Abstract

The interval between the Cambrian ‘explosion’ and the Great Ordovician Biodiversification Event (GOBE) is marred by biotic turmoil.Several mass extinction events have been recognized throughout the Cambrian,with multiple Furongian (late) Cambrian extinctions mostly seen within trilobite fauna of North America.These extinctions appear to correspond with episodic ocean anoxia and carbon cycle perturbations, all of which are set to the backdrop of an extensive greenhouse climate.Here we will present new chemostratigraphic and compiled biostratigraphic data from Cambrian successions of the NorthChina/Sino-Korean block(NCB). These data help identify newly recognized trilobite extinctions in upper Cambrian carbonate platform deposits of the NCB that may be correlative to North American events, demonstrating their global extent.These extinctions also correspond with marked G13Cexcursions. The cessation of these repeated extinctions coincides with global reductions in arc-volcanism and climatic cooling, implying a causal linkage between baseline climate conditions and this extensive interval of biospheric perturbations.Collectively, these data help further elucidate the harsh environmental conditions that hindered early animal evolution. Abstract

Copernicus is the European programme for monitoring the Earth. It consists of a set of systems that collect data from satellites and in-situ sensors, process this data and provide users with reliable and up-to-date information on a range of environmental and security issues. The data and information processed and disseminated puts Copernicus at the forefront of the big data paradigm, giving rise to all relevant challenges, the so-called 5 Vs: volume, velocity, variety, veracity and value. In this short paper, we discuss the challenges of extracting information and knowledge from huge archives of Copernicus data. We propose to achieve this by scale-out distributed deep learning techniques that run on very big clusters offering virtual machines and GPUs. We also discuss the challenges of achieving scalability in the management of the extreme volumes of information and knowledge extracted from Copernicus data. Abstract

There is growing interest in using observational data to assess the safety, effectiveness, and cost effectiveness of medical technologies, but operational, technical, and methodological challenges limit its more widespread use. Common data models and federated data networks offer a potential solution to many of these problems. The open-source Observational and Medical Outcomes Partnerships (OMOP) common data model standardises the structure, format, and terminologies of otherwise disparate datasets, enabling the execution of common analytical code across a federated data network in which only code and aggregate results are shared. While common data models are increasingly used in regulatory decision making, relatively little attention has been given to their use in health technology assessment (HTA). We show that the common data model has the potential to facilitate access to relevant data, enable multidatabase studies to enhance statistical power and transfer results across populations and settings to meet the needs of local HTA decision makers, and validate findings. The use of open-source and standardised analytics improves transparency and reduces coding errors, thereby increasing confidence in the results. Further engagement from the HTA community is required to inform the appropriate standards for mapping data to the common data model and to design tools that can support evidence generation and decision making.

113. The health of adolescents in detention: a global scoping review

Abstract

Adolescents detained within the criminal justice system are affected by complex health problems, health-risk behaviours, and high rates of premature death. We did a global synthesis of the evidence regarding the health of this population. We searched Embase, PsycINFO, Education Resources Information Center, PubMed, Web of Science, CINCH, Global Health, the Cochrane Database of Systematic Reviews, the Campbell Library, the National Criminal Justice Reference System Abstract database, and Google Scholar for peer-reviewed journal articles, including reviews, that reported the prevalence of at least one health outcome (physical, mental, sexual, infectious, and neurocognitive) in adolescents (aged <20 years) in detention, and were published between Jan 1, 1980, and June 30, 2018. The reference lists of published review articles were scrutinised for additional relevant publications. Two reviewers independently screened titles and abstracts, and three reviewed full texts of relevant articles. The protocol for this Review was registered with PROSPERO (CRD42016041392). 245 articles (204 primary research articles and 41 reviews) were included, with most primary research (183 [90%]) done in high-income countries. A high lifetime prevalence of health problems, risks, and conditions was reported in detained adolescents, including mental disorders (0–95%), substance use disorders (22–96%), selfharm (12–65%), neurodevelopmental disabilities (2–47%), infectious diseases (0–34%), and sexual and reproductive conditions (pregnant by age 19 years 20–37%; abnormal cervical screening test result 16%). Various physical and mental health problems and health-risk behaviours are more common among adolescents in detention than among their peers who have not been detained. Abstract

Traditional approaches to understanding and responding to children and crime are fundamentally based on ‘miniaturised’ adult models. The assumption appears to be that children are adults in the making, essentially just smaller, developing versions of grown-ups. This view of children is increasingly being challenged. Children are not simply putative adults, they are different, distinct and developing. This article sets out to explore the notion that children essentially think and behave ‘in the moment’. The implications of this for our understanding of children and crime are also explored.

