Cardiac magnetic resonance imaging (MRI) and computed tomography (CT) have been developed rapidly in the last decade. Technical improvements and broad availability of modern CT and MRI scanners have led to an increasing and regular use of both diagnostic methods in clinical routine. Therefore, this German consensus document has been developed in collaboration by the German Cardiac Society, German Radiology Society, and the German Society for Pediatric Cardiology. It is not oriented on modalities and methods, but rather on disease entities. This consensus document deals with coronary artery disease, cardiomyopathies, arrhythmias, valvular diseases, pericardial diseases and structural changes, as well as with congenital heart defects. For different clinical scenarios both imaging modalities CT and MRI are compared and evaluated in the specific context.

Fragestellung: Die bestehenden Abweichungen zwischen den jahrlichen Expansionsraten von Aortensegmenten in der CT, die aus dem Gefassegmentvolumen, dem maximalen orthogonalen Durchmesser, der maximalen orthogonalen Querschnittsflache und der Gefassegmentausenoberflache berechnet wurden, sollen bestimmt werden. Material und Methoden: Es wurden 73 Patienten (66±8,0 Jahre) retrospektiv in die Studie eingeschlossen, die zur Beurteilung des Gefasdiameters zwei CT-Angiografien der thorakalen und/oder abdominellen Aorta im mittleren Verlaufsintervall von 1,8±0,8 Jahren erhielten. Die Aorta wurde in 9 Segmente unterteilt (sinotubularer Ubergang bis Iliakalbifurkation). 346 Segmente konnten hinsichtlich Gefassegmentvolumen, maximalen orthogonalen Durchmesser, maximaler orthogonaler Querschnittsflache und Gefassegmentausenoberflache ausgewertet, die jahrlichen Expansionsraten berechnet und jeweils mit der des Gefassegmentvolumens als Standard verglichen werden. Ergebnisse: Die mittleren Differenzen zwischen Ausgangs- und Folgeuntersuchung betrugen fur das Volumen 5,7%±13,1 (4,8cm3±12,2), fur den Durchmesser 2,1%±4,9 (0,7mm±1,6), fur die Querschnittsflache 4,6%±13,6 (0,5cm2±1,1) und fur Ausenoberflache 4,8%±7,9 (3,7cm2±6,1). Im Vergleich mit der prozentualen Expansionsrate des Volumens war die prozentuale Expansionsrate des Durchmessers 2,7-fach, die der Querschnittsflache und der Ausenoberflache 1,2-fach niedriger. Transposition of the great arteries (TGA) is a rare disease representing not more than 3-5% of all congenital heart diseases. TGA is a cardiac anomaly in which the aorta arises entirely or largely from the morphological right ventricle and the pulmonary artery from the morphological left ventricle. This is called a ventriculo-arterial discordant connection and when accompanied by an atrio-ventricular concordant connection it is called a complete or D-transposition (D-TGA). The terms congenitally corrected TGA (ccTGA) or L-TGA describe an atrio-ventricular discordant connection. In D-TGA survival can only be achieved if additional shunting is simultaneously present, which possibly has to be created post-natal by the so-called Rashkind maneuver.Nowadays, an early anatomic correction using the arterial switch operation is the treatment of choice. Up to the 1980s, an atrial switch operation according to Senning/Mustard was performed. Apart from echocardiography the imaging modality of choice is usually MRI to assess the complex postoperative anatomy, viability of the myocardium and to perform a volumetric and functional assessment, including MR flow measurements. Multidetector computed tomography (MDCT) is used if there are contraindications to MRI or if an assessment of cardiac and especially coronary anatomy is the main interest.

Nora's lesion, also known as "bizarre parosteal osteochondromatous proliferation" (BPOP), was first described in 1983 by the pathologist Nora. This lesion is defined as a proliferation of the bone. In most cases the lesion emanates from the intact cortical substance of short bones. It used to be assigned to reactive, heterotopic ossifications. More recent publications described constant genetic alterations supposing a tumorous genesis. Nora lesions are mostly found in the third or fourth decade of life; a preference of sexes is not described in the literature. They are characterized by a typical appearance in radiological diagnostics, but the diagnosis is ultimately determined by histopathological examination. Surgical resection is the therapy of choice.We report the case of a 29-year-old patient with an undetermined proliferation of the proximal ulna. The diagnosis of a Nora's lesion was made. The therapeutic approach, differential diagnosis and corresponding literature are presented and discussed.

