107. Single‐dose (4.5 mg/m2) gemtuzumab ozogamicin in combination with fludarabine, cytarabine and anthracycline as reinduction therapy in relapsed or refractory paediatric acute myeloid leukaemia.

Summary: Despite major therapeutic improvements, children with relapsed/refractory Acute Myeloid Leukaemia still have poor outcomes and overall survival does not exceed 40%. New treatments are required to improve their outcome; Gemtuzumab ozogamicin (GO), an anti‐CD33 immunoconjugate antibody, is a potent cytotoxic agent whose efficacy has been demonstrated mainly in adults. The main objective of this retrospective multicentre study was to assess the outcome of children treated, between February 2008 and August 2019, with GO at a single 4.5 mg/m2 dose, in combination with Fludarabine, Cytarabine and antssshracyclines, in context of a first relapse (n = 26) or refractory disease (n = 3). The remission rate was 83% (24/29 children) and 20 children (69%) were allografted. With a median follow‐up of 1.2 years (range: 0.1–8), the overall survival was 49% (CI95% = 33; 72). Most common adverse event was febrile neutropenia with microbiological identification in 55% of cases. Veno‐occlusive disease occurred in 6 patients (21%), of which 5 subvened after bone marrow transplantation, and resolved within 2–32 days (median 10.5 days). Administration of GO in combination with FLA‐anthracyclines chemotherapy appears to be a good reinduction regimen for relapsed or refractory AML with a good safety profile. International audience; BackgroundSafety data about rilpivirine use during pregnancy remain scarce, and rilpivirine plasma concentrations are reduced during second/third trimesters, with a potential risk of viral breakthroughs. Thus, French guidelines recommend switching to rilpivirine-free combinations (RFCs) during pregnancy.ObjectivesTo describe the characteristics of women initiating pregnancy while on rilpivirine and to compare the outcomes for virologically suppressed subjects continuing rilpivirine until delivery versus switching to an RFC.MethodsIn the ANRS-EPF French Perinatal cohort, we included women on rilpivirine at conception in 2010–18. OBJECTIVES: We investigated the role of household exposure to pesticides in the etiology of childhood hematopoietic malignancies. METHODS: The national registry-based case--control study ESCALE (Etude sur les cancers de l'enfant) was carried out in France over the period 2003-2004. Population controls were frequency matched with the cases on age and sex. Maternal household use of pesticides during pregnancy and paternal use during pregnancy or childhood were reported by the mothers in a structured telephone questionnaire. Insecticides (used at home, on pets, or for garden crops), herbicides, and fungicides were distinguished. We estimated odds ratios (ORs) using unconditional regression models closely adjusting for age, sex, degree of urbanization, and type of housing (flat or house). RESULTS: We included a total of 764 cases of acute leukemia (AL), 130 of Hodgkin lymphoma (HL), 166 of non-Hodgkin lymphoma (NHL), and 1,681 controls. Insecticide use during pregnancy was significantly associated with childhood AL [OR = 2.1; 95% confidence interval (CI), 1.7-2.5], both lymphoblastic and myeloblastic, NHL (OR = 1.8; 95% CI, 1.3-2.6), mainly for Burkitt lymphoma (OR = 2.7; 95% CI, 1.6-4.5), and mixed-cell HL (OR = 4.1; 95% CI, 1.4-11.8), but not nodular sclerosis HL (OR = 1.1; 95% CI, 0.6-1.9). Paternal household use of pesticides was also related to AL (OR = 1.5; 95% CI, 1.2-1.8) and NHL (OR = 1.7; 95% CI, 1.2-2.6); but for AL the relationships did not remain after adjustment for maternal pesticide use during pregnancy. CONCLUSION: The study findings strengthen the hypothesis that domestic use of pesticides may play a role in the etiology of childhood hematopoietic malignancies. The consistency of the findings with those of previous studies on AL raises the question of the advisability of preventing pesticide use by pregnant women. RESUME L’incidence du cancer et le nombre de sujets en vie apres un cancer augmentent. En 2008, en France, trois millions de personnes ayant eu un cancer sont en vie et 18 millions sont prevus en 2022. La reinsertion de ces patients dans la vie « normale » souleve des problemes societaux, familiaux et professionnels. Ces trois sujets sont abordes dans ce rapport. A la fin des traitements quel que soit l’impact psychologique du cancer, le retour a la vie « normale » passe par l’acceptation d’etre gueri de la peur de mourir. Cette acceptation est d’autant plus difficile qu’une surveillance est proposee et que la prise en charge du cancer en Affection de Longue Duree (ALD) se poursuit. Les patients doivent s’investir fortement avec si necessaire un soutien psychologique et une aide de l’Association Francaise de Soins Oncologiques de Support (AFSOS), de la convention tripartite signee entre pouvoirs publics, professionnels de la banque et assureurs (AERAS) qui leur permettra de s’assurer et d’emprunter si besoin et enfin des actions en faveur du retour a l emploi. Dans tous les cas un encouragement a l activite physique et des conseils alimentaires identiques a ceux donnes en prevention primaire doivent etre formules. Le maintien d’une cohesion familiale est important et passe par le retour a une vie sexuelle normale et une proposition de preservation de la fertilite avec conservation des gametes. Chez l’enfant, avant la puberte, une proposition de conservation de tissu germinal est possible mais sa reutilisation reste du domaine de la recherche. Ce rapport est un plaidoyer pour coordonner toutes les mesures existantes garantissant au maximum de patients un retour a la « vie normale ».

