Abstract: Background A multidisciplinary international working subgroup of the third Perioperative Quality Initiative consensus meeting appraised the evidence on the influence of preoperative arterial blood pressure and community cardiovascular medications on perioperative risk. Methods A modified Delphi technique was used, evaluating papers published in MEDLINE on associations between preoperative numerical arterial pressure values or cardiovascular medications and perioperative outcomes. The strength of the recommendations was graded by National Institute for Health and Care Excellence guidelines. Results Significant heterogeneity in study design, including arterial pressure measures and perioperative outcomes, hampered the comparison of studies. Nonetheless, consensus recommendations were that (i) preoperative arterial pressure measures may be used to define targets for perioperative management; (ii) elective surgery should not be cancelled based solely upon a preoperative arterial pressure value; (iii) there is insufficient evidence to support lowering arterial pressure in the immediate preoperative period to minimise perioperative risk; and (iv) there is insufficient evidence that any one measure of arterial pressure (systolic, diastolic, mean, or pulse) is better than any other for risk prediction of adverse perioperative events. Conclusions Future research should define which preoperative arterial pressure values best correlate with adverse outcomes, and whether modifying arterial pressure in the preoperative setting will change the perioperative morbidity or mortality. Additional research should define optimum strategies for continuation or discontinuation of preoperative cardiovascular medications.

Abstract: Introduction: Patients with hematologic malignancies, including multiple myeloma (MM), experience worse SARS-CoV-2 infection outcomes and sub-optimal vaccine responses. Monoclonal gammopathy of undetermined significance (MGUS) and smoldering multiple myeloma (SMM) precede MM and affect ~5% of individuals age >=50. We previously showed that individuals with MGUS and SMM exhibit immune dysregulation. Here, we investigate the immune response to SARS-CoV-2 vaccination in these asymptomatic but potentially immunocompromised individuals. Methods: The IMPACT study (IRB #20-332) is a prospective study at Dana-Farber Cancer Institute in collaboration with MMRF, which enrolled individuals nationwide with a diagnosed plasma cell dyscrasia and healthy individuals. As of October 2021, 3,005 individuals completed a questionnaire regarding prior infection or vaccination. We obtained 1,350 blood samples from 628 participants and analyzed anti-SARS-CoV-2 IgG antibody titer by ELISA. Results: 2,771 (92%) participants were fully vaccinated (2 doses BNT162b2 or mRNA-1273; 1 dose Ad26.COV2.s), 269 (9%) had received a 3rd mRNA vaccine dose, and 234 (8%) were unvaccinated. 1,387 (46%) and 1,093 (36%) participants received mRNA vaccines (BNT162b2 and mRNA-1273), and 139 (5%) participants received an adenovirus vector vaccine (Ad26.COV2.S). 34 (1%) individuals reported SARS-CoV-2 infection after full vaccination. We measured antibody titers in 201 MGUS, 223 SMM, 40 smoldering Waldenstrom macroglobulinemia (SWM), 64 MM, and 100 healthy controls. Multiple linear regression model estimated the association between various clinical variables and post-vaccination antibody titers. As previously reported, having MM was associated with low antibody titer (p < 0.001). Of note, having SMM, regardless of risk stratification by 2/20/20 criteria, was also associated with low antibody titers, indicating that even low-risk SMM patients have a poor response to vaccination. MGUS and SWM diagnoses were not significantly associated with antibody titers. Additionally, male sex (p < 0.010), elapsed time after vaccination (p < 0.001), and BNT162b2 vaccine (p < 0.001) were associated with low antibody titers. SARS-CoV-2 infection prior to vaccination was associated with high antibody titers. We identified 25 patients (6 MGUS, 10 SMM, 2 SWM, 7 MM) who submitted blood samples after both the 2nd and 3rd dose. In these patients we observed a significant increase in antibody titer after a 3rd dose (p = 0.002). We also observed that antibody titers of patients after a 3rd dose (13 MGUS, 12 SMM, 2 SWM, 31 MM) were comparable to that of healthy individuals after a 2nd dose (p = 0.833). Conclusion: Our data indicates that suboptimal response to SARS-CoV-2 does not only occur with MM and cancer patients receiving therapy but also in precursor asymptomatic patients including low-risk SMM. Abstract: AimThis study sought to evaluate the feasibility and effectiveness of TAVI for treatment of aortic stenosis in the ACHD population.Methods and resultsReview of 802 patients that underwent TAVI from January 2008 to November 2019. 13 ACHD patients with different underlying congenital anatomy were identified (isolated bicuspid aortic valve was excluded and not defined as ACHD). The main endpoints were: paravalvular aortic regurgitation> grade 2 post-procedure, and all-cause 30-day mortality. Periprocedural and post-procedural complications were defined according to the UK TAVI registry. Median age was 66.7 years (range 29–84 years). Eleven patients had severe aortic stenosis (AS), 1 had both severe AS and severe aortic regurgitation (AR) and 1 had severe AR. Median hospital stay was 9 days (range 6–28 days). One patient required a pacemaker post-TAVI. No mortality or paravalvular AR > grade 2 at 30-days. Twelve patients had a reduction in NYHA class on their first post TAVI follow-up.ConclusionTAVI is viable option as an alternative to surgery for ACHD patients. Further experience with the use of TAVI in the ACHD patients is required to assess long-term outcomes in this unique group of patients.

