Introduction: Adolescents with Down syndrome (DS) show high rate of overweight and tend to accumulate high amount of fat compared to the same people without the syndrome. Objective: To estimate the cutoff point of the Body Mass Index (BMI) for the diagnosis of obesity in adolescents with DS according to different references for BMI in relation to the percentage of body fat (%BF) measured by dual-energy X-ray absorptiometry (DXA). Methods: The sample was composed of 34 adolescents with DS (aged: 10 to 17 years old). BMI was evaluated according to the references of the International Obesity Task Force (IOTF), the World Health Organization (WHO) for the general population, and Myrelid et al. and Styles et al. for people with DS. The %BF was assessed by whole body DXA and classified according to National Health And Nutrition Examination Survey (NHANES, 2011). Results: The boys were significantly taller than the girls and this %BF higher than boys. All references who have used BMI to assess obesity was positively associated with %BF measured by DXA in the diagnosis of obesity. Using the ROC curve in relation to %BF by DXA, all references showed high sensitivity, but the z-score of BMI by WHO showed better specificity, with the value of the accuracy of 0.82 for the cutoff point above 2.14. Conclusions: All the references used for the diagnosis of obesity were associated with %BF measured by DXA, and the cutoff point of z-scores above 2.14 by WHO showed better specificity. Introducción: adolescentes con síndrome de Down (SD) muestran alta tasa de sobrepeso y tienden a acumular gran cantidad de grasa en comparación con los adolescentes sin el síndrome. Objetivo: estimar el punto de corte del índice de masa corporal (IMC) para el diagnóstico de obesidad en adolescentes con SD de acuerdo con las diferentes referencias para el IMC en relación con el porcentaje de grasa corporal (%GC) obtenido por el absorciometría con rayos X de doble energía (DXA). Métodos: la muestra se compone de 34 adolescentes con SD (10 a 17 años). El IMC se evaluó de acuerdo con las referencias de la International Obesity Task Force (IOTF), Organización Mundial de la Salud (OMS) para la población general, y Myrelid et al. y Estyles et al. para personas con SD. El %GC fue evaluado por DXA y clasificado según NHANES (2011). Resultados: los niños fueron significativamente más altos que las niñas, y estas con mayor %GC que los niños. Todas las referencias que han utilizado el IMC se asociaron positivamente con el %GC medido por DXA en el diagnóstico de la obesidad. Utilizando la curva ROC en relación con %GC por DXA todas las referencias mostraron alta sensibilidad, pero el score-z del IMC por la OMS mostró mejor especificidad, con el valor de exactitud de 0,82 para el punto de corte por encima de 2,14. Conclusiones: todas las referencias utilizadas para el diagnóstico de obesidad se asociaron con el %GC medido por DXA, y el punto de corte del score-z por encima de 2,14 por la OMS mostró mejor especificidad.

To review the prevalence of overweight and obesity and their determinants in youth with DS. The health consequences and the effectiveness of interventions were also examined. A search using MEDLINE, Embase, Web of Science, Scopus, CINAHL, PsycINFO, SPORTDiscus, LILACS, and COCHRANE was conducted. From a total of 4280 studies, we included 45 original research articles published between 1988 and 2015. The combined prevalence of overweight and obesity varied between studies from 23% to 70%. Youth with DS had higher rates of overweight and obesity than youths without DS. Likely determinants of obesity included increased leptin, decreased resting energy expenditure, comorbidities, unfavorable diet, and low physical activity levels. Obesity was positively associated with obstructive sleep apnea, dyslipidemia, hyperinsulinemia, and gait disorder. Interventions for obesity prevention and control were primarily based on exercise-based programs, and were insufficient to achieve weight or fat loss. Population-based research is needed to identify risk factors and support multi-factorial strategies for reducing overweight and obesity in children and adolescents with DS COORDENAÇÃO DE APERFEIÇOAMENTO DE PESSOAL DE NÍVEL SUPERIOR - CAPES Fechado

The aim of this study was to systematically review studies on intervention programs involving physical activity promotion and/or nutritional education to reduce health-related risk factors (overweight/obesity and metabolic profilealterations) in Brazilian adolescent students. A search was performed in the following electronic databases: Medline (PubMed), Lilacs, Embase, Scielo and Capes Thesis Database. A total of 1,568 studies were identified, of which 21 full papers were analyzed and 5 were included in the systematic review. The majority of the studies involved physical activity practice combined with nutritional education in Brazilian adolescent students and all interventions showed positive effect on reducing health-related risk factors among Brazilian adolescents.It was concluded that all studies included in this review showed positive alterations in health-related risk factors after interventions, reinforcing the importance of intervention programs to promote a healthier lifestyle and reduce health-related risk factors in adolescents. Resumo O objetivo do estudo foi realizar uma revisão sistemática sobre os programas de intervenção, com ações de atividades físicas e/ou educação nutricional, na redução de fatores de risco à saúde (sobrepeso/obesidade e alterações no perfil metabólico), em adolescentes escolares brasileiros. Foi realizada busca nas bases de dados eletrônicas Medline (PubMed), Lilacs, Embase, Scielo e Banco de Teses da Capes. Identificaram-se 1.568 estudos e 21 textos completos foram analisados, dos quais, cinco foram incluídos na revisão. A maioria dos estudos envolveu a prática de atividades físicas combinada à educação nutricional em diferentes regiões do país e todas as intervenções mostraram efeitos positivos na redução de fatores de risco à saúde. Conclui-se que todos os estudos nesta revisão demonstraram alterações positivas após as intervenções, mesmo que não significativas estatisticamente, reforçando a importância de programas de intervenção para a promoção de uma vida mais saudável e para reduzir os fatores de risco à saúde, em adolescentes.

