Deep neural network models have recently achieved state-of-the-art performance gains in a variety of natural language processing (NLP) tasks (Young, Hazarika, Poria, & Cambria, 2017). However, these gains rely on the availability of large amounts of annotated examples, without which state-of-the-art performance is rarely achievable. This is especially inconvenient for the many NLP fields where annotated examples are scarce, such as medical text. To improve NLP models in this situation, we evaluate five improvements on named entity recognition (NER) tasks when only ten annotated examples are available: (1) layer-wise initialization with pre-trained weights, (2) hyperparameter tuning, (3) combining pre-training data, (4) custom word embeddings, and (5) optimizing out-of-vocabulary (OOV) words. Experimental results show that the F1 score of 69.3% achievable by state-of-the-art models can be improved to 78.87%., Comment: 10 pages, 4 figures, 4 tables

- Abstract
Background: Alzheimer's disease (AD) is a neurodegenerative condition for which there is currently no available medication that can stop its progression. Previous studies suggest that mild cognitive impairment (MCI) is a phase that precedes the disease. Therefore, a better understanding of the molecular mechanisms behind MCI conversion to AD is needed. Method: Here, we propose a machine learning-based approach to detect the key metabolites and proteins involved in MCI progression to AD using data from the European Medical Information Framework for Alzheimer's Disease Multimodal Biomarker Discovery Study. Proteins and metabolites were evaluated separately in multiclass models (controls, MCI and AD) and together in MCI conversion models (MCI stable vs converter). Only features selected as relevant by 3/4 algorithms proposed were kept for downstream analysis. Results: Multiclass models of metabolites highlighted nine features further validated in an independent cohort (0.726 mean balanced accuracy). Among these features, one metabolite, oleamide, was selected by all the algorithms. Further in-vitro experiments in rodents showed that disease-associated microglia excreted oleamide in vesicles. Multiclass models of proteins stood out with nine features, validated in an independent cohort (0.720 mean balanced accuracy). However, none of the proteins was selected by all the algorithms. Besides, to distinguish between MCI stable and converters, 14 key features were selected (0.872 AUC), including tTau, alpha-synuclein (SNCA), junctophilin-3 (JPH3), properdin (CFP) and peptidase inhibitor 15 (PI15) among others. Conclusions: This omics integration approach highlighted a set of molecules associated with MCI conversion important in neuronal and glia inflammation pathways.

- Abstract
Protein structure determination has made progress with the aid of deep learning models, enabling the prediction of protein folding from protein sequences. However, obtaining accurate predictions becomes essential in certain cases where the protein structure remains undescribed. This is particularly challenging when dealing with rare, diverse structures and complex sample preparation. Different metrics assess prediction reliability and offer insights into result strength, providing a comprehensive understanding of protein structure by combining different models. In a previous study, two proteins named ARM58 and ARM56 were investigated.These proteins contain four domains of unknown function and are present in Leishmania spp. ARM refers to an antimony resistance marker. The study’s main objective is to assess the accuracy of the model’s predictions,thereby providing insights into the complexities and supportingmetrics underlying these findings. The analysis also extends to the comparison of predictions obtained from other species and organisms. Notably, one of these proteins shares an ortholog with Trypanosoma cruzi and Trypanosoma brucei, leading further significance to our analysis. This attempt underscored the importance of evaluating the diverse outputs from deep learning models, facilitating comparisons across different organisms and proteins. This becomes particularly pertinent in cases where no previous structural information is available.

