To investigate the prevalence and risk factors of oral Candida colonization in psoriatic patients at Siriraj Hospital.Sixty patients with psoriasis, aged older than 18 years, were recruited for the study group. Sixty healthy individuals similar to the patients in the study group in terms of age and gender were recruited for the control group. Candida spp. was isolated from oral swabs and oral rinses taken from all subjects.During the study period, 27 (45.0%) psoriatic patients used only topical treatment and the remaining patients were on systemic treatment. Oral Candida colonization was significantly higher in patients with psoriasis (30%), as compared with healthy controls (13.3%). Candida albicans was the predominant Candida species isolated. Presence of oral candidiasis was significantly associated with systemic treatment.Oral Candida colonization is associated with psoriasis, especially in patients who receive systemic treatment.

To determine bacterial colonization of skin, scalp, nares, nails, and psoriasis lesion in Thai psoriasis patients and compare findings with those of healthy controls.Sixty patients with chronic plaque-type psoriasis and 60 healthy controls of similar age and gender were enrolled. Swabs of nares, scalp, nails, and non-lesional skin were taken from all subjects. Swabs of lesional skin were taken from psoriasis patients. Aerobic bacteria were isolated from swab specimens.Patients with psoriasis had significantly higher rate of bacteria colonization in nares, scalp, and nails than those of healthy controls. Firmicutes spp. was the most common phyla, followed by Proteobacteria spp. in both groups. Coagulase-negative staphylococci (CoNS) were the most common pathogens isolated from lesional skin, non-lesional skin, scalp, and nares of psoriasis patients. Streptococcus spp. was found only in psoriasis patients.Similar to findings from Caucasian psoriasis patients, Firmicutes spp. was found to be the most common phyla colonizing the skin of Asian psoriasis patients. Streptococcus spp. was found to colonize only the skin. Further studies are needed to determine the clinical significance of streptococcal skin colonization in psoriasis patients.

Background: Chronic urticaria (CU) has negative impacts on patients’ daily lives. The Chronic Urticaria Quality of Life Questionnaire (CU-Q 2 oL) evaluates quality of life impairment attitudes among chronic urticarial patients. Although the CU-Q 2 oL has been validated in several languages, the minimal clinical important difference (MCID) of the CU-Q 2 oL has never been determined. Objective: This study aimed to investigate the validity, reliability, responsiveness to change, and MCID of the Thai CU-Q 2 oL. Methods: The Thai CU-Q 2 oL was translated with permission from the authors of the original Italian version. The Thai CU-Q 2 oL, the validated Thai Dermatology Life Quality Index (DLQI), and the Urticaria Activity Score were assessed for 166 patients to evaluate validity and internal consistency. The three questionnaires were then administered to 124 patients to determine the test-retest reliability, responsiveness, and MCID of the Thai CU-Q 2 oL. Results: The Thai CU-Q 2 oL contained only three domains, whereas the Italian version revealed six domains. Nevertheless, the total variance of the Thai CU-Q 2 oL (60.5%) was very close to that of the Italian version (60.0%). The validity of the Thai CU-Q 2 oL was shown by strong correlations between CU-Q 2 oL and DLQI scores. The Thai CU-Q 2 oL also had high internal consistency and test-retest reliability. Distribution-based, receiver operating characteristic analysis, and anchor-based approaches yielded MCID values of 3.9–8.0, 15, and 21.1, respectively. Conclusions: The Thai CU-Q 2 oL is a valid and reliable instrument. We propose that a difference in the Thai CU-Q 2 oL score of 15 (MCID) is the smallest change patients perceive as a meaningful improvement. DOI 10.12932/AP0674.34.2.2016

Monkey esophagus (ME) is a well-accepted substrate for diagnosing pemphigus vulgaris (PV) by indirect immunofluorescence (IIF). However, its availability is sometimes limited due to ethical concerns. This study aimed to investigate the usefulness of human cervix (HC) as a substrate in the diagnosis of PV by IIF. Initially, serum from 1 PV patient was incubated with tissues from 48 HCs. Median IIF titers on HCs that had different demographic and clinical characteristics were compared. Sera from 5 PV patients were then incubated with ME and 21 HCs. For each serum, the titer of IIF on HC that was not different from ME by more than two-fold dilutions was acceptable. Last, sera from 42 PV, 14 pemphigus foliaceous, and 62 non-pemphigus patients were used to evaluate sensitivity and specificity. The results demonstrate that differences in demographic data among HCs did not affect IIF titers. Titers obtained from ME and HC were comparable (81–100 % acceptable values). Sensitivity of HC for diagnosis of PV was better than for diagnosis of pemphigus foliaceus (90.5 and 71.4 %, respectively). Specificity for PV and PF was 96.2 %. We proposed that HC substrate can be used as an alternative substrate for diagnosis of PV by IIF.

