Dissertação de Mestrado em Estatística, Matemática e Computação apresentada à Universidade Aberta O evoluir do conhecimento e da tecnologia faz com que todos os dias melhoremos as nossas condições de vida. Este trabalho pretende mostrar como a aplicação de algumas técnicas estatísticas fornecem, orientações para melhor conhecermos o que nos rodeia, como por exemplo, o funcionamento de alguns mecanismos do nosso corpo. A utilização de técnicas estatísticas selecionadas já é, por si só, o resultado da evolução do conhecimento que se pretende abrangente e sem fronteiras científicas, culturais ou territoriais. Este trabalho reflete a aplicação de alguns métodos estatísticos na área da investigação em Saúde. São utilizados métodos paramétricos e não paramétricos e, para objetivos mais específicos, a regressão logística e a análise de correspondências (AC) sendo, este último, um dos temas da área da análise de dados que, nos dias de hoje, é alvo da atenção de grupos de cientistas internacionais, assistindo-se a uma crescente utilização. O estudo aqui apresentado pretende identificar e avaliar os principais parâmetros inflamatórios em doentes em hemodiálise e permite exemplificar, com a aplicação de dados reais, algumas das técnicas estatísticas mais apropriadas para a natureza deste tipo de dados e estudos. As principais variáveis em estudo são a proteína C-reativa, a albumina, a Kt/V (dose de diálise) e o tipo de acesso vascular. The evolution of knowledge and technology makes possible the daily improvement of our living conditions. This essay intends to show how the use of some statistical techniques provides guidelines to understand better what surrounds us, for example the functioning of some mechanisms of our body. The use of some selected statistic techniques is, on its own, the result of the evolution of knowledge, which is intended to be broad and without scientific, cultural or land borders. In this essay, one intended to exemplify the application of some statistic methods in the field of health research. Parametric and non-parametric methods are used and, for more specific goals, Logistic Regression and Correspondence Analysis, this last one being one of the themes of the data analysis field, which is nowadays the focus of the attention of groups of international scientists, and increasingly being used. The study presented here aims to identify and evaluate key inflammatory parameters in hemodialysis patients and enables an example, with application to real data, of some of the statistical techniques most appropriate to the nature of this kind of data and studies. The main variables under study are the C-reactive protein, Albumin, Kt/V (dialysis dose) and the type of Vascular access.

The Allergic Rhinitis and Its Impact on Asthma (ARIA) initiative started more than 20 years ago and has developed and disseminated evidence-based guidelines and projects in the field of allergic rhinitis. This initiative is currently focused on providing patient-centred guidelines that contribute to an integrated care pathway between the various levels of care and take advantage of digital solutions, and the introduction of integrated care pathways in clinical practice has been recommended. In this article we describe the adaptation for Portugal of the ARIA Integrated Care Pathways document. After a brief review of the epidemiology and impact of allergic rhinitis in Portugal and the activities carried out in Portugal within the ARIA initiative, we describe the broad knowledge base used for the development of recommendations for the pharmacological treatment of allergic rhinitis, and these recommendations are based on the GRADE methodology, real world evidence acquired by mobile technology (mHealth) and resulting from allergenic exposure chamber studies. What follows is a summary of integrated care pathways for allergen immunotherapy produced in 2019. Allergen immunotherapy is considered an example of precision medicine where the use of mHealth technologies will improve stratification for patient selection and response monitoring. These recommendations were considered as best practices of integrated patient-centred care supported by digital systems from Directorate General for Health and Food Safety of the European Union (DG Santé) and represent the ARIA Phase 4 Change Management strategy.

