Abstract: SummaryBackground Accurate predictors of natural tolerance development to cooked and uncooked egg are needed in egg-allergic patients. Objective To compare the diagnostic performance of different immunological tests in relation to egg allergy versus tolerance. Methods Children aged 5–18 years diagnosed with IgE-mediated egg allergy were prospectively recruited. All followed an egg-free diet. Prick test and specific IgE (sIgE) to ovalbumin, ovomucoid and egg white, ovalbumin-sIgG4 and ovomucoid-sIgG4 were determined. By boiled and raw egg challenges, children were classified as cooked egg allergic (CEA, n = 50) or tolerant (CET, n = 35), and uncooked egg allergic (UEA, n = 64) or tolerant (UET, n = 21). Statistics. Comparative analysis (CEA vs. CET and UEA vs. UET). Multivariate logistic regression. Partial receiver operating characteristic curve analysis of tests in relation to CEA and UEA. Negative decision points were defined as cut-offs with sensitivity 95%. Results Ovalbumin-sIgG4 resulted an independent protective factor for uncooked egg allergy. To identify patients with high probability of egg tolerance, ovalbumin-sIgE/sIgG4 tended to perform better than sIgE and prick, specifically in children with ovalbumin-sIgE

Abstract: One of the causes of overusing compound and simple fertilizers in "Oriente Antioqueño" region is the low offer of research articles on fertilization in broccoli. Two experiments were carried out at municipality of Rionegro, Antioquia (Colombia). The first experiment was conducted in the second semester of 2013 in which two broccolis cultivars, Compact and De Cicco, were used. A randomized complete block design (RCBD) with four nitrogen doses (0, 50, 90 and 130 kg N ha-1) was arranged in a factorial 2 x 4, getting eight treatments. Using "San Cristobal" model in a RCBD design, Avenger and Legacy cultivars were grown in the second semester of 2014 including four levels of N, P2O5 and K2O as factors, arranging a total of 12 treatments in an incomplete factorial. Results of this experiment showed that in the first test, Compact cultivar reached a maximum production of 15.5 t ha-1 and De Cicco developed non-tradable inflorescences where a dose of 90 kg N ha-1 was applied, showing a low adaptation to this environmental conditions. In the second experiment, the relative growth height rates -stem diameter and yields- did not show significant statistical differences (P>0.05) regarding the treatments and cultivars evaluated. Avenger and Legacy got the highest yields, 19.0 and 12.2 t N ha-1 respectively, using doses of 60 kg N ha-1, 40 kg P2O5 ha-1 and 55 kg K2O ha-1. [ABSTRACT FROM AUTHOR]

103. Asma agudo severo en niños: ¿Fenotipo diferente?

Abstract: La magnitud de la respuesta al tratamiento de una exacerbacion de asma es variable entre los pacientes y una proporcion significativa de ellos debe hospitalizarse. Objetivos: Definir el perfil de los ninos que se hospitalizaron por asma grave y los posibles indicadores y determinantes de la respuesta desfavorable al tratamiento. Metodo: Estudio prospectivo en ninos de 4 anos o mas, con busqueda etiologica de la exacerbacion y estudio de perfil inflamatorio en esputo. Resultados: 60 ninos entre 4 y 15 anos. El 50% tenia diagnostico previo de asma sin uso regular de corticoesteroides inhalados en dos tercios. Hospitalizaciones previas por asma en el 40%. La etiologia de la exacerbacion fue identificada en el 52% siendo los agentes mas frecuentes Rhinovirus, Metapneumovius, VRS y Mycoplasma pneumoniae. El perfil inflamatorio fue determinado en 33 ninos: eosinofilico en 36% y eosinoflico/neutroflico en 64%. Comentario: El asma severa con exacerbaciones graves seria un fenotipo cuyos aspectos destacados en esta cohorte serian: ninos con hospitalizaciones previas, falta de tratamiento profilactico, infeccion viral como desencadenante frecuente, patron inflamatorio combinado del esputo y rinitis atopica.

