104. Co-dominant expression of the HLA-G gene and various forms of alternatively spliced HLA-G mRNA in human first trimester trophoblast

Abstract

Udgivelsesdato: 1998-Feb, Genes may be silenced at the transcriptional level by 'genomic imprinting' in such a way that only one of the parental alleles is expressed. Imprinting may be tissue-specific and in some cases it seems also to be time-dependent during development. The phenomenon has been studied in pre- and post-implantation developmental processes. Animal studies of genomic imprinting of major histocompatibility complex (MHC) antigens in the placenta have shown discordant results. To address this issue in the human placenta, we examined the expression of the non-classical human leukocyte antigen (HLA) class I gene, HLA-G. Genomic imprinting of the HLA-G locus could have implications for the interaction in the feto-maternal relationship. Restriction Fragment Length Polymorphism (RFLP), allele-specific amplification and Single Strand Conformation Polymorphism (SSCP) analysis followed by DNA sequencing were performed on Reverse Transcription (RT) Polymerase Chain Reaction (PCR) products of HLA-G mRNA to examine the expression of maternal and paternal alleles. Our results demonstrate that HLA-G is co-dominantly expressed in first trimester trophoblast cells. A "new" non-synonymous base substitution in exon 4 was detected. We also investigated the different alternatively spliced forms of HLA-G mRNA in first trimester trophoblast and found the full-length transcript to be the far most abundant.

109. Differential expression of neural cell adhesion molecule and cadherins in pancreatic islets, glucagonomas, and insulinomas.

Abstract

The endocrine cells of the pancreas develop from the endoderm and yet display several characteristics of a neuronal phenotype. During embryonic life, ductal epithelial cells give rise to first the glugagon-producing cells (alpha-cells) and then cells that express insulin (beta-cells), somatostatin (delta-cells), and pancreatic polypeptide (PP-cells) in a sequential order. The endocrine cells are believed to arise from a stem cell with neuronal traits. The developmental lineage from a common neuron-like progenitor is evidenced by: transient coexpression of more than one cell type-specific hormone in immature cells, expression of neuronal markers during islet cell development, and the pluripotentiality of clones of insulinoma cells to develop into cells expressing other islet cell hormones. The four mature endocrine cell types assume a particular organization within the islets of Langerhans in a process where cell adhesion molecules are involved. In this study we have analyzed the expression of neural cell adhesion molecule (NCAM) and cadherin molecules in neonatal, young, and adult rat islet cells as well as in glucagonomas and insulinomas derived from a pluripotent rat islet cell tumor. Whereas primary islet cells at all ages express unsialylated NCAM and E-cadherin, as do insulinomas, the glucagonomas express the polysialylated NCAM, which is characteristic for developing neurons. The glucagonomas also lose E-cadherin expression and instead express a cadherin which is similar to N-cadherin in brain. Insulinoma cells express E-cadherin but differ from primary islet cells by expressing a second cadherin molecule, which is similar to N-cadherin. The expression of NCAM and cadherin isoforms in the glucagonoma suggest that this transformed alpha-cell type has converted to an immature phenotype with strong neuronal traits, reflecting the early palce of glucagon-producing cells in the islet cell lineage. Abstract

Udgivelsesdato: 1992-Apr, Tumour cell adhesion, detachment and aggregation seem to play an important part in tumour invasion and metastasis, and numerous cell adhesion molecules are expressed by tumour cells. Several families of cell-cell adhesion molecules have been described, of which two groups are particularly well characterised, the cadherin family and the Ig superfamily member, neural cell adhesion molecule (NCAM). We investigated expression of these two adhesion molecule families in small cell lung cancer (SCLC) cell lines and xenografts by immunoblotting. Nineteen tumours established from 15 patients with SCLC were examined. All tumours but one expressed both cadherin and NCAM. The tumours expressed one, two or rarely three cadherin bands, and different combinations of two major isoforms of NCAM with M(r)'s of approximately 190,000 and 135,000. Polysialylation of NCAM, a feature characteristic of NCAM during embryonic development, which may play a role in connection with tumour invasion and metastasis, was found in 14/18 NCAM expressing SCLC tumours. Individual tumours grown as cell lines and as nude mouse xenografts showed no qualitative differences in cadherin or NCAM expression.

