Abstract

Background: The COVID-19 pandemic highlighted gaps in health surveillance systems, disease prevention, and treatment globally. Among the many factors that might have led to these gaps is the issue of the financing of national health systems, especially in low-income and middle-income countries (LMICs), as well as a robust global system for pandemic preparedness. We aimed to provide a comparative assessment of global health spending at the onset of the pandemic; characterise the amount of development assistance for pandemic preparedness and response disbursed in the first 2 years of the COVID-19 pandemic; and examine expectations for future health spending and put into context the expected need for investment in pandemic preparedness. Methods: In this analysis of global health spending between 1990 and 2021, and prediction from 2021 to 2026, we estimated four sources of health spending: development assistance for health (DAH), government spending, out-of-pocket spending, and prepaid private spending across 204 countries and territories. We used the Organisation for Economic Co-operation and Development (OECD)'s Creditor Reporting System (CRS) and the WHO Global Health Expenditure Database (GHED) to estimate spending. We estimated development assistance for general health, COVID-19 response, and pandemic preparedness and response using a keyword search. Health spending estimates were combined with estimates of resources needed for pandemic prevention and preparedness to analyse future health spending patterns, relative to need. Findings: In 2019, at the onset of the COVID-19 pandemic, US$9·2 trillion (95% uncertainty interval [UI] 9·1–9·3) was spent on health worldwide. We found great disparities in the amount of resources devoted to health, with high-income countries spending $7·3 trillion (95% UI 7·2–7·4) in 2019; 293·7 times the $24·8 billion (95% UI 24·3–25·3) spent by low-income countries in 2019. Abstract

Methicillin‐resistant Staphylococcus aureus (MRSA) is an important zoonotic pathogen associated with a wide range of infections in humans and animals. Thus, the emergence of MRSA clones poses an important threat to human and animal health. This study is aimed at elucidating the genomics insights of a strong biofilm‐producing and multidrug‐resistant (MDR) S. aureus MTR_BAU_H1 strain through whole‐genome sequencing (WGS). The S. aureus MTR_BAU_H1 strain was isolated from food handlers' hand swabs in Bangladesh and phenotypically assessed for antimicrobial susceptibility and biofilm production assays. The isolate was further undergone to high throughput WGS and analysed using different bioinformatics tools to elucidate the genetic diversity, molecular epidemiology, sequence type (ST), antimicrobial resistance, and virulence gene distribution. Phenotypic analyses revealed that the S. aureus MTR_BAU_H1 strain is a strong biofilm‐former and carries both antimicrobial resistance (e.g., methicillin resistance; mecA, beta‐lactam resistance; blaZ and tetracycline resistance; tetC) and virulence (e.g., sea, tsst, and PVL) genes. The genome of the S. aureus MTR_BAU_H1 belonged to ST1930 that possessed three plasmid replicons (e.g., rep16, rep7c, and rep19), seven prophages, and two clustered regularly interspaced short palindromic repeat (CRISPR) arrays of varying sizes. Phylogenetic analysis showed a close evolutionary relationship between the MTR_BAU_H1 genome and other MRSA clones of diverse hosts and demographics. The MTR_BAU_H1 genome harbours 42 antimicrobial resistance genes (ARGs), 128 virulence genes, and 273 SEED subsystems coding for the metabolism of amino acids, carbohydrates, proteins, cofactors, vitamins, minerals, and lipids. This is the first‐ever WGS‐based study of a strong biofilm‐producing and MDR S. aureus strain isolated from human hand swabs in Bangladesh that unveils new information on the resistomes (ARGs and correlated mechanisms) and virulence potentials that might be linked to staphylococcal pathogenesis in both humans and animals. Abstract

Introduction: Empyema thoracis is a condition in which pus andfluidfrom infected tissue collects in the pleural cavity. The optimal treatment of empyema thoracis, especially in the acute condition remains controversial, whether to go for a non-operative approach (antibiotics and chest tube drainage) or an early operative regimen, VATS (Video-assisted thoracoscopic surgery). The study is conducted to determine the effectiveness of VATS in the treatment of acute empyema thoracis. Methods: We conducted a prospective comparative study between two methods of treatment i.e., chest tube insertion and VATS as primary intervention in the acute empyema thoracis. Each group consisted of 40 patients who were diagnosed as acute empyema thoracis during the period of January 2021 and June 2022. The duration of chest tube in- situ, hospital stay, cost, complications, success and outcomes were compared between the two groups. Results: It was found that VATS was better than the conventional chest tube insertion in terms of the mean duration of hospital stay (16.05 ± 3.58 vs 25.78 ± 4.04 days), mean duration of the chest tube in-situ (8.23 ± 2.09 vs 17.18 ± 3.30 days), mean cost of the treatment (Taka 12962.50 ± 1469.29 vs 19175.00 ± 1443.60), complications (42.5% vs 77.5%) and success rate (80% vs 45%). There was no death in any group. Conclusion: Video-assisted thoracoscopic surgery (VATS) is feasible and effective in the management of acute empyema thoracis. [ABSTRACT FROM AUTHOR]