Abstract: Left ventricular ejection fraction (LVEF) has a limited role in predicting outlook in heart diseases including heart failure. We quantified the independent geometric factors that determine LVEF using cardiac MRI and sought to provide an improved measure of ventricular function by adjusting for such independent variables. A mathematical model was used to analyse the independent effects of structural variables and myocardial shortening on LVEF. These results informed analysis of cardiac MRI data from 183 patients (53 idiopathic dilated cardiomyopathy (DCM), 36 amyloidosis, 55 hypertensives and 39 healthy controls). Left ventricular volumes, LVEF, wall thickness, internal dimensions and longitudinal and midwall fractional shortening were measured. The modelling demonstrated LVEF increased in a curvilinear manner with increasing mFS and longitudinal shortening and wall thickness but decreased with increasing internal diameter. Controls in the clinical cohort had a mean LVEF 64 ± 7%, hypertensives 66 ± 8%, amyloid 49 ± 16% and DCM 30 ± 11%. 108. The Person of Christ

Abstract: Jonathan Edwards’s doctrine of the Person of Christ has provoked a flurry of recent curiosity. Much of this curiosity has to do with how we make sense of the philosophical foundations on which the structure of Edwards’s Christology rests. In this chapter, I attempt to locate Edwards’s two-natures doctrine within the larger picture of catholic Christianity by situating his various claims about the God-man within the context of that historic distinction between concrete-nature and abstract-nature Christology, and all this, in light of his philosophical commitment to immaterialism. Putting Edwards’s philosophical speculations about the doctrine of the person of Christ into conversation with such important developments in the tradition is a step that must be taken if we are going to further enrich our understanding of his contribution to catholic Christology.

109. Ultrasound does not activate but can inhibit in vivo mammalian nerves across a wide range of parameters

Abstract: Ultrasound (US) has been shown to stimulate brain circuits, however, the ability to excite peripheral nerves with US remains controversial. To the best of our knowledge, there is still no in vivo neural recording study that has applied US stimulation to a nerve isolated from surrounding tissue to confirm direct activation effects. Here, we show that US cannot excite an isolated mammalian sciatic nerve in an in vivo preparation, even at high pressures (relative to levels recommended in the FDA guidance for diagnostic ultrasound) and for a wide range of parameters, including different pulse patterns and center frequencies. US can, however, reliably inhibit nerve activity whereby greater suppression is correlated with increases in nerve temperature. By prohibiting the nerve temperature from increasing during US application, we did not observe suppressive effects. Overall, these findings demonstrate that US can reliably inhibit nerve activity through a thermal mechanism that has potential for various health disorders, though future studies are needed to evaluate the long-term safety of therapeutic ultrasound applications.

110. Restorative Justice Conferencing in an Environmental Offending Context: Case Studies

Abstract: This book is concerned with restorative justice conferencing embedded within the prosecution of environmental offending and not as an alternative to it (a back-end model). This chapter will explore three case studies of conferencing in this context—Canterbury Regional Council v Interflow (NZ) Ltd [2015] NZDC 3323 (Canterbury, New Zealand; water pollution); and, Garrett v Williams (2007) 151 LGERA 92 and Chief Executive, Office of Environment and Heritage v Clarence Valley Council (2018) 236 LGERA 291 (New South Wales, Australia), cases involving Aboriginal cultural heritage. These New South Wales cases are the only examples of restorative justice conferencing in the prosecution of environmental offending in Australia. Collectively, these cases provide guidance for the future use of restorative justice conferencing in an environmental offending context.

111. The potential of restorative justice in promoting environmental offenders' acceptance of responsibility

Abstract: Restorative justice conferencing offers promise in dealing with the harm flowing from environmental crimes. The United Nations Office on Drugs and Crime outlined four critical ingredients necessary for restorative justice to achieve its objectives. One of these critical ingredients is that the offender accepts responsibility for the offence. In another article, we explored this requirement, outlining how contrition and remorse can be evidence of an acceptance of responsibility. In this article, we go further by questioning whether environmental crime offenders who do not accept responsibility for the offending should necessarily be excluded from participating in restorative justice conferencing. As will be seen, the benefits of conferencing with offenders who seem reluctant to accept responsibility may outweigh its disadvantages. This is particularly because the facilitated dialogue can provide a golden opportunity for the offender to come to realise the harm caused by their offending and its impact on victims.