This cross-sectional study compared 55 ALL survivors, of chronological age between 15 years and 24 years, assigned into two groups according to the exposure to cranial radiation therapy (CRT; 25 irradiated and 30 nonirradiated) with 24 leukemia-free controls, and assessed body fat mass (dual-energy X-ray absorptiometry), computed tomography scan-derived abdominal adipose tissue, lipid profile, blood pressure (BP), adipokines, and cIMT by a multiple regression analysis. Treatment with CRT had an effect on all of the variables derived from the computed tomography scan: visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT) (P,0.050). In a multiple linear regression model, cIMT positively correlated with exposure to CRT (P=0.029), diastolic BP (P=0.016), and leptin-to-adiponectin ratio (P=0.048), while negatively related to SAT (P=0.007). In young survivors of childhood ALL, CRT modified the distribution of fat and played a critical role in determining cIMT. Leptin-to-adiponectin ratio, a biomarker of abdominal obesity and metabolic syndrome, and diastolic BP also influenced cIMT, a marker of subclinical atherosclerosis. Nonetheless, adiposity-associated vascular disease might be attenuated by SAT. Changes in body fat must be evaluated in this group of patients in the early course of survivorship in order to avoid premature cardiovascular disease associated with atherosclerosis. OBJETIVO: Avaliar o crescimento e a composição corporal de crianças e adolescentes com diabetes tipo 1 (DM1). SUJEITOS E MÉTODOS: Estudo de uma coorte de 44 pacientes com DM1 acompanhados em média por quatro anos, comparados a um grupo controle. Avaliaram-se peso, estatura, índice de massa corporal (IMC), porcentagem de massa gorda (%MG), índice de massa gorda, cintura e razão cintura/estatura. RESULTADOS: No sexo feminino, na primeira avaliação a %MG das pacientes foi menor, enquanto na segunda avaliação a média da cintura das pacientes foi maior que a dos controles. No sexo masculino, a estatura dos pacientes foi menor na primeira avaliação, enquanto o IMC foi maior na segunda. Não foram observadas diferenças entre as variações dos escores z de estatura, peso e IMC e da %MG ou na distribuição desses escores entre as duas avaliações em ambos os grupos. A análise multivariada mostrou diferença no IMC e na razão cintura/estatura dos dois sexos e também na cintura das meninas. CONCLUSÃO: Os pacientes apresentaram crescimento adequado, porém diferiram na composição corporal durante o período do estudo. OBJECTIVE: To evaluate the growth and body composition of children and adolescents with type 1 diabetes mellitus (T1DM). SUBJECTS AND METHODS: A cohort of 44 patients with T1DM were followed up for approximately four years and compared with a control group. Weight, height, body mass index (BMI), body fat percentage (BF%), fat mass index, waist circumference (WC) and waist-height ratio were determined. RESULTS: In females, in the first evaluation, BF% was lower in patients than in controls, while in the second evaluation, mean WC was higher in patients than in controls. In males, height of the patients was lower in the first evaluation, while body mass index (BMI) was higher in the second one. We did not find any differences among the changes in height, weight and BMI z-scores and BF% or in the distribution of those z-scores between the two evaluations, in both groups. Multiple regression analysis found differences in BMI and waist-height ratio in both sexes and also in WC in females. CONCLUSION: The patients had adequate growth but showed discrepancy in their body composition during the study.

OBJECTIVE: To verify if the frequency of spontaneous pubertal development among girls with Turner syndrome (TS) diagnosed in infancy and childhood is greater than that of patients diagnosed later. SUBJECTS AND METHODS: Thirty three girls aged < 10 years at the time of diagnosis were evaluated regarding pubertal development. The frequency of spontaneous puberty was compared with that of girls aged > 13 years diagnosed at the same service. RESULTS: Sixteen of 32 informative patients had signs of spontaneous puberty, a frequency greater than that of patients diagnosed later. In six patients, there was no progression of puberty; menarche occurred in six, and one became pregnant, but the fetus was a stillborn. Spontaneous puberty was absent in all cases with 45,X karyotype. CONCLUSIONS: The greater prevalence of spontaneous puberty in girls whose diagnosis was not based on pubertal delay suggests that, among those diagnosed later, there is a bias towards patients with hypogonadism. Arq Bras Endocrinol Metab. 2012;56(9):653-7 OBJETIVO: Verificar se a frequência de puberdade espontânea em meninas com síndrome de Turner (ST) diagnosticadas na infância é superior a de pacientes diagnosticadas posteriormente. SUJEITOS E MÉTODOS: Foram avaliadas 33 meninas < 10 anos ao diagnóstico quanto ao desenvolvimento puberal. A frequência de puberdade espontânea foi comparada com a de pacientes com mais de 13 anos diagnosticadas no mesmo serviço. RESULTADOS: Dezesseis das 32 pacientes informativas tiveram sinais puberais espontâneos, frequência superior a daquelas diagnosticadas posteriormente. Em seis delas, não houve progressão da puberdade; a menarca ocorreu em seis casos e uma paciente ficou grávida, porém o feto foi natimorto. Em todos os casos com cariótipo 45,X não ocorreu puberdade espontânea. CONCLUSÕES: A maior prevalência de puberdade espontânea em meninas cujo diagnóstico não se baseou em atraso puberal sugere que naquelas detectadas posteriormente haja distorção em favor de pacientes com hipogonadismo. Disorders of sex development (DSD) involve several conditions that result from abnormalities during gonadal determination and differentiation. Some of these disorders may manifest at birth by ambiguous genitalia; others are diagnosed only at puberty, by the delayed onset of secondary sexual characteristics. Sex determination and differentiation in humans are processes that involve the interaction of several genes such as WT1, NR5A1, NR0B1, SOX9, among others, in the testicular pathway, and WNT4, DAX1, FOXL2 and RSPO1, in the ovarian pathway. One of the major proteins in mammalian gonadal differentiation is the steroidogenic nuclear receptor factor 1 (SF1). This review will cover some of the most recent data on SF1 functional roles and findings related to mutations in its coding gene, NR5A1. Os distúrbios do desenvolvimento sexual (DDS) envolvem várias condições que resultam de anormalidades que podem acontecer tanto na determinação como durante a diferenciação gonadal. Algumas dessas doenças podem se manifestar ao nascimento principalmente por genitália ambígua, outras são diagnosticadas apenas na puberdade por atraso no aparecimento de características sexuais secundárias. A determinação e a diferenciação do sexo em seres humanos são processos que envolvem interações entre vários genes nas vias testicular, tais como NR5A1, NR0B1, WT1, SOX9, entre outros, e ovariana, tais como WNT4, DAX1, FOXL2 e RSPO1. Uma das principais proteínas na diferenciação gonadal de mamíferos é o fator esteroidogênico e receptor nuclear 1 (SF1). Esta revisão cobrirá alguns dos dados mais recentes sobre os papéis funcionais de SF1 e as últimas descobertas relacionadas a mutações em seu gene, NR5A1.

