Abstract

Aims: Early transcatheter aortic valve implantation (TAVI) devices have established the feasibility of TAVI in patients who are unsuitable for surgical valve replacement, but these devices may face challenges with precise valve positioning, paravalvular leakage, and undesirable rates of stroke, bleeding, or vascular complications. The repositionable and fully retrievable LotusTM Valve was designed to facilitate accurate positioning and minimise paravalvular leakage in patients with severe aortic stenosis who are at high or extreme risk for surgery. The device demonstrated favourable efficacy and safety primary outcomes at 30 days in the REPRISE II CE-Mark trial. This analysis will present the first report of 6-month outcomes in the full cohort of patients in REPRISE II. Methods and Results: The prospective, single-arm, multicentre REPRISE II study was designed to assess the safety and performance of the Lotus Valve System in symptomatic patients aged >=70 years with severe calcific aortic stenosis who are at high or extreme surgical risk. The primary device performance endpoint was the mean aortic valve pressure gradient at 30 days post-procedure, as assessed by an independent core laboratory. The primary safety endpoint was 30-day all-cause mortality. Among 120 enrolled patients, the mean age of 84.4+/-5.3 years, 57% (68/120) were female, mean STS Score was 7.1+/-4.6, mean euroSCORE II was 6.9+/-5.8, and 26% had medically treated diabetes. The mean baseline aortic valve area was 0.7+/-0.2 cm, and the mean aortic valve pressure gradient was 46.4+/-15.0 mmHg. All patients (100%) were successfully implanted with a Lotus valve. A total of 28.6% (34/119) patients had a permanent pacemaker implanted within 30 days post-procedure. The primary device performance endpoint of 30-day mean aortic valve pressure gradient was 11.5+/-5.2 mmHg, which was significantly less than the performance goal of 18 mmHg (P<0.001). Abstract

Background: Transcatheter aortic valve replacement (TAVR) has emerged as an alternative for the treatment of inoperable or surgical high-risk patients with severe aortic stenosis (AS). Early TAVR devices have been limited by challenges with precise valve positioning, paravalvular leakage, stroke, bleeding, and vascular complications. The repositionable and fully retrievable Lotus Valve was designed to facilitate accurate positioning and minimize paravalvular leakage in these patients. The initial safety and performance of the valve were demonstrated in the REPRISE II CE-mark trial at the primary endpoint of 30 days, but longer-term follow-up with the full 120-patient cohort has not been reported. Cryptococcal meningitis is an emerging opportunistic infection among HIV infected patients and an important cause of mortality among these patients. The incidence of cryptococcal meningitis varies from place to place. A total of 31 specimens of CSF out of 89 samples processed from known HIV positive cases yielded Cryptococcus neoformans during the period of 3 years. C.neoformans was the most common opportunistic pathogen isolated from CSF samples of these patients with an incidence of 34.8%

Abstract

Florid osseous dysplasia a rare benign fibro-osseous multiquadrant dysplastic lesion confined to the alveolar process of jaws, generally asymptomatic and usually detected incidentally during radiologic examination and requires no treatment unless symptomatic or cosmetically concerning. In this article, we present two rare cases of florid osseous dysplasia in Indian women with their clinical, radiographic and histologic findings and a brief review of literature is discussed. The first case was asymptomatic and the lesion was detected during routine radiographic examination and required no treatment whereas, the second case presented with features of osteomyelitis. Based on the clinical findings, the case was diagnosed as chronic suppurative osteomyelitis, but, after radiologic examination, diagnosis of florid osseous dysplasia with secondary osteomyelitis was made. Besides diagnostic challenge, management of the lesion is difficult due to poor vascularity. In our case, antibiotic prophylaxis along with surgical debridement and sequestrectomy was done with regular recall visits. [ABSTRACT FROM AUTHOR]