Background: Different clinical features of skin diseases have been addressed between aging patients and non-aging patients. However, data focusing on the clinical features of chronic urticaria (CU) in aging patients especially in the Asian population are still limited. Objective: This study aimed to investigate the clinical characteristics of CU in aging and non-aging patients in the Asian population. Methods: Case records of 1622 CU patients attending the Urticaria Clinic, Department of Dermatology, Siriraj Hospital, Mahidol University, Thailand between 2000 and 2013 were retrospectively reviewed. All CU patients older than 60 years were recruited. Twice the number of CU patients who were non-aging were enrolled using a systematic sampling method. Results: Of the 1622 CU patients, 67 (4.1%) were aging patients. From these, 134 non-aging patients with CU were recruited. The majority of patients for both groups were female, with 67.2% and 77.6% of the aging and non-aging groups, respectively. In both groups, the most common cause of CU was chronic spontaneous urticaria. In the aging group, positive autologous serum skin test, anti-thyroid antibodies and antinuclear antibodies were found more commonly than in the non-aging group, without a statistically significant difference. The mean duration of the disease tended to be shorter in the aging group. Conclusion: Our study showed that CU in aging patients was uncommon (4.1%). Aging patients with CU seemed to have shorter disease duration and higher percentages of autoantibodies than non-aging patients with CU without a statistically significant difference. Background Most study concerning the prevalence and dermatological manifestations of the extrinsic and the intrinsic form of atopic dermatitis (AD) were performed in children and adult AD related to the early-onset AD extending to adult life. Adult-onset AD is a subgroup of AD. Apart from the typical eczematous flexural distribution pattern of AD, this group may also have nontypical morphology and localization. Objective The purpose of this study was to compare the clinical and diagnostic features of Thai patients with extrinsic and intrinsic type of adult-onset AD. Methods We retrospectively studied case records of patients diagnosed as adult-onset AD at the skin allergy clinic, Department of Dermatology, Siriraj Hospital, Mahidol University, Bangkok, Thailand from June, 2006 to May, 2008. The diagnosis of AD was made according to the criteria of Hanifin and Rajka and the severity of AD in each patient were assessed using the eczema area and severity index and the Rajka and Langeland score. Results Fifty six patients were enrolled. Eighty-seven percent of patients were extrinsic AD (eAD). Females predominated in both groups. Patients with eAD more commonly had typical lichenified/exudative eczematous lesions, especially on the antecubital and popliteal areas, when compared with patients with intrinsic AD (iAD). Nummular and follicular lesions were more commonly seen in iAD group than the eAD group. The most common area of involvement in the iAD was non-flexural area, followed by flexural area and extensor area. The severity of both iAD and eAD did not show a significant difference. Conclusion The eAD type of adult-onset AD was more common than the iAD type. Patients with eAD frequently had flexural lichenification whereas the iAD group tended to have nonflexural area involvement. The severity of both iAD and eAD did not show a significant difference.

Background Physical urticarias (PU) are an urticarial response to different specific physical stimuli. PU can occur concurrently with chronic spontaneous urticaria or another type of PU. Objective We aimed to study the prevalence, type, clinical data and natural course of each type of PU and other inducible urticarias and also the prognostic factors for remission of patients visiting a tertiary referral hospital. Method We performed a retrospective study of 1200 chronic urticaria patients who visited our Urticaria Clinic during a period of 5 years. Results Of the 1200 chronic urticaria patients, 86 (7.2%) were diagnosed as PU and other inducible urticarias. The most common type of PU was symptomatic dermographism (n = 35, 40.7%) followed by cold urticaria (n = 20, 23.3%) and delayed pressure urticaria (n = 11, 12.8%), respectively. Twelve patients (13.9%) had associated chronic spontaneous urticaria. None of the cases had multiple types of PU. Erythrocyte sedimentary rate elevation was the most common abnormal laboratory result. Nevertheless, only 4.6% declared a related infection. For each type, the median time after onset before 50% remission showed that cholinergic urticaria took the shortest course (34 months) and delayed pressure urticaria took the longest period (110 months). After 1 year and 5 years from the onset of symptoms, 13% and 50% of PU patients were free of symptoms, respectively. Conclusion This study emphasized the variety of PU, other inducible urticarias and natural courses based on tertiary hospital care. PU and other inducible urticarias have tendency to have a longer course than chronic spontaneous urticaria.

Mosquito bite allergic reactions are due to sensitization to mosquito salivary proteins. The purpose of our study was to assess the clinical features of Thai patients with mosquito allergy, and to define the natural desensitization course. Seventy patients with mosquito allergy were enrolled. Most patients were female. Half of the patients had a personal history of atopy. The average age of onset of mosquito reaction was 5.7 years old (range 2-58). Four patients were in the desensitization phase in which clinical symptoms disappeared within 9.5 years (median). The common cutaneous lesions were erythematous papules (68.6%) and immediate wheals (67.1%). The most common area of involvement was the leg. Forty of 50 patients (80%) had positive skin prick testing to mosquito allergen (Culex quinquefasciatus) and 31 of 42 patients (73.8%) had positive immunoglobulin E antibodies against allergens in C. quinquefasciatus saliva proteins.

PURPOSE Across Hong Kong, Malaysia, the Philippines, Singapore, Thailand and Vietnam, (referred to as Asia) approximately 30-53 million individuals of the 151 million employed suffer from allergic rhinitis (AR) and urticaria. It is estimated that approximately 90% of patients with these allergic conditions are insufficiently treated, impacting the socioeconomic burden in terms of absence from work and decreased productivity. This study aims to estimate the socioeconomic burden of allergies in Asia and the cost savings that their adequate management can provide. Due to the limited availability of regional data, this study focused AR and urticaria in selected countries. METHODS Published literature, information from statistical bureaus, clinician surveys and extrapolation of selected data from the European Union were used to determine the socioeconomic costs of AR and urticaria. RESULTS Many patients in Asia suffer from perennial allergies and experience symptoms of AR and urticaria for up to 298 days per year. An estimate of the indirect costs of patients insufficiently treated for AR and urticaria amounts to USD 105.4 billion a year, which equates to USD 1,137-2,195 per patient due to absenteeism and presenteeism. Adherence to guideline-approved treatment can lead to estimated savings of up to USD 104 billion. CONCLUSIONS The current study suggests that within Asia, the socioeconomic impact of AR and urticaria is similar to that seen in the European Union in spite of the lower wages in Asia. This is due to the mainly perennial allergens prevailing in Asia, whereas the sensitization patterns observed in the European Union are dominated by seasonal exposure to pollen. These results underline the need for governmental initiatives to increase public awareness on the prevention and treatment of these and other allergic diseases as well as greater research funding and large-scale studies to reduce their growing socioeconomic burden in coming years.

Background: Colloid bodies (CB) in direct immunofluorescence (DIF) studies are usually found in interface dermatitis. Furthermore, CB can be found in various skin diseases and even in normal skin. Aim: To evaluate the diagnostic value of CB deposits in DIF studies. Methods: From 1996-2007, data from 502 patients where DIF studies showed immunoreactants at CB were enrolled. The definite diagnoses of these patients were based on clinical, histopathological and immunofluorescent findings. The results of DIF studies were analyzed. Results: Immunoreactants at CB were detected in 44.4%, 43.8%, 4.2%, 3.8%, and 2.2% of interface dermatitis, vasculitis, autoimmune vesiculobullous disease, panniculitis, and scleroderma/morphea, respectively. The most common immunoreactant deposit of all diseases was Immunoglobulin M (IgM). Brighter intensity and higher quantity of CB was detected frequently in the group with interface dermatitis. Conclusions: Immunoreactant deposits at CB alone can be found in various diseases but a strong intensity and high quantity favor the diagnosis of interface dermatitis. CB plus dermoepidermal junction (DEJ) deposits are more common in interface dermatitis than any other disease. Between lichen planus (LP) and discoid lupus erythematosus (DLE), CB alone is more common in LP; whereas, CB plus DEJ and superficial blood vessel (SBV) is more common in DLE. The most common pattern in both diseases is CB plus DEJ. The quantity and intensity of CB in LP is higher than in DLE.

