who were diabetic. At one time it was thought that a high level of glucose in the blood was directly harmful to the brain, which had already been compromised by lack of oxygen. Later studies suggested that the action of glucose might be indirect, a consequence of stress caused by the severity of the stroke. However, not all studies confirmed a poorer prognosis among diabetics, but this is a complicated research issue because diabetes mellitus often goes undiagnosed. The relationship of glucose and outcome is important because the glucose level in the blood can be modified with treatment. In this study, 304 subjects with acute stroke were divided into three groups based on the type of stroke. They were examined within two days of the event, and again after three months. Higher glucose levels on admission were found to be associated with poorer outcomes, but only in subjects with intracerebral hemorrhage, bleeding inside the brain. Diabetics did not have a higher mortality than nondiabetics. They were worse off at the time they left the hospital, but after three months there was no difference in function level between diabetics and nondiabetics. Therefore it seems that the association between glucose level and outcome is most likely an indication of stress related to the severity of the stroke, and not a result of the adverse effects of glucose on damaged neurons. Atrial natriuretic peptide (ANP) is a small protein hormone which is produced by the atrium (upper heart chamber) when it is subjected to pressure or stretch. ANP secretion is thus expected during heart failure or chronic kidney failure. ANP influences blood pressure and volume, and stimulates excretion of salt in urine. Little is known about the response of ANP secretion to quick reductions in atrial pressure. ANP response was characterized during the pacemaker treatment of four patients with blocks in the conduction of electrical current from the atrium to the ventricle (lower heart chamber). Following successful pacemaker adjustment, right atrial pressure decreased by over twofold, pulmonary arterial pressure decreased similarly, and the volume cardiac output increased, indicating more effective pumping had been achieved. These changes stabilized after two minutes and remained stable. However, ANP decreased steadily for 20 minutes, to levels half those of initial values, and these values then persisted for another 40 minutes. This time-dependent decay in ANP secretion after an abrupt change in cardiovascular functional status should be considered when interpreting changes in ANP levels. The electron temperature and density pedestals tend to vary in their relative radial positions, as observed in DIII-D (Beurskens et al 2011 Phys. Plasmas 18 056120) and ASDEX Upgrade (Dunne et al 2017 Plasma Phys. Control. Fusion 59 14017). This so-called relative shift has an impact on the pedestal magnetohydrodynamic (MHD) stability and hence on the pedestal height (Osborne et al 2015 Nucl. Fusion 55 063018). The present work studies the effect of the relative shift on pedestal stability of JET ITER-like wall (JET-ILW) baseline low triangularity (δ) unseeded plasmas, and similar JET-C discharges. As shown in this paper, the increase of the pedestal relative shift is correlated with the reduction of the normalized pressure gradient, therefore playing a strong role in pedestal stability. Furthermore, JET-ILW tends to have a larger relative shift compared to JET carbon wall (JET-C), suggesting a possible role of the plasma facing materials in affecting the density profile location. Experimental results are then compared with stability analysis performed in terms of the peeling-ballooning model and with pedestal predictive model EUROPED (Saarelma et al 2017 Plasma Phys. Control. Fusion). Stability analysis is consistent with the experimental findings, showing an improvement of the pedestal stability, when the relative shift is reduced. This has been ascribed mainly to the increase of the edge bootstrap current, and to minor effects related to the increase of the pedestal pressure gradient and narrowing of the pedestal pressure width. Pedestal predictive model EUROPED shows a qualitative agreement with experiment, especially for low values of the relative shift. EURATOM 633053 Swedish Energy Agency 40146-1

The dependence of plasma transport and confinement on the main hydrogenic ion isotope mass is of fundamental importance for understanding turbulent transport and, therefore, for accurate extrapolations of confinement from present tokamak experiments, which typically use a single hydrogen isotope, to burning plasmas such as ITER, which will operate in deuterium-tritium mixtures. Knowledge of the dependence of plasma properties and edge transport barrier formation on main ion species is critical in view of the initial, low-activation phase of ITER operations in hydrogen or helium and of its implications on the subsequent operation in deuterium-tritium. The favourable scaling of global energy confinement time with isotope mass, which has been observed in many tokamak experiments, remains largely unexplained theoretically. Moreover, the mass scaling observed in experiments varies depending on the plasma edge conditions. In preparation for upcoming deuterium-tritium experiments in the JET tokamak with the ITER-like Be/W Wall (JET-ILW), a thorough experimental investigation of isotope effects in hydrogen, deuterium and tritium plasmas is being carried out, in order to provide stringent tests of plasma energy, particle and momentum transport models. Recent hydrogen and deuterium isotope experiments in JET-ILW on L-H power threshold, L-mode and H-mode confinement are reviewed and discussed in the context of past and more recent isotope experiments in tokamak plasmas, highlighting common elements as well as contrasting observations that have been reported. The experimental findings are discussed in the context of fundamental aspects of plasma transport models.

