102. Evaluation of mortality predictors i̇n patients over 65 years requiring acute hemodialysis

Amaç: Bu çalışmanın amacı 65 yaş üzeri akut hemodiyalize giren hastalarda mortaliteyi öngördürecek faktörlerin belirlenmesidir. Yöntem: Ege Üniversitesi Tıp Fakültesi İç Hastalıkları Yoğun Bakım Ünitesine yatan 65 yaş üzeri akut hemodiyaliz ihtiyacı olan hastalar retrospektif olarak incelenmiştir. Hastane içi, 28 günlük ve 90 günlük mortalite belirteçleri ayrı ayrı araştrılmıştır. Bulgular: Toplam 144 hasta çalışmaya dahil edildi. Hastane içi mortalite %35.4, 28 günlük mortalite %47.2, 90 günlük mortalite %56.2 olarak bulunmuştur. İnotrop ihtiyacı olan hastalarda mortalite anlamlı olarak daha yüksek saptanmıştır. Albumin, CRP, üre, kreatinin, NLR, NT-proBNP lenfosit sayısı, kan basıncı mortalite prediktörleri olarak saptanmıştır. Sonuç: Beklenen yaşam süresinin arttığı yaşlı populasyonda hastaneye yatış ve yoğum bakım ihtiyacı giderek artmaktadır. Diyaliz ihtiyacının olması yoğun bakım hastasında mortaliteyi arttıran bir durumdur. Özellikle yaşlı populasyonda bu hastalarda mortaliteyi öngördürecek çalışmalara ihtiyaç vardır., Objective: The aim of this study is to determine the factors predicting mortality in patients over 65 years requiring acute hemodialysis. Method: Patients over 65 years who require acute hemodialysis in the Ege University Medical Faculty Internal Diseases Intensive Care Unit were retrospectively analyzed. In-hospital, 28-day and 90-day mortality predictors were investigated separately. Results: A total of 144 patients were included in the study. In-hospital mortality was 35.4%, 28-day mortality was 47.2%, 90-day mortality was 56.2%. Mortality was significantly higher in patients who needed vasopressors. Albumin, CRP, urea, creatinine, NLR, NT-proBNP lymphocyte count, blood pressure were determined as predictors of mortality. Conclusion: Hospitalization and need for intensive care are increasing in the elderly population, where the life expectancy increases. The need for dialysis is a condition that increases mortality in intensive care patients. There is a need for studies that predict mortality in these patients, especially in the elderly population.

Motor lateralization is viewed as anatomical or functional asymmetry of the two sides of the body. Functional motor asymmetry can be influenced by musical practice. This study explored whether piano playing experience modulates motor asymmetry and leads to an altered pattern of hand selection, reflecting an altered handedness. We asked two groups of right-handed participants—piano players and non-piano players—to reach targets in their frontal space with both arms, and we tested the motor performance of each arm on this task and then on an arm preference test. As musical practice can decrease motor asymmetry between arms, we hypothesized that participants with piano playing experience would display less interlimb asymmetry and that this, in turn, would change their arm preference pattern, compared with participants without piano playing experience. We found support for both hypotheses, and we conclude that arm selection (preference) is not biologically fixed, but, rather, can be modulated through long-term piano playing.

To determine the diagnostic accuracy of abdominal CT with compression to the right lower quadrant (RLQ) in adults with acute appendicitis. 168 patients (age range, 18–78 years) were included who underwent contrast-enhanced CT for suspected appendicitis performed either using compression to the RLQ (n = 71) or a standard protocol (n = 97). Outer diameter of the appendix, appendiceal wall thickening, luminal content and associated findings were evaluated in each patient. Kruskal-Wallis, Fisher’s and Pearson's chi-squared tests were used for statistical analysis. There was no significant difference in the mean outer diameter (MOD) between compression CT scans (10.6 ± 1.9 mm) and standard protocol (11.2 ± 2.3 mm) in patients with acute appendicitis (P = 1). The estimation of uncertainty -taking into account all sources of error- is an important tool allowing the identification the influence of each stage of the analytical procedure on the overall quality of the results. Measurement uncertainty - reported with the measurement result - is a parameter that indicates the distribution of the probabilities attributable to the measurement results. The aim of this study is the first time estimation of measurement uncertainty for total antioxidant capacity (TAC) of a Turkish commercial apple juice by the main CUPRAC method that uses Cu(II)-neocuproine (Nc)/Cu(I)-Nc redox couple to measure the TAC levels of various biological fluids and foods. The individual source of uncertainties such as repeatability, calibration curve, concentration of the trolox (TR, reference) solutions, pH, temperature and redox factor in the CUPRAC reaction between TR and Cu(II)-Nc were considered in this work. It was found that the measurement uncertainty was dominantly affected by the total CTR standard solution (u(Ctr) =0.0124). Each uncertainty parameter was evaluated separately, and the relative expanded uncertainty value was calculated as ±3.05% with a 95% confidence interval (k=2). [ABSTRACT FROM AUTHOR]

