103. Renal function and venous thromboembolic diseases

Abstract

Anticoagulant agents have been approved by international regulatory agencies to prevent and treat venous thromboembolism (VTE). However, chronic kidney disease (CKD) is: (1) highly frequent in VTE patients; (2) strongly linked to VTE; and (3) a risk factor for cardiovascular morbidity/mortality and fatal pulmonary embolism. Therefore, an increasing number of patients are presented with CKD and VTE and more and more physicians must face the questions of the management of these patients and that of the handling of anticoagulant agents in CKD patients because of the pharmacokinetic modifications of these drugs in this population. These modifications may lead to overdosage and dose-related side effects, such as bleeding. It is therefore necessary to screen VTE patients for CKD and to modify the doses of anticoagulants, if necessary.

Les antivitamiques K (AVK) et les anticoagulants oraux directs (AOD) sont aujourd’hui en concurrence. Les arguments développés autour des AOD amènent à se poser la question de savoir si les AVK doivent totalement disparaître au profit des AOD, ou du moins ne garder que les niches laissées par les contre-indications des AOD.Les questions que suscite l’arrivée des AOD sont nombreuses. Le but de cette mise au point est de tenter d’apporter une vision objective. Il ne s’agit pas de dénigrer l’un ou l’autre mais de pouvoir choisir entre ces deux familles d’anticoagulants afin que tout prescripteur puisse utiliser l’un ou l’autre en toute connaissance de cause et en toute sécurité.

108. Stock enhancement or sea ranching? Insights from monitoring the genetic diversity, relatedness and effective population size in a seeded great scallop population (Pecten maximus)

Abstract

The mass release of hatchery-propagated stocks raises numerous questions concerning its efficiency in terms of local recruitment and effect on the genetic diversity of wild populations. A seeding program, consisting of mass release of hatchery-produced juveniles in the local naturally occurring population of great scallops (Pecten maximus L.), was initiated in the early 1980s in the Bay of Brest (France). The present study aims at evaluating whether this seeding program leads to actual population enhancement, with detectable effects on genetic diversity and effective population size, or consists of sea ranching with limited genetic consequences on the wild stock. To address this question, microsatellite-based genetic monitoring of three hatchery-born and naturally recruited populations was conducted over a 5-year period. Results showed a limited reduction in allelic richness but a strong alteration of allelic frequencies in hatchery populations, while genetic diversity appeared very stable over time in the wild populations. A temporal increase in relatedness was observed in both cultured stock and wild populations. Effective population size (Ne) estimates were low and variable in the wild population. Moreover, the application of the Ryman-Laikre model suggested a high contribution of hatchery-born scallops to the reproductive output of the wild population. Overall, the data suggest that the main objective of the seeding program, which is stock enhancement, is fulfilled. Moreover, gene flow from surrounding populations and/or the reproductive input of undetected sub-populations within the bay may buffer the Ryman-Laikre effect and ensure the retention of the local genetic variability.

Abstract

The Chondrostoma genus is widespread in Europe, with numerous endemic species in northern Mediterranean rivers. We reconstructed the phylogenetic relationships of this genus, using the whole cytochrome b sequence and compared the two freshwater fish dispersion hypotheses: (1) dispersion around the Mediterranean Sea during the Lago Mare phase of the Messinian salinity crisis (Bianco’s hypothesis) and (2) an older and more gradual colonization of the Mediterranean rivers (Banarescu’s hypothesis). All phylogenetic analyses identified two levels of divergences, implying two radiation events in the Chondrostoma genus. The first radiation mainly concerned Mediterranean species, whereas the second one includes Danubian and Mesopotamian species. This phylogeographic pattern was already observed for the genus Squalius, which exhibits a similar geographic range distribution in Europe and probably is shared with several other Mediterranean genera, such as Scardinius, Rutilus, and Telestes. Furthermore, assuming a molecular clock of 1% per million years, the first radiation appears consistent with a Messinian dispersion during the Lago Mare, 5.3 million years ago, whereas the second one may correspond to a Mesopotamian dispersion through the Black Sea to the Danube system. According to our results, the Lago Mare theory is strengthened, and a more recent and pre-Pleistocene colonization of the Danube from Mesopotamian freshwater fishes is suggested

