Selective field ionization is used to determine the state or distribution of states to which a Rydberg atom is excited. By evolving a small perturbation to the ramped electric field using a genetic algorithm, the shape of the time-resolved ionization signal can be controlled. This allows for separation of signals from pairs of states that would be indistinguishable with unperturbed selective field ionization. Measurements and calculations are presented that demonstrate this technique and shed light on how the perturbation directs the pathway of the electron to ionization. Pseudocode for the genetic algorithm is provided. Using the improved resolution afforded by this technique, quantitative measurements of the $36p_{3/2}+36p_{3/2}\rightarrow 36s_{1/2}+37s_{1/2}$ dipole-dipole interaction are made.

Caves, though seemingly inhospitable due to the lack of light in areas furthest from the cave entrance and the low availability and variety of resources, can harbor many species of vertebrates and invertebrates, including phlebotomine sand flies, the vectors of species of parasites of the genus Leishmania that cause leishmaniasis. This study aimed to evaluate the species composition of sand flies in located caves at Moeda Sul (MS) and Parque Estadual Serra do Rola Moça (PESRM), in southeastern Brazil at region of the Quadrilátero Ferrífero, state of Minas Gerais. The collections of sand flies were performed with automatic light traps. Were used the Non-Metric Multidimensional Scaling based on dissimilarity matrix calculated with the Jaccard índex and Multivariate Analysis of Permutation to evaluate similarity of the species composition of sand flies among entrance, interior, and surroundings environments of each sampled cave at MS and PESRM, and thus to infer biological mechanisms from the patterns of distribution of sand flies among these different cave environments. A total of 375 phlebotomine sand flies were collected from six genera and 14 species. The most abundant species were Evandromyia tupynambai (54.7%), Brumptomyia troglodytes (25.6%), Evandromyia edwardsi, (6.1%) Psathyromyia brasiliensis (4.8%) and Lutzomyia longipalpis (4.3%). In place MS, 30 individuals were collected, 16 inside the MS cave and 14 its surroundings. In place PESRM, five individuals were collected in surroundings of RM38 cave; a total of 190 individuals at RM39 cave (48 in the cave and 142 its surroundings) and 150 individuals at RM40 cave, 42 in the cave and 108 its surroundings. Our results showed rich fauna of sand flies with a species composition of sand flies similar among the entrance, interior, and surroundings environments of each sampled cave, suggesting that both caves and their surroundings are important for the maintenance of sand fly communities.

- Abstract
In recent years, the concept of Agriculture 4.0 has emerged as an evolution of precision agriculture (PA) through the diffusion of the Internet of things (IoT). There is a perception that the PA adoption is occurring at a slower pace than expected. Little research has been carried out about Agriculture 4.0, as well as to farmer behavior and operations management. This work explores what drives the adoption of PA in the Agriculture 4.0 context, focusing on farmer behavior and operations management. As a result of a multimethod approach, the factors explaining the PA adoption in the Agriculture 4.0 context and a model of irrigation operations management are proposed. Six simulation scenarios are performed to study the relationships among the factors involved in irrigation planning. Empirical findings contribute to a better understanding of what Agriculture 4.0 is and to expand the possibilities of IoT in the PA domain. This work also contributes to the discussion on Agriculture 4.0, thanks to multidisciplinary research bringing together the different perspectives of PA, IoT and operations management. Moreover, this research highlights the key role of IoT, considering the farmer’s possible choice to adopt several IoT sensing technologies for data collection.