Objectives: The aim of this study was to translate the Central Sensitization Inventory (CSI) into the Turkish language, to perform a psychometric validation, and to investigate its reliability in patients with chronic spinal pain with an organic origin, patients with fibromyalgia, and pain-free control individuals. Patients and methods: Between April 2016 and February 2017, the translation of the original English version of the CSI into Turkish was performed using the forward-backward translation method. A total of 100 fibromyalgia patients (6 males, 94 females; mean age: 45.0±8.4 years; range, 25 to 60 years), 100 patients with chronic spinal pain with an identified organic origin (CSPO), (10 males, 90 females; mean age: 43.8±9.7 years; range, 21 to 60 years), and 100 healthy controls (8 males, 92 females; mean age: 35.8±10.1 years; range, 25 to 55 years) were included in the study. Demographic characteristics were collected. Test-retest reliability was determined by re-administering the CSI-Turkish (CSI-Turk) two weeks after the first application. Results: The internal consistency (Cronbach's alpha) was found to be 0.92 and the intraclass correlation coefficient was 0.93. Patients with fibromyalgia, a very common central sensitivity syndrome (CSS), had the highest mean CSI-Turk scores, and healthy controls had the lowest. Using the recommended cut-off score of 40 resulted in 87% sensitivity and 90% specificity in distinguishing between fibromyalgia and control individuals. Conclusion: This study suggests that the CSI-Turk can be effectively used as a screening tool to elucidate CS-related symptomology among patients with chronic pain with a high internal consistency, test-retest reliability, sensitivity, and specificity. Topkapı Sarayı Müzesi Kütüphânesi El Yazması Eserler Koleksiyonunda bulunan erken dönem Kur’ân-ı Kerîm nüshaları, sahip oldukları tezyînî unsurlar itibarıyla, İslâmî dönem tezhip sanatının görüldüğü en erken örneklerdir. Önemine karşın yeterli ölçüde çalışma yapılmamış, hatta birçok kaynakta bahis olarak dahi ele alınmamıştır. Bu çalışma, tezhip sanatının zamanla ulaştığı klâsik formların filizlendiği ilk belgeleri araştırmak, tezyîn unsurlarının hangi sanat ilkeleri temel alınarak uygulandığını ve form/ fonksiyon dengesini incelemek amacıyla yapılmıştır. Bu bağlamda kullanılan desenler ve uygulandığı alanlarla birlikte klâsikleşmiş mushaf tezyînâtı ile olan bağları incelenerek kavramsal karşılaştırmalı bir yöntem izlenmiştir. Fotoğraf ve çizimlerle desteklenen çalışmada, konunun kolay anlaşılması hedefiyle genel kabul görmüş terimlerin yanı sıra özgün tâbirlere de yer verilen bir anlatım yolu izlenmiştir. Sözü edilen yöntemler nezdinde bu teze, tezhip sanatının başlangıç döneminin maddi belgelerle kanıtlanması yönünde katkı sağlaması gayesi ile çalışılmıştır.--------------------The early Quranic copies in the Collection of the Manuscripts of the Topkapı Palace Museum Library are the earliest examples of Islamic art of illumination in terms of their intrinsic elements. Despite the importance of this issue, it has not been adequately studied, and has not been dealt with even in many sources. This study was carried out in order to investigate the first documents in which the classical forms of the art of illumination reached the time, and to understand the elements and the form / function balance of the elements of the art. In this context, a conceptual comparative method was followed by examining the ties used with the classical mushaf motifs, together with the patterns and the fields used. In the study supported by photographs and drawings, a narrative path was followed which included the original words as well as generally accepted terms with the aim of understanding the subject easily. This thesis is prepared with the aim of contributing to the proof of the history of the beginning of the art of illumination with the help of material documents.

