In the past years infections caused by multidrug-resistant Gram-negative bacteria have dramatically increased in all parts of the world. This consensus paper is based on presentations, subsequent discussions and an appraisal of current literature by a panel of international experts invited by the Rudolf Schülke Stiftung, Hamburg. It deals with the epidemiology and the inherent properties of Gram-negative bacteria, elucidating the patterns of the spread of antibiotic resistance, highlighting reservoirs as well as transmission pathways and risk factors for infection, mortality, treatment and prevention options as well as the consequences of their prevalence in livestock. Following a global, One Health approach and based on the evaluation of the existing knowledge about these pathogens, this paper gives recommendations for prevention and infection control measures as well as proposals for various target groups to tackle the threats posed by Gram-negative bacteria and prevent the spread and emergence of new antibiotic resistances., In den letzten Jahren haben die durch multiresistente Gram-negative Bakterien (MRGN) verursachten Infektionen in allen Teilen der Welt dramatisch zugenommen. Der vorliegende Konsensus basiert auf Vorträgen mit sich anschließenden Diskussionen und späterer Auswertung der einschlägigen Literatur durch ein internationales Expertengremium, das von der Rudolf Schülke Stiftung, Hamburg, zu dem Meeting nach Hamburg eingeladen worden war. Im Fokus standen die Epidemiologie und die besonderen Eigenschaften Gram-negativer Bakterien, die Ausbreitung der Antibiotikaresistenz, die Reservoire, Übertragungswege und Risikofaktoren für Infektionen, die Mortalität, die Therapie und die Möglichkeiten der Prävention einschließlich der Konsequenzen des Vorkommens in der industriellen Tierhaltung. Objectives: To document the occurrence and distribution of antibiotic resistance of clinically important bacteria in the seawater of Gaza strip-Palestine. Methods: Seawater samples were collected at 16 location distributed along the coast of the Gaza strip. Sampling was accomplished during 12 months, from March 2014 to June 2015. The microbial composition including Enterobacteriaceae, Staphylococcus aureus, fecal streptococci and Pseudomonas aeruginosa was recorded and tested for their resistance to specific antimicrobial agents according to CLSI using the disc diffusion method. Results: A total of 816 isolates of Enterobacteriaceae (377), S. aureus (29), fecal enterococci (FS) (369), and P. aeruginosa (29) were recovered and identified. Enterobacteriaceae, P. aeruginosa, FS and S. aureus isolates exhibited the highest rates of resistance against β-lactam drugs. The isolates also showed resistance to at least one antimicrobial in the range between 99.7 to 78%. Multiple resistance occurred in almost 85% of all isolates; 99.2% of Enterobacteriaceae, 96.6% of P. aeruginosa, 72.1% of FS and 61% of S.aureus. The incidence of multiple resistance of isolates from all sampling locations ranged from 69.2 to 94.1%. Antibiotic resistance indices were found to be highest in P. aeruginosa (0.57), followed by E. coli (0.53), FS (0.49), Enterobacter (0.41), S. marcescens (0.40), Klebsiella (0.39) and finally Proteus (0.28). Most of the isolates showed multiple antibiotic resistance (MAR) index value higher than 0.2. Conclusions: This study demonstrated that the seawater of the Gaza strip is highly contaminated with antibiotic resistant bacteria which can be transmitted to humans through recreational and other activities. Therefore, there is a need to apply appropriate and rationale use of antibiotic to minimize the occurrence of multiple antibiotic resistant bacteria in the marine environment. Several exposure scenarios have been evaluated taking into account the material used, information related to BPA leaching, the duration of a single treatment and the frequency of treatments, giving rise to toxicologically relevant acute, short and long term exposure. The SCENIHR concludes that risk for adverse effects of BPA may exist when the BPA is directly available for systemic exposure after non-oral exposure routes, especially for neonates in intensive care units, infants undergoing prolonged medical procedures and for dialysis patients. Although the benefit of medical devices has also to be considered, the SCENIHR recommends that, where practicable, medical devices that do not leach BPA should be used. The possibility of replacing BPA in these products should be considered against their efficiency in the treatment, as well as the toxicological profile of the alternative materials. However, better data on exposure would be beneficial for the refinement of the present risk assessment, to be carried out when new data on exposure via medical devices will be available.

