Abstract

National audience; Under the frame of an EEC - INCO-DC programme, 'APRIMED' research project is under development with the objective to characterize, to preserve and to identify the apricot genetic resources in part of the Mediterranean Region (Turkey, Spain, Italy, France,. Morocco and Tunisia). An integrated and multidisciplinary approach divided in 3 main tasks has been developed. Prospection and preservation of genetic resources - During the prospection, 103 accessions have been identified in Morocco and S8 in Tunisia; while the preservation of 430 clones has been performed in France, 279 in Italy and 94 in Turkey.

Abstract

Although the treatment of anal squamous carcinoma is primarily medical, abdomino-perineal resection remains the only alternative treatment if primary chemoradiotherapy is contraindicated or is unsuccessful. Salvage surgery for anal squamous carcinoma requires wide excision of the anal sphincter and permanent abdominal colostomy. Such major surgery in an irradiated field is associated with significant morbidity from delayed wound healingm, but this can be reduced by the use of myocutaneous flaps, whilst perineal colostomy allows a permanent abdominal stoma to be avoided. These techniques have been employed in the management of low rectal cancer with good oncological and functional results. However, there is little data on perineal colostomy and its feasibility in squamous carcinoma is limited by the extent of the perineal surgical resection.

Abstract

SERMs are developed in HRT in order to provide the beneficial effects of estradiol on bone and the cardiovascular system. SERMs are antiestrogens and their properties depend upon the pharmacological class they belong to. Tibolone is a progestin with mixed properties. Our studies on breast cells in vitro demonstrated that it behaves as a progestin in these cells. The clinical data obtained with these various therapies on breast are presented.

Abstract

The silicone implant controversy wavers between reassuring epidemiological studies and about 300 case reports of patients developing a definite or incomplete/atypical connective tissue disease (CTD) after receiving a silicone gel-filled breast implant (SBI). Since Hashimoto's thyroiditis (HT) is rarely reported in this context, we report here two new cases of HT associated with a history of bilateral cosmetic SBIs. The first patient was a 45-year-old white woman who had SBIs in 1976. In 1991 she developed HT, evolving to thyroid deficiency which was compensated with levothyroxine treatment. In addition, the patient complained of fatigue, arthralgia, morning stiffness and developed a sicca syndrome necessitating artificial tears. The 1995 evaluation disclosed the presence of antinuclear antibodies at a titre of 1/640, and high level anti-thyroid microsomal antibodies (1/256,000). Gamma globulins rose to 22.6%. Thyroid ultrasonography showed an enlarged thyroid gland with a diffusely hypoechogenic pattern. The implants were painful, and in 1996 they were removed. Microscope examination of the fibrous capsule surrounding the prostheses showed extremely dense connective tissue with fibrosis. The second patient was a 55-year-old white woman who had SBIs in 1984. In 1995, she developed HT with clinical pain and tenderness of the thyroid gland, with mild hyperthyroidism and positive antithyroglobulin antibodies, and was given corticosteroid treatment for 5 months. In 1996, the implants were again painful and the patient developed positive antinuclear antibodies with a titre of 1/200. Ultrasonography showed a heterogeneous thyroid gland, and implant removal was advised. Hashimoto's thyroiditis is recognized as a subset of chronic auto-immune thyroiditis, and its association with SBI is rare. In these 2 observations, an association without relation is possible, but a future survey of similar cases seems warranted.

Abstract

Soilless culture is one of the alternative techniques to cope with saline soil. The objective of this study is to identify a new local substrate based on cactus fiber. Three mixtures of cactus fiber and coconut fiber substrates (M0, M1, M2, M3, and C) at a ratio of (1:0, 3:1, 1:1, 1:3, and 0:1), respectively, were evaluated on round tomato cultivation under greenhouse. The mixtures had an organic matter content that exceeded 82%, a C/N ratio between 41.3% and 46%. The mixtures did not cause any phytotoxicity in lettuce and bean. The economic study showed that the use of M2 substrate was the most profitable.

Abstract

Anaemia and iron deficiency are frequent in patients scheduled for cardiac surgery. Perioperative patient blood management (PBM) is widely recommended in current practice guidelines. The aim of this protocol is to analyse the effect of a global perioperative PBM programme on the red blood cell (RBC) transfusion ratio, morbidities and rehabilitation score in elective cardiac surgery.This study is a prospective, single-centre trial with a 2-step protocol, A and B, as follows: A: non-drug intervention: the caregiver is given a blood management educational programme; B: drug intervention: systematic correction of perioperative iron, vitamin deficiencies, and anaemia. This study was designed to enrol 900 patients (500 in group A and 400 in group B) in a rolling period starting at anaesthesia consultation and ending 3 months after surgery. The primary objective was a 20% reduction in RBC transfusion after implementation of PBM programmes (protocol A + B) when compared to our previous transfusion ratio in the first half of 2018 (30.4% vs 38%). The secondary objectives were to evaluate the impact for each step of the study on the RBC transfusion rate, morbidity and the quality of postoperative rehabilitation.The strength of this study is its evaluation of the effect of a global PBM programme on RBC transfusion in cardiac surgery through a 2-step protocol. We aim to assess for the first time the impact of non-drug and drug interventions on RBC transfusion, comorbidities and delayed rehabilitation parameters. Trials registrations: ClinicalTrials.gov, NCT04040023: registered 29 July 2019.