The aim of this research is to determine the pre-service music teachers' attitudes towards online learning during the COVID-19 pandemic. The study investigated the attitudes of music teacher candidates toward online learning and if they differed by the variables of gender, year of study and academic achievement. The "Online Learning Attitude Scale" was used to collect data for the study. The study was conducted with 164 pre-service music teachers studying at the education faculties of Aksaray University, Çanakkale 18 Mart University, and Necmettin Erbakan University in the 2020-2021 academic year. The independent groups t-test and one-way analysis of variance were used in the data analysis. Results from the study suggested that pre-service music teachers' attitudes regarding online learning in the COVID-19 pandemic were negative to moderate. In addition, the study indicated that the attitudes of music teacher candidates towards online learning differed by gender, year of study, and academic achievement variables during the COVID-19 pandemic. 121. Fluoroscopy-guided placement of pull-type gastrostomy tubes; a single-center experience

Aim: Gastrostomy tubes provide nutritional access in patients with impaired oral intake. Gastrostomy tubes can be inserted with endoscopy or fluoroscopy assistance. The aim of this study was to describe the clinical features and complications of fluoroscopy-guided gastrostomy tube insertion and to compare these complications with those of the endoscopy-assisted gastrostomy technique.Material and Methods: A retrospective data analysis was performed for gastrostomy tube placement procedures. Patients’ age, gender, medical, neurological, and surgical diseases, major and minor complications, mortality rates, and length of hospital stay were compared between fluoroscopy-guided and endoscopy-guided pull-type gastrostomy tube placement. Results: The medical records of 92 patients (60 male, 32 female; mean age ± SD: 63.1 ± 15.8 years; range: 24-92 years) who underwent endoscopy-guided (n=50) or fluoroscopy-guided (n=42) gastrostomy tube placement were reviewed. The indications for gastrostomy tube insertion mainly included neurological disease (n=39, 78%), such as cerebrovascular accident (n=22, 44%), in the endoscopy group and surgical disease (n=33, 73.3%), such as head and neck cancer (n=27, 60%), in the fluoroscopy group. There were no mortalities related to gastrostomy tube insertion in either group. There was no significant difference between the major complication rates of the two groups (P=0.62). The minor complication rate was higher in the endoscopy group (P=0.03). One patient in the fluoroscopy group required surgical intervention to treat a complication related to gastrostomy insertion.Conclusion: Gastrostomy tube insertion via both endoscopy and fluoroscopy guidance provides a safe route for nutrition delivery. Fluoroscopy-guided placement of pull-type gastrostomy tubes is a reliable technique and should be the first choice for gastrostomy tube placement, particularly for patients with head and neck tumors in whom endoscopic placement is technically difficult.

Bu çalışmada buğday, arpa, yulaf, kinoa, amarant, yeşil mercimek, nohut ve maş fasulyesi taneleri hedef alınmış olup, bu tanelerin farklı sürelerde (0, 1, 3 ve 5 gün) çimlendirilmesi ve çimlendirme sonucunda değişimleri incelenmiştir. Çimlendirilen tanelerde; bazı fiziksel (renk değerleri), kimyasal (nem, kül, ham protein ve ham yağ) ve besinsel (toplam fenolik madde ve fitik asit) özellikler araştırılmıştır. Çimlendirme süresi arttıkça, tanelerin L*, a*, b* ve C* değerlerinin azaldığı ve h değerlerinin arttığı tespit edilmiştir. Çimlendirme ile tanelerin; nem miktarı azalırken; kül, ham protein ve ham yağ miktarlarında artışlar meydana gelmiştir. Ham tanelere kıyasla çimlendirilmiş örneklerin, daha yüksek fenolik madde içerdiği ve çimlendirme süresinin artmasına bağlı olarak, anti besinsel bir faktör olan fitik asit miktarının azaldığı tespit edilmiştir. Elde edilen sonuçlar genel olarak değerlendirildiğinde; baklagillerin ham protein oranının, tahıllara kıyasla daha yüksek olduğu, tahılların da ham yağ oranının, baklagillere kıyasla daha düşük olduğu, en yüksek toplam fenolik madde içeriğinin pseudo-tahıllarda olduğu ve bunu baklagillerin takip ettiği gözlenmiştir. Sonuç olarak tahıl, pseudo-tahıl ve baklagil tanelerinin çimlendirilmesi ile mevcut besinsel, kimyasal özelliklerinin geliştirilebileceği ve fonksiyonelliğinin arttırılabileceği, başta tahıl ve tahıl ürünlerinde olmak üzere birçok gıda üretiminde kullanılabileceği kanaatine varılmıştır., In this study, grains of wheat, barley, oat, quinoa, amaranth, green lentil, chickpea and mung bean are targeted and their changes in germination and germination results at different times (0, 1, 3 and 5 days) have been studied. In the grassed seeds; some physical (color values), chemical (moisture, ash, crude protein and crude fat) and nutritional (total phenolic content and phytic acid) features have been searched. As the germination period increased, the L*, a*, b* and C* values of the beads decreased and h values increased. With germination, the amount of ash, crude protein and crude fat increased as the moisture content of the grains decreased. It has been found that the germinated specimens contained higher phenolic material compared to the raw germ and decreased the amount of phytic acid, which is an anti-nutritional factor due to the increase of the germination period. The results obtained generally show that the proportion of crude protein in legumes is higher than that of grains, that the raw fat content of cereals is lower than legumes, the highest total phenolic content is in pseudo grains, followed by legumes. As a result, germination of grains, pseudo grains and legume grains has led to the conclusion that the present nutritional and chemical properties can be improved and their functionality can be increased, especially in the production of many foods, especially in grain and cereal products.

