Objectives Gonadotropin-releasing hormone agonist (GnRHa) could influence the levels of sex hormones and thyroid hormones. The aim of this study was to investigate the effect of GnRHa on thyroid function. Materials and methods The data of the patients were collected from the registrations of July 2014–October 2014. A total of 41 women who underwent one-time IVF cyclus were evaluated in this cross-sectional study. The patients were categorized into two groups according to the serum T3, T4, and TSH levels before and 2 weeks’ after the administration of GnRHa. Results Mean basal TSH and mean TSH levels on hCG day were 1.98 ± 0.77 and 1.75 ± 0.70, respectively. The difference between the two groups was statistically significant (p 3 and T4 levels. Conclusions In conclusion, our results demonstrate that GnRHa led to a decrease on serum TSH level. Serum-free T3 and T4 levels were remained unchanged and this might be due to early measurement of the hormone levels (just 2 weeks later from GnRHa administration).

Amaç Preterm bebekleryenidoğan yoğun bakım ünitelerinde (YYBÜ) uzun süre takip edilirler ve bunedenle uzun süreli damar yolu ihtiyaçları olur. Uzun süreli damar yolusağlamak amacıyla umbilikal ven kateterleri, santral venöz kateterler veperiferik olarak yerleştirilmiş santral kateterler (PYSK) kullanılır. Buçalışmada PYSK uygulanan yeni doğan bebeklerin klinik özelliklerinideğerlendirmeyi amaçladık.Gereç ve YöntemlerHaziran 2016 - Haziran 2019 tarihleri arasında YYBÜ’de PYSK yerleştirilenyenidoğan bebeklerin tıbbi kayıtları retrospektif olarak incelendi. Hastalarındemografik özellikleri, PYSK yerleştirilme nedenleri, bebeklerin PYSKyerleştirme sırasındaki klinik özellikleri ve uygulama komplikasyonlarıkaydedildi.Bulgular 129 yenidoğan bebekteyerleştirilen 151 PYSK’ya ait veriler toplandı. PYSK takılan 134 (% 88,7)hastada enfeksiyon nedeniyle antibiyotik tedavisi gerekti. Sepsis bu vakaların72'sinde (% 47.6) PYSK yerleştirilmeden önce, 17'sinde (% 11.2) ise PYSK yerleştirildikten sonra tespit edildi. PYSKyerleştirilen hastalarda vücudun üst bölge damarları, alt bölge damarları vesağ taraf damarları ve sol taraf damarları arasında tıkanma, sızıntı ve / veyatromboflebit sıklığı açısından istatistiksel bir fark bulunmadı (p> 0.05).Sonuç Sonuçlarımız PYSKuygulamasının yenidoğan bebeklerde periferik damar yolu girişim sayısını önemliölçüde azalttığını, uzun süreli TPN ve antibiyotik uygulamasının mümkünolduğunu, ayrıca üst veya alt ekstremite venlerinden ve sağ veya sol taraftakivenlerden PYSK uygulanmasının komplikasyon sıklığını etkilemediğinigöstermektedir., Objective Preterm babies are followed in neonatal intensivecare units (NICU) for long times, sothey need long-term vascular access. For long-term vascular access umbilicalvein catheters, central venous catheters and peripherally inserted centralcatheters (PICC) are used. In present study we aimed to evaluate the clinicalfeatures of newborn infants in whom PICC was inserted.Materialsand Methods The medicalrecords of the newborn infants in whom PICC had been inserted from June 2016 toJune 2019 are evaluated retrospectively. Demographic features of the patients,reasons for PICC insertion, clinical features of the infants during PICCinsertion, and complications of the application are recorded.Results The data of 151 PICC that were inserted in 129newborn infants were collected. Antibiotic treatment was needed because ofinfections in 134 (88.7%) PICCs. In 72 (47.6%) of them sepsis was presentbefore PICC insertion, and in 17 (11.2%) after PICC insertion. No statistical difference was found in termsof the frequency of occlusion, leak and/or thrombophlebitis between patients inwhom PICCs were inserted via the upper body veins and lower body veins, andright veins and left veins (p>0.05).Conclusion Our results indicate thatthe PICC application significantly decreases the number of peripheral vascularaccess attempts in newborn infants, long-term total parenteral nutrition (TPN) and antibiotic administration becomes possible and insertion fromupper or lower extremity veins and right or left side veins does not affect thefrequency of complications.

The most common cause of subarachnoid hemorrhage at the period of pregnancy and during puerperium is rupture of an intracranial aneurysm. It is five times more common in pregnant women than in non-pregnant women. This pathology is more common in primiparous women and in the third trimester of pregnancy. A 37-year-old woman who was admitted to the emergency department with sudden-onset headache and loss of consciousness was diagnosed with intracranial hemorrhage due to middle cerebral artery aneurysm rupture. The patient, who gave birth with emergency cesarean delivery, underwent surgery for subarachnoid hemorrhage. The case is presented here because of its rarity.

Bu kapsamda, Türkiye’de cumhuriyetin ilanından günümüze kadar, Kur’ân meâli çalışmaları konusunda oldukça bereketli bir birikim söz konusudur. Bu meâllerden birisi de Prof. Dr. Hüseyin Peker tarafından kaleme alınan “Kur’an Meâli” adlı eserdir. Bu makalede araştırmanın konusu, Hüseyin Peker tarafından kaleme alınan “Kur’an Meâli” adlı eserin değerlendirilmesidir. Araştırmanın amacı, Peker’in “Kur’an Meâli” adlı eserinde, bazı ayetler veya ayetler içerisindeki bazı kelime ve kavramlar hakkında, diğer mütercimlerin kanaatlerinden farklılık arz eden görüşlerini incelemektir. Araştırmanın kapsamı, ele alınan konular çerçevesinde Peker’in “Kur’an Meâli” adlı eseri ve Türkiye’de yapılan diğer Kur’ân tercümeleriyle sınırlı tutulmuştur. Araştırmanın yöntemi, literatür çalışmasına dayanmakta olup, Peker’in ilgili konulardaki kanaatleri ile Cumhuriyet döneminde kaleme alınan Kur’ân’ın Türkçe çevirilerindeki görüşler kıyaslanarak değerlendirilmektedir. Bu çerçevede, araştırmada toplam altı kelime, kavram ve ifade incelenmektedir. Bunlar; Besmele’nin anlamı, muttakî kavramı, birr kelimesi, nefs kelimesi, istevâ fiili ve Duhân suresi 24. ayetteki vetruki’l-bahra rahven ifadesinin anlamı konularından oluşmaktadır.

