Abstract

In patients with pancreatic ductal adenocarcinoma (PDAC), optimal treatment selection, including multimodality regimens such as neoadjuvant chemoradiotherapy (NACRT), can be clinically transformative. Unfortunately, currently no predictive biomarkers are available that can guide the use of NACRT in PDAC patients. Accordingly, herein we developed a novel gene signature that can preoperatively predict NACRT‐sensitivity in PDAC patients. Herein, we evaluated the performance of a 10‐gene panel in 749 PDAC cases, which included two public datasets (The Cancer Genome Atlas and International Cancer Genome Consortium; n = 276), and three clinical specimen cohorts (n = 417), and a pre‐NACRT endoscopic ultrasound‐guided fine‐needle aspiration (EUS‐FNA) biopsy cohort (n = 56). The potential predictive performance of this signature was evaluated and compared to CA‐19‐9 levels and key clinicopathological factors. We first evaluated the prognostic potential of a 10‐gene panel which significantly predicted overall survival in both public datasets (P <.01, P <.01), and two in‐house patient cohorts (P <.01, P =.04). In the pre‐NACRT EUS‐FNA cohort, we established a radio‐sensitivity gene panel (RSGP) which yielded highly robust (area under the curve [AUC] = 0.91; 95% CI: 0.81‐0.97) for predicting response to gemcitabine‐based NACRT. Multivariate logistic regression analysis revealed that RSGP was an independent predictor for response to NACRT (OR = 2.70; 95% CI: 1.25‐5.85), and this response‐prediction was even more robust when CA‐19‐9 levels were included into the model. In conclusion, we have validated and developed a novel gene signature that is highly robust in predicting response to NACRT, even in preoperative settings, highlighting its clinical significance for optimizing and personalizing treatment strategies in PDAC patients. While the benefits of radiation treatment have been well established in solid cancers, the criteria for optimal indication of radiotherapy in pancreatic ductal adenocarcinoma and the best regimens for its clinical application remain unclear. The availability of biomarkers that could help predict response to radio‐sensitivity before treatment could lead to better‐informed decision‐making. 114. Persistent acute kidney injury: Postoperative impact and predictors in patients undergoing liver resection.

Abstract

Aim Methods Results Conclusions Persistent acute kidney injury (AKI) has not been investigated in patients undergoing liver resection. We aimed to identify the predictors of persistent AKI, its effect on postoperative outcomes and long‐term renal function in patients following liver resection, and its impact on survival in patients with hepatocellular carcinoma (HCC).We examined 990 patients who underwent liver resection, including a subgroup analysis of 384 patients with curative resection for initial HCC. Persistent AKI was defined as residual impairment of serum creatinine ≥ 0.3 mg/dL or ≥50% from baseline 1 month after surgery.The persistent AKI group had significantly worse postoperative outcomes, including overall morbidity, major morbidity, longer hospital stay, and 90‐day mortality. In the subgroup analysis of patients with HCC, persistent AKI was associated with a significantly poorer overall survival (OS) rate (p < 0.001), and the multivariate analysis confirmed persistent AKI as an independent poor prognostic factor for OS (p = 0.005). The long‐term postoperative estimated glomerular filtration rate decline was significantly greater in the persistent AKI group than in the no AKI and transient AKI groups (p < 0.001 for both). Chronic kidney disease, albumin–bilirubin grade ≥2, and anatomical resection were independent predictors of persistent AKI (p = 0.001, p = 0.039, and p = 0.015, respectively).Persistent AKI adversely affects postoperative outcomes and long‐term renal function in patients undergoing liver resection. Furthermore, it is associated with poor prognosis in patients with HCC. Therapeutic strategies to prevent persistent AKI are critical for improving postoperative outcomes in these patients. Abstract

BACKGROUND: Although recent large-scale clinical studies have shown that preoperative renal insufficiency is associated with increased risk of postoperative complications after pancreatoduodenectomy (PD), it is unknown whether asymptomatic renal dysfunction has an impact on postoperative course after PD. METHODS: Two hundred and fifty-four patients who underwent PD between 2007 and 2013 were enrolled. Renal function was evaluated by the preoperative estimated glomerular filtration rate (eGFR). Patients were divided into two groups according to the cutoff value of 55 of eGFR. RESULTS: Thirty-five patients were classified as the low eGFR group, while 219 were classified as the normal group. There were differences between groups in age, comorbidity and pancreatic texture. The incidence of overall postoperative complication, grade B/C pancreatic fistula and severe complication in the low eGFR group was significantly higher than that in the normal group. Multivariate analysis identified low eGFR as an independent risk factor for severe postoperative complications and grade B/C pancreatic fistula after PD. However, there were no differences in mortality and survival between the low and normal eGFR groups. Abstract