Background Angioedema without urticaria can be caused by drugs. The purpose of our study was to assess the prevalence and clinical features of patients with drug-induced angioedema without urticaria. Methods This study retrospectively reviewed case records at Siriraj Hospital, between January 2007 and December 2008. Patients aged at least 15 years were included. Results The prevalence of drug-induced angioedema without urticaria among patients with adverse drug reactions was 2.3%/year. Non-steroidal anti-inflammatory drugs (NSAID) were the most common cause (50%), followed by antibiotics (20%). The commonest NSAID which induced angioedema were ibuprofen and diclofenac. The common sites were periorbital area (67.3%) and lips (27.6%). The median duration of suspected drug therapy before the development of angioedema was 1 day with the range of 10 min to 23 days. Conclusions Non-steroidal anti-inflammatory drugs and antibiotics were the most common drugs causing angioedema without urticaria. The duration of onset ranged from minutes to days. After stopping the suspected drugs, symptoms disappeared within 2–5 days in most patients.

Background Direct immunofluorescence (DIF) studies in patients with lichen planus (LP) show the deposition of multiple immunoglobulins (Igs) at the cytoid bodies (CBs) and fibrin at the dermoepidermal junction (DEJ). The deposition of Ig at the DEJ, as in patients with lupus erythematosus (LE), is occasionally found. For cases with no specific clinical and histologic characteristics, or with ambiguous features, DIF studies may be helpful in disease differentiation. Methods From 1996 to 2004, data from 72 patients with LP, diagnosed on the basis of clinical and histologic criteria at the Department of Dermatology, Siriraj Hospital, Bangkok, Thailand, were collected. The results of DIF studies were analyzed. Results Deposits at the DEJ and CBs were detected in 53% and 60% of cases, respectively. A combination of DEJ (mostly fibrin) and CB (mostly IgM) deposits was found in 38% of cases. A combination of IgM and other immunoreactant deposits, including fibrin at the CBs, was found in 56% of cases. Conclusions This study showed that the positive yield of DIF in LP was 75%. Shaggy fibrin deposition at the DEJ, which is the single best indicator in the diagnosis of LP, was found in 56% of cases. The presence of CBs only, which is a poorer indicator than the shaggy deposition of fibrin along the DEJ, was found in 22% of cases. There were no statistically significant differences in positive DIF yield between specimens derived from glabrous skin and oral lesions (P = 0.67). Forty-four per cent of cases had immunoreactants other than fibrin deposited along the DEJ, which resembled those of LE.

The purpose of our study was to assess the prevalence and clinical course of patients with chronic idiopathic urticaria (CIU), as well as possible causes or associated findings, laboratory findings and the duration of the disease in patients with chronic urticaria (CU). We retrospectively reviewed the 450 case record forms of patients with CU and/or angioedema who attended the Department of Dermatology, Siriraj Hospital, during the period 2000-2004. Of 450 patients with CU, 337 patients (75%) were diagnosed as CIU. Forty-three patients (9.5%) had physical urticaria, while 17 patients (3.8%) had infectious causes. Other possible causes were food, thyroid diseases, atopy, drugs, dyspepsia and collagen vascular diseases. In eighty-nine percent of patients, no abnormalities were detected at the time of physical examination. The most common abnormal laboratory finding was minimal elevation of the erythrocyte sedimentary rate (42%). In 61 patients, autologous serum skin tests had been done. Fifteen patients (24.5%) had positive results i.e. autoimmune urticaria. Anti-thyroglobulin and anti-microsomal antibodies were positive in 16 % and 12% of CIU patients respectively. After 1 year from the onset of the symptoms, 34.5% of CIU patients were free of symptoms and after 1.2 years from the onset of the symptoms, 56.5% of autoimmune urticaria patients were free of symptoms. The median disease duration of CIU and autoimmune urticaria were 390 days and 450 days respectively. Our study provided an overview of CU and CIU in a large series of Thai patients, based on etiological aspects and clinical courses.

Angioedema is an abrupt swelling of the skin, mucous membrane, or both including respiratory and gastrointestinal tracts. This study aimed to report an experience of angioedema in a university hospital with respect to etiologies, clinical features, treatment, and outcome. One hundred and five patients were enrolled. About half had angioedema without urticaria. The common sites of involvement were periorbital area and lips. Forty five patients (49%) had systemic symptoms. The most common cause of angioedema was allergic angioedema. Nonsteroidal anti-inflammatory drug-induced angioedema and idiopathic angioedema were detected in 20% and 18%, respectively. Among patients with allergic angioedema, 41.7% were caused by food, 39.6% by drugs. Thirty seven patients (39%) had recurrent attacks of angioedema. Mean standard deviation (SD) number of attacks in patients with recurrent angioedema was 3.9 (2.7) (ranging from 2 to 10 times). Patients who had older age and multiple sites of skin involvement had tendency to have systemic symptoms.

Topical retinoids are considered to be the first-line agents and maintenance therapy of acne; however, irritation side effects are major concern issues. Noncomedogenic moisturizers are sometimes added to relieve cutaneous irritations. This study aimed to compare tolerability and efficacy of moisturizers containing licochalcone A, l-carnitine and 1,2-decanediol (active formulation) with a placebo in mild to moderate severe acne in Asian subjects.This was an 8-week double-blind, prospective, randomized controlled study. All patients (n = 120) were randomized equally into three groups: (A) adapalene gel, (B) adapalene gel with the active formulation and (C) adapalene gel with the placebo. The severity of acne, skin bioengineering measurements and skin tolerability were recorded during the study.Compared to baseline, the active formulation group showed significant reductions in inflammatory lesions and total lesions at the end of the study without flare-up. Moreover, skin irritations were less detected than in the other two groups by corneometer and transepidermal water loss measurements.The concomitant usage of adapalene gel and the moisturizer containing licochalcone A, l-carnitine and 1,2-decanediol could reduce undesirable side effects without interfering the efficacy of adapalene. This moisturizer may be superior to placebo to prevent cutaneous irritations and enhance patients' adherence to acne medications.