Patent ductus arteriosus is an important problem in preterms. We aimed to investigate the relation of patent ductus arteriosus with shunt index.The preterm infants with a birth weight of ≤1500 g and/or gestational age of30 weeks and an indication for umbilical artery and venous catheterization formed the study group. Between the postnatal 24-48 hours, the first arterial and venous blood samples were obtained and the patients were evaluated by echocardiography. In patients with hemodynamically significant patent ductus arteriosus (patient group), during the first 24 hours after the competition of the first course of medical treatment, the second blood samples were obtained and echocardiography repeated. In patients without patent ductus arteriosus (control group), second blood samples were taken after the postnatal 72 hours. Also, echocardiography was performed.A total of 60 infants, (female = 29, male = 31), were included in the study. We did not find a statistically significant relation between shunt index and the presence of patent ductus arteriosus (p .05). A statistically significant positive correlation between the fraction of inspired oxygen and shunt index was found. As the postnatal ages progressed, the shunt index values tended to decrease significantly.Our results suggest that shunt index cannot be used as an indicator of hemodynamically significant patent ductus arteriosus in preterm infants. The postnatal age and fraction of inspired oxygen have a significant effect on shunt index in these patients. It was thought that the other possible factors that affect the shunt index should be investigated in preterms between certain postnatal ages.

1950’li yıllardan itibaren başlayan kırdan kente göç sürecinde kentlerin nüfusu hızla artmış, yerel yönetimler ise kentli nüfusa hizmet sunmada yetersiz kalmışlardır. Bu noktada, 1982 Anayasası’nın 127. maddesine istinaden 1984 tarih ve 3030 sayılı Kanunla İstanbul, Ankara ve İzmir büyükşehir belediyelerinin kurulmasıyla büyükşehir yönetim sistemine geçilmiştir. Büyükşehir sayısı ihtiyaca binaen artırılarak 2000’de 16’ya 2013’te ise 30’a ulaşmıştır. Aynı zamanda, büyükşehir sisteminin 2004’te 5216 ve 2012’de de 6360 sayılı kanunlarla yeniden düzenlenmesi gerekmiştir. Tüm bu düzenlemelerin gayesinin büyükşehirlerde yaşayan nüfusa daha kaliteli hizmet götürmek olduğu söylenebilir. Bu bağlamda, araştırmada 6360 sayılı Kanunla büyükşehirlerde belediye sınırının il mülki sınırına genişletilmesinin ya da hizmet ölçeğindeki artışın yerel halkın hizmet kalitesi algısı üzerindeki etkisi incelenmiştir. Araştırmanın amacı, büyükşehirlerde hizmet ölçeğinin genişletilmesinin yerel halkın hizmet kalitesi algısını nasıl etkilediğinin ortaya konmasıdır. Türkiye’de 6360 sayılı Kanun kapsamında büyükşehre dönüştürülen on dört il büyükşehir belediyesi araştırma evrenini oluştururken, zaman ve maliyet kısıtları nedeniyle araştırma Balıkesir Büyükşehir Belediyesi’yle sınırlandırılmıştır. Araştırma tarama araştırması şeklinde tasarlanmış, araştırmacılar tarafından geliştirilen anket yoluyla veriler toplanmıştır. Verilerin analizi SPSS 20.0 istatistik paket programıyla yapılmıştır. Sonuçta, hizmet ölçeğinin genişletilmesinin Balıkesir Büyükşehir Belediyesi örneğinde yerel halkın hizmet kalitesi algısını olumlu yönde etkilediği görülmüştür.

The aim of the study was to investigate knee cartilage thickness in polycystic ovary syndrome (PCOS) patients using ultrasonography and to assess the effects of sex steroids on osteoarthritis development. A total of 90 female patients were enrolled in the study, including 47 patients diagnosed with PCOS as study group and 43 patients admitted to our hospital for any other complaints as control group. Ultrasonographic evaluation of both knees was performed in all patients. The knee cartilage thickness was measured at lateral condyle, intercondylar area and medial condyle. The measurements were averaged for each region. The bibliometric studies in the field of orthopaedics have gained popularity since they can determine the characteristics and research trends of most influential papers in specific subjects. This study aimed to evaluate the 50 most-cited articles related to hallux rigidus, and analyze possible factors associated with increased citation counts.The available literature about hallux rigidus on Web of Science database until January 2021 were analyzed and the 50 most-cited articles were determined. The characteristics of the papers were documented and analyzed for any relationship or correlation with total citation or citation per year (citation density) values.The average citation number and citation density were 52.8 (range: 26-243) and 3 (range: 0.8-12.8), respectively. The average author number was 3.4, while the average institution number was 1.9. The USA had the highest number of articles with 29. Twenty-six papers were case-series, only 3 studies had level 1 and only 2 had level 2 evidence. The highest number of articles were published in Foot and Ankle International with 22 papers. Forty papers were clinical studies while 10 were reviews. Five studies were multicentric.This study provided the general characteristic and research trends of 50 most influential hallux rigidus papers. The number of institutions and references were positively correlated with citation density, indicating a higher chance of getting cited for papers with collaborations from different institutions and papers using more references from other works. Publication year was also positively correlated with citation density, indicating the better performance of more recent articles.Level 3.