OBJECTIVE: To evaluate insulin resistance and lipid profile in women with congenital adrenal hyperplasia (CAH) caused by classical 21-hydroxylase deficiency (21OHD), and their association with body mass index (BMI) and corticosteroid dosage. SUBJECTS AND METHODS: We assessed BMI, waist circumference, current glucocorticoid dosage, glucose, insulin and lipid profile in eighteen young women (mean ± SD, 19.3 ± 3.0 years) with 21OHD CAH. RESULTS: BMI was normal in 12 patients, 5 of them were overweight, and 1 was obese. Waist circumference was high in 7 patients. Fasting insulin and HOMA-IR were elevated in seven and eight patients, respectively. Total cholesterol and triglycerides were high in only two patients, and HDL-cholesterol was low in four. Insulin resistance was not associated with BMI, waist circumference or glucocorticoid dose. CONCLUSIONS: Young women with 21OHD CAH had infrequent dyslipidemia, but had a higher prevalence of insulin resistance and central obesity, that were independent of BMI or corticosteroid dosage. OBJETIVO: Avaliar a presença de resistência insulínica e dislipidemia em mulheres com hiperplasia adrenal congênita (HAC) por deficiência da 21-hidroxilase (21OHD) e investigar a associação com índice de massa corporal (IMC) e dose de glicocorticoide prescrita. PACIENTES E MÉTODOS: Em 18 mulheres jovens (média ± DP, 19,3 ± 3,0 anos), avaliamos IMC, circunferência abdominal, dose de glicocorticoide, glicemia, insulinemia e perfil lipídico. RESULTADOS: O IMC foi normal em 12 pacientes; 5 apresentavam sobrepeso e 1 apresentou obesidade. Circunferência abdominal estava aumentada em 7 pacientes. Insulinemia de jejum e HOMA-IR estavam elevados em 7 e 8 pacientes, respectivamente. Apenas 2 pacientes apresentaram aumento de colesterol total ou de triglicérides e 4, diminuição dos níveis de HDL-colesterol. Resistência insulínica não apresentou associação com IMC, circunferência abdominal ou dose de glicocorticoide prescrita. CONCLUSÃO: Mulheres jovens com CAH 21OHD apresentaram pouca dislipidemia, mas tiveram alta prevalência de resistência insulínica e obesidade central, independentemente do IMC e da dose de glicocorticoide prescrita.

The Y-chromosome-located SRY gene encodes a small testis-specific protein containing a DNA-binding motif known as the HMG (high mobility group) box. However, mutations in SRY are not frequent especially in cases of 46,XY partial gonadal dysgenesis. Several sex-determining genes direct the fate of the bipotential gonad to either testis or ovary. In addition, heterozygous small deletions in 9p can cause complete and partial XY gonadal dysgenesis without other symptoms. Human DMRT1 gene, which is located at 9p24.3, is expressed in testis and ovary and has been considered, among others, a candidate autosomal gene responsible for gonadal dysgenesis. In this report we describe a nucleotide insertion in DMRT1 3'UTR in a patient of XY partial gonadal dygenesis. The 3'UTR+11insT is located within a conserved motif important for mRNA stabilization. O gene SRY, localizado no cromossomo Y, codifica uma proteína testículo-específica contendo um domínio HMG (grupo de alta mobilidade) de ligação ao DNA. No entanto, mutações no gene SRY não são frequentes, especialmente nos casos de disgenesia gonadal parcial em indivíduos 46,XY. São atualmente conhecidos vários genes que participam do processo de diferenciação gonadal, tanto para o desenvolvimento testicular quanto para o ovariano. Além disso, pequenas deleções heterozigotas em 9p podem causar disgenesia gonadal XY completa ou parcial, sem outros sintomas associados. O gene DMRT1 humano, que está localizado em 9p24.3, é expresso no testículo e ovário no período fetal e tem sido considerado um dos genes autossômicos envolvido na etiologia das disgenesias gonadais. Neste trabalho, descrevemos a inserção de um nucleotídeo em 3'UTR do gene DMRT1 em um paciente 46,XY com disgenesia gonadal parcial. A mutação 3'UTR+11insT está localizada dentro de um motivo conservado importante para a estabilização do mRNA.

Multiple endocrine neoplasia type 1 (MEN1) is an autosomal dominant hereditary cancer syndrome characterized mostly by parathyroid, enteropancreatic, and anterior pituitary tumors. We present a case of an 8-year-old boy referred because of hypoglycemic attacks. His diagnosis was pancreatic insulinoma. Paternal grandmother died due to repeated gastroduodenal ulcerations and a paternal aunt presented similar manifestations. At a first evaluation, the father presented only gastric ulceration but subsequently developed hyperparathyroidism and lung carcinoid tumor. During almost 15 years of follow-up, three brothers and the index case presented hyperparathyroidism and hyperprolactinemia. Molecular study showed a G to A substitution in intron 4, at nine nucleotides upstream of the splicing acceptor site, causing a splicing mutation. All affected members of the family have the same mutation. Paternal grandmother and aunt were not studied and the mother does not carry any mutation. MEN1 is a rare condition that requires permanent medical assistance. Early clinical and genetic identification of affected individuals is essential for their own surveillance and also for genetic counseling. A neoplasia endócrina múltipla tipo 1 (NEM1) é uma doença hereditária autossômica dominante, caracterizada principalmente por tumores de paratireoide, enteropancreáticos e adeno-hipofisários. Apresentamos o caso de um menino com 8 anos encaminhado por crises de hipoglicemia. Seu diagnóstico foi insulinoma pancreático. Sua avó paterna faleceu por úlceras gastroduodenais de repetição e a tia paterna tinha as mesmas manifestações. Na primeira avaliação, o pai apresentou apenas úlcera gástrica, porém com a evolução desenvolveu hiperparatireoidismo e tumor carcinoide pulmonar. Durante cerca de 15 anos de seguimento, os três irmãos e o caso índice desenvolveram hiperparatireoidismo e hiperprolactinemia. O estudo molecular mostrou a substituição G por A no intron 4, a nove nucleotídeos do sítio aceptor de splicing, criando um novo sítio de splicing. Todos os membros da família afetados e estudados tinham a mesma mutação. A NEM1 é uma condição rara que requer assistência médica permanente. As identificações clínicas e genéticas precoces são essenciais para o tratamento e aconselhamento genético.

