Abstract

Childhood cancer is a success story of modern Medicine in which effective treatments have been identified for a previously untreatable disease. The current 5-years survival rate for all type of pediatric cancer reaches now 80%. Damages resulting from chemotherapy and radiotherapy are various and may not become clinically evident for many years after completion of treatment. Among them, cardiotoxicity is one of the most serious chronic complications mainly caused by anthracyclines and mediastinal radiation therapies. In this context, the purpose of our work was to evaluate the role of cardiac MRI in the early detection of late complications induced by cancer treatment in children. First, the role of cardiac MRI in the evaluation of early detection of late-onset treatment-induced cardiomyopathy. Patients with earlier detection of cardiac dysfunction or myocardial necrosis by cardiac MRI could indeed benefit from a better management. Second, the detection of iron overload in children undergoing cancer treatment. Iron overload, more often caused by long-term blood transfusions, is responsible for life-threatening cardiovascular and hepatic dysfunctions. Although repeated transfusions are needed during the treatment of most cancers, paediatric patients are not routinely screened for iron overload. In this work, we aimed to evaluate the prevalence of long-term iron overload in patients undergoing cancer treatments. Eighty-two children under 18 years of age were prospectively identified as potential candidates for cardiac evaluation during their cancer treatment from October 2009 to September 2013, regardless of their diagnosis. All patients were enrolled in a pre-chemotherapy and then yearly protocol including 1) a clinical evaluation, 2) a laboratory evaluation 3) an electrocardiogram, 4) an echocardiogram, 5) a CPET and 6) a cardiac and liver MRI. Abstract

Les avancées thérapeutiques en oncologie pédiatrique permettent à un nombre important d’enfants d’être guéris et d’atteindre l’âge adulte. Cependant, les chimiothérapies utilisées et la radiothérapie sont potentiellement toxiques et contribuent à une morbidité voire une mortalité significative au long cours. Ces patients traités en pédiatrie vont en plus bénéficier de transfusions multiples durant leurs traitements. Quel est l’impact de la surcharge en fer secondaire aux transfusions chez les survivants d’un cancer pédiatrique alors qu’on en connait la toxicité multi-organique chez les patients poly-transfusés comme les thalassémiques. Cette revue résume brièvement la physiopathologie de la surcharge en fer ainsi que les méthodes dont nous disposons pour l’évaluer. Actuellement, peu d’études ont été réalisées sur la prévalence de la surcharge en fer et son rôle potentiel dans les dysfonctionnements hépatiques et cardiaques tardifs dans cette population. Les conclusions seront exposées en partant d’une étude effectuée dans notre institution. La nécessité d’un traitement de chélation chez les patients est également discutées

120. Pseudomonas aeruginosa endocarditis in a 3-Year-old boy treated for acute lymphoblastic leukemia

Abstract

Background: Pseudomonas aeruginosa has been described as an uncommon cause of endocarditis in adults but to our knowledge no pediatric case has been reported in the literature for the past 20 years. Clinical case: We describe a 3-year-old boy diagnosed with acute lymphoblastic leukemia who first developed a fully susceptible Pseudomonas aeruginosa bacteremia while he was in neutropenia. An initial antibiotherapy was started using piperacillintazobactam and amikacin. Recurrent bacteremias occurred during the following 101 days despite central venous access device replacements and multiple courses of antibiotics, including meropenem, ceftazidime, ciprofloxacin, rifampicin and colimycin. Several cardiac ultrasound were performed but evidence of endocarditis was observed only after 60 days of bacteremia when both transoesophageal and transthoracic ultrasound showed the rupture of a tricuspid cord and the presence of a millimetric image on the atrial side of the tricuspid valve. Due to failure of medical treatment, cardiac surgery was performed. Peroperative findings confirmed a purulent granuloma and perforations on the tricuspid valve.The abcess was removed and the valve repaired. The valvular biopsy yielded Pseudomonas aeruginosa. Antibiotics were extended for 30 days after surgery with no further positive blood cultures at 5 months of follow-up. Conclusion: Pseudomonas aeruginosa is an exceptional cause of endocarditis in children. Prompt removal of central venous device should be performed in case of Pseudomonas aeruginosa bacteremia. Surgical intervention must be considered without delay if not responding to targeted antibiotics, particularly in leukemic patients with a greater risk of negative impact on their hematological outcome.