Abstract

Reverse Osmosis is the dominant technology in water desalination. However, some critical issues remain open: improvement of water quality, enhancement of the recovery factor, reduction of the unit water cost, minimizing the brine disposal impact. This book aims to solve these problems with an innovative approach based on the integration of different membrane operations in pre-treatment and post-treatment stages. Membrane-Based Desalination: An Integrated Approach (acronym MEDINA) has been a three years project funded by the European Commission within the 6th Framework Program. The project team has developed a work programme aiming to improve the current design and operation practices of membrane systems used for water desalination, trying to solve or, at least, to decrease the critical issues of sea and brackish water desalination systems. In the book, the main results achieved in the nine Work Packages constituting the project will be described, and dismissed by the leaders of the various WPs. The following areas are explored in the book: * the development of advanced analytical methods for feed water characterization, appropriate fouling indicators and prediction tools, procedures and protocols at full-scale desalination facilities; * the identification of optimal seawater pre-treatment strategies by designing advanced hybrid membrane processes (submerged hollow fibre filtration/reaction, adsorption/ion exchange/ozonation) and comparison with conventional methods; * the optimisation of RO membrane module configuration, cleaning strategies, reduction of scaling potential by NF; * the development of strategies aiming to approach the concept of Zero Liquid Discharge (increasing the water recovery factor up to 95% by using Membrane Distillation - MD; bringing concentrates to solids by Membrane Crystallization or Wind Intensified Enhanced Evaporation) and to reduce the brine disposal environmental impact and cost; * increase the sustainability of desalination process by reducing energy consumption(evaluation of MD, demonstration of a new energy recovery device for SWRO installations)and u se of renewable energy (wind and solar).

Abstract

The cyanobacterium Trichodesmium is an important link in the global nitrogen cycle due to its significant input of atmospheric nitrogen to the ocean. Attempts to incorporate Trichodesmium in ocean biogeochemical circulation models have, so far, relied on the observed correlation between temperature and Trichodesmium abundance. This correlation may result in part from a direct effect of temperature on Trichodesmium growth rates through the control of cellular biochemical processes, or indirectly through temperature influence on mixed layer depth, light and nutrient regimes. Here we present results indicating that the observed correlation of Trichodesmium with temperature in the field reflects primarily the direct physiological effects of temperature on diazotrophic growth of Trichodesmium. Trichodesmium IMS-101 (an isolate of Trichodesmium) could acclimate and grow at temperatures ranging from 20 to 34°C. Maximum growth rates (μmax=0.25 day–1) and maximum nitrogen fixation rates (0.13 mmol N mol POC−1 h–1) were measured within 24 to 30°C. Combining this empirical relationship with global warming scenarios derived from state-of-the-art climate models sets a physiological constraint on the future distribution of Trichodesmium that could significantly affect the future nitrogen input into oligotrophic waters by this diazotroph.

Abstract

International audience; Thromboembolic events are an increasingly commonly recognized secondary complications in children treated for serious, life-threatening primary diseases. Nevertheless the incidence of venous thromboembolic disease remains 100 times less frequent in hospitalized children than hospitalized adults, as recent data suggest an incidence of 5,3/10.000 pediatric patients compared to an incidence of 2,5-5/100 in adult patients. Diseases usually associated with thromboembolic events in children are neoplasia, autoimmune or cardiac malformative disease. Contrarily to what is observed in the adult, the majority of deep vein thrombosis events occur at the upper limbs veins, usually at the place where devices for venous access like port-a-cath or central venous lines are inserted. Because of the relatively low incidence of venous thromboembolic events in children, the diagnostic approach used are largely extrapolated from guidelines obtained from adult studies. However, the diagnostic performances of some diagnostic tools like Doppler-Ultrasound are probably diminished in children. Moreover, the concept of developmental hemostasis, which stresses the point that the hemostatic system in children is quite different from the adult one, is widely accepted and clearly suggests that the diagnostic and therapeutic approach in pediatric patients may not be simply extrapolated from data obtained in adult studies. From a more practical point of view, the fact that the hemostatic system in children is a dynamic and evolving system, renders the therapeutic approach quite complex. In particular, doses of anticoagulants vary markedly across the pediatric age. The absence of adapted formulations of commonly used anticoagulants, for example the absence of liquid formulations of anti-vitamin K drugs, further complicates the administration and the correct monitoring of anticoagulation in children, and may diminish observance in adolescent patients. Even though the concept that "children are not little adults" is nowadays widely accepted, there is an urgent need for prospective studies to better assess the modalities of diagnosis and treatment of venous thromboembolic disease in this particular population.