Objective To evaluate clinical pregnancy rates, miscarriage rates, ongoing pregnancy rates, and in vitro fertilization-intra cytoplasmic sperm injection outcomes of gonadotropin releasing hormone agonist (GnRHa) administration compared with human chorionic gonadotropin (hCG) application for luteal phase support. Materials and Methods A total of 456 patients were included in the study. The patients were divided into two groups according to luteal phase support type: in group 1 (n=158), single-dose triptorelin acetate 0.1 mg was given on the sixth day after the oocyte pick-up (OPU). In group 2 (n=298), hCG 1500 IU was given on day 4, 7 and 10 after the OPU. Results Both groups were homogeneous in relation with age and antral follicle count. Amaç: Endometriyal örnekleme, jinekoloji pratiğinin en sık başvurulan invazif işlemlerinin başında gelmektedir. Anormal uterin kanamanın jinekoloji polikliniklerinde çok sık karşılaşılan bir semptom olması endometriyal örneklemenin sıklığını açıklamaktadır. Bu çalışmanın amacı endometriyal örnekleme yaptığımız hastaların histopatolojik sonuçlarının literatür eşliğinde değerlendirilmesidir. Gereç ve Yöntem: Ocak 2014/Aralık 2018 tarihleri arasında Kadın Hastalıkları ve Doğum polikliniğinde yapılan endometriyal örneklemeleri kapsayan, retrospektif nitelikte bir çalışmadır. Tıbbi nedenli tahliyeler, isteğe bağlı yapılan gebelik terminasyonları ve mol gebelikler çalışma dışında tutulmuştur. Patoloji sonuçları ise atrofik endometrium, endometrit, proliferatif endometrium, sekretuar endometrium, desidualizasyon, düzensiz proliferasyon gösteren endometrium, endometrial polip, endometrial hiperplazi, endometrium adenokarsinomu olarak gruplanmıştır. Objective Telomere length is used as an indicator of biological aging. It is well known that one of the most remarkable risk factors of recurrent pregnancy losses is advanced maternal age. The objective of this study was to investigate the correlation between idiopathic recurrent pregnancy loss and telomere length. Method The study group included 40 women, while the control group consisted of 41 healthy women whose age and body mass index were matched. A venous blood sample was taken from all participants into EDTA tubes in the early follicular phase, and telomere length was measured through the qPCR technique. Results When the mean TL of the groups was compared, it was determined that TL was significantly shorter among the iRPL group (7763.89±924.58 base pair) compared to the control group (8398.84±1102.95 base pair) (p Conclusion Shortened telomere length might play a role in the etiology of iRPL. We are of the opinion that patients with RPL should be screened for the presence of cardiovascular diseases and other chronic diseases, as is the case for POF.

Acute kidney injury (AKI) is a common complication after major surgery and is associated with adverse short-term and long-term outcomes. However, there is a large variation in the incidence of reported AKI rates. The establishment of an accurate epidemiology of surgery-associated AKI is important for healthcare policy, quality initiatives, clinical trials, as well as for improving guidelines. The objective of the Epidemiology of Surgery-associated Acute Kidney Injury (EPIS-AKI) trial is to prospectively evaluate the epidemiology of AKI after major surgery using the latest Kidney Disease: Improving Global Outcomes (KDIGO) consensus definition of AKI. Methods and analysis EPIS-AKI is an international prospective, observational, multicentre cohort study including 10 000 patients undergoing major surgery who are subsequently admitted to the ICU or a similar high dependency unit. The primary endpoint is the incidence of AKI within 72 hours after surgery according to the KDIGO criteria. Secondary endpoints include use of renal replacement therapy (RRT), mortality during ICU and hospital stay, length of ICU and hospital stay and major adverse kidney events (combined endpoint consisting of persistent renal dysfunction, RRT and mortality) at day 90. Further, we will evaluate preoperative and intraoperative risk factors affecting the incidence of postoperative AKI. In an add-on analysis, we will assess urinary biomarkers for early detection of AKI. Ethics and dissemination EPIS-AKI has been approved by the leading Ethics Committee of the Medical Council North Rhine-Westphalia, of the Westphalian Wilhelms-University Münster and the corresponding Ethics Committee at each participating site. In this study, we evaluated to the relationship between the type of accessory navicular bone (ANB) and radiological parameters of foot in patients with bilateral ANB of different types.Patients with bilateral ANB of different types participated in this study, from May 2019 to April 2020. Patient data, including age, sex, body mass index (BMI), and presence of symptoms were obtained. We aimed to compare the radiological parameters of both the feet for evaluate the differences from one another in patients with bilateral ANB of different types (one side type 1 and contralateral side type 2) because the foot angles may differ in each person. Seven radiographic parameters were measured, including calcaneal pitch angle, talocalcaneal angle, tibiocalcaneal angle, naviculocuboid overlap, talonavicular coverage angle, anteroposterior talo-first metatarsal angle, and the lateral talo-first metatarsal angle, which evaluated hindfoot, midfoot, and forefoot alignment.Twenty patients (13 women and 7 men) with a mean age (and standard deviation) of 38.5 {plus minus} 12.3 years were included in the study. The patients had a mean height of 168.1 {plus minus} 7.1 cm, a mean weight of 77.2 {plus minus} 10.5 kg, and a mean BMI of 27.4 {plus minus} 4.3 kg/m2. There was no significant difference between type 1 and type 2 in all radiological parameters. There was no significant correlation between radiological parameters and age, BMI, or the presence of symptoms.We found that the type of ANB had no effect on the radiological measurements of the foot in which we evaluate the parameters patients with bilateral ANB of different types. Additionally, age, BMI, and the presence of symptoms, also demonstrated no correlation with the radiological parameters of the foot.