Importance The effects of recruitment maneuvers and positive end-expiratory pressure (PEEP) titration on clinical outcomes in patients with acute respiratory distress syndrome (ARDS) remain uncertain. Objective To determine if lung recruitment associated with PEEP titration according to the best respiratory-system compliance decreases 28-day mortality of patients with moderate to severe ARDS compared with a conventional low-PEEP strategy. Design, Setting, and Participants Multicenter, randomized trial conducted at 120 intensive care units (ICUs) from 9 countries from November 17, 2011, through April 25, 2017, enrolling adults with moderate to severe ARDS. Interventions An experimental strategy with a lung recruitment maneuver and PEEP titration according to the best respiratory–system compliance (n = 501; experimental group) or a control strategy of low PEEP (n = 509). All patients received volume-assist control mode until weaning. Main Outcomes and Measures The primary outcome was all-cause mortality until 28 days. Secondary outcomes were length of ICU and hospital stay; ventilator-free days through day 28; pneumothorax requiring drainage within 7 days; barotrauma within 7 days; and ICU, in-hospital, and 6-month mortality. Results A total of 1010 patients (37.5% female; mean [SD] age, 50.9 [17.4] years) were enrolled and followed up. At 28 days, 277 of 501 patients (55.3%) in the experimental group and 251 of 509 patients (49.3%) in the control group had died (hazard ratio [HR], 1.20; 95% CI, 1.01 to 1.42; P = .041). Compared with the control group, the experimental group strategy increased 6-month mortality (65.3% vs 59.9%; HR, 1.18; 95% CI, 1.01 to 1.38; P = .04), decreased the number of mean ventilator-free days (5.3 vs 6.4; difference, −1.1; 95% CI, −2.1 to −0.1; P = .03), increased the risk of pneumothorax requiring drainage (3.2% vs 1.2%; difference, 2.0%; 95% CI, 0.0% to 4.0%; P = .03), and the risk of barotrauma (5.6% vs 1.6%; difference, 4.0%; 95% CI, 1.5% to 6.5%; P = .001). There were no significant differences in the length of ICU stay, length of hospital stay, ICU mortality, and in-hospital mortality. Conclusions and Relevance In patients with moderate to severe ARDS, a strategy with lung recruitment and titrated PEEP compared with low PEEP increased 28-day all-cause mortality. These findings do not support the routine use of lung recruitment maneuver and PEEP titration in these patients. Trial Registration clinicaltrials.gov Identifier:NCT01374022

Background: COVID-19 is primarily known as a respiratory illness; however, many patients present to hospital without respiratory symptoms. The association between non-respiratory presentations of COVID-19 and outcomes remains unclear. We investigated risk factors and clinical outcomes in patients with no respiratory symptoms (NRS) and respiratory symptoms (RS) at hospital admission. Methods: This study describes clinical features, physiological parameters, and outcomes of hospitalised COVID-19 patients, stratified by the presence or absence of respiratory symptoms at hospital admission. RS patients had one or more of: cough, shortness of breath, sore throat, runny nose or wheezing; while NRS patients did not. Results: Of 178,640 patients in the study, 86.4 % presented with RS, while 13.6 % had NRS. NRS patients were older (median age: NRS: 74 vs RS: 65) and less likely to be admitted to the ICU (NRS: 36.7 % vs RS: 37.5 %). NRS patients had a higher crude in-hospital case-fatality ratio (NRS 41.1 % vs. RS 32.0 %), but a lower risk of death after adjusting for confounders (HR 0.88 [0.83–0.93]). Conclusion: Approximately one in seven COVID-19 patients presented at hospital admission without respiratory symptoms. These patients were older, had lower ICU admission rates, and had a lower risk of in-hospital mortality after adjusting for confounders.

- Abstract
The first Team Haemophilia Education (THE) Meeting was held on 7–8 May 2015 in Amsterdam, The Netherlands. It aimed to promote the optimal care of patients with haemophilia through education of the multidisciplinary treatment team. This was achieved by reviewing the latest developments in haemophilia management, considering how these can be implemented in the clinic to improve patient care and providing a platform for networking and debate for all haemophilia treatment team members. The second THE Meeting was held on 19–20 May in Frankfurt, Germany, and participants included doctors, nurses, physiotherapists, patient representatives and data management staff from 20 different countries. Topics covered the role of the multidisciplinary team in delivering the best haemophilia care, challenges in the management of haemophilia across Europe, available clotting factor treatments, future treatments and the use of genetics in advising carriers of haemophilia. This report is a summary of the key developments in haemophilia care presented by various investigators and healthcare professionals at THE Meeting 2016.