Panniculitis is an uncommon cutaneous manifestation in dermatomyositis. It not only occurs in idiopathic dermatomyositis, but also rarely occurs in the setting of malignancy-associated dermatomyositis. Calcinosis cutis is also less likely to be found in adult-onset dermatomyositis. In malignancy-associated dermatomyositis, panniculitis can precede, concur, or follow the diagnosis of malignancy. We report the case of a 26-year-old Thai female with calcific panniculitis in the setting of adult-onset dermatomyositis associated with nasopharyngeal cancer. The clinical course of calcific panniculitis in our case was not parallel to the course of malignancy. Calcific panniculitis can appear many years after, despite the remission of the cancer. A thorough review of the previously reported literature is also provided.

The management of acne in South-East Asia is unique, as Asian skin and local variables require a clinical approach unlike that utilized in other parts of the world. There are different treatment guidelines per country in the region, and a group of leading dermatologists from these countries convened to review these guidelines, discuss current practices and recent advances, and formulate consensus guidelines to harmonize the management of acne vulgaris in the region. Emphasis has been placed on formulating recommendations to impede the development of antibiotic resistance in Propionibacterium acnes. The group adopted the Acne Consensus Conference system for grading acne severity. The group recommends that patients may be treated with topical medications including retinoids, benzoyl peroxide (BPO), salicylic acid, a combination of retinoid and BPO, or a combination of retinoids and BPO with or without antibiotics for mild acne; topical retinoid with topical BPO and a oral antibiotic for moderate acne; and oral isotretinoin if the patient fails first-line treatment (a 6- or 8-week trial of combined oral antibiotics and topical retinoids with BPO) for severe acne. Maintenance acne treatment using topical retinoids with or without BPO is recommended. To prevent the development of antibiotic resistance, topical antibiotics should not be used as monotherapy or used simultaneously with oral antibiotics. Skin care, comprised of cleansing, moisturizing and sun protection, is likewise recommended. Patient education and good communication is recommended to improve adherence, and advice should be given about the characteristics of the skin care products patients should use.

The autosomal recessive disorder lipoid proteinosis results from mutations in extracellular matrix protein 1 (ECM1), a glycoprotein expressed in several tissues (including skin) and composed of two alternatively spliced isoforms, ECM1a and ECM1b, the latter lacking exon 7 of this 10-exon gene (ECM1). To date, mutations that either affect ECM1a alone or perturb both ECM1 transcripts have been demonstrated in six cases. However, lipoid proteinosis is clinically heterogeneous with affected individuals displaying differing degrees of skin scarring and infiltration, variable signs of hoarseness and respiratory distress, and in some cases neurological abnormalities such as temporal lobe epilepsy. In this study, we sequenced ECM1 in 10 further unrelated patients with lipoid proteinosis to extend genotype-phenotype correlation and to add to the mutation database. We identified seven new homozygous nonsense or frameshift mutations: R53X (exon 3); 243delG (exon 4); 507delT (exon 6); 735delTG (exon 7); 785delA (exon 7); 892delC (exon 7) and 1190insC (exon 8), as well as two new compound heterozygous mutations: W160X/F167I (exon 6) and 542insAA/R243X (exons 6/7), none of which were found in controls. The mutation 507delT occurred in two unrelated subjects on different ECM1 haplotypes and may therefore represent a recurrent mutation in lipoid proteinosis. Taken with the previously documented mutations in ECM1, this study supports the view that exons 6 and 7 are the most common sites for ECM1 mutations in lipoid proteinosis. Clinically, it appears that mutations outside exon 7 are usually associated with a slightly more severe mucocutaneous lipoid proteinosis phenotype, but neurological features do not show any specific genotype-phenotype correlation.

Background There have been reports concerning an association between human immunodeficiency virus (HIV) infection and autoimmune and rheumatic diseases. Objective The purpose of this study was to examine autoimmune and rheumatic manifestations in HIV-infected patients and their correlation with antinuclear antibody (ANA) tests. Methods The clinical and laboratory results of HIV-infected patients attending the Department of Dermatology, Siriraj Hospital, Bangkok, Thailand, from February 1999 to January 2000, were analyzed. Laboratory studies included serum CD4 lymphocyte count and ANA tests. Results Sixty-two patients were enrolled prospectively in the study. Myalgia was the most common clinical presentation (50%). Others included photosensitivity (on history) (39%), arthralgia (26%), vasculitis (18%), sicca complex (10%), arthritis (7%), and Reiter's syndrome (2%). A history of hair loss was given by 23% of patients. A positive ANA test was detected in 3%. No cases of systemic lupus erythematosus, scleroderma, or dermatomyositis were seen. Conclusions Autoimmune and rheumatic manifestations were not uncommonly detected in patients with HIV infection. HIV infection may sometimes mimic systemic lupus erythematosus clinically.

Context: In vivo epidermal nuclear staining (ENS) can be found in patients with autoimmune connective tissue diseases (CNTDs) and other diseases. Aims: The aim of this study was to reveal the underlying diseases and direct immunofluorescence (DIF) characters of patients with in vivo ENS and association of in vivo ENS with circulating autoantibodies. Settings and Design: A retrospective analysis was conducted involving skin biopsy specimens submitted for DIF study at the Dermatoimmunology Laboratory at Siriraj Hospital between 2002 and 2012. Subjects and Methods: The findings of DIF study, clinical manifestations, and diagnosis of patients who had positive ENS were investigated. Statistical Analysis Used: The SPSS software version 18.0. Descriptive statistics were used to report demographic data, clinical characteristics, and laboratory investigation results. Moreover, Chi-squared test or Fisher's exact test were used to compare the categorical variables. Results: One hundred and thirty-eight out of 3735 submitted specimens (3.7%) showed positive ENS. The most common diagnosis was CNTD (79%) followed by vasculitis (10.1%). Lupus erythematosus was the most common diagnosis among CNTD (78%). No association between levels of serum antinuclear antibodies (ANA) titer and intensity of ENS (P = 0.660). However, we found that patients with positive in vivo ANA had lower prevalence of systemic involvement. Conclusions: Although lupus erythematosus was the most common diagnosis among patients with in vivo ENS, the presence of ENS does not indicate any specific diagnosis. However, patients with ENS tend to have less systemic involvement.