Objective To evaluate the relationship between the maternal pulmonary function test (PFT) and the Apgar score of the newborn, umbilical cord blood (UCB) gas values and the duration of delivery. Material and Methods The present study included 41 volunteer nulliparous pregnant women who presented to our obstetrics clinic and PFTs were performed by using spirometry. Results A significant positive correlation was observed between 5th-minute Apgar scores and maternal FEV1 (forced expiratory volume in the first second) (lt), FEV1%, FVC (lt), FVC% (r=0.509, p=0.003; r=0.47, p=0.007; r=0.434, p=0.013; r=0.417, p=0.017; respectively). A significant positive correlation was observed between UCB pH value and maternal FEV1 (lt), FVC (lt) (r=0.515, p=0.003; r=0.351, p=0.049; respectively). Conclusion Our results showed that FEV1 and FVC values, which indicate maternal respiratory functions, may affect Apgar scores of the newborn and UCB gas values.

Background: This study aimed to compare the radiologic parameters of both feet in patients with unilateral accessory navicular bone (ANB) and evaluate the differences from one another. Methods: Forty-one patients with unilateral ANB volunteered to participate in this study from August 2019 to January 2020. Patient data, including age, sex, body mass index (BMI), type of ANB, and presence of symptoms were obtained. Group 1 comprised 23 patients with asymptomatic unilateral ANB, and group 2 comprised 18 patients with symptomatic unilateral ANB. Seven radiologic parameters were evaluated assessing hindfoot, midfoot, and forefoot alignment—calcaneal pitch angle, talocalcaneal angle, tibiocalcaneal angle, naviculocuboid overlap (NCO), talonavicular coverage angle (TNCA), anteroposterior talo–first metatarsal angle, and lateral talo–first metatarsal angle. Results: The mean age of patients was 40.1 years in group 1 and 42.6 years in group 2. Mean BMI was 25.2 in group 1 and 26.6 in group 2. No significant differences were noted in the radiologic parameters between the ANB and contralateral sides in all patients. The radiologic parameters of both feet in symptomatic and asymptomatic patients were not significantly different. No significant differences were noted between the affected sides of type 1 and 2 ANB and contralateral sides in terms of the radiologic parameters. BMI was significantly correlated with NCO and TNCA. Conclusion: This study demonstrated that the presence of an accessory navicular bone did not affect radiologic parameters of the foot. The aim of this research is to examine the methods of nursing students to deal with future anxiety and stress.It is a cross-sectional survey conducted in Turkey with 291 students in Çukurova University of Faculty of Health Sciences, Kahramanmaraş Sütçü İmam University Health College and Batman University Health College Nursing Department in June 2020. Personal Data Form, Stress Coping Scale, State and Trait Anxiety Scale were sent online to students' smartphones and/or e-mails as data collection tools and it was collected this way.The mean age of the participants was 21.09 ± 2.02 (years). 78% of respondents were women. 48.4% of the participants were students of Çukurova University. It was determined that 201 (69.1%) of the participants isolated themselves during the pandemic. It was found that 171 students (58.8%) spent 23 to 24 hours at home, whereas 284 students (97.6%) spent time with their parents/siblings. 47.4% of respondents stated that they had spent the pandemic watching a series/film. 47.1% of respondents had good family relationships. 50.2% of respondents had good relationships with college friends. 74.9% of respondents said they were happy. Women's trait anxiety scale scores were higher than men's (P .05). Men have higher problem-Oriented coping scores than women (P .05). Significant differences were found in the Status Anxiety Scale scores and trait anxiety scale scores according to self-isolation status (P .05). A significant difference was found in terms of state anxiety scale and trait anxiety scale according to happiness status (P .05). The problem-based coping scores of those who were happy with the Stress Coping Scale were higher than those who were not happy (P .05). The state anxiety scale of the students was 42.54, and the trait anxiety scale was 45.16.Nursing students' status and sustained Anxiety Scale scores were moderate. It is important for individuals to have good family and friends and to be happy in the process of the Corona Virus Disease pandemic.

Ensefalosel, nöral tüp defektlerinin bir varyantıdır. Yüzey ektoderminin nöral ektodermden gebeliğin 4. haftasında yetersiz ayrılması, ensefalosellerin oluştuğu temel mekanizmadır. İnsidansı canlı doğumlarda 1-3/1000’dir. Ensefalosellerin sınıflandırılması lezyon bölgesine göre yapılmaktadır ve yapılan epidemiyolojik çalışmalar ensefalosel tiplerinin görülme oranlarının ırklar ve coğrafi bölgeler arasında farklılık olduğunu göstermiştir. Frontal ensefalosel batı ülkelerinde daha az sıklıkla görülen anterior ensefaloselin nadir görülen bir örneğidir. Frontal ensefalosel yaşamla bağdaşabilen bir ensefalosel türü olmasına rağmen ek anomali varlığında gebelik terminasyonu da aile ile beraber değerlendirilmesi gereken bir konudur. Yirmi altı yaşında ve ilk gebeliği olan hastanın gebeliğinin 18. haftasında frontal ensefalosel tanısı konuldu. Yapılan ultrasonografik değerlendirmede ek anomali görülmeyen hastanın doğumu, gebeliğinin 386/7. haftasında sezaryen ile gerçekleştirildi.

