Tracer techniques to measure carbon-14 incorporation from labeled food can seriously overestimate the growth of an organism. The degree of overestimation can change with varying food supply and correlates with changes in the tracer activity in lipid and protein pools of the organism. These changes may be associated with differential metabolic partitioning of newly assimilated food.

A case of IgD “nonsecretory” multiple myeloma in a 46-yr-old woman is reported. Despite the presence of disseminated osteolytic lesions, both serum protein electrophoresis and serum and urine immunoelectrophoresis were normal. In addition, bone scintigraphic study was normal. Bone marrow biopsy and aspirate obtained from the left femur lytic lesion showed only myelomatous proplasmocytes; when examined by immunofluorescence with monospecifc antisera, the cytoplasm showed only the presence of delta and kappa chains, suggesting that the neoplastic plasma cells might belong to a single clone. Lymphocyte studies indicated the presence of a normal amount of both B and T cells.

This research evaluated the diagnostic and analytic effectiveness of 201-thallium imaging in a group of patients with coronary artery disease, each one of them with different localization, extent and number of coronary vessel obstructions. The thallium-201 imaging was performed immediately after ergometric test and repeated 4 hours later. In a large percentage of patients thallium-201 imaging of reversible and irreversible perfusion defects have been found; thallium-201 scintigraphy showed a greater sensitivity than exercise ECG. With particular evidence 201-Tl imaging showed perfusion defects in a high percentage of patients with obstruction of one and two coronary vessels; in these patients, in the former especially, the results of exercise ECGs showed a lower sensitivity. Both thallium-201 and exercise ECG had a similar sensitivity in patients with obstruction of three coronary vessels. In patients with obstruction of one or two vessels, thallium-201 imaging clearly showed a regional hypoperfusion corresponding to the specific perfusion coronary vessels, while exercise ECG gave aspecific topographic results. Irreversible perfusion defects resulted corresponding to the myocardial segments that were infarcted, but were also observed in patients without previous myocardial infarction.

In 176 patients who were hospitalized with a suspected anterior myocardial infarction, the total enzymatic activity of CPK and LDH in addition to the percentage of various isoenzymes have been determined through electrophoretic separation. The blood samples were taken upon the patients admittance to the hospital and repeated every 6 hours until the 24th hour after admittance. We have considered significant an increase of CPK-MB of more than 6% of the total, along with the LDH1/LDH2 ratio greater than 1. We have also taken into consideration the LDH1/LDH-total ratio. The patients were then subdivided into four groups and the results obtained were compared to the electrocardiographic data. Our results confirmed the need to examine both isoenzymatic groups for a correct laboratory diagnosis of anterior myocardial infarction.

From 1978 to 1985 a total of 151 patients were treated for endometrial carcinoma. Of these, 25 patients underwent extrafascial abdominal hysterectomy and pelvic lymphadenectomy, 25 were treated according to Wertheim procedure and pelvic lymphadenectomy and 32 underwent intrafascial abdominal hysterectomy; 62 women underwent vaginal surgery, 7 of whom according to Shauta. A correlation between the degree of myometrial invasion, histological grading, hystological type and stage of the tumor showed no statistically significant difference. The 5-year actuarial survival rate was found to be 76.5%. A comparison between survival and age of patients showed a significant difference in the survival (p less than .01) of the group less than 55 years as compared to the older age group. As far as the surgical treatment instituted is concerned, no statistical difference in survival was found between patients operated vaginally and those operated abdominally (p greater than .05). The site of recurrences were then analyzed in 22 patients, 50% were local recurrences, and the remaining distant metastases. Of these only one patient was cured and is still free of disease 5 years after recurrence. The criteria used to select patients for vaginal surgery are also indicated.