Objectives There is a paucity of data regarding clinical differences between early-onset psoriasis (EOP) and late-onset psoriasis (LOP) in Asian populations. This study aimed to investigate clinical differences between EOP (onset at the age of

A 17-year-old Thai man developed erythematous papules on the dorsal surfaces of both feet 7 years previously. The papules progressed to superficial ulcers which slowly increased in size and, although treated by several modalities, never completely healed. Physical examination revealed two irregular-shaped superficial ulcers, 4 × 1 and 5 × 1 cm in size, on the dorsal surface of his right and his left foot, respectively (Fig. 1). The ulcers had granulation tissue at the bases with well-defined borders surrounded by brownish patches. During admission, the superficial vein of the patient's right arm showed painful, warm, cord-like induration. A biopsy specimen from the edge of the ulcer of the left leg showed ulcerated epidermis with pseudoepitheliomatous hyperplasia. Diffuse mixed cellular infiltration composed of lymphocytes, histiocytes, and neutrophils was seen in the dermis with fibrosis and some capillary proliferation. The results of skin biopsy cultures for fungus, bacteria, and mycobacteria were negative. The biopsy specimen from the superficial vein of the right arm showed obliteration of the vessel lumen and mixed cellular infiltration composed of lymphocytes, histiocytes, and neutrophils within the wall compatible with thrombophlebitis. Laboratory studies revealed a normal complete blood cell count, SMA 18, prothrombin time, and partial thromboplastin time. The erythrocyte sedimentation rate was 29 and lupus anticoagulant was not present. Anticardiolipin IgG, IgM, ANA, cryoglobulin, and anti-HIV antibody were all negative. Levels of C3, C4, and CH50 were normal. Bilateral venogram showed deep vein thrombosis of the left leg. The free protein S level was 41% (normal 60%-140%) (Microlatex particle-mediated immunoassay), the protein C level was 90% (normal 75%-150%), and the antithrombin III level was normal. Heparin treatment (Nadroparine 20,000 units subcutaneous Injection twice a day for 10 days) was followed by coumadin 3 mg/day, 2 days per week (days 1 and 4), and 2 mg/day, 5 days per week (days 2, 3, 5, 6 and 7), to keep the prothrombin time between 1.5 and 2 times normal control. The ulcers completely healed 3 months after treatment. The studies of the patient's family members were consistent with hereditary protein S deficiency as in the pedigree shown in Fig. 2. The patient's brother (III-2), mother (11–2), and aunts (11–1, 11–5) all had low levels of protein S (35%, 63%, 51%, and 69% respectively). His uncle (11–3) died with pulmonary embolism in a private hospital. Unfortunately an autopsy was not permitted. We could not obtain blood samples from the patient's father and grandfather.

To identify the factors that affect quality of life in Thai psoriasis patients.Data collected from 326 psoriasis patients that visited dermatology clinic at Siriraj Hospital, Bangkok, Thailand between 2001 and 2007 was used. Dermatology Life Quality Index (DLQI) was used to measure quality of life. Severity was evaluated by Psoriasis Area and Severity Index (PASI).Overall, psoriasis had moderate to very large negative effect on the patient's life. There was a tendency that elderly patients had a slightly better quality of life than younger adult and middle-age patients. However other demographic variables (i.e., gender, occupation, and income) had no influence on quality of life. This study also identified a linear trend of increased overall DLQI with greater PASI.Only older age and lesser severity of disease had association with few burdens in Thai psoriasis patients.

Background: Previous studies have shown that calcium channel blockers can cause cutaneous adverse reactions; however, the amounts of data collected are limited. Recently, there have been new drugs available for which only a few reports have been published with regard to cutaneous adverse reactions. Objective: The purpose of our study was to estimate the rate and to study clinical patterns of cutaneous adverse drug reactions to calcium channel blockers. Methods: Medical records of patients who had cutaneous adverse reactions to calcium channel blockers between January 2004 and December 2010, at the Adverse Drug Reaction Center of Siriraj hospital, Mahidol University, Bangkok, Thailand were reviewed. Results: From 996,583 prescriptions of calcium channel blockers, forty six patients developed cutaneous adverse effects. Diltiazem was the drug that showed the highest rate of cutaneous reactions per million prescriptions. Maculopapular rash was the most common dermatologic manifestation (41.7%), followed by ankle/pedal edema (18.8%). Three patients (6.2%) developed Stevens-Johnson syndrome due to amlodipine and 1 patient (2.1%) developed toxic epidermal necrolysis due to manidipine. Four patients (8.7%) had renal or cardiovascular involvement. Conclusions: It is important to keep in mind that some patients may develop cutaneous adverse reactions, including severe reactions, from calcium channel blockers.

BACKGROUND Daily bathing with antiseptic cleansers are proposed by some physicians as an adjunctive management of atopic dermatitis (AD). As atopic skin is sensitive, selection of cleansing products becomes a topic of concern. OBJECTIVE Our purpose is to evaluate the pH of various antiseptic body cleansers to give an overview for recommendation to patients with AD. METHODS Commonly bar and liquid cleansers consisted of antiseptic agents were measured for pH using pH meter and pH-indicator strips. For comparison, mild cleansers and general body cleansers were also measured. RESULTS All cleansing bars had pH 9.8-11.3 except syndet bar that had neutral pH. For liquid cleansers, three cleansing agents had pH close to pH of normal skin, one of antiseptic cleansers, one of mild cleansers and another one of general cleansers. The rest of antiseptic cleansers had pH 8.9-9.6 while mild cleansers had pH 6.9-7.5. Syndet liquid had pH 7 and general liquid cleansers had pH 9.6. CONCLUSION The pH of cleanser depends on composition of that cleanser. Adding antiseptic agents are not the only factor determining variation of pH. Moreover, benefit of antiseptic properties should be considered especially in cases of infected skin lesions in the selection of proper cleansers for patients with AD.