Objective The aim of this study was to assess the diagnostic values of new biochemical markers that may be an alternative to the oral glucose tolerance test (OGTT) and determine the differences in these markers among three groups of women with varying degrees of glucose homeostasis dysregulation. Material and Methods This was a prospective study. All women were screened with 50 gram (g) oral glucose and a 100 g OGTT for gestational diabetes mellitus (GDM). The patients were divided into three groups depending on the result of the tests: no evidence of glucose metabolism abnormality (controls); impaired glucose tolerance (IGT); and GDM. All three groups were evaluated for serum human advanced glycation end-products (AGEs) concentrations, carboxymethyl lysine (CML) concentration and receptor for advanced glycation end-product concentrations (RAGE/AGER), body mass index (BMI), age, fasting glucose levels, obstetrical parameters and gestational age. Results The study included 180 women divided into 59 (32.8%) GDM, 50 (27.8%) IGT and 71 (39.4%) controls. Age was similar among the three groups. Amaç: D vitamini kalsiyum dengesi ve kemik sağlığının sağlanmasında önemli bir yağda çözünen vitamindir. D vitamini eksikliği tüm dünyayı etkileyen önemli bir halk sağlığı sorunudur. Bu çalışmanın amacı; bölgemizdeki gebelerin D vitamini seviyesini saptamak ve olası gebelik komplikasyonları ile olan ilişkisini değerlendirmektir. Materyal ve Metod: Kesitsel tipte yapılan bu çalışmaya kadın hastalıkları ve doğum polikliniğine rutin gebelik muayenesi için başvuran 175 gebe dahil edildi. Çalışmaya katılan gebelerin demografik özellikleri ve obstetrik hikayeleri kayıt edildi. Maternal serum D vitamini seviyeleri ile gebelik sonuçları karşılaştırıldı. Bulgular: Çalışmaya katılan gebelerin ortalama serum D vitamini değeri 11,16±7,15 ng/ml olarak saptandı ve gebelerin % 57.7’sinde ciddi D vitamini eksikliği (< 10 ng/ml) vardı. Gebelik sonuçları ile maternal D vitamini seviyeleri arasında ilişki saptanmadı. Sonuç: Çalışmamızda ciddi D vitamini eksikliğinin % 57.7 olarak saptanmasına rağmen maternal D vitamini seviyeleri ile gebelik sonuçları arasında herhangi bir ilişki saptanmadı.

Life-long anticoagulation therapy is mandatory in patients with mechanical heart valve to prevent the valve thrombosis. It may cause teratogenic effects and hemorrhage complications in the pregnant woman. Reflektör, çizgisel kaynak tarafından odak-dışı olarak beslenmiştir. Köşe kırınım alanları, Köşe Noktası Metodu (Edge Point Method) kullanılarak asimptotik olarak hesaplanmıştır. Toplam saçılan, kırınan ve yansıyan alanlar sayısal olarak, parabol genişliği, çizgisel kaynağın konum açısı gibi problemin bazı parametreleri için çizdirilmiştir., Fields of a line source diffracted by a cylindrical parabolic Perfectly Electric Conducting (PEC) reflector are investigated by employing the scattering integral of the Modified Theory of Physical Optics (MTPO). The reflector that is symmetrically located with respect to x-axis is offset-fed by a line source. The line source is lying parallel to the z-axis and off the focus of the reflector. Diffracted fields are evaluated asymptotically by means of the Edge Point Method. An approximate way of expressing uniform diffracted fields is used by utilizing the Signum and Fresnel functions together to overcome the non-uniform situations. Total scattered, diffracted, and reflected fields are plotted numerically for some parameters such as parabola width and location angle of the line source in the problem.

We introduce a new modification of $(p,q)$(p,q)-analogue of Szász–Mirakjan operators. Firstly, we give a recurrence relation for the moments of $(p,q)$(p,q)-analogue of Szász–Mirakjan operators and present some explicit formulae for the moments and central moments up to order 4. Next, we obtain quantitative estimates for the convergence in the polynomial weighted spaces. In addition, we give the Voronovskaya theorem for the new $(p,q)$(p,q)-Szász–Mirakjan operators.

Giriş ve Amaç: Bu çalışma, elit düzeydeki kadın hentbolcularda dominant ve nondominant bacak ve omuzda 60 ve 3000 /sn açısal hızlardaki diz ekstansiyon-fleksiyon (E-F) ve omuz internal-eksternal (İ-E) izokinetik kuvvet ölçüm performanslarını ve oluşan bilateral kuvvet farklılıklarını saptamayı ve belirlenen bu izokinetik kuvvet ölçümlerinden dominant ve nondominant diz ekstansiyon ve fleksiyon zirve tork değerlerinin sprint, çeviklik, dayanıklılık ve 5 tekrarlı squat; dominant ve nondominant omuz internal ve eksternal zirve tork değerlerinin de 5 tekrarlı bench press ile olan korelasyonu belirlemeyi amaçlamaktadır. Gereç ve Yöntemler: Bu çalışmaya Türkiye Kadın A-Milli Hentbol Takımında yer alan 15 sporcu gönüllü olarak katılmıştır. Çalışma süresince çeviklik, sürat, 5 tekrarlı maksimal squat ve bench press, izokinetik ölçümler ve 3000 m dayanıklılık değerlendirilmiştir. İlk test gününde T çeviklik testi ve 0-30 m sürat testi yapılmıştır. İkinci test gününde 5 tekrarlı maksimal squat ve bench press testleri gerçekleştirilmiştir. Üçüncü test gününde diz ekstansiyonfleksiyon ve omuz internal-eksternal izokinetik kuvvet ölçümleri yapılmıştır. Son test gününde ise 3000 m dayanıklılık testi uygulanmıştır. OBJECTIVE The aim of this study was to evaluate endometrial receptivity by measuring HOXA-10, HOXA-11, and LIF gene expressions in women with polycystic ovary syndrome. METHODS A total of 48 women were included in this clinical study. Thepatients were allocated to two groups: study group consisted of 28 patients with myoma uteri and control group consisted of 20 patients without myoma uteri. Endometrial sampling was performed during the proliferative phase. The biopsies obtained from the patients with myoma uteri were taken from the place where the fibroids were localized. HOXA-10, HOXA-11, and LIF expressions were measured in the endometrial sampling material. Demographic data of the patients such as age, obstetric and gynecologic history, medical conditions, medications, surgical history, last menstrual period were recorded. ÖZET Amaç: Bu çalışmanın amacı; infertil ve fertil kadınların sosyokültürel ve ekonomik durumlarını karşılaştırmak, bu etkilerin infertilite etyolojisindeki yerini belirlemektir. ObjectiveGlucagon-like peptide 1 (GLP-1), a gut-derived peptide has been reported to have insulin-like effects. Our aim is to examine GLP1 levels in hyperemesis gravidarum (HEG).Materials-methodsThe study population consisted of 2 groups: Group 1 (control subjects) consisted of 22 women with uncomplicated singleton pregnancies in the first trimester. Group 2 consisted of 22 singleton pregnancies complicated by HEG. Glucose and GLP1 levels were determined. Enzyme-linked Immunosorbent Assay Kit for Glucagon like Peptide 1 (GLP1) was used (Uscn, Life Science Inc.).ResultsNo significant differences in maternal age, gestational age and gravida were observed between hypermetric and control groups. Maternal serum GLP1 levels were significantly higher in HEG compared with control group (P = 0.004).ConclusionThe results of our study revealed that the presence of increased GLP1 levels in women with HEG could contribute to the pathogenesis of the disease. Our results indicated that increased GLP1 levels may be associated with hyperemesis gravidarum. The limitation of our study was the restricted number of patients. Large prospective and randomized studies are required to evaluate the effect of GLP1 levels on hyperemesis gravidarum.

