Abstract

AIMS: Increased exposure to fondaparinux, as observed in patients with renal impairment, may increase bleeding risk. This study aims to determine the time course of major bleeding after major orthopaedic surgery, identify predictors of bleeding and simulate the effect of a reduced dose of fondaparinux on bleeding for patients with moderate renal impairment (creatinine clearance = 20–50 ml min(−1)). METHODS: Data including fondaparinux anti‐Xa activities from two multicentre prospective cohorts were used. In the first cohort, patients (n = 957) received fondaparinux 2.5 mg once a day. In the second, patients with moderate renal impairment (n = 436) received 1.5 mg once per day. The time‐to‐major bleeding after the end of surgery was modelled using a parametric survival analysis in NONMEM. RESULTS: The observed rate of major bleeding up to day 11 was 5.2%. The time‐to‐event analysis indicated that the hazard of bleeding was highest in the first days following surgery and then remained low thereafter. Independent significant predictors of an increased hazard of major bleeding were male sex, lower body weight and increased drug exposure. Simulated rates of major bleeding up to day 11 in patients with moderate renal impairment were 6.5% with fondaparinux 2.5 mg once daily and 3.8% with fondaparinux 1.5 mg once daily. CONCLUSION: The hazard of major bleeding is highest in the first postoperative days and increases with fondaparinux exposure. To reduce the risk of bleeding in patients with moderate renal impairment, this study supports the use of a lower dose of fondaparinux 1.5 mg once daily.

The aim of this study was to distinguish the passive and the active phases during the pre-impact period of a fall. It was hypothesized that it should be feasible if one uses a specific quantitative kinematic analysis to evaluate the dispersion of the body segments trajectories across trials. Moreover, a multi-joint kinematical model was created for each subject, based on a new 3-D minimally invasive stereoradiographic X-ray images to assess subject-specific geometry and inertial parameters. The simulations allowed discriminating between the contributions of the passive (inertia-induced properties) and the active (neuromuscular response) components during falls. Our data show that there is limited time to adjust the way one fall from a standing position. We showed that the pre-impact period is truncated of 200 ms. During the initial part of a fall, the observed trajectory results from the interaction between the destabilizing external force and the body: inertial properties intrinsic to joints, ligaments and musculotendinous system have then a major contribution, as suggested for the regulation of static upright stance. This passive phase is later followed by an active phase, which consists of a corrective response to the postural perturbation. We believe that during a fall from standing height, it takes about 300 ms for postural responses to start correcting the body trajectory, while the impact is expected to occur around 700 ms. It has been argued that this time is sufficient to change the way one falls and that this makes it possible to apply safer ways of falling, for example by using martial arts fall techniques. Abstract

In research on L2 English, recent corpus-based studies indicate that some non- standard forms are shared by indigenized (ESL) and foreign (EFL) varieties of English, which challenges the idea of a clear dichotomy between innovation and error. We present a data-driven large-scale method to detect innovations, test it on verb + preposition structures (including phrasal verbs) and adjective + preposition structures, and describe similarities and differences between EFL and ESL. We use a dependency-parsed version of the International Corpus of Learner English to automatically extract potential innovations, defined as patterns of overuse compared to the British National Corpus as reference corpus. We measure overuse by means of collocation measures like O/E or T-score, and compare our results with similar results for ESL. In both quantitative and qualitative analyses, we detect similarities between the two varieties (e.g. discuss about) and dissimilarities (e.g. accuse for, only distinctive for EFL). We report more verb/adjective + preposition combinations than previous studies and discuss the roles of analogy and transfer.