- Abstract
IMPORTANCE: The effects of recruitment maneuvers and positive end-expiratory pressure (PEEP) titration on clinical outcomes in patients with acute respiratory distress syndrome (ARDS) remain uncertain. OBJECTIVE: To determine if lung recruitment associated with PEEP titration according to the best respiratory-system compliance decreases 28-day mortality of patients with moderate to severe ARDS compared with a conventional low-PEEP strategy. DESIGN, SETTING, AND PARTICIPANTS: Multicenter, randomized trial conducted at 120 intensive care units (ICUs) from 9 countries from November 17, 2011, through April 25, 2017, enrolling adults with moderate to severe ARDS. INTERVENTIONS: An experimental strategy with a lung recruitment maneuver and PEEP titration according to the best respiratory-system compliance (n = 501; experimental group) or a control strategy of low PEEP (n = 509). All patients received volume-assist control mode until weaning. MAIN OUTCOMES AND MEASURES: The primary outcome was all-cause mortality until 28 days. Secondary outcomes were length of ICU and hospital stay; ventilator-free days through day 28; pneumothorax requiring drainage within 7 days; barotrauma within 7 days; and ICU, in-hospital, and 6-month mortality. RESULTS: A total of 1010 patients (37.5% female; mean [SD] age, 50.9 [17.4] years) were enrolled and followed up. At 28 days, 277 of 501 patients (55.3%) in the experimental group and 251 of 509 patients (49.3%) in the control group had died (hazard ratio [HR], 1.20; 95% CI, 1.01 to 1.42; P = .041). Compared with the control group, the experimental group strategy increased 6-month mortality (65.3% vs 59.9%; HR, 1.18; 95% CI, 1.01 to 1.38; P = .04), decreased the number of mean ventilator-free days (5.3 vs 6.4; difference, −1.1; 95% CI, −2.1 to −0.1; P = .03), increased the risk of pneumothorax requiring drainage (3.2% vs 1.2%; difference, 2.0%; 95% CI, 0.0% to 4.0%; P = .03), and the risk of barotrauma (5.6% vs 1.6%; difference, 4.0%; 95% CI

IMPORTANCE: The receipt of surgery in freestanding ambulatory surgery centers (ASCs) is often less costly compared with surgery in hospital-based outpatient departments. Although increasing numbers of surgical procedures are now being performed in freestanding ASCs, questions remain regarding the existence of disparities among patients receiving care at ASCs. OBJECTIVE: To examine the association of patient race, health insurance status, and household income with the location (ASC vs hospital-based outpatient department) of ambulatory surgery. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used data from the State Ambulatory Surgery and Services Databases of the Healthcare Cost and Utilization Project to perform a secondary analysis of patients who received ambulatory surgery in New York and Florida between 2011 and 2013. Patients aged 18 to 89 years who underwent 12 different types of ambulatory surgical procedures were included. Data were analyzed from December 2018 to June 2019. MAIN OUTCOMES AND MEASURES: Receipt of surgery at a freestanding ASC and 30-day unplanned hospital visits after ambulatory surgery. RESULTS: A total of 5.6 million patients in New York (57.4% female; 68.9% aged ≥50 years; and 62.5% White) and 7.5 million patients in Florida (57.3% female; 77.4% aged ≥50 years; 74.3% White) who received ambulatory surgery were included in the analysis. After adjusting for age, comorbidities, health insurance status, household income, location of surgery, and type of surgical procedure, the likelihood of receiving ambulatory surgery at a freestanding ASC was significantly lower among Black patients (adjusted odds ratio [aOR], 0.82; 95% CI, 0.81-0.83; P