To evaluate the opinions and perceptions on acne and the effect of acne on the quality of life in Thai teenagers.Five hundred ninety seven students of five high schools in Bangkok were administered a self-reported questionnaire about acne. Of these, 537 students were further examined by dermatologists to grade acne severity.The study population consisted of 392 (65.7%) female and 205 (34.3%) male. More than half believed that inadequate sleep, stress, sweat/exercise/hot weather; cosmetics, pre-menstrual period, oily food, and sun exposure aggravated their acne. Regarding Dermatology life quality index (DLQI), the question concerning embarrassment had the highest mean DLQI score, which reflect the most impact by acne.Some of the Thai adolescents still had misconceptions about the disease. Even mild acne can have an extremely large effect on their quality of life.

Atopic dermatitis (AD) is a common chronic inflammatory skin disease with epidermal barrier defects which leads to dry skin that is easily disturbed by external exacerbating factors. It is now well established that moisturizers play an important role in preventing skin inflammation in AD, including reducing the amount of topical corticosteroid use. Thus, the use of moisturizers is currently recognized as one of standard treatment for AD. This review summarizes the role and classification of moisturizers. We also review some ingredients that are commonly added in moisturizers which are claimed to have an anti-inflammatory effects in AD.

Atopic dermatitis is a chronic inflammatory skin condition result from epidermal barrier defect and immune dysfunction. Not only has lesional skin increase pH and transepidermal water loss but also non-lesional skin. Regular use of moisturizers helps maintain skin hydration and improve barrier dysfunction. Active ingredients in moisturizers can be classified into occlusives, humectants, and emollients according to their properties. Agents with anti-inflammatory effects are also added. This review mainly focus on active agents in moisturizers claimed of suitable for atopic skin, in order to provide more information and aid physicians in selecting and giving advice to patients with atopic dermatitis.

Background. Studies of chronic discoid lupus erythematosus (DLE) lesions by direct immunofluorescence (DIF) were heterogeneous with respect to classes of immunoglobulins and sites where these were deposited. Most of the studies were done in the USA and European countries. Materials and Methods. To obtain representative data from Asiatic countries, we analyzed the direct immunofluorescent abnormalities of 100 DLE lesions in Thai patients who were diagnosed on the basis of clinical and histologic criteria. Results. Granular deposits at the dermoepidermal junction (DEJ) were detected in 90% of cases. The common immunoreactants at the DEJ were IgG (63%) and IgM (47%). The deposits were usually combinations of various classes of immunoglobulins, mostly IgG (53%) and IgM (41%). Deposits of IgG and IgM alone at the DEJ were observed in 12% and 8%, respectively. Deposits at colloid bodies, dermal blood vessel walls, and epidermal nuclei were sometimes also seen. Conclusions. The DIF test of skin biopsy specimens is diagnostically significant in chronic DLE. Our study in Thai patients showed that the most common deposit was a combination of various classes of immunoglobulins, mostly IgG and often IgM as well as C3, and occurred at the DEJ of the involved area.

To reveal the clinical manifestations, aggravating factors, factor associated with severity, and treatment of psoriasis in Thai patients.The data of psoriasis patients who had been visited Dermatologic outpatient clinic, Siriraj Hospital between July 2002 and July 2008 were retrospectively reviewed.One thousand eighty two patients were studied. The male to female ratio was 1.2:1 and the peak age of onset was in the 40 to 49 year-old age group. The most common aggravating factor was stress (50%), followed by trauma (39%) and weather condition (35%). The majority of patients had plaque type (72.8%). Male gender, smoking, alcohol intake, and nail abnormalities were related to severe psoriasis (PASI10).The present study demonstrated the demographic data of Thai psoriasis patients in a large number of population. These data would be beneficial for national public health development of Thailand in order to provide the better care for Thai psoriasis patients.

Nummular eczema is an idiopathic inflammatory skin disease that mostly has a chronic and relapsing course. Many factors are thought to be responsible for this condition, such as atopic dermatitis, dry skin, emotional stress or seasonal variation.The purpose of the study was to assess clinical findings and aggravating factors of nummular eczema and to evaluate characteristics, severity of disease and quality of life of patients.One hundred patients aged from 18 years old diagnosed as nummular eczema were asked about general data, characteristics and clinical findings of nummular eczema, and completed the Thai version of the Dermatology Life Quality Index (DLQI) questionnaires. They also underwent a physical examination.Clinical features and possible aggravating factors of nummular eczema were reported. The result confirmed the chronic course of the disease. The nummular eczema affected quality of life of patients and itching caused the most impairment.Mean (SD) age was 42.1 (18.4) years and two-thirds were women. Fourteen percent had history of atopic dermatitis and half had history of atopy or contact dermatitis. From patch tests, the most common allergen was found to be nickel. Ninety percent of lesions located in lower extremities and two-thirds had a co-existing dryness of the skin. Emotional stress and drinking alcohol aggravated the disease. Those who had exacerbation in summer or skin dryness tended to have persisting disease. Mean(SD) total DLQI score among our patients was 9.2(6.8). Quality of life of the patients was significantly impaired in younger life, office workers, those with trunk-distributed lesions and those having extensive lesions.Clinical features and possible aggravating factors of nummular eczema were reported. The result confirmed the chronic course of the disease. The nummular eczema affected quality of life of patients and itching caused the impairment.

Acute cutaneous lupus erythematosus (ACLE) is a specific lesion in systemic lupus erythematosus (SLE) patients. ACLE can be categorized into localized ACLE, generalized ACLE and toxic epidermal necrolysis-like ACLE. Toxic epidermal necrolysis (TEN) that occurs in SLE patients has been infrequently reviewed. This condition is complicated to diagnose because medication can produce a blistering eruption that resembles vesiculobullous disease in SLE.To describe two cases of newly diagnosed SLE that had a cutaneous presentation compatible with TEN-like ACLE.Characteristics of two patients presenting with TEN-like ACLE and SLE are presented.The authors have described two cases of TEN-like ACLE which occurred in the context of systemic involvement of SLE. The cutaneous lesion was gradually progressed, with less mucosal involvement and mainly photodistributed. The authors suggest that the complexity and rarity of this condition could be related to systemic severity of SLE.