Abstract

Among the numerous non-point sources of diffuse water pollution with nitrogen, agriculture is counted one of the main sources. The agricultural policies of the Agenda 2000 and a decoupling of direct payments for farmers from their production decisions are exemplarily evaluated as nitrogen reduction measures using the Regional Agricultural and Environmental Information System RAUMIS. The results show that until the target year 2010 the risk of diffuse pollution of water bodies with nitrogen is a regional problem in Germany. These problems are neither mitigated by the policies of Agenda 2000 nor by a decoupling of direct payments from production decisions of farmers. While total nitrogen surplus reduces considerably after a decoupling of direct payments due to decreases of land-use the nitrogen surplus on the remaining cultivated area increases resulting from structural changes. Granting the same amount of direct payments to farmers in both policy alternatives the agricultural sector income would be higher after a decoupling of direct payments opposed to the Agenda 2000 resulting from a more efficient allocation of inputs. 123. Platelet function after coronary artery bypass grafting: is there a procoagulant activity after off-pump compared with on-pump surgery?

Abstract

Objective-After off-pump coronary artery bypass (OPCAB) haemostasis might be better preserved compared with on-pump coronary artery bypass grafting (CABG). The aim of this study was to investigate whether this possibly better preserved haemostasis results in a procoagulant activity of the platelets. Design-Thirty patients were studied prospectively, 15 undergoing on-pump CABG and 15 undergoing OPCAB. Platelet function was evaluated four times within the first 24 h: preoperatively, postoperatively, 4 h and 1 day after surgery with a bedside whole blood clotting test. Results-A significant increase of platelet-activating-factor-induced platelet aggregation was observed postoperatively after OPCAB (p < 0.01). Only two patients did not reach preoperative values within 1 day postoperatively and four patients had a more than twofold increase. Platelet aggregation immediately after on-pump CABG was reduced to near half of preoperative values, but within 1 day postoperatively normal platelet aggregation was regained in half of the patients. Conclusion-This study has mainly indicated that platelets after OPCAB were more easily activated in the early postoperative period. After CABG with cardiopulmonary bypass we found a temporary platelet dysfunction which seemed to be overcome within the first postoperative day. [ABSTRACT FROM AUTHOR]

124. Antibiotic prophylaxis in vascular reconstructive surgery: a double-blind placebo-controlled study.

Abstract

In a prospective randomized double-blind study of 141 patients referred for reconstructive vascular surgery on the abdominal aorta and the lower extremities, placebo was compared to antibiotic prophylaxis. The prophylaxis group received three doses of a combination of methicillin, 2 g and netilmicin, 200 mg. Antibiotic prophylaxis reduced postoperative wound infections as compared to placebo, i.e. 4/69 (5.8%) vs. 12/72, (16.7%) respectively (=0.04). No graft infections occurred. Two cases of postoperative septicaemia were seen in the placebo group, none in the antibiotic group. Among different procedures aortic-femoral bypass operations showed the highest wound infection rates. The two treatment groups were comparable with regard to all other postoperative complications registered, including nephro- and ototoxicity. The antibiotic regimen was considered safe, but had only marginal value as prophylaxis in vascular reconstructive surgery on the abdominal aorta and the lower extremities. [ABSTRACT FROM PUBLISHER]

Abstract

A new surgical procedure is presented in which a levator ani muscle repair is performed via an abdominal approach. The method is discussed in light of the traditional techniques. The preliminary results in 50 patients treated by this method are presented. Clinical and objective cure was found to a high extent.

137. RAPID RADIATION IN <e1>AFRAMOMUM</e1> (<e1>ZINGIBERACEAE</e1>): EVIDENCE FROM NUCLEAR RIBOSOMAL DNA INTERNAL TRANSCRIBED SPACER (ITS) SEQUENCES