Abstract

In southern Bangladesh excessive levels of As in shallow groundwater have led to deeper groundwater becoming the main alternative source of As-free potable water. Hydrogeological configuration indicates that tube-wells pumping from these depths may be vulnerable to As breakthrough from shallow levels. The thesis explores a range of methods of representing lithological heterogeneity of the Bengal Aquifer System (BAS) in models of groundwater flow and travel time. The aim is to support models of arsenic (As) flux to the deep groundwater flow-system of BAS, and hence to aid assessment of the vulnerability of deep groundwater to invasion by As. The research uses an array of geological information including geophysical logs (n=12), hydrocarbon exploration data (n=11), and drillers' logs (n=589) from a 5000 km2 area to characterise the aquifer heterogeneity as a basis for alternative representations of hydrogeological structure in groundwater flow modelling. Groundwater samples from southern Bangladesh were analysed for 14C in order to determine groundwater age (n=23) and for hydrochemical (n=75) and isotopic (n=50) characterisation. A new hypothesis `SiHA (Silt-clay layers influence Hierarchical groundwater flow systems and Arsenic progression in aquifer)' is presented which integrates sedimentological heterogeneities, groundwater flow, and geochemical processes to explain the distribution and geological evolution of groundwater As in the aquifer. The hypothesis explains the spatio-vertical variability of groundwater As concentration by 'groundwater flow systems and differential flushing' in the aquifer. Groundwater flow models based on eight different yet plausible aquifer representations provide adequate simulations of hydraulic head, but contrasting implications for well catchments and travel times. The better representations are judged by comparing model outcomes of travel time with groundwater age determination using 14C. Comparisons demonstrate the importance of incorporating hydrostratigraphy and spatial heterogeneity in order to optimise model representations, and implications for the security of As-free deep groundwater in the BAS.

Abstract

Ionospheric irregularities impair Global Navigation Satellite System (GNSS) signals and, in turn, affect the performance of GNSS positioning. Such effects are especially evident at low and high latitudes, which are currently gaining the attention of research and industry sectors. This study evaluates the impact of ionospheric irregularities on GNSS positioning in Greenland. We assess the performance of positioning methods that meet the demands of a wide range of users. In particular, we address the needs of the users of mass-market single-frequency receivers and those who require a solution of high precision provided by geodetic dual-frequency receivers. We take advantage of the datasets collected during three ionospheric storms: the St. Patrick’s Day storm of March 17, 2015, the storm on June 22, 2015, and another on August 25–26, 2018. We discover a significant impact of the ionospheric disturbances on the ambiguity resolution performance and the accuracy of the float solution in Real Time Kinematics (RTK) positioning. Next, assessing the single-frequency ionosphere-free Precise Point Positioning (PPP), we demonstrate that the model is generally unaffected by ionospheric disturbances. Hence, the model is predestined for the application by the users of single-frequency receivers in the areas of frequent ionospheric disturbances. Finally, based on the observation analyses, we reveal that phase signals on the L2 frequency band are more prone to cycle slips induced by ionospheric irregularities than those transmitted on the L1. Such signal properties explain a noticeable decline in the dual-frequency RTK performance during the ionospherically disturbed period and merely no effect for the single-frequency ionosphere-free PPP model.

Abstract

This research focuses on the formation and assessment of sol–gel auto-combustion-produced nanocrystalline Co0.40Ni0.6AlxFe2−xO4 (x = 0.00, 0.05, 0.1, 0.2) ferrites. The investigation explores the structural, morphological, electrical, dielectric, and magnetic characteristics of the spinel ferrites sintered at different temperatures (850 °C and 950 °C), with a specific emphasis on the impact of Al3+ substitution. A cubic single-phase spinel structure exhibiting excellent crystallinity and homogeneity is found across all samples, based on XRD evaluation. The nanoparticles had typical crystallite sizes between 40 and 48 nm and average particle sizes between 122 and 186 nm. Increasing Al3+ content resulted in increased nanoparticle porosity. According to infrared spectroscopy, the adsorption band v1 rose from 538 to 554 cm−1 whereas the v2 band dropped from 374 to 366 cm−1. With an increase in Al3+ content, the experimental magnetic moment (ηexp) and saturation magnetization (Ms) of the nanoparticles showed a notable upward trend. Furthermore, the samples sintered at 950 °C displayed higher AC resistivity, attributed to a reduction of the hopping electron within the grains. All of the ferrite nanoparticles under investigation also showed high coercivity values (1028.32–1222.76 Oe), designating them as ferrimagnetic materials and emphasizing the possibility for use in spintronics, high-frequency and microwave equipment like radar, antenna, and so on. Abstract

Background: Persistent pneumonia in children pose a significant challenge to the pediatricians as there are underlying reasons of persistence. There are limited data on the underlying diseases predisposing to persistence of pneumonia in children. Objective: To identify the underlying diseases of persistent pneumonia in children. Methods: The cross sectional observational study was conducted at Bangladesh Shishu (Children) Hospital & Institute from November, 2019 to October, 2020. Children 2 months to 5 years of age admitted with persistent pneumonia were analyzed to find out the underlying diseases. Results: Among 49 cases of persistent pneumonia, underlying disease could be determined in 41 (83.7%) cases. The diseases were cystic fibrosis 13 (26.5%), congenital heart disease 10 (20.4%), primary immunodeficiency 10 (20.4%), gastroesophageal reflux disease 5 (10.2%), pulmonary tuberculosis 4 (8.1%) and congenital anomalies of respiratory tract 1 (2%). Conclusion: The most common underlying diseases of persistent pneumonia are cystic fibrosis, congenital heart disease, primary immunodeficiency followed by gastroesophageal reflux disease, pulmonary tuberculosis and congenital anomalies of respiratory tract.