X-linked adrenoleukodystrophy (X-ALD) is an inherited disease with clinical heterogeneity varying from presymptomatic individuals to rapidly progressive cerebral ALD forms. This disease is characterized by increased concentration of very long chain fatty acids (VLCFAs) in plasma and in adrenal, testicular and nervous tissues. Affected individuals can be classified in different clinical settings, according to phenotypic expression and age at onset of initial symptoms. Molecular defects in X-ALD individuals usually result from ABCD1 gene mutations. In the present report we describe clinical data and the ABCD1 gene study in two boys affected with the childhood cerebral form that presented with different symptomatic manifestations at diagnosis. In addition, their maternal grandfather had been diagnosed with Addison's disease indicating phenotypic variation for X-ALD within this family. The mutation p.Trp132Ter was identified in both male patients; additionally, three females, out of eleven family members, were found to be heterozygous after screening for this mutation. In the present report, the molecular analysis was especially important since one of the heterozygous females was in first stages of pregnancy. Therefore, depending on the fetus outcome, if male and p.Trp132Ter carrier, storage of the umbilical cord blood should be recommended as hematopoietic stem cell transplantation could be considered as an option for treatment in the future. A adrenoleucodistrofia é uma doença genética com padrão de herança ligado ao X (X-ALD) que apresenta heterogeneidade clínica e varia desde a forma infantil cerebral severa até casos de indivíduos pré-sintomáticos. Essa doença é caracterizada pelo acúmulo de ácidos graxos de cadeia muito longa (VLCFA) no plasma, nas adrenais, nos testículos e no sistema nervoso. Indivíduos afetados podem apresentar diferentes formas clínicas, as quais são classificadas de acordo com a expressão fenotípica e a idade de aparecimento dos sintomas iniciais. Alterações moleculares em indivíduos com X-ALD são geralmente mutações no gene ABCD1. No presente trabalho, descrevemos os dados clínicos e a investigação molecular do gene ABCD1 em uma família com duas crianças do sexo masculino afetadas com a forma infantil cerebral, que apresentaram diferenças nas primeiras manifestações sintomáticas para o diagnóstico. Além disso, houve referência ao avô materno diagnosticado com doença de Addison's, indicando a variabilidade fenotípica da X-ALD nessa família. A análise molecular indicou a mutação p.Trp132Ter nos dois pacientes masculinos, e três indivíduos do sexo feminino, entre os onze estudados, mostraram-se heterozigotos para mutação. O conhecimento molecular descrito no presente relato adquiriu maior importância uma vez que uma das portadoras da mutação apresentou-se nos primeiros estágios de gestação. Assim, poderá ser oferecida a possibilidade de armazenamento de sangue de cordão umbilical para que se possa considerar, no futuro, o transplante de células-tronco hematopoiéticas como forma de tratamento, caso a criança seja do sexo masculino e afetada.

In 2004, Costa-Santos and cols. reported 24 patients from 19 Brazilian families with 17α-hydroxylase deficiency and showed that p.W406R and p.R362C corresponded to 50% and 32% of CYP17A1 mutant alleles, respectively. The present report describes clinical and molecular data of six patients from three inbred Brazilian families with 17α-hydroxlyse deficiency. All patients had hypogonadism, amenorrhea and hypertension at diagnosis. Two sisters were found to be 46,XY with both gonads palpable in the inguinal region. All patients presented hypergonadotrophic hypogonadism, with high levels of ACTH (> 104 ng/mL), suppressed plasmatic renin activity, low levels of potassium (< 2.8 mEq/L) and elevated progesterone levels (> 4.4 ng/mL). Three of them, including two sisters, were homozygous for p.W406R mutation and the other three (two sisters and one cousin) were homozygous for p.R362C. The finding of p.W406R and p.R362C in the CYP17A1 gene here reported in additional families, confirms them as the most frequent mutations causing complete combined 17α-hydroxylase/17,20-lyase deficiency in Brazilian patients. Em 2004, segundo Costa-Santos e cols., p.W406R e p.R362C correspondiam a 50% e 32% dos alelos mutantes do gene CYP17A1, respectivamente, em 24 pacientes de 19 famílias brasileiras com deficiência da 17α-hidroxilase. Apresentamos os dados clνnicos e moleculares de seis pacientes de três famílias consanguíneas brasileiras com deficiência da 17α-hidroxilase. Todas as pacientes apresentavam hipogonadismo, amenorreia e hipertensão ao diagnóstico. Duas irmãs tinham cariótipo 46,XY, ambas com gônadas palpáveis na região inguinal. Todas tinham hipogonadismo hipergonadotrófico, com nível aumentado de ACTH (> 104 ng/mL), atividade de renina plasmática suprimida, baixos níveis de potássio (< 2,8 mEq/L) e progesterona aumentada (> 4,4 ng/mL). Três delas, incluindo duas irmãs, apresentaram homozigose para a mutação p.W406R e as outras três (duas irmãs e uma prima) foram homozigotas para a mutação p.R362C. A recorrência das mutações p.W406R e p.R362C no gene CYP17A1 aqui relatada em famílias adicionais confirma que essas são as mais frequentes causadoras do fenótipo completo da deficiência combinada de 17α-hidroxilase/17,20-liase em pacientes brasileiros.

O estudo teve como objetivo verificar o efeito da atividade física programada sobre os testes de aptidão física em escolares adolescentes durante um ano de período letivo. Amostra foi composta de 383 alunos, divididos em dois grupos: caso, com 186 (96 meninos e 90 meninas) e controle, com 197 (108 meninos e 89 meninas), com idade entre 10 e 15 anos. Trata-se de estudo de intervenção com pré e pós-teste, com um ano de duração, no qual o grupo caso foi submetido à atividade física programada e o grupo controle com aulas convencionais de educação física escolar. A aptidão física foi avaliada por testes de sentar-e-alcançar (flexibilidade), resistência muscular (flexão e extensão do cotovelo) e aptidão cardiorrespiratória (corrida e caminhada de nove minutos). O desempenho motor verificado nos três testes analisados (flexibilidade, força e resistência) não apresentou melhora significativa quando envolveu a variável tempo do pré para o pós-teste, em ambos os grupos, mas quando comparado (tipo) o grupo caso (intervenção) com o controle, mostrou diferença significativa com melhora nos testes de força e resistência em ambos os sexos para o caso. Os meninos de ambos os grupos também mostraram predomínio nos testes de força e resistência. De uma forma geral, os valores foram maiores nos meninos em relação às meninas para resistência muscular e aptidão cardiorrespiratória e menor para flexibilidade, e todos aumentaram no pós-teste em relação ao pré-teste e foram maiores no grupo caso em relação ao controle. Em relação à flexibilidade, foi observada diferença estatisticamente significativa apenas entre os gêneros. Em relação à resistência muscular, após ajuste para idade e nível socioeconômico, foi observada diferença estatisticamente significativa entre os gêneros e entre os tipos. Em relação à aptidão cardiorrespiratória, foi observada diferença estatisticamente significativa entre os gêneros e entre os tipos. The objective of this study was to determine the influence of programmed physical activity on the physical fitness of adolescent students over one school year. The sample consisted of 383 students (age range: 10 to 14 years) divided into two groups: 186 cases (96 boys and 90 girls) and 197 controls (108 boys and 89 girls). An intervention study with pre- and post-tests was conducted, in which the intervention group was submitted to programmed physical activity, while the control group underwent conventional classes of school physical education. Physical fitness was assessed by sit-and-reach (flexibility), muscle endurance (elbow flexion and extension) and aerobic endurance (run/walk, 9 min) tests. Motor performance observed in the three tests (flexibility, strength and endurance) did not improve from pre-test to post-test in either group, but comparison of the intervention and control groups showed significant improvement in the strength and endurance tests for both genders in the intervention group. Boys of the two groups also showed dominance in the strength and endurance tests. In general, higher muscle strength and cardiorespiratory fitness and lower flexibility were observed for boys when compared to girls, and all parameters increased in the post-test and were higher in the intervention group compared to control A significant difference in flexibility was only observed between genders. With respect to muscle strength, a significant difference was observed between genders and between the intervention and control group after adjustment for age and socioeconomic level. Cardiorespiratory fitness differed significantly between genders and between the intervention and control group.

