28 p., ill, En Belgique, chaque femme enceinte peut, si elle le souhaite, obtenir une estimation du risque que le fœtus soit porteur du syndrome de Down (trisomie 21 – T21). Actuellement, cette estimation est basée sur une biochimie sanguine et une échographie. Lorsque ce risque s'avère élevé, un test invasif (une biopsie de villosités choriales ou une amniocentèse) est alors pratiqué pour confirmer ou infirmer ce diagnostic. Toutefois, le nombre de fœtus porteurs d’une T21 non décelés par cette approche demeure important. A cela s’ajoute le fait que ces tests invasifs sont associés à un risque de fausse-couche de l’ordre de 1 %. Le test prénatal non invasif (NIPT) est un test génétique relativement récent effectué sur le sang de la mère, plus sensible et plus précis que le dépistage classique. Toutefois, il est toujours relativement onéreux. La ministre a demandé au Conseil Supérieur de la Santé (CSS) et au Centre Fédéral d'Expertise des Soins de Santé (KCE) d'effectuer une évaluation du NIPT. L'évaluation du KCE concernait surtout les aspects relatifs à l'économie de la santé. L'examen a porté sur deux façons dont le NIPT pourrait être introduit dans le cadre du dépistage prénatal du syndrome de Down : (1) Soit le NIPT est effectué en première ligne au lieu de la procédure de dépistage actuelle ; (2) Soit le NIPT est pratiqué en seconde ligne et uniquement lorsque les tests de dépistage actuels sur la base d'une analyse sanguine et d'une échographie indiquent un risque accru. Chacune de ces deux options permet d'aboutir à une estimation du risque nettement plus fiable que celle obtenue dans le cadre du dépistage actuel. Elles permettent en outre de réduire le nombre de tests invasifs pratiqués. Dès lors, elles représentent une amélioration par rapport au dépistage actuel. Au niveau purement technique, la préférence doit être accordée à l'option (1), dans laquelle le NIPT est proposé comme test de dépistage en première instance. 28 p., ill, Alle zwangere vrouwen in ons land die dit wensen kunnen vandaag het risico op het syndroom van Down (trisomie 21 - T21) laten vaststellen door een biochemische bloedtest en een echografie. Bij een verhoogd risico wordt vervolgens de diagnose bevestigd of weerlegd door een invasieve test (vlokkentest of vruchtwaterpunctie). Bij deze aanpak worden echter nog heel wat foetussen met T21 gemist, en bij de invasieve testen is er ook een risico van 1% op het onvrijwillig afbreken van de zwangerschap. De niet-invasieve prenatale test (NIPT) is een relatief nieuwe genetische bloedtest, die gevoeliger en nauwkeuriger is dan de klassieke testen, maar die nog erg duur is.De minister vroeg een evaluatie van de NIPT aan de Hoge Gezondheidsraad (HGR) en het Federaal Kenniscentrum voor de Gezondheidszorg (KCE). De evaluatie door het KCE ging vooral over de gezondheidseconomische aspecten. Twee mogelijke manieren om NIPT in te voeren werden onderzocht: (1) NIPT in de eerste lijn ter vervanging van de huidige screeningsprocedure, of (2) NIPT in de tweede lijn enkel nadat de huidige screeningtesten met bloed en echo een verhoogd risico aangeven. Beide opties geven een veel betere voorspelling dan de huidige screening, doen het aantal invasieve testen dalen en zijn een verbetering ten opzichte van de huidige screening. Optie (1), waarbij NIPT wordt aangeboden als screeningstest is puur technisch te verkiezen omdat niet alleen het aantal invasieve testen daalt maar ook minder foetussen die drager zijn van T21 gemist worden. Maaronder meer omdat de kost van de NIPT test vandaag relatief hoog is (460€) wordt op dit ogenblik echter aanbevolen NIPT te vergoeden in optie (2). Mits een substantiële daling van de prijs van de NIPT-test en voldoende kwaliteitsgaranties wordt de terugbetaling van NIPT in optie (1) aanbevolen. p. 1-10, The first- and second-trimester screening for trisomy 21 (T21) are reimbursed for all pregnant women in Belgium. Using a cut-off risk of 1:300 for T21, about 5% of all pregnant women are referred for definitive prenatal diagnosis using an invasive test, at a sensitivity of (only) 72.5%. The sensitivity and specificity of the non-invasive prenatal test (NIPT) are over 99% but come at a cost of €460 (£373) per test. The objective is to estimate the consequences of introducing NIPT for the detection of T21. METHODS: A cost-consequences analysis was performed presenting the impact on benefits, harms and costs. Context-specific real-world information was available to set up a model reflecting the current screening situation in Belgium. This model was used to construct the second and first line NIPT screening scenarios applying information from the literature on NIPT's test accuracy. RESULTS: Introducing NIPT in the first or second line reduces harm by decreasing the number of procedure-related miscarriages after invasive testing. In contrast with NIPT in the second line, offering NIPT in the first line additionally will miss fewer cases of T21 due to less false-negative test results. The introduction of NIPT in the second line results in cost savings, which is not true for NIPT at the current price in the first line. If NIPT is offered to all pregnant women, the price should be lowered to about €150 to keep the screening cost per T21 diagnosis constant. CONCLUSIONS: In Belgium, the introduction and reimbursement of NIPT as a second line triage test significantly reduces procedure-related miscarriages without increasing the short-term screening costs. Offering and reimbursing NIPT in the first line to all pregnant women is preferred in the long term, as it would, in addition, miss fewer cases of T21. However, taking into account the government's limited resources for universal reimbursement, the price of NIPT should first be lowered substantially before this can be realis