Objective. The purpose of this study was to determine the clinical and biological characteristics at diagnosis in children and adolescents with chronic myelogenous leukemia (CML) in contemporary practice. Methods. Analysis was conducted on data from 3 prospective trials conducted in children and adolescents with CML. Forty pediatric patients were evaluated in 16 French pediatric oncology units between 1991 and 2003. Results. The disease predominately affected children who were older than 10 years (67% of the patients), with a higher prevalence in boys than gifts (gender ratio: 1.5). Approximately 20% of cases were diagnosed incidentally. The main presenting symptoms were asthenia, weight loss, and complaints related to splenomegaly. Occasional patients presented with signs evocative of leukostasis. Symptoms were more common in patients with splenic enlargement, which was present in 70% of patients, and higher leukocyte counts. Markedly raised leukocyte counts were common (median white blood cell count: 242 x [10.sup.9]/L). The age and the gender of the patients had no effect on the leukocyte count, the hemoglobin level, or the platelet count. A predominance of b3a2 transcript was observed in the 16 children who were studied for the type of chimeric BCR-ABL mRNA. Conclusions. This is largest reported series of CML at diagnosis in children and adolescents. It shows that the characteristics of CML seem to differ in children compared with previously published adult series; in particular, the presenting leukocyte counts are often higher in children. - Abstract
To the editor: B-cell precursor acute lymphoblastic leukemia (BCP-ALL) is the most common childhood cancer and is molecularly heterogeneous. Identical twins with concordant BCP-ALL have provided a unique and tractable model to delineate the natural history and clonal evolution of the disease. Such studies have led to the 2-hit model for pediatric BCP-ALL, elucidated by Greaves, which provides unambiguous evidence that an initiating alteration, often occurring in utero, generates a preleukemic clone, which eventually gives rise to an overt leukemia upon acquisition of secondary cooperating mutations. Importantly, genome-wide sequencing has revealed a strikingly silent mutational landscape in childhood BCP-ALL, further indicative of a developmental cancer of prenatal origin, with a short window of time to accumulate molecular alterations.

- Abstract
An exome sequencing strategy employed to identify pathogenic variants in patients with pediatric-onset systemic lupus or Evans syndrome resulted in the discovery of six novel monoallelic mutations in PTPN2. PTPN2 is a phosphatase that acts as an essential negative regulator of the JAK/STAT pathways. All mutations led to a loss of PTPN2 regulatory function as evidenced by in vitro assays and by hyperproliferation of patients’ T cells. Furthermore, patients exhibited high serum levels of inflammatory cytokines, mimicking the profile observed in individuals with gain-of-function mutations in STAT factors. Flow cytometry analysis of patients’ blood cells revealed typical alterations associated with autoimmunity and all patients presented with autoantibodies. These findings further supported the notion that a loss of function in negative regulators of cytokine pathways can lead to a broad spectrum of autoimmune manifestations and that PTPN2 along with SOCS1 haploinsufficiency constitute a new group of monogenic autoimmune diseases that can benefit from targeted therapy.
