Abstract

Purpose: Balancing mobile-bearing (MB) unicondylar knee arthroplasty (UKA) is challenging. If performed improperly, potential complications include pain, implant loosening, or progression of osteoarthritis in the preserved compartment. The purpose of this study was to document effects of improper balancing on knee kinematics and joint contact stress. It was hypothesized that over-stuffing would lead to more valgus and higher lateral contact force.Methods: Seven fresh-frozen cadaver legs were mounted in a kinematic rig that applied three motion patterns to the specimens: passive flexion-extension, open chain extension, and squatting. During testing, an infrared camera system recorded the trajectories of markers rigidly attached to femur and tibia, while a pressure sensor measured contact pressure in the lateral compartment. Prior computer tomography scans allowed identification of coordinate frames of the bones and calculations of anatomical rotations and translations. Collateral ligament strains were calculated, and quadriceps forces recorded. Following testing on the native knee, a medial MB UKA was implanted in each specimen and all motion trials were repeated. Three inlay thicknesses were tested to simulate optimal balancing as well as under- (1 mm thinner) and over-stuffing (1 mm thicker) of the medial compartment relative to the optimal thickness.Results: Under-stuffing of the medial compartment leads to kinematics closest to the native knee. Subjectively balanced and over-stuffed MB UKA knees were in more valgus. Lateral peak contact stress was higher from mid- to deep flexion following UKA in all three tested states; however, these results were not significant. Peak strain in the superficial medial collateral ligament (sMCL) was significantly higher in MB UKA, regardless of the inlay thickness mainly in mid-flexion. Inlay thickness had no significant impact on measured quadriceps force during squatting.Conclusion: The results underline the importance of optimal balancing. Over-stuffing should be avoided as it results in the largest kinematic changes relative to the native condition and induces higher strains in the sMCL. Abstract

Self-Indication Neutron Resonance Densitometry (SINRD) is a passive non-destructive method that is being investigated to quantify the 239Pu content in a spent fuel assembly. The technique relies on the energy dependence of total cross sections for neutron induced reaction. The cross sections show resonance structures that can be used to quantify the presence of materials in objects, e.g. the total cross-section of 239Pu shows a strong resonance close to 0.3 eV. This resonance will cause a reduction of the number of neutrons emitted from spent fuel when 239Pu is present. Hence such a reduction can be used to quantify the amount of 239Pu present in the fuel. A neutron detector with a high sensitivity to neutrons in this energy region is used to enhance the sensitivity to 239Pu. This principle is similar to self-indication cross section measurements. An appropriate detector can be realized by surrounding a 239Pu-loaded fission chamber with appropriate neutron absorbing material. In this contribution experiments performed at the GELINA time-of-flight facility of the JRC at Geel (Belgium) to validate the simulations are discussed. Abstract

Self-interrogation neutron resonance densitometry (SINRD) is a non-destructive analysis technique that can be used to quantify the amount of 239Pu in spent nuclear fuel. It is a passive method that relies on the detection of neutrons, which are emitted by the fuel. The amount of 239Pu is estimated from the ratio of the neutron intensity in the fast energy region and in a region close to the 0.296 eV resonance of 239Pu. The neutron intensity in the resonance region is obtained from a detection system with a high sensitivity to 0.296 eV neutrons. This can be realized by using two neutron detectors with 239Pu as convertor material. One of the detectors is covered by a thin Gd foil and the other by a thin Cd foil. The Gd and Cd foils are referred to as SINRD filters. An approach based on the measurement of a fuel assembly in air and surrounded by a slab of polyethylene was developed at SCK·CEN. This approach foresees the insertion of small neutron detectors in the guide tubes of the assembly, and optimisation studies of SINRD were based on Monte Carlo simulations. Experiments to support the results of such simulations were carried out at the time-of-flight facility GELINA of the Joint Research Centre (JRC) in Geel (Belgium). Transmission measurements were performed to verify the quality of the nuclear data that are used to define the optimum thickness of the SINRD filters. Results of self-indication measurements were used to confirm the basic principle of SINRD, that is, that the best results are obtained with a detector that has a high sensitivity to neutrons with energy close to the energy of a strong resonance of the material under investigation. Abstract

The anterolateral ligament of the knee (ALL) has caused a lot of rumors in orthopaedics these days. The structure that was first described by Segond back in 1879 has experienced a long history of anatomic descriptions and speculations until its rediscovery by Claes in 2013. Its biomechanical properties and function have been examined recently, but are not yet fully understood. While the structure seems to act as a limiter of internal rotation and lateral meniscal extrusion its possible proprioceptive effect remains questionable. Its contribution to the pivot shift phenomenon has been uncovered in parts, therefore it has been recognized that a concomitant anterolateral stabilization together with ACL reconstruction may aid in prevention of postoperative instability after severe ligamentous knee damages. However, there are a lot of different methods to perform this procedure and the clinical outcome has yet to be examined. This concise review will give an overview on the present literature to outline the long history of the ALL under its different names, its anatomic variances and topography as well as on histologic examinations, imaging modalities, arthroscopic aspects and methods for a possible anterolateral stabilization of the knee joint.