Abstract: A binary conceptualisation of justice is achieved in the prosecution of environmental offending with minimal to no offender and victim voice, interaction, and input, leading to the questioning of the sufficiency of that binary conceptualisation. Drawing on various approaches to harm under green criminology (environmental justice, ecological justice, climate justice, species justice, and earth jurisprudence/deep ecology) a third conceptualisation of justice is devised—justice as meaningful involvement. As well as exploring justice as meaningful involvement, this chapter explores how it can be achieved through restorative justice conferencing. Vital for its acceptance amongst the legal profession, government and judiciary is the fact that conferencing does not displace justice as procedure and justice as outcome, thereby leading to the achievement of justice as a tripartite conceptualisation.

Abstract: Background: The coronavirus disease-2019 (COVID-19) pandemic significantly affected management of cardiovascular disease around the world. The effect of the pandemic on volume of cardiovascular diagnostic procedures is not known. Objectives: This study sought to evaluate the effects of the early phase of the COVID-19 pandemic on cardiovascular diagnostic procedures and safety practices in Asia. Methods: The International Atomic Energy Agency conducted a worldwide survey to assess changes in cardiovascular procedure volume and safety practices caused by COVID-19. Testing volumes were reported for March 2020 and April 2020 and were compared to those from March 2019. Data from 180 centers across 33 Asian countries were grouped into 4 subregions for comparison. Results: Procedure volumes decreased by 47% from March 2019 to March 2020, showing recovery from March 2020 to April 2020 in Eastern Asia, particularly in China. The majority of centers cancelled outpatient activities and increased time per study. Practice changes included implementing physical distancing and restricting visitors. Although COVID testing was not commonly performed, it was conducted in one-third of facilities in Eastern Asia. The most severe reductions in procedure volumes were observed in lower-income countries, where volumes decreased 81% from March 2019 to April 2020. Conclusions: The COVID-19 pandemic in Asia caused significant reductions in cardiovascular diagnostic procedures, particularly in low-income countries. Further studies on effects of COVID-19 on cardiovascular outcomes and changes in care delivery are warranted.

Abstract: This chapter will explore the benefits and limitations of restorative justice conferencing drawing on the use of the back-end model in New Zealand (including Canterbury Regional Council v Interflow (NZ) Ltd [2015] NZDC 3323) and the Land and Environment Court of New South Wales experience in Garrett v Williams (2007) 151 LGERA 92 and Chief Executive, Office of Environment and Heritage v Clarence Valley Council (2018) 236 LGERA 291. Benefits include offender and victim voice, interaction, and input into the prosecution process; opportunity for apology and forgiveness; and better outcomes which are more innovative and targeted to repair the harm occasioned by the offending. Conferencing can be more expensive, take longer and require more energy than traditional prosecution and this could be considered its primary limitation.

Abstract: The purpose of this chapter is twofold. Firstly, it will explore the literature proffering the potential of restorative justice for environmental offending. The literature on restorative justice in the wake of environmental offending is scant and underdeveloped. Secondly, the chapter will explore some of the existing uses of restorative justice for environmental harm. Abstract: This chapter explores some of the barriers to conferencing. Arguably, the main barrier to conferencing, and the reason for its embryonic state before the Land and Environment Court of New South Wales (‘NSWLEC’), is the lack of a necessary architecture. That architecture requires a legislative framework, consideration as to the judicial oversight of conferencing outcomes, proliferation of stakeholder, practitioner and judicial knowledge of conferencing, consideration of the selection criteria for participation in conferencing, and the actual facilitation of conferencing and follow up. On a practical level, consideration needs to be given to the representation of offenders and stakeholders and their role at conferencing. Finally, consideration will be given to the potential of a restorative justice unit within the NSWLEC and the role it would fulfil.

Abstract: This chapter conceptualises the work of environmental/green victimology and its focus on victims of environmental harm. Green criminology has been slow to engage with victims of environmental harm and crime. This may be a result of the view that environmental crime is not real crime, in that it is not intrinsically bad because some harm to the environment is lawful and licensed. It may also derive from the fact that it may take many years for the effects of the offending to be felt and therefore is viewed by some as victimless. This means that many victims of environmental crime will not be recognised as such, will not participate in the resolution of the crime, and will not receive compensation for the harm suffered.

