Abstract: Background: Mitral annulus calcification (MAC) may be a form of atherosclerosis. The goal of the present work was to investigate whether helical CT can determine the presence of MAC and to clarify its possible association with coronary artery disease (CAD) in elderly patients. Design and Methods: Three hundred and twenty-nine consecutive elderly patients (165 men and 164 women, age range 60–79 years) underwent double helical CT of the heart to determine MAC and coronary calcifications (CC) according to a previously described protocol. Results: MAC was documented in 60 patients (25 men, 35 women; mean age 69±4.5 years, range 60–78 years). The non-MAC group (control) included 269 patients (140 men, 129 women; mean age 67±4.6 years, range 60–79 years). Age was the only risk factor which had significant association with MAC (p=0.01). A significant difference was found between MAC and control group for mean total CC score and advanced CC (total CC>300) (323±565 vs. 184±429, p=0.033 and 30% vs. 16%, p=0.017, respectively). A significant difference was also found between groups for the prevalent proven CAD (30% vs. 16%, p=0.008). Stepwise logistic regression analysis identified age [odds ratio (OR) 3.3, 95% confidence interval (CI) 1.7–6.4, p<0.001)], gender (male) (OR 3.1, 95% CI 1.6–6.0, p=0.001), and MAC (OR 2.4, 95% CI 1.2–5.0, p=0.016) as the independent variables significantly associated with CAD. The independent variables significantly associated with advanced CC (TCS>300) were MAC (OR 2.6, 95% CI 1.3–5.2, p=0.005), gender (male) (OR 2.3, 95% CI 1.2–4.2, p=0.012) and age (OR 1.9, 95% CI 1.0–3.7, p=0.052). Conclusions: Our study demonstrated the usefulness of helical computed tomography in the detection of mitral annular calcification as an additional marker of prevalent CAD. However, the diagnostic significance of the MAC detection is relatively minor and should not be considered as a direct proof for coronary atherosclerosis. [Copyright &y& Elsevier]

Background. We examined contrast-enhanced harmonic gray-scale sonographic findings of pancreatic carcinoma in relation to the pathological findings in resected specimens to evaluate correlations between observations made by this modality and the pathological findings. Methods. The pathological findings of surgical specimens obtained from 16 patients were examined in relation to the contrast-enhanced harmonic gray-scale sonography findings. Lesion vascularity was examined by contrast-enhanced harmonic gray-scale sonography from 20 to 50 s after the injection of Levovist (Schering, Berlin, Germany) (early phase), and lesion enhancement was also monitored at approximately 90s after injection (delayed phase). Results. Contrast-enhanced harmonic gray-scale sonography showed positive enhancement in 12 of the 16 lesions (peripheral tumor region alone, n = 9; entire tumor, n = 3), while the other 4 lesions showed no contrast enhancement in any region. Twelve enhanced regions (9 peripheral tumor region and 3 entire tumor regions) detected by contrast-enhanced harmonic gray-scale sonography showed: (1) mild fibrosis with inflammation, in 10 regions (83%); (2) the presence of both carcinoma cells and residual acinar cells in 8 (67%); and (3) presence of relatively large arteries in 2 (17%). In contrast, 13 non-enhanced regions (4 entire tumor regions and 9 central regions) showed: (1) severe fibrosis in 10 regions (77%); (2) necrosis in 7 (54%); and (3) mucin in 4 (31%). Conclusions. Objective: Localized low attenuated areas (pseudolesions) in the medial segment of left liver lobe are not rarely seen in the screening of abdomen using helical CT. The purpose of this study was to determine the prevalence of pseudolesions in the routine helical CT of abdomen and to evaluate the morphologic and enhancement features of pseudolesions in the unenhanced and enhanced CT examinations. Materials and Methods: We retrospectively evaluated 333 contrast enhanced abdominal CT examination of 328 patients with no known liver disease, to detect the presence of pseudolesion of liver. In the presence of unenhanced and arterial phase examinations, these images were also analyzed. The imaging criteria for pseudolesion of liver was localized low attenuated area with geometric, ovoid or nodular shaped and with no mass effect adjacent to the falciform ligament, gallbladder, or porta hepatis. Previous CT, CTAP and MR examinations were also reviewed to understand the evolution of pseudolesion in patients in whom a pseudolesion was detected in the portal phase of helical CT examination. Results: We identified a pseudolesion in the 65 (19.8%) of 328 patients in portal phase of helical CT examinations. Pseudolesions were identified in the medial segment of left liver lobe adjacent to falciform ligament in the 92.8% of patients, both sides of falciform ligament in the 1.5% of patients, adjacent to porta hepatis in the 3% of patients and adjacent to gallbladder 3% of patients. These lesions had triangular shape in the 66.1% of patients, ovoid shape in the 18.6% of patients, and nodular shape in the 15.3% of patients. Unenhanced, arterial and portal phase images were exist in the 50.7% of 65 patients. The pseudolesions were not identified on the unenhanced images in the 75.7% of patients and on the arterial phase images in the 55.6% of patients. Conclusion: Pseudolesions around the falciform ligament are not rarely seen in the routine helical CT examination of liver and abdomen. The pseudolesions are more encountered in the portal phase of helical CT examination. These lesions seem to be likely focal fatty infiltration or perfusion defect due to venous supply variation or both. Nodular shaped pseudolesions may be interpreted as true tumors and further study may require for differential diagnosis. [ABSTRACT FROM AUTHOR]

Summary: The aim of this study is to investigate the efficacy of dynamic computed tomography (CT) during selective angiography (CT-arteriography) of orbital tumors in the evaluation of intratumoral vascular anatomy, feeding artery territory, and histological diagnosis. Among 35 consecutive cases with various orbital lesions, those cases showing tumor staining or pooling of the contrast medium on digital subtraction angiography (DSA) were evaluated by CT-arteriography (n=14). The information obtained by CT-arteriography was compared with that provided by enhanced MRI (n=31) and dynamic MRI (n=21), in which the contrast medium was injected intravenously. In addition to the visualization of fine vascular anatomy, CT-arteriography emphasized areas of nodular enhancement and non-enhancing cystic/necrotic components as well as the intratumoral feeding arteries. Patterns of CT-arteriography were categorized into three subgroups: homogeneous enhancement (benign lymphoid lesion), partial enhancement (schwannomas and carcinomas), and patchy multinodular enhancement (specific for cavernous angiomas). In addition, CT-arteriography with selective arterial catheterization clearly delineated the feeding artery territories. CT-arteriography, with a minimal dose of contrast medium, can offer significant advantages over intravenously injected dynamic neuroimaging, and provides additional valuable preoperative information about the orbital tumor under investigation. Aim: To define the value of different radiologic modalities in determining the patients who believed to be stone-free after extracorporeal shock wave lithotripsy (SWL) with plain abdominal X-ray, by evaluating the same patients with ultrasonography (USG) and helical computed tomography (CT). Patients and methods: Between March 2002 and February 2003, 76 patients with urolithiazis who were treated with SWL and considered to be stone-free with plain abdominal X-ray, were evaluated with USG and helical CT. The results were compared for the accuracy of the stone-free diagnosis. Results: Residual stones were detected in 9 (11.8%) with USG and in 17 (22.3%) with CT of 