Ziele: Ziel war es die, Ubereinstimmung und Variabilitat zwischen Computertomographie (CT) und Rotationsangiographie (Dyna-CT) in Bezug auf die Vermessung aller relevanten anatomische Mase fur einen transapikalen AKE zu bestimmen. Methode: Es wurden 27 Patienten (82+-11 Jahre, 21w/6m) mit hochgradiger Aortenklappenstenose und Indikation fur einen transapikalen Aortenklappenersatz (AKE) retrospektiv in unsere Studie eingeschlossen. Alle Patienten erhielten aus klinischer Indikation ein praoperatives kontrastmittelverstarktes EKG-synchronisiertes 64-Zeilen-CT und ein intraoperatives Dyna-CT unter rapid pacing zur Darstellung der Aortenwurzel und thorakalen Aorta. Nachbearbeitung und Bildanalyse wurden von 2 Radiologen verblindet durchgefuhrt. Die Ubereinstimmung beider Methoden wurde nach Bland-Altman ermittelt. Ergebnis: Alle fur einen transapikalen AKE relevanten Strukturen waren mit beiden Bildgebungsmethoden in ausreichendem Mase darstellbar. Die mittlere Differenz zwischen CT und Dyna-CT fur den Diameter des sinotubularen Ubergangs betrug 0.15±1.03mm, fur die Klappenebene 1.22±2.22mm und fur den Abstand der Koronarostien vom Leafletansatz 0.19±1.34mm links und 0.91±2.02mm rechts. Es ergab sich ein signifikanter Unterschied fur die Diametermessungen der Klappenebene zwischen CT und Dyna-CT (p=0.01). Diametermessungen des sinotubularen Ubergangs, des Koronarostienabstands zur Klappenebene, des Bulbus aortae und der Aorta ascendens ergaben keinen signifikanten Messunterschied zwischen CT und Dyna-CT (p=0.482–0.783), bei signifikanter Korrelation der Messwerte (r=0.881, r=0.878, r=0.744, r=0.886, r=0.911; p

Ziele: Uber die Funktion des rechten Ventrikels bei chronischer Myokarditis ist bisher wenig bekannt. Einige Studien haben bereits die Bedeutung der rechtsventrikularen Funktion fur die Prognose anderer kardialer Erkrankungen beschrieben. Daher wurden in dieser Arbeit funktionelle Veranderungen beider Ventrikel untersucht und interventrikulare Interaktionen beurteilt. Methode: 101 Patienten (durchschnittlich 45±14 Jahre alt) mit chronischer Myokarditis wurden an einem 1,5 Tesla MRT untersucht (Diagnose durch Endomyokardbiopsie gesichert). Zusatzlich wurde ein Normkollektiv aus 24 herzgesunden Probanden gebildet (durchschnittlich 31±14 Jahre alt). Die volumetrische Analyse beider Ventrikel wurde in der Kurzen Achse anhand von Steady-State-Free-Precession- Sequenzen durchgefuhrt. Ergebnis: Im Vergleich zum Normkollektiv konnte bei Patienten mit chronischer Myokarditis eine signifikante Einschrankung sowohl der links- als auch der rechtsventrikularen Ejektionsfraktion festgestellt werden. Das enddiastolische und endsystolische Volumen des linken Ventrikels waren signifikant vergrosert. Im rechten Ventrikel blieb das enddiastolische Volumen unverandert, das endsystolische Volumen vergroserte sich leicht. Diese Veranderung blieb jedoch ohne statistische Signifikanz. Die linksventrikulare Muskelmasse zeigte sich signifikant vermehrt, wahrend die rechtsventrikulare Muskelmasse unverandert blieb. Dennoch stellte sich heraus, dass die Volumina beider Ventrikel positiv miteinander korrelierten. So zeigte sich bei einer Zunahme der enddiastolischen und endsystolischen Volumina des linken Ventrikels eine- wenn auch weniger stark ausgepragte- Vergroserung der enddiastolischen und endsystolischen Volumina des rechten Ventrikels. Ebenso korrelierten die rechts- und die linksventrikulare Muskelmasse miteinander. Die positive Korrelation zwischen den Ejektionsfraktionen beider Ventrikel verstarkte sich bei einer rechtsventrikularen