105-110, Background Innovative high-risk medical devices, such as new types of heart valves or hip prostheses, become available on the European market more rapidly than in USA. This is due to the European legislation allowing early marketing of innovative high-risk medical devices before high-quality clinical evidence is obtained from randomized controlled trials. Methods We studied the premarket clinical evaluation of innovative high-risk medical devices in Europe compared with the USA. We also discussed patient safety and the transparency of information. The literature and regulatory documents were checked. Representatives from industry, competent authorities, notified bodies, ethics committees, and health technology assessment agencies were consulted. Results In contrast to the US, there is no requirement in Europe to demonstrate the clinical efficacy of high-risk devices in the premarket phase. For the patient, this implies earlier access to innovative technology, but at the risk of potential safety issues. At this moment, European requirements for clinical studies are lower for medical devices than for drugs, and data from premarket clinical trials are scarce or remain unpublished. The European Medical Device Directives are currently being reworked. Conclusions For innovative high-risk devices, and while awaiting a reworked Medical Device Directive, patient risk should be minimized by limiting the market introduction of novel high-risk devices with minimal clinical data to physicians with the necessary training and expertise. The new European legislation should require the premarket demonstration of clinical efficacy and safety, using a randomized controlled trial if possible, and a transparent clinical review, preferably centralized.

35-41, Objectives: We studied the cost-effectiveness of tenofovir and entecavir in e antigen positive (CHBe+) and negative (CHBe-) chronic hepatitis B. Methods: Using a multicenter survey including 544 patients we measured patient quality of life and attributable costs by clinical disease stage. Natural disease progression was studied in 278 patients in a single center. A Markov model was constructed to follow hypothetical cohorts of treated and untreated 40-year-old CHBe+ and CHBe- patients and 50-year-old patients with compensated cirrhosis. Results: We did not find an improvement in quality of life when viral load was reduced under treatment. Transition rates to liver cirrhosis were found to be age-dependent. Assuming equal effectiveness, tenofovir dominates the entecavir strategy because of its lower price in Belgium. The incremental cost-effectiveness ratio (ICER) of tenofovir after 20 years is more favorable for treating Caucasian cirrhotic patients (mean ICER €29,000/quality-adjusted life-year [QALY]) compared with treating non-cirrhotic patients (mean ICER €110,000 and 131,000/QALY for CHB e+ and e-, respectively). Within the non-cirrhotic patients the ICER decreases with increasing cohort starting age from 30 to 50 years. Conclusions: Results of long-term models for tenofovir or entecavir treatment of CHB need to be interpreted with caution as long-term trials with hard end points are lacking. 29 p., Ill., Een biosimilar is een “look –alike” van een biologisch geneesmiddel, dat echter wel 20 tot 34 % goedkoper is dan het originele product. Vrij recent, in de zomer van 2012, nam de minister van Volksgezondheid maatregelen om het gebruik van biosimilaire geneesmiddelen aan te moedigen, zonder aanwijsbaar succes tot nu toe. Op vraag van de minister ging het Federaal Kenniscentrum voor de Gezondheidszorg (KCE) na waarom biosimilairen in ons land alsnog niet doorbreken. De kortingen (tot 75% voor sommige producten) en andere voordelen van de fabrikanten van de originelen aan de ziekenhuizen maken dat de biosimilairen uiteindelijk voor hen niet concurrentieel zijn. Tegelijkertijd betaalt het RIZIV de ziekenhuizen immers nog steeds terug op basis van de officiële prijs van het origineel. Daarnaast twijfelen veel artsen aan de doeltreffendheid en veiligheid van biosimilairen, en dit is zeker mee te wijten aan het gebrek aan informatie en klinische gegevens.

