Background: The Global Anticoagulant Registry in the FIELD–Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. Methods and results: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012–2016). Abstract Background The optimal approach to the drainage of malignant obstruction at the liver hilum remains uncertain. We aim to compare percutaneous transhepatic biliary drainage (PTBD) to endoscopic retrograde cholangiography (ERC) as the first intervention in patients with cholestasis due to suspected malignant hilar obstruction (MHO). Methods The INTERCPT trial is a multi-center, comparative effectiveness, randomized, superiority trial of PTBD vs. ERC for decompression of suspected MHO. One hundred and eighty-four eligible patients across medical centers in the United States, who provide informed consent, will be randomly assigned in 1:1 fashion via a web-based electronic randomization system to either ERC or PTBD as the initial drainage and, if indicated, diagnostic procedure. All subsequent clinical interventions, including crossover to the alternative procedure, will be dictated by treating physicians per usual clinical care. Enrolled subjects will be assessed for successful biliary drainage (primary outcome measure), adequate tissue diagnosis, adverse events, the need for additional procedures, hospitalizations, and oncological outcomes over a 6-month follow-up period. Subjects, treating clinicians and outcome assessors will not be blinded. Discussion The INTERCPT trial is designed to determine whether PTBD or ERC is the better initial approach when managing a patient with suspected MHO, a common clinical dilemma that has never been investigated in a randomized trial. The Maggi-Rubinowicz technique for scalar and vector fields can be interpreted as a transformation of an integral over an open surface to a line integral around the rim and geometrical optics terms. Using this transformation, Maggi-Rubinowicz analogues are obtained for several integral representations of the electric field. In the case of diffraction from a circular aperture, numerical comparisons between a physical optics and the corresponding Maggi-Rubinowicz formulation show the methods to be in good agreement. To circumvent certain convergence difficulties in the Maggi-Rubinowicz solution that occur as the observer approaches the shadow boundary, a variable mesh integration is used. For the examples considered, where the ratio of the aperture diameter to wavelength is about ten, the Maggi-Rubinowicz formulation yields a significant decrease in computation time relative to the physical optics formulation.

SUMMARY In the early postoperative period, oxygen consumption was close to basal (range 71–135 per cent of basal) in fourteen non-shivering patients but raised within the range 135–486 per cent in ten shivering patients. There was no correlation between oxygen consumption and arterial Po2, which ranged from 57 to 89 mm Hg in non-shivering patients and from 61 to 96 mm Hg in shivering patients. In every case, ventilation was adequate for the increased metabolic demand of shivering and the highest arterial Pco2 was 42.6 mm Hg. Two non-shivering patients had Pco2 of 46 and 52 mm Hg respectively. In two out of three shivering patients, cardiac output was adequate for demand and mixed venous oxygen content was within the normal range. In the third shivering patient, a cardiac output of 9.4 l./min was inadequate for an oxygen consumption of 1,192 ml/min and the arterial/mixed venous oxygen content difference was 12.7 ml/100 ml. This was a factor in the associated arterial hypoxaemia (65 mm Hg). In other patients arterial Po2 was mainly influenced by venous admixture which ranged from 3.5 to 13.8 per cent.

ROENTGEN could neither have visualized the magnitude of his discovery nor the perfection of technic which was to follow. Its applications have become infinitely numerous and conveniently yield positive information, in obstetrics alone, to say nothing of the dependence of the other branches of medicine upon this specialty. According to Orley (1), the first radiography of the pelvis was performed by Varnier and Chappius in 1896, on the cadaver, and required an exposure of three hours. In 1897, Professor Budin exhibited a radiograph of a Naegele pelvis; the exposure time was 45 minutes. Our expectations, constantly becoming more demanding, have resulted in the production of sharp films with a minimum time of exposure. Oxygen tension in lumbar cerebrospinal fluid was monitored during halothane-N2O anaesthesia in 6 patients undergoing craniotomy. Beckman's oxygen-micro electrode was used. Results showed (1) a rise in oxygen tension during the beginning of the anaesthesia which is largely ascribed to an increase in cerebral blood flow induced by the anaesthetic, (2) a fall in oxygen tension during 30 minutes of hyper-ventilation where arterial CO2 tension decreased to 20 mm Hg, (3) a minimum oxygen tension measured during hyperventilation which is within the limits of normal CSF oxygen tension reported by others. It is argued that lumbar CSF oxygen tension may represent an average tissue oxygen tension of the central nervous system.

SUMMARY The physiological derangements during and following the administration of higher oxides of nitrogen have been studied in dogs anaesthetized with pentobarbitone. The dogs were exposed to concentrations of nitric oxide or nitrogen dioxide between 0.1 and 2.0 per cent over periods between 5 and 136 minutes. Despite the inhalation of 98 per cent oxygen, death was always associated with a critical reduction in arterial oxygen content. This was caused by one or more of the following three factors: methaemoglobinaemia, low arterial Po2, and acidaemia which caused a shift of the oxyhaemoglobin dissociation curve. The reduction of arterial Po2 was caused by an outpouring of fluid into the alveoli.

The pathology and classification of brain tumors has been dealt with in previous chapters. If one attempts to group brain tumors into clinicopathological entities to provide guidance for therapeutic decisions as well as for comparisons of treatment results, simplifications, omissions and generalizations have to be made. It may therefore be pudent to repeat and emphasize certain unique and puzzling properties characteristic of brain tumors. These are reflected in the number of classifications. The study of existing classifications and their historical evolution will provide insight into the behavior of these tumors. It appears mandatory that close cooperation exists between the neuropathologist and his clinical colleagues in order to provide for rational treatment decisions and meaningful evaluation of treatment results.

This paper presents a distributed approach to performing real-time optimization of large wind farms. Wind turbines in a wind farm typically operate individually to maximize their own performance regardless of the impact of aerodynamic interactions on neighboring turbines. This paper optimizes the overall power produced by a wind farm by formulating and solving a nonconvex optimization problem where the yaw angles are optimized to allow some turbines to operate in misaligned conditions and shape the aerodynamic interactions in a favorable way. The solution of the nonconvex smooth problem is tackled using a proximal primal-dual gradient method, which provably identifies a first-order stationary solution in a global sublinear manner. By adding auxiliary optimization variables for every pair of turbines that are coupled aerodynamically, and properly adding consensus constraints into the underlying problem, a distributed algorithm with turbine-to-turbine message passing is obtained; this allows for turbines to be optimized in parallel using local information rather than information from the whole wind farm. This algorithm is computationally light, as it involves closed-form updates. This approach is demonstrated on a large wind farm with 60 turbines. The results indicate that similar performance can be achieved as with finite-difference gradient-based optimization at a fraction of the computational time and thus approaching real-time control/optimization.

The SIEBM (Iberian Symposium of Studies on Marine Biology) has a long tradition. The conference was first convened in 1979 and it has since been held approximately biannually. It was originally focused on marine benthos, but two editions ago it was decided to widen its objectives to include other related subjects. The 15th edition was held at the University of Alicante from 6th to 10th September 2010. One of the strong points of SIEBM is that it provides the opportunity for interaction between well–known researchers and young marine scientists, mainly from Spain and Portugal. In the 2010 edition, however, it was attended by 268 participants from 15 countries. SIEBM 2010 focused on the human impact on marine ecosystems and the conservation of marine biodiversity to coincide with the United Nations declaring 2010 the International Year of Biodiversity. From a total number of 330 communications received, the Scientific Committee selected 70 for oral presentations and the rest were presented as posters. Each day a plenary conference covered what we considered as the fastest growing areas in marine biology: landscape ecology. The impact of global warming on marine ecosystems, approaches to fisheries management and the study and conservation of deep ecosystems. The oral presentations were divided into 15 sessions that included both basic research on biodiversity, phylogeny or marine ecology, and applied research in management, fisheries, conservation and the human impact on marine ecosystems. Abstracts, posters and presentations are available at http://www.siebm.org. The studies included in this volume show the variety of subjects and approaches addressed during the conference. These papers were selected and edited by a group of referees and we wish to thank them for their efforts and conscientious work. A list of their names is included. Of the 28 studies received, 13 were finally accepted for this volume. We also would like to thank all the people who helped in one way or another to make the 15th SIEBM Conference and the proceedings such a success. We thank the Scientific Committee for revising the abstracts and selecting the oral presentations. Most members of this committee also chaired sessions and undertook this task with commitment and dedication. The planning, logistics and running of the conference were made possible thanks to the enthusiasm of the organising committee and a group of student helpers. Layout design and wake steering through wind plant control are important and complex components in the design and operation of modern wind power plants. They are currently optimized separately, but with more and more computational and experimental studies demonstrating the gains possible through wake steering, there is a growing need from industry and regulating bodies to combine the layout and control optimization in a co-design process. However, combining these two optimization problems is currently infeasible due to the excessive number of design variables and large solution space. In this article, we present a method that enables the coupled optimization of wind power plant layout and wake steering with no additional computational expense than a traditional layout optimization. We developed a geometric relationship between wind turbines to find an approximate optimal yaw angle, bypassing the need for either a nested or coupled wind plant control optimization. It also provides a significant and immediate improvement to wind power plant design by enabling the co-design of turbine layout and yaw control for wake steering. A small co-designed plant shown in this article produces 0.8 % more energy than its sequentially designed counterpart. This additional energy production comes with no additional infrastructure, turbine hardware, or control software; it is simply the outcome of optimizing the turbine layout and yaw control together, resulting in millions of dollars of additional revenue for the wind power plants of the future.

Wind farm design and analysis heavily rely on computationally efficient engineering models that are evaluated many times to find an optimal solution. A recent article compared the state-of-the-art Gauss-curl hybrid (GCH) model to historical data of three offshore wind farms. Two points of model discrepancy were identified therein: poor wake predictions for turbines experiencing a lot of wakes and wake interactions between two turbines over long distances. The present article addresses those two concerns and presents the cumulative-curl (CC) model. Comparison of the CC model to high-fidelity simulation data and historical data of three offshore wind farms confirms the improved accuracy of the CC model over the GCH model in situations with large wake losses and wake recovery over large inter-turbine distances. Additionally, the CC model performs comparably to the GCH model for single- and fewer-turbine wake interactions, which were already accurately modeled. Lastly, the CC model has been implemented in a vectorized form, greatly reducing the computation time for many wind conditions. The CC model now enables reliable simulation studies for both small and large offshore wind farms at a low computational cost, thereby making it an ideal candidate for wake-steering optimization and layout optimization.

Annual energy production (AEP) is often the objective function in wind plant layout optimization studies. The conventional method to compute AEP for a wind farm is to first evaluate power production for each discrete wind direction and speed using either computational fluid dynamics simulations or engineering wake models. The AEP is then calculated by weighted-averaging (based on the wind rose at the wind farm site) the power produced across all wind directions and speeds. We propose a novel formulation for time-averaged wake velocity that incorporates an analytical integral of a wake deficit model across every wind direction. This approach computes the average flow field more efficiently, and layout optimization is an obvious application to exploit this benefit. The clear advantage of this new approach is that the layout optimization produces solutions with comparable AEP performance yet is completed 2 orders of magnitude faster. The analytical integral and the use of a Fourier expansion to express the wind speed and wind direction frequency create a relatively smooth solution space for the gradient-based optimizer to excel in comparison to the existing weighted-averaging power calculation.

146. Objective and algorithm considerations when optimizing the number and placement of turbines in a wind power plant

Optimizing turbine layout is a challenging problem that has been extensively researched in the literature. However, optimizing the number of turbines within a given boundary has not been studied as extensively and is a difficult problem because it introduces discrete design variables and a discontinuous design space. An essential step in performing wind power plant layout optimization is to define the objective function, or value, that is used to express what is valuable to a wind power plant developer, such as annual energy production, cost of energy, or profit. In this paper, we demonstrate the importance of selecting the appropriate objective function when optimizing a wind power plant in a land-constrained site. We optimized several different wind power plants with different wind resources and boundary sizes. Results show that the optimal number of turbines varies drastically depending on the objective function. For a simple, one-dimensional, land-based scenario, we found that a wind power plant optimized for minimal cost of energy produced just 72 % of the profit compared to the wind power plant optimized for maximum profit, which corresponded to a loss of about USD 2 million each year. This paper also compares the performance of several different optimization algorithms, including a novel repeated-sweep algorithm that we developed. We found that the performance of each algorithm depended on the number of design variables in the problem as well as the objective function.