76 patients who were thought to be stone-free with plain abdominal X-ray alone. Conclusions: Although plain abdominal X-ray has been accepted as the first line diagnostic tool in the follow-up after SWL with its cheap and practical use, helical CT was found to be more valuable in diagnosis of residual stone fragments which has not been found in plain abdominal X-ray. If we take these considerations which can change our clinical approach and patient follow-up into account, we believe that the routine use of helical CT can give more accurate information in patient controls after SWL. The purpose of this study was to evaluate the feasibility of high-field magnetic resonance imaging (MRI) of the lung using a T2-weighted fast-spin echo (TSE) sequence. Comparison was made with helical computed tomography CT findings in patients with diffuse pulmonary diseases. Prospective segment-wise analysis of high-field MR imaging findings in 15 patients with diffuse pulmonary diseases was made using helical CT and HRCT as the standard of reference. The MR studies were performed on a 3.0-T whole body system (Intera 3T, Philips Medical Systems) using a T2w TSE sequence with respiratory and cardiac gating (TE 80 ms TR 1,500-2,500 ms; turbo factor 17; 22 slices with 7/2-mm slice thickness and gap; 256x192 matrix). MR artifacts were graded on a three-point scale (low, moderate, high). Lung MR studies were prospectively analyzed segment-by-segment and diagnosed as healthy or pathological; results were compared with helical CT findings. In all 15 patients, MR imaging of the lung was successful. All 15 MR studies were compromised by artifacts; however, the severity of these artifacts was classified as low or moderate in 8/15, respectively, 7/15 cases. A total of 143/285 lung segments showed diffuse lung disease in helical CT. With MRI, 133 of these 143 segments (93%) were judged to be diseased. The ten segments that received false negative MR diagnoses displayed non-acute pulmonary lesions with inherently low proton density (scars, granulomas). MRI at 3.0 T can detect diffuse pulmonary disease with a high sensitivity. Based on this experience, further pulmonary studies with high-field systems appear justified and promising. This paper describes a flexible new methodology for accurate cone beam reconstruction with source positions on a curve (or set of curves). The inversion formulas employed by this method- ology are based on first backprojecting a simple derivative in the projection space and then applying a Hubert transform inversion in the image space. The local nature of the projection space filtering distinguishes this approach from conventional filtered-backprojection methods. This characteristic together with a degree of flexibility in choosing the direction of the Hubert transform used for inversion offers two important features for the design of data acquisition geometries and reconstruction algorithms. First, the size of the detector necessary to acquire sufficient data for accurate reconstruction of a given region is often smaller than that required by previously documented approaches. In other words, more data truncation is allowed. Second, redundant data can be incorporated for the purpose of noise reduction. The validity of the inversion formulas along with the application of these two properties are illustrated with reconstructions from computer simulated data. In particular, in the helical cone beam geometry, it is shown that 1) intermittent transaxial truncation has no effect on the reconstruction in a central region which means that wider patients can be accommodated on existing scanners, and more importantly that radiation exposure can be reduced for region of interest imaging and 2) at maximum pitch the data outside the Tam-Danielsson window can be used to reduce image noise and thereby improve dose utilization. Furthermore, the degree of axial truncation tolerated by our approach for saddle trajectories is shown to be larger than that of previous methods. [ABSTRACT FROM AUTHOR]

Our aim is to review the utility of the different imaging techniques in the field of renal transplantation (RT). A total of 500 potential RT recipients have been evaluated and were included in the waiting list. From 1978 to 2003, a total of 900 RT recipients were clinically followed-up and different imaging techniques assessed. The main indications for the different techniques were: (1) MR angiography to establish the vascular anatomy of the living donor with 95% accuracy for the detection of multiple renal vessels. (2) Abdominal ultrasound of the donor in order to detect lithiasis, incidental renal tumors or anatomic anomalies. (3) Vascular evaluation of the recipients was essential considering the aging character of this population. X-ray film of the abdomen was a routine technique. Doppler ultrasound (DOP-US) of the iliac vessels and an angiographic study (DSA, CT, MR) of the iliac and splenic arteries were indicated only in selected cases. (4) DOP-US was useful in the diagnosis of graft dysfunction after RT. It allowed the exclusion of thrombosis and urinary obstruction, and was useful to distinguish between acute tubular necrosis, cyclosporine nephrotoxicity and acute rejection (AR). (5) Finally, ultrasound was used as a percutaneous approach to pyelocaliceal systems of grafts affected by ureteral stenosis or fistula and for draining fluid collections after RT (lymphocele, abscess, hematoma or urinoma). Objectives: To compare images of the aorta obtained with helical CT (HCT) scanning and MRI for the follow-up of adults with coarctation of the aorta (CoA). Design: Longitudinal study. Setting: Department of adult congenital heart disease in a tertiary university hospital. Patients: A total of 37 adults (age range, 16 to 68 years; women, 13) with CoA (after surgery, 34 patients; native, 2 patients; after balloon-angioplasty, 1 patient) Measurements and results: All patients underwent both HCT and MRI of the thoracic aorta within a mean (± SD) time interval of 1.86 ± 1.11 years. Aortic diameters measured at six intrathoracic levels showed a high correlation (r = 0.79 to 0.94). On average, slightly lower diameters were measured with MRI (1.2 mm). But there was a substantial variation between the two measurements with differences of up to 9 mm. All other pathomorphologic abnormalities were detected and classified similarly with both methods. Conclusions: HCT and MRI are similarly useful for the noninvasive evaluation of the thoracic aorta in patients with CoA. But there can be a substantial variation in two subsequent measurements without an overall substantial bias toward larger diameter in one of the two methods. In repetitive studies, changes of the diameters should be interpreted with care, especially when assessing the progression of aortic diameters. [ABSTRACT FROM AUTHOR]