The objective of this study was to describe the change in diagnosis and prognosis of a child with testicular dysgenesis and 46,XY karyotype after detection of a 45,X cell line and to discuss the difficulties caused by the terms mixed gonadal dysgenesis (MGD) and XY partial gonadal dysgenesis (XYPGD). One case was reported including clinical and laboratory findings of a child of 41-day-old infant with 1.3-cm phallus, penoscrotal hypospadias and left prepubertal testis. Karyotype 46,XY (16 cells), normal hormone levels. Right streak gonad, epididymis and müllerian remnants were removed; initial diagnosis was XYPGD. Persistent growth retardation led to further cytogenetic analysis (50 cells) and detection of a 45,X cell line. Detection of a 45,X lineage changed both the diagnosis to MGD and also the prognosis.The number of cells analyzed in karyotyping is critical. Use of MGD and XYPGD to designate both a histological picture and a syndromic diagnosis, results in lack of emphasis on clinical differences between 46,XY and 45,X/46,XY subjects. O objetivo deste trabalho foi descrever a mudança no diagnóstico e no prognóstico de criança com disgenesia testicular e cariótipo 46,XY após detecção de linhagem 45,X e discutir as dificuldades causadas pelas denominações disgenesia gonadal mista (DGM) e disgenesia gonadal parcial XY (DGPXY). Relatou-se um caso incluindo achados clínicos e laboratoriais de uma criança de 41 dias com falo de 1,3 cm, hipospadia penoescrotal e testículo pré-puberal à esquerda. Cariótipo 46,XY (16 células), níveis hormonais normais. Gônada direita (disgenética), epidídimo e remanescentes müllerianos foram removidos; o diagnóstico inicial foi DGPXY. Retardo de crescimento persistente levou à ampliação da análise citogenética (50 células) e à detecção de linhagem 45,X. A detecção de linhagem 45,X modificou o diagnóstico para DGM e também o prognóstico. No cariótipo, o número de células analisadas é crítico. O uso de DGM e DGPXY para designar tanto quadro histológico quanto diagnóstico clínico resulta em falta de ênfase nas diferenças clínicas entre indivíduos 46,XY e 45,X/46,XY.

OBJETIVO: Verificar o efeito da atividade física programada na escola sobre a composição corporal em escolares adolescentes durante 1 ano letivo. MÉTODOS: Amostra foi composta por 383 alunos, divididos em dois grupos: caso com 186 (96 meninos e 90 meninas) e controle com 197 (108 meninos e 89 meninas), com idade entre 10 e 15 anos. Trata-se de estudo de intervenção com pré e pós-teste, no qual o grupo caso foi submetido a atividade física programada e o grupo-controle a aulas convencionais de educação física escolar. A composição corporal foi avaliada por medidas antropométricas e cálculos de índice de massa corporal, percentual de gordura e massas gorda e magra. RESULTADOS: O grupo caso apresentou estabilidade na prega cutânea subescapular, índice de massa corporal, percentual de gordura e na massa gorda; redução significativa na prega cutânea triciptal, perímetro do abdome nas meninas e aumento significativo dos perímetros do braço, cintura e panturrilha e da massa magra. No grupo-controle houve aumento do índice de massa corporal, prega cutânea triciptal, perímetro do abdome e da massa gorda nas meninas. O grupo caso apresentou diminuição significativa na proporção de sobrepesos e obesos no pós em relação ao pré-teste, o mesmo não ocorrendo no grupo-controle. CONCLUSÃO: A atividade física programada resultou em melhoria e manutenção nas variáveis da composição corporal e redução da frequência de sobrepeso e obesidade no grupo que sofreu intervenção. OBJECTIVE: To verify the influence of programmed physical activity on body composition among adolescent students during 1 school year. METHODS: The sample included 383 students (age range: 10 to 15years) separated into two groups: 186 cases (96 male and 90 female) and 197 controls (108 male and 89 female). This was an intervention study with pre- and post-test assessments in which interventions consisted of programmed physical activity; the control group had conventional school physical education. Body composition was assessed by anthropometric measurements, body mass index (BMI), body fat percentage and fat and lean body mass. RESULTS: In the case group, subscapular skinfold thickness, BMI, body fat percentage and fat body mass remained stable; there were significant reductions in tricipital skinfold thickness and in abdominal perimeter among girls and significant increases in arm, waist and calf perimeters and in lean body mass. In the control group, there were significant increases in BMI, tricipital skinfold thickness, abdominal perimeter and fat body mass among girls. At post-test, overweight and obesity significantly decreased among case group subjects, but not among controls. CONCLUSION: Programmed physical activity resulted in improvement or maintenance of body composition parameters and in reduction of overweight and obesity in the intervention group.

AIM: To present phenotypic variability of WT1-related disorders. METHODS: Description of clinical and genetic features of five 46,XY patients with WT1 anomalies. RESULTS: Patient 1: newborn with genital ambiguity; he developed Wilms tumor (WT) and chronic renal disease and died at the age of 10 months; the heterozygous 1186G>A mutation compatible with Denys-Drash syndrome was detected in this child. Patients 2 and 3: adolescents with chronic renal disease, primary amenorrhea and hypergonadotrophic hypogonadism; patient 2 had a gonadoblastoma. The heterozygous IVS9+4, C>T mutation, compatible with Frasier syndrome was detected. Patient 4: 9-year-old boy with aniridia, genital ambiguity, dysmorphisms and mental deficiency; a heterozygous 11p deletion, compatible with WAGR syndrome was detected. Patient 5: 2 months old, same diagnosis of patient 4; he developed WT at the age of 8 months. CONCLUSIONS: Constitutional abnormalities of WT1 cause gonadal and renal anomalies and predisposition to neoplasia and must be investigated in patients with ambiguous genitalia, chronic renal disease and(or) Wilms tumors; primary amenorrhea with chronic renal disease; and aniridia, genital ambiguity and dysmorphisms. OBJETIVO: Descrever a variabilidade fenotípica das anomalias relacionadas ao WT1. MÉTODOS: Descrição das características clínicas e genéticas de cinco pacientes 46,XY com anomalias no WT1. RESULTADOS: Paciente 1: Recém-nascido com ambigüidade genital desenvolveu tumor de Wilms (TW) e insuficiência renal crônica (IRC), com óbito aos 10 meses. Detectada a mutação 1186G>A em heterozigose, compatível com síndrome de Denys-Drash. Pacientes 2 e 3: Adolescentes com IRC, amenorréia primária e hipogonadismo hipergonadotrófico; a paciente 2 apresentava gonadoblastoma. Ambas apresentavam mutação IVS9+4, C>T em heterozigose, característica da síndrome de Frasier. Paciente 4: Idade 9 anos, aniridia, ambigüidade genital, dismorfismos e deficiência mental; deleção 11p, compatível com síndrome WAGR foi encontrada em heterozigose. Paciente 5: Dois meses, mesmo diagnóstico do paciente 4, desenvolveu TW aos 8 meses. CONCLUSÕES: Alterações constitucionais do WT1 determinam anomalias gonadais, renais e predisposição a neoplasias; devem ser pesquisadas em casos de ambigüidade genital associada a IRC e(ou) TW; de amenorréia primária com IRC; e aniridia, ambigüidade genital e dismorfismos.