Motor lateralization is viewed as anatomical or functional asymmetry of the two sides of the body. Functional motor asymmetry can be influenced by musical practice. This study explored whether piano playing experience modulates motor asymmetry and leads to an altered pattern of hand selection, reflecting an altered handedness. We asked two groups of right-handed participants—piano players and non-piano players—to reach targets in their frontal space with both arms, and we tested the motor performance of each arm on this task and then on an arm preference test. As musical practice can decrease motor asymmetry between arms, we hypothesized that participants with piano playing experience would display less interlimb asymmetry and that this, in turn, would change their arm preference pattern, compared with participants without piano playing experience. We found support for both hypotheses, and we conclude that arm selection (preference) is not biologically fixed, but, rather, can be modulated through long-term piano playing.

Objective: To evaluate radiation dose and image quality of prospectively electrocardiography (ECG)-triggered and retrospectively ECG-gated coronary computed tomography (CT) angiography and to establish cut-off values of heart rates (HRs) for each technique in firstgeneration dual-source CT. Methods: A total of 200 consecutive patients with suspected coronary artery disease were accepted into the study. Patients were selected randomly for each technique (prospective triggering group n=99, mean age 55.85±10.74 and retrospective gating group n=101, mean age 53.38±11.58). Two independent radiologists scored coronary artery segments for image quality using a 5-point scale. Also, attenuation values of each coronary artery segment and dose-length product values were measured. For each technique, cut-off HR values were determined for the best image quality. Objectives: The first aim was to evaluate the stiffness of thyroid parenchyma with shear wave elastography (SWE) in patients with Graves disease (GD) and compare the elastographic values with those of asymptomatic volunteers. The second aim was to evaluate whether there was a correlation between SWE values and grayscale ultrasound (US) and laboratory parameters. Methods: In this prospective study, the thyroid gland parenchyma of 50 patients whose clinical and laboratory findings were indicative for GD and 54 asymptomatic participants with normal laboratory values was examined by SWE. Grayscale US images of the thyroid and submandibular gland were recorded. The volume of the thyroid gland was measured. Elastographic measurements were performed with a free region of interest. The quantitative SWE values (meters per second and kilopascals) of the patient and control groups were compared. A correlation analysis between the SWE values and grayscale US and laboratory parameters was performed. Results: The median (range) SWE values of the thyroid gland in patients with GD were significantly higher than those of the control group [17.34 (12.58–56.15) versus 12.05 (7.72–23.67) kPa and 2.28 (1.83–4.12) versus 1.92 (1.55–2.61) m/s; P <.001 for both comparisons]. When 14.5 kPa or 2.115 m/s was designated as the cutoff value, GD could be diagnosed with a high sensitivity and specificity. We showed a negative weak correlation between the SWE values and parenchymal echogenicity in the GD group. Conclusions: Shear wave elastography can be used as an effective imaging method with high sensitivity and specificity rates in the diagnosis of GD. These strategies were attention, repetition, elaboration, articulation-organization and comprehension monitoring strategies. There were a total of 515 musicians, 412 amateurs and 103 professional, in the study group. The group of amateur musicians participating in the study consisted of 139 students studying at four different fine arts high schools in Turkey and 273 students studying at music education departments of four different universities. The group of professional musicians consisted of 103 orchestra artists who were working in four different orchestras in Turkey in 2016. The data was collected through Personal Information Form and Scale for Strategies Used during the Practice and Learning of Instrumental Music. As a result of the study, it was determined that amateur musicians’ attention and articulation-organization levels were higher than professional musicians’ attention and articulation-organization levels. It was also determined that professional musicians’ comprehension monitoring strategy levels were higher than amateur musicians’ comprehension monitoring strategy levels. Based on the study results, it was suggested that researchers can conduct experimental new studies examining amateur and professional musicians’ strategies while they practice and learn in instrumental music