Ejektionsfraktion ≤45%, wahrend sich bei einer linksventrikularen Ejektionsfraktion ≤45% kein Zusammenhang zwischen den beiden Parametern zeigte. Schlussfolgerung: Eine chronische Myokarditis geht haufig mit einer eingeschrankten Ejektionsfraktion beider Ventrikel einher. Es kommt vornehmlich zu einer Dilatation des linken Ventrikels, die sich jedoch bei starkerer linksventrikularer Volumenvergroserung auf den rechten Ventrikel ausweitet. Die globale Funktion zeigt sich am starksten durch eine ausgepragte rechtsventrikulare Funktionsstorung beeinflusst. Ziele: Vergleich der Bildqualitat einzelner Herzsegmente anhand des SNR und CNR bei FIESTA- sowie verschiedenen MR-Black-Blood-SE-Sequenzen fur die kardiale Bildgebung bei 1.5 und 3.0 T an Probanden. Methode: Es wurden 18 herzgesunde Probanden mit einem mittleren Alter von 28 (±5) an einem GE Signa 3.0 T und einem GE Signa 1.5 T Twinspeed Excite (GE Medical Systems, Milwaukee, USA) jeweils mithilfe einer 4-Elemente-„phased array“-Oberflachenspule untersucht. Als Sequenzen wurden EKG-getriggerte „steady state free precession“ (SSFP) FIESTA- Sequenzen sowie Fast-Spin-Echo-Sequenzen (FSE) mit T1- (Double-IR), T2-Wichtung und T1-Stir (Triple-IR) verwendet. Im 2-Kammer-Blick „kurze Achse“ erfolgte die SNR- und CNR-Bestimmung fur den gesamten linken Ventrikel (LV) und fur die 4 LV-Segmente: Septum (S), Vorder- (VW), Hinter- (HW) und Seitenwand (SW). Ergebnis: Es kam zu einem Anstieg des SNR und CNR des gesamten linken Ventrikels bei allen drei FSE-Sequenzen sowie der FIESTA-Sequenz bei 3.0 Tesla (FSE: SNR min. 8%, max. 25%/CNR min. 11%, max. 25%; FIESTA: SNR 111%/CNR 40%). Es zeigte sich in Abhangigkeit vom jeweiligen Herzsegment ein unterschiedlicher SNR-/CNR-Anstieg, welcher bei den FSE-Sequenzen am grosten in der VW und SW ausgepragt war (SNR min.17%, max. 39%) Im Septum fiel der SNR-/CNR-Anstieg am geringsten aus (SNR min. -1%, max. 9%) und war in keinem der Falle signifikant. Sowohl bei 1.5 als auch bei 3.0 Tesla wies die Hinterwand die niedrigsten Werte fur SNR u. CNR auf. Bei der FIESTA-Sequenz fand sich bei beiden Feldstarken das hochste SNR und zugleich der groste SNR-Zuwachs (129%) im Septum. Das SNR war bei 1.5 sowie 3.0 Tesla in der SW am kleinsten. Schlussfolgerung: Das mittlere SNR und CNR bei 3.0 T im Vergleich zu 1.5 T konnte bei allen FSE- sowie der FIESTA-Sequenz gesteigert werden. Die einzelnen Herzsegmente des LV profitieren in unterschiedlichem Mase von der hoheren Feldstarke. Bei beiden Feldstarken ist im Bereich der Seiten- und insbesondere der Hinterwand das SNR/CNR gegenuber den anderen Segmenten am kleinsten, was vorrangig auf Signalinhomogenitaten zuruckzufuhren ist. Diese Signalinhomogenitaten werden bei hoherer Feldstarke verstarkt, weshalb ein primares Ziel der Verbesserung des Sequenzdesigns kardialer MR-Sequenzen bei hoherer Feldstarke diesem Problem gewidmet werden muss. Ziele: Myokarditispatienten tragen trotz einer haufig unauffalligen Klinik das erhohte Risiko eine dilatative Kardiomyopathie zu entwickeln. Ziel dieser Untersuchungen war es daher herauszufinden, ob Myokardschadigungen in Form von Dyskinesien nachweisbar sind, und ob diese die Pumpfunktion beeinflussen Methode: 146 Myokarditispatienten (davon 114 mit durch Biopsie gesicherter Diagnose) wurden an einem 1,5 Tesla-MRT untersucht (Durchschnittsalter 45±14 Jahre). SSFP- Sequenzen im 4- Kammerblick und im 2- Kammerblick dienten zur Auswertung der Funktionsparameter. Die Beurteilung der Wandbewegungsstorungen erfolgte anhand des 4- Kammerblicks und des 2- Kammerblicks in der kurzen Achse mithilfe eines Segmentmodells. Ergebnis: 52 Patienten (36%) wiesen globale rechtsventrikulare und 37 Patienten (26%) globale linksventrikulare Wandbewegungsstorungen auf. Lokale rechtsventrikulare Dyskinesien zeigten sich bei 108 Patienten (74%). 