105. Search for an Association between V249I and T280M CX3CR1 Genetic Polymorphisms, Endothelial Injury and Preeclampsia: The ECLAXIR Study.

Abstract

Background: Preeclampsia and coronary-artery disease share risk factors, suggesting common pathophysiological mechanisms. CX3CR1/CX3CL1 mediates leukocyte migration and adhesion and has been implicated in the pathophysiology of several inflammatory diseases. M280/I249 variants of CX3CR1 are associated with an atheroprotective effect and reduced endothelial dysfunction. The aim of this study was to search for an association between V249I and T280M polymorphisms of CX3CR1, preeclampsia and endothelial dysfunction. Methodology/Principal Findings: We explored these polymorphisms with real-time polymerase chain reaction in a casecontrol study (184 white women with preeclampsia and 184 matched normotensive pregnant women). Endothelial dysfunction biomarkers including von Willebrand factor, VCAM-1 and thrombomodulin, as well as the soluble form of CX3CL1 were measured by enzyme-linked immunosorbent assays (ELISA). The I249 and M280 alleles were associated neither with preeclampsia, nor with its more severe form or with endothelial injury. In contrast, we found a trend toward increased CX3CL1 levels in preeclampsia patients, especially in early-onset- preeclampsia as compared to its level in later-onsetpreeclampsia. Conclusions/Significance: This is the first study to characterize the CX3CR1 gene polymorphisms in patients with preeclampsia. We found no differences in genotype or haplotype frequencies between patients with PE and normal pregnancies, suggesting that maternal CX3CR1 V249I and T280M polymorphisms do not increase susceptibility to preeclampsia. Further studies should be performed to directly evaluate the pathophysiological role of CX3CL1, a molecule abundantly expressed in endometrium, which has been shown to stimulate human trophoblast migration. Abstract

As we previously described, plasma infusion increased platelet count (PC) in four patients with IIB von Willebrand disease with severe thrombocytopenia. In a sixty years old patient in the same family, with chronic thrombocytopenia (PC = 30 000/ml) associated to an absence of large von Willebrand Factor multi-mers (vWF) in plasma, we successfully treated :1° A gastrointestinal bleeding episode with fresh frozen plasma infusion (15ml/Kg/day).2° Three months later a severe epistaxis with cryoprecipi-tate supernatant (15ml/Kg/day).During these bleeding episodes, the efficiency of these two treatments on the PC could be ascertained according to the following figureWe observed after ten days of these two treatments the following biological effects : a normalisation of vWF cross immunoelectrophoresis, of ristocetin induced normal platelet aggregation by patient's plasma, and of patient's plasma vWF binding to control platelets.In conclusion a factor appears to be present in both fresh frozen plasma and cryoprecipitate supernatant which prevents the abnormal binding of von Willebrand Factor (in this IIB von Willebrand disease) to the patient's platelets.

Abstract

This open, randomised multicenter trial compares the efficacy and safety of Fragmin administered subcutaneously twice daily with standard heparin administered by continuous infusion in the treatment of deep vein thrombosis (DVT). The initial dose of Fragmin is 100 U anti-Xa/kg/12 h and the further doses are adjusted according to the anti-Xa activity between 0.5 and 0.8 U/ml, 3 hours after the morning injection. The initial dose of standard heparin is 240 UI/kg/12 h. The dose adjustments are based on the daily results of APTT (1.5 - 3 times the control). Treatments efficacy are appreciated when comparing the venography performed before and after 10 days of treatment. The safety is evaluated on clinical parameters and iterative biological tests. Sixty-six patients have been included in this study. Efficacy of the two treatments is equivalent with a phlebographic improvement in respectively 79.3 p. 100 (Heparin Group) and 71.0 p. 100 (Fragmin Group) of the cases and an aggravation in 3.4 p. 100 and 6.4 p. 100 (NS) respectively. The frequency of dosage adjustments is lower and the stability of biological tests is better in the Fragmin group. In conclusion, the administration of Fragmin twice daily by subcutaneous route seems to be equivalent at least to standard heparin continuous infusion in the treatment of recent DVT. The better convenience and safety of Fragmin have to be verified on a larger panel of patients.

Abstract

Iodine concentrations were determined in 848 samples of milk from tanks of 537 dairy factories collected in 1983 and 1984 during winter and summer. Industrial processing, like pasteurization, UHT sterilization and spray-drying, had no effect on iodine concentrations. Of the samples of bovine milk, 94.7% ranged between 10 to 250 μg/kg. Iodine in bovine milk averaged 97 and 93 μg/kg in the winter of 1983 and 1984, and 44 and 23 μg/kg in the summer of 1983 and 1984. The frequency of iodine concentrations above the 500 μg/kg limit was 0.5% in 1983 and 0.3% in 1984. Iodine in ovine milk varied from 116 to 1183 μg/kg. Of the ovine milk samples 53.3% were above 500 μg/kg. Thus the iodine content of milk, is not yet a health problem in France.