Maneviyat ve manevi danışmanlık ve rehberlik son dönemlerde dikkatleri üzerine çekmiş ve birçok araştırmaya konu olmuştur. Ülkemizde, Diyanet İşleri Başkanlığı ve Sağlık Bakanlığı arasında imzalanan protokolün ardından konu ile ilgili yapılan araştırmalar artmıştır. Manevi danışmanlık ve rehberlik hizmetinin sağlıklı ve sürdürülebilir olması için disiplinler arası çalışma oldukça önemlidir. Çalışmada, manevi danışmanlık ve rehberlik ile yakından ilişkili olan tıp (N=99), psikoloji (N=128) ve ilahiyat(N=125) alanında eğitim gören öğrencilerin (N=352) manevi danışmanlık ve rehberlikle ilgili algı ve tutumlarının ne olduğu tespit edilmiştir. Yine ilgili disiplinlerin maneviyat ve manevi danışmanlık ve rehberlik tanımı, manevi danışmanlık hakkında ne kadar bilgi sahibi oldukları, manevi danışmanlık hizmetinin hangi disiplin tarafından verilmesi gerektiği hakkındaki düşünceleri de araştırmada çerçevesinde tespit edilmiştir. Verilerin toplanmasında `Manevi Destek Algısı Ölçeği` ve `Kişisel Bilgi formu` kullanılmıştır.Katılımcıların büyük çoğunluğu maneviyat ve manevi danışmanlık ve rehberliği din ile ilişkili olarak tanımlamıştır. Katılımcıların manevi danışmanlık hakkındaki bilgilerinin yetersiz olduğu, manevi danışmanlığın psikoloji ve ilahiyat eğitimi almış kişiler tarafından verilmesi gerektiği, manevi danışmanlığı yararlı buldukları araştırmamızda ortaya konmuştur. Bunun yanı sıra, psikoloji ve ilahiyat öğrencilerinin manevi danışmanlık ve rehberlik algısının yüksek olduğu ve iki bölüm arasında istatiksel olarak anlamlı bir fark olmadığı belirlenmiştir. A protocol that was signed between The Presidency of Religious Affairs and Ministry of Health of Turkey had a boosting effect on the number of studies focused on this topic. An interdisciplinary academic study is of critical importance in maintaining the sustainability and efficiency of spiritual counseling and guidance related services. In this study, attitudes and behaviors of college students, who are studying at the faculty of medicine (N=99), psychology (N=128), and theology (N=125), towards spiritual counseling and guidance were analyzed. Since these three different academic departments were considered to be closely linked to the field of spiritual counseling and guidance, we also investigated thoughts of each faculty students in regard of the following inquiries: What spirituality as well as spiritual counseling and guidance mean to them, how much information they have about spiritual counseling, which faculty should offer spiritual counseling services. `Spiritual Support Perception Scale` and `Personal Information Form` were used for data gathering purpose.A vast majority of the participants were described spirituality as well as spiritual counseling and guidance as a concept which is closely linked to religion. Besides, we found that participants think that applying spiritual counseling services shall be beneficial though they do not have enough accurate information about spiritual counseling. Additionally, they think that spiritual counseling services should be offered by the graduates of psychology and theology departments. The findings of this study showed that there is no statistical significance between the perception of psychology and theology students on spiritual counseling and guidance, however, medicine students were found to have a significantly low level perception toward spiritual counseling and guidance in comparison to psychology and theology students.Keywords :Spirituality, Spiritual Counselling and Guiding, Psychology, Medicine, Theology,Perception, Health 135