Glioblastoma is the most malignant brain tumor of the central nervous system, having a high rate of proliferation and invasive growth, which limits its treatment. Cyanobacteria are photoautotrophic prokaryote organisms, which are recognized as being large producers of secondary metabolites with unique structural characteristics, many of them with biological activities known with the antibacterial, anti-inflammatory and antitumor. To evaluate the possible antitumoral effect of crude extracts from different species of cyanobacteria collected in different hypersaline environment against human glioblastoma cells. The collection of cyanobacteria of the genus Oscilatorium sp., Phormidium sp., Lyngbya sp. and Aphanothece sp. was made in the Lagoon of Araruama and salines. The collected material was lyophilized and extracted 3 times by static maceration in Ethyl Acetate:Methanol (1:1) over a period of 2h. MTT cell viability assay was performed with two human glioblastoma cell lines U251 and T98G. The cells were incubated with the crudes extracts at concentrations of 62,5μg/mL, 125μg/mL, 250μg/mL, 500μg/mL and 1000μg/mL maintained for 72h in 5% CO2 at 37°C. The control condition consisted of cells cultured with the extract dilution vehicle (0.5% DMSO). The percent viability calculation was performed from the ratio: [(A570nm of treated cells/A570nm of untreated cells)x100]. Thin layer chromatography was performed on flexible aluminum plates having silica gel as adsorbent material; two different elution systems were used (Hex:CH2Cl2 1.5:8.5) and (AcOEt 100%). Each of the plates was revealed by the Dragendorff, KMnO4, FeCl3, EtOH 5%H2SO4 and Ninhyndrin reagents. In order of response in lineage U251 the crude extracts Oscilatorium sp. demonstrated IC50=51,91μg/ml, followed by Phormidium sp. with IC50=197,5μg/ml and the Lyngbya sp. with IC50=299,2μg/ml. In the T98G cell line the crude extracts Oscilatorium sp. demonstrated IC50=124.9μg/mL, followed by Phormidium sp. with IC50=261,8μg/ml. The crude extract of Aphanothece sp. did not present cytotoxic activity in any of the studied cell lines. The analysis of the thin layer chromatography allowed the visualization of the presence different classes of secondary metabolites of cyanobacteria Article extracts, showing a great chemical diversity. The results suggest that the secondary metabolites of cyanobacteria Oscilatorium sp., Phormidium sp., And Lyngbya sp. of hypersaline systems can present bioactive compounds with antitumor effect in vitro and that these extracts present a diversity of chemical substances.

Mental nerve neuropathy is a peripheral sensory neuropathy, characterized by acute numbness of the chin area. It is a rare entity with diverse aetiology including, among others, local odontogenic causes and malignancy. In rare cases, it might be associated with sickle cell disease, due to the combined presence of hyperviscosity and the sinuous course of the mental nerve and artery through the mental foramen. The authors present the case of an adolescent girl with numb chin symptoms during a multifocal sickle cell crisis. The aim is to briefly review the causes of numb chin syndrome, emphasizing the differential diagnosis in sickle cell patients., Portuguese Journal of Pediatrics, Vol. 49 No. 4 (2018)

In this article, we investigated structural and thermal properties of an amorphous alloy of the Ga-Se system. The amorphous GaSe9 alloy was produced by mechanical alloying and it was studied using EXAFS spectroscopy and cumulant expansion method. We also made reverse Monte Carlo simulations using the total structure factor S(K) obtained from x-ray diffraction and the EXAFS x(k) oscillations on Se and Ga K edges as input data. Several parameters, such as average coordination numbers and interatomic distances, structural and thermal disorders, asymmetry of the partial distribution functions gij(r), and Einstein and Debye temperatures, were determined. The gijE(r) functions were reconstructed from the cumulants C1, C2, and C3 obtained from the Einstein model, and they were compared to the gijRMC(r) functions obtained from the simulations. The simulations also furnished the partial bond angle distribution functions Θijl(cos θ), which describe the angular distribution of bonds between first neighbors, and give information about the kind of structural units present in the alloy. We investigated the thermal and photothermal properties of an amorphous GaSe9 alloy produced by mechanical alloying considering the photoacoustic spectroscopy and differential scanning calorimetry (DSC) techniques. The room temperature thermal diffusivity of GaSe9 was determined using the open photoacoustic cell configuration considering the thermal diffusion and thermoelastic bending effects. The glass transition and crystallization processes were investigated through DSC measurements obtained at five heating rates, and glass transition, crystallization temperatures, and activation energies were determined. The crystallization mechanism was also determined. [ABSTRACT FROM AUTHOR]

