102. Correction: Combining MAD and CPAP as an effective strategy for treating patients with severe sleep apnea intolerant to high-pressure PAP and unresponsive to MAD

Abstract

This study aimed to determine the effect of combining positive airway pressure (PAP) therapy and mandibular advancement device (MAD) in patients with severe obstructive sleep apnea (OSA) who were pressure intolerant for PAP and were unresponsive to MAD.This retrospective study reviewed the medical records of severe OSA patients with apnea-hypopnea index (AHI) ≥ 30/hr who were diagnosed between October 1, 2008 and June 30, 2014. Patients were initially treated with 2 weeks of PAP, and those who were intolerant to high-pressure PAP (≥15 cm H2O) were switched to 12 weeks of MAD, which is a monobloc designed at 75% of maximum protrusion. Patients who had high residual AHI (≥15/hr) on MAD underwent 12 weeks of combination therapy (CT) with MAD and CPAP and were enrolled in the present study. Enrolled subjects who completed the 12-week CT were followed-up until June 30, 2016.A total of 14 male patients were included. All three treatments effectively reduced AHI, oxygen desaturation index (ODI), and total sleep time with SpO290% (% TST-SpO290%) compared to pretreatment values. The residual AHI and ODI on CT was lower than that on MAD or PAP. The residual % TST-SpO290% was lower than that on MAD and similar to that on PAP. The therapeutic pressure on CT was on average 9.2 cm H2O lower than that on PAP. For the 11 patients who completed CT, only CT reduced ESS compared to pretreatment value. No treatment had significant impact on % slow wave sleep or overnight change of blood pressure. For patients who completed CT, the average usage was 5.9±1.7 hr/night at 12th week and 6.4±1.5 hr/night at a median follow-up of 36.5-months.Combining MAD and CPAP showed additive effects on reducing AHI and ODI, and lowered the therapeutic pressures.

Abstract

We have demonstrated the fabrication of homogeneously distributed In0.3Ga0.7N/GaN quantum nanodisks (QNDs) with a high density and average diameter of 10 nm or less in 30-nm-high nanopillars. The scalable top-down nanofabrication process used biotemplates that were spin-coated on an In0.3Ga0.7N/GaN single quantum well (SQW) followed by low-damage dry etching on ferritins with 7 nm diameter iron cores. The photoluminescence measurements at 70 K showed a blue shift of quantum energy of 420 meV from the In0.3Ga0.7N/GaN SQW to the QND. The internal quantum efficiency of the In0.3Ga0.7N/GaN QND was 100 times that of the SQW. A significant reduction in the quantum-confined Stark effect in the QND structure was observed, which concurred with the numerical simulation using a 3D Schrödinger equation. These results pave the way for the fabrication of large-scale III–N quantum devices using nanoprocessing, which is vital for optoelectronic communication devices.

124. The efficacy of chitosan-based coating in reducing surface attached Salmonella on tomatoes

Abstract

Recently, an increasing number of outbreaks related to produce consumption have been reported. Tomatoes are considered as one of the main produce consumed in the United States; approximately 14 million tons of tomatoes are produced every year. However, the microbial safety of raw tomatoes has been questioned following many outbreaks associated with eating tomatoes contaminated with human pathogens. The CDC reports that eating raw tomatoes caused at least 12 Salmonella multistate outbreaks from 1990-2009, and resulted in over 2,000 illnesses. Therefore, developing a novel and alternative method to improve the microbial safety and quality of raw tomatoes is necessary. In the U.S., post-harvest tomatoes are often washed in chlorinated water (50-150 ppm) to remove debris, soil and prevent cross-contamination, with an additional effect of lowering the microbial load on the tomatoes. However, previous studies showed that tomatoes washed with chlorinated water (320 ppm) failed to completely inactivate Salmonella. Chitosan is a non-toxic, bio-degradable and antimicrobial compound, which has been reported for its efficacy of extending the shelf-life and reducing the microbial decay of fresh produce. The objective of this study is to investigate the efficacy of chitosan-based coating against surface attached Salmonella and natural microbiota on fresh tomatoes. All tomatoes were inoculated with Salmonella. After drying, tomatoes were washed in 100 ppm chlorinated water for 90 s. Tomatoes were then dipped in either 0.2% or 1.0% chitosan solution for 90 s. Controls were only dipped in tap water. Change in population of Salmonella and total aerobic microbes (Aerobic Plate Count, APC) were evaluated at pre-determined times. Results showed that the population of Salmonella and total aerobic microbes with the treatment of 1.0% chitosan solution following 100 ppm chlorinated wash step provided immediate inactivation on day 0, compared to control. On day 1 and day 5 post-treatment, the population of Salmonella and total aerobic microbes on tomatoes both increased during the five days of storage, except on tomatoes treated with 1.0% chitosan solution following 100 ppm chlorine wash step. Future research should consider evaluating up to 4.0% chitosan solutions for controlling microbial growth and extending the shelf-life of tomatoes.