Abstract: This chapter introduces the reader to the prosecution of environmental offending. It does so through the exploration of water pollution and breach environment protection licence offending prosecuted before the Land and Environment Court of New South Wales, Australia. Outlined are the central features of environmental offending and its prosecution, such as the nature of the offence (strict liability v mens rea); motive behind offending (intentional, negligent, accidental); guilty pleas; the offenders (individuals and organisations); sentencing options; reoffending; victims (individuals, communities, commercial operators and non-humans); and, offender and victim voice, interaction and input into prosecution. Such features are important when considering the use of restorative justice conferencing in the prosecution of environmental offending.

Abstract: OBJECTIVES: The INCAPS COVID Oceania study aimed to assess the impact caused by the COVID-19 pandemic on cardiac procedure volume provided in the Oceania region. METHODS: A retrospective survey was performed comparing procedure volumes within March 2019 (pre-COVID-19) with April 2020 (during first wave of COVID-19 pandemic). Sixty-three (63) health care facilities within Oceania that perform cardiac diagnostic procedures were surveyed, including a mixture of metropolitan and regional, hospital and outpatient, public and private sites, and 846 facilities outside of Oceania. The percentage change in procedure volume was measured between March 2019 and April 2020, compared by test type and by facility. Abstract: A 73 year old female patient presented 14 days post aortic valve replacement (AVR) for bicuspid valve stenosis; she was diagnosed with COVID-19 infection, acute myocardial infarction, pulmonary embolism and retinal artery embolism. CT identified extensive thrombus formation on the valve struts. This demonstrated complete resolution at 11 weeks with warfarin therapy. Although venous thromboembolism is now well-documented in COVID-19 patients, this case highlights the increased risks of systemic arterial embolism following recent cardiac procedures; it also serves as a timely reminder of the consequences of COVID-19 and the radiological vigilance required in identifying AVR thrombosis on acute cardiothoracic CT. The observations made warrant careful thought with respect to perioperative anticoagulation strategy.

Abstract: Introduction The quantification of aortic valve (AoV) calcification on computed tomography (CT) has been recommended for the grading of patients with aortic stenosis. We aim to characterise the relationship between AoV calcification and the aortic valve area. Methods Data was prospectively collected on all patients undergoing CT for consideration of transcatheter aortic valve implantation (TAVI) from July 2019 - January 2020. CT was performed on multidetector scanners, (Siemens SOMATOM AS+ and Canon Aquilion ONE) and measurements of AoV calcification and AoV area were performed using validated software (TeraRecon, California). The Agatson method was used to calculate AoV calcification. Spearman correlation analysis was performed using R v3.3.3. Results The cohort consisted of 81 consecutive patients. There were a range of AoV calcium scores from 129–7791, with a mean score of 2,592 arbitrary units. There was a very weak negative correlation between AoV calcification and the CT derived AoV area (rs=-.08, p=0.46). Subgroup analysis demonstrated weak negative correlation in patients with tricuspid valves (n=62), rs=-.17, p=0.18 and in bicuspid valve patients (n=19), rs=-.26, p Conclusion The relationship between AoV calcification and AoV area is unclear, with no significant correlation demonstrated. It is important to understand the relationship between AoV calcification and AoV area before its use in clinical practice can be advocated.

Abstract: Introduction Computed tomography (CT) prior to trans-catheter aortic valve implantation (TAVI) requires adequate vascular opacification. With expert radiographer-led contrast dosing (standard protocol), a wide variation in arterial opacification was observed. Methods Considering the time required for the electrocardiographically (ECG)-gated and helical acquisitions (Siemens Somatom AS+ 128-slice scanner), an optimised weight-based protocol was developed using 30 s contrast bolus of iodine flux 15–19 mg/kg/s. Radiodensity [Hounsfield Units (HU)] was assessed with a circular region of interest in (a) ascending aorta (0.75 mm ECG-gated systolic acquisition) and (b) ascending, descending thoracic (at carina), infra-renal abdominal aorta and right external iliac artery (1–1.5 mm helical acquisition). Thirty-six sequential optimised scans were compared to 36 prior standard scans. All patients had adequate renal function (eGFR>45 ml/min/1.73 m2). There was no difference in the CARE kV between the protocols (p=0.67). Results The mean patient age was 78 and 80 years (standard, optimised). The male to female ratio was 47:53 and 61:39 (standard, optimised). The mean bolus durations were 25±5.3 s and 30±0.3 s (standard, optimised); a significant variation (p 0.9), there was a significant improvement in the arterial radiodensity at all other anatomical points in the helical scans of the optimised protocol (p Conclusion Optimising contrast flux and matching bolus duration to the CT technology dramatically improves TAVI planning scans with objectively improved arterial opacification. This results in better contrast opacification for optimal assessment of aortic valve prosthesis sizing and access strategies.