Abstract

BACKGROUND: Iron overload is responsible for severe morbidity and mortality in polytransfused patients. Although repeated blood transfusions are needed during the treatment of most cancers, pediatric patients are not routinely screened for subsequent iron overload. PROCEDURE: Seventy-five patients were identified as candidates for cancer treatment and enrolled prospectively in a yearly protocol including a cardiac and liver magnetic resonance imaging coupled with ferritin level measurements. Patients were divided into four groups using the intensity of treatment rating (ITR-3). RESULTS: Fifty-nine patients reached 1-year of follow-up and liver iron overload was found in up to 66% of them. Such overload correlated with the total volume of red blood cells transfused and persisted at least 2 years after the initiation of therapy. Moderate myocardial overload was also, but less frequently (14%), observed in these patients. CONCLUSIONS: Our study demonstrated that severe liver iron overload as well as moderate myocardial iron overload can be found 1 year after cancer treatment and that this overload persists overtime. The patients with higher ITR and those who have received more than a liter of blood red cells per square meter, regardless of their diagnosis or ITR, are at risk of iron overload and should be screened carefully. Les avancées thérapeutiques en hématologie et oncologie pédiatrique permettent à un nombre important d’enfants d’être guéris et d’atteindre l’âge adulte. Cependant, les traitements du cancer, et en particulier les anthracyclines et la radiothérapie médiastinale, sont potentiellement cardio toxiques et contribuent à une morbidité et une mortalité significatives à long terme. Cette revue résume brièvement les différentes techniques actuellement disponibles ou émergentes pour le suivi cardiaque des patients, les moyens de prévention pour limiter la cardiotoxicité et les traitements de ces complications cardiaques une fois qu’elles ont été diagnostiquées. Les pathologies cardiovasculaires étant la cause principale de décès chez ces survivants, le dépistage précoce d’une dysfonction cardiaque reste la clé de voûte du devenir à long terme de ces patients.

Cette revue résume brièvement la physiopathologie des maladies cardiovasculaires liées aux traitements du cancer, leur incidence, ainsi que les facteurs de risque principaux. Actuellement, peu d’études sur les effets secondaires cardiaques au long cours chez les survivants d’un cancer ont été réalisées. Les études les plus significatives sont exposées dans cette revue et toutes démontrent un risque majoré de complications cardiaques par rapport à la population normale.

Abstract

Les accidents cardiovasculaires lors d’activités sportives chez l’enfant et l’adolescent sont heureusement rares mais peuvent être à l’origine de morts subites. Différentes mesures peuvent être proposées en vue de diminuer leur incidence. Il n’existe malheureusement pas de consensus sur la question du screening pré-sportif et d’intenses controverses portent sur ce qu’il doit éventuellement comporter, notamment l’adjonction d’un électrocardiogramme ou d'une épreuve d'effort au bilan clinique classique. Une évaluation appropriée comportant une bonne anamnèse et un examen cardiovasculaire soigneux est indispensable et permettra la plupart du temps de découvrir une éventuelle anomalie, que préciseront ensuite les examens complémentaires.

Abstract

BACKGROUND: Infants with cyanotic congenital heart disease palliated with placement of a systemic-to-pulmonary-artery shunt are at risk for shunt thrombosis and death. We investigated whether the addition of clopidogrel to conventional therapy reduces mortality from any cause and morbidity related to the shunt. METHODS: In a multicenter, double-blind, event-driven trial, we randomly assigned infants 92 days of age or younger with cyanotic congenital heart disease and a systemic-to-pulmonary-artery shunt to receive clopidogrel at a dose of 0.2 mg per kilogram of body weight per day (467 infants) or placebo (439 infants), in addition to conventional therapy (including aspirin in 87.9% of infants). The primary efficacy end point was a composite of death or heart transplantation, shunt thrombosis, or performance of a cardiac procedure due to an event considered to be thrombotic in nature before 120 days of age. RESULTS: The rate of the composite primary end point did not differ significantly between the clopidogrel group (19.1%) and the placebo group (20.5%) (absolute risk difference, 1.4 percentage points; relative risk reduction with clopidogrel, 11.1%; 95% confidence interval, -19.2 to 33.6; P=0.43), nor did the rates of the three components of the composite primary end point. There was no significant benefit of clopidogrel treatment in any subgroup, including subgroups defined by shunt type. Clopidogrel recipients and placebo recipients had similar rates of overall bleeding (18.8% and 20.2%, respectively) and severe bleeding (4.1% and 3.4%, respectively). CONCLUSIONS: Clopidogrel therapy in infants with cyanotic congenital heart disease palliated with a systemic-to-pulmonary-artery shunt, most of whom received concomitant aspirin therapy, did not reduce either mortality from any cause or shunt-related morbidity. Abstract

Therapeutic advances in paediatric oncology allowed increasing numbers of children to survive until adulthood. However, chemotherapy and radiotherapy are potentially cardiotoxic and contribute to a significant morbidity and mortality, cardiovascular events remaining the leading cause of death among survivors. This review summarizes the physiopathology of treatment-related cardiovascular diseases, their incidence, and the risk factors associated with each specific therapy. Few studies have investigated the cardiac outcomes of adult surviving from childhood cancers but all demonstrated a substantial risk for late cardiac effects. Cardiovascular monitoring, prevention, and early detection of cardiac dysfunction are, therefore, the keystones of an improved long-term outcome.