Amaç: Çalışmada yetişkinlerde fiziksel aktivite düzeyi ve uyku kalitesi ile ilişkili faktörlerin araştırılması amaçlanmıştır. Yöntem: Kesitsel tipteki bu araştırma Mersin Mezitli'de 15 Mart - 15 Nisan 2015 tarihleri arasında yapıldı. Çalışmaya 20-65 yaş arası 400 kişi alındı. Fiziksel aktivite düzeyi "Uluslararası Fiziksel Aktivite Anketi Kısa Formu" , uyku kalitesi ise "Pittsburgh Uyku Kalitesi İndeksi" ile değerlendirildi. Verilerin özetlenmesinde tanımlayıcı istatistikler kullanıldı. Fiziksel aktivite ve uyku kalitesini etkileyen risk faktörleri Binary Logistic Regression modelinde değerlendirildi. Bulgular: Çalışmaya 194’ü (%48.5) erkek, toplamda 400 kişi katıldı. Katılanların 59’unun (%14.8) fiziksel aktivite düzeyinin yeterli olduğu saptandı. Erkek ve kadınlarda bu oran sırasıyla %22.2, %7.8 olup, fark istatistiksel olarak da anlamlı idi (p≤0.05). Yetersiz fiziksel aktivite düzeyini; eşi ölmüş, boşanmış ayrı olmanın (OR=12.27), kadın olmanın (OR=2.27), lise ve üniversite düzeyinde eğitim almanın (OR=3.77), işsiz (OR=3.28) ve emekli olmanın (OR=5.83) arttırdığı saptanmıştır. Katılanların 187’sinin (%46.8) uyku kalitesinin iyi olduğu belirlendi. Kötü uyku kalitesini; vücut kitle indeksi artışının (OR=1.07) ve kronik hastalık varlığının (OR=1.71) arttırdığı saptanmıştır. Yeterli fiziksel aktivite düzeyi ile uyku kalitesi arasında istatistiksel olarak anlamlı bir ilişki tespit edilmemiştir. Sonuç: Yetersiz fiziksel aktivite düzeyi açısından kadınların, eğitim durumu lise ve üzeri olanların, meslek grubu olarak işsiz ve emeklilerin; kötü uyku kalitesi yönünden ise beden kitle indeksi artışının ve kronik hastalık varlığının dikkatle değerlendirilmesi önerilir.Anahtar kelimeler: Erişkin, fiziksel egzersiz, uyku

In this study, grains of wheat, barley, oat, quinoa, amaranth, green lentil, chickpea and mung bean were investigated in terms of their changes after germination at different times (1, 3 and 5 days). As the germination time increased, the L*, a* and b* and values of the grains decreased. Germination led to increase in the amount of ash, crude protein and fat. It was found that along with the increase in germination time, the germinated samples contained higher phenolic material compared to the raw grain and decreased the amount of phytic acid. Also, crude protein in legumes was higher than common grains, that the crude fat content of common grains was lower than legumes, besides pseudocereals contained the highest amount of total phenolics which was followed by legumes. As a result, germination of grains, pseudocereals and legume grains showed that their present chemic al properties and functionality can be improved and they can be used in the production of many foods, especially in cereal products. We report the case of a 51-year-old man with advanced renal cell carcinoma (RCC), without inferior vena cava (IVC) involvement, who was treated with chemotherapy. Computed tomography of the thorax and abdomen revealed metastatic invasion of the liver, mediastinal lymph nodes, right adrenal gland, and the head of pancreas. Heart involvement via the IVC is a well-known pattern of metastasis during RCC progression. There are very few cases worldwide that have reported RCC with cardiac metastasis without IVC involvement.

A 47-year-old male was admitted to our hospital with numbness and clumsiness in both hands. These symptoms developed 4 months ago and exacerbated progressively. A neurological examination revealed mild distal symmetrical sensory loss. He had no apparent muscle weakness in his shoulder muscles, but the medial section of the scapula was winging, especially after 120 of elevation and when he was asked to push the wall with both hands. The patient did not complain of pain. Nerve conduction studies (Table 1) demonstrated conduction blocks, slowed nerve conduction velocities, and prolonged distal latencies and F latencies in more than 2 peripheral nerves, meeting the electrophsyiological diagnostic criteria for CIDP [1]. By needle electromyography, we observed spontaneous denervation potentials in the serratus anterior muscle. There were no abnormalities in the other neck and shoulder muscles. His spinal magnetic resonance images were within normal limits. Examination of cerebrospinal fluid (CSF) revealed nothing except elevated protein levels (80 mg/dL). The patient was negative for anti-GM1, and no elevation of monoclonal antibodies was noted by blood or urine electrophoresis. There was nothing in his medical or family history that helped us exclude underlying hereditary polyneuropathy. Due to symmetrical diffuse demyelinating involvement in the nerve conduction studies, duration of the complaints, and elevated CSF protein levels, the patient was diagnosed with CIDP after excluding paraneoplastic conditions, diabetes mellitus, vasculitis, and rheumatological and hematological diseases. The patient was treated daily with 1 mgr/kg cts. His clinical symptoms, including the winging scapula, improved by the end of Week 8. In addition, the results of the nerve conduction studies improved slightly, and the conduction block abated (Table 2). Further, there were no abnormalities by repeated needle electromyography.