OBJETIVO: Avaliar o estado nutricional em escolares da rede municipal de ensino da cidade de Porto Velho (RO) em relação à idade, ao sexo e às características sócio-demográficas em uma população de baixo nível socioeconômico. MÉTODOS: A amostra constituiu-se de 1 057 escolares com baixo nível socioeconômico, de ambos os sexos, entre 7 e 10 anos, selecionada por meio de amostragem aleatória proporcional por setor e, divisão intencional por conglomerado de turma. Foi utilizado um questionário para obter informações sobre os dados sóciodemográficos. Os escolares foram classificados segundo o peso para a estatura em desnutridos, eutróficos, sobrepesos e obesos; e segundo o peso para a estatura e estatura para idade em eutróficos, desnutridos agudos, pregressos ou crônicos. RESULTADOS: Não foi observada diferença significativa entre os sexos para peso ou estatura. Em relação apenas ao peso para a estatura, 86,0% foram classificados com eutróficos, 4,0% desnutridos, 7,0% sobrepesos e 3,0% obesos. Em relação aos índices de peso para estatura e estatura para idade, 89,0% foram classificados em eutróficos, 2,5% desnutridos agudos, 7,0% desnutridos pregressos e 1,5% desnutridos crônicos. CONCLUSÃO: Os escolares do ensino fundamental da rede pública municipal de Porto Velho (RO) com baixo nível socioeconômico apresentaram baixa prevalência de desnutrição ou sobrepeso/obesidade. Estes dados encontrados podem ser comparados aos de outras cidades e regiões do Brasil. OBJECTIVE: This study aimed to determine the nutritional status of schoolchildren in public schools in Porto Velho, Rondônia, Brazil, in relation to age, gender and sociodemographic features in a population of low socioeconomic status. METHODS: The sample comprised 1,057 schoolchildren of low socioeconomic status of both genders, aging from 7 to 10 years, selected by stratified random sampling by sector and purposeful sampling by group. Sociodemographic data were obtained with a questionnaire. The schoolchildren were classified according to their weight-for-height into malnourished, well nourished and overweight/obese and according to their weight-for-height and height-for-age into well nourished, severely malnourished, chronically malnourished or formerly malnourished. RESULTS: There was no significant difference between males and females for weight and height. In relation to weight-for-height alone, 86.0% were normal, 4.0% were malnourished, 7.0% were overweight and 3.0% were obese. In relation to weight-for-height and height-for-age, 89% were normal, 2.5% were acutely malnourished, 7.0% were formerly malnourished and 1.5% were chronically malnourished. CONCLUSION: The prevalence of malnutrition or overweight/obesity was low among elementary schoolchildren of low socioeconomic status in public schools of Porto Velho, Rondônia, Brazil. These data can be compared with data from other Brazilian cities and regions.

O seguimento de pacientes com síndrome de Turner (ST) freqüentemente revela alterações transitórias, recorrentes e assintomáticas de TSH e/ou hormônios tireóideos (HT). Neste trabalho foram avaliadas estrutura e função da tireóide em portadoras da ST com história de alterações prévias desses hormônios. A casuística incluiu 24 pacientes, 17 com menos de 20 anos, avaliadas laboratorialmente pelas concentrações séricas de TSH, T4 livre, T3 e anticorpos anti-TPO e anti-Tg, e morfologicamente por ultra-sonografia (USG) e cintilografia com pertecnetato-99mTc. Havia alterações de TSH e/ou HT em 13 casos, e em 23 havia alterações USG compatíveis com doença crônica da tireóide, particularmente tireomegalia (estabelecida de acordo com o volume esperado para a estatura) e heterogeneidade do parênquima. A captação foi normal em 21 casos e a distribuição do radiofármaco, homogênea em 22. As alterações hormonais foram independentes da idade, do tempo de evolução, da presença de anticorpos, do número de anomalias USG e de alterações cintilográficas. Pacientes maiores de 20 anos apresentaram maior freqüência de anticorpos e de hipoecogenicidade do parênquima, e houve correlação positiva entre o volume tireóideo e o tempo de evolução, indicando comprometimento progressivo da glândula. Esses resultados sugerem que as alterações encontradas decorram de doença tireóidea auto-imune crônica. Patients with Turner syndrome (TS) frequently exhibit transient, recurrent and asymptomatic variations of TSH and/or thyroid hormones (TH). This work was carried out to evaluate thyroid function and structure in patients with TS who had had such variations in hormone concentrations. Our sample comprised 24 patients, 17 less than 20-years old. Evaluation included serum levels of TSH, free T4, total T3, TPO and Tg autoantibodies, thyroid ultrasound (US) and scintigraphy with 99mTc-pertechnetate. Thirteen patients had abnormal TSH and/or TH levels; 23 exhibited US features compatible with chronic thyroid disorder, particularly thyromegaly (established according to volume expected for stature) and heterogeneous echogenicity. Uptake was normal in 21 cases and tracer distribution was homogeneous in 22. The finding of abnormal hormone concentrations was independent of age, length of time since the first similar finding, thyroid autoantibodies, number of abnormalities at US and abnormal scintigraphic findings. Patients aged more than 20 years had higher frequency of thyroid antibodies and heterogeneous echogenicity, and thyroid volume was significantly correlated to length of time since detection of the first hormone variation, indicating progressive thyroid disease. These results suggest that subclinical thyroid dysfunction in TS is due to chronic autoimmune thyroid disease.