125. Concomitant Genetic Alterations are Associated with Worse Clinical Outcome in EGFRMutant NSCLC Patients Treated with Tyrosine Kinase Inhibitors

Abstract

Epidermal growth factor receptor- tyrosine kinase inhibitors (EGFR-TKI) are recommended first-line therapy for advanced non-small cell lung cancer (NSCLC) with sensitizing EGFRmutations. It is of clinical interest to identify concurrent genetic mutations in NSCLC patients with EGFRmutations in the hopes of discovering predictive biomarkers towards EGFR-TKI treatment. We retrospectively analyzed a cohort of patients with advanced EGFRmutant NSCLC who underwent treatment with first generation TKIs at our hospital by a multi-gene panel via next generation sequencing. A total of 33 patients with mutant EGFR were enrolled. Up to 26 (78.8%) patients had at least one concomitant genetic alteration coexisting with mutant EGFR. Among the concomitant genetic alterations discovered, TP53mutation was most common (n = 10,30.3%), followed by CDK4 (n = 8, 24.2%) and CDKN2A (n = 7, 21.2%)copy number variation (CNV). Progression-free survival was shorter in patients with concomitant FGFR3mutation (1.6 vs. 12.6 months, P = .003) and CDKN2ACNV loss (6.5 vs. 13.4months, P = .019). Patients with any concomitant genetic alterations also had significant worse overall survival (24.1 vs. 40.8 months, P = .029). In summary, our study revealed an unfavorable association between concomitant genetic mutations and treatment response towards EGFR-TKI. FGFR3 mutation and CDKN2A CNV loss may be potential predictive markers for treatment outcome and warrant further investigation.

Abstract

The palladacycle complex [LsPdOAc]2 bearing 2‐phenyl benzothiazole was synthesized and characterized by NMR and X‐ray crystallography. [LsPdOAc]2 was used as a catalyst in the Suzuki–Miyaura cross coupling reaction of 4‐bromotoluene with phenylboronic acid, which resulted in a conversion of >90% with 5 mol% of the Pd complex within 10 min at 60°C. The palladacycle complex [LsPdOAc]2 bearing 2‐phenyl benzothiazole was used as a catalyst in the Suzuki–Miyaura cross coupling reaction. In a case of 4‐bomotoluene and phenylboronic acid with Cs2CO3 as a base in a 1:1 mixture solvent (3 mL) of DMF + H2O at 60 °C after 10 min, the conversion is 90 %. [ABSTRACT FROM AUTHOR]

Abstract

Abstract: Aim: Lung cancer is typically categorized into small cell lung cancer (SCLC) and non–small cell lung cancer (NSCLC). NSCLC comprises of the majority of lung cancer with a poor prognosis in advanced cases. Transcriptional profiling studies, including microarrays and RNA‐sequencing studies, have significantly enriched our knowledge of gene expression patterns in NSCLC. A recent transcriptional profiling study identified high prevalence of CBX3/HP1‐gamma upregulation in human NSCLC samples. CBX3/HP1‐gamma is an isoform of the heterochromatin protein 1 family, which plays a role in heterochromatin formation and is linked to cancer. Methods: We examined lung cancer samples from our hospital using immunohistochemistry for CBX3/HP1‐gamma staining. We also analyzed publicly available databases of NSCLC transcriptional profiling to validate our results. Results: We identified a high prevalence (77.2%) of samples with positive CBX3/HP1‐gamma staining by immunohistochemistry in NSCLC patient samples. Independently, we queried a publicly available dataset (GSE40419) containing RNA‐seq data from 77 patients. Upregulation of CBX3/HP1‐gamma in tumor samples was present in 60.2% of the patients. A similar correlation was also observed in the The Cancer Genome Atlas (TCGA) database. Interestingly, we discovered a highly significant association between positive CBX3/HP1‐gamma staining and EGFR mutation in our patient samples (40 of 42 patients, P < 0.001). Treatment of EGFR mutant NSCLC cell lines with the EGFR inhibitor gefitinib failed to yield a change in CBX/HP1‐gamma expression, suggesting that CBX/HP1‐gamma expression may be independent of EGFR downstream signaling. Conclusion: We report a significant upregulation of CBX3/HP1‐gamma in NSCLC patients, and also a possible relationship between CBX3/HP1‐gamma expression and EGFR mutation. [ABSTRACT FROM AUTHOR]