Abstract: SummaryBackground Strict avoidance is the only accepted management for cow's milk (CM) allergy. CM oral immunotherapy (CM-OIT) is under investigation. Objectives To evaluate long-term safety of CM-OIT. To identify clinical/immunological predictors of adverse events. Methods Prospective longitudinal epidemiological intervention study. CM-allergic children aged 5–18 underwent a Spanish-approved CM-OIT protocol without premedication. Clinical data, skin prick test (SPT) and specific IgE (sIgE) at baseline and 1 year after OIT were registered. All dose-related reactions, treatments needed and cofactors involved were recorded. Through survival analysis, we studied the cumulative probability of reactions resolution over time and clinical/immunological risk factors of reactions persistence. Results 81 children were recruited. Mean follow-up was 25 months. 95% of children suffered reactions, 91% of which affected a single organ. Reactions were heterogeneously distributed: (a) 60 children (75%) had occasional symptoms which ceased over time. 86% of them reached complete desensitization (200 mL). (b) 20 children (25%) suffered frequent (78% of total reactions), more severe and unpredictable reactions, which persisted during follow-up or led to withdrawal (6 cases). Reactions persistence was associated with a higher frequency and severity. Kaplan–Meier estimate revealed a cumulative probability of reactions resolution of 25% at 3 months (95% CI: 1.9–4.1) and 50% (95% CI: 6.1–9.9) at 8 months based on all patients. Cox proportional hazards multivariate regression model identified 3 variables (CM-sIgE ≥ 50 KU L−1, CM-SPT ≥ 9 mm and Sampson's severity grades 2, 3 and 4 at baseline food challenge) as independent risk factors of reactions persistence. Abstract: Cow's milk allergy is the most frequent childhood food allergy. Children older than 5 who have not become tolerant have less probabilities of natural tolerance. Specific oral desensitization methods are being investigated in reference centres. The aims of our study were to assess the efficacy of our guideline of specific oral desensitization to cow's milk in children and to know its suitability for anaphylactic children. Both clinical and specific IgE outcomes were evaluated. Eighty-seven children aged 5 to 16 years with a history of cow's milk allergy were included. Prior to desensitization, skin prick test, specific IgE to cow's milk proteins and a double-blind placebo control food challenge were performed in all. Of the 87 patients, 21 had a negative challenge; they were considered tolerant, and they were told to follow a free diet. Of the positive, 44 were anaphylactic and 22 non-anaphylactic. All of them were included. In non-anaphylactic patients, 6 achieved partial and 16 maximum desensitization after 23.1 weeks. In the anaphylactic group, 7 achieved partial and 35 maximum desensitization after 26.4 weeks. Cow's milk-specific IgE levels and casein-specific IgE levels were significantly lower in the tolerant patients at baseline. One year after desensitization, the medium specific cow's milk levels and casein IgE levels had dropped significantly. Conclusions: Our guideline for specific oral desensitization to cow's milk is efficacious even in patients with anaphylactic reactions to cow's milk and represents a significant life change. Immunological changes in 1 year show a drop in cow's milk protein-specific IgE.

106. Experimental and numerical results of a high frequency rotating active magnetic refrigerator

Abstract: Experimental results for a recently developed prototype magnetic refrigeration device at The Technical University of Denmark (DTU) were obtained and compared with numerical simulation results. A continuously rotating active magnetic regenerator (AMR) using 2.8 kg packed sphere regenerators of commercial grade gadolinium (Gd) was employed. With operating frequencies up to 10 Hz and volumetric flow rates up to 600 L/h, the prototype has shown high performance and the results are consistent with predictions from numerical modelling. Magnetocaloric properties of the Gd spheres were obtained experimentally and implemented in a one-dimensional numerical AMR model that includes also the parasitic losses from the prototype. The temperature span for a thermal load of 200 W as a function of frequency was measured and modelled. Moreover, the temperature span dependence on the cooling capacity as a function of cycle frequency was determined. It was found that thermal losses increase as the frequency increases. Therefore, a detailed study of these parasitic losses was carried out experimentally and numerically.