Esta publicación se deriva de la Tesis Doctoral de Dña Cristina Bell Fenellos defendida en la Universidad Complutense de Madrid. Investigación con un enfoque interdisciplinar necesario al tratarse de un estudio cuya población es una enfermedad rara., Chromosome-5p minus syndrome (5p-Sd, OMIM #123450) formerly known as Cri duChat syndrome results from the loss of genetic material at the distal region of the short arm of chromosome 5. It is a neurodevelopmental disorder of genetic cause. So far, about 400 patients have been reported worldwide. Individuals affected by this syndrome have large phenotypic heterogeneity. However, a specific phenotype has emerged including global developmental delay, microcephaly, delayed speech, some dysmorphic features, and a characteristic and monochromatic high-pitch voice, resembling a cat’s cry. We here describe a cohort of 70 patients with clinical features of 5p- Sd characterized by means of deep phenotyping, SNP arrays, and other genetic approaches. Individuals have a great clinical and molecular heterogeneity, which can be partially explained by the existence of additional significant genomic rearrangements in around 39% of cases. Thus, our data showed significant statistical differences between subpopulations (simple 5p deletions versus 5p deletions plus additional rearrangements) of the cohort. Background: In recent years, cases of leishmaniosis have been described in animals housed in captivity in zoos in Spain [Bennett's wallaby (Macropus rufogriseus rufogriseus), orangutan (Pongo pygmaeus pygameus), and European otter (Lutra lutra)]. Some of these zoological parks are in endemic areas for both human and animal leishmaniosis, thus it should be very important to include this zoonosis in the differential diagnosis. Methods: The study was carried out in two zoological parks in Madrid, Madrid Zoo and Faunia, and analyzed seven meerkats. Serological tests [rK-39 and enzyme-linked immunosorbent assay (ELISA)] and molecular tests [nested polymerase chain reaction (PCR) and real-time PCR] were performed to detect Leishmania DNA. Additionally, an entomological study was carried out in both zoological parks, with molecular tests performed on female Phlebotomus perniciosus sand flies to determine their blood meal source and detect Leishmania DNA. Results: Two meerkats were positive for L. infantum. A 9-year-old male from the Madrid Zoo died suddenly, showing pale mucous membranes and bilateral noninflammatory alopecia and hyperpigmentation in the lateral area of the eyes. Positive results were obtained in serology, nested PCR, and real-time PCR (blood, conjunctival and oral swabs, hair, spleen, lymph node, liver, kidney, and skin), as well as numerous amastigotes in the liver and kidney tissue samples. The other meerkat, a 12-year-old male from Faunia that is still alive, presented an alopecic lesion at the base of the tail. Positive results were obtained by nested and real-time PCR from different tissues such as blood, hair, oral, and conjunctival swabs. It was treated with oral allopurinol (25 mg/kg) and miltefosine (2 mg/kg), but the molecular diagnosis remained positive after 8 months, regarding it as a mild stage of the disease. The rest of the tested meerkats were negative. The presence of P. perniciosus phlebotomine sand flies was also detected in both zoos. Although no L. infantum DNA was detected in any of sand flies analyzed, it was determined that their food sources were rabbits and humans. Conclusions: To our knowledge, this study describes, for the first time, the detection and infection by L. infantum in meerkats (Suricata suricatta). [ABSTRACT FROM AUTHOR]

We present the second paper of a series of publications aiming at obtaining a better understanding regarding the nature of type Ia supernovae (SNIa) progenitors by studying a large sample of detached F, G and K main sequence stars in close orbits with white dwarf companions (i.e. WD+FGK binaries). We employ the LAMOST (Large Sky Area Multi-Object Fibre Spectroscopic Telescope) data release 4 spectroscopic data base together with GALEX (Galaxy Evolution Explorer) ultraviolet fluxes to identify 1,549 WD+FGK binary candidates (1,057 of which are new), thus doubling the number of known sources. We measure the radial velocities of 1,453 of these binaries from the available LAMOST spectra and/or from spectra obtained by us at a wide variety of different telescopes around the globe. The analysis of the radial velocity data allows us to identify 24 systems displaying more than 3sigma radial velocity variation that we classify as close binaries. We also discuss the fraction of close binaries among WD+FGK systems, which we find to be ~10 per cent, and demonstrate that high-resolution spectroscopy is required to efficiently identify double-degenerate SNIa progenitor candidates., Comment: accepted for publication by MNRAS

