The need for early diagnosis of respiratory diseases, especially in the context of the COVID-19 coronavirus infection pandemic, by means of pulse oximetry screening and monitoring has been substantiated. The expediency of using portable pulse oximeters by therapists and general practitioners is shown. The main respiratory diseases accompanied by respiratory failure, which can be promptly detected by pulse oximetry, are chronic obstructive pulmonary disease and obstructive sleep apnea-hypopnea syndrome. Early detection of these diseases is an urgent task due to the low severity of symptoms of these diseases in the early stages and, as a consequence, the prevalence of late diagnosis. Pulse oximetry to detect coronavirus infection COVID-19 deserves special attention, since this infection is also accompanied by respiratory failure. The use of oxygen concentrators and CPAP devices for the treatment of respiratory failure has been argued. The effectiveness of the appointment of long-term oxygen therapy using oxygen concentrators in patients with chronic obstructive pulmonary disease, CPAP therapy using automatic CPAP devices in patients with obstructive sleep apnea-hypopnea syndrome with mandatory pulse oximetry monitoring has been confirmed. A retrospective analysis of 120 cases of treatment of moderate and severe COVID-19 infection complicated by the development of pneumonia was carried out. The efficiency of using oxygen concentrators to supply patients with oxygen at a flow rate of up to 5 l / min has been proven. It was found that no more than 10% of patients needed an oxygen flow at a rate of more than 5 l/min. At the same time, the possibility of using CPAP devices for non-invasive ventilation of the lungs using full-face masks has been shown. The expediency of using oxygen concentrators and CPAP devices for the rehabilitation of patients who have undergone COVID-19 was noted.

Background Current molecular diagnostics are limited in the number and type of detectable pathogens. Metagenomic next-generation sequencing (mNGS) is an emerging, and increasingly feasible, pathogen-agnostic diagnostic approach. Translational barriers prohibit the widespread adoption of this technology in clinical laboratories. We validate an end-to-end mNGS assay for detection of respiratory viruses. Our assay is optimized to reduce turnaround time, lower cost per sample, increase throughput, and deploy secure and actionable bioinformatic results. Methods We validated our assay using residual nasopharyngeal swab specimens from Vancouver General Hospital (n = 359), which were reverse-transcription polymerase chain reaction positive, or negative for influenza, severe acute respiratory syndrome coronavirus 2, and respiratory syncytial virus. We quantified sample stability, assay precision, the effect of background nucleic acid levels, and analytical limits of detection. Diagnostic performance metrics were estimated. Results We report that our mNGS assay is highly precise and semiquantitative, with analytical limits of detection ranging from 103 to 104 copies/mL. Our assay is highly specific (100%) and sensitive (61.9% overall: 86.8%; reverse-transcription polymerase chain reaction cycle threshold < 30). Multiplexing capabilities enable processing of up to 55 specimens simultaneously on an Oxford Nanopore GridION device, with results reported within 12 hours. Conclusions This study report outlines the diagnostic performance and feasibility of mNGS for respiratory viral diagnostics, infection control, and public health surveillance. Migrants face various challenges in new environments, which are further compounded for those with sensory disabilities. Deaf migrants often encounter unique obstacles that affect their integration and quality of life. This study aims to explore disparities in education, employment, social integration, and healthcare access between deaf and non-deaf migrants. This research employed quantitative methods including logistic regression and Mann–Whitney U tests to analyse data collected from both deaf and non-deaf migrant groups. The study adjusted for demographic variables to isolate the effects of hearing status on various outcomes. Findings indicate that deaf migrants are less likely to achieve higher educational levels and are employed at lower rates compared to non-deaf migrants. Additionally, deaf migrants reported significantly lower subjective social status and faced greater barriers in accessing healthcare services, attributed to communication difficulties and insufficient support systems. The results underscore significant disparities faced by deaf migrants in critical areas of life. These challenges are primarily due to systemic barriers and a lack of adequate resources and accommodations that address their specific needs. The study highlights the need for targeted policies and programs to enhance educational opportunities, employment support, social integration, and healthcare accessibility for deaf migrants, ensuring their successful integration and participation in society. Pseudomonas aeruginosa is a versatile opportunistic bacterium that presents a considerable risk in medical environments because of its strong adaptability and resistance to multiple medications. Targeting the LasR quorum sensing system, which plays a crucial role in controlling virulence factors and biofilm formation, is a key intervention point. In this study, in silico molecular docking, machine learning‐based Quantitative Structure‐Activity Relationship (QSAR) techniques along with molecular dynamics simulation were employed to screen phytochemical compounds for their ability to inhibit the LasR QS system, a key regulator of virulence in Pseudomonas aeruginosa. This study screened 1652 phytochemicals using the ML‐based QSAR model to identify 52 phytochemicals that had better activity than the control (N‐{[3,5‐dibromo‐2‐(methoxymethoxy)phenyl]methyl}‐2‐nitrobenzamide). The in silico molecular docking approach that targeted LasR identified compounds 5281647, 57331045, and 5281672 with high binding affinity and hydrogen bonds that were comparable to the control (docking score=−10.3 kcal/mol and hydrogen bonds=4). In the 200 ns post‐molecular dynamics simulation, 5281647 exhibited a stable RMSD of 0.25 nm, which was comparable to the control. The maximum number of hydrogen bonds was exhibited by 57331045, while 5281647 and 5281672 consistently exhibited four hydrogen bonds. Overall, the Principal component analysis (PCA) and Free Energy Landscape (FEL) analyses of the complexes demonstrated that these three compounds were in stable states. In comparison to the control (ΔGTOTAL=−39.58 kcal/mol), the cumulative binding free energy (ΔGTOTAL) for 5281647 and 57331045 was −39.95 kcal/mol and −39.25 kcal/mol, respectively. This further confirms the superior binding affinity of the two compounds. Both 5281647 and 57331045 were identified as potent inhibitors of the LasR transcription factor, which is essential for the quorum sensing of Pseudomonas aeruginosa, in the present investigation. These findings underscore the importance of further exploration and optimization of phytochemicals for combating bacterial infections, offering promising avenues for future drug discovery efforts targeting this resilient pathogen. The present study aims to synthesize, characterize, and evaluate the cytotoxic effects of novel 1,2,3-triazole derivatives (1-5) on MCF-7 breast cancer cells. Their theoretical studies were performed using DFT and docking computations. New five 1,2,3-triazole derivatives (1-5) were synthesized using different azide and maleimide moieties. The structural authenticity of the target compounds was elucidated using various analytical techniques, including 1H and 13C-NMR, FTIR, and mass spectra. By applying an MTT method, the synthesized triazoles was assessed to determine their cytotoxicity toward MCF-7 breast cancer cell line. DFT and molecular docking computations were applied to evaluate physicochemical properties and predict the potential interactions of the active compounds with relevant biological targets, respectively. Among the synthesized triazoles, compound 5 exhibited high activity towards the MCF-7 cell line with IC50 value of 5.03 µM compared to doxorubicin (a standard drug), whose IC50 value was 3.16 µM. Other derivatives 1-4 showed moderate activity with IC50 values of 82.45, 316.81, 328.48, and 348.34 µM, respectively. The molecular docking results showed that compound 5 has efficient interactions with breast cancer proteins (5KCV, 3ERT, and 4FX3), with low values of binding energy and RMSD. The experimental and theoretical findings suggest that the synthesized 1,2,3-triazole derivatives hold potential as effective agents in breast cancer therapy, and they are highly recommended to be a promising anti-breast cancer agent.
