Using MEDLINE, (limited to the English language and the reference lists of the randomized controlled trials (RCTs), we assessed the impact of religion on health outcomes via systematic, critical review of the medical literature. All RCTs published from 1966 to 1999 and all non-RCTs published from 1996 to 1999 that assessed a relationship between religion and measurable health outcome were examined. We excluded studies dealing with non-religious spirituality, ethical issues, coping, well-being, or life satisfaction. We used the Canadian Medical Association Journal's guidelines for systematic review of the medical literature to evaluate each manuscript. Nine RCTs and 25 non-RCTs met these inclusion/exclusion criteria. Randomized controlled trials showed that intercessory prayer may improvehealth outcomes in patients admitted to a coronary care unit but showed no effect on alcohol abuse. Islamic-based psychotherapy speeds recovery from anxiety and depression in Muslims. Non-RCTs indicate that religious activities appear to benefit blood pressure, immune function, depression, and mortality. [ABSTRACT FROM AUTHOR]

BACKGROUND. Physicians often have unanswered clinical questions. The purpose of this study was to determine how often the answers to these questions can be found in the medical literature. METHODS. We collected unanswered clinical questions from family physicians at the end of clinical half-days. The authors and medical librarians then used textbooks and MEDLINE to find answers to each question. We returned to the physicians one to five selected references for each question. Each physician rated these sources on how well they answered the questions and how they might influence the physician's practice. RESULTS. One hundred three questions were gathered. Physicians asked an average of 0.5 questions per half-day. We searched for answers to 86 questions, and the physicians returned ratings for 84. Forty-five (54%) of these questions were fully or nearly fully answered by the materials returned to the physicians. Of the questions for which answers were found, MEDLINE searches accounted for 71%; textbooks, 20%; and a combination, 9%. MEDLINE searches took an average of 27 minutes, whereas textbook searches averaged 6 minutes. CONCLUSIONS. The medical literature can provide answers to a majority of clinical questions; however, finding these answers is time-consuming and expensive. The article reports on mutually isolated medical literatures. It is argued that the fragmentation of science into specialties suggests that unintended and undiscovered relationships within the literature cannot be uncommon. The goal of this article is to develop methods and strategies for finding such relationships. A prototype of one possible structure underlying such a process was suggested. It is argued that the biomedical literatures on migraine and magnesium are mutually isolated to a high degree with a few exceptions, they do not cite or refer to each other and are not cocited.

This study demonstrates that certain unintended logical connections within the scientific literature, connections potentiality revealing of new knowledge, are unmarked by reference citations or other bibliographic clues. Specifically, 25 biomedical articles central to the argument that dietary fish oil causes certain blood changes are compared with 34 articles on how similar blood changes might ameliorate Raynaud's disease. The two groups of articles are thus connected by a chain of reasoning implicitly suggesting that dietary fish oil might benefit Raynaud patients, an hypothesis not heretofore published explicitly. By retrieving and bringing together these two literatures, that implicit, unstated, and perhaps unnoticed hypothesis becomes apparent. The more general problem is posed of whether systematic search techniques for bringing together logicality connected literatures can be developed and described, in the hope of discovering other implicit, unstated hypotheses. The example analyzed shows that the problem, while solved in this case by trial-and-error search methods, may be inherently and peculiarly difficult because there are virtually no references in either literature to the other, nor are there any clues from cocitation, bibliographic coupling, or statistician association of descriptors that the two literatures are logicality related. This article addresses file maintenance of subject headings in the National Library of Medicine's MEDLINE, a bibliographic retrieval file which is indexed using Medical Subject Headings (MeSH), NLM's subject authority. The emphasis is class maintenance in which sets of records in MEDLINE are changed by maintenance actions which reflect yearly changes in MeSH. Specific types of maintenance action are described with examples to highlight problem areas. Class maintenance failures, checking class maintenance, and informing end- users of MeSH changes resulting in class maintenance are discussed. Technical constraints and feasibility are not analyzed in detail; rather, the approach is geared toward requirements and impact of maintenance from the standpoint of vocabulary specialists and end-users. Two trends, the growth of interdisciplinary research and the proliferation of machine-readable data bases, require that new techniques and tools be applied to facilitate use of scientific and medical literature in support of research. In order to permit systematic searching of multiple data bases to satisfy requests in interdisciplinary areas, a mapping of a portion of Medical Subject Headings (MeSH) to three other controlled vocabularies has been constructed. A description of the development of the mapping is followed by details of a preliminary test of its effectiveness based on searches in the subject areas of applications of physics and engineering in medicine. The test shows: (1) Index Medicus provides an average of 81% of the citations retrieved in searches using more than one source; (2) in some subject areas use of the mapping does allow an increase in the number of items retrieved, but with a loss in precision. Limitations of the mapping revealed by the test suggest possible alternative applications of the mapping in controlled vocabulary development and in on-line searching. [ABSTRACT FROM AUTHOR]