National audience; La découverte de micromycètes du genre Fusarium dans le réseau hydrique du CHU de Dijon nous a conduit à mettre en place un Protocole Hospitalier de Recherche Clinique (PHRC) dont l’objectif a été de décrire en prospectif, dans le temps et dans l’espace, la contamination par Fusarium dans l’eau de 2 CHU français (Dijon et Nancy), à des périodes d’activités de restructuration et de construction. Cette étude a ainsi permis de mettre en évidence la présence chronique de Fusarium spp. dans les circuits de distribution de l’eau de certains bâtiments hospitaliers.1 Nous avons en effet observé que ces micromycètes étaient "installés" dans le réseau, que leur densité de population variait au cours du temps malgré i) le traitement chloré de l'eau, ii) l'existence de portions de la tuyauterie en cuivre (libérant des ions Cu susceptibles d'inhiber le développement des champignons), iii) et l'absence a priori de sources de C et N utilisables comme substrat de croissance par les champignons. L’identification des souches par biologie moléculaire (amplification du gène du facteur d’élongation TEF-1α par le amorces EF1 et EF2) a permis de montrer la présence de 2 espèces de Fusarium dans le réseau hydrique du CHU: Fusarium oxysporum (majoritaire) et Fusarium dimerum. A notre surprise, ces 2 mêmes espèces ont également été retrouvées dans les prélèvements d’eau de Nancy. En utilisant d’autres outils de biologie moléculaire, mais aussi une étude de compatibilité végétative, il a été possible de montrer l’existence de populations clonales de Fusarium (espèces oxysporum et dimerum), présentes à la fois dans le réseau d’eau de Dijon (Bocage et Hôpital général), celui du CHU de Nancy, mais également dans les circuits d’alimentation en eau d'hôpitaux d'autres pays du monde.2 A ce stade, nous avons donc émis l’hypothèse que certaines souches de Fusarium étaient donc plus adaptées à une vie en milieu aquatique que d’autres souches, et qu’il serait intéressant de comparer le comportement phénotypique de ces souches hydriques avec des souches de Fusarium d’origine tellurique (le sol étant le réservoir naturel du champignon). Divers tests en microplaques ont donc été mis au point pour confirmer cette hypothèse. Nous avons ainsi comparé la tolérance de souches hydriques ou telluriques vis-à-vis de divers agents chimiques comme le chlore ou le cuivre (élément fongistatique), et nous avons par exemple pu constater une meilleure tolérance au cuivre de la part de tous les isolats d’origine hydrique. Enfin, des études de dynamique de population ont également été envisagées pour mieux comprendre le comportement atypique de ces souches isolées de l’eau. Une première expérience réalisée en milieu « sol stérile » a révélé que les croissances fongiques de souches issues du sol étaient plus faibles que celles de souches issues de l’eau. Une deuxième expérience réalisée en milieu « eau du CHU de Dijon » a montré une chute de la croissance des souches telluriques, et à l’inverse une reprise de la croissance pour les souches d’origine hydrique. L’ensemble des ces résultats nous a permis de conclure que nous étions en présence de souches fongiques de Fusarium particulièrement adaptées à une vie en milieu aquatique. 1 Sautour M. et al. 2012. The harmonisation of training programmes for infection control and hospital hygiene (IC/HH) professionals in Europe is a requirement of the Council recommendation on patient safety. The European Centre for Disease Prevention and Control commissioned the ‘Training Infection Control in Europe’ project to develop a consensus on core competencies for IC/HH professionals in the European Union (EU). Core competencies were drafted on the basis of the Improving Patient Safety in Europe (IPSE) project’s core curriculum (CC), evaluated by questionnaire and approved by National Representatives (NRs) for IC/HH training. NRs also re-assessed the status of IC/HH training in European countries in 2010 in comparison with the situation before the IPSE CC in 2006. The IPSE CC had been used to develop or update 28 of 51 IC/HH courses. Only 10 of 33 countries offered training and qualification for IC/ HH doctors and nurses. The proposed core competencies are structured in four areas and 16 professional tasks at junior and senior level. They form a reference for standardisation of IC/HH professional competencies and support recognition of training initiatives., peer-reviewed

