Objective To assess the prevalence and isotypes of anti-nodal/paranodal antibodies to nodal/paranodal proteins in a large chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) cohort, compare clinical features in seronegative vs seropositive patients, and gather evidence of their isotype-specific pathogenic role. Methods Antibodies to neurofascin-155 (Nfasc155), neurofascin-140/186 (Nfasc140/186), contactin-1 (CNTN1), and contactin-associated protein 1 (Caspr1) were detected with ELISA and/or cell-based assay. Antibody pathogenicity was tested by immunohistochemistry on skin biopsy, intraneural injection, and cell aggregation assay. Results Of 342 patients with CIDP, 19 (5.5%) had antibodies against Nfasc155 (n = 9), Nfasc140/186 and Nfasc155 (n = 1), CNTN1 (n = 3), and Caspr1 (n = 6). Antibodies were absent from healthy and disease controls, including neuropathies of different causes, and were mostly detected in patients with European Federation of Neurological Societies/Peripheral Nerve Society (EFNS/PNS) definite CIDP (n = 18). Predominant antibody isotypes were immunoglobulin G (IgG)4 (n = 13), IgG3 (n = 2), IgG1 (n = 2), or undetectable (n = 2). IgG4 antibody-associated phenotypes included onset before 30 years, severe neuropathy, subacute onset, tremor, sensory ataxia, and poor response to intravenous immunoglobulin (IVIG). Immunosuppressive treatments, including rituximab, cyclophosphamide, and methotrexate, proved effective if started early in IVIG-resistant IgG4-seropositive cases. Five patients with an IgG1, IgG3, or undetectable isotype showed clinical features indistinguishable from seronegative patients, including good response to IVIG. IgG4 autoantibodies were associated with morphological changes at paranodes in patients' skin biopsies. We also provided preliminary evidence from a single patient about the pathogenicity of anti-Caspr1 IgG4, showing their ability to penetrate paranodal regions and disrupt the integrity of the Nfasc155/CNTN1/Caspr1 complex. Conclusions Our findings confirm previous data on the tight clinico-serological correlation between antibodies to nodal/paranodal proteins and CIDP. Despite the low prevalence, testing for their presence and isotype could ultimately be part of the diagnostic workup in suspected inflammatory demyelinating neuropathy to improve diagnostic accuracy and guide treatment. Classification of evidence This study provides Class III evidence that antibodies to nodal/paranodal proteins identify patients with CIDP (sensitivity 6%, specificity 100%).

Antisynthetase syndrome (ASSD) is a rare clinical condition that is characterized by the occurrence of a classic clinical triad, encompassing myositis, arthritis, and interstitial lung disease (ILD), along with specific autoantibodies that are addressed to different aminoacyl tRNA synthetases (ARS). Until now, it has been unknown whether the presence of a different ARS might affect the clinical presentation, evolution, and outcome of ASSD. In this study, we retrospectively recorded the time of onset, characteristics, clustering of triad findings, and survival of 828 ASSD patients (593 anti-Jo1, 95 anti-PL7, 84 anti-PL12, 38 anti-EJ, and 18 anti-OJ), referring to AENEAS (American and European NEtwork of Antisynthetase Syndrome) collaborative group's cohort. Comparisons were performed first between all ARS cases and then, in the case of significance, while using anti-Jo1 positive patients as the reference group. The characteristics of triad findings were similar and the onset mainly began with a single triad finding in all groups despite some differences in overall prevalence. The "ex-novo" occurrence of triad findings was only reduced in the anti-PL12-positive cohort, however, it occurred in a clinically relevant percentage of patients (30%). Moreover, survival was not influenced by the underlying anti-aminoacyl tRNA synthetase antibodies' positivity, which confirmed that antisynthetase syndrome is a heterogeneous condition and that antibody specificity only partially influences the clinical presentation and evolution of this condition. ispartof: JOURNAL OF CLINICAL MEDICINE vol:8 issue:11 ispartof: location:Switzerland status: published