Pure acute Pandysautonomia (PAP), first described in 1969 by Young, is a rare but well defined clinical entity, in which the isolate involvement of Autonomic Nervous System (ANS) gives rise to a characteristic clinical admixture of symptoms and signs all due to the alteration of parasympathetic and sympathetic control of neurovegetative functions. The most prominent clinical features are: 1) postural hypotension without changes in heart rate leading to orthostatic syncopal or near-syncopal episodes, 2) abnormalities of intestinal tract and urinary bladder motility, leading to constipation alternating with diarrhea and urinary retention, 3) loss of sweating, 4) ocular abnormalities. We report a case of a 17 year old man with PAP, in whom almost complete resolution of clinical symptoms and signs of ANS involvement was observed within 9 months from the onset of illness. In our case, besides an accurate clinical and laboratory follow-up, particular attention was paid to investigate the cardiac responses to autonomic reflex tests and pharmacological interventions (atropine and beta-blocking drugs administration) during the entire course of illness. During the acute phase, complete vagal and almost complete beta-sympathetic "denervation" of the heart leading to disabling postural hypotension, fixed resting tachycardia and total unresponsiveness to Valsalva manoeuvre, were observed. PAP is a rare condition which must be known from cardiologists for a more complete knowledge of the etiology and pathophysiological mechanisms of postural syncopal attacks and must be distinguished from the less favourable progressive autonomic failure of the Shy-Drager syndrome.

A case of acute organophosphate poisoning (Malatox P20) in a 24-year-old woman, is reported. The signs and symptoms of acute poisoning are described and a rational approach to specific treatment is discussed.

A number of the mechanisms implicated in the phenomenon of immunosuppression present in tumour-carrying patients are described. In particular, stress is laid on factors soluble to immunosuppressive activity identified in the serum or in other biological fluids of these patients. From among these are analysed the circulating immunocomplexes, the proteins of the acute phase (alpha-globulins) and other inhibiting substances derived from tumour cells. The nature and origin of these substances are discussed as well as their effects on the cancer patient's immune system and the therapeutic type implications that arise in some cases.

Until now, three patients with IgG and only one with IgM class monoclonal gammopathy (without the classical features of Waldenström's macroglobulinemia) have been reported in subjects with myasthenia gravis. A case of Waldenström's macroglobulinemia which occurred before thymectomy in a myasthenic patient is described in this paper. Both neuromuscular disease and the lymphocyte dyscrasia worsened after operation and before starting steroid treatment. No evidence of circulating immune complexes nor of anti-acetylcholine receptor antibodies belonging to the IgM class was found. The patient's HLA type shared A2 and B15 antigens with an IgG-lambda monoclonal gammopathy previously reported in a myasthenic woman, and his genotype included the Bw15 specificity which has been described to be frequent in Waldenström's macroglobulinemia. It appears that a persistent thymic abnormality, responsible for myasthenia gravis, may be associated with a lymphoreticular neoplasm.

The results of a study performed on a case series of 250 women treated in our out-patient department for abnormal uterine bleeding are reported. Half of these patients (125) were 45 years of age or over and therefore at risk of adenocarcinoma or endometrial hyperplasia while the other half were under 45 years of age. All the patients were submitted to hysteroscopic examination as out-patients (no important side-effects occurred), together with cytologic and/or histological examination of the endometrium. This procedure revealed no disease in 59 cases (23.6%) and the presence of one or more benign conditions in 188 patients (75.2%) (inflammation, polyposis, myomatosis, endocervical or uterine adenomiosis, endometrial hyperplasias, dysfunctional patterns, intrauterine foreign bodies). Endometrial adenocarcinoma was diagnosed in 3 patients (1.2%). The discovery of 3 cases (1.2%) of adenocarcinomas and 62 cases (24.8%) of endometrial hyperplasias (58 simple glandular hyperplasia, 3 cystic-glandular hyperplasias and 1 polypoid hyperplasia) emphasive the reliability of hysteroscopy in the diagnosis of endometrial neoplasias and their precursors.

Some nonspecific components (IgG, C3c, C4,) of circulating immune complexes (IC) precipitable by 3.5% PEG were assayed by laser nephelometry in the sera of 71 patients with solid tumours, and of 39 patients with autoimmune diseases. PEG-IgG, PEG-C3c, and PEG-C4 were higher in cancer and in autoimmune diseases than in controls. In cancer patients, PEG-C4 levels and PEG-C4 positivity rate correlated well with tumour burden. PEG-C3c and PEG-IgG were present at higher levels and PEG-C4 was present at lower concentration in cancer than in autoimmune diseases. The results of the present study, performed on a large population of patients with cancer and autoimmune diseases, indicate a different composition of circulating IC in these diseases. Particularly, these data suggest a preferential involvement of the alternative pathway of complement activation in cancer, as already described by other authors. The aggregation of C3c to IC, mediated by the alternative pathway, seems to be the "key" event in the process of IC solubilization. In cancer patients' sera, the presence of "solubilized" IC could partly explain their peculiar biological behaviour as well as the disagreement observed among the different assay methods based upon the binding with complement factors.