Abstract

BACKGROUND: It remains incompletely understood whether patients with transfusion related cardiac iron overload without signs of heart failure exhibit already subclinical alterations of systolic left ventricular (LV) dysfunction. Therefore we performed a comprehensive evaluation of systolic and diastolic cardiac function in such patients using tagged and phase-contrast CMR. METHODS: 19 patients requiring regular blood transfusions for chronic anemia and 8 healthy volunteers were investigated using cine, tagged, and phase-contrast and T2* CMR. LV ejection fraction, peak filling rate, end-systolic global midventricular systolic Eulerian radial thickening and shortening strains as well as left ventricular rotation and twist, mitral E and A wave velocity, and tissue e' wave and E/e' wave velocity ratio, as well as isovolumic relaxation time and E wave deceleration time were computed and compared to cardiac T2*. RESULTS: Patients without significant iron overload (T2*>20 ms, n=9) had similar parameters of systolic and diastolic function as normal controls, whereas patients with severe iron overload (T2*<10 ms, n=5), had significant reduction of LV ejection fraction (54+/-2% vs. 62+/-6% and 65+/-6% respectively p<0.05), of end-systolic radial thickening (+6+/-4% vs. +11+/-2 and +11+/-4% respectively p<0.05) and of rotational twist (1.6+/-0.2 degrees vs. 3.0+/-1.2 and 3.5+/-0.7 degrees respectively, p<0.05) than patients without iron overload (T2*>20 ms) or normal controls. Patients with moderate iron overload (T2* 10-20 ms, n=5), had preserved ejection fraction (59+/-6%, p=NS vs. pts. with T2*>20 ms and controls), but showed reduced maximal LV rotational twist (1.8+/-0.4 degrees). The magnitude of reduction of LV twist (r=0.64, p<0.001), of LV ejection fraction (r=0.44, p<0.001), of peak radial thickening (r=0.58, p<0.001) and of systolic (r=0.50, p<0.05) and diastolic twist and untwist rate (r=-0.53, p<0.001) in patients were directly correlated to the logarithm

Although normothermia and warm blood cardioplegia are widely used in adults, cold crystalloids and hypothermia remain routinely used in pediatric cardiac surgery. The superiority of either technique in both brain and myocardial protection remains controversial. We designed a prospective randomized study to compare both approaches in terms of early myocardial protection and late neurodevelopmental status.

Abstract

Objectives: To describe fetal spectrum and echocardiographic characteristics of anomalous systemic venous return (ASVR, cardinal veins) without other structural heart defects (isolated ASVR), evaluate associated extracardiac and genetic anomalies and review neonatal outcome. Methods: From 2003 to 2009, 369 consecutive fetuses were diagnosed with cardiac malformation, including 27 cases with an isolated ASVR. The following variables were collected: type of ASVR, extracardiac anomalies, karyotype and short-term outcome. Results: The most common forms of ASVR were persistent left superior vena cava (SVC) (n = 15) and interrupted inferior vena cava (IVC) (n = 6). The other ASVR types were a persistent left SVC without a right SVC (n = 4), bilateral agenesis of the SVC with cephalic venous drainage toward the IVC through the azygos system (n = 1) and total ASVR to the coronary sinus (n = 1). Genetic disorders were confirmed in five cases, all with a persistent left SVC, with extracardiac anomalies present in four of them. Ten other patients had extracardiac anomalies detected prenatally or postnatally. When available, ASVR diagnosis was confirmed postnatally in all cases. Conclusion: Isolated ASVRs can be accurately diagnosed prenatally and are frequently associated with extracardiac and genetic anomalies. A detailed morphological ultrasound and fetal karyotype should be suggested. Neonatal outcome was mainly related to the associated anomalies. Copyright © 2011 John Wiley & Sons, Ltd.

Les cardiomyopathies regroupent l’ensemble des affections lésant le myocarde tout en respectant les autres structures cardiaques (comme les valves, le péricarde, les coronaires). Elles ?peuvent être primaires ou secondaires à différents facteurs (médicament, hypertension artérielle, inflammation, trouble métabolique, ischémie, etc.). Plus fréquemment, ces maladies sont classées en fonction de leur mode de présentation. On distingue ainsi les cardiomyopathies dilatées, hypertrophiques et restrictives. Une même étiologie peut néanmoins présenter différents visages, appartenant à des classes différentes de cardiomyopathies. Nous vous proposons ici la 1ère partie de cet article.