Abstract: The critical period of rice‐weed competition was studied for the rice varieties Oryzica 1 and CICA 8, in 1989 and 1990. Weed species were Eleusine indca, Echinochloa crus‐galli, Echinochloa colonum, Cyperus difformis, Cyperus esculentus, Cyperus iria, Leptochloa filiformis and Eclipta alba. Competition effects were similar for both varieties. Weeds emerging with the crop were the most damaging ones. Rice yields increased with longer weed‐free periods up to 70 days after emergence (d.a.e.). Weed emergence and growth after 70 d.a.e. was suppressed by the crop. Both rice cultivars required a critical height of 44–50 cm to suppress weed growth at 70 d.a.e. Though both varieties were equally weed suppressive by 70 d.a.e., leaf area index of CICA 8 was much larger than that of Oryzica 1 when rice grew weed‐free after emergence. Abstract: S1 is the most important locus acting as a reproductive barrier between Oryza sativa and O. glaberrima. It is a complex locus, with factors that may affect male and female fertility separately. Recently, the component causing the allelic elimination of pollen was fine mapped. However, the position and nature of the component causing female sterility remains unknown. To fine map the factor of the S1 locus affecting female fertility, we developed a mapping approach based on the evaluation of the degree of female transmission ratio distortion (fTRD) of markers. Through implementing this methodology in four O. sativa × O. glaberrima crosses, the female component of the S1 locus was mapped into a 27.8-kb (O. sativa) and 50.3-kb (O. glaberrima) region included within the interval bearing the male component of the locus. Moreover, evidence of additional factors interacting with S1 was also found. In light of the available data, a model where incompatibilities in epistatic interactions between S1 and the additional factors are the cause of the female sterility barrier between O. sativa and O. glaberrima was developed to explain the female sterility and the TRD mediated by S1. According to our model, the recombination ratio and allelic combinations between these factors would determine the final allelic frequencies observed for a given cross.

110. 19th century cataract surgery and surgeons in Mexico

Abstract: Cataract surgery has at least a 3000-year history, but it was not until the 19th century when it was refined to become a routine and successful procedure. Until early this century, the extracapsular technique, introduced by Daviel in 1748, was generally performed in Europe and America, and Mexico was no exception. The brightest and most distinguished Mexican physicians practiced that surgery. Several of their names designate the streets of "Colonia de los Doctores" in Mexico City. For the last quarter of the 19th century, ophthalmology was a defined and independent medical discipline in Mexico. The first Ophthalmologic Society in Latin America was established here in 1893, and in 1898 the oldest current journal of this specialty began its publication in Spanish. Since the 19th century--and perhaps before--cataract surgery in Mexico has been practiced at the same level as in the U.S. and Europe.

Abstract: Se formula una nueva propuesta sobre la economía de la salud que involucre a las instituciones. Las decisiones relacionadas con la salud se toman bajo algún tipo de incertidumbre, en especial en lo que tiene que ver con las instituciones que se ocupan o se desenvuelven en esta área. El abordar estas instituciones de una manera seria permite que pasemos de la economía de la atención médica (lugar en dónde se concentran la mayor parte de las investigaciones) a una economía real de la salud. Este paso también permite que expliquemos el aumento del gasto en salud como un resultado de la interacción entre las instituciones sociales. En este enfoque, la unidad del análisis se ha desplazado más hacia la norma que hacia algún agente individual; en resumidas cuentas, el agente del estudio es la institución. / Abstract. A reformulation of health economics is proposed that explicitly involves institutions. Decisions regarding health are taken under uncertainty, in the face of which institutions work and allow dealing with it. Taking institutions seriously allows moving forward from the economics of medical care (on which the mainstream has focused) to a real economics of health; it also allows explaining the growth of health expenditure as the result of the interaction of social institutions. In this approach, the unit of analysis is changed to the rule, rather than the individual agent. The subject of study is the institution.