Abstract

BACKGROUND: New-generation coronary stents that release zotarolimus or everolimus have been shown to reduce the risk of restenosis. However, it is unclear whether there are differences in efficacy and safety between the two types of stents on the basis of prospectively adjudicated end points endorsed by the Food and Drug Administration. METHODS: In this multicenter, noninferiority trial with minimal exclusion criteria, we randomly assigned 2292 patients to undergo treatment with coronary stents releasing either zotarolimus or everolimus. Twenty percent of patients were randomly selected for repeat angiography at 13 months. The primary end point was target-lesion failure, defined as a composite of death from cardiac causes, any myocardial infarction (not clearly attributable to a nontarget vessel), or clinically indicated target-lesion revascularization within 12 months. The secondary angiographic end point was the extent of in-stent stenosis at 13 months. RESULTS: At least one off-label criterion for stent placement was present in 66% of patients. The zotarolimus-eluting stent was noninferior to the everolimus-eluting stent with respect to the primary end point, which occurred in 8.2% and 8.3% of patients, respectively (P<0.001 for noninferiority). There were no significant between-group differences in the rate of death from cardiac causes, any myocardial infarction, or revascularization. The rate of stent thrombosis was 2.3% in the zotarolimus-stent group and 1.5% in the everolimus-stent group (P = 0.17). The zotarolimus-eluting stent was also noninferior regarding the degree (±SD) of in-stent stenosis (21.65±14.42% for zotarolimus vs. 19.76±14.64% for everolimus, P = 0.04 for noninferiority). In-stent late lumen loss was 0.27±0.43 mm in the zotarolimus-stent group versus 0.19±0.40 mm in the everolimusstent group (P = 0.08). Abstract

Background: Acute pancreatitis refers to an acute inflammatory process of the pancreas, usually accompanied by abdominal pain and elevations of serum pancreatic enzymes. This syndrome is usually a discrete episode, which may cause varying degrees of injury to the pancreas, and adjacent and distant organs. Acute pancreatitis is a serious disease with high morbidity and mortality rates some 80% were mid attack which recovers rapidly with conservative management. The rest of 20% were severe, with protracted course that needs intensive care and specialized management. Materials and methods: It was a prospective study. First 50 patients attending the surgical emergency ward with clinical features of Acute Pancreatitis were evaluated clinically and subjected to laboratory and radiological investigations as per the designed Performa. Data pertinent to the scoring systems were recorded within 24 hours of admission to the hospital. For each of 50 patients included in the study, BISAP and MCTSI scores were calculated by the manner described by Knaus, et al. and Cardinal Health Database system. Results: BISAP and MCTSI was correlated well for mortality with high positive value of 0.904 which was highly significant (0.01). The ROC analysis for Mortality showed BISAP score had AUC of 0.904, P value (0.001) which was more than MCTSI score which had AUC of 0.845, P value (0.007). So BISAP was highly accurate with P value (0.001) and confidence interval of 0.873. BISAP score was highly sensitive (100%), specificity (60%) at score more than 3.5. MCTSI score sensitivity was 85%, specificity was 77% at score more than 7. Conclusions: BISAP score was found to have more sensitivity, specificity and Diagnostic accuracy than MCTSI score in prediction of assessing the severity of acute pancreatitis. Hence, BISAP score found to predict more number of patients and likelihood of progressing to severe disease. Larven, et al. stated the same in their study 42. Abstract

Throughout the Northern Trust, two different thrombolytic agents, either reteplase or tenecteplase, are used as part of the treatment of acute ST elevation myocardial infarction. Having found no other comparative studies, this retrospective study was designed to compare the efficacy of the two drugs using rate of follow-on emergency angioplasty as the primary outcome. The study retrospectively recruited 40 patients who had received reteplase and 40 who had received tenecteplase. Of the patients who received reteplase, 5 required emergency angiography. Of those who received tenecteplase, 15 required further intervention. This was a significant difference with a ratio of 37.5%:12.5% (p=0.01; significance was assumed to be p, Sotos syndrome is a relatively common overgrowth disorder, following autosomal dominant inheritance, caused by mutations and deletions in the nuclear receptor Set domain containing protein-1, NSD1 gene. Affected individuals generally have advanced bone age, macrocephaly, characteristic facial gestalt and learning difficulties. Other features include scoliosis, seizures, cardiac defects and genitourinary anomalies. Tumours are a rare occurrence. Genotype-phenotype correlations are unclear, though those with a deletion appear to have more severe mental retardation. Full penetrance is seen, although familial Sotos syndrome is extremely rare. The low vertical transmission rate, which is not fully explained by cognitive impairment, is of great importance, particularly for mildly affected patients. Here we report a 3-generation pedigree with 7 affected individuals shown to harbour the NSD1 missense mutation c. 6115C>T. To our knowledge this is the largest Sotos family to be reported. The phenotype is extremely variable, thus highlighting the clinical heterogeneity that may occur. Detailed study of individuals with NSD1 gene abnormalities will be invaluable for further clarification of the phenotype and may lead to NSD1 gene analysis having prognostic value. Long-term follow up of these rare cases of familial Sotos syndrome should make an important contribution to the clarification of these uncertainties.