Adrenal involvement by Paracoccidioides brasiliensis was described at necropsies and in many clinical studies, but only in adults. Therefore, the aim of this study was to evaluate adrenal function in children with paracoccidioidomycosis. Twenty-three children with the systemic form of paracoccidioidomycosis were evaluated and divided in two Groups: Group A (n = 8) included children before treatment and Group B (n = 15) children after the end of treatment. Plasma cortisol (basal and after ACTH test), ACTH, renin activity, aldosterone, sodium and potassium were measured. They were within normal range in all cases, except for renin activity and aldosterone, which were elevated in some cases. Group A patients showed basal and post-ACTH cortisol levels significantly greater than Group B patients. The results showed that adrenal function was not compromised in these children with paracoccidioidomycosis. O acometimento adrenal pelo Paracoccidioides brasiliensis é descrito em necropsias e em estudos clínicos, mas apenas em adultos. Portanto, o objetivo deste estudo foi avaliar a função adrenal em crianças com paracoccidioidomicose. Vinte e três crianças com forma sistêmica de paracoccidioidomicose foram avaliadas e divididas em dois grupos: Grupo A (n = 8) pacientes antes de iniciar o tratamento e Grupo B (n = 15) pacientes após o termino do tratamento. Dosagens plasmáticas de cortisol (basal e após teste com ACTH), ACTH, atividade de renina, aldosterona, sódio e potássio foram realizadas. Estas dosagens foram normais em todos os casos, com exceção da atividade da renina e da aldosterona que foram elevadas em alguns casos. Os pacientes do Grupo A mostraram valores de cortisol basal e após-ACTH significativamente maiores que os do Grupo B. Os resultados demonstraram que a função adrenal não foi comprometida neste grupo de crianças com paracoccidioidomicose.

The objective this study was to verify the schoolgirl sexual maturation in a public school from Rio Branco (AC). It was a longitudinal study for 3 consecutive years (2003 to 2005), with the same group of girls who studied at grades 5 or 6 in a school from Rio Branco (AC). The method proposed by Marshall and Tanner to evaluate beast and pubic. The age of menarche was obtained prospectively. The classification of sexual maturation was done according to Vitalle et al.. The nutritional status was evaluated by bone mass index according to NCHS data and the socioeconomic evaluation, by ABEP (2003) patterns. From 44 schoolgirls who completed the study, 39 (89%) were from middle-lower or lower socio-economic level; and 30 (68%) in 2003 and 21 (48%) were malnourished. In 2003, 35 (79%) had initiated the puberty and only 8 (18%) have presented menarche; while in 2005, 39 (89%) presented advanced or complete stages of sexual development and 37 (84%) menarche. The mean age of menarche was 12.7 + 0.7 years. This sample of girls of a public school from North Region of Brazil showed similar data of menarche in comparison from other national and international studies. The advanced puberty observed in some recent studies was not observed in this sample, probably due to the socio-economic level and the nutritional status without obesity. O objetivo deste estudo foi verificar a maturação sexual de escolares do sexo feminino em uma escola pública da cidade de Rio Branco (AC). Trata-se de um estudo longitudinal por três anos consecutivos (2003 a 2005), com um grupo de meninas que em 2003 cursavam a 5ª ou 6ª série do Ensino Fundamental II em Rio Branco (AC). O método proposto por Marshall e Tanner para avaliação de mamas e de pêlos pubianos. A idade da menarca foi obtida prospectivamente. A classificação da maturação sexual foi feita de acordo com Vitalle et al. (inserir data). O estado nutricional foi avaliado pelo índice de massa corporal de acordo com o critério do NCHS e a avaliação socioeconômica pelas normas da ABEP (2003). Das 44 meninas que completaram o estudo, 39 (89%) pertenciam ao nível socioeconômico médio-baixo ou pobre; e 30 (68%) em 2003 e 21 (48%) em 2005 estavam desnutridas. Em 2003, 35 (79%) estavam em estadio inicial de desenvolvimento puberal e apenas 8 (18%) haviam apresentado menarca, enquanto que em 2005, 39 (89%) estavam em estadio avançado ou completo de desenvolvimento e 37 (84%) apresentavam maturação. A média da idade da menarca foi de 12,7 + 0,7 anos. Conclui-se que esta amostra de meninas de escola pública da região Norte do Brasil, a idade da menarca foi compatível com os dados descritos na literatura nacional e internacional. A antecipação da puberdade descrita em alguns estudos mais recentes, não foi observada, provavelmente, devido ao nível socioeconômico baixo e ao padrão nutricional com a ausência de obesidade.

OBJETIVOS: Avaliar o crescimento e a composição corporal de diabéticos tipo 1, pré-púberes, em relação à idade de início e tempo da doença, sexo, dose de insulina e hemoglobina glicada média. PACIENTES E MÉTODOS: Foram incluídas no estudo 59 crianças diabéticas (30 M; 29 F), entre 1,2 e 11,5 anos, e 67 controles (36 M; 31 F), entre 1,2 e 11,7 anos. Peso, altura, IMC, perímetro braquial, pregas cutâneas e áreas de massa gorda e muscular braquial foram avaliados e transformados em escore z. RESULTADOS: Verificou-se que entre os diabéticos a média de escore z de altura foi -0,13 (± 0,97), enquanto no grupo controle foi de 0,28 (± 0,86) (p= 0,013). A diferença entre os escores de altura inicial e atual mostrou perda estatural (p< 0,001) e a análise multivariada demonstrou associação com tempo de doença. Também observou-se diferença na área de gordura braquial (p< 0,001). As médias de escore z de peso, IMC, soma de 3 dobras e área muscular braquial não diferiram entre os grupos. CONCLUSÕES: As crianças diabéticas apresentaram perda de estatura durante o período de acompanhamento e eram significativamente mais baixas que os controles, embora suas alturas ainda estivessem dentro dos padrões de normalidade. Também mostraram área de gordura braquial aumentada em relação aos controles. OBJECTIVE: To evaluate the growth and body composition of pre-pubertal diabetic children, and to check for influence of the age of diabetes onset and length, sex, insulin requirement and glycosylated hemoglobin. PATIENTS AND METHODS: 59 diabetic children (39 M; 29 F), age 1.2-11.5 years, and 67 controls (36 M; 31 F), age 1.2-11.7 years were included. Weight, height, body mass index (BMI), arm circumference, skin folds, fat mass and muscle areas were evaluated and transformed into standard deviation scores (SDS). RESULTS: Among the diabetic children the mean height SDS was -0.13 (± 0.97) while in the control group it was 0.28 (± 0.86) (p= 0.013). The difference between the first and the current height SDS showed that the height SDS decreased significantly (p< 0.001) and multiple regression analysis indicated correlation with the duration of the disease. The mean arm fat SDS also revealed difference (p< 0.001). The means for weight, BMI, addition of 3 skinfolds and muscle mass did not demonstrate difference between the groups. CONCLUSIONS: The diabetic children showed reduction of height SDS during the period studied and they were significantly shorter than the controls, even though their statures were within the population standards. The arm fat area also showed to be increased in relation with the controls.