OBJECTIVE: Smoking may affect pharmacokinetics of chemotherapeutic agents and hemodynamics of the smokers, thereby influencing adverse events and efficacy of chemotherapy in patients with pancreatic cancer (PC). The aim of this study was to clarify how smoking totally affected patients with PC receiving current chemotherapy. METHODS: We evaluated the impact of smoking status on the performance of chemotherapy and survival in 262 patients with PC including 158 resectable and 104 unresectable PC. RESULTS: There were more male and younger patients in current smokers than in nonsmokers. In unresectable PC, current smokers had more metastatic tumors than locally advanced tumors compared with nonsmokers. In current smokers receiving chemotherapy, the baseline white blood cell count, neutrophil count, and hemoglobin concentration were significantly higher in current smokers than in nonsmokers. Furthermore, grades 3 to 4 neutropenia was observed more often in nonsmokers than smokers. On the other hand, the performance and efficacy of the planned adjuvant chemotherapy were similar between smokers and nonsmokers. More importantly, there was no significant difference in overall prognosis between smokers and nonsmokers receiving chemotherapy. Abstract

BACKGROUND: Nectin-4 belongs to the nectin family that has diverse physiological and pathological functions in humans. Recent studies have also suggested some roles for Nectin-4 in several human cancers. However, the precise roles and clinical relevance of Nectin-4 in tumors are largely unknown. METHODS: Nectin-4 expression was investigated in 123 patients with pancreatic cancer by immunohistochemistry. Furthermore, we investigated the association of Nectin-4 in pancreatic cancer with tumor proliferation, angiogenesis and immunity by using immunohistochemistry and siRNA interference method. RESULTS: Patients with high Nectin-4 expression had poorer postoperative prognosis than those with low expression. Importantly, multivariate analysis indicated that Nectin-4 expression had a significant independent prognostic value in pancreatic cancer (HR = 1.721, 1.085-2.730; P = 0.021). Tumor Nectin-4 expression was significantly correlated with Ki67 expression. In addition, siRNA-mediated gene silencing of Nectin-4 significantly inhibited the cell proliferation in human pancreatic cancer cells, Capan-2 and BxPC-3. Furthermore, Nectin-4 expression was also positively correlated with VEGF expression and intratumoral microvessel density. However, there were no significant correlations of tumor Nectin-4 expression with tumor-infiltrating T cells. CONCLUSION: Nectin-4 is a significant prognostic predictor, and may play a critical role in pancreatic cancer. 127. The association of physical activity level in lifestyle with obesity : Survey concerning lifestyle characteristics among Nagoya University alumni

Abstract

We conducted a survey concerning lifestyle characteristics among Nagoya University alumni who were diagnosed as obesity and those who were not diagnosed as obesity at annual medical check-ups between 1974 and 1979. We divided them four groups. Group A: obesity at the check-ups and at present Group B: obesity at the check-ups and non-obesity at present Group C: non-obesity at the check-ups and obesity at present. Group D: non-obesity at the check-ups and at present. The results were as follows: 1) About 50% of subjects did not have the special physical exercise practice in a week. 2) In group B, about 50% of subjects selected the index II (medium) in the list of daily physical activity level showed by the recommended dietary allowances for the Japanese. 3) Subjects in group B, maintained a level of physical activities after graduation as same as that in their university days. These results suggest that maintaining moderate physical activity level might be effective for preventing and improving obesity.

128. Changes in protein metabolism during rapid weight reduction in boxers

Abstract

To investigate protein metabolism during rapid weight reduction, present study was carried out. Five male boxing players ranging from 20 to 30 years of ages put on a restrictied diet of their own will for two weeks. Body weight changes were observed and biochemical analyses of urine and blood were performed. The results obtained were as follows. 1 Changes in weights: The initial mean body weights 66.2 ± 3.2kg was decreased to 65.1 ± 3.3kg after one week. After two weeks the final mean body weight was 63.2 ± 3.2kg. They achieved weight loss of 3.0kg during the study. 2 Changes in caloric intake; Caloric intake before the study was 2785kcal. Caloric intake loss 1155kcal (41.5% reduced) after two weeks was significantly greater than that of 624kcal (22.4% rednced) after one week. 129. The study to develop the effective diet program for obese persons (2nd report) : the report of three cases who lost weight through repeated educations about their diet habit

Abstract

In 1994, we educated repeatedly obese students how to reduce their weight effectively through diet and exercise therapies. In this paper, we reported three cases (case A: male 22 y.o., case B: male 18 y.o., case C: female 20 y.o.) who succeeded in losing weight and discussed about the effective diet therapy for obese persons. 1. The reduced weights were 6.4 kg in case A, 20.1 kg in case B, and 12.0 kg in case C. 2. After the educational program, the mean total energy intake was about 1450 kcal, which were below their ideal energy intake. The protein energy ratios were increased in case B (from 11.9 to 17.3 %) and case C (from 11.3 to 15.3%). 3. All cases assessed themselves that they had carried out improved dietary habits and eating behaviors. These results suggested that it might be effective for treatment of obesity to educate obese persons repeatedly about their dietary habits.

Abstract

We surveyed the dietary habits in obese students in Nagoya University in order to develop the effective diet program for obese persons. The results are as follows. 1. The severe obese students (BMI ≧ 30.8) tend to have no breakfast every morning. 2. There is a tendency in the severe obese students to eat between meals compared with the other obese students (30.8 > BMI). 3. The most of the severe obese students eat quickly by themselves until they feel full. 4. The severe obese students frequently eat meats, non-green-and-yellow vegetables and milks (dairy products). They infrequently eat small fishes, fruits and potatoes. These results suggest that we should educate obese persons not to eat quickly until they feel full and to eat balanced and moderate calorie food.

