110. Colorectal cancer screening among African American church members: A qualitative and quantitative study of patient-provider communication

Abstract

Abstract Background A healthcare provider's recommendation to undergo screening has been shown to be one of the strongest predictors of completing a colorectal cancer (CRC) screening test. We sought to determine the relationship between the general quality of self-rated patient-provider communication and the completion of CRC screening. Methods A formative study using qualitative data from focus groups and quantitative data from a cross-sectional survey of church members about the quality of their communication with their healthcare provider, their CRC risk knowledge, and whether they had completed CRC screening tests. Focus group participants were a convenience sample of African American church members. Participants for the survey were recruited by telephone from membership lists of 12 African American churches located in rural counties of North Carolina to participate in the WATCH (Wellness for African Americans Through Churches) Project. Results Focus Groups. Six focus groups (n = 45) were conducted prior to the baseline survey. Discussions focused on CRC knowledge, and perceived barriers/motivators to CRC screening. A theme that emerged during each groups' discussion about CRC screening was the quality of the participants' communication with their health care provider. Survey. Among the 397 participants over age 50, 31% reported CRC screening within the recommended guidelines. Participants who self-rated their communication as good were more likely to have been screened (36%) within the recommended guidelines than were participants with poor communication (17%) (OR = 2.8, 95% CI 1.2, 6.4; p = 0.013). Participants who had adequate CRC knowledge completed CRC screening at a higher rate than those with inadequate knowledge (p = 0.011). The percentage of participants with CRC screening in the recommended guidelines, stratified by communication and knowledge group were: 42% for good communication/adequate knowledge; 27% for good communication/inadequate knowledge; 29% for poor communication/adequate knowledge; and 5% for poor communication/inadequate knowledge. Conclusions Participants who rated their patient-provider communication as good were more likely to have completed CRC screening tests than those reporting poor communication. Among participants reporting good communication, knowledge about colorectal cancer was also associated with test completion. Interventions to improve patient-provider communication may be important to increase low rates of CRC screening test completion among African Americans.

Abstract

PURPOSE: To examine how demographic, general health, religious, and political characteristics influenced beliefs about mandatory school vaccinations and history of vaccination refusal for children among Ohio Appalachian parents. METHODS: In 2013 and 2014, baseline data were obtained from parents (n = 337) of girls aged 9–17 from 12 counties in rural Ohio Appalachia enrolled in the Community Awareness, Resources and Education (CARE II) Project. Multivariate logistic regression models were used to identify correlates of parental beliefs about mandatory school vaccinations and history of refusing a doctor-recommended vaccine for their child(ren). RESULTS: About 47% of parents agreed that parents should have the right to refuse mandatory school vaccinations for their child(ren). Participants who reported their political affiliation as Republican (OR = 2.45, 95% CI: 1.28–4.66) or Independent (OR = 3.31, 95% CI: 1.70–6.44) were more likely to agree that parents should have the right to refuse school-mandated vaccinations than parents who reported their political affiliation as Democrat. Approximately 39% of parents reported ever refusing a vaccine for their child(ren). Participants who were female (OR = 3.90, 95% CI: 1.04–14.58) and believed that parents should have the right to refuse mandatory school vaccinations (OR = 3.27, 95% CI: 1.90–5.62) were more likely to report ever refusing a vaccine for their child(ren). CONCLUSION: The study findings provide information to better understand factors related to vaccination refusal among parents in Appalachia Ohio that can be used to design interventions to improve vaccination uptake.

113. The preoperative diagnosis of the ulcerated carotid atheroma

Abstract

Arteriography involves injecting a radiopaque dye to visualize arteries on X-ray; ultrasound is a noninvasive procedure that uses sonar to visualize internal structures. Both arteriography and ultrasound can accurately determine the amount of vascular narrowing or stenosis caused by atheroma (plaque of cholesterol, fat and cellular debris formed within the inner lining within an artery) of the carotid artery, which supplies blood to the brain. An ulcerated plaque is considered to be a high-risk indicator for a subsequent neurologic event or stroke, and the formation of an ulcerated plaque may be associated with hemorrhage within the plaque itself. There has been disagreement regarding the ability of B-mode carotid imaging by ultrasonography to detect ulcerated plaques in the carotid artery. In this prospective study of 126 patients, the results of previous arteriography and ultrasound were compared by examining the plaque specimens obtained during carotid endarterectomy (excision of the core or inner lining of the carotid artery). Of the 126 plaques removed, 60 percent (76) were ulcerated. The sensitivity of the two diagnostic procedures was 47 percent (correctly identified) for ultrasound, and 53 percent for arteriography; the degree of stenosis affected the diagnostic sensitivities. Plaques which narrowed the lumen, or opening, of the artery by 50 percent or less were detected in 77 percent of the cases by both arteriography and ultrasound. When the degree of stenosis was more than 50 percent, the sensitivities were 41 percent for ultrasound and 48 percent for arteriography. These results demonstrate that there is no significant difference in the diagnostic sensitivities of arteriography and B-mode carotid imaging. There is potential diagnostic error in cases of high-grade stenosis when B-mode carotid imaging is used. This should not affect patient care because patients with severe atheroma would most likely be advised to undergo carotid endarterectomy in any case. Abstract