The quantitative literature review is presented as a unique type of research endeavor complete with formal stages that parallel those associated with primary or experimental research. This article provides a tutorial overview of the quantitative literature review procedure and, in con. junction with the companion article published in this issue, serves to illustrate the application of this technique in the review process. The stages in quantitative review include: (1) problem formation; (2) data collection; (3) data evaluation; (4) analysis and interpretation; and (5) reporting results. It is argued that inferences made in the research review process are as central to the establishment of valid biomedical and clinical knowledge as inferences made in primary research. Despite some limitations, quantitative reviewing procedures constitute a significant advance over the traditional narrative methods of integrating empirical research in an area of interest. The use of quantitative reviewing procedures represents a paradigm shift in which the literature review is conceptualized as a unique form of scientific inquiry complete with formal stages and methods. The adoption of these methods should assist researchers in the behavioral and biomedical sciences in establishing scientifically valid data bases to guide theory development and direct future clinical investigation. In the medical literature on the 'cocktail party syndrome' very little attention has been directed towards contextual elements in evaluating children's behaviour. This research note reports upon how adults in one therapeutic community accounted the behaviour of Linda, identified as a cocktail party speaking hydrocephalic child. The descriptions of Linda as 'speaking like an adult' and of 'living in language' are examined in terms of their contextual constituents. Finally I examine briefly how adult staff members interact with Linda on a day-to-day basis and attempt to cope with the difficulties they see her as presenting. [ABSTRACT FROM AUTHOR]

Spontaneous pneumothorax (SPTX) is a potentially devastating rare complication of the thoracic radiation therapy. Most of the cases in the medical literature, have been described in lymphoma patients receiving radiation therapy. The pathogenesis of this adverse event remains undefined although different mechanisms have been proposed. We present a case of post-radiation therapy SPTX in a non-small cell lung cancer (NSCLC), following intensity modulated radiation therapy (IMRT), which to our knowledge is the first such reported case related to this newer mode of radiation therapy. This report highlights the importance of keeping a close eye for this complication as timely treatment with chest tube insertion and drainage of the pneumothorax can be a lifesaving in these patients. The article discusses one of the basic facets involved in the mechanization of documentation--the utilization of machines to facilitate the compilation of conventional type indexes. It is shown how automatic IBM punched-card equipment was experimentally employed to advantage in the preparation of a specific scientific index. The production of any large index is a complicated task. In the particular case of the "Current List of Medical Literature" there were specific operations that were especially troublesome and were particularly susceptible to mechanization. In order to make the necessary parallels the methods and format of the "Current List" is briefly discussed.

In a follow-up of earlier studies, Biological Abstracts (BA), Chemical Abstracts (CA), Excerpta Medico (EM), Psychological Abstracts (PA) and Current List of Medical Literature (CL) were searched for 240 published articles that resulted from oral research reports given at bio-medical meetings in 1957 and 1958. Seventy per cent of the 240 papers appeared in at least one of the four abstracting services, and almost 90% were indexed in CL; however, no single abstracting service covered more than about one-half of all the papers in any of the three research fields. The average time-lag between primary and secondary publication for cardiovascular and endocrine papers was about four months in CL, four in CA, six in BA, and eight to ten months in EM. For psy-chopharmacologic papers, all of the services were slower. This "tracer" technique makes possible and practical a continuous assessment of how a given research field is being served by abstracting and indexing services. [ABSTRACT FROM AUTHOR]