To evaluate the role of multiphasic helical computed tomography (CT) in planning surgical treatment for pelvi-ureteric junction obstruction (PUJO). Multiphasic helical CT was used in 60 consecutive patients (mean age 32 years, sd 13.72) with primary PUJO. The different images were interpreted before surgery by a urologist and radiologist. The number, type and relation of the crossing vessels to the PUJ were detected. Pyeloplasty (open or laparoscopic) was used in patients where there was a significant crossing vessel (i.e. > 2 mm in diameter and passing at the PUJ) or with ballooning of the renal pelvis. Endopyelotomy (antegrade or retrograde) was used in the other patients. Subjective and objective success rates were determined 3–6 months after treatment and then every 6 months. Hyperdense renal stones were detected in precontrast images in 15 patients, significant crossing vessels in 33 (55%), and anterior malrotation of the renal pelvis in 23 (38%; 19 associated with significant crossing vessels, i.e. ureterovascular hydronephrosis). A large para-pelvic cyst compressing the PUJ and upper ureter was detected in one patient. Pyeloplasty was used in 45 patients, among whom the sensitivity of multiphasic helical CT in detecting crossing vessels was 97%, the specificity 92% and accuracy 96%. Uncomplicated endopyelotomy was used in 14 patients and percutaneous aspiration of the para-pelvic cyst in one. Multiphasic helical CT can accurately delineate the spatial anatomy of the renal and peri-renal area; it may be important in planning surgical treatment for PUJO, especially when endopyelotomy is considered. This paper discusses image reconstruction with a tilted gantry in multislice computed tomography (CT) with helical (spiral) data acquisition. The reconstruction problem with gantry tilt is shown to be transformable into the problem of reconstructing a virtual object from multislice CT data with no gantry tilt, for which various algorithms exist in the literature. The virtual object is related to the real object by a simple affine transformation that transforms the tilted helical trajectory of the X-ray source into a nontilted helix, and the real object can be computed from the virtual object using one-dimensional interpolation. However, the interpolation may be skipped since the reconstruction of the virtual object on a Cartesian grid provides directly nondistorted images of the real object on slices parallel to the tilted plane of the gantry. The theory is first presented without any specification of the detector geometry, then applied to the curved detector geometry of third-generation CT scanners with the use of Katsevich's formula for example. Objective: To determine the accuracy of triphase enhanced helical CT in judging the invasion and metastasis of gastric carcinoma, and to discuss the relation between imaging signs and pathological findings. Methods: Triphase enhanced helical CT scanning was performed in 46 patients with gastric car- cinoma. Imaging findings were compared with postoperative pathologic results. Results: (1) The accuracy of helical CT for diagnosing involvement of tunica serosa, lymph node metastasis and distant metastasis was 84.8%, 87.0% and 100% respectively. (2) CT signs of serosal involvement, lymph node metastasis and distant metastasis were in good accordance with pathological findings (P<0.05). Conclusion: Triphase enhanced helical CT scans can comprehensively and precisely reflect the pathologic characteristics of gastric carcinoma, thus it is a reliable technique for the diagnosis of this disease. The purpose of this study was to evaluate helical CT angiography in the assessment of occlusive arterial disease of abdominal aorta and the lower extremities. Sixteen patients underwent both transcatheter angiography and helical CT. Helical CT was inconclusive in 6.2% of segments whereas angiography was inconclusive in 5%. The overall sensitivity of helical CT was 91% and specificity 93%. Segmental analysis found a sensitivity of 43% in infrapopliteal arteries, and a specificity of 86%. [Copyright &y& Elsevier]

The purpose of this study was to investigate the condylar displacement from intercuspal position (IP) to reference position (RP), using a jaw movement measuring system with 6 d.f. and helical computed tomography (CT). The 19 patients included in the present study were divided into a group with bilateral condylar bone change (10 subjects, including the sub-groups with flattening and osteophyte formation) and a group with no condylar bone change (9 subjects). The results showed that the bone change group had significantly more superior, posterior and absolute horizontal IP to RP slides than the no bone change group. There was also a significant difference in the amount of antero-posterior and supero-inferior condylar IP–RP displacements related to the type of condylar bone change within the bone change group. The osteophyte sub-group showed the largest posterior displacement, and the flattening sub-group showed the largest superior displacement. These results suggested that this large IP–RP difference in the bone change group might be related to their temporomandibular joint (TMJ) pathology, and it might therefore be useful to keep this difference in mind as one of the clinical indices suggesting the presence of TMJ osteoarthritis (OA). With modern imaging technologies, including helical CT and MR imaging, the evaluation of liver morphology, blood flow and detection and characterization of liver neoplasms is possible in a noninvasive manner. Experiences in numerous institutions have established that both imaging methods are highly accurate in diagnosing hepatic pathology. Three-dimensional evaluation is feasible with fast imaging techniques such as multislice CT. With tissue-specific MR contrast agents such as SPIO, our capability is enhanced not only with respect to lesion detection but also lesion characterization. [ABSTRACT FROM AUTHOR]

Acute pancreatitis, leaking abdominal aortic aneurysm, and renal trauma frequently occur in the setting of patients with abdominal nontraumatic and traumatic injury; it represents the most urgent conditions that may determine the presence of fluid collections or haematoma in the retroperitoneum. Single spiral CT and multidetector-row CT (MDCT) play an important role in diagnosis of retroperitoneal emergencies, providing useful informations on the type, site, extent and management of the fluid collections. An accurate CT assessment requires the awareness of the existence of dissectable retroperitoneal fascial planes. Fluid collections or haematoma tends to escape the retroperitoneal site of origin into planes extend from the diaphragm to the pelvic floor. We assess the multicompartimental anatomy of the retroperitoneum and the pathway of spread of the most frequent retroperitoneal fluid collections or haematoma by helical CT. [Copyright &y& Elsevier]

The object of this study is to assess whether aneurysm surgery can be performed in patients with ruptured and unruptured cerebral aneurysms by using three-dimensional computerized tomography angiography (3D-CTA) alone, without conventional catheter angiography (CCA).We have operated on 153 consecutive patients with ruptured aneurysms and 89 patients with unruptured aneurysms based on 3D-CTA findings since December 1996 and February 1997, respectively. In the cases of ruptured aneurysm, 153 ruptured aneurysms including 62 associated unruptured aneurysms were detected by 3D-CTA. CCA was performed in 7 of 153 patients after they underwent 3D-CTA, which included four dissecting vertebral artery (VA) aneurysms, two basilar artery (BA) tip aneurysms, and one BA–superior cerebellar artery (SCA) aneurysm.In 89 patients with unruptured aneurysms, 101 unruptured aneurysms were detected by 3D-CTA. In 5 of 101 unruptured aneurysms, which included giant or large aneurysms and a case of an aneurysm associated with infarction, CCA was needed. All of the ruptured and unruptured aneurysms were confirmed during surgery and treated successfully.The authors stress that 3D-CTA could replace CCA in the diagnosis of saccular aneurysms and that surgery could be performed in almost all ruptured and unruptured aneurysms by using only 3D-CTA without CCA. Abstract The diagnosis of pulmonary embolism (PE) is difficult with many patients treated without the disease or left untreated without an adequate diagnostic work up. Recent advances in PE diagnosis are reviewed. The use of risk stratification in PE diagnosis is strongly recommended and evidence on how it can best be performed summarized. The Ginsberg/Wells stratification rule is recommended currently as the best validated rule. Computed tomographic pulmonary angiography (CTPA) was found to have quite poor sensitivity and to be poorly validated. It is recommended as adequate as a positive test in moderate/high risk groups and an exclusionary test in low risk groups or where an adequate alternative diagnosis is found. For D-Dimer tests the only test with adequate sensitivity and