The noninvasive diagnosis of deep vein thrombosis (DVT) has relied on detection of alterations in blood flow in the affected area. Phleborheography is one of the most sensitive tests; however, it cannot identify all thrombi (clots blocking blood flow). A new technique called venous duplex imaging (VDI) has been studied and is being rapidly accepted as a method to identify thrombi. The role of this test as an alternative to other methods is reviewed. Patient selection is important for assessing the capability of hemodynamic tests for DVT. One hundred and three patients with possible DVT were evaluated. Overall sensitivity and the sensitivity of detecting above-the-knee thrombi were significantly different between the two noninvasive tests used. VDI was significantly more sensitive in detecting both above-the-knee and below-the-knee thrombi than phlebography. VDI detected smaller clots that were missed by ascending phlebography. This non-invasive technique can thus replace hemodynamic tests for detection of DVT and may become the new diagnostic standard for deep venous thrombosis. 128. Mindfulness-based Interventions Across the Cancer Continuum in the United States: A Scoping Review

Abstract

Objective To review mindfulness-based interventions (MBIs) tested in randomized controlled trials (RCT) across the cancer continuum.Data Source Articles identified in PubMed, CINAHL, Web of Science, PsycINFO, and Embase.Study Inclusion and Exclusion Criteria Two independent reviewers screened articles for: (1) topic relevance; (2) RCT study design; (3) mindfulness activity; (4) text availability; (5) country (United States); and (6) mindfulness as the primary intervention component.Data Extraction Twenty-eight RCTs met the inclusion criteria. Data was extracted on the following variables: publication year, population, study arms, cancer site, stage of cancer continuum, participant demographic characteristics, mindfulness definition, mindfulness measures, mindfulness delivery, and behavioral theory.Data Synthesis We used descriptive statistics and preliminary content analysis to characterize the data and identify emerging themes.Results A definition of mindfulness was reported in 46% of studies and 43% measured mindfulness. Almost all MBIs were tested in survivorship (50%) or treatment (46%) stages of the cancer continuum. Breast cancer was the focus of 73% of cancer-site specific studies, and most participants were non-Hispanic white females.Conclusion The scoping review identified 5 themes: (1) inconsistency in defining mindfulness; (2) differences in measuring mindfulness; (3) underrepresentation of racial/ethnic minorities; (4) underrepresentation of males and cancer sites other than breast; and (5) the lack of behavioral theory in the design, implementation, and evaluation of the MBI.

To examine male students' and their parents' human papillomavirus (HPV) vaccine communication in relation to males' willingness to discuss the vaccine with their health care provider and the likelihood of being vaccinated.Dyads (n = 111) of students and parents.Participants completed a HPV vaccine survey based on the risk perception attitude framework in 2009.Male students' perceived susceptibility for HPV and self-efficacy to talk to their provider were directly related to their intention to discuss the HPV vaccine, and their intention was directly related to their likelihood of being vaccinated. Parents' perceived self-efficacy to talk to their son and response efficacy of the vaccine were directly related to their intention to talk to their son; however, parents' intention was not related to the likelihood of their son being vaccinated.College males may benefit from HPV vaccine educational programs that include communication skills training to discuss the HPV vaccine with their parents.

Abstract

Cervical cancer (CC) incidence and mortality rates are increased, and CC screening rates are low among Appalachian Ohio women. Mailing human papillomavirus (HPV) self-tests to women to complete at home is a potential new strategy to engage women in CC screening. The authors aimed to gain insights into the perceived acceptability of mailed HPV self-tests. Focus groups were conducted (August 2014–January 2015) among providers (physicians, nurse practitioners, nurses) and women in Appalachian Ohio. Providers (n= 28) and women (n= 15; age range: 32–62 years) reported general acceptance of HPV self-tests, however, for different reasons. Providers thought HPV self-testing would increase the proportion of under-screened women returning to the health-care system, while women thought self-testing would eliminate logistical and reduce psychological CC screening barriers. Findings provide insights into facilitators and barriers of completing an HPV self-test at home, returning it, reporting results, and providing needed follow-up care. To the authors’ knowledge, no systematic research exists addressing providers’ views regarding women’s use of HPV self-tests and the relation of such use to fostering subsequent Pap testing. This information will be useful in developing CC screening programs that include mailed HPV self-tests, as well as encouragement of follow-up Pap testing to meet existing CC screening guidelines.