The present article explores the way translated literature informs on (i) how exile shapes the cities’ landscapes (both the starting city and the arrival), as well as (ii) the emotional hardship of the exilic condition, which entails a feeling of estrangement and the longing for imaginary homelands. To attain this twofold aim, it focuses on the eighteenth- and nineteenth-century Portuguese migrant movements to Paris. It searches, on the one hand, to retrace exilience in descriptions of Lisbon and Paris in biographical accounts of Portuguese exiles. On the other hand, it analyses an 1848 rewriting of Rabelais’ Gargantua in Portuguese. It is contented that Gargantua Portuguez [Portuguese Gargantua] bears testimony of the presence of anonymous Portuguese-language exiles in mid-nineteenth-century Paris, while creating a “safe house” for them, by seeking historical justice which would, in turn, assist in coping with the exilic condition.

The epic poem "Asia Jaag Utha," written in 1950 by Ali Sardar Jafri of the Indian Progressive Writers Association, was a battle hymn of its time, a celebration of Asia’s history and geography, with a vision for Asian liberation and communist revolution at the dawn of the Cold War in the aspirational Third World. Are its messages still applicable today, or is it strictly a period piece? This essay analyzes what Jafri was trying to do in his own context and explores whether it has anything to say to ours. In order to do this, the author enters into dialogue with Jafri’s poetry, and proposes some updates to its political agenda that might be needed to carry its energy into the present. While the mid-twentieth century vehicles of progress and liberation (such as industrial development and the postcolonial nation-state) require critique, Jafri’s emancipatory impulses and ideals of solidarity ring true.

Background Auscultation and ultrasonography are noninvasive techniques used to assess gastrointestinal motility in horses. Recently, noninvasive acoustic gastrointestinal surveillance (AGIS) biosensors evaluating intestinal motility have been validated in humans. Objectives To compare AGIS to auscultation and ultrasonography for detecting decreased motility after xylazine administration. Study design Randomised, blinded, controlled cross-over proof of principle study. Methods Six healthy horses were evaluated under fasted and nonfasted conditions and randomly assigned to receive treatment with 0.4 mg/kg xylazine or an equivalent volume of 0.9% NaCl intravenously. After a 48-h washout period, the process was repeated with the alternate treatment. Motility was assessed pre and posttreatment. Borborygmi were assessed in each abdominal quadrant and graded on a scale of 0-3, with 3 being continuous borborygmi. Duodenal, jejunal and caecal contractions were assessed ultrasonographically in consistent locations. Four AGIS biosensors were applied in standardised locations (duodenum, caecum, ventral midline, right dorsal colon). The biosensors measure acoustic signals and data were recorded in transport metric. Data were analysed using cross-classified multilevel random effects logistic regression including area under the receiver operator characteristic curve (AUC ROC). Sensitivity, specificity and accuracy were calculated for each modality. Results All three modalities detected a reduction in gastrointestinal motility following xylazine administration with AUC ROC being 0.85, 0.84 and 0.86 for auscultation, ultrasonography and AGIS respectively. The sensitivity, specificity and accuracy for auscultation was 88, 71 and 75%; for ultrasonography was 67, 63 and 64%; and for AGIS was 69, 70 and 70%, respectively. Main limitations The study was performed in normal healthy horses and application of this device to clinical patients warrants further investigation. Conclusions In this proof of principle study, AGIS was able to discriminate between horses given xylazine from those given 0.9% NaCl with comparable accuracy as auscultation and ultrasonography.

Recommendation systems are commonly used in websites with large datasets, frequently used in e-commerce or multimedia streaming services. These systems effectively help users in the task of finding items of their interest, while also being helpful from the perspective of the service or product provider. However, successful applications to other domains are less common, and the number of personalized food recommendation systems is surprisingly small although this particular domain could benefit significantly from recommendation knowledge. This work proposes a contextaware food recommendation system for well-being care applications, using mobile devices, beacons, medical records and a recommender engine. Users passing near a food place receives food recommendation based on available offers order by appropriate foods for everyone’s health at the table in real time. We also use a new robust recipe recommendation method based on matrix factorization and feature engineering, both supported by contextual information and statistical aggregation of information from users and items. The results got from the application of this method to three heterogeneous datasets of recipe’s user ratings, showed that gains are achieved regarding recommendation performance independently of the dataset size, the items textual properties or even the rating values distribution. info:eu-repo/semantics/publishedVersion