Abstract

In the Middle East, species of Garra are believed to have invaded the area in two independent waves from the Indo-Malayan biogeographic region. This hypothesis is based on the structure of the mental disc, a unique specialization of the lower lip, which is believed to be an adaptation to fast-flowing waters. While several species have such a mental disc, others completely lack a mental disc, being adapted to slow-moving water or to subterranean life. In this study, the phylogenetic relationships of Middle Eastern Garra species, including 16 described and 4 undescribed species, were analysed using mitochondrial cytochrome c oxidase I sequences. The results are concordant with traditional hypotheses on two invasion events; however, these invasion events are independent from the presence, absence or shape of the mental disc. We postulate convergent reduction of the mental disc in 5-6 independent lineages of Garra in the Middle East. [ABSTRACT FROM AUTHOR]

Abstract

We present recent results on the direct heterogeneous integration of InP HBTs and Si CMOS on a silicon template wafer or SOLES (Silicon On Lattice Engineered Substrate). InP HBTs whose performance are comparable to HBTs on the native InP substrates have been repeatedly achieved. 100% heterogeneous interconnect yield has been achieved on daisy chain test structures with CMOS-InP HBT spacing (interconnect length) as small as 2.5um. In DARPA COSMOS Phase 1 we designed and fabricated a differential amplifier that met the program Go/NoGo metrics with first pass design success. As the COSMOS Phase 2 demonstration vehicle we designed and fabricated a low power dissipation, high resolution, 500MHz bandwidth digital-to-analog converter (DAC)., United States. Abstract

Marine N2 fixing microorganisms, termed diazotrophs, are a key functional group in marine pelagic ecosystems. The biological fixation of dinitrogen (N2) to bioavailable nitrogen provides an important new source of nitrogen for pelagic marine ecosystems and influences primary productivity and organic matter export to the deep ocean. As one of a series of efforts to collect biomass and rates specific to different phytoplankton functional groups, we have constructed a database on diazotrophic organisms in the global pelagic upper ocean by compiling about 12 000 direct field measurements of cyanobacterial diazotroph abundances (based on microscopic cell counts or qPCR assays targeting the nifH genes) and N2 fixation rates. Biomass conversion factors are estimated based on cell sizes to convert abundance data to diazotrophic biomass. The database is limited spatially, lacking large regions of the ocean especially in the Indian Ocean. The data are approximately log-normal distributed, and large variances exist in most sub-databases with non-zero values differing 5 to 8 orders of magnitude. Reporting the geometric mean and the range of one geometric standard error below and above the geometric mean, the pelagic N2 fixation rate in the global ocean is estimated to be 62 (52–73) Tg N yr−1 and the pelagic diazotrophic biomass in the global ocean is estimated to be 2.1 (1.4–3.1) Tg C from cell counts and to 89 (43–150) Tg C from nifH-based abundances. Reporting the arithmetic mean and one standard error instead, these three global estimates are 140 ± 9.2 Tg N yr−1, 18 ± 1.8 Tg C and 590 ± 70 Tg C, respectively. Uncertainties related to biomass conversion factors can change the estimate of geometric mean pelagic diazotrophic biomass in the global ocean by about ±70%.