Antisynthetase syndrome (ASSD) is a rare clinical condition that is characterized by the occurrence of a classic clinical triad, encompassing myositis, arthritis, and interstitial lung disease (ILD), along with specific autoantibodies that are addressed to different aminoacyl tRNA synthetases (ARS). Until now, it has been unknown whether the presence of a different ARS might affect the clinical presentation, evolution, and outcome of ASSD. In this study, we retrospectively recorded the time of onset, characteristics, clustering of triad findings, and survival of 828 ASSD patients (593 anti-Jo1, 95 anti-PL7, 84 anti-PL12, 38 anti-EJ, and 18 anti-OJ), referring to AENEAS (American and European NEtwork of Antisynthetase Syndrome) collaborative group's cohort. Comparisons were performed first between all ARS cases and then, in the case of significance, while using anti-Jo1 positive patients as the reference group. The characteristics of triad findings were similar and the onset mainly began with a single triad finding in all groups despite some differences in overall prevalence. The "ex-novo" occurrence of triad findings was only reduced in the anti-PL12-positive cohort, however, it occurred in a clinically relevant percentage of patients (30%). Moreover, survival was not influenced by the underlying anti-aminoacyl tRNA synthetase antibodies' positivity, which confirmed that antisynthetase syndrome is a heterogeneous condition and that antibody specificity only partially influences the clinical presentation and evolution of this condition.

Background: In order to provide effective care to patients suffering from chronic pain secondary to neurological diseases, health professionals must appraise the role of the psychosocial factors in the genesis and maintenance of this condition whilst considering how emotions and cognitions influence the course of treatment. Furthermore, it is important not only to recognize the psychological reactions to pain that are common to the various conditions, but also to evaluate how these syndromes differ with regards to the psychological factors that may be involved. As an extensive evaluation of these factors is still lacking, the Italian Consensus Conference on Pain in Neurorehabilitation (ICCPN) aimed to collate the evidence available across these topics. Objectives: To determine the psychological factors which are associated with or predictive of pain secondary to neurological conditions and to assess the influence of these aspects on the outcome of neurorehabilitation. Methods: Two reviews were performed. In the first, a PUBMED search of the studies assessing the association between psychological factors and pain or the predictive value of these aspects with respect to chronic pain was conducted. The included papers were then rated with regards to their methodological quality and recommendations were made accordingly. In the second study, the same methodology was used to collect the available evidence on the predictive role of psychological factors on the therapeutic response to pain treatments in the setting of neurorehabilitation. Results: The first literature search identified 1170 results and the final database included 189 articles. Factors such as depression, anxiety, pain catastrophizing, coping strategies, and cognitive functions were found to be associated with pain across the various conditions. However, there are differences between chronic musculoskeletal pain, migraine, neuropathy, and conditions associated with complex disability with regards to the psychological aspects that are involved. The second PUBMED search yielded 252 studies, which were all evaluated. Anxiety, depression, pain catastrophizing, coping strategies, and pain beliefs were found to be associated to different degrees with the outcomes of multidisciplinary programs, surgery, physical therapies, and psychological interventions. Finally, sense of presence was found to be related to the effectiveness of virtual reality as a distraction tool. Conclusions: Several psychological factors are associated with pain secondary to neurological conditions and should be acknowledged and addressed in order to effectively treat this condition. These factors also predict the therapeutic response to the neurorehabilitative interventions. Background: It is increasingly recognized that treating pain is crucial for effective care within neurological rehabilitation in the setting of the neurological rehabilitation. The Italian Consensus Conference on Pain in Neurorehabilitation was constituted with the purpose identifying best practices for us in this context. Along with drug therapies and physical interventions, psychological treatments have been proven to be some of the most valuable tools that can be used within a multidisciplinary approach for fostering a reduction in pain intensity. However, there is a need to elucidate what forms of psychotherapy could be effectively matched with the specific pathologies that are typically addressed by neurorehabilitation teams. Objectives: To extensively assess the available evidence which supports the use of psychological therapies for pain reduction in neurological diseases. Methods: A systematic review of the studies evaluating the effect of psychotherapies on pain intensity in neurological disorders was performed through an electronic search using PUBMED, EMBASE, and the Cochrane Database of Systematic Reviews. Based on the level of evidence of the included studies, recommendations were outlined separately for the different conditions. Results: The literature search yielded 2352 results and the final database included 400 articles. The overall strength of the recommendations was medium/low. The different forms of psychological interventions, including Cognitive-Behavioral Therapy, cognitive or behavioral techniques, Mindfulness, hypnosis, Acceptance and Commitment Therapy (ACT), Brief Interpersonal Therapy, virtual reality interventions, various forms of biofeedback and mirror therapy were found to be effective for pain reduction in pathologies such as musculoskeletal pain, fibromyalgia, Complex Regional Pain Syndrome, Central Post-Stroke pain, Phantom Limb Pain, pain secondary to Spinal Cord Injury, multiple sclerosis and other debilitating syndromes, diabetic neuropathy, Medically Unexplained Symptoms, migraine and headache. Conclusions: Psychological interventions and psychotherapies are safe and effective treatments that can be used within an integrated approach for patients undergoing neurological rehabilitation for pain. The different interventions can be specifically selected depending on the disease being treated. A table of evidence and recommendations from the Italian Consensus Conference on Pain in Neurorehabilitation is also provided in the final part of the paper. - Abstract
Objective To evaluate if the timing of appearance with respect to disease onset may influence the arthritis presentation pattern in antisynthetase syndrome (ASSD). Methods The patients were selected from a retrospective large international cohort of ASSD patients regularly followed-up in centres referring to AENEAS collaborative group. Patients were eligible if they had an antisynthetase antibody testing positive in at least two determinations along with arthritis occurring either at ASSD onset (Group 1) or during the course of the disease (Group 2). Results 445 (70%; 334 females, 110 males, 1 transsexual) out of the 636 ASSD we collected had arthritis, in the majority of cases (367, 83%) from disease onset (Group 1). Patients belonging to Group 1 with respect to Group 2 had an arthritis more commonly polyarticular and symmetrical (p=0.015), IgM-Rheumatoid factor positive (p=0.035), erosions at hands and feet plain x-rays (p=0.036) and more commonly satisfying the 1987 revised classification criteria for rheumatoid arthritis (RA) (p=0.004). Features such as Raynaud's phenomenon, mechanic's hands and fever (e.g. accompanying findings) were more frequently reported in Group 2 (p=0.005). Conclusion In ASSD, the timing of appearance with respect to disease onset influences arthritis characteristics. In particular, RA features are more common when arthritis occurs from ASSD onset, suggesting an overlap between RA and ASSD in these patients. When arthritis appears during the follow-up, it is very close to a connective tissue disease-related arthritis. Also, the different prevalence of accompanying features between these two groups is in line with this possibility

