Abstract

In December 2019, novel coronavirus (SARS-CoV-2) infection started in Wuhan and resulted in a pandemic within a few weeks' time. Organ transplant recipients being at a risk for more severe COVID-19 if they get SARS CoV-2 viral infection, COVID-19 vaccine has a significant role in these patients. The vaccine is a safer way to help build protection and would either prevent COVID-19 infection or at least diminish the severity of the disease. It would also reduce the risk of the continuing transmission and enhance herd immunity. Immuno-compromised patients should not receive live vaccines as they can cause vaccine-related disease and hence the guidelines suggest that all transplant recipients should receive age-appropriate 'inactivated vaccine' as recommended for general population. Though trials have not been undertaken on transplant recipients, efficacy and safety of COVID-19 vaccine have been scientifically documented for few vaccines among the general population. Abstract

Although end-stage renal disease (ESRD) related to AA amyloidosis nephropathy secondary to tuberculosis is most common in our country, there are limited data concerning patient and graft outcome after renal transplantation (RTx). To the best of our knowledge, this is the first report of RTx in ESRD patient with secondary amyloidosis due to tuberculosis from India. A 30-year-old female with past history of pulmonary tuberculosis 3 years back was admitted with complaint of gradually progressive pedal oedema and nausea for 3 months. Renal biopsy was suggestive of secondary renal amyloidosis with vascular involvement and chronic tubulointerstitial involvement. She was transplanted with kidney from her 28-year-old brother with 3/6 human leukocyte antigen match. She had immediate good graft function without any perioperative complications (cardiovascular, infections, rejection and delayed graft function). She was discharged with serum creatinine of 0.8 mg/dL. Her last serum creatinine level was 0.9 mg/dL with cyclosporine level of 100 mg/dL at 9-month followup without any medical or surgical complication. The quality of life also improved after transplantation. With careful selection, ESRD patients with secondary amyloidosis due to tuberculosis are eligible for RTx with favorable outcome and improved quality of life.

115. Health-related Quality of Life in Postrenal Transplant Patients: A Single-center Study.

Abstract

Background: The study was conducted to look for association of health-related quality of life (HRQoL) with renal functions in renal transplant recipients and to examine which clinical measures after renal transplantation are connected to aspects of their HRQoL. Materials and Methods: The study was carried out at IKDRC/ITS, Ahmedabad, between 2013 and 2016. Only patients who completed minimum 3 months posttransplant were included in the study. The responses were summarized and transformed to give eight summary scales which were grouped as physical component summary (PCS) and mental component summary (MCS) to give total HRQoL. Results: Out of 54 patients, 74% were males and 26% patients were females. Age distribution was between 18 and 62 years. Most common cause of end-stage renal disease was chronic glomerulonephritis (42.6%). Forty-seven patients underwent live-related kidney transplant while 7 received kidneys from cadaver. Mental health and MCS scores in males were significantly higher (<0.05) than females. With respect to age, the transplant patients were divided into four groups: younger than 30 years, 30-39 years, 40-49 years, and 50-year-old or older. No difference was found in any scale score between the age groups. The scale scores of PCS in patients with serum creatinine level >2 mg/dL were significantly lower (<0.05) than those of patients with serum creatinine <2 mg/dl. When compared in between patients who had rejection to those who did not, there was no significant difference in MCS and PCS. The scores of patients with cadaveric transplantation were similar to those who received a living-related transplantation. Longer time since the transplant operation was significantly associated with lower scores of the vitality scale. Conclusion: SF-36 V2 is a good tool to monitor HRQoL in renal transplant recipients. Abstract

Aim: Complex vascular anatomy poses a major challenge to the donor surgeon. Here, we have described the technical nuances in retroperitoneoscopic living donor nephrectomy for the left kidney in the situations of a rare vascular anomaly of duplication of inferior vena. Materials and Methods: Between September 2005 and June 2016, 1460 retroperitoneoscopic living donor nephrectomy were carried out in single surgical unit of our institution. Out of these four donors were found to have duplication of inferior vena cava (IVC). We retrospectively analyzed the prospectively collected data of these donors and studied the operative details for managing the duplicated limb of the IVC. Results: The mean age of the donors was 42.5 (range 30-54) years. Mean body mass index was 26.9 (range 25.2-28.6) kg/m2. Mean operative time (defined as between giving skin incision to the skin closure [O. T]), was 230 (range 185-310 min). Mean Warm ischemia time (defined from clamping of the renal artery to the starting of the cold HTK perfusion, [WIT]) was 136 s (range 105-178 s). In two cases, the renal vein could be controlled distal to the duplicated limb. In one case, the duplicated limb was clipped while in another a stapler was used to take a cuff of IVC. Conclusion: Retroperitoneoscopic donor nephrectomy can be performed safely in cases of duplication of IVC. Preoperative computerized tomography angiography with vascular reconstruction and surgical expertise is desirable in carrying out the procedure. [ABSTRACT FROM AUTHOR]