OBJETIVO: O diabetes mellitus tipo 1 (DM1) é a mais importante doença endócrino-metabólica crônica entre crianças e adolescentes, podendo ser causa de atraso do crescimento e da puberdade. Avaliamos a influência do DM1 no estirão de crescimento e na puberdade de indivíduos com doença iniciada antes ou no começo desta fase. CASUÍSTICA E MÉTODOS: Foram avaliados retrospectivamente os prontuários de 40 pacientes (25 meninas), que apresentavam altura final, com coleta dos dados para o cálculo do tempo total de doença, os escores de DP da altura e do peso a cada consulta, a altura-alvo parental, as velocidades de crescimento (VC) anuais, o pico do estirão de crescimento, a duração da puberdade, o ganho total de crescimento durante o estirão puberal e a hemoglobina glicada (HbG). RESULTADOS: Em relação à adequação da altura final para a altura-alvo parental, 37 pacientes ficaram dentro da previsão e 3 ficaram abaixo do limite inferior. Não houve associação entre as variáveis analisadas com exceção da VC no pico do estirão, quando esta foi menor ou igual a 6cm/ano. A idade de início do DM1 e do pico do estirão puberal coincidiram com a faixa etária descrita na literatura; não foi observado atraso na idade de início da puberdade. No entanto, o ganho de altura durante a puberdade foi menor que o descrito na literatura, o mesmo ocorrendo em relação ao pico de VC puberal. De acordo com os valores da HbG, todos os pacientes avaliados apresentaram um controle crônico ruim do DM1. CONCLUSÕES: Neste grupo de DM1 com controle inadequado da doença, houve um prejuízo na altura final em relação à altura do início do DM1, provavelmente causado por uma baixa VC durante a puberdade, que, no entanto, não teve influência na altura final em relação ao alvo parental. OBJECTIVE: Type 1 diabetes mellitus (DM1), the most important chronic endocrine-metabolic disease in children and adolescents, may lead to delayed growth and puberty. In this study we analyzed the influence of DM1 on growth spurt and puberty of patients whose onset of the disease was before or at the beginning of this phase. PATIENTS AND METHODS: Data from 40 patients, 25 females, who had attained final height were retrospectively obtained, including duration of disease, patient's height and weight SDS at each consultation, parental target height, yearly growth velocities (GV), peak of growth spurt, duration of puberty, magnitude of growth spurt and glycated hemoglobin (HbA1C) levels. RESULTS: 37 patients had an adequate final height to parental target height, and only 3 were below the lower limit. There was no significant association among the variables and the appropriate final height to the target height, except for GV on growth spurt, when it was lower than or equal to 6cm/year. The age of onset of DM1 and the age of peak of growth spurt was similar to previously data reported; regarding the age of the onset of puberty, there was no delay. However, the magnitude of the growth spurt and the peak of GV were lower. According to HbA1C levels, all patients exhibited a bad chronic control of DM1. CONCLUSIONS: In this group of inadequately controlled patients, the final height was lower than expected when compared to the height at onset of DM1, probably due to a slow GV during puberty, which however had no influence on the final height to parental target height.

OBJETIVO: Apresentar dados de pacientes com Hermafroditismo Verdadeiro (HV) diagnosticados e acompanhados em dois serviços de referência para o diagnóstico e tratamento de anomalias da diferenciação sexual no Estado de São Paulo. CASUÍSTICA: Foi composta de 26 HV acompanhados no Instituto da Criança - USP entre 1975 e 2004 e outros 10 casos acompanhados no GIEDDS - UNICAMP entre 1989 e 2004. MÉTODOS: Foram analisados retrospectivamente os dados clínicos e anatomopatológicos, bem como a definição do sexo de criação. RESULTADOS: A queixa prevalente foi ambigüidade genital (34 em 36 casos) e a mediana da idade à primeira consulta foi de 20 meses. A opção para o sexo feminino foi tomada em 55,6% dos pacientes. Prevaleceu o cariótipo 46,XX (47,2%) seguido por mosaicos (27,8%). O SRY foi negativo em todos os pacientes com cariótipo 46,XX e a gônada mais freqüentemente encontrada foi ovotéstis (OT-47%), seguida de ovário (OV-27%) e testículo (TT - 24%). As associações mais freqüentes foram OV + TT (30,5%), OT + OT (22,2%) e OT + OV (22,2%). A opção do sexo de criação independeu do cariótipo, do tamanho do falo e da dosagem sérica de testosterona, mas foi influenciada pelo posicionamento do meato uretral, que, quando não era perineal (11 casos), levou todos à opção masculina. Em cinco pacientes, todas 46,XX com opção para sexo feminino, pôde-se preservar a porção ovariana do ovotéstis. CONCLUSÃO: O HV continua a desafiar clínicos e investigadores pelo fato de, na maioria dos casos, o cariótipo ser 46,XX, o SRY estar ausente e, ainda assim, haver o desenvolvimento de tecidos ovariano e testicular. A opção preferencial é para o sexo feminino, e a tentativa de preservar o componente ovariano do ovotéstis pode permitir puberdade espontânea e fertilidade. AIM: to show data on true hermaphrodites (TH) followed in two reference centers in the state of São Paulo. PATIENTS: Twenty-six TH patients have been followed from 1975 to 2004 in the Pediatric Endocrinology Unit, São Paulo University Medical School, and the other 10 patients from 1989 to 2004 in the GIEDDS - UNICAMP. METHODS: Clinical and pathological data were retrospectively analyzed, as well as the decision about the sex of rearing. RESULTS: genital ambiguity was the most frequent complaint (34/36 cases) and the age at the first appointment was 20 mo (median). In 55.6% of the patients the female option was taken. The most frequent karyotype was 46,XX (47.2%), followed by mosaicisms (27.8%). SRY was negative in all 46,XX TH patients and the prevalent gonad was ovotestis (OT-47%), followed by ovary (OV-27%) and testis (TT-24%). The prevalent gonadal associations were OV+TT (30.5%), OT+OT (22.2%) and OT+OV (22.2%). The decision about the sex of rearing did not depend on karyotype, phallus size, and testosterone levels. However, the location of the urethral meatus was taken into account in this decision: 20 (80%) patients out of the 25 with perineal urethra were reared as females while all 11 with "non-perineal" urethra were reared as males. In five 46,XX patients reared as females it was possible to maintain the ovarian portion of the ovotestis. CONCLUSION: TH keeps on defying clinicians and investigators since most patients are 46,XX and SRY-negative, and yet they are able to develop testicular and ovarian tissues. The female sex of rearing seems to be the most adequate, and it is important to try to preserve, in these cases, the ovarian portion of the ovotestis, allowing the patient to have normal puberty and fertility.
