Exosomes are extracellular vesicles of 40-150 nm in size, which are widely distributed in various body fluids. Gastric cancer is the most common digestive tract malignancy in China. Exosomes are involved in the malignant evolution of gastric cancer in multiple ways including growth, progression, metastasis and microenvironment of tumor. Exosomes might be a non-invasive biological marker for early diagnosis of gastric cancer and have also shown potential as drug carriers in the treatment of gastric cancer. This article reviews recent research on exosomes in the pathogenesis and treatment of gastric cancer.

The video analysis and identification technology of coal mine safety production is the core technical support to ensure the intelligent construction of our country's coal mines and the high-quality development of the coal industry. In order to carry out real-time monitoring and early warning for potential safety hazards in coal mines, the key technologies of video AI (Artificial Intelligence) identification have become the research hotspot in the field of safety production in coal mines. In this paper, the development status of safety monitoring in the process of intelligent construction of coal mines are first expounded. Then, the problems of low efficiency, slow response and poor effect of the current mine video monitoring and safety hazard identification as well as early warning system are concluded. Combined with advanced technologies such as computer vision, edge computing, big data processing, cloud services, and intelligent terminals, the top-level design of AI recognition for coal mine safety production video is carried out. Furthermore, the “cloud-edge-terminal” collaborative computing system architecture of “human-machine-environment” global video AI perception in coal mines is also proposed, followed with a video recognition end node sensor, edge computing equipment, and video recognition scene cloud service application system constructed. By this way, the intelligent identification and early warning linkage control response mechanism are clarified, and the “cloud-edge-terminal” information interactive perception and linkage control data chain has been dredged, resