101. Improving Lung Cancer Screening at an Academic Medical Center.

Abstract

Introduction: Lung cancer ranks as the third most prevalent cancer in the United States. The use of low-dose computed tomography (LDCT) screening significantly reduces mortality from this disease. Unfortunately, Texas lags in completing lung cancer screening (LCS) for high-risk patients, ranking 48th among all states. It is crucial to implement quality improvement (QI) initiatives in Texas. In collaboration with the American Cancer Society, the primary care center (PCC) at our institution led a multidisciplinary QI project aimed at enhancing LCS through LDCT for eligible PCC patients. Methods: The study included patients eligible for screening and who fall into the following categories: established patients with Medicaid, low-income or uninsured established patients, and established patients with either Medicare or commercial insurance. Enhancements to electronic medical records, education for clinical staff and patients, and a coordinated, multidisciplinary effort were implemented. Results: The study revealed a substantial 40.2% improvement in LCS rates. Conclusion: The US Preventive Services Task Force guidelines rely on an accurate history of patient's tobacco use to identify patients eligible for LCS. This QI project achieved success in improving the thoroughness of tobacco use history documentation and surpassed our target for increasing LCS by more than 10%. [ABSTRACT FROM AUTHOR]

102. What Schools Want: How City Leaders Can Help or Hinder Great School Growth

Abstract

Given the importance of school operators in meeting Education Cities members' school growth goals, Education Cities was interested in the question: What do operators want? What roles and activities of local harbormasters are most, and least, helpful to those running great schools? Between October and November 2015, they interviewed 18 operators in eight member cities. The range of operators included new single-site schools, well-established homegrown charter networks with multiple campuses, and local charter operators that have scaled nationally. Operators also represented a diverse group of cities with varying levels of charter market share and maturity. The goal of the study was to shed light on the various roles operators can and should play for their charter schools. They hope the feedback from operators will help city leaders assess and reflect on the roles they currently play, and tease out the root causes of common tensions they may feel with operators in their day-to-day work.

105. Recruiting and Enrolling Rural Students: A Model for Increasing Diversity in STEM

Abstract

The Rural and Diverse Student Scholars Program at George Mason University is a strategic initiative to recruit and retain rural and diverse Virginia students in STEM majors. A grant facilitated a partnership between an academic college and the Office of Admissions, enabling new recruiting methods to be tested. In this article we describe these recruitment efforts, summarize the outcomes, and share lessons learned. We observed themes of family involvement, finances, and perceived opportunity as relevant to this population's decision to enroll. Our experience recruiting for this program may inform other institutions that desire to diversify their student body in STEM.

106. Adolescent-Specific Motivation Deficits in Autism versus Typical Development

Abstract

Differences in motivation during adolescence relative to childhood and adulthood in autism was tested in a cross-sectional study. 156 Typically developing individuals and 79 individuals with autism ages 10-30 years of age completed a go/nogo task with social and non-social cues. To assess age effects, linear and quadratic models were used. Consistent with prior studies, typically developing adolescents and young adults demonstrated more false alarms for positive relative to neutral social cues. In autism, there were no changes in attention across age for social or non-social cues. Findings suggest reduced orienting to motivating cues during late adolescence and early adulthood in autism. The findings provide a unique perspective to explain the challenges for adolescents with autism transitioning to adulthood.

107. The Role of Perceived Support and Local Culture in Undergraduate Research Mentoring by Underrepresented Minority Faculty Members: Findings from a Multi-Institutional Research Collaboration

Abstract

Undergraduate research is value-added experiential learning that cultivates creative and intentional learners in and out of the classroom. However, only recently have researchers begun investigating the mechanisms related to mentoring undergraduate researchers, with scant attention being paid to the experiences of underrepresented minority (URM) faculty members. Using unique data collected from an online survey of faculty members (overall N = 215, URM n = 25), we find departmental and institutional support to be the key factors correlated with mentoring undergraduate research students. Reported support is more influential for URM faculty than white faculty, providing evidence of the importance of institutional policy as a mechanism to facilitate URM faculty participation in the high impact practice of mentoring undergraduate researchers. The findings are discussed in the context of institutional initiatives designed to support student and faculty success in the 21st century.

121. The effect of β-alanine supplementation on neuromuscular performance

Abstract

Carnosine (β-alanyl-L-histidine), a histidine containing dipeptide, is one of the most abundant small-molecular compounds in human skeletal muscle. Supplementation with the rate limiting amino acid, β-alanine, has resulted in significant improvements to high-intensity exercise performance. The role of carnosine as an intracellular pH buffer is undisputable, yet other physiological roles have been proposed, including the potential influence of increased carnosine content on regulation of skeletal muscle calcium (Ca2+) kinetics. The movement of Ca2+ is vital during both skeletal muscle contraction and relaxation phases. The overall aim of this thesis was to investigate the effect of β-alanine supplementation on voluntary and electrically evoked contractile properties of in-vivo human skeletal muscle. To examine this research question, there were several aims of this thesis, initially to examine the effect of β-alanine supplementation on intrinsic in-vivo isometric knee extensor force production and skeletal muscle contractility in both fresh and fatigued human skeletal muscle in young (Studies 1 and 2; Chapters 4 and 5) and older (Study 3; Chapter 6) adults. The distribution of the carnosine molecule across subcellular fractions within rat skeletal muscle tissue was explored, as well as the impact of increased carnosine availability on ATPase activity, a measure associated with skeletal muscle relaxation, estimated by Pi generation. In young adults, 28-days of β-alanine supplementation did not significantly influence voluntary and evoked force responses, or the force-frequency relationship, the in-vivo analogue of the force-Ca2+ relationship, in either fresh (Studies 1 and 2; Chapters 4 and 5) or fatigued (Study 2; Chapter 5) skeletal muscle. Furthermore, older adults experiencing pre-existing declines in skeletal muscle function due to ageing, demonstrated no beneficial effect of 28-days β-alanine supplementation on voluntary or electrically evoked skeletal muscle contractions (Study 3; Chapter 6). In young adults, there was, however, a significant decline in skeletal muscle half-relaxation time (HRT) during electrically evoked octet contractions, resting and potentiated twitches (Studies 1 and 2; Chapters 4 and 5). Two possible steps influence skeletal muscle relaxation speed include the Ca2+ removal from the myoplasm and Ca2+ dissociation from troponin followed by cross-bridge detachment. Based on the in-vivo research, it was proposed that there was a direct or indirect mechanism associated with activity of the sarcoplasmic/endoplasmic reticulum Ca2+ ATPase (SERCA) pump, the proposed rate-limiting step of muscle relaxation. In-vitro analysis of ATPase activity demonstrated that SERCA activity was unaffected by increased carnosine concentrations, although there was a significant increase in overall ATPase activity (Study 4; Chapter 7). The results in this thesis showed that β-alanine supplementation was effective in improving skeletal muscle HRT in young adults, although not in healthy older adults. The exact mechanism associated with the in-vivo decline in skeletal muscle HRT remains unclear, yet raising the availability of carnosine in-vitro, does increase overall ATPase activity, although not Ca2+-dependent or SERCA activity.

125. Which is the best imaging modality to diagnose a manica flexoria tear?

Abstract

Summary: The manica flexoria (MF) is an extension of the medial and lateral borders of the superficial digital flexor tendon (SDFT) that forms a sleeve around the deep digital flexor tendon (DDFT), within the digital flexor tendon sheath (DFTS), just proximal to the metacarpo/metatarsophalangeal joint. Tears of the MF have been described associated with cases of nonseptic tenosynovitis presenting with lameness, with ponies and cobs being over‐represented. Ultrasonography and contrast tenography are the predominant imaging modalities used to diagnose tears of the MF, with tenoscopic evaluation still serving as the gold standard. However, the sensitivity and specificity of additional techniques such as contrast computed tomographic tenography and MRI are now being investigated as other modalities that could be of value to identify MF tears. Additional techniques have been described to combine with traditional ultrasonography, with the aim to improve the sensitivity of identifying MF tears, such as dynamic extension/flexion techniques and saline injections of the DFTS. The overall aim of this paper was to review current evidence to determine the best noninvasive diagnostic imaging modality, with a high sensitivity and specificity, to more accurately diagnose MF tears and aid more thorough surgical planning. 129. A Decade of Discovery—Eukaryotic Replisome Disassembly at Replication Termination

Abstract

Simple Summary: During cell division, DNA is duplicated through a process called DNA replication, so that each new cell inherits a copy of its own. A high level of accuracy is essential in this for the maintenance of genome stability and the prevention of genetic disorders and ageing-related diseases. In this review, we describe the current knowledge around DNA replication termination, in particular comparing and contrasting the proteins and mechanisms identified in different organisms—from archaea through to humans—but with a specific focus upon eukaryotic replication termination. We discuss when and where termination takes place, the mechanisms of replication fork convergence and the process of replisome disassembly, in both S-phase and in mitosis. Recent advances in the field have revealed high levels of regulation in the process of replisome disassembly, demonstrating the importance of timely and appropriate unloading of replication machinery. Finally, we summarise how replication termination defects may impact cellular health and raise questions to be addressed in the future within the field. Abstract: The eukaryotic replicative helicase (CMG complex) is assembled during DNA replication initiation in a highly regulated manner, which is described in depth by other manuscripts in this Issue. During DNA replication, the replicative helicase moves through the chromatin, unwinding DNA and facilitating nascent DNA synthesis by polymerases. Once the duplication of a replicon is complete, the CMG helicase and the remaining components of the replisome need to be removed from the chromatin. Research carried out over the last ten years has produced a breakthrough in our understanding, revealing that replication termination, and more specifically replisome disassembly, is indeed a highly regulated process. 132. The effectiveness of workplace coaching : a meta-analysis of learning and performance outcomes : scale development : theoretical model of individual differences and longitudinal study