Topological insulating phases are primarily associated with condensed-matter systems, which typically feature short-range interactions. Nevertheless, many realizations of quantum matter can exhibit long-range interactions, and it is still largely unknown the effect that these latter may exert upon the topological phases. In this Letter, we investigate the Su-Schrieffer-Heeger topological insulator in the presence of long-range interactions. We show that this model can be readily realized in quantum simulators with trapped ions by means of a periodic driving. Our results indicate that the localization of the associated edge states is enhanced by the long-range interactions, and that the localized components survive within the ground state of the model. Simpson‐Golabi‐Behmel syndrome (SGBS) is a rare congenital overgrowth condition characterized by macrosomia, macroglossia, coarse facial features, and development delays. It is caused by pathogenic variants in the GPC3 gene on chromosome Xq26.2. Here, we performed a comprehensive literature review and phenotyping of known patients with molecularly confirmed SGBS and reviewed a novel cohort of 22 patients. Using these data, we characterized the tumor risk for Wilms tumor and hepatoblastoma to suggest appropriate screening for this patient population. In addition, we discuss the phenotypic overlap between SGBS and Beckwith‐Wiedemann Spectrum. [ABSTRACT FROM AUTHOR]

Non‐immune hydrops fetalis (NIHF) is a rare entity characterized by excessive accumulation of fluid within the fetal extravascular compartments and body cavities. Here we present two intrauterine fetal demises with NIHF presenting with oligohydramnios, cystic hygroma, pleural effusion, and generalized hydrops with predominance of subcutaneous edema. The fetuses also presented with ascites, severe and precocious IUGR and skeletal anomalies. Whole exome sequencing was applied in order to screen for a possible genetic cause. The results identified biallelic variants in MYBBP1A in both fetuses. A previous report described another case with a similar phenotype having compound heterozygous variants in the same gene. The protein encoded by MYBBP1A is involved in several cellular processes including the synthesis of ribosomal DNA, the response to nucleolar stress, and tumor suppression. Our functional protein analysis through immunohistochemistry indicates that MYBBP1A is a gene expressed during fetal stages. Altogether, we concluded that MYBBP1A is associated with the development of hydrops fetalis. More cases and further studies are necessary to understand the role of this gene and the mechanism associated with NIHF. [ABSTRACT FROM AUTHOR]

Introduction. This article analyses the impact of an intercultural learning intervention in a Spanish university post-European Higher Education Area implementation. Objective. Our research’s main objective consisted of measuring the development of intercultural competence in the first cohort of a Translation and Intercultural Communication bachelor’s degree in a Spanish university, before and after taking specific courses in intercultural studies and spending a study abroad semester. Methodology. A mixed methodology was implemented, in which the Intercultural Development Inventory (IDI) was used as a measuring instrument. Additionally, in-depth personal interviews were conducted to complement the data obtained. Results. The results of this study show that the programming and sequencing of specific courses, together with experiences abroad, contribute to the development of intercultural competence. Conclusions. More intercultural learning interventions are needed in higher education courses to develop and build an intercultural campus and educate global citizens. When applying intercultural learning interventions and intercultural methodologies, students develop their intercultural competence, a key competence for 21st-century graduates.