142. Use of Support Vector Machines in Predicting Success of Intracardiac Cardioversion by Electric Shocks in Patients with Atrial Fibrillation.

Abstract

The objective of this study, was to build support vector machines (SVM) for predicting success of electric shocks in the internal cardioversion (IC) of patients with atrial fibrillation (AF). Some investigations have found correlations between parameters and necessary energy for defibrillating AF, but no tool exist for predicting whether an electric shock will be successful or not in low energy IC. Thirty eight patients with AF, for elective DC cardioversion at the Royal Victoria Hospital in Belfast, were included in our study. Two catheters were positioned in the right atrial appendage (RAA) and the coronary sinus (CS), to deliver a biphasic shock waveform, synchronized with the R wave of the electrocardiogram (ECG) signal. A voltage step-up protocol (50-300 V) was used for patient cardioversion. The ECG was analyzed for an average time interval of 52,8±10.1 s (corresponding to segments before each shock). Residual atrial fibrillatory signal (RAFS) was estimated by means of bandpass filtering and ventricular activity (QRST) cancellation. QRST complexes were cancelled using a recursive least squared (RLS) adaptive filter. The atrial fibrillatory frequency (AFF) and the instantaneous frequency (IF) series were extracted from the RAFS. AFF was calculated from whole segments and from the 10 seconds of the RAFS previous shocks. The mean, standard deviation and approximate entropy of the IF time series were computed. RR intervals of the ECG segments were also analyzed. A total of 26 patients were successfully cardioverted, employing 167 shocks (141 non successful). SVMs were built for classifying success on shocks for energy up to 2, 3 and 6 Joules, employing different combinations of the computed parameters. A maximal exactitude of 93.42% (sensitivity=92.31% and specificity= 93.65%) was obtained classifying shocks below 2 Joules, 95.51% (sensitivity=92.86% and specificity=96%) for shocks up to 3 Joules, and 92.91% (sensitivity=78.95% and specificity= 95.37%) for shocks ≤6 Joules. Abstract

Acute myocardial infarctions were generally believed to result from plaque rupture and thrombosis at the site of a "mild to moderate" coronary stenosis. To assess the severity of coronary stenoses that predisposed to acute coronary syndrome, the 317 patients prospectively included were (1) 102 patients with acute ST-elevation myocardial infarction (STEMI) referred for primary percutaneous coronary intervention (PCI), (2) 135 patients with non-STEMI or unstable angina pectoris (UAP) referred for semiurgent PCI, and (3) 80 patients with stable angina pectoris (SAP) admitted for elective PCI. Patients with STEMI were included if thrombus aspiration could restore normal antegrade coronary blood flow. After aspiration (but before PCI), a high-quality angiogram was obtained and the reference diameter, minimal luminal diameter, and percentage of diameter stenosis of the culprit lesion were quantified. In patients with non-STEMI/UAP and SAP, aspiration was not performed. Average diameter of stenosis was similar in patients with STEMI and those with SAP (66 +/- 12% vs 65 +/- 10%, respectively; p = NS), but was slightly larger in patients with non-STEMI/UAP (71 +/- 12%; p <0.05 vs both STEMI and SAP). In patients with STEMI, only 11% of culprit stenoses were found to have diameter stenosis <50% after removal of the thrombus. In conclusion, most STEMIs occurred at the site of severe coronary stenosis. Diameter stenosis severity was <50% in a minority of cases. [ABSTRACT FROM AUTHOR]