- Abstract
Here we report biallelic mutations in the sorbitol dehydrogenase gene (SORD) as the most frequent recessive form of hereditary neuropathy. We identified 45 individuals from 38 families across multiple ancestries carrying the nonsense c.757delG (p.Ala253GlnfsTer27) variant in SORD, in either a homozygous or compound heterozygous state. SORD is an enzyme that converts sorbitol into fructose in the two-step polyol pathway previously implicated in diabetic neuropathy. In patient-derived fibroblasts, we found a complete loss of SORD protein and increased intracellular sorbitol. Furthermore, the serum fasting sorbitol levels in patients were dramatically increased. In Drosophila, loss of SORDorthologs caused synaptic degeneration and progressive motor impairment. Reducing the polyol influx by treatment with aldose reductase inhibitors normalized intracellular sorbitol levels in patient-derived fibroblasts and in Drosophila, and also dramatically ameliorated motor and eye phenotypes. Together, these findings establish a novel and potentially treatable cause of neuropathy and may contribute to a better understanding of the pathophysiology of diabetes.

Background: Transverse myelitis (TM) is an inflammatory disorder that can be idiopathic or associated with central nervous system autoimmune/dysimmune inflammatory diseases, connective tissue autoimmune diseases, or post-infectious neurological syndromes. Prognosis of initial TM presentations is uncertain. Objective: To identify outcome predictors in TM. Methods: Retrospective study on isolated TM at onset. Scores ⩾3 on the modified Rankin scale (mRS) marked high disability. Results: A total of 159 patients were identified. TM was classified as follows: idiopathic (I-TM, n = 53), post-infectious (PI-TM, n = 48), associated with multiple sclerosis (MS-TM, n = 51), or neuromyelitis optica spectrum disorders/connective tissue autoimmune diseases/neurosarcoidosis (n = 7). At follow-up (median, 55 months; interquartile range, 32–80), 42 patients were severely disabled, and patients with I-TM or PI-TM showed the worst outcomes. Predictors of disability were infectious antecedents, sphincter and pyramidal symptoms, high mRS scores, blood–cerebrospinal fluid barrier damage, lumbar magnetic resonance imaging (MRI) lesions on univariate analysis, and older age (odds ratio (OR), 1.1; 95% confidence interval (CI), 1.0–1.1), overt/subclinical involvement of the peripheral nervous system (PNS) (OR, 9.4; 95% CI, 2.2–41.0), complete TM (OR, 10.8; 95% CI, 3.4–34.5) on multivariate analysis. Conclusion: Our findings help define prognosis and therapies in TM at onset. Infectious antecedents and PNS involvement associate with severe prognosis. Nerve conduction studies and lumbar MRI could improve the prognostic assessment of this condition. [ABSTRACT FROM AUTHOR]

- Abstract
Nella clinica dei gliomi cerebrali un passo decisivo è la definizione del corretto grading. La possibilità di stabilire con precisione il grado di malignità di una lesione possiede notevoli implicazioni cliniche, in termini di outcome e atteggiamento terapeutico da assumere. La valutazione istopatologica della lesione è resa spesso difficoltosa dall’inaccessibilità chirurgica della stessa o da errori di campionamento, e lo studio di imaging attraverso la Risonanza Magnetica fornisce importanti informazioni di tipo morfologico, ma indicative di malignità soltanto in modo presuntivo. Negli ultimi anni si è assistito così ad un notevole sviluppo delle tecniche avanzate di RM, che permettono di completare le informazioni di tipo anatomico, con preziose indicazioni sulla funzionalità delle lesioni indagate. La perfusione rappresenta la misura diretta della microvascolarizzazione di un tessuto e può essere utilizzata come marker di imaging per valutare l’angiogenesi tumorale e di conseguenza il grado di malignità. Con il nostro lavoro si è voluto valutare il ruolo della perfusione RM nello stabilire il corretto grading dei gliomi cerebrali: sono state analizzate le mappe di perfusione di 22 pazienti affetti da gliomi di alto e basso grado, con o senza conferma istologica, e sono stati poi confrontati tra loro i gradi tumorale II, III, IV (sicuri o presunti) e i gradi semplificati (High Grade Gliomas e Low Grade Gliomas) attraverso i principali parametri di perfusione: CBV (Cerbral Blood Volume) e CBF (Cerebral Blood Flow). È stata trovata una correlazione positiva tra i due parametri e tra i parametri e il grado tumorale e ciò significa che all’aumentare del grado di malignità aumenta anche la perfusione. Questo concorda con il dato istologico che indica come lesioni a più alta malignità le lesioni a più alta vascolarizzazione. Mentre è stato possibile distinguere attraverso i parametri di perfusione i gliomi di alto grado dai gliomi di basso grado, la distinzione tra II/III e III/IV grado non è stata possibile, a causa della scarsa numerosità del campione di pazienti affetti da gliomi di grado III. Lo studio di perfusione RM sembra rappresentare dunque un utile strumento per la diagnosi differenziale tra HGG e LGG, con importanti implicazioni anche nel follow-up. We introduce a new class of games where each player's aim is to randomise her strategic choices in order to affect the other players' expectations aside from her own. The way each player intends to exert this influence is expressed through a Boolean combination of polynomial equalities and inequalities with rational coefficients. In this paper, we investigate the definability of classes of t-norms and uninorms in the logic 1/2. In particular we provide a complete characterization of definable continuous t-norms, weak nilpotent minimum t-norms, conjunctive uninorms continuous on [0, 1), and idempotent conjunctive uninorms, and give both positive and negative results concerning definability of left-continuous t-norms (and uninorms). We show that the class of definable uninorms is closed under construction methods as annihilation, rotation and rotation-annihilation. Moreover, we prove that every logic based on a definable uninorm is in PSPACE, and that any finitely axiomatizable logic based on a class of definable uninorms is decidable. Finally we show that the Uninorm Mingle Logic (UML) and the Basic Uninorm Logic (BUL) are finitely strongly standard complete w.r.t. the related class of definable left-continuous conjunctive uninorms.