Abstract

The questionnaires for the assessment of health condition and life style of the students in Nagoya University have been performed from 1982. The results in 1989 are as follows. The subjects who participated in the study were the first year students (male; 1576, female; 393; unknown; 2, total; 1971). Diet; There were 22.3% of the students who did not have breakfast and the percentage of the male students reached 26.0%. At supper, a quarter of the male students dined out. Sleeping; Many students had a tendency to be lack of sleep. The percentage of the students who slept for less than seven hours was 70.9%. Smoking; There were 18.6% of the male and 3.3% of the female students who answered that they smoked habitually. Alcohol; About 80% of the students drank alcohol. Some male students drank everyday. We would like to make use of these results for the health education to the students in Nagoya University.

The aim of this study was to identify the risk factors associated with poor prognosis after UFS for CRLM.Methods: Data about consecutive patients with CRLM who underwent liver resection at Nara Medical University Hospital between January 2000 and December 2015 were retrieved from a prospective database. Recurrence that developed within 2 years after liver resection and could not be surgically resected was defined as unresectable recurrence (UR). Preoperative risk factors associated with UR after UFS were analyzed. Among the patients with the identified risk factors, the patients who were treated with UFS were compared with those who received preoperative chemotherapy via propensity score-matching analysis.Results: There were 167 patients treated with UFS, and 71 of them developed UR (the UR group). The overall survival (OS) rate of the UR group was significantly worse than that of the non-UR group (5-year survival rate: 3.8 vs. 66.8%, p < 0.001). Multivariate analysis identified a primary colorectal cancer N factor of N2–3 as a risk factor for UR (hazard ratio 2.72, p = 0.004). Propensity score-matching analysis demonstrated that among patients with N2–3 primary colorectal cancer the post-initial treatment OS of the patients treated with UFS was significantly worse than that of the patients who received preoperative chemotherapy (5-year survival rate: 11.1 vs. 30.0%, p = 0.046).Conclusions: Patients with CRLM with a primary colorectal cancer N factor of N2–3 should be considered for preoperative chemotherapy. Abstract

BACKGROUND: Nectin-4 belongs to the nectin family that has diverse physiological and pathological functions in humans. Recent studies have also suggested some roles for Nectin-4 in several human cancers. However, the precise roles and clinical relevance of Nectin-4 in tumors are largely unknown. METHODS: Nectin-4 expression was investigated in 123 patients with pancreatic cancer by immunohistochemistry. Furthermore, we investigated the association of Nectin-4 in pancreatic cancer with tumor proliferation, angiogenesis and immunity by using immunohistochemistry and siRNA interference method. RESULTS: Patients with high Nectin-4 expression had poorer postoperative prognosis than those with low expression. Importantly, multivariate analysis indicated that Nectin-4 expression had a significant independent prognostic value in pancreatic cancer (HR = 1.721, 1.085-2.730; P = 0.021). Tumor Nectin-4 expression was significantly correlated with Ki67 expression. In addition, siRNA-mediated gene silencing of Nectin-4 significantly inhibited the cell proliferation in human pancreatic cancer cells, Capan-2 and BxPC-3. Furthermore, Nectin-4 expression was also positively correlated with VEGF expression and intratumoral microvessel density. However, there were no significant correlations of tumor Nectin-4 expression with tumor-infiltrating T cells. CONCLUSION: Nectin-4 is a significant prognostic predictor, and may play a critical role in pancreatic cancer. Abstract

Purpose: Hepatocellular carcinoma (HCC) patients beyond the Milan criteria (MC) who undergo liver resection have high recurrence rates and poor prognosis, and sometimes experience very early recurrence (VER) within six months after surgery. This study aimed to identify predictive factors, including the newly proposed C-reactive protein (CRP)-albumin-lymphocyte (CALLY) index, for VER after surgery for HCC beyond MC. Methods: We included patients who underwent initial liver resection for HCC beyond MC between 2000 and 2021. We defined VER as recurrence within six months after surgery and compared the clinicopathological factors and long-term prognosis between the VER and non-VER groups. Multivariate analysis identified risk factors for VER and evaluated the potential for prognostic stratification using these factors. Results: The overall survival (OS) and post-recurrence survival were significantly worse in the VER group compared to patients with recurrence in 7–12 months, over 12 months, and without recurrence (median survival time (MST) 1.16 vs. 5.14, 7.26, and undefined; and MST 0.81 vs. 4.34, and 5.48, respectively, P < 0.01). Alpha-fetoprotein (AFP) ≥ 200, non-simple nodule (SN) type on preoperative imaging, and CALLY index < 2.8 were independent prognostic factors (P < 0.01 for all). An increased risk factor count was correlated with poorer VER and OS rates, allowing for effective stratification. Conclusion: VER after hepatic resection for HCC beyond MC was associated with a significantly poorer prognosis. AFP, non-SN type on imaging, and CALLY index are valuable preoperative indicators.