Abstract: Introduction The imaging standard in the diagnosis and grading of aortic stenosis is the echocardiogram. However, computed tomography (CT) offers a more anatomically precise measurement of the left ventricular outflow tract (LVOT) area and aortic valve (AoV) area. The purpose of this study was to compare the AoV area measured by echocardiogram and CT. Methods Data was prospectively collected on all patients undergoing CT for consideration of transcatheter aortic valve implantation (TAVI) from July 2019 - January 2020. Measurements of LVOT and AoV area were performed using validated software (TeraRecon, California). Echocardiographic measurements of the LVOT, peak velocity across the LVOT and the AoV were used to derive the AoV area using the continuity equation. Spearman correlation analysis was performed using R v3.3.3. Results There were 19 patients in whom contemporaneous echocardiography was available. CT derived AoV area was positively correlated with echocardiographic measurements of AoV area (rs=0.54, p= 0.02) and LVOT area (rs=0.58, p Conclusion The use of the continuity equation on echocardiography leads to underestimation of the AoV area. The combination of echocardiography and CT measurements makes the continuity equation estimate of CT area near the same as CT planimetry.

Abstract: Clinical Presentation A 45 year old gentleman seen in Rapid Access Chest Pain Clinic with typical angina (exertional chest pain). No risk factors for CAD. Normal ECG. Pre-test probability of significant CAD 51% . Diagnostic techniques and findings The patient was referred for adenosine stress CMR. CMR was performed on a 1.5 Tesla scanner. Cine was performed first, followed by stress perfusion, gadolinium enhance images and finally rest perfusion images. LV systolic function was normal. A significant perfusion defect of the LAD territory was noted on the rest but not on the stress perfusion images. The radiographers confirmed that stress and rest perfusion images were performed normally (stress first). A good physiological response was obtained during the assumed vasodilator stress with > 20% in the HR (from 65-75bpm to 91-107bpm respectively). During the presumed rest perfusion acquisition, HR was similar to the previously recorded resting HR. The spleen was not adequately visible to comment on splenic switch off. This type of perfusion defect (normal stress/abnormal rest) had not been described before, and there is no evidence that it was due to acquisition error. A coronary CT was advised to assess the LAD. CT coronary angiogram showed mild coronary calcification. A severe (80%), focal, non-calcified mid LAD stenosis was identified. A mild proximal OM1 stenosis was also noted. The patient subsequently underwent an invasive angiogram which confirmed the presence of a severe mid LAD stenosis. PCI was performed in the same setting with deployment of a single 4x18mm XIENCE drug eluting stent. The successful procedure was optimised with OCT guidance. Learning points Resting perfusion is often considered an integral part of adenosine perfusion imaging in order to overcome dark ring artefact, but it requires an additional contrast injection, prolongs imaging time and adds to cost. Some experts suggest that resting perfusion does not add to clinical interpretation and is not required. Our case is interesting, showing a previously unreported phenomenon. We cannot explain the reason for it but do not believe that the stress/rest order was reversed (as this would require two radiographers to go off protocol, and they and the supervising physician to be lying) and the physiological response is as expected. It therefore suggests that a rest study may occasionally provide helpful diagnostic information. Abstract P707 Figure.

It is currently unclear whether the cerebral circulation can maintain cerebral blood flow (CBF) during reductions in cardiac output (CO) in hypertensive patients thereby avoiding hypoperfusion of the brain. We hypothesized that hypertension would impair the ability to effectively regulate CBF during simulated hypovolemia. Abstract: We investigate the impact of dipper status on cardiac structure with cardiovascular magnetic resonance (CMR). Ambulatory blood pressure monitoring and 1.5T CMR were performed in 99 tertiary hypertension clinic patients. Subgroup analysis by extreme dipper (n = 9), dipper (n = 39), non‐dipper (n = 35) and reverse dipper (n = 16) status was performed, matched in age, gender and BMI. Left ventricular (LV) mass was significantly higher for extreme dippers than dippers after correction for covariates (100 ± 6 g/m(2) vs 79 ± 3 g/m(2), P = .004). Amongst extreme dippers and dippers (n = 48), indexed LV mass correlated positively with the extent of nocturnal blood pressure dipping (R = .403, P = .005). On post‐hoc ANCOVA, the percentage of nocturnal dip had significant effect on indexed LV mass (P = .008), but overall SBP did not (P = .348). In the tertiary setting, we found a larger nocturnal BP drop was associated with more LV hypertrophy. If confirmed in larger studies, this may have implications on nocturnal dosing of anti‐hypertensive medications.