112. Infecciones respiratorias agudas bajas en niños: Estudio etiológico prospectivo

Abstract: Resumen: Las infecciones respiratorias agudas bajas (IRAb) son causa importante de morbilidad y de hospitalizacion en los ninos. El objetivo de esta investigacion fue determinar la etiologia de IRAb grave de ninos hospitalizados por varios metodos diagnosticos en forma protocolizada. Metodo: Estudio prospectivo en dos anos consecutivos. Resultados: Un posible agente causal fue detectado en 155 (68%) de 229 pacientes: Infeccion viral fue identificada en 60%, bacteriana: 15% y mixta: 4%. Virus, principalmente virus respiratorio sincicial (VRS) fue el agente etiologico mas frecuente en menores de 24 meses. Mycoplasma pneumoniae y Streptococcus pneumoniae fueron las principales bacterias aisladas. Conclusiones: Un posible agente causal de la IRAb en ninos pudo ser identificado en la mayoria de los casos. Los datos confirman la importancia de los virus, especialmente VRS, en la IRAb en ninos hospitalizados

113. [Ultrastructure of the anterior capsule of the lens in different pathologies]

Abstract: We undertook this study to compare the ultrastructural analysis of capsules of normal lenses (phacorefractive procedures) and those of other types of cataracts also such as senile cataracts, with trans-surgical staining.Capsule samples were obtained by circular continuous capsulorhexis during phacoemulsification procedures. All samples were fixed and conventionally processed for light- and transmission-electron microscopy analysis.Seventeen analyzed samples were grouped as follows: a) phacorefractive (n = 2), b) senile cataract (n = 3); c) senile diabetic (n = 3), d) pseudoexfoliation syndrome (n = 3); e) stained with trypan blue (n = 2), f) indocyanine green (n = 2), and g) gentian violet (n = 2). According to morphological analysis, all samples of cataract demonstrated lacunae zones between epithelial cells. Filamentous annular bodies were found near the epithelium in capsule of senile cataracts. Senile diabetic cataract samples showed more abnormalities in epithelial morphology; specifically we found conformation of two or three layers of epithelial cells. Samples stained with trypan blue showed a regular basal membrane and minor loss of epithelial cells than other trans-surgical stain techniques.After an exhaustive medical literature search, we published the first report about the observation of filamentous annular bodies near the epithelium in capsules of senile cataracts. Additionally, it is important to note the presence of stratification of the epithelium at the anterior pole of capsules from diabetic patients.

Abstract: In recent years, the evaluation of in utero exposure to drugs of abuse has been achieved by testing biological matrices coming from the fetus or newborn (eg, meconium, fetal hair, cord blood, neonatal urine), the pregnant or nursing mother (eg, hair, blood, oral fluid, sweat, urine, breast milk), or from both the fetus and the mother (placenta, amniotic fluid). Overall, these matrices have the advantage of noninvasive collection (with the exception of amniotic fluid) and early detection of exposure from different gestational periods. Matrices such as amniotic fluid, meconium, fetal hair, and maternal hair provide a long historical record of prenatal exposure to certain drugs and can account for different periods of gestation: amniotic fluid from the early pregnancy, meconium for the second and third trimester of gestation, fetal hair for the third, and finally maternal hair (when long enough) for the whole pregnancy. Placenta may reveal the passage of a substance from the mother to the fetus. Cord blood and neonatal urine are useful for determining acute exposure to drugs of abuse in the period immediately previous to delivery. Drug detection in maternal blood, oral fluid, and sweat accounts only for acute consumption that occurred in the hours previous to collection and gives poor information concerning fetal exposure. Different immunoassays were used as screening methods for drug testing in the above-reported matrices or as unique analytical investigation tools when chromatographic techniques coupled to mass spectrometry were not commonly available. However, in the last decade, both liquid and gas chromatography-mass spectrometric methodologies have been routinely applied after appropriate extraction of drugs and their metabolites from these biological matrices.

Abstract: In this paper we employ techniques from artificial intelligence such as reinforcement learning and agent based modeling as building blocks of a computational model for an economy based on convention. First we model the interaction among firms in the private sector. These firms behave in an information environment based on conventions meaning that a firm is likely to behave as it neighbors if it observes that their actions lead to a good pay-off. On the other hand, we propose the use of reinforcement learning as a computational model for the role of goverment in the economy, as the agent that determines the fiscal policy, and whose objective is to maximize economy growth. We present the implementation of a simulator of the proposed model based on SWARM, that employs the SARSA algotithm combined wiht a multilayer perceptron as the function approximation for the action value function.