validation in management studies is the VIDAS© D-Dimer. This is in low/intermediate risk groups in the ED population. The Simpli-Red© test is also reviewed but is too insensitive for most populations. Echocardiography: this is good in compromised patients as it is a bedside test which when negative virtually excludes PE. If positive in the right setting it has a high positive predictive value. A negative echocardiogram predicts a benign clinical course for PE. The rest of the paper details the authors approach to integrating these new techniques with established algorithms and where progress is likely to occur in the next few years. These include improvements in CTPA (plus the addition of CT venography), new point of care D-Dimer tests, better risk stratification rules and integration of new strategies with artificial neural networks or computerized guidelines. Our objective was to assess the effects of the injection rate of contrast material and of a 5% dextrose flush on enhancement in multiphase hepatic CT using a multislice CT scanner. Most patients had chronic hepatitis and/or liver cirrhosis. One hundred eighty examinations, in which two sequential acquisitions were performed during a single breath-hold followed by third- and fourth-pass acquisitions, were randomized into four protocols: contrast injection at 0.1 ml/kg body weight s(-1) over 21 s without and with a 30-ml flush in groups 1 and 2, respectively, and contrast injection at 0.07 ml/kg body weight s(-1) over 30 s without and with a flush in groups 3 and 4, respectively. Contrast enhancement in each acquisition was measured in the aorta, portal vein, and liver. The visualization of hepatic arterial branches was scored by visual assessment. The highest aortic enhancement was observed in the first-pass acquisition in all groups. At the higher injection rate (groups 1 and 2), aortic enhancement in the first-pass acquisition was significantly more intense, whereas portal venous and hepatic enhancement was significantly less intense. The use of a flush considerably improved aortic enhancement at the beginning of the second-pass acquisition. In the visual assessment of hepatic arterial branches, the protocols with the higher injection rate received significantly higher grades. Multislice CT permits the entire liver to be imaged during an almost exclusively arterial phase by shortening the injection duration for a given volume of contrast material. [ABSTRACT FROM AUTHOR]

CT pulmonary angiography has become the screening exam of choice to evaluate for pulmonary embolism (PE), especially in subjects with an abnormal chest radiograph. A good-quality CT pulmonary angiogram has a high accuracy rate for the evaluation of PE. Investigators have reported that subsegmental emboli can be missed; however, visualization of smaller arterial branches and, therefore, detection of small emboli have improved with the availability of multidetector scanners. Some of the advantages of using CT for PE compared to lung scintigraphy include: (1) direct visualization of emboli on CT; (2), evaluation of lung parenchyma and mediastinum (this may provide an alternate diagnosis); (3) capability to acquire a CT venogram without additional contrast with 'one-stop examination' for the evaluation of thromboembolic disease. Our objective was to define the possibility of using oral-contrast-enhanced helical CT for the assessment of patients with cholecystolithiasis and of cholecystectomized symptomatic patients. Twenty-seven patients with cholecystolithiasis and 20 with a painful abdominal symptomatology after cholecystectomy (12 laparoscopic and 8 laparotomic) were recruited for this study. Cholangio-CT was performed 12–14 h after oral administration of 6 g of hyopanoic acid. The acquired data were then transferred to a second workstation and 3D reconstruction of the biliary tract was obtained. In all the cases the extrahepatic bile ducts were recognizable. Seven patients had no gallbladder opacification: CT images showed in 3 cases an infundibular stone and in 4 cases sclero-atrophic gallbladder. The intrahepatic bile ducts were visible in 21 of 27 patients with lithiasis and in 14 of 20 cholecystectomized patients. In 5 cholecystectomized patients cholangio-CT demonstrated the presence of residual choledochal or intrahepatic stones. In 3 cholecystectomized patients 3D reconstruction allowed identification of a long and winding stump of the cystic duct. Cholangio-CT is a non-invasive method to evaluate the biliary tract in patients who cannot be subjected to cholangio-MR, or as a preliminary to endoscopic retrograde cholangiopancreatography in cases of doubtful diagnosis after US and cholangio-MR. Our objective was to evaluate the ability of multiplanar reformatted (MPR) images combined with 0.5-mm axial images to depict the pancreatic and intrapancreatic bile ducts and compare the results with those of 0.5-mm axial, 2-mm axial, and 6-mm axial images alone. Seventy-seven patients without obstruction of the main pancreatic ducts (MPD) underwent dual-phase helical scanning of the pancreas using multislice computed tomography (MSCT). The MPR images were generated from 0.5-mm-thick images. Visualization of the pancreatic and intrapancreatic bile ducts and their confluence was graded on a four-point scale by a consensus of two radiologists. The results for 0.5-mm axial images in early-phase CT, 2-mm axial images in early-phase CT, MPR images combined with 0.5-mm axial images in early-phase CT, and 6-mm axial images in late-phase CT were then compared. The relationships of the focal pancreatic lesions with the pancreatic ducts were analyzed. The MPR images combined with 0.5-mm axial images were significantly superior to the other three types of images for the visualization of the pancreatic and intrapancreatic bile ducts and their confluence (p<0.01). The depiction rate of the MPD using MPR images combined with 0.5-mm axial images was 94, 94, 95, and 75%, respectively in the head, neck, body, and tail of the pancreas. Accessory pancreatic ducts, intrapancreatic bile ducts, and duct confluence were depicted in 48, 99, and 92%, respectively. In comparison with evaluation based on axial images alone, the use of MPR images more clearly demonstrated the relationship between the lesions and the pancreatic ducts in 14 of 19 lesions. The MPR images combined with 0.5-mm axial images improve the CT depiction of the pancreatic and intrapancreatic bile ducts in comparison with 0.5-mm axial, 2-mm axial, and 6-mm axial images alone. We performed a study of three-dimensional (3-D) linear measurements in the maxillofacial region using helical computer tomography (CT). The high accuracy of the linear measurements showed errors of less than 5% from the actual measures. But, it is possible that the accuracy was influenced by inaccurate head positions. In this study, we evaluated the errors when the head positions were tilted using the 3-D measurement system. Helical CT was used to scan a dry skull, and the data were reconstructed into a 3-D image. A total of 18 points were plotted on the 3-D images, and the distance between two points was calculated when the points were expressed as coordinates. A dry skull was tilted by 10° from the reference position in the horizontal, sagittal, and frontal planes and was then tilted in a combination of directions. Scanning was performed with slice thicknesses of 1 mm, 3 mm, 5 mm, and 7 mm. The length between two points measured by 3-D cephalometry was compared with the actual length determined using an antenna meter and a caliper and expressed as percentage errors of the actual length. In all head positions, errors in all linear measurements on the images and the actual length measured on the skull were less than 5% when a slice thickness of 1 mm or 3 mm was used. But, on using a slice thickness of 5 mm or 7 mm, some linear measurements showed larger measurement errors. Therefore, a thickness of less than 3 mm was thought to be clinically appropriate because the accuracy of the measurements was not influenced by head rotation. [ABSTRACT FROM AUTHOR]