Abstract

Le changement climatique exige une remise en question des pratiques d’urbanisation. Les environnements urbains influencent la relation des résidents avec les espaces extérieurs et intérieurs, avec des conséquences sur la consommation d’énergie et les rejets de chaleur à l’intérieur de ses limites. Les avantages de l’éclaircissement des revêtements de sol des espaces publics pour réduire les îlots de chaleur urbains ont été confirmés par la littérature scientifique, mais les effets de rebond en termes d’augmentation de la sensation thermique extérieure (OTS) doivent encore être analysés par des expériences empiriques. Cette étude présente les résultats d’une expérience participative de confort thermique impliquant 75 volontaires exprimant des votes de sensation thermique (TSV) à l’aveugle sur des revêtements de surface de différentes réflectivités. Chaque TSV était lié aux caractéristiques physiques et psychologiques du sujet, à son emplacement et aux conditions microclimatiques au moment du vote. 714 chaînes de données collectées lors de la canicule d’août 2022 ont été traitées par des méthodes d’intelligence artificielle (IA) successives : apprentissage supervisé, explications additives de Shapley, réduction de dimension par approximation et projection uniforme de la variété (UMAP), et regroupement hiérarchique basé sur la densité des applications avec bruit (HDBSCAN). Ce post-traitement a permis de définir des clusters caractérisés par une répartition similaire des 10 variables les plus influentes sur le TSV, résultant en des valeurs TSV similaires. Les calculs d’IA et l’analyse des graphiques sur DesignExplorer ont montré que les pavés réfléchissants aggravent le stress thermique des personnes. Ces résultats montrent que l’éclaircissement des espaces publics, souvent employé dans de nombreux réaménagements urbains, n’est pas une solution adaptée pour protéger les citoyens des risques sanitaires du changement climatique.

Abstract

Idraparinux, a long acting inhibitor of factor (F) Xa, is as effective as standard anticoagulant therapy for patients with symptomatic deep venous thrombosis. We investigated the potential use of the biotinylated molecule, idrabiotaparinux. Biotinylation enables reversal of the anticoagulant effect.We performed a randomized double-blind trial in 757 patients with symptomatic deep venous thrombosis, comparing equimolar doses of idrabiotaparinux (3 mg) with idraparinux (2.5 mg), both given subcutaneously, once weekly for 6 months. Inhibition of FXa activity was measured at days 15, 36, 57, 92 and 183. The efficacy outcome was recurrent venous thromboembolism. The safety outcomes were clinically relevant bleeding and death.Inhibition of FXa was similar in the two treatment groups at each time point of measurement. Recurrent venous thromboembolism during the 6-month treatment period occurred in nine of 386 patients (2.3%) in the idrabiotaparinux group and in 12 of 371 patients (3.2%) in the idraparinux group, a difference of - 0.9% (95% confidence interval, -3.2-1.4%). The incidence of clinically relevant bleeding was 5.2% in the idrabiotaparinux group and 7.3% in the idraparinux group (P = 0.29), a difference of - 2.1% (95% confidence interval, -5.6-1.4%). Six patients (1.6%) who received idrabiotaparinux died, compared with 12 patients (3.2%) given idraparinux, a difference of - 1.7% (95% confidence interval, -3.9-0.5%).Idrabiotaparinux has a similar time course of FXa inhibition, efficacy and safety to idraparinux for the treatment of deep venous thrombosis.? 2010 International Society on Thrombosis and Haemostasis.

Abstract

Le Groupe d’intérêt en hémostase périopératoire (GIHP) et le Groupe français d’études sur l’hémostase et la thrombose (GFHT) en collaboration avec la Société française d’anesthésie et de réanimation (SFAR) ont fait des propositions sur la gestion des agents antiplaquettaires (AAP) en cas de procédure invasive non programmée ou d’hémorragie. Ces propositions ont été discutées puis validées par un vote, elles font toutes l’objet d’un accord fort. La gestion des AAP en urgence nécessite de prendre en compte leurs caractéristiques pharmacocinétiques et pharmacodynamiques, d’évaluer l’affaiblissement de la compétence hémostatique liée aux AAP et le risque hémorragique qu’il entraîne. Les tests fonctionnels plaquettaires peuvent aider à cette évaluation. Lorsque le risque hémorragique lié aux AAP est susceptible d’aggraver le pronostic, la neutralisation des AAP doit être envisagée, en prenant en compte l’efficacité des moyens de neutralisation (qui sont limités pour le prasugrel et le ticagrelor) mais aussi les risques associés à ces moyens. Ceux-ci incluent la transfusion plaquettaire, à des doses adaptées à l’AAP considéré, et les agents hémostatiques (facteur VII activé recombinant et acide tranexamique). Pour les procédures invasives non programmées, le report de quelques jours, voire de quelques heures, doit être envisagé lorsqu’il ne compromet pas le pronostic vital ou fonctionnel du patient, jusqu’à élimination ou diminution suffisante de l’effet de l’AAP ou de son métabolite actif.