The clinical role of house dust mite (HDM) in atopic dermatitis (AD) is still controversial.The aim of the study is to assess the prevalence, clinical relevance and characteristics of adult-onset AD patients with positive skin prick tests (SPT) to mites.The case record forms of adult-onset AD patients who underwent SPT at the Skin Allergy Clinic, Siriraj Hospital were reviewed.Forty-one of 62 patients (66.1%) had positive SPT to mites. The frequency of intrinsic AD among adult-onset AD was 4.8% (3/62). SPT to HDM tended to be positive in patients who had personal or family history of atopy, positive SPT to several specific antigens or who presented with elevated serum IgE, chelitis, recurrent conjunctivitis and perifollicular accentuation, respectively. CONCLUSION The prevalence of adult-onset AD patients with mite sensitivity was high. There were some notable features that tended to be present in mite sensitive adult-onset AD patients.

Background Melasma is a common hyperpigmentation disorder that has a significant effect on an individual's quality of life. However, there is no preference-based measurement that reflects quality of life in patients with melasma. The objective of this study was to assess the impact of melasma on quality of life by using a health status measurement - the Dermatology Life Quality Index (DLQI) - and a preference-based measurement - Willingness to Pay (WTP) and Time Trade-Off (TTO). Methods A cross-sectional descriptive study was conducted. Seventy-eight patients with melasma who attended the melasma clinic at Siriraj Hospital from February to March 2009 were recruited in this study. The Thai version of the DLQI, questionnaires about WTP, standard TTO, and daily TTO were used to assess patients' quality of life. Results Seventy-seven (98.7%) patients were female with a mean age of 47.8 ± 7.9 years. The mean health utility based on standard TTO was 0.96. The utility obtained by the daily TTO method was 0.92 and was significantly correlated with an economically inactive occupation (p < 0.05). The mean monthly WTP for the most effective treatment was 1,157 baht (7.2% of monthly income), ranging from 100 to 5,000 baht (1 USD ~ 35.1 baht). The WTP was significantly correlated with monthly personal income and the total DLQI score. Conclusion The WTP method could be a useful tool with which to measure the quality of life of patients with melasma.

Sulfonamides are divided into two main groups which are sulfonamide antibiotics and sulfonamide non-antibiotics. The wide use of sulfonamide antibiotics leads to increasing incidence of sulfonamide cutaneous reactions.The purpose of this study is to explore the cutaneous manifestations induced by sulfonamide antibiotics in a large number of Thai patients, including human immunodeficiency virus (HIV) and non-HIV infected individuals. The second purpose is to determine the risk factors for development of sulfonamide cutaneous reactions.We retrospectively studied 191 patients with sulfonamide antibiotics cutaneous reactions attending the adverse drug reaction center, Siriraj Hospital, Bangkok between 2006 and 2010.Majority of the patients was female (59.7%).Maculopapular rash was the most common cutaneous manifestation (37.7%) followed by fixed drug eruption (22%), angioedema with or without urticaria (12.6%) and urticaria alone (12%). Among those with known HIV serology, maculopapular eruption occurred more frequently in the HIV positive group while fixed drug eruption occurred more frequently in HIV-negative group.From our study, there were no significant determination factors to develop serious drug reactions. However, the HIV-positive status and lower level of CD4 count had a tendency to increase risk of developing serious cutaneous reactions.

Background: Systemic sclerosis (SSc) is a multisystem disorder that affects various organ systems. Although SSc patients have both physical and psychological illness, psychological distress is sometimes underestimated as most physicians usually pay more attention to physical problems. Aims: To evaluate dermatology-specific health-related quality of life (QoL) in Thai patients with SSc. Methods: SSc patients, who attended the Department of Dermatology, Siriraj Hospital, Bangkok, Thailand, between August 2009 and April 2010, were enrolled. The demographic data and skin manifestations of the patients were recorded. Skin thickness of each patient was analyzed by modified Rodnan skin score. QoL was evaluated by using the validated Thai version of dermatology life quality index (DLQI) questionnaire. Results: A total of 80 patients of SSc were enrolled in this study. Twelve patients had limited SSc, while 68 patients had diffuse SSc. The mean (SD) disease duration before period of evaluation was 9 (12.8) years. The mean total of DLQI score was 6.3 (range, 0-23). Pemphigus is a rare, organ-specific autoimmune disease. The epidemiology and clinical course vary between reports from different countries.To evaluate clinical manifestations, investigation and clinical course of Thai patients with pemphigus.Demographic data, clinical presentations, laboratory investigations and treatment outcomes in 124 pemphigus patients who had attended the specialized autoimmune skin clinic at Siriraj Hospital during the period from January 1991 to December 2009 were retrospectively studied.Of the 124 pemphigus patients, 79% were diagnosed with pemphigus vulgaris (PV) and 15.3% with pemphigus foliaceus (PF). The male to female ratio was approximately 1:2 in both groups. The mean age of onset was 45.4 years in PV patients and 57.4 years in PF patients. Oral mucosal involvement at the onset of disease was presented in 37.8% of PV patients. The sensitivity and specificity of DIF in the diagnosis of pemphigus was 97.8% and 98.3% while that of IIF was 94.7% and 98.4%. Disease control was achieved in 93.9% of PV patients and 94.7% of PF patients. Remission (off therapy) was achieved in 31.6% of patients in both groups.PV is the most common subtype of pemphigus in Thailand and usually affects females more than males. The disease usually occurs in the fifth decade of life and mucosal involvement is common. Immunofluorescence studies yields very high sensitivity and specificity. Corticosteroids are the mainstay of treatment. The majority of patients attain disease control and one-third of them achieve remission (off therapy).