The A-bomb survivors represent the best source of data for risk estimates of radiation-induced cancer. It is clear that children are ten times more sensitive than adults to the induction of cancer. The introduction of helical CT has transformed diagnostic radiology, especially in pediatric patients. The undoubted benefits carry the price tag of much higher doses, and in children, even higher effective doses. The A-bomb data have "matured" and we now have cancer risk estimates for a dose range which coincides with the organ doses from pediatric CT. Individuals exposed 50 years ago to doses comparable to those associated with helical CT today, show a small but statistically significant excess incidence of cancer. There are no assumptions, and no extrapolations involved. An abdominal helical CT scan in a young girl results in a risk of fatal cancer later in life that amounts to about one in a thousand. The risk to the individual is small, and readily balanced by the medical benefits. The public health problem is, however, significant when the small individual risk is multiplied by the 2.7 million of such procedures performed annually. Every effort is needed to minimize doses by an appropriate choice of peak kilovoltage (kVp) and milliampere-seconds (mAs), and at the same time to urge a more selective use of pediatric CT. Our objective was to assess the ability of dual-phase helical CT (DHCT) to predict resectability of carcinoma of gallbladder (CaGB). Thirty-two consecutive patients suspected of having CaGB on clinical examination and sonography presented to our centre over 10-month period. All these 32 patients underwent DHCT. Fifteen patients were considered inoperable and 2 had xanthogranulomatous cholecystitis. The remaining 15 patients (10 women, 5 men; age range 33–72 years) underwent surgery and had histopathological confirmation of CaGB and were included in the study based on the following criteria: presence of mass in gallbladder fossa on sonography and DHCT, and confirmation at surgery and histopathological examination. Axial reconstructions of 2 mm were obtained (collimation 3 mm, table speed 4.5 mm/s) for arterial (scan delay 20 s) and venous (scan delay 60 s) phases on a helical scanner. The criteria used for unresectability were: distant metastasis (liver, peritoneum, lymph nodes), extensive local contiguous organ spread, involvement of secondary biliary confluence of both lobes of liver, tumoral invasion of main portal vein, or proper hepatic artery or simultaneous invasion of one side hepatic artery and the other side portal vein. The CT findings related to unresectability were correlated with surgical findings. On the basis of CT findings, 10 patients were unresectable and 5 were resectable. Of the 10 patients considered unresectable, 9 had tumours that were unresectable at surgery (sensitivity 100%, positive predictive value 90%). Five patients had more than one reason and 4 had one reason alone for being unresectable (lymph nodes, n=2; hepatic metastasis, n=1; and vascular invasion, n=1). All 5 patients considered resectable based on CT findings had resectable tumours at surgery (negative predictive value 100%). The overall accuracy of CT was 93.3%. Combining helical volumetric CT acquisition and thin-slice thickness during breath hold provides an accurate assessment of both focal and diffuse airway diseases. With multiple detector rows, compared with single-slice helical CT, multislice CT can cover a greater volume, during a simple breath hold, and with better longitudinal and in-plane spatial resolution and improved temporal resolution. The result in data set allows the generation of superior multiplanar and 3D images of the airways, including those obtained from techniques developed specifically for airway imaging, such as virtual bronchography and virtual bronchoscopy. Complementary CT evaluation at suspended or continuous full expiration is mandatory to detect air trapping that is a key finding for depicting an obstruction on the small airways. Indications for CT evaluation of the airways include: (a) detection of endobronchial lesions in patients with an unexplained hemoptysis; (b) evaluation of extent of tracheobronchial stenosis for planning treatment and follow-up; (c) detection of congenital airway anomalies revealed by hemoptysis or recurrent infection; (d) detection of postinfectious or postoperative airway fistula or dehiscence; and (e) diagnosis and assessment of extent of bronchiectasis and small airway disease. Improvement in image analysis technique and the use of spirometrically control of lung volume acquisition have made possible accurate and reproducible quantitative assessment of airway wall and lumen areas and lung density. Acute flank pain is a common and complex clinical problem which might be caused by a variety of urinary and extraurinary abnormalities among which ureterolithiasis being the most frequent cause. Plain abdominal radiographs combined with intravenous urography (IVU) have been the standard imaging procedures of choice for the evaluation of acute flank pain over the last decades. Direct detection of even small ureteral calculi is achieved in 40–60%, whereas using indirect signs such as ureteral and renal pelvic dilatation stone detection is possible in up to 80–90% of all cases. However, IVU might be hampered by poor quality due to lack of bowel preparation, by nephrotoxicity of contrast agents, by serious allergic and anaphylactic reactions in 10% and 1% of the patients, respectively, and by significant radiation exposure. The use of ultrasonography (US) in the management of acute flank pain has been growing and when combining the findings of pyeloureteral dilatation, direct visualization of stones, and the absence of ureteral ejaculation, the sensitivity to detect ureteral dilatation can be as high as 96%. Recently, unenhanced helical CT (UHCT) has been introduced as imaging modality with a high sensitivity and specificity for the evaluation of acute flank pain. UHCT has been demonstrated to be superior since (1) it detects ureteral stones with a sensitivity and specificity from 98% to 100% regardless of size, location and chemical composition, (2) it identifies extraurinary causes of flank pain in about one third of all patients presenting with acute flank pain, (3) it does not need contrast agent, and (4) it is a time saving imaging technique being performed within 5 min. Based on the data published, one can predict that UHCT will become the imaging procedure of choice for evaluation of acute flank pain within the next years.The purpose of this review is to critically evaluate the role all imaging modalities available for a modern approach of diagnosis and management of acute flank pain with regard to their sensitivity, specificity, positive and negative predictive values and their complications, toxicicty and morbidity. Hepatocarcinogenesis in the cirrhotic liver has recently become a subject of intense investigation. The development of hepatocellular nodules demonstrating varying degrees of cellular and architectural atypia suggests that these nodular lesions represent a pathway of carcinogenesis in cirrhosis of different etiologies. This pathway involves processes, such as capillarization and neoangiogenesis, leading to a gradual change in blood supply from portal to arterial, as a dysplastic nodule becomes hepatocellular carcinoma. These changes in intranodular blood supply create different enhancement patterns in the two phases of liver circulation after an intravenous contrast injection on multi-phase helical CT or dynamic gadolinium-enhanced MRI. This article reviews the current concepts regarding the vascular changes occurring in dysplastic nodules in the multistep process of hepatocarcinogenesis, along with the