Abstract: Today, these exist worldwide organizations related with the practice of medicine that are interested in marking recommendations and in formulating guidelines for better performance of physicians and health professionals. These organizations are headed by universities, associations, colleges, research center, and work groups and are made up of well-known physicians with academic prestige from the private and public sectors. This report presents a brief overview concerning the development of three clinical guides related with illnesses frequently observed in the physician's office: rhinosinusitis gastroesophageal reflux diseases (GERD), and prostate benign hyperplasia. These guides are issued by one of the most important academic institutions in the country. In the above-mentioned works, the efforts of specialist who are opinion leaders in these topics form different states of the country converge. Likewise, critical points to consider for producing clinical guides are provided mechanisms of diffusion, consultation, feedback and updating of these documents, have already been provided and focus on national medical practice quality of services, and the manner in which to carry out diagnosis and therapy at the different levels of medical care, with emphasis on preventing complications.

Abstract: A procedure based on liquid chromatography-mass spectrometry (LC-MS) is described for determination of amphetamine, methamphetamine, and methylendioxy derivatives in meconium, using 3,4-methylendioxypropylamphetamine as internal standard. The analytes were initially extracted from the matrix by 17 mM methanolic HCl. Subsequently, a solid-phase extraction with Bondelut Certify columns was applied. Chromatography was performed on a C(18) reversed-phase column using a linear gradient of 10 mM ammonium bicarbonate, pH 9.0-methanol as a mobile phase. Analytes were determined in LC-MS single ion monitoring mode with an atmospheric pressure ionization-electrospray interface. The method was validated in the range 0.005-1.00 microg/g using 1 g of meconium per assay. Mean recoveries ranged between 61.1 and 87.2% for different analytes. The quantification limits were 0.005 microg/g meconium for amphetamine, methamphetamine, and 4-hydroxy-3-methoxymethamphetamine and 0.004 microg/g meconium for 3,4-methylenedioxyamphetamine, 3,4-methylenedioxymethamphetamine, 3,4-methylenedioxyethylamphetamine, and N-methyl-1-(3,4-methylenedioxyphenyl)-2-butanamine. The method was applied to analysis of meconium in newborns to assess eventual fetal exposure to amphetamine derivatives.

Abstract: Este ensayo busca señalar las diferencias de perspectiva que hay entre la economía institucional y la ciencia económica walrasiana, como parte de un intento por ilustrar la forma en que la economía institucional representa una alternativa, aunque incompleta, al enfoque convencional de la economía. La comparación entre las concepciones de individuo, el conocimiento, la eficiencia y el orden económico son las principales herramientas utilizadas para mostrar estas diferencias entre ambas perspectivas. Se señalan también algunas discusiones metodológicas derivadas de estas diferencias. This article presents a comparative analysis of the approaches of institutional economics and Walrasian economics. After reviewing the differences between both perspectives, it argues that institutional economics is an alternative, however incomplete, to general equilibrium economics. It compares the way both tendencies conceive the individual, knowledge, efficiency and the economic order, to point out the differences. Some methodological discussion derived from these differences are indicated.

Abstract: In Latin America early weed competition is a constraint upon upland rice production, leading to intensive herbicide use. Breeding for rice competitiveness may help reduce the need for herbicides. Two consecutive screenhouse pot experiments were conducted with 13 upland cultivars grown alone or in mixtures with Brachana brizantha to identify plant characteristics for early rice competitiveness which could serve as selection criteria. Rice growth reduction from B. brizantha competition was evident in all cultivars by 30 d.a.e., and the traits most related to cultivar competitiveness were: total above‐ground plant biomass > leaf weight > stem weight > leaf area. Plant height and tillering were least affected by competition. Rice and B. brizantha partially avoided competition since the competing canopies did not fully intercept the incoming light. Allergic reactions to CM: 29 children have had reactions, as follows: A) Non anaphylactic reactions: 15 children. 8 children suffer daily from mild symptoms (4, mouth itching; 1, abdominal pain; 1, hives; 2, rhinitis) and 4 children from mild asthma 2-3/week, with no triggers associated. 6 children had 1 to 6 mild respiratory or digestive symptoms related to trigger factors. B) 20 children have suffered from anaphylactic reactions, 50% of which are related to trigger factors. 14 patients had skin and respiratory involvement (rank: 65 to 1 episodes). 3 children had skin and digestive symptoms (rank: 4 to 1 episodes). 3 children, respiratory and digestive (rank:13 to 1 episodes). 7 children received epinephrine (39 doses). Number of children reacting to CM because of the following trigger factors: fasting, 5; exercise, 8 (66 episodes); lying, 12 (16 episodes); tiredness, 3; stress, 3; asthma exacerbation, 4; infection, 6. 9 children reacted to goat’s or sheep’s cheese and 4 to cow’s cheese. 7 children have reduced in 10 to 75% their daily milk doses because of allergic reactions. 2 children withdrew SOTI during the maintenance period due to intense and frequent reactions.