- Abstract
Digital anamorphosis is used to define a distorted image of health and care that may be viewed correctly using digital tools and strategies. MASK digital anamorphosis represents the process used by MASK to develop the digital transformation of health and care in rhinitis. It strengthens the ARIA change management strategy in the prevention and management of airway disease. The MASK strategy is based on validated digital tools. Using the MASK digital tool and the CARAT online enhanced clinical framework, solutions for practical steps of digital enhancement of care are proposed., (© 2020 EAACI and John Wiley and Sons A/S. - Abstract
Allergen immunotherapy (AIT) is a proven therapeutic option for the treatment of allergic rhinitis and/or asthma. Many guidelines or national practice guidelines have been produced but the evidence-based method varies, many are complex and none propose care pathways. This paper reviews care pathways for AIT using strict criteria and provides simple recommendations that can be used by all stakeholders including healthcare professionals. The decision to prescribe AIT for the patient should be individualized and based on the relevance of the allergens, the persistence of symptoms despite appropriate medications according to guidelines as well as the availability of good-quality and efficacious extracts. Allergen extracts cannot be regarded as generics. Immunotherapy is selected by specialists for stratified patients. There are no currently available validated biomarkers that can predict AIT success. In adolescents and adults, AIT should be reserved for patients with moderate/severe rhinitis or for those with moderate asthma who, despite appropriate pharmacotherapy and adherence, continue to exhibit exacerbations that appear to be related to allergen exposure, except in some specific cases. Immunotherapy may be even more advantageous in patients with multimorbidity. In children, AIT may prevent asthma onset in patients with rhinitis. mHealth tools are promising for the stratification and follow-up of patients., (© 2019 EAACI and John Wiley and Sons A/S. - Abstract
Background: Allergic rhinitis and asthma (ARA) are frequent respiratory diseases that often coexist, causing a high social and economic impact. It is important to maintain ARA disease control to reduce the disease burden., Objective: To assess control in patients with 1 or both pathologies through the application of validated questionnaires at community pharmacies., Methods: A cross-sectional study was conducted in the pharmacies of the Portuguese county of Covilhã (located in the central region of Portugal with about 53,000 inhabitants). Subjects aged between 18 and 70 years who presented a prescription for an asthma and/or allergic rhinitis medication were invited to participate in the study by responding to a questionnaire that included the Control of Allergic Rhinitis and Asthma Test (CARAT). The CARAT is a validated tool to simultaneously assess the control of ARA with scores that range from 0 to 30 points, 0 meaning the worst and 30 meaning the best possible control of disease., Results: Of the 224 participants, 58% were female and the median age was 48.5 years. The median CARAT score was 19 (mean = 17.8 ± 6.4), and 87% of participants had a score < 25, indicating noncontrolled disease. Female participants, the elderly, and those with less school education responded with scores that demonstrated significantly less disease control., Conclusions: Using a simple self-assessment questionnaire, such as the CARAT, pharmacists can help identify patients with uncontrolled ARA disease, which is an important first step to change patients' knowledge about their disease, with an ultimate goal of improving ARA outcomes.

- Abstract
Background: Rhinitis prevalence is increasing worldwide and is frequently associated with asthma, for which it is a risk factor. The aims of the study were to characterise the adult population with rhinitis attending the Cova da Beira Hospital Allergy Clinic, and to assess the relationship between rhinitis and asthma., Methods: In total, 686 patients were characterised by clinical history and anterior rhinoscopy, and classified according to international guidelines. Atopy was determined by skin prick testing to aeroallergens and quantification of specific IgE., Results: Seventy two percent of patients had allergic rhinitis (AR), and 28% had non-allergic rhinitis (NAR). NAR was more frequently associated with older age, perennial symptoms and female gender. NAR patients more frequently had bronchial asthma. In addition, more NAR than AR patients also had drug allergy, pharyngitis, sinusitis and urticaria. AR patients with nasal polyps more frequently had asthma. Grass pollen and mites were the major sensitisers for AR patients. Sensitisation profiles were not significantly different between urban- and rural-based AR patients., Conclusions: Asthma was more frequently associated with non-allergic than with allergic rhinitis. The two types of rhinitis did not differ in clinical severity. Although sensitisation profiles were not different between the urban and rural patients, allergic rhinitis prevalence was higher in urban patients.