We retrospectively reviewed the records of 195 patients with suspected cutaneous reactions from NSAIDs. Two hundred and six different non-steroidal anti-inflammatory drugs (NSAIDs) were suspected of causing cutaneous reactions, and the most frequent suspected causative NSAID was ibuprofen (25.7%). Angioedema and/or urticaria were the most frequent cutaneous reactions (54.4%), and the foremost suspected causative drug for these reactions was ibuprofen. The second most frequently found cutaneous reaction was maculopapular eruption (26.2%), and celecoxib was the most commonly suspected causative NSAID for it. The primary suspected NSAIDs causing fixed drug eruption were in enolic acid group. Furthermore, drug hypersensitivity syndrome was diagnosed in five patients, and Stevens-Johnson syndrome and toxic epidermal necrolysis were detected in five patients.

Background To date, even though there are some large studies on adverse reactions related to fluoroquinolones, there is no comprehensive study focusing on cutaneous adverse drug reactions associated with their use. Objective To demonstrate the pattern of cutaneous reaction related to the administration of each type of fluoroquinolone and to study the cross-sensitivity between various types of fluoroquinolones. Methods Voluntary reports of any adverse events associated with fluoroquinolone use as reported from January 2004 to December 2008 and obtained from the Adverse Drug Reaction Center, Siriraj Hospital, Thailand, were reviewed. Results Among 166,736 patients who had been treated with fluoroquinolones, the prevalences of adverse reaction and cutaneous adverse reaction were 0.13% and 0.09%, respectively. The prevalence of cutaneous reaction to individual fluoroquinolones varies between 0.04% and 0.37%. One hundred fifty-one subjects were enrolled. Most cutaneous reactions were attributed to ciprofloxacin (n = 93), and the most frequent adverse cutaneous reaction seen was maculopapular rash (39.7%). Thirteen cases (8.6%) involved a previous history of fluoroquinolone hypersensitivity; among these, 15.4% had cross-reactivity potential. Conclusion These data suggest that individual fluoroquinolones are responsible for different types and frequencies of cutaneous reactions, with some potentiality of cross-reactivity between different generations.

Bullous pemphigoid (BP) is a rare, subepidermal autoimmune blistering disease. Studies from different regions show discrepancies in clinical features and courses.To reveal clinical characteristics, investigations and clinical outcomes of Thai patients with BP and to evaluate the association of BP with malignancy, diabetes mellitus and neurologic diseases.Patients diagnosed as BP who had visited the autoimmune skin clinic at Siriraj Hospital between 1991 and 2009 were retrospectively studied.Fifty-eight patients were enrolled. Mean age of onset was 69.3 years. The female to male ratio was 2.7:1. Fifteen percent of the patients had mucosal involvement and 38.9% showed peripheral blood eosinophilia. The sensitivity of the direct and indirect immunofluorescence test in the diagnosis of BP was 95.7% and 73.5%, respectively. The frequency of diabetes mellitus in BP patients was significantly higher than that in the general population (p0.001). BP patients had a significantly higher chance of having neurologic diseases compared with other autoimmune vesiculobullous disease patients (adjusted odd ratios 4, 95% confidence interval 1.2-13.3). Disease control was achieved in 89.7% of the patients. One-year and three-year 6.4% remission rate was and 66.3%, respectively.BP usually occurred in the seventh and eighth decade of life and affected females more than males. BP is associated with diabetes mellitus and neurologic diseases. Corticosteroids are the mainstay of the treatment. Two-thirds of patients are likely to be in remission within three years.

Acitretin, Ro 10-1670, the principal and free acid metabolite of etretinate, was used to treat twenty patients with disorders of keratinization. An open, prospective study of clinical efficacy, tolerability, and the effects of acitretin on lipid metabolism, hepatic function, and the osteoarticular system was performed over a one year period. Each patient was initially treated with 30 mg/day of acitretin or approximately 0.6 mg/kg/day. Doses were adjusted according to the clinical efficacy and maintained for one year. There were no statistically significant changes in liver function tests or lipid profile. Twelve of eighteen evaluated patients developed asymptomatic skeletal changes; the most common change was disc space narrowing, especially at thoracic-spine level (7 of 18 patients). The earliest bone change was detected 9 months after treatment. Acitretin is effective in improving the disorders of keratinization with mild mucocutaneous side effects and asymptomatic osteoarticular changes.

Background Diphencyprone is a universal contact immunotherapy. The mechanism of action is based on an induction of the delayed-type hypersensitivity. Diphencyprone has been used in various forms for treatments of recalcitrant and facial warts, and alopecia areata. However, this treatment modality has not been generally used in immunocompromised patients.

Mosquito bite allergic reactions are due to sensitization to mosquito salivary proteins. The purpose of our study was to assess the clinical features of Thai patients with mosquito allergy, and to define the natural desensitization course. Seventy patients with mosquito allergy were enrolled. Most patients were female. Half of the patients had a personal history of atopy. The average age of onset of mosquito reaction was 5.7 years old (range 2-58). Four patients were in the desensitization phase in which clinical symptoms disappeared within 9.5 years (median). The common cutaneous lesions were erythematous papules (68.6%) and immediate wheals (67.1%). The most common area of involvement was the leg. Forty of 50 patients (80%) had positive skin prick testing to mosquito allergen (Culex quinquefasciatus) and 31 of 42 patients (73.8%) had positive immunoglobulin E antibodies against allergens in C. quinquefasciatus saliva proteins.

Allergic reactions to mosquito bites, such as generalized urticaria or severe local reactions are common problems worldwide. The diverse sources of allergen prepared from different mosquito body parts usage are a major obstacle to obtaining safe and effective tests and immunotherapy for mosquito bite allergy. Thus, the reactions are often not recognized and allergen immunotherapy is seldom used for severe reaction to mosquito bites. In a search for appropriate allergen sources, the protein profiles of saliva, salivary glands and whole body extracts were comparatively analyzed from 4 common mosquito species of Thailand and/or South East Asia; viz. Culex quinquefasciatus, Aedes aegypti, Aedes albopictus and a zoophilic strain, Anopheles minimus. The major allergens in the extracts which elicited specific IgE responses in the pooled sera of subjects allergic to mosquito bites were identified. It was concluded that mosquito saliva was the best source of allergens. Additionally, both species-specific and species-shared allergens of the 4 mosquito species were identified. The major saliva allergens having MWs of 36, 32 and 22 kDa were identified. The identificstion of major allergens should facilitate the production of specific recombinant allergens and contribute to improvement in the diagnosis and specific immunotherapy of Thai mosquito bite allergy patients.