C-reactive protein and rheumatoid factor assays are relevant laboratory parameters, among the most used in the clinical practice, being very useful in diagnosing and "monitoring" rheumatic diseases. A quantitative test to detect these two serum proteins is an important, more specific, new tool for clinical medicine. In our study we evaluated two new laser nephelometric assays, both quantitative, based on the agglutination of polystyrene latex particles, able to show serum concentration of CRP and RF. With a similar technique we tested the presence of circulating immune complexes. We performed our tests in a group of patients with rheumatoid arthritis, other autoimmune diseases, and healthy blood bank donors assaying simultaneously, whenever possible, the "classic" Rose-Waaler and RA tests. We found both methods easy to perform, well correlating with the semi-quantitative techniques and highly reproducible.

The first direct experimental measurements of the scattering of a millimeter-wave beam by plasma blobs in a simple magnetized torus are reported. The wavelength of the beam is comparable to the characteristic size of the blob. In situ Langmuir probe measurements show that fluctuations of the electron density induce correlated fluctuations of the transmitted power. A first-principles full-wave model, using conditionally sampled 2D electron density profiles, predicts fluctuations of the millimeter-wave power that are in agreement with experiments.

Lo studio delle società antiche ha compiuto significativi progressi integrando la prospettiva di genere; i décalages tra piano umano e rappresentazione del divino potrebbero rivelarsi strumenti euristici altrettanto significativi per indagare la peculiare “fisionomia” delle dèe e degli dèi del pantheon ellenico.

Background and aims The effectiveness of adalimumab in the treatment of ulcerative colitis is under debate. Although controlled trials have shown that adalimumab is significantly better than placebo, the absolute clinical benefit is modest. We report data on the effectiveness of adalimumab in a cohort of ulcerative colitis patients treated in 22 Italian centres. Methods All patients with active disease treated with adalimumab were retrospectively reviewed. Co-primary endpoints were clinical remission at weeks 4, 12, 24 and 54. Secondary endpoints were sustained clinical remission, steroid discontinuation, endoscopic remission and need for colectomy. Results Eighty-eight patients were included. Most patients had received previous infliximab treatment. Clinical remission rates were 17%, 28.4%, 36.4% and 43.2% at 4, 12, 24 and 54 weeks respectively. Twenty-two patients required colectomy. Clinical remission and low C-reactive protein at week 12 predicted clinical remission at week 54 (OR 4.17, 95% CI 2.36–19.44; OR 2.63, 95% CI 2.32–14.94, respectively). Previous immunosuppressant use was associated with a lower probability of clinical remission at week 54 (OR 0.67, 95% CI 0.08–0.66) and with a higher rate of colectomy (HR 9.7, 95% CI 1.46–9.07). Conclusion In this large “real-life” experience adalimumab appears effective in patients with otherwise medically refractory ulcerative colitis. Patients achieving early remission can expect a better long-term outcome.

Il chimerico organismo cibernetico, capace di sperimentare continue metamorfosi, ridelinea il corpo come concetto limite, trasformandolo, nei suoi commerci con l’eteroreferenza macchinica, in un sistema complesso in cui autosviluppo e progettazione esterna si compenetrano reciprocamente. Immagine condensata di scoperte immaginative e realizzazioni materiali, i due centri congiunti che strutturano le dinamiche del mutamento, il cyborg, il quale attraversa il tempo sociologico, scandito dai ritmi intrinseci al potenziamento della comunicazione, trascende i dualismi con cui per secoli si è drasticamente spiegato il mondo, superando le distinzioni antinomiche tra identità e alterità, natura e cultura, maschio e femmina, segnate da una logica speculare spesso funzionale alle pratiche di dominio sulle categorie sociali costruite come altre.

The essay offers an introduction to the Women Film Pioneers Project, a collaborative research launched about the end of last century and now involving contributions by several dozens of scholars across the world. Three key methodological issues are examined: periodization and the choice to focus on silent cinema, the adoption of a transnational approach, and the project’s orientation toward a concept of social history. In this frame, the ongoing investigation concerning the work of Italian women film pioneers is presented as a valuable contribution to a deeper appreciation of the global phenomenon of women’s participation into the film industries and cultures of early XXth century.