128. Clinical experiences with 6-monthly paliperidone palmitate after 12 months of use. A retrospective study

Introduction Long-acting injectable antipsychotics (LAIA) have provided a significant improvement in the treatment of schizophrenia. Although there is already significant clinical experience with paliperidone palmitate, it is important to evaluate the clinical response of patients to this new 6-monthly presentation, so descriptive studies based on real clinical evidence can be very useful for this purpose. Objectives The main objective of the study is to describe the use of 6-monthly paliperidone palmitate in routine clinical practice, providing variables that objectify the evolution such as the number of admissions and visits to the emergency room. Methods Retrospective descriptive study with a sample selected by non-probabilistic consecutive sampling, retrospective type, in a time interval of 12 months (n=40). The patients selected were all those who received 6-monthly paliperidone palmitate treatment, with a diagnosis of schizophrenia, in 12 months of use at Hospital Universitario Infanta Elena. A descriptive analysis was performed. Mean and standard deviation were calculated for quantitative variables and N and percentage for categorical variables. Results A total of 40 administrations of 6-monthly paliperidone palmitate were performed in the study. None of the patients presented adverse reactions related to the administration of the drug, not reporting local pain or inflammation of the puncture area, except for the characteristic discomfort of an intramuscular puncture. Regarding the efficacy of 6-monthly paliperidone palmitate, none of the patients presented a psychotic decompensation after its administration, maintaining psychopathological stability after the change. The switch to 6-monthly paliperidone palmitate was made from both 1-monthly paliperidone palmitate and 3-monthly paliperidone palmitate, both showing the same efficacy. Regarding tolerability, all the patients who were administered 6-monthly paliperidone palmitate were previously treated with the monthly and quarterly presentation of the same molecule, having presented good tolerability to it, maintaining said tolerability after treatment. change to 6-monthly paliperidone palmitate, with no adverse reaction being recorded after the change. The adherence presented by the patients was very good, performing 100% of the administrations of 6-monthly paliperidone palmitate Conclusions 6-monthly paliperidone palmitate may be an effective and well-tolerated treatment for the treatment of schizophrenia. In the present study, the use of said LAIA in a group of 40 patients is objectified, showing excellent efficacy and tolerability. All study patients were already stable with the 1-monthly and 3-monthly paliperidone palmitate formulations, maintaining said psychopathological stability when switching to the 6-monthly paliperidone palmitate formulation, with excellent adherence and adverse effect profile . Introduction 6-monthly paliperidone palmitate features an initiation regimen through 1-monthly paliperidone palmitate or 3-monthly paliperidone palmitate. Some patients do not have sufficient adherence to treatment and it is necessary at the clinical level to start directly with 6-monthly paliperidone palmitate. There is little clinical experience with these alternative initiations and through this work those that have been carried out for 12 months at the Hospital Universitario Infanta Elena are exposed. Objectives The main objective of the study is to describe the alternative initiations performed with 6-monthly paliperidone palmitate in routine clinical practice, having opted for a regimen different from the standard for clinical reasons. Methods A retrospective selection of patients will be made through non-probabilistic consecutive sampling, including all patients who have been administered 6-monthly paliperidone palmitate with a start different from the standard during the last 4 months. To do this, the electronic medical record will be used, first selecting the patients who have started 6-monthly paliperidone palmitate through the anonymized digital records and, later, including in the study only those who have followed an alternative initiation pattern. The variables studied will be the following: age, sex, diagnosis, dose of paliperidone palmitate, initiation regimen, consumption of toxic substances, absenteeism from 6-monthly paliperidone palmitate, and visits to the emergency room and admissions. Results The study included a total of 20 patients (n: 20). 80% of the patients were male and 20% were female. The mean age was 39.7 years. 75% of the patients had an associated substance use disorder. The following alternate starting schedules were performed with biannual paliperidone palmitate: monthly paliperidone palmitate on days 1 and 8, and 6-monthly paliperidone palmitate on day 38 (n: 11); monthly paliperidone palmitate 150 mg together with semi-annual paliperidone palmitate both on day 1 (n: 5); biannual paliperidone palmitate on day 1 supplemented with oral paliperidone for 45 days (n:4). A total of 0 visits to the emergency department and 0 admissions were observed after the 6-monthly paliperidone palmitate regimen. Conclusions Alternative initiations with 6-monthly paliperidone palmitate may be a useful and safe clinical alternative in patients with very low adherence who, due to clinical needs, require starting 6-monthly paliperidone palmitate earlier in order to guarantee adherence. A partir del año 2018, en el agua subterránea de los centros poblados de la costa norte del Perú se determinaron concentraciones de arsénico por encima de los límites máximos permitidos por la normativa peruana vigente. Considerando que esta agua es utilizada para el consumo humano y del ganado vacuno, se planteó como objetivo de investigación determinar si la leche fresca producida presenta concentraciones de arsénico. Se analizaron 21 muestras procedentes de 8 centros poblados diferentes, determinando los parámetros fisicoquímicos por ultrasonido y la presencia de arsénico por espectrofotometría de emisión óptica (Método EPA 200.7). En 15 muestras se encontraron parámetros fisicoquímicos acorde con la Norma Técnica Peruana NTP 202.001 y en 6 muestras se encontraron concentraciones de arsénico en un rango de 0,003 a 0,023 ppm, muestras procedentes de los centros poblados Cruz del Médano, Arbolsol, Lagunas, La Quemazón, Casa Blanca y Lagartera del distrito de Mórrope, departamento de Lambayeque.