90 Patienten (62%) zeigten lokale linksventrikulare Dyskinesien. Bei 21 Patienten (14%) waren beide Ventrikel von globalen Dyskinesien betroffen. Patienten mit globaler rechtsventrikularer Dyskinesie zeigten sowohl eine signifikant geringere rechtsventrikulare als auch linksventrikulare Ejektionsfraktion im Vergleich zu Myokarditispatienten ohne globale rechtsventrikulare Wandbewegungsstorungen. Die Gruppe mit globaler linksventrikularer Dyskinesie wies im Vergleich mit der Gruppe ohne globale linksventrikulare Dyskinesie eine signifikant verminderte linksventrikulare Ejektionsfraktion auf. Die rechtsventrikulare Ejektionsfraktion zeigte sich dabei nicht beeinflusst. Lokale linksventrikulare Dyskinesien korrelierten mit einer signifikanten Verschlechterung der linksventrikularen Ejektionsfraktion. Schlussfolgerung: Wandbewegungsstorungen finden sich bei Myokarditispatienten im rechten Ventrikel haufiger als im linken Ventrikel. Globale rechtsventrikulare Dyskinesien beeintrachtigen die Pumpfunktion beider Ventrikel wahrend globale Dyskinesien des linken Ventrikels lediglich zu einer linksventrikularen Funktionseinschrankung fuhren. Bereits lokale Dyskinesien des linken Ventrikels reichen aus, um seine Pumpfunktion einzuschranken. Die Auswurfleistung bleibt lediglich bei lokalen rechtsventrikularen Dyskinesien unbeeintrachtigt. BACKGROUND: The technique of stereolithography, which automatically fabricates models from X-ray computed tomography or magnetic resonance imaging (MRI) data linked to computer-aided design programs, has been applied to the fabrication of scaffolds for tissue engineering. We previously reported on the application of stereolithography in scaffold fabrication of a trileaflet heart valve. In our current experiment we demonstrate a new technique for the fabrication of custom-made conduits for the potential replacement of a coarcted aortic segment. METHODS AND RESULTS: In this experiment the image data derived from a 12-year-old male patient with aortic coarctation scanned by MRI were processed by a computer-aided design program to reconstruct the aortic arch with isthmus stenosis three dimensionally. By defining the stenotic section and the adjacent normal vessel a custom-made nonstenotic descending aorta was reconstructed to replace the stenosed part. The rapid prototyping technique was used to establish stereolithographic models for fabricating biocompatible and biodegradable vascular scaffolds with the anatomic structure of the recalculated human descending aorta through a thermal processing technique. CONCLUSION: Our results suggest that the re-creation and reproduction of complex vascular structures by computer-aided design techniques may be useful to fabricate custom-made polymeric scaffolds for the tissue engineering of living vascular prostheses.

The role of nuclear medicine imaging in the diagnosis of vascular parkinsonism (VP) has been addressed by only few studies up to now. Most previous reports suggest no or only mild impairment of DAT and D2 receptors in VP. In contrast, in four patients with VP, reported here, the combined DAT and D2 receptor SPECT showed highly unusual changes in the pre- and/or postsynaptic dopaminergic system. The possible value of combined DAT/D2 receptor SPECT imaging should be investigated by future prospective studies.