In summary, the CVLP cardiovascular pathology code represents a classification of malfunctions of the heart and peripheral blood vessels designed with a therapeutically oriented axis of approach. Data coded in accordance with this classification may be retrieved on three bases: (1) the functional type of malfunction, (2) the anatomical location and (3) structural lesions. The code has a dual purpose -- its immediate use is to make available by IBM techniques highly specific references to the information of biological response to chemical treatment contained in the medical literature. Its ultimate purpose is to provide data of a very fundamental nature, which may contribute to detailed studies of the possible relationships between biological activity and chemicals with their internal complexities of structure and physical attributes. It is hoped that this classification may find other applications in related fields of documentation. In its present form it is capable of expansion, and additional categories can easily be added to accommodate conditions such as congenital malfunction or inflammatory diseases, which are beyond the scope and definition of the CVLP interests. The Cochrane Library of Systematic Reviews is published quarterly. Issue 2 2009 contains 3737 complete reviews, 1939 protocols for reviews in production, and 9403 one-page summaries of systematic reviews published in the general medical literature. In addition, there are citations of 564,000 randomized controlled trials, and 11,000 cited papers in the Cochrane methodology register. The health technology assessment database contains 7591 citations. Abstract: The Delphi Method (DM) is the most frequently used technique to acquire structured expert-opinion elicitation (EOE). It has been increasingly applied to construct guidelines in medicine and to evaluate the appropriateness of clinical procedures. In this study, the RAND/UCLA appropriateness method was used as a structured EOE process to evaluate the appropriateness of a dataset concerning liver transplantation in adult and pediatric recipients for an information system funded by the Italian Ministry of Health. The original dataset was obtained using an interdisciplinary pool of regional experts (n = 60). This dataset held 280 items stratified into three groups: I. pretransplant items (n = 123); II. transplant items (n = 65); III. early posttransplant and follow-up items (n = 92). In the second DM round, the dataset was subjected to an extraregional panel of independent experts (n = 9) to assess a score ranging from 1 to 9 on each item based on increasing appropriateness, according to the RAND/UCLA Appropriateness Method. Overall agreement, uncertainty, and disagreement between experts was 95.89%, 3.12%, and 0.99%, respectively. For each group, agreement-uncertainty-disagreement were 99.35%/0.65%/0% (group I), 91.53%/5.30%/3.17% (group II), and 96.87%/3.13%/0% (group III), respectively. This study supported the use of a structured EOE process to evaluate the appropriateness of a large dataset for liver transplantation activity. [Copyright &y& Elsevier]

Abstract: Recent data from the medical literature indicates that cyclooxygenase-2 (COX-2) plays a key role in the production of vascular endothelial growth factor (VEGF), a glycoprotein that has the ability to increase the permeability of blood vessels and to induce angiogenesis. This study was undertaken to investigate the immunohistological co-expression of COX-2 and VEGF in inflamed human pulp, in conjunction with the expression of CD34, a transmembrane glycoprotein expressed in endothelial cells. Pulp tissue of extracted carious human third molars with a recent history of spontaneous pain were collected and processed for immunostaining of COX-2, VEGF, and CD34 using the biotin-streptoavidin method. Healthy pulp samples served as controls. COX-2 expression was not observed in healthy pulps, whereas all inflamed pulps demonstrated COX-2-expressing cells. Similarly, VEGF was not expressed in normal pulp tissue, but was strongly positive in inflamed pulps. CD34 was expressed in the endothelium of both normal and inflamed pulp tissues. Co-expression of COX-2 and VEGF in all consecutive sections of inflamed pulps could be suggestive of a possible release of VEGF via a COX-2-dependent pathway. [Copyright &y& Elsevier]

The article discusses the systematic review of the medical literature available through the PubMed and Scopus databases. The review utilized PubMed's Batch Citation Matcher (BCM) in order to determine the level over overlap in the material. The author found that BCM has many useful applications, including the isolation of medical literature not indexed by PubMed but available in lesser-known resources such as Scopus, and the ability to automatically remove duplicate bibliographic citations in literature search results.

The article argues for the introduction of a formal definition to the update of a systematic review of medical research. The authors define such an update as a "discrete event with the aim to search for and identify new evidence to incorporate into a previously completed systematic review." "New evidence" encompasses evidence that is both chronologically new and that was not considered in the past but is now thought to be important. Systematic reviews should take place at pre-arranged intervals to review the literature and see if an update is required. Re-analysis of inclusion criteria should be a part of an update.