28 p., Ill., Les biosimilaires sont des « quasi-copies » de médicaments biologiques, mais dont le prix est de 20 à 34 % inférieur à celui de l’original. La ministre de la Santé publique a récemment – dans le courant de l’été 2012 – pris un certain nombre de mesures afin d’en promouvoir l’usage, mais jusqu’ici sans succès démontré. A sa demande, le Centre Fédéral d'Expertise des Soins de Santé (KCE) a examiné les raisons pour lesquelles les biosimilaires n’ont toujours pas percé dans notre pays. On constate d’abord que lorsque l’on prend en compte les ristournes (jusqu’à 75 % pour certaines spécialités) et les autres avantages offerts aux hôpitaux par les fabricants des produits originaux, les biosimilaires ne sont pas concurrentiels pour les hôpitaux d’autant que le montant que l’INAMI rembourse aux établissements de soins reste en effet toujours basé sur le prix officiel du produit original. On constate également que les médecins éprouvent une certaine méfiance quant à l’efficacité et la sécurité des biosimilaires, une attitude certainement liée au manque d’informations et de données cliniques.

In this scientific policy advice report, the Superior Health Council provides recommendations and policy options which are fostering an ethically and medically appropriate offer of direct-to-consumer genetic tests, as the one provided through the healthcare system in Belgium

278-284, Objectives: High-quality clinical evidence is most often lacking when novel high-risk devices enter the European market. At the same time, a randomized controlled trial (RCT) is often initiated as a requirement for obtaining market access in the US. Should coverage in Europe be postponed until RCT data are available? We studied the premarket clinical evaluation of innovative high-risk medical devices in Europe compared with the US, and with medicines, where appropriate. Methods: The literature and regulatory documents were checked. Representatives from industry, Competent Authorities, Notified Bodies, Ethics Committees, and HTA agencies were consulted. We also discuss patient safety and the transparency of information. Results: In contrast to the US, there is no requirement in Europe to demonstrate the clinical efficacy of high-risk devices in the premarket phase. Patients in Europe can thus have earlier access to a potentially lifesaving device, but at the risk of insufficiently documented efficacy and safety. Variations in the stringency of clinical reviews, both at the level of Notified Bodies and Competent Authorities, do not guarantee patient safety. We tried to document the design of premarket trials in Europe and number of patients exposed, but failed as this information is not made public. Furthermore, the Helsinki Declaration is not followed with respect to the registration and publication of premarket trials. Conclusions: For innovative high-risk devices, new EU legislation should require the premarket demonstration of clinical efficacy and safety, using an RCT if possible, and a transparent clinical review, preferably centralized.

p. 126-136, Assessing the overall burden of disease which can be attributed to hospital-acquired infections (HAIs) remains a challenge. A matched cohort study was performed to estimate excess mortality, length of stay and costs attributable to HAIs in Belgian acute-care hospitals, using six matching factors (hospital, diagnosis-related group, age, ward, Charlson score, estimated length of stay prior to infection). Information was combined from different sources on the epidemiology and burden of HAIs to estimate the impact at national level. The total number of patients affected by a HAI each year was 125 000 (per 10·9 million inhabitants). The excess mortality was 2·8% and excess length of stay was 7·3 days, corresponding to a public healthcare cost of €290 million. A large burden was observed outside the intensive-care unit setting (87% of patients infected and extra costs, 73% of excess deaths).

477-89, The European Conformity (CE) marking grants early market introduction to innovative high risk medical devices based on safety and device performance only, without any requirement to demonstrate clinical efficacy or effectiveness. Hence healthcare providers, patients and payers are informed neither about the added clinical value compared to an existing medical device nor about the risks incurred by using such innovations. In addition there is a lack of coherence and uniformity of approach in the assessment of high risk medical devices. These gaps may put the health and safety of patients in danger. The European Commission, in concert with Competent Authorities, industry, Notified Bodies, and other stakeholders, is working on a "recast" of the directives regulating medical devices. This article identifies and discusses the critical points of the pre-market clinical evaluation of innovative high-risk medical devices in the European legal framework and compares it with the USA.

Les décideurs ont demandé au Centre fédéral d’expertise des soins de santé (KCE) d’identifier quels groupes de patients sont susceptibles de bénéficier le plus de ce traitement, et à quel prix.