Amaç: Bu çalışmanın amacı kliniğimize başvuran preeklampsi vakalarından HELLP sendromu gelişen olguların klinik, biyokimyasal değişiklerinin, maternal ve fetal komplikasyonlarının incelenmesidir. Çalışmanın yapıldığı yer: Bozok Üniversitesi Kadın Hastalıkları ve Doğum Anabilim Dalı, Yozgat Yöntem: Ocak 2017-Mart 2019 tarihleri arasında kliniğimize yatırılan 56 preeklamptik hastadan HELLP sendromu gelişen olgular retrospektif olarak incelendi. Tüm olguların klinik, laboratuvar parametreleri, fetal ve maternal komplikasyonları değerlendirildi. Bulgular: Kliniğimizde yatırılan 56 preeklamptik gebenin 22 ‘si ağır (%39.2), 34’ü (%60.8) hafif preeklampsi idi. Şiddetli preeklampsi vakalarından 3 tanesinde HELLP sendromu geliştiği saptandı. Bu kapsamda Kur’ân ayetlerinin 1787 tanesi kıssalardan oluşmaktadır. Kur’ân’da anlatılan kıssalar, insanlık tarihinde yaşanmış hayat gerçekliklerini konu edinmektedir. Bu yüzden Kur’ân kıssalarının, yaşanmış, yaşanması muhtemel veya yaşanmamış olayları konu edinen “hikâye” kavramıyla tanımlanıp tanımlanamayacağı meselesi tartışılmıştır. Bu makalede söz konusu tartışmanın hangi zeminde sürdürüldüğü ve Kur’ân kıssalarını “hikâye” kavramıyla tanımlamanın imkânı tartışılırken meseleye hangi açılardan yaklaşılması gerektiği üzerinde durulmuştur. A 2-D Computational Fluid Dynamics (CFD) model was used to investigate erosion in standard elbows (r/D = 1.5) under different flow and particle conditions. Afterwards, dimensional analysis was performed to first obtain the governing dimensionless groups and then extract self-similar behavior among them when plotted against the ratio of erosion rate/gas velocity. This resulted in the development of a new dimensionless group named penetration rate (Pnr) correlated to dimensionless erosion ratio as ( ER / V g ) ∝ ( Pnr ) 1.0612 . This correlation was plotted for different experimental data; the same trend was found.

OZET AMAC: Hamilelik esnasinda gecirilen asi ile onlenebilen hastaliklar sadece anne icin degil fetus icin de risklidir. Bu calismanin amaci; Turkiye’nin Yozgat ilindeki hamile kadinlarda secilmis asi ile onlenebilen hastalik etkenleri olan kizamik, kizamikcik, kabakulak, sucicegi (KKKS) ve tetanoz hastaligi antikorlarinin seroprevalansini arastirmaktir. METHOD: Kan ornekleri, hastanemiz kadin hastaliklari ve dogum poliklinigine basvuran 176 hamile kadindan alindi. KKKS virusleri ve tetanoza karsi spesifik antikor titreleri ELISA ile kantitatif olarak saptandi. Hastalik, asilama gecmisi ve demografik bilgilerinin dahil oldugu anket formu dolduruldu. SONUCLAR: Katilimcilarin yas ortalamasi 27.2±4.9 (18-45) idi. Tetanoz ,KKKS IgG seropozitifligi sirasiyla % 90.34, % 97.16, % 98.30, % 99.4, % 100 idi. Son 5 yil icerisinde tetanoz asisi olanlarda bagisiklik orani olmayanlara gore daha yuksek bulundu (93.7% ye 84.6 % , p= 0.049). Tetanoza karsi bagisik olanlarin yas ortalamasi 26.9±5, bagisik olmayanlarin yas ortalamasi 29.8±3.7 bulundu (p=0.0195). Yas ile tetanoz bagisiklik oranlari arasinda ters korelasyon tespit edildi. Yas artikca tetanoz bagisiklik oraninin azaldigi tespit edildi (p=0.002). TARTISMA: Calismamizda gebelerde yuksek oranda tetanoz ve KKKS bagisikligi bulunmustur. Ancak, hamilelik esnasinda enfeksiyonun neden oldugu riskler goz onunde bulunduruldugunda, asi ile onlenebilen hastaliklara karsi antikorlar; daha once asilanmamis ya da hastalik gecirmemis dogurganlik caginda olan kadinlarda taranmalidir ve seronegatif olanlar asilanmalidir. Aim: Patent ductus arteriosus (PDA) is a frequent health problem in premature infants. Pharmacologic closure is recommended only for hemodynamically significant PDA (hsPDA) that is defined according to the clinical and echocardiographic criteria. The aim of this study was to explore the value of commonly used criteria in defining hsPDA and predicting the required number of courses of ibuprofen treatment to close PDA in premature infants. Methods: Sixty premature infants with a gestational age of ≤33 weeks were evaluated prospectively. Clinical and echocardiographic criteria [O2 requirement, ductus diameter (DD) and left atrial-to-aortic root diameter ratio (LA:Ao)] were used to define hsPDA. Clinical improvement after pharmacologic closure of PDA and association between the criteria and required number of ibuprofen courses were investigated. Results: O2 requirement decreased by PDA closure but was not different between patients with hsPDA and the others with PDA. Also, O2 requirement was not found to be associated with required number of ibuprofen courses. DD and LA:Ao were greater in patients with hsPDA. DD was found to be associated with required number of courses of ibuprofen treatment. Conclusion: Although there was an improvement in O2 requirement with PDA closure, echocardiographic criteria were found to be more valuable in defining hsPDA. DD should also be used to estimate the duration of treatment.