143. Quality Indicators of Transanal Total Mesorectal Excision (TaTME) for Rectal Cancer

Introduction Transanal total mesorectal excision (TaTME) has revolutionized the surgical techniques for lower-third rectal cancer. The aim of the present study was to analyze the outcomes of quality indicators of TaTME for rectal cancer compared with laparoscopic TME (LaTME). Methods A cohort prospective study with 50 (14 female and 36 male) patients, with a mean age of 67 (range: 55.75 to 75.25) years, who underwent surgery for rectal cancer. In total, 20 patients underwent TaTME, and 30, LaTME. Every TaTME procedure was performed by experienced colorectal surgeons. The sample was divided into two groups (TaTME and LaTME), and the quality indicators of the surgery for rectal cancer were analyzed. Results There were no statistically significant differences regarding the patients and the main characteristics of the tumor (age, gender, American Society of Anesthesiologists [ASA] score, body mass index [BMI], tumoral stage, neoadjuvant therapy, and distance from the tumor to the external anal margin) between the two groups. The rates of: postoperative morbidity (TaTME: 35%; LaTME: 30%; p = 0.763); mortality (0%); anastomotic leak (TaTME: 10%; LaTME: 13%; p = 0.722); wound infection (TaTME: 0%; LaTME: 3.3%; p = 0.409); reoperation (TaTME: 5%; LaTME: 6.6%; p = 0.808); and readmission (TaTME: 5%; LaTME: 0%; p = 0.400), as well as the length of the hospital stay (TaTME: 13.5 days; LaTME: 11 days; p = 0.538), were similar in both groups. There were no statistically significant differences in the rates of positive circumferential resection margin (TaTME: 5%; LaTME: 3.3%; p = 0.989) and positive distal resection margin (TaTME: 0%; LaTME: 3.3%; p = 0.400), the completeness of the TME (TaTME: 100%; LaTME: 100%), and the number of lymph nodes harvested (TaTME: 15; LaTME: 15.5; p = 0.882) between two groups. Conclusion Transanal total mesorectal excision is a safe and feasible surgical procedure for middle/lower-third rectal cancer.

En la ciudad de Iquitos, en la Amazonía peruana, los materiales utilizados para las construcciones sólidas o permanentes (arena, cemento y ladrillos) son llamados nobles, lo que implica que los otros materiales no lo son, por ejemplo, la madera. Este material es considerado primordial en las construcciones tradicionales de las culturas locales y también por los ciudadanos menos favorecidos instalados en zonas urbanas. La madera también presenta muchas ventajas con respecto al impacto ambiental (huella de carbono), los costos de construcción en la Amazonía y la pasividad térmica (no acumula el calor). Deseamos así presentar una obra arquitectónica realizada en la ciudad de Iquitos en los años 2012-2013, concebida con madera de construcción e inspirada en los principios de ventilación de las construcciones tradicionales de culturas locales. Y que además ha sabido responder al paso del tiempo manteniendo su eficiencia en materia de comportamiento energético.

145. M{\o}lmer-S{\o}rensen entangling gate for cavity QED systems

The M{\o}lmer-S{\o}rensen gate is a state-of-the-art entangling gate in the ion trap quantum computing where the gate fidelity can exceed 99\%. Here we propose an analogous implementation in the setting of cavity QED. The cavity photon mode acts as the bosonic degree of freedom in the gate in contrast of that played by a phonon mode in ion traps. This is made possible by utilising cavity assisted Raman transitions interconnecting the logical qubit states embedded in a four-level energy structure, making the "anti-Jaynes-Cummings" (AJC) term available under the rotating-wave approximation. We identify practical sources of infidelity and discuss their effects on the gate performance. Our proposal not only demonstrates an alternative entangling gate scheme but also sheds new light on the relationship between ion traps and cavity QED, in the sense that many techniques developed in the former are transferable to the latter through our framework., Comment: 12 pages