- Abstract
Summary Three Standardbred foals, a colt approximately one month old and 2 fillies approximately 4.5 months old, from the same farm, presented to University of Pennsylvania's New Bolton Centre over the course of 3 months for further evaluation due to acute onset of colic. All foals had a history of diarrhoea prior to presentation. On presentation, the foals had abdominal distension, dull demeanour and repeatedly lay down and rolled. Exploratory celiotomy revealed a segment of stenotic (lumen diameter 1–4 mm) small colon with marked distension of the colon proximal to the stenotic segment. Post mortem examination of the foals confirmed stenosis of the small colon with a segment of ulcerative colitis associated with the stenotic region. An aetiological agent was not identified.

Physeal fractures are common musculoskeletal injuries in foals and should be included as a differential diagnosis for the lame or nonweightbearing foal. Careful evaluation of the patient, including precise radiographic assessment, is paramount in determining the options for treatment. Prognosis mostly depends on the patient's age, weight, and fracture location and configuration.

Objective To determine serum amyloid A (SAA) concentrations in the postoperative (PO) colic horse and its association with PO complications and infection. Study design Prospective case series. Animals Fifty-one horses after colic surgery. Methods Blood samples from horses undergoing exploratory celiotomy were analyzed for quantitative SAA and fibrinogen concentrations and for neutrophil count at days 0, 1, 2, and 4-6 and at hospital discharge. Results Complications developed in 35 (69%) horses and included PO colic (n = 13, 25%), PO reflux (n = 11, 22%), intravenous catheter (IVC) complications (n = 11, 22%), low-grade fever (n = 11, 22%); surgical site infection (SSI; n = 9, 18%), high-grade fever (n = 7, 14%), and diarrhea (n = 4, 8%). There was an association between day 2 SAA and PO colic (P = .004), diarrhea (P = .042), IVC complications (P = .008), and PO reflux (P = .008) as well as day 4-6 SAA and PO colic (P = .004) and diarrhea (P = .018). Discharge SAA concentration was associated with SSI (P = .001). Fibrinogen concentrations at days 4-6 and at discharge were associated with PO colic (P = .003), diarrhea (P = .004), IVC complications (P = .002), and PO reflux (P = .023). No differences were seen in SAA, fibrinogen, or neutrophils between horses with PO infection vs those with non-infection-associated complications. Conclusion Serum amyloid A was markedly increased in the PO period in all horses but did not differ between PO infection and noninfection complications. The amount of inflammation associated with PO colic and colic surgery was high and may have affected the predictive value of SAA for early PO infection. Clinical significance Although increases occurred earlier, measurement of SAA with a point-of-care analyzer may not be a more sensitive indicator of infection in the early PO colic horse than fibrinogen concentration. Serum amyloid A may help detect SSI in the late PO period.

Sustainable development policies are the convergence point of plans and actions aiming to attain healthy, prosper and successful societies, while safeguarding natural resources and ecosystems. Progress on sustainable development must be measured on a regular basis to follow and to account for the defined political commitments. Sustainable development policies have impact on people's life; thus, monitoring must occur at the local scale. Using the framework of the sustainable development goals indicators and the European Union correspondent targets, this work is a contribution to evaluate living conditions at the municipal scale, taking Portugal as the case study. Fifty-four variables were statistically analysed and combined in a composite index whose conception, methodological procedures and validation process were previously described. Composite indicators are recognized as useful tools to monitor and to compare countries' evolution and this work focus on the results of an index that was tailored designed to be applied at a local level enabling to monitor SDGs. Results show that Portugal is struggling with the implementation of SDGs, namely: SDG 3, by the uneven distribution of health professionals alongside the increase of deaths by tumours and respiratory diseases; SDG 4, visible by the reduction of students enrolled in basic education; SDGs 7 and 12, by the increase of natural gas per capita consumption. Additionally, the aggravation of fires and the increasing number of incinerators and landfills affect many SDGs targets. At present, generally, living conditions are harsh than in 2009 during the financial crisis. [ABSTRACT FROM AUTHOR]