Although erosion under multiphase flow conditions can be observed frequently, due to its inherent complex nature, it has received much less attention compared to that in single phase flow. In the present study, a Computational Fluid Dynamics (CFD) model was adopted to analyze air-water-sand flow in a Horizontal-Horizontal (H-H) standard elbow with a diameter of 76.2 mm (r/D=1.5). For this configuration, erosion was examined under slug/pseudo-slug flow regimes. The CFD model was previously assessed using experimental data obtained using an ultrasonic technique. It was noticed that the passage of slugs was intermittent and only then the particles impacted the pipe elbow at high velocities. Moreover, it was observed that in H-H elbow, maximum erosion occurred at the top of the elbow's outer wall. Qualitative results such as phase distributions, particle concentration, particle velocity and erosion pattern were used to investigate the erosion mechanism under such multiphase flow conditions.

In this pilot study, exposure to airborne microorganisms in solid recovered fuel (SRF) plants has been measured. Different steps in SRF production processes such as separation and size reduction can cause generation of airborne microorganisms. Exposure to these biological agents can cause some health problems, so their levels should be measured to assess the potential risks. Therefore, exposure to airborne total bacteria, Gram-negative bacteria, actinomycetes and fungi were measured in the running SRF plant. Sample locations were chosen considering work areas and wind direction in the plant. Airborne microorganisms were collected via AES Sampl'air Lite on particular agar plates and incubated for microorganism growth. No thermophilic actinomycetes were obtained from the samples. The measured concentrations of mesophilic heterotrophic bacteria were almost similar at each point throughout the SRF plant (ranged from 920 to 1750 CFU/m(3)) except the spare waste stock area (427 CFU/m(3)) and the monitoring room (ranged from 420 to 770 CFU/m(3)). The highest concentrations were measured near the pre-shredder (1750 CFU/m(3)) and in the manual sorting unit (1610 CFU/m(3)). However, the concentrations of Gram-negative bacteria varied at each point (28 to 700 CFU/m(3)). Fungi concentrations varied from 280 to 1750 CFU/m(3) and were high near the sorting unit, the ballistic separator, the fine shredder, the end-product stock area and the monitoring room (1750 CFU/m(3)). The concentrations of mesophilic heterotrophic bacteria, Gram-negative bacteria and fungi did not exceed the recommended limit values.

Objective Tonsillectomy is one of the most common surgical procedures performed at ear, nose, and throat clinics. Chronic recurrent tonsillitis, obstructive tonsillitis, and halitosis are among the most common indications for surgery. Determining whether the infection is chronic and the patient's annual number of infections are important in estimating the necessity for surgery to be performed due to infectious causes. Red blood cell distribution width (RDW) is a numerical value present in normal complete blood count that provides information about erythrocytes and their dimensions. Studies in recent years have shown that RDW increases in chronic infections, hypoxia, and oxidative stress. This study investigated the changes in RDW in patients with chronic tonsillitis and the effect tonsillectomy has on this value by comparing RDW between patients scheduled for tonsillectomy and normal population and examining preoperative and postoperative changes in RDW. Materials and Methods Sixty-three patients scheduled for tonsillectomy due to recurrent tonsillitis aged 4-14 years were included in the study. The control group consisted of 60 subjects comparable in terms of age and sex. Hemoglobin level and RDW were recorded by collecting 2 mlof blood before surgery and at 4 months postoperatively from all patients. Results Preoperative RDW was significantly higher in the patient group than in the control group. Comparison of patients' preoperative and postoperative RDW revealed a significant decrease in RDW after surgery. Conclusion As a biomarker showing chronic infection in patients with tonsillitis, RDW can provide support to the clinician in deciding on surgery. However, this has to be confirmed in further studies with greater participation.