Like every field of learning, it is important to use various strategies in instrumental music to learn it easily and permanently and to attain the skills to learn independently. This requires determining the strategies used by students studying instrument education. The purpose of this study was to examine the strategies fine arts high school students use during the practice and learning of instrumental music. These strategies are the attention, rehearsal, elaboration, articulation-organization and comprehension monitoring strategies. Students’ levels of using practice and learning strategies were examined according to seven variables. The study group consisted of 139 students who were studying at Aksaray, Kayseri, Mersin and Niğde Fine Arts High Schools during the 2015-2016 academic year. The study data was collected using Personal Information Form and Scale for Strategies Used during the Practice and Learning of Instrumental Music. The results showed that students’ levels of elaboration strategies were higher than their levels of articulation-organization strategies. It was also determined that students’ levels of rehearsal and comprehension monitoring strategies were significantly higher than their levels of other strategies. Furthermore, it was found that students’ levels of using practice and learning strategies showed significant differences according to students’ experience, instrument practice duration, how they plan their instrument practice duration, from where they learned the strategy, their academic achievement level on the instrument and the factor motivating them to play instrument variables.

Birt-Hogg-Dubé syndrome is characterized by clinical manifestations such as hamartomas of the skin, renal tumors and lung cysts with spontaneous pneumothoraces. Patients with Birt-Hogg-Dubé syndrome may present with only multiple lung cysts. We report the chest computerized tomography (CT) features of three patients with Birt- Hogg-Dubé syndrome. Each patient had multiple lung cysts of various sizes according to chest CT evaluation, most of which were located in lower lobes and related to pleura. The identification of unique characteristics in the chest CT of patients with Birt-Hogg-Dubé syndrome may provide an efficient mechanism for diagnosis. © 2014 by the Atatürk University School of Medicine.

The aim of this study is to develop a valid and reliable measurement tool to designate the learning strategies exploited during the piano lesson by the answers of the students studying at the departments of music education. The sampling group is composed of 372 students of 5 music education departments around Turkey at second, third and fourth grades between 2010 and 2011. The scale which is developed for the study is based on the classification of the learning strategies by Weinstein and Meyer (1986). The phrases in the scale are determined via the essays of the students and screening of the literature. The hypothesis that each item is evaluating the related psychological formation is tested by confirmative factor analysis. The scale is made up of 5 sub-dimensions and 66 items. The factorial weights of items in the scale takes place between 0.452 and 0.802. The reliability coefficient for the scale (Cronbach alpha) as a whole was 0.955. The confidence coefficients for sub-scales were determined as 0.940, 0.888, 0.858, 0.884 and 0.867. The results of the study suggests that this scale may help determining and assess the aplication levels of learning strategies exploited during the piano lesson and organizing the teaching and learning environment this direction. ÖzetBu çalışmanın amacı; müzik eğitimi anabilim dallarında öğrenim görmekte olan öğrencilerin, piyano dersinde öğrenme stratejilerini kullanma düzeylerini kendi yanıtlarına göre belirleyen geçerli ve güvenilir bir ölçme aracı geliştirmektir. Çalışma grubu: 2010-2011 eğitim- öğretim yılında Türkiye’deki 5 müzik eğitimi anabilim dalının 2, 3 ve 4. sınıflarında öğrenim görmekte olan 372 öğrencidir. Çalışma kapsamında geliştirilen ölçekte Weinstein ve Mayer’in (1986) öğrenme stratejileri sınıflandırmaları temel alınmıştır. Ölçekte yer alacak ifadelere, öğrenci kompozisyonları ve alan yazın taraması yoluyla karar verilmiştir. Her bir maddenin ilgili psikolojik yapıyı ölçtüğü hipotezi doğrulayıcı faktör analizi uygulanarak test edilmiştir. Ölçek 5 alt boyuttan ve toplam 66 maddeden oluşmaktadır. Ölçekte yer alan maddelerin faktör yük değerleri 0.452 ile 0.802 arasında değişmektedir. Ölçeğin bütününe ilişkin güvenirlik katsayısı (Cronbach alfa): 0.955 olarak bulunmuştur. Alt ölçeklere ilişkin güvenirlik katsayıları ise 0.940, 0.888, 0.858, 0.884 ve 0.867 olarak elde edilmiştir. Çalışma sonucunda, bu ölçekten öğrencilerin piyano dersindeki öğrenme stratejilerini kullanma düzeylerinin belirlenmesinde ve öğretme- öğrenme çevrelerinin bu doğrultuda düzenlenmesinde faydalanılabileceği önerisi getirilmiştir.