Minimally invasive resection of hyperfunctional parathyroid glands is an alternative to open surgery. However, it requires a precise preoperative localization. This study evaluated the diagnostic use of magnetic resonance (MR) imaging, parathyroid scintigraphy, and consecutive image fusion.17 patients (9 women, 8 men; age: 29-72 years; mean: 51.2 years) with primary hyperparathyroidism were included. Examination by MRI used unenhanced T1- and T2-weighted sequences as well as contrast-enhanced T1-weighted sequences. (99m)Tc-MIBI scintigraphy consisted of planar and SPECT (single photon emission tomography) imaging techniques. In order to improve the anatomical localization of a scintigraphic focus, SPECT-data were fused with the corresponding MR-data using a modified version of the Express 5.0 software (Advanced Visual Systems, Waltham, MA). Results of image fusion were then compared to histopathology.In 14/17 patients, a single parathyroid adenoma was found. There were 3 cases with hyperplastic glands. MRI detected 10 (71%), scintigraphy 12 (86%) adenomas. Both modalities detected 1/3 patients with hyperplasia. Image fusion improved the anatomical assignment of the 13 scintigraphic foci in five patients and was helpful in the interpretation of inconclusive MR-findings in two patients.Both MRI and (99m)Tc-MIBI scintigraphy sensitively detect parathyroid adenomas but are less reliable in case of hyperplastic glands. In case of a scintigraphic focus, image fusion considerably improves its topographic assignment. Furthermore, it facilitates the evaluation of inconclusive MRI findings.

Evaluation of the use of statistical parametrical mapping (SPM) of FDG-PET for seizure lateralization in frontal lobe epilepsy.38 patients with suspected frontal lobe epilepsy supported by clinical findings and video-EEG monitoring.Statistical parametrical maps were generated by subtraction of individual scans from a control group, formed by 16 patients with negative neurological/psychiatric history and no abnormalities in the MR scan. The scans were also analyzed visually as well as semiquantitatively by manually drawn ROIs.SPM showed a better accordance to the results of surface EEG monitoring compared with visual scan analysis and ROI quantification. In comparison with intracranial EEG recordings, the best performance was achieved by combining the ROI based quantification with SPM analysis.These findings suggest that SPM analysis of FDG-PET data could be a useful as complementary tool in the evaluation of seizure focus lateralization in patients with supposed frontal lobe epilepsy.

Comparison of the performance of virtual and conventional colonoscopy for the detection of colorectal polyps and inflammatory colon diseases using a 16-slice spiral CT scanner. Furthermore, presentation of the first experiences with a new three-dimensional reconstruction mode ("colon-dissection") that dissects the colonic wall like a pathologic-anatomic preparation to increase the sensitivity.Forty patients were studied using a 16-slice spiral CT (Lightspeed 16, General Electric Medical Systems, Milwaukee, Wisconsin 53201, USA). The examination was performed after standard oral preparation for colonoscopy. The colonic distension was achieved with room air and intravenous butylscopolamine. Images were obtained in supine and prone position using a detector configuration of 16 x 0.625 mm, pitch 1.7, rotation time 0.5 s, 160 mAs and 120 kV. Axial reconstruction with a slice thickness of 0.625 mm. The CT data were assessed by two radiologists on an Advantage Workstation (Volume Analysis 2, USA) using a software with the capabilities of axial, multiplanar and volume rendering, virtual endoscopy, and colon dissection. Conventional colonoscopy was used to determine the sensitivity.A total of 30 polyps were found in 8 patients and a carcinoma was detected in two patients. Colonography identified 4 polyps with a diameter of 10 mm or more, 6 polyps with a diameter of 5 mm to 9.9 mm, 11 polyps with a diameter of 3 to 4.9 mm and 9 polyps with a diameter of 3.0 mm or less. There were two false negative findings (one polyp of 3 mm and one of 4 mm had been overseen) and two false positive findings for polyps (polyps of 4 mm and 6 mm). The sensitivity and specificity for the detection of colonic polyps were 93% and 94% with the "colonic-dissection" mode, 87 % and 94 % with the "virtual-endoscopy" mode and 63 % and 97 % with multi-planar reconstruction, respectively. Depending on the diameter of the colonic polyps, the "colon-dissection" mode ("virtual-coloscopy") had a sensitivity and specificity of 100 % and 100 % for polyps with a diameter over 5.0 mm, 91 % and 82 % for polyps with a diameter from 3.0 to 4.9 mm and 89 % and 78 % for polyps with a diameter under 3.0 mm, respectively. Inflammatory colon diseases presented as thickening of the colon wall over 5 mm.Virtual colonoscopy with 16-slice spiral CT allows accurate detection of colonic polyps, including small polyps below a diameter of 3 mm. In comparison with the 2D- and 3D-"virtual-endoscopic" reconstruction, the 3D-reconstruction software "colon-dissection" achieves the highest sensitivity for the detection of colonic masses. Therefore, the combination of a 16-slice spiral CT and the "colon-dissection" reconstruction software provides a high resolution in the z-axis for detecting colonic masses and polyps down to a diameter of less than 2 mm, with a sensitivity of about 90 %. This sensitivity is much higher than the sensitivity achievable with 4-slice spiral CT and without "colon-dissection" mode. A thickened colon wall over 5 mm indicates inflammatory colon disease.