Model-based field development planning and optimization often require computationally intensive reservoir simulations, where the models need to be run several times in the context of input uncertainty or seeking optimal results. Reduced Order Modeling (ROM) methods are a class of techniques that are applied to reservoir simulation to reduce model complexity and speed up computations, especially for large scale or complex models that may be quite useful for such optimization problems. While intrusive ROM methods (such as proper orthogonal decomposition (POD) and its extensions, trajectory piece-wise linearization (TPWL), Discrete Empirical Interpolation Method (DEIM) etc.) have been proposed for application to reservoir simulation problems, these remain inaccessible or unusable for a large number of practical applications that use commercial simulators. In this paper, we describe a novel application of a non-intrusive ROM method, namely dynamic mode decomposition (DMD). We specifically look at reducing the time complexity involved in well control optimization problem, using a variant of DMD called DMDc (DMD with control). We propose a workflow using a training dataset of the wells and predict the state solution (pressure and saturation) for a new set of bottomhole pressure profiles encountered during the optimization runs. We use a novel strategy to select the basis dimensions to prevent unstable solutions. Since the objective function of the optimization problem is usually based on fluid production profiles, we propose a strategy to predict the fluid production rates from the predicted states from DMDc using machine learning techniques. The features for this machine learning problem are designed based on the physics of fluid flow through well perforations, which result in very accurate rate predictions. We compare the proposed methodology using another variant of DMD called ioDMD (input-ouput DMD) for system identification to predict output production flow rates. The methodology is demonstrated on a benchmark case and a Gulf of Mexico deepwater field that shows significant time reduction in production control optimization problem with about 30 – 40 times speedup using the proposed DMDc workflow as compared to fine scale simulations, while preserving the accuracy of the solutions. The proposed "non-intrusive" method in this paper to reduce model complexity can substantially increase the range of application of ROM methods for practical field development and reservoir management.

Amaç: Bu çalışmanın amacı non-diyabetik gebelerin vücut kitle indeksinin maternal ve fetal sonuçlara etkisini değerlendirmektir. Yöntem: Çalışmaya 2016 yılında Bozok Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Kliniğinde doğum yapan ve dosya kayıtlarına tam olarak ulaşılan 37 hafta üstü, 360 gebe dahil edilmiştir. Gebeler vücut kitle indeksi ≤30 ve >30 olanlar olarak ikiye ayrıldı. Gebeliğinde diyabetes mellitus ve hipertansiyon gibi komplikasyon gelişen kadınlar ve 37 hafta altı gebeler çalışma dışı bırakılmıştır. İstatistiksel analizler Statistical Package for Social Sciences (SPSS Inc; Chicago, IL, USA) versiyon 20.0 yazılımı kullanılarak yapılmıştır. Bulgular: Çalışmaya dahil edilen hastaların yaş ortalamaları 28.24±5.74, VKİ ortalamaları 30,49±4,85, son adet tarihine göre gebelik haftası ortalamaları 38.39±2.22 idi. Obez olan ve olmayan gruplar arasında makrozomik fetus, ikiz gebelik ve sezaryen oranlarında istatistiksel olarak anlamlı fark saptandı (sırasıyla p=0.007, p=0.041, p=0.046). Sonuç: Çalışmamızda non-diyabetik gebelerde obezitenin sezaryen oranlarını artırdığı, yenidoğanda pH değerlerinin düşmesine neden olduğu ve özellikle makrozomi insidansını artırdığı saptanmıştır.

Thrombus incidence is higher among neonates, especially in preterm infants, due to the associated additional risk factors.The medical recordings of premature infants who had been diagnosed as having intracardiac thrombus between January 2016 and January 2019 were evaluated retrospectively. We use recombinant tissue plasminogen activator when the thrombus is relatively large compared to left atrium, pedunculated, mobile, or snake shaped.A total of 13 premature patients were diagnosed as having intracardiac thrombus during the 3-year period. All were diagnosed during echocardiographic studies. Low molecular weight heparin was administered in four patients. In three, recombinant tissue plasminogen activator was started with low dose (0.01 mg/kg/h) and increased gradually to 0.06 mg/kg/h. In three, recombinant tissue plasminogen activators were started with standard dose (0.5 mg/kg/h). In one recombinant tissue, plasminogen activator was started with low dose (0.01 mg/kg/h) and increased to standard dose. Two patients died before treatment, three patients died during treatment, follow-up was not available for two patients, and thrombus completely resolved in six patients.In preterm babies with risk factors, intracardiac thrombus should be kept in mind during all echocardiographic studies. In our patients, low and standard dose regimens were used, and the treatment results were similar.

Objective: The aim of this study was to assess the awareness level of women about cervical smears, human papilloma virus (HPV), and HPV vaccine in a rural city in the central part of Anatolia. Materials and Methods: A total of 553 patients were included in the study. A 16 item questionnaire developed by our group was completed by all participants. The first part of the questionnaire collected the demographic and socioeconomic information of the participants. In the second part, it was questioned whether this information had a relationship with HPV, HPV vaccine awareness, and cervical screening tests. In the third part, the participants were asked questions related to the acceptance of an HPV vaccine for themselves and their willingness to give consent to have their children vaccinated. Yûsuf kıssasında ahkâma dair bazı âyetler, kıssadaki önceliğine göre (âyet numaraları esas alınarak) ele alınmakta ve bu bağlamda her bir âyetin hangi konuyla ilişkili olduğu açıklanmaktadır. Firstly,the place of stories in the Islamic law and what to pay attention while makinga judgement are mentioned concisely, the some verses in the story of ProphetJoseph about judgment are discussed with reference to the priority by using thenumber of them as base and, each verse is explained in terms of its relationwith story. In this context, the Surah of Joseph, inwhich the narrative of Joseph is told, concern on provisions about the subjectsmentioned for example: The verses 16 and 18 describe the “necessity of judgmentaccording to the evidence which is strong when the arguments put forward in amatter conflict” and the 19th verse tells “the issue of freedom of lakît”. Besides, 26th verse mentions“the judgment should be given according to the expert's report in the law andcriminal cases” and the 38th verse tells about the “determination of thegrandfather's share in the legacy when father is absent”, the 55th verse yettells that “when someone is better qualified than himself, he/she has to aspirefor duty”.