Background: The Rudolf Schuelke Foundation addresses topics related to hygiene, infection prevention and public health. In this context a panel of scientists from various European countries discussed “The Role of Surface Disinfection in Infection Prevention”. The most important findings and conclusions of this meeting are summarised in the present consensus paper. Aim: Although the relevance of surface disinfection is increasingly being accepted, there are still a number of issues which remain controversial. In particular, the following topics were addressed: Transferral of microbes from surface to patients as a cause of infection, requirements for surface disinfectants, biocidal resistance and toxicity, future challenges. Methods and findings: After discussion and review of current scientific literature the authors agreed that contaminated surfaces contribute to the transmission of pathogens and may thus pose an infection hazard. Targeted surface disinfection based on a risk profile is seen as an indispensable constituent in a multibarrier approach of universal infection control precautions. Resistance and cross-resistance depend on the disinfectant agent as well as on the microbial species. Prudent implementation of surface disinfection regimens tested to be effective can prevent or minimize adverse effects. Conclusions: Disinfection must be viewed as a holistic process. There is a need for defining standard principles for cleaning and disinfection, for ensuring compliance with these principles by measures such as written standard operating procedures, adequate training and suitable audit systems. For the prevention of the toxic risks linked to water consumption, it is broadly possible to use a classical risk assessment process and to be able to at quality limits with a guarantee either an absence of risk or a risk limited to one case of sickness for 105 or 106 consumer-full live. For the prevention of microbiological risks it is much more difficult to conduct a dull risk assessment process ; thus, since more than one century the classical indicators of fecal contamination are lasting, in the framework of the precaution principle. Because of the evolution of the population in developed countries, the knowledge of biological hazards unlinked to fecal contamination or more resistant to the water treatments, as the classical fecal indicators, it is now necessary to think in term of risk assessment and to to try to propose relevant indicators., Pour la prévention des risques toxicologiques liés à l’eau, il est en général possible de dérouler la démarche d’évaluation du risque et d’aboutir à la fixation de limites de qualité garantissant soit l’absence de risque soit un risque limite à un cas d’atteinte sur 105 ou 106 consommateurs-vie entière. Pour la prévention des risques microbiologique il est beaucoup plus difficile de mener complètement cette même démarche d’évaluation des risques, et depuis plus d’un siècle ont été mis en place les classiques indicateurs de contamination fécale, qui ressortent du principe de précaution. Le plomb est un métal exploité depuis des millénaires, mais son usage s'estaccentué avec la révolution industrielle. L'exposition de populations travaillantdans les industries utilisant le plomb a révélé les effets délétères de ce métal surdifférentes fonctions biologiques. L'introduction du plomb dans certaines peinturesen tant que pigment, puis dans l'essence après la découverte de ses propriétésantidétonantes, a eu pour conséquence une diffusion encore plus large dansl'environnement. Cette pollution, touchant l'ensemble de la planète, conduit às'interroger maintenant sur les conséquences en termes de santé publique d'uneexposition permanente au cours de la vie à des doses faibles ou moyennes. Lesenfants sont une population particulièrement vulnérable puisque les effets du plombsont importants sur le cerveau en développement, mais ces effets toxiquespourraient également se manifester à d'autres périodes de la vie.L'analyse des études épidémiologiques effectuées dans les populationsprofessionnellement exposées au plomb ou en population générale et des travauxexpérimentaux in vivo et in vitro, permet de répertorier les différents effets duplomb sur l'organisme.