Abstract Background Vitamin D deficiency is associated with heightened risk of cardiovascular disease. Potential mechanisms include involvement of vitamin D in regulation of renin-angiotensin system and manufacture and secretion of cardiac natriuretic peptides. Our aim was to document relationships between 25 hydroxyvitamin [25(OH)D] and N-terminal pro B-type natriuretic peptide (NT-proBNP) and plasma renin activity (PRA) levels and to document the effect of vitamin D administration on NT-proBNP and PRA levels in vitamin D deficient subjects. Methods Serum 25(OH)D, parathyroid hormone (PTH), plasma or serum NT-proBNP and PRA levels were measured at baseline in nulliparous and lactating women and after 2 months of oral vitamin D2 (2,000 IU/day or 60,000 IU/month) supplementation to lactating women. Results Baseline levels of 25(OH)D were low ( Conclusion We found no significant correlations between 25(OH)D or PTH with NT-proBNP and PRA in vitamin D deficient women. Further information is required to clarify the effects of vitamin D administration on cardiac structure and function.

Left ventricular (LV) function and survival can be improved with captopril when initiated later than 24 hours after acute myocardial infarction. Animal studies suggest additional benefits may be obtained with earlier initiation of angiotensin-converting enzyme (ACE) inhibitors. The effects on LV function of captopril and enalapril initiated within 24 hours of myocardial infarction were studied. Two hundred twenty-five patients with acute myocardial infarction were enrolled within 24 hours of the onset of chest pain. They were randomized to receive either captopril 25 mg three times daily, enalapril 5 mg three times daily, or placebo. LV ejection fraction (EF) and volumes were measured by radionuclide ventriculography at baseline during treatment and at 3 months after a 3-day withdrawal from therapy. The ACE inhibitor group had a significant increase in EF (45 [+ or -] 1 to 47 [+ or -] 1%; p = 0.005) and significantly attenuated LV dilatation compared with results in the placebo group (175 [+ or -] 6 to 189 [+ or -] 7 ml in the placebo group vs 168 [+ or -] 4 to 172 [+ or -] 4 ml in the ACE inhibitor group; p = 0.051 for LV end-diastolic volume; and 99 [+ or -] 6 to 108 [+ or -] 7 ml in the placebo group vs 94 [+ or -] 3 to 94 [+ or -] 4 ml; p = 0.026 for LV end-systolic volume). The beneficial effects of ACE inhibitor therapy on LV function were observed irrespective of the degree of initial LV dysfunction and were comparable in bath the captopril and enalapril groups. Survival at 90 days and 12 months was significantly improved in the enalapril group (7 placebo, 9 captopril, 1 enalapril [p = 0.038] and 12 placebo, 10 captopril, 2 enalapril [p = 0.022]; deaths at 90 days and 12 months, respectively). Immediate administration of captopril and enalapril improved LV function and prevented LV dilatation after acute myocardial infarction. The benefit was similar with both ACE inhibitors and was in excess of the benefits of optimal conventional therapy. (Am J Cardiol 1994;73:1180-1186)

Erosion of plasma-facing materials and successive transport and redeposition of eroded material are crucial processes determining the lifetime of plasma-facing components and the trapped tritium inventory in redeposited material layers. Erosion and deposition in the JET divertor were studied during the second JET ITER-like wall campaign ILW-2 in 2013-2014 by using a poloidal row of specially prepared divertor marker tiles including the tungsten bulk tile 5. The marker tiles were analyzed using elastic backscattering with 3-4.5 MeV incident protons and nuclear reaction analysis using 0.8-4.5 MeV 3He ions before and after the campaign. The erosion/deposition pattern observed during ILW-2 is qualitatively comparable to the first campaign ILW-1 in 2011-2012: deposits consist mainly of beryllium with 5-20 at.% of carbon and oxygen and small amounts of Ni and W. The highest deposition with deposited layer thicknesses up to 30 μm per campaign is still observed on the upper and horizontal parts of the inner divertor. Outer divertor tiles 5, 6, 7 and 8 are net W erosion areas. The observed D inventory is roughly comparable to the inventory observed during ILW-1. The results obtained during ILW-2 therefore confirm the positive results observed in ILW-1 with respect to reduced material deposition and hydrogen isotopes retention in the divertor.

Post mortem analyses of JET ITER-Like-Wall tiles and passive diagnostics have been completed after each of the first two campaigns (ILW-1 and ILW-2). They show that the global fuel inventory is still dominated by co-deposition; hence plasma parameters and sputtering processes affecting material migration influence the distribution of retained fuel. In particular, differences between results from the two campaigns may be attributed to a greater proportion of pulses run with strike points in the divertor corners, and having about 300 discharges in hydrogen at the end of ILW-2. Recessed and remote areas can contribute to fuel retention due to the larger areas involved, e.g. recessed main chamber walls, gaps in castellated Be main chamber tiles and material migration to remote divertor areas. The fuel retention and material migration due to the bulk W Tile 5 during ILW-1 are presented. Overall these tiles account for only a small percentage of the global accountancy for ILW-1.

© 2017 Technical University of Denmark. We demonstrate the measurement of a 2D MeV-range ion velocity distribution function by velocity-space tomography at JET. Deuterium ions were accelerated into the MeV-range by third harmonic ion cyclotron resonance heating. We made measurements with three neutron emission spectrometers and a high-resolution γ-ray spectrometer detecting the γ-rays released in two reactions. The tomographic inversion based on these five spectra is in excellent agreement with numerical simulations with the ASCOT-RFOF and the SPOT-RFOF codes. The length of the measured fast-ion tail corroborates the prediction that very few particles are accelerated above 2 MeV due to the weak wave-particle interaction at higher energies.