Abstract

Internal transcribed spacer (ITS) nrDNA sequences of 42 accessions (representing 28 species) of Aframomum revealed an unusually low level of sequence variation, suggesting a recent radiation of the genus in Africa. The sample of species analysed includes all the main morphological variation and is based on wide geographical sampling. The Aframomum sequences varied from 187bp to 190bp (ITS 1) and 215bp to 216bp (ITS 2). Pair-wise sequence difference between accessions varied from 0% (e.g. A. luteoalbum and A. thonneri) to 2.74% (e.g. A. sp. nov. B to A. pseudostipulare). This contrasts with a comparable data set for the SE Asian genus Alpinia in the same tribe (Alpineae ) in which maximum pair-wise difference is six times greater (range 0.5–15.6%). A parsimony analysis of the in-group and out-group taxa supports the monophyly of the genus Aframomum, but does not resolve the relationships between the in-group species. Four putative multi-species groups, however, have some jackknife support. The species sampled vary greatly in vegetative, floral and fruit characters. This morphological variation is not reflected in the ITS sequence data. This may be a result of rapid radiation under conditions of Pleistocene climatic change and effective dispersal of seeds by primates.

139. Nephropexy ad modum Albarran Marion: Long-term Follow-up.

Abstract

Objective: To report a long-term follow-up of nephropexy ad modum (a.m.) Albarran Marion. Material and Methods: Between 1980 and 1999, 18 women (mean age 42.6 years) underwent nephropexy a.m. Albarran Marion. The duration of symptoms varied from 7 months to 9 years. All patients were examined preoperatively using excretory urography and renography in both the supine and erect positions in order to document renal descent. Patients with decreased renal function also underwent postoperative renography. A questionnaire concerning pre- and post-treatment symptoms was sent to all patients. Results: Hospitalization averaged 7.2 days (range 4-15 days). Pneumothorax occurred in 7 patients, 3 of whom required temporary drainage. One patient developed pneumonia and another a retroperitoneal haematoma; no other complications occurred. All postoperative renography findings were normal. Median follow-up time was 11.2 years (range 2-21 years). At follow-up, 87.5% of patients were satisfied with the results and were free from pain. Two patients were not satisfied with the results; retrospectively the indications for surgery in these 2 cases were not absolute. Conclusions: Operative treatment for symptomatic nephroptosis should only be performed in selected cases. We present a success rate of 87.5% for nephropexy a.m. Albarran Marion, at a mean follow-up of 11.2 years, which equals the best results obtained with other procedures. The choice of operative procedure for symptomatic nephroptosis must be based on the special interests and skills available at the institutions concerned. [ABSTRACT FROM AUTHOR]

140. Pivmecillinam plus Pivampicillin in Complicated Urinary Tract Infection. Double-Blind Comparison of the Combination Pivmecillinam/Pivampicillin and Pivmecillinam Alone in Patients with Urinary Tract Infection

Abstract

Twenty-six surgical-urological patients with severe underlying diseases of the urinary tract and an acute urinary tract infection received a 10-day treatment with either pivmecillinam, 400 mg three tmes daily (twelve patients), or the fixed dose combination of pivmecillinam/pivampicillin (pivmecillinam 200 mg plus pivampicillin 250 mg) three times daily (fourteen patients). Eleven of the fourteen patients given combined therapy were cured bacteriologically, compared to only four out of twelve patients taking pivmecillinam alone. Clinical success was achieved in eleven out of fourteen patients who received combination therapy and in seven out of twelve subjects given pivmecillinam. Mild gastro-intestinal discomfort was recorded in a few patients in both treatment groups. The results suggest that the combination of pivmecillinam and pivampicillin is a promising alternative in patients with complicated urinary tract infections.

142. The Effect of Buserelin Versus Conventional Antiandrogenic Treatment in Patients with T2-4NXM1Prostatic Cancer

Abstract

A prospective, randomized multicentre phase III trial was undertaken to compare the effectiveness and safety of Buserelin, a gonadotropin-releasing hormone analogue (GnRHa), with conventional antiandrogenic treatment in patients with painful metastases from T2-4NXM1prostatic cancer. Seventy-two patients received Buserelin, 22 received estrogens and 46 were subjected to orchiectomy. The trial was completed one year after allocation of the patients. No significant differences as regards suppression of testosterone or survival were found in favour of one of the three treatment modalities. The performance index improved significantly both during the first months of treatment with Buserelin and following orchiectomy. No detectable improvement of performance index was seen during treatment with estrogens. Treatment with estrogens also failed to alleviate pain or general symptoms of cancer. Tolerability. safety and compliance of Buserelin was although administered intranasally clearly evidenced as palliation of advanced symptomatic cancer and the efficacy and sideeffects were fully comparable to those following orchiectomy.