Abstract

The extant literature on workplace coaching is characterised by a lack of theoretical and empirical understanding regarding the effectiveness of coaching as a learning and development tool; the types of outcomes one can expect from coaching; the tools that can be used to measure coaching outcomes; the underlying processes that explain why and how coaching works and the factors that may impact on coaching effectiveness. This thesis sought to address these substantial gaps in the literature with three linked studies. Firstly, a meta-analysis of workplace coaching effectiveness (k = 17), synthesizing the existing research was presented. A framework of coaching outcomes was developed and utilised to code the studies. Analysis indicated that coaching had positive effects on all outcomes. Next, the framework of outcomes was utilised as the deductive start-point to the development of the scale measuring perceived coaching effectiveness. Utilising a multi-stage approach (n = 201), the analysis indicated that perceived coaching effectiveness may be organised into a six factor structure: career clarity; team performance; work well-being; performance; planning and organizing and personal effectiveness and adaptability. The final study was a longitudinal field experiment to test a theoretical model of individual differences and coaching effectiveness developed in this thesis. An organizational sample of 84 employees each participated in a coaching intervention, completed self-report surveys, and had their job performance rated by peers, direct reports and supervisors (a total of 352 employees provided data on participant performance). The results demonstrate that compared to a control group, the coaching intervention generated a number of positive outcomes. The analysis indicated that coachees’ enthusiasm, intellect and orderliness influenced the impact of coaching on outcomes. Mediation analysis suggested that mastery goal orientation, performance goal orientation and approach motivation in the form of behavioural activation system (BAS) drive, were significant mediators between personality and outcomes. Overall, the findings of this thesis make an original contribution to the understanding of the types of outcomes that can be expected from coaching, and the magnitude of impact coaching has on outcomes. The thesis also provides a tool for reliably measuring coaching effectiveness and a theoretical model to understand the influence of coachee individual differences on coaching outcomes.

Abstract

Introduction: It is unknown how intestinal B cell populations and B cell receptor (BCR) repertoires are established and maintained over time in humans. Following intestinal transplantation (ITx), surveillance ileal mucosal biopsies provide a unique opportunity to map the dynamic establishment of recipient gut lymphocyte populations in immunosuppressed conditions. Methods: Using polychromatic flow cytometry that includes HLA allele groupspecific antibodies distinguishing donor from recipient cells along with high throughput BCR sequencing, we tracked the establishment of recipient B cell populations and BCR repertoire in the allograft mucosa of ITx recipients. Results: We confirm the early presence of naïve donor B cells in the circulation (donor age range: 1-14 years, median: 3 years) and, for the first time, document the establishment of recipient B cell populations, including B resident memory cells, in the intestinal allograft mucosa (recipient age range at the time of transplant: 1-44 years, median: 3 years). Recipient B cell repopulation of the allograft was most rapid in infant (<1 year old)-derived allografts and, unlike T cell repopulation, did not correlate with rejection rates. While recipient memory B cell populations were increased in graft mucosa compared to circulation, naïve recipient B cells remained detectable in the graft mucosa for years. Comparisons of peripheral and intra-mucosal B cell repertoires in the absence of rejection (recipient age range at the time of transplant: 1-9 years, median: 2 years) revealed increased BCR mutation rates and clonal expansion in graft mucosa compared to circulating B cells, but these parameters did not increase markedly after the first year post-transplant. Furthermore, clonal mixing between the allograft mucosa and the circulation was significantly greater in ITx recipients, even years after transplantation, than in deceased adult donors. In available pan-scope biopsies from pediatric recipients, we observed higher percentages of naïve recipient B cells in colon allograft compared to small bowel allograft and increased BCR overlap between native colon vs colon allograft compared to that between native colon vs ileum allograft in most cases, suggesting differential clonal distribution in large intestine vs small intestine. Discussion: Collectively, our data demonstrate intestinal mucosal B cell repertoire establishment from a circulating pool, a process that continues for years without evidence of stabilization of the mucosal B cell repertoire in pediatric ITx patients. Abstract

The cornerstone of obesity treatment is behavioural weight management, resulting in significant improvements in cardio-metabolic and psychosocial health. However, there is ongoing concern that dietary interventions used for weight management may precipitate the development of eating disorders. Systematic reviews demonstrate that, while for most participants medically supervised obesity treatment improves risk scores related to eating disorders, a subset of people who undergo obesity treatment may have poor outcomes for eating disorders. This review summarises the background and rationale for the formation of the Eating Disorders In weight-related Therapy (EDIT) Collaboration. The EDIT Collaboration will explore the complex risk factor interactions that precede changes to eating disorder risk following weight management. In this review, we also outline the programme of work and design of studies for the EDIT Collaboration, including expected knowledge gains. The EDIT studies explore risk factors and the interactions between them using individual-level data from international weight management trials. Combining all available data on eating disorder risk from weight management trials will allow sufficient sample size to interrogate our hypothesis: that individuals undertaking weight management interventions will vary in their eating disorder risk profile, on the basis of personal characteristics and intervention strategies available to them. The collaboration includes the integration of health consumers in project development and translation. An important knowledge gain from this project is a comprehensive understanding of the impact of weight management interventions on eating disorder risk. [ABSTRACT FROM AUTHOR]