associated imaging manifestations. Despite of the diagnostic potential of conventional CT (CCT), limitations being inherent in this technology reduce its diagnostic confidence and limit clinical CT applications as 3D imaging. Helical CT (HCT) has far overcome the limitations of CCT and has become the standard CT technology. After a short overview on the technique of HCT and its advantages over CCT, the impact of HCT on the detection of disorders of the urinary organs is discussed. Due to the high quality of 3D reconstructions, vessels are visualized free of artefacts resulting in a dramatic improvement and acceptance of CT angiography, which has become a clinically important examination in the evaluation of obstructive renal artery disease. Fast HCT provides a precise assessment of the three phases of the nephrogram and it is a prerequisite for an improved depiction of abnormal vascular perfusion and impaired tubule transit of contrast material. Helical CT enables an improved characterization of cystic mass lesions reducing the diagnosis of indeterminate masses and thus facilitating a better therapeutic management. The diagnosis of renal cell carcinomas (RCC) has improved due to an increased sensitivity in detecting small RCCs, and an increased specificity in the diagnosis of neoplastic lesions. Improved staging of RCCs is the result of accurate assessment of venous tumour extension. When planning nephron-sparing surgery 3D display of the renal tumour helps to determine the resectability of the mass depicting its relation to major renal vessels and the renal collecting system. In the evaluation of renal trauma HCT provides shorter scanning time and thus fewer artefacts in the examination of traumatized patients who cannot cooperate adequately. Three-dimensional postprocessing modalities allow the assessment of the renal vascular pedicel by CT angiography and improve the demonstration of complex lacerations of the renal parenchyma. In the evaluation of the upper urinary tract unenhanced HCT has become the imaging method of choice in the diagnosis and differential diagnosis of acute flank pain since it is highly sensitive and specific in detecting calculus disease. Unenhanced HCT may furthermore demonstrate causes of flank pain unrelated to urolithiasis. Gapless volume scanning and improved resolution in the z-axis during the excretory phase enables improved visualization of the renal collecting systems and ureters, resulting in a better demonstration of intraluminal and extraluminal pathology. To analyze helical CT false-positive multifocal breast cancers and to assess the relevance of the attenuation of tumors for diagnosing enhanced lesions. Helical CT studies of 156 invasive breast cancers before breast conserving surgery were examined. A lesion was defined as positive if focal enhancement was detected by CT within 100 seconds after contrast material administration. The attenuation and enhancement percent ratio [(post-contrast value/pre-contrast value)%] were obtained. Attenuation of false-positive and malignant lesions was compared. Helical CT enabled the detection of all 156 invasive tumors with 95 intraductal tumor extensions. The sensitivity and specificity of multifocal/multicentric disease detection by helical CT were 69% and 90%, respectively. False-positive multifocal/multicentric findings were obtained in 11 (7%) of 156 cases. The mean value of the enhancement percent ratio of the index tumors was 237%. Significant differences in the attenuation on post-contrast enhanced scans between the enhanced lesions (index tumors; mean, 82 HU), the true-positive multifocal/multicentric lesions (mean, 73 HU), the false-positive multifocal/multicentric lesions (mean, 87 HU) and normal breast tissue (mean, 32 HU) were found ( p < 0.0001). The attenuation of the true-positive multifocal/multicentric lesions on post-contrast enhanced scans was significantly less than that of the false-positive multifocal/multicentric lesions ( p = 0.03). Attenuation of tumor is not useful for differential diagnosis of enhanced lesions on helical CT of the breast. The presence of enhancement alone does not always indicate a malignant lesion. Purpose: To investigate the diagnostic role of helical CT in aortic intramural hematoma. Material and methods: We retrospectively evaluated CT images obtained during a 6-year period, between 1994 and 2000, in 427 patients who underwent helical CT. Only 21 patients had typical findings of aortic intramural hematoma. All studies were performed with helical technique before and after a power injection of nonionic contrast material. Results: Twenty-one patients had aortic wall thickening (9 type A and 12 type B in Stanford's classification). Wall thickening was more than 4 mm in all cases, it was irregular in shape in 16/21 patients (76 %) and had a regular concentric shape in the remaining 5/21 patients (24 %). One patient with type A hematoma died soon after CT diagnosis because of arrhythmia caused by hemopericardium. Five patients (4 type B and one type A) underwent pharmacological therapy and radiological follow-up. Fifteen patients (7 type A and 8 type B) underwent prosthetic surgery and six of these died of postoperative complications. Conclusion: Helical CT represents the first step in the early diagnosis of aortic intramural hematoma before complications develop. In our experience, which agrees with the reports of other authors, helical CT is an accurate and valuable investigation for identifying the location and extent of an aortic intramural hematoma. The purpose of this study was to determine hepatic and vascular enhancement, clinical tolerance, and iconographic quality of Iobitridol (300 mg/ml) at dual-phase helical CT and to compare it with Iohexol (300 mg/ml). One hundred forty-six patients were randomly divided into two groups. Each group received 120 ml of Iohexol (group A) or Iobitridol (group B). Mean enhancement of liver, aorta and portal vein was obtained at the arterial phase and at the portal-venous phase. Overall image quality was assessed by two independent blinded investigators. Adverse reactions were recorded. There were no significant differences in demographic characteristics and distribution of patient intrinsic parameters between the two groups, except for blood pressure but without statistical correlation between the difference in blood pressure and the impact on enhancement measurements. There was no significant difference in clinical tolerance and image quality. Mean liver as well as aortic and portal vein enhancement measurements did not show any significant difference. Iobitridol compares favorably with Iohexol. Both products have similar safety, tolerance, and efficacy. We compared contrast-enhanced harmonic gray-scale imaging with helical CT and US angiography to evaluate vascularity in advanced hepatocellular carcinoma (HCC). Contrast-enhanced harmonic gray-scale imaging using Levovist (Nihon Schering, Tanabe) as the contrast agent and enhanced helical CT were performed on 38 patients with 45 lesions (29 men and 9 women aged 41 to 83 years; mean age, 66 years; mean maximum tumor diameter, 30.5±23.0 mm), and angiography was performed to evaluate 37 lesions from 32 of these 38 patients (24 men and 8 women, aged 41 to 79 years; mean age, 65 years; mean maximum tumor diameter, 27.9±17.9 mm). Contrast-enhanced harmonic gray-scale imaging showed hypervascular enhancement in 41 of the 45 lesions; the other 4 lesions were not visualized as hypervascular because 3 of the them could not be detected with non-enhanced US and the remaining lesion was situated deep in the liver and more than 11 cm from the surface of the body. Helical CT showed areas of high attenuation in 40 of the 45 lesions, leaving the other 5 lesions equivocal, while US angiography achieved positive enhancement in 36 of 37 lesions. Intratumoral vessels were visualized with contrast-enhanced harmonic gray-scale imaging in 25 of the 45 lesions; however; intratumoral vessels were seen in only 4 of the 45 lesions examined with helical CT. In evaluating vascularity in advanced HCC, contrast-enhanced harmonic gray-scale imaging with Levovist was as effective as US angiography and more effective than helical CT. Motion artifacts produced by the heart make it difficult to evaluate vascularity in advanced HCC located in the left lobe of the liver with Doppler sonography. Contrast-enhanced harmonic gray-scale imaging can show intratumoral vessels and hypervascular enhancement of the tumor without motion artifacts, however, even when the tumor is located near the heart or large vessels. Contrast-enhanced harmonic gray-scale imaging is useful for evaluating vascularity in advanced HCC when the tumor can be visualized with non-enhanced US. [ABSTRACT FROM AUTHOR]