This paper presents a model to incorporate the secondary effects of wake steering in large arrays of turbines. Previous models have focused on the aerodynamic interaction of wake steering between two turbines. The model proposed in this paper builds on these models to include yaw-induced wake recovery and secondary steering seen in large arrays of turbines when wake steering is performed. Turbines operating in yaw-misaligned conditions generate counter-rotating vortices that entrain momentum and contribute to the deformation and deflection of the wake at downstream turbines. Rows of turbines can compound the effects of wake steering that benefit turbines far downstream. This model quantifies these effects and demonstrates that wake steering has greater potential to increase the performance of a wind farm due to these counter-rotating vortices especially for large rows of turbines. This is validated using numerous large-eddy simulations for three-turbine, five-turbine, and wind farm scenarios.

Methods of turbine wake modeling are being developed to more accurately account for spatially variant atmospheric conditions within wind farms. Most current wake modeling utilities are designed to apply a uniform flow field to the entire domain of a wind farm. When this method is used, the accuracy of power prediction and wind farm controls can be compromised depending on the flow-field characteristics of a particular area. In an effort to improve strategies of wind farm wake modeling and power prediction, FLOw Redirection and Induction in Steady State (FLORIS) was developed to implement sophisticated methods of atmospheric characterization and power output calculation. In this paper, we describe an adapted FLORIS model that features spatial heterogeneity in flow-field characterization. This model approximates an observed flow field by interpolating from a set of atmospheric measurements that represent local weather conditions. The objective of this method is to capture heterogeneous atmospheric effects caused by site-specific terrain features, without explicitly modeling the geometry of the wind farm terrain. The implemented adaptations were validated by comparing the simulated power predictions generated from FLORIS to the actual recorded wind farm output from the supervisory control and data acquisition (SCADA) recordings and large eddy simulations (LESs). When comparing the performance of the proposed heterogeneous model to homogeneous FLORIS simulations, the results show a 14.6 % decrease for mean absolute error (MAE) in wind farm power output predictions for cases using wind farm SCADA data and a 18.9 % decrease in LES case studies. The results of these studies also indicate that the efficacy of the proposed modeling techniques may vary with differing site-specific operational conditions. This work quantifies the accuracy of wind plant power predictions under heterogeneous flow conditions and establishes best practices for atmospheric surveying for wake modeling.

Wind turbine wake models typically require approximations, such as wake superposition and deflection models, to accurately describe wake physics. However, capturing the phenomena of interest, such as the curled wake and interaction of multiple wakes, in wind power plant flows comes with an increased computational cost. To address this, we propose a new hybrid method that uses analytical solutions with an approximate form of the Reynolds-averaged Navier–Stokes equations to solve the time-averaged flow over a wind plant. We compare results from the solver to supervisory control and data acquisition data from the Lillgrund wind plant obtaining wake model predictions which are generally within 1 standard deviation of the mean power data. We perform simulations of flow over the Columbia River Gorge to demonstrate the capabilities of the model in complex terrain. We also apply the solver to a case with wake steering, which agreed well with large-eddy simulations. This new solver reduces the time – and therefore the related cost – it takes to simulate a steady-state wind plant flow (on the order of seconds using one core). Because the model is computationally efficient, it can also be used for different applications including wake steering for wind power plants and layout optimization.

We examine the effect of rotor design choices on the power capture and structural loading of each major wind turbine component. A harmonic model for structural loading is derived from simulations using the National Renewable Energy Laboratory (NREL) aeroelastic code FAST to reduce computational expense while evaluating design trade-offs for rotors with radii greater than 100 m. Design studies are performed, which focus on blade aerodynamic and structural parameters as well as different hub configurations and nacelle placements atop the tower. The effects of tower design and closed-loop control are also analyzed. Design loads are calculated according to the IEC design standards and used to create a mapping from the harmonic model of the loads and quantify the uncertainty of the transformation. Our design studies highlight both industry trends and innovative designs: we progress from a conventional, upwind, three-bladed rotor to a rotor with longer, more slender blades that is downwind and two-bladed. For a 13 MW design, we show that increasing the blade length by 25 m, while decreasing the induction factor of the rotor, increases annual energy capture by 11 % while constraining peak blade loads. A downwind, two-bladed rotor design is analyzed, with a focus on its ability to reduce peak blade loads by 10 % per 5∘ of cone angle and also reduce total blade mass. However, when compared to conventional, three-bladed, upwind designs, the peak main-bearing load of the upscaled, downwind, two-bladed rotor is increased by 280 %. Optimized teeter configurations and individual pitch control can reduce non-rotating damage equivalent loads by 45 % and 22 %, respectively, compared with fixed-hub designs.