Abstract: Introduction: Multiple myeloma (MM) is a complex hematological malignancy with the heterogenous immune bone marrow (BM) environment contributing to tumor growth, drug resistance, and immune escape. T-Cells play a critical role in the clearance of malignant plasma cells from the tumor environment. However, T-Cells in multiple myeloma demonstrate impaired cytotoxicity, proliferation, and cytokine production due to the activation of immune inhibitory receptors from ligands produced by the myeloma cells. In this study, we investigate the behavior of T-Cells in MM patients by using single-cell RNA-Seq (scRNA-Seq) to compare the transcriptomic profiles of BM T-Cells of patients with rapid progressing (FP; PFS < 18mo) and non-progressing (NP; PFS > 4yrs) disease. Methods: Newly diagnosed MM patients (n=18) from the Multiple Myeloma Research Foundation (MMRF) CoMMpass study (NCT01454297) were identified as either rapid progressors or non-progressors based on their progression free survival since diagnosis. To capture transcriptomic data, scRNA-Seq was performed on 48 aliquots of frozen CD138-negative BM cells at three medical centers/universities (Beth Israel Deaconess Medical Center, Boston, Washington University in St. Louis, and Mount Sinai School of Medicine, NYC). Samples were collected at diagnosis prior to treatment. Surface marker expression for 29 proteins was captured for at least one sample per patient using CITE-Seq. After integration and batch correction, clustering was performed to identify cells of T or NK lineage. Uniform Manifold Approximation and Projection (UMAP) and differential expression were used to identify T-Lymphoid subtypes, and differences in NP and FP samples. Results: In this study, single cell transcriptomic profiles were identified for ~102,207 cells from 48 samples of 18 MM patients. 40,328 T (CD3+) and NK (CD3-, NKG7+) cells were isolated, and subclustered for further analysis (Fig 1A). Using differentially expressed markers for each cluster, the T-Lymphoid subset was refined into seven subtypes, consisting of various CD4+ T-Cells, CD8+ T-Cells, and NK cells (Fig 1B). The CD8+ cells were divided into three distinct phenotypes, namely a GZMK-, GZMB- CD8+ T-Cell cluster, a GZMK+ CD8+ Exhausted T-Cell cluster enriched in TIGIT and multiple chemokines (CCL3, CCL4, XCL2), and a GZMB+ NkT cluster enriched in cytolytic markers (PRF1, GNLY, NKG7) (Fig 1C). Differential expression between NP and FP samples in this CD8+ subset showed enrichment of the NkT cytotoxic markers in NP samples, while FP samples were enriched in the CD8+ Exhausted chemokine markers (Fig 1D). Furthermore, the proportion of CD8+ Exhausted T-Cells was enriched in FP samples (p.val < 0.05) (Fig 1E). Exhaustion markers were measured through both RNA and surface marker levels. In RNA, TIGIT was uniquely associated with the FP-enriched CD8+ Exhausted T-Cell cluster, and CD160 was uniquely expressed in FP samples (Fig 1F). CITE-Seq surface marker expression confirms enrichment of both TIGIT and PD1 in the CD8+ Exhausted T-Cell cluster, and along with more exhaustion in FP samples (p.val < 0.01). Conclusion: In this study, we have identified significant differences in T-Cell activity in patients with non-progressing and rapid-progressing multiple myeloma. T-Cells in rapid progressing patients appear to be in a suppressed state, with low cytolytic activity and enriched exhaustion markers. This GZMK+ T-Cell population shows strong similarities with an aging-associated subtype of effector memory T-Cells found to be enriched in older populations (Mogilenko et al, Immunity 54, 2021). These findings will be further validated in an expanded study, consisting both of a larger number of samples, and multiple samples at different timepoints from the same patient. Figure 1 Figure 1. Abstract: Multiple Myeloma (MM) is the second most common hematologic malignancy, marked by uncontrolled clonal expansion of plasma cells. MM genome is complex and heterogeneous, with a high frequency of structural variants (SVs) and copy-number abnormalities (CNAs). Single-cell sequencing technologies offer advantages over traditional bulk methods in cancer genomics research for evaluating cellular heterogeneity and investigating evolution of cellular subpopulations within the tumor. Thus, there is an impetus to translate the growing understanding of the genomic landscape of MM into dissecting the tumor heterogeneity using scRNA-seq. So far, there are no reported studies systematically comparing tumor and immune populations differences between MM NON-progressors (NPs) and RAPID-progressor (RPs) and investigating how clonal plasma cells contribute to progression in a large cohort. In addition, the bone marrow microenvironment plays an important role in the evolution of premalignant MM and MM progression. A previous study of single-cell transcriptomics analysis of Monoclonal gammopathy of undetermined significance (MGUS) and MM tumor microenvironment (TME) revealed that natural killer (NK) cell abundance is frequently increased in the early stages and associated with altered chemokine receptor expression. This study shed the light on the role of immune cells on disease progression from asymptomatic MGUS to symptomatic MM. However to date, how immune cells influence disease progression within symptomatic MM is still unclear. Here, we subjected 344 CD138-negative Bone Marrow Mononuclear cells (BMMC) samples to scRNA-seq. To control for the first line therapy, 272 patients were treated with RVD and Autologous stem cell transplant (ASCT) and were stratified into 3 groups based on progression: 200 NON-progressors (NPs) (PFS> 5 years), 7 Intermediate Progressors (IPs, PFS 2-4 years) and 38 RAPID-progressors (RPs) (PFS Our preliminary analysis of scRNA-seq of 6 samples treated by the same first-line therapies (RVD but without ASCT) revealed that plasma cells (PCs) from two patients (MMRF2550 and MMRF2562) with chromosome 13q deletion clustered together, whereas PCs from a patient (MMRF2187) with t(8;14) Myc translocation formed a distinct cluster. Interestingly, PCs from a patient (MMRF2271) with both chromosome 13q deletion and t(8;14) formed another independent cluster. These observations highlight the important role of genetic drivers in transcriptome profiles of plasma cells. In addition, we further investigated how progression features correlate with the immune microenvironment and identified differentially expressed genes between NK cells from RPs versus those from NPs, which could potentially serve as MM progression markers. Interestingly, several cytotoxicity genes are significantly upregulated in FPs, such as GZMB and KLRB1 (log Fold Change=1.44, and 8.41 respectively). Overall, our preliminary results provide a small glimpse of the interconnected nature of driver genetic alterations and progression features in MM tumor and TME. This study will provide a sufficiently broad, deep, and diverse vast dataset for accurately characterizing MM at single-cell resolution to help interrogate how genetic alterations and disease progression interplay MM tumor and TME. We hope this study could identify novel candidate targets for therapeutic approaches, and ultimately stratify patients by risk of progression for early intervention in the clinic. Abstract: This book explores the use of restorative justice approaches in the context of environmental crimes. It critically assesses regular criminal justice approaches with regard to green crimes and explores restorative justice conferencing as an alternative. Focussing on justice approaches in Australia and New Zealand, it argues that court processes following environmental offending provide minimal to no offender and victim voice, interaction, and input, rendering them invisible. It proposes a third measure of justice – that of meaningful involvement, beyond that of fair procedure and outcome. It suggests the use of restorative justice conferencing, a facilitated dialogue between stakeholders to crime or conflict, as a vehicle to operationalise and achieve justice as meaningful involvement. This book speaks to those interested in green criminology, victimology and environmental law.