Recent progress in helical CT technology has given rise to considerable interest and expectations regarding diagnosis based on 3D CT images. In this context, computer-aided diagnosis techniques for 3D CT images are required. This study introduces a method for analysis of thoracic 3D CT images, which contain important information for lung cancer diagnosis. The lung field includes the bronchi and pulmonary arteries and veins, as well as other organs; lung cancer influences these organs in a complicated and intermixed way. The proposed method supports extraction and classification of bronchi, arteries, and veins from thoracic 3D CT images using anatomical knowledge and 3D image processing technologies, and analysis of the relations between these pulmonary organs and suspected lung cancer. The extraction of pulmonary organs includes removal of bias components, extraction of bronchi and blood vessels, discrimination between bronchi and blood vessels, and identification of arteries and veins among blood vessels. Quantitative relations between suspected lung cancer and adjacent pulmonary organs are found. The aim of this study was to examine the effectiveness of helical CT in the assessment of intraocular foreign bodies, evaluating two protocols with different collimation. We performed helical-CT studies in 30 patients. Fifteen patients were examined with 1.5-mm collimation and the other 15 patients with 3.0-mm collimation. All other imaging parameters were identical in both protocols. Multiplanar images were reconstructed. The examinations were reviewed for presence, localization and size of intraocular foreign bodies. We compare our results with the surgical data. We estimate the required examination time. In the first group (collimation 1.5 mm) an intraorbital foreign body was detected in 8 of 15 patients. In 3 of 8 patients an intraocular foreign body (all were metallic) was detected. In the second group (collimation 3.0 mm) an intraorbital foreign body was detected in 9 of 15 patients. In 8 of 9 patients an intraocular foreign body (all were metallic) was detected. Our results were confirmed by surgery in all cases. Examination time was 36 s in the first group and 18 s in the second group. Computed tomography should be considered the imaging modality of choice in the assessment of metallic intraocular foreign bodies and 3.0-mm collimation is optional, because of reduced examination time and radiation exposure. Insulinomas are pancreatic neoplasms that can be radiologically characterized typically because of their tendency to present intense and early contrast enhancement with a wash-out phenomenon. In this sense, we report an unusual case of a hypovascular solid pancreatic insulinoma confirmed with surgery and pathologic analysis, in a patient with normal serum insulin levels. In the two-phase helical CT, the mass behaved as a hypodense lesion with respect to the surrounding pancreatic parenchyma during the arterial phase and as a hypointense lesion during the dynamic contrast-enhanced MR imaging. Pathologic examination demonstrated a hypercellular tumor with poor vascularization of intervening stroma which showed prominent amyloid deposits. [ABSTRACT FROM AUTHOR]

The aim of this study was to evaluate the value of automatic bolus tracking for monophasic spiral CT of the liver and to assess the liver-to-lesion conspicuity in comparison with time-delay examinations. In 40 patients scheduled for therapy control of known hypovascular hepatic metastases a monophasic spiral CT was completed either with time delay of 65 s (n = 20) or with automatic bolus tracking in the liver parenchyma (n = 20). Examinations were performed with 120 ml of contrast material and a flow rate of 3.0 ml/s. For automatic bolus tracking a parenchymal enhancement threshold of 40 HU was used. Contrast enhancement in the liver parenchyma and in liver lesions was obtained by means of regions of interest (ROI). Mean parenchymal enhancement was not significantly different between time delay and bolus-tracking group. In 4 of 20 patients in the bolus-tracking group the threshold level of 40 HU was not reached. With automatic bolus tracking a significantly higher liver-to-lesion density difference was observed (P < 0.0001). Automatic bolus tracking allows a better liver-to-lesion conspicuity in monophasic spiral CT. The purpose of this study was to evaluate the accuracy of contrast-enhanced high resolution helical computed tomography (CT) for assessing locoregional staging of palpable Tl-2 invasive breast cancer. Methods: Helical CT studies of 156 lesions from 156 patients with invasive breast cancer before breast-conserving surgery were examined. A lesion was defined as positive if focal enhancement was detected by CT within 100 seconds after contrast material administration. After resection, tumors were histopathologically mapped and comparison made with the extent of contrast enhancement. Helical CT enabled detection of all 156 index tumors. CT enabled detection of 28 of 43 multifocal lesions (65%) and five of five multicentric lesions (100%). In 24 of 33 lesions (73%), CT revealed additional cancers not seen on mammography. The extent of tumor significantly correlated with CT measurements ( r=0.76, p<0.0001). The aim of this study was to evaluate the value of automatic bolus tracking for biphasic spiral CT of the liver in comparison with time delay examinations. Forty patients scheduled for a biphasic spiral CT of the liver randomly were examined either with time delay of 25 s for the arterial phase and 55 s for the portal-venous phase (n = 20), or with an automatic scan start triggered by contrast enhancement in the aorta (n = 20). Examinations were performed with 120 ml of contrast material and a flow rate of 4.0 ml/s. Density measurements of the aorta, of the liver parenchyma, and of the spleen were obtained by means of regions of interest (ROI). The end of the arterial phase was considered when hepatic parenchymal enhancement was greater than 20 HU. In all patients of the group with automatic bolus tracking arterial scanning was completed in the arterial phase of the liver. In 25 % of patients with fixed time delay, however, an enhancement of liver parenchyma during arterial phase greater than 20 HU was observed. During the portal-venous phase there was no significant difference in parenchymal enhancement between both groups. Automatic bolus tracking allows an individualized timing of the arterial phase in biphasic spiral CT of the liver. The timing is more accurate than in time delay scanning. [ABSTRACT FROM AUTHOR]