Abstract

Background: KCNJ11 mutations cause permanent neonatal diabetes through pancreatic ATP-sensitive potassium channel activation. 90% of patients successfully transfer from insulin to oral sulfonylureas with excellent initial glycaemic control; however, whether this control is maintained in the long term is unclear. Sulfonylurea failure is seen in about 44% of people with type 2 diabetes after 5 years of treatment. Therefore, we did a 10-year multicentre follow-up study of a large international cohort of patients with KCNJ11 permanent neonatal diabetes to address the key questions relating to long-term efficacy and safety of sulfonylureas in these patients. Methods: In this multicentre, international cohort study, all patients diagnosed with KCNJ11 permanent neonatal diabetes at five laboratories in Exeter (UK), Rome (Italy), Bergen (Norway), Paris (France), and Krakow (Poland), who transferred from insulin to oral sulfonylureas before Nov 30, 2006, were eligible for inclusion. Clinicians collected clinical characteristics and annual data relating to glycaemic control, sulfonylurea dose, severe hypoglycaemia, side-effects, diabetes complications, and growth. The main outcomes of interest were sulfonylurea failure, defined as permanent reintroduction of daily insulin, and metabolic control, specifically HbA 1c and sulfonylurea dose. Neurological features associated with KCNJ11 permanent neonatal diabetes were also assessed. This study is registered with ClinicalTrials.gov, number NCT02624817. Findings: 90 patients were identified as being eligible for inclusion and 81 were enrolled in the study and provided long-term (>5·5 years cut-off) outcome data. Median follow-up duration for the whole cohort was 10·2 years (IQR 9·3–10·8). At most recent follow-up (between Dec 1, 2012, and Oct 4, 2016), 75 (93%) of 81 participants remained on sulfonylurea therapy alone. Excellent glycaemic control was maintained for patients for whom we had paired data on HbA 1c and sulfonylurea at all time points (ie, pre-transfer [for HbA 1c ], year 1, and most recent follow-up; n=64)—median HbA 1c was 8·1% (IQR 7·2–9·2; 65·0 mmol/mol [55·2–77·1]) before transfer to sulfonylureas, 5·9% (5·4–6·5; 41·0 mmol/mol [35·5–47·5]; p

Abstract

2-Ynamides can be synthesised through Pd(II) catalysed oxidative carbonylation, utilising low catalyst loadings. A variety of alkynes and amines can be used to afford 2-ynamides in high yields, whilst overcoming the drawbacks associated with previous oxidative methods, which rely on dangerous solvents and gas mixtures. The use of [NBu4 ]I allows the utilisation of the industrially recommended solvent ethyl acetate. O2 can be used as the terminal oxidant, and the catalyst can operate under safer conditions with low O2 concentrations.

Abstract

Universities, like cities, have embraced novel technologies and data-based solutions to improve their campuses with ‘smart’ becoming a welcomed concept. Campuses in many ways are small-scale cities. They increasingly seek to address similar challenges and to deliver improved experiences to their users. How can data be used in making this vision a reality? What can we learn from smart campuses that can be scale up to smart cities? A short research study was conducted over a three-month period at a public university in the United Kingdom employing stakeholder interviews and user surveys, aiming at gaining insight into these questions. Based on the study, the authors suggest that making data publicly available could bring many benefits to different groups of stakeholders and campus users. These benefits come with risks and challenges such as data privacy and protection and infrastructure hurdles. However, if these challenges can be overcome, open data could contribute significantly to improving campuses and user experiences, and potentially set an example for smart cities.

Abstract

Background: Chronic impairment following childhood traumatic brain injury has the potential to increase risk of negative outcomes. This highlights potential value of community-based rehabilitation programs. Objectives: To identify research studies examining existing intervention programmes available in community-based rehabilitation to adolescents following TBI to assist with the transition back into the community. Methods: A systematic review of community-based interventions was conducted across different national contexts. All included studies involved a clinical population with TBI, aged 11 to 25 years inclusive. Risk of bias was rated for each included study. Results: Seventeen studies were identified for inclusion in the review, of these eleven distinct interventions were found. The quality of evidence was largely weak and highly variable. Conclusion: The results suggest some improvement in adolescent outcomes following community-based interventions, however higher quality evidence is needed to support specific interventions.