The Fontan operation causes an acute decrease of volume overload of the univentricular heart followed by changes in ventricular geometry. The postoperative increase of myocardial mass-volume-index (MVI) may alter ventricular diastolic function. In this study, we analysed whether the increase in MVI and changes of the ventricular geometry have an effect on the decrease of the exercise capacity in patients with Fontan surgery.We examined the cardiopulmonary function of 24 patients with functional single ventricle and Fontan operation 3.63 +/- 1.97 years after surgery (m = 14, w = 10, age: 14.57 +/- 9.74 years) using a bicycle cardiopulmonary exercise testing. The parameters of exercise capacity and cardiopulmonary function were correlated with Magnetic Resonance Imaging (MRI) parameters such as the MVI, enddiastolic ventricular muscle mass (EDMM) und endsystolic volume (ESV).The exercise capacity was 2.06 +/- 0.54 W/kg (60.63 +/- 15.75% of the norm) and VO(2)max was 27.41 +/- 8.87 ml/min/kg (60.91 +/- 19.7% of the norm). There was a positive correlation of exercise capacity and VO(2)max with ESV (r(2) = 0.2572, p = 0.033) and EDMM (r(2) = 0.2544, p = 0.024), but none with the MVI.Myocardial hypertrophy may influence the myocardial performance of the univentricular heart and thereby the physical performance in children and adults with Fontan circulation.

To assess the new method of 3-dimensional echocardiography in comparison to the "gold standard" MRI as to its ability to calculate left ventricular volumes in patients with congenital heart disease.Eighteen patients between the ages of 3.9 to 37.3 years (mean: 12.8 +/- 9.7) with a geometrically abnormal left ventricle were examined using a 1.5T scanner with a fast gradient-echo sequence (TR = 14 ms, TE = 2.6 - 2.9 ms, FOV = 300 - 400 mm, flip angle = 20 degrees, matrix = 128 : 256, slice thickness = 5 mm, retrospective gating) in multislice-multiphase technique. Transthoracic 3D-echocardiography was performed with a 3.5 MHz transducer and a Tomtec (Munich, Germany) system for 3D reconstruction.Volume calculation was possible in all patients with 3D-echocardiography, but the muscle mass calculation only succeeded in 11 of 18 patients (61 %) due to inadequate visualization of the entire myocardium. Comparing MRI and 3D-echocardiography, the correlation was r = 0.97 for the end-systolic volumes, r = 0.98 for the end-diastolic volumes, r = 0.79 for the end-systolic muscle mass and r = 0.77 for the end-diastolic muscle mass. The agreement between both methods was considered good for the calculated end-diastolic volumes and sufficient for the calculated end-systolic volumes. The muscle mass calculations showed larger differences especially for the end-systolic mass. Mean intraobserver variability was 18.6 % for end-systolic and 8.3 % for end-diastolic volumes.In patients with an abnormal left ventricular configuration due to congenital heart disease, the new method of 3D-echocardiography is sufficient for volume calculations in preselected patients. The high intraobserver variability is still a limitation of transthoracic 3D-echocardiography in comparison to MRI.