The goal of this study was to determine the diagnostic value of the helical CT without contrast in suspected cases of choledocholithiasis, comparing this test with endoscopic retrograde cholangiopancreatography (ERCP). Forty patients with possible choledocholithiasis were studied prospectively. There were 23 women and 17 men, ranging in age from 24 to 91 years. Helical CT was performed immediately before ERCP (time interval between the two procedures was less than 1 h). A biliary area previously selected was studied with a technique of pitch 1 and slice thickness of 3.2 mm. Average time was 30 s. Reconstruction with different increments and windows were made. Stone presence was evaluated in bile duct and Vater's ampulla. Biliary dilation was evaluated too. Endoscopic retrograde cholangiopancreatography found stones in 19 patients and absence of stones in 20. One case was failed, but stones in bile duct were demonstrated during intraoperative cholangiography. Helical CT demonstrated stones in 15 of the 19 patients with positive ERCP. There were no false positives with CT. Patients without stones in ERCP were also negative in CT. The patient having the failed ERCP was considered positive in CT. The CT sensitivity was 80 % and specificity was 100 %, with an accuracy of 90 %. Helical CT without contrast has sensitivity and specificity good enough to be used as a screening technique in patients with suspected choledocholithiasis. [ABSTRACT FROM AUTHOR]

Objective. To evaluate the accuracy of three-dimensional (3D) helical computed tomography (CT) for assessing the extent of breast cancer of candidates for breast conserving surgery. Methods. Results of helical CT were studied in 144 lesions of 144 patients with breast cancer before breast-conserving surgery. A lesion was defined as positive if focal enhancement was detected by CT within 100s after contrast material administration. After resection, tumors were histopathologically mapped and correlated with the extent of 3D images. Results. Helical CT enabled detection of 143 tumors but not of one ductal carcinoma in situ (DCIS). The median deviation of the tumor extension revealed by 3D helical CT images from pathological assessment was 7.7mm (range 0–60mm). The extent of tumors was significantly correlated with CT measurements ( r=0.714, p<0.0001). By multivariate analysis, the presence of invasive tumors with intraductal extensions beyond the edge of the invasive tumor and histologic type (DCIS) were significant risk factors for deviation of the tumor extension revealed by 3D helical CT images from pathological assessment. Conclusion. Three dimensional helical CT of the breast is an accurate preoperative imaging modality for assessing the extent of breast cancer candidates for breast conserving surgery. In the past decade alternatives to urography have been proposed for the study of patients with renal colic. In 1992 it was suggested to replace urography with KUB and ultrasonography. In 1993 the combination of KUB and ultrasonography followed by urography in unresolved cases was proposed and, in 1995, it was suggested to replace urography with unenhanced helical CT (UHCT). This article illustrates the contribution of UHCT to the study of patients with renal colic and analyses advantages and shortcomings of the technique compared with other diagnostic approaches. Diagnostics of the patient with renal colic is based on the detection of direct and indirect signs which allow identification of not only the calculus, with a sensitivity of 94–100 % and accuracy of 93–98 % according different authors, but also other signs that can serve to guide patient management and evaluate long-term prognosis. Unenhanced helical CT has the capability to detect extraurinary abnormalities which present with flank pain and mimic renal colic. The examination technique affects the quality of the images and therefore diagnostic accuracy as well as the dose to the patient. With regard to setting parameters, the choice of thickness and table feed should be guided by numerous factors. Multiplanar reconstruction is indicated in the study of the entire ureter course to identify the exact site of the calcification for the urologist to perform an evaluation similar to that obtained by urography. Many authors consider UHCT to be a valuable tool for suggesting the best therapeutic approach. Among these there are also urologists. The evaluation is based on the stone detection, its size and level in the urinary tract. Cost analysis shows that the cost of UHCT is equal to or inferior to the cost of urography. With regard to the dose, different data are reported in the literature. A high pitch (more than 1.5) and a thin collimation (3-mm thickness) are good compromise between quality and dose which can be compared to the dose of normal urography. What is to be done if helical CT is not available? If helical CT is not available, plain film plus ultrasonography should be considered. This approach does not solve all the cases; in unresolved cases urography is indicated. It should also be noted that US has a good sensitivity in detecting other conditions such as biliary lithiasis, acute pancreatitis, acute appendicitis and abdomino-pelvic masses which are responsible for pain that mimics renal colic. In conclusion, IVU should not have any more the priority in investigating the patients with renal colic. Helical CT should be the first choice in imaging a patient with renal colic. Purpose: The fast helical CT technique allows examination of the kidneys during different phases of contrast medium enhancement. However, every additional phase increases the radiation dosage to the patients. We investigated the detection rate and characterisation of renal lesions during different phases and evaluated them separately, and considered the possibility of excluding phases without loss of important information.Material and Methods: Sixty patients who underwent contrast-enhanced multiphasic renal helical CT examination were included. Every CT phase was evaluated separately. The number of lesions and the characteristics of the lesions were noted and all lesions were viewed together.Results: A total of 153 cysts and 17 solid lesions were detected. The largest and an equal number of cysts (142/143) was detected in the nephrographic and excretory phases. However, the nephrographic phase detected more cortical cysts and the excretory phase detected more sinus cysts. All solid lesions were detected in all phases. Renal parenchymal tumours were best characterised in the cortical phase and angiomyolipomas in the native phase.Conclusion: The cortical phase was best for characterisation of renal parenchymal tumours. The nephrographic and excretory phases were best in detecting and characterising renal cysts. The nephrographic phase was the phase giving the least diagnostic information. [ABSTRACT FROM AUTHOR]

Purpose: To compare helical CT to conventional CT imaging in the assessment of orbital metallic foreign bodies with regard to image quality, scanning time, and radiation exposure. Methods: Twenty-four standardized metallic foreign bodies, placed into the orbit (anterior, epibulbar, posterior) of cadaver heads were studied. Helical CT scanning in the axial plane with multiplanar reconstruction of coronal and sagittal images was performed as well as conventional CT imaging with direct scanning in the axial and the coronal planes (Tomoscan SR 7000, Philips, The Netherlands). Two masked observers consensually graded the studies using 5 predetermined criteria. Radiation dose delivered to the lens and scanning time were measured for the helical and the conventional CT imaging workup. Results: Helical CT imaging scored statistically significantly better with regard to overall accuracy of foreign body localization and presence of beam-hardening streak artifacts from dental fillings. Conventional CT scored significantly better with regard to stair-step artifacts. The radiation dose delivered to the lens was 35.4 mGy for helical CT imaging and 73.9 mGy for conventional CT workup (axial and coronal scanning). Total scanning time was 18 seconds for helical CT axial scanning and 104 seconds for conventional CT axial and coronal scanning. Conclusion: Helical CT is superior to conventional CT imaging, because it can provide adequate information about orbital metallic foreign bodies with a single acquisition, thus reducing both the number of examinations and the radiation exposure for the patient.