147. My Country and My Peopleand Sydney Opera House: The missing link

- Abstract
It is known that Jørn Utzon (1918–2008), the principal architect for the Sydney Opera House project (1957–66), had a lifetime obsession for Chinese art and architecture. However, previous studies did not explore the relationship between Utzon and his venerated Chinese writer Lin Yutang (1895–1976). How Utzon represented the ideas and ideals he received from Lin Yutang׳s conceptualization of Chinese art and architecture in My Country and My People(1935) has not been systematically documented. To this end, this article examines the role of Lin Yutang׳s work in Utzon׳s architectural career generally and the architect׳s Sydney Opera House design in particular. It argues that My Country and My Peoplenurtured young Utzon׳s own architectural philosophy, as reflected in his early manifestoes and design projects. Eventually, Lin Yutang׳s Chinese aesthetics encapsulated in calligraphy, painting and architecture helped Utzon to initiate, articulate and further communicate the design principles of his Sydney Opera House, as well as several other important architectural works before and after. Although Utzon never fully realized his Opera House due to forced resignation in 1966, the inspiration from Lin Yutang vividly remains in Utzon׳s yet to be finished masterpiece.

ABSTRACTIn his 1958 study trip to China, Danish architect Jørn Utzon (1918–2008) acquired two copies of the 1925-edition Yingzao fashi (Chinese Building Standards), first published in 1103 AD. Utzon’s own Yingzao fashi meticulously documented the imperial building practice in feudal China with numerous illustrations. The monograph later became one of the most important sources for Utzon to study the design, structure, construction and decoration of Chinese monuments during his enduring interest in Chinese building culture. However, the precise role of Utzon’s ideas and ideals inspired by the Yingzao fashi in the Opera House design still remains largely unexplored. By surveying the primary sources in The Utzon Archives, the State Library of New South Wales and the architectural collection of Utzon’s family, as well as interviewing his staff and colleagues, the authors argue the Yingzao fashi delivered an important impact both on the aesthetic ideal and architectonic characteristics of Utzon’s Opera House design. This presents that illustrations of Utzon’s Yingzao fashi served not only as a conceptual means to initiate his design but also as practical implements for him and his team to solve the problems of design, production and construction, before their forced resignation in 1966.

The purpose of this study is to examine the learning strategies, which fine arts and sports schools music department students’ use, in terms of some variables while learning piano repertoire. These variables are the opinions related with sex, the fine arts and sports school in which the training is done, the class, the weekly time period allocated for the study of piano course, type of planning the working time, the place where the piano studies are done, the situation of getting assistance to piano course from outside the school and presence of a family member who is interested in music, and piano courses. The sample group of the study is the 644 students who are studying in the music department of fine arts and sports high schools in Turkey in the first semester of 2011 - 2012 school year. In this study, the data, related with the level of the sample group of students to use learning strategies, has been collected through the " Scale of the Situations of Using Learning Strategies in Teaching Piano Repertoire" developed by Kılınçer and Aydıner Uygun (2011). As the data shows compliance with the normal distribution, One-Way Analysis of Variance (One-Way ANOVA) was used in the comparisons between the using levels of learning strategies and variables specified in the study. The findings from the analysis have been interpreted by taking 0.01 and 0.05 meaningfulness level into consideration. According to the results of the study; at the highest level ( x= 3.89) replication strategies, at the lowest level (x= 2.82) attention strategies are used in learning of piano repertoire. In addition, the differences in the result of the comparisons between the using levels of learning strategies and each variable specified in the study have been found meaningful (p, Bu çalışmanın amacı; güzel sanatlar ve spor liseleri müzik bölümü öğrencilerinin piyano repertuvarını öğrenmede kullandıkları öğrenme stratejileri düzeylerini, bazı değişkenlere göre incelemektir. 150. Seizure-induced unstable L1 burst fracture without trauma: A case report

A 62-year-old female presented with severe low back pain following a generalized tonic-clonic epileptic seizure. There was no history of trauma. Neurologic examination was normal. Radiologic examinations revealed burst fracture at the L1 level with %50 spinal canal narrowing. She underwent L1 corpectomy, cage, and plate fixation via anterolateral approach. The postoperative period was uneventful. Most vertebral fractures induced by seizure are asymptomatic and stable. The current case is a rare example of seizure-induced burst fracture of thoracolumbar junction with significant canal compromise. The mechanisms of seizure induced burst fractures were reviewed and treatment options were discussed.