Upper probability measures are measures of uncertainty that generalize probability measures in order to deal with non-measurable events. Following an approach that goes back to previous works by H ajek, Esteva, and Godo, we show how to expand Rational Lukasiewicz Logic by modal operators in order to reason about upper probabilities of classical Boolean events ' so that ( ' ) can be read as \the upper probability of ' ". We build the logic U (R L) for representing upper probabilities and show it to be complete w.r.t. a class of Kripke structures equipped with an upper probability measure. Finally, we prove that the set of U (R L)-satis able formulas is NP-complete.

We introduce the logics E(G) for reasoning about probabilistic expectation over classes G of games with discrete polynomial payoff functions represented by finite-valued Lukasiewicz formulas and provide completeness and complexity results. In addition, we introduce a new class of games where players' expected payoff functions are encoded by E(G)-formulas. In these games each player's aim is to randomise her strategic choices in order to affect the other players' expectations over an outcome as well as their own. We offer a logical and computational characterisation of this new class of games.

In this paper, we study generalized possibility and necessity measures on MV-algebras of [0, 1]-valued functions (MV-clans) in the framework of idempotent mathematics, where the usual field of reals ℝ is replaced by the max-plus semiring ℝ max We prove results about extendability of partial assessments to possibility and necessity measures, and characterize the geometrical properties of the space of homogeneous possibility measures. The aim of the present paper is also to support the idea that idempotent mathematics is the natural framework to develop the theory of possibility and necessity measures, in the same way classical mathematics serves as a natural setting for probability theory. © 2012 Springer-Verlag.

The Craig interpolation property is investigated for substructural logics whose algebraic semantics are varieties of semilinear (subdirect products of linearly ordered) pointed commutative residuated lattices. It is shown that Craig interpolation fails for certain classes of these logics with weakening if the corresponding algebras are not idempotent. A complete characterization is then given of axiomatic extensions of the >R-mingle with unit> logic (corresponding to varieties of Sugihara monoids) that have the Craig interpolation property. This latter characterization is obtained using a model-theoretic quantifier elimination strategy to determine the varieties of Sugihara monoids admitting the amalgamation property. © 2012 WILEY-VCH Verlag GmbH & Co. This work presents a model-theoretic approach to the study of the amalgamation property for varieties of semilinear commutative residuated lattices. It is well-known that if a first-order theory T enjoys quantifier elimination in some language L, the class of models of the set of its universal consequences T ∀ has the amalgamation property. Let Th(K) be the theory of an elementary subclass K of the linearly ordered members of a variety V of semilinear commutative residuated lattices. We show that whenever Th(K) has elimination of quantifiers, and every linearly ordered structure in V is a model of Th ∀(K), then V has the amalgamation property. We exploit this fact to provide a purely model-theoretic proof of amalgamation for particular varieties of semilinear commutative residuated lattices. © 2011 Springer-Verlag.

Possibility and necessity measures are commonly defined over Boolean algebras. This work considers a generalization of these kinds of measures over MV-algebras as a possibilistic counterpart of the (probabilistic) notion of state on MV-algebras. Two classes of possibilistic states over MV-algebras of functions are characterized in terms of (generalized) Sugeno integrals. For reasoning about these representable classes of possibilistic states, we introduce many-valued modal logics based on the Rational Łukasiewicz Logic, that are shown to be complete with respect to corresponding classes of Kripke models equipped with those states. © The Author, 2009. In this paper we present several fuzzy logics trying to capture different notions of necessity (in the sense of possibility theory) for Gödel logic formulas. Based on different characterizations of necessity measures on fuzzy sets, a group of logics with Kripke style semantics is built over a restricted language, namely, a two-level language composed of non-modal and modal formulas, the latter, moreover, not allowing for nested applications of the modal operator N. This work presents a model-theoretic approach to the study of firstorder theories of classes of BL-chains. Among other facts, we present several classes of BL-algebras, generating the whole variety of BL-algebras whose firstorder theory has quantifier elimination. Model-completeness and decision problems are also investigated. Then we investigate classes of BL-algebras having (or not having) the amalgamation property or the joint embedding property and we relate the above properties to the existence of ultrahomogeneous models. © 2011 by University of Notre Dame.