Abstract: To use a locally designed and simple lower-body negative-pressure (LBNP) device and 1.5 T magnetic resonance imaging (MRI) to demonstrate the ability to assess changes in cardiovascular function during preload reduction. These effects were evaluated on ventricular volumes and great vessel flow in healthy volunteers, for which there are limited published data.After ethical review, 14 volunteers (mean age 33.9 ± 7 years, mean body mass index [BMI] 23.1 ± 2.5) underwent LBNP prospectively at 0, -5, -10, and -20 mmHg pressure, using a locally designed LBNP box. Expiratory breath-hold biventricular volumes, and free-breathing flow imaging of the ascending aorta and main pulmonary artery were acquired at each level of LBNP.At -5 mmHg, there was no change in aortic flow or left ventricular volumes versus baseline. Right ventricular output (p=0.013) and pulmonary net flow (p=0.026) decreased. At -20 mmHg, aortic and pulmonary net flow (p0.001) decreased, as were left and right ventricular end diastolic volume (p0.001) and left and right end systolic volumes (p=0.038 and p=0.003 respectively).Use of a MRI-compatible LBNP device is feasible to measure changes in ventricular volume and great arterial flow in the same experiment. This may enhance further research into the effects of preload reduction by MRI in a wide range of important cardiovascular pathologies.

146. Self-Determination Theory and Risk Behavior in a Collectivistic Society: Preventing Reckless Driving in Urban Nepal

Abstract: Traffic road accidents are one of the leading causes of mortality in Nepal and around the world. Drivers in Nepal are not adequately educated about road safety rules. Road conditions are chaotic as traffic regulations are also not strictly enforced. Public safety campaigns may be able to alter drivers' attitudes and behaviors; however, little is known about which persuasive strategies may be most effective. Drawing on self-determination theory and the Health Belief Model, the current study used a post-only experimental design to test the impact of a short video message. The video included collective vs. individual appeals, and messages emphasizing one's ability to make the right choice (autonomy support) vs. directive language. Participants were Nepali college students (mean age 20, N = 199). Using structural equation modeling, the study found that directive messages rather than autonomy support influenced an individual seeing value in the recommended behavior (identified regulation), which in turn influenced perceived susceptibility, perceived severity, and behavioral intention. The study also proposed a behavior change model by incorporating the stage of identification with the message upon exposure. This model aims to expand the model proposed by the Health Belief Model, to include a stage of value identification before cues to action influence perception of threat. Further implications are discussed.

147. The In Vivo Morphology of Post-Infarct Ventricular Septal Defect and the Implications for Closure

Abstract: Objectives The aim of this study was to define the dynamic in vivo morphology of post-infarct ventricular septal defect (PIVSD), which has not been previously described in living patients. Background PIVSD is a devastating complication of acute myocardial infarction. Methods The anatomic features of PIVSD, as demonstrated by computed tomography or magnetic resonance imaging, were retrospectively reviewed. Results Thirty-two PIVSDs were assessed, 16 left coronary artery and 16 right coronary artery PIVSDs. PIVSDs were large (mean maximum dimension 26.5 ± 11.5 mm, mean area 5.2 ± 4.2 cm 2 ) and oval (mean eccentricity index 1.7 ± 0.5), with thin margins (diastolic mean thickness 5 mm from the edge of the PIVSD 6.4 ± 3.0mm), and only 22% of PIVSDs were entirely confined to the septum. The defects could be larger in diastole or systole. The stem of the largest available Amplatzer occluder stem (St. Jude Medical, St. Paul, Minnesota) filled only 50% of defects. Patients with small defects may survive without closure. Without closure, those with large defects die. If accepted for closure, PIVSD size and coronary territory did not predict survival >1 year (overall 60%). Conclusions This is the first detailed anatomic description of PIVSD in living patients. Defects may be larger in systole or diastole, meaning that single-phase measurement is unsuitable. Its complex nature means that the most commonly available occluder device is frequently unsuitable. Successful closure leads to prolonged survival and should be attempted where possible. This study may lead to improved patient selection, closure techniques, and device design.

Abstract: Jonathan Edwards′ New England theology has a great deal more to say that is of contemporary doctrinal interest than it is often credited with, particularly as it relates to the doctrine of atonement. This article explores several anomalous claims made be this 18th and 19th century tradition, and in this way, challenges the recent and growing consensus that Edwards espoused the penal substitution model and his successors a moral government model. I argue that of all that is yet to be considered about their doctrine of atonement, we ought to begin with those claims made about the nature and demands of divine justice.

149. Microbial risk from source-separated urine used as liquid fertilizer in sub-tropical Australia

Abstract: The inactivation rates of Escherichia coli ( E. coli ) and MS2 phage were determined in fresh undiluted, diluted and six months aged stored urine samples at three temperatures ranging from 15 to 35 °C in a subtropical region of Australia. In addition, Quantitative Microbial Risk Assessment (QMRA) calculations were undertaken to estimate the storage times that would be required to reduce the risk of infection by Campylobacter jejuni (using E. coli inactivation data) and rotavirus (using MS2 phage data) to an annual probability of infection of 10 −4 during irrigation and consumption of lettuce. Higher inactivation rates were observed at a higher temperature (35 °C) compared to lower temperatures (15 and 25 °C) for both E. coli and MS2 phage. Stored urine samples also showed higher rates of inactivation for both E. coli and MS2 compared to undiluted and diluted urine samples at all temperatures. QMRA calculations indicated that inactivation of both bacteria and viruses to meet the health target of an annual probability of infection of 10 −4 would take approximately four months at 15 °C, 10 days at 25 °C and five days of storage times at 35 °C. The results also indicated that an increase in temperature has a more substantial effect on reducing storage time than varying the urine dilution for both E. coli and MS2 phage. Combining the QMRA-based approach with pathogen reduction interventions as presented in this study provides a range of management options for regulators, and may reduce barriers to the application of source-separated urine associated with long storage times.

Abstract: Cardiac computed tomography (CCT) has become an invaluable cross-sectional imaging modality in congenital heart disease (CHD) patients. However, altered anatomical connections and cardiovascular physiology makes CHD arguably the most challenging area in CCT imaging, which remains a complimentary modality to cardiac magnetic resonance and echocardiography. A bespoke CT protocol is often required to achieve a diagnostic examination; this can be achieved through careful consideration of the basic principles of image acquisition and contrast administration. This article reviews these principles and demonstrates how they can be applied to CCT in CHD using the Fontan circulation as an example.