143. Biosynthesis of the neural cell adhesion molecule: characterization of polypeptide C.

Abstract

The biosynthesis of the neural cell adhesion molecule (N-CAM) was studied in primary cultures of rat cerebral glial cells, cerebellar granule neurons, and skeletal muscle cells. The three cell types produced different N-CAM polypeptide patterns. Glial cells synthesized a 135,000 Mr polypeptide B and a 115,000 Mr polypeptide C, whereas neurons expressed a 200,000 Mr polypeptide A as well as polypeptide B. Skeletal muscle cells produced polypeptide B. The polypeptides synthesized by the three cell types were immunochemically identical. The membrane association of polypeptide C was investigated with methods that distinguish peripheral and integral membrane proteins. Polypeptide C was found to be a peripheral membrane protein, whereas polypeptides A and B were integral membrane proteins with cytoplasmic domains of approximately 50,000 and approximately 25,000 Mr, respectively. The affinity of the membrane binding of polypeptide C increased during postnatal development. The posttranslational modifications of polypeptide C were investigated in glial cell cultures, and it was found to be N-linked glycosylated and sulfated.

Abstract

SummaryA study was carried out to determine serum and renal tissue concentrations of antibiotics after a single intravenous injection of 1.0 g ampicillin plus 0.4 g mecillinam, given up to 4 hours before surgery, in 13 patients with normal or near normal kidney function who were undergoing nephrectomy. Both antibiotics showed higher tissue concentrations than the corresponding serum levels. The mean kidney:serum ratio was 2.4 for ampicillin and 1.4 for mecillinam. Serum half-lives were calculated to be 57 minutes for ampicillin and 52 minutes for mecillinam. Corresponding elimination half-lives in the kidney were 45 minutes and 44 minutes, respectively.

Abstract

This article discusses a study conducted in Denmark to improve the clinical management of adnexal masses, which are growths in the ovaries or fallopian tubes. The study compared the diagnostic efficiency of two ultrasound-based risk models, ADNEX and RMI, for making clinical decisions. The researchers implemented systematic ultrasound descriptions according to IOTA criteria in gynecological departments across the Central Denmark Region. The results showed that ADNEX had a higher diagnostic performance compared to RMI for identifying malignancy in adnexal masses. The study highlights the feasibility and advantages of implementing IOTA terminology and predictive models in a national setting, but also emphasizes the need for ultrasound education. Abstract

The study was designed to show whether there was any relation between muscle capillary basement membrane thickness, HLA-antigens, anti-insulin antibodies and proliferative retinopathy. Electron microscopic measurements of muscle capillary basement membrane thickness were performed on muscle biopsies from 15 insulin-dependent diabetics and severe proliferative retinopathy, 24 insulin-dependent diabetics with minimal retinopathy and 18 age- and sex matched non-diabetics. All the patients had had diabetes for 20 years or more. None had biochemical or clinical evidence of diabetic nephropathy. Basement membrane thickness was measured according to the methods of Siperstein and Williamson. Muscle capillary basement membrane thickening occurred in 32 of 39 diabetics, using the Siperstein method, but patients with proliferative retinopathy did not exhibit thicker basement membranes than patients with no or minimal changes in the retina. There were apparent differences in HLA-antigens between diabetics with and without proliferative retinopathy, but they did not reach statistical significance. There was no correlation between muscle capillary basement membrane thickness and the quantity of insulin antibodies. The results indicate that factors other than basement membrane thickening and genetic factors in the HLA-region, are responsible for the development of proliferative retinopathy.

150. Preservation of beta cell function in adult human pancreatic islets for several months in vitro

Abstract

Islets of Langerhans were isolated from four human kidney donors, aged 16 to 21 years by the collagenase method described for isolation of rodent islets. So far the human islets have been kept in tissue culture, without attachment, in medium RPMI 1640 supplemented with 10% calf serum for more than 9 months, with preservation of the ability to release insulin in response to glucose stimulation. Replacement of calf serum with serum from normal human subjects did not affect B-cell survival, but resulted in elevated insulin values partly due to lower insulin degrading activity. Thus the described technique presents a valuable tool for studying chronic effects of metabolites and hormones on islet function, as well as for islet storage prior to transplantation into humans.