Abstract

Efficacy of a low molecular weight heparin (Kabi 2165) was compared with that of non-fractionated heparin in the prevention of abdominal surgery postoperative venous thrombosis by means of a double-blind randomized trial in 79 patients. Determination of D. dimers in these patients allowed assessment of specificity of this assay for detection of postoperative venous thrombosis.

124. Économie marchande et développement de la coopération aux Iles-de-la-Madeleine : enjeux et stratégies à Havre-Aubert

Abstract

Étude de cas de l’implantation de coopératives de pêcheurs à Havre-Aubert (Iles-de-la-Madeleine, Québec) et répercussions sur l’économie marchande en se souciant de replacer cette innovation dans le cadre historique des conditions sociales, économiques et politiques associées à leur émergence et à leur évolution, non seulement au niveau local et régional, mais aussi dans le contexte international nord-américain et européen du marché du poisson. Si les structures coopératives se sont établies en respectant les structures traditionnelles de la parenté, du voisinage (« rangs-de-pêche ») et du « canton », elles ont bénéficié du contexte éminemment favorable de la crise économique mondiale de 1929 et de l’affaissement momentané de l’économie marchande. Les coopératives ont libéré les pêcheurs des liens de dépendance vis-à-vis de l’économie marchande (de troc) et les ont fait pénétrer dans une économie monétaire à échanges libres et concurrentiels. Avec l’arrivée de la Seconde guerre mondiale, les pêcheurs ont connu une période de prospérité qui s’est accompagnée d’une évolution technique (nécessité d’investissements accrus) et d’une transformation des besoins de consommation (achat à crédit). Afin de concurrencer les marchands, les coopératives ont payé les prises de poisson comptant et ont soutenu l’achat des biens de consommation à crédit. Ce faisant, elles ont compromis leur propre survie (manque de solidité financière suffisante pour soutenir les pratiques courantes d’achat et de vente) et elles ont enfermé les pêcheurs dans un autre système de dépendance qui, bien qu’étant moins rigoureux que celui de l’économie marchande, leur laissait peu de liberté de manoeuvre. Une fausse identification des véritables enjeux coopératifs par les leaders les a conduit à élaborer des stratégies d’action inappropriées. Le succès partiel de l’innovation coopérative s’explique encore par plusieurs autres conditions sociales, économiques et politiques locales et régionales documentées dans cette étude. A ce propos, on accorde une attention particulière aux difficultés associées au processus de régionalisation des coopératives., A case study of the introduction of fishing cooperatives at Havre-Aubert (Magdelen Islands, Quebec) and of the ensuing consequences on the traditional trade economy. The innovating process is being replaced within the historical context of the social, economic and political conditions which were associated to its birth and evolution not only at the local and regional levels, but also within the wider north-american and european international fish market. If cooperative structures, in order to be successful, based themselves upon the traditional kinship (extended family), neighborhood (« Rangs-de-pêche ») and « canton » pattems, they also took advantage of the highly favorable conditions of the Great economic crisis of 1929 and of the temporary breakdown of the trade economy. Cooperatives freed fishermen from their dependency upon the trade economy and brought them in a free enterprise monetary economy. The Second World War brought back prosperity to fishermen which was accompanied by technological changes in the fishing industry (new investments became imperative) and by profound changes in consumers’ needs. In order to become competitive with traders, cooperatives paid fish catches in cash and backed up the consuming credit patterns of fishermen. In so doing, they endangered their own survival (the lack of financial solidity to allow for their current purchasing and selling practices) and confined fishermen in another dependency system which gave them little freedom of action, although it was less rigorous than the dependency system of the trade economy. A false identification by lay leaders of real cooperation targets led them to elaborate inappropriate action strategies. The partial success of the cooperative innovation is being explained by a large number of other social, economic and political conditions, at the local and regional levels, analyzed in this study. Of particular interest are the difficulties associated with the regionalization process of cooperatives.

140. FRANCE. Paris. François MITTERRAND posing for his bust by sculptor Daniel DRUET. The President of the French Republic posed for ten sittings for the sculptor Daniel DRUET. During the 8th sitting the sculptor, unsatisfied with his work, destroyed the bust. The final sculpture, is realized during these last two sittings a year and a half later. One hundred years after the Massacre at Wounded Knee, Sioux Lakota Indians trace the route of Chief Big Foot and their ancestors as they fled their reservation after the killing of Chief Sitting Bull by the U.S. Cavalry. The memorial ride, 'Si Tanka Wokiksuye,' commemorates the Indians' journey in search of safety that ultimately ended in their slaughter at Wounded Knee on December 29, 1890. Chief Big Foot, 25 U.S. soldiers, and more than 250 Lakota men, women, and children were killed in the attack. The Massacre at Wounded Knee was the last of the American Indian wars.