Abstract: En La Comarca Lagunera de México se siembran anualmente 24 000 ha de maíz (Zea mays L.) forrajero y 15 000 ha de maíz de grano. En este trabajo se determinó la aptitud combinatoria general (ACG) de líneas autofecundadas y la aptitud combinatoria específica (ACE) de sus cruzas, en maíz de grano adaptado a las condiciones climáticas de esta región. En la primavera del 2004 se formaron dos grupos de 10 líneas; un grupo se utilizó como macho y otro como hembra, para formar 100 cruzas simples directas posibles, bajo el diseño de apareamiento genético de Carolina del Norte II. En el verano del 2004 se evaluaron las cruzas en un diseño de bloques al azar con dos repeticiones. Las líneas macho (M1) y la hembra (H12) presentaron la mayor ACG y los rendimientos más altos de mazorca y de grano. Las cruzas 3x20, 7x14, 5x17, 1x16 y 2x11 tuvieron los mejores valores de ACE para los principales componentes de rendimiento evaluados; para la cruza 1x16 el rendimiento dependió del número de granos por hilera y del peso de mil granos, características aportadas por M1 y H16, respectivamente, mientras que para la cruza 5x17 fueron los componentes longitud de mazorca y granos por hilera, aportados por M5 y H17, respectivamente, cuya interacción se expresó en la ACE. El rendimiento de grano de la cruza 6x12 dependió principalmente del número de hileras por mazorca proveniente del M6 y el peso de mil granos aportado por la H12.

Abstract: This article presents the results of a pilot study about the implementation of informatics tools of social use HIUS (known as social software) perform on students of fourth grade on high school on a public school. We explored four dimensions: frequency of use, handling level, usage and type of interaction. The tools are grouped into 12 types. The population said they did not know/use many of the tools. We found that there is a high frequency of use and high skills in handling social networks, video sharing and chat services, while there is very little use and handling of tools like blogs and social bookmarking. The most commonly used mode was "chat/enjoy" followed by "to learn" and "to study". Abstract: Seven months following the volcanic eruption that destroyed the small town of Armero, 200 victims were screened for emotional problems with the Self-Reporting Questionnaire, a simple and reliable instrument. Fifty-five percent of the victims were found to be emotionally distressed. Variables associated with the presence of emotional distress included living alone, having lost previous job, feeling not being helped, not knowing date for leaving temporary shelter, being dissatisfied with living arrangements, complaining of non-specific physical symptoms or epigastric pain, and presenting several physical problems. The high prevalence of emotional distress supports the need to deliver mental care to disaster victims in developing countries through the primary level of care. Abstract: A novel rotary magnetic refrigeration device has been designed and constructed following the concepts recently outlined in Bahl et al. (2011). The magnet and flow system design allow for almost continuous usage of both the magnetic field and the magnetocaloric material in 24 cassettes, each containing an active magnetic regenerator (AMR) bed. As outlined in Pryds et al. (2009) a small scale AMR test device has been used for materials choice and optimising operation, with each component being thoroughly characterised and tested before implementation. The prototype design facilitates easy exchange of the 24 cassettes, allowing the testing of different material amounts and compositions. Operating with 2.8 kg of commercial grade Gd spheres a maximum no-span cooling power of 1010 W and a maximum zero load temperature span of 25.4 K have been achieved. For the purpose of actual operation, simultaneous high span and high performance is required. At a heat load of 200 W a high temperature span of 18.9 K has been obtained, dropping to a span of 13.8 K at the higher heat load value of 400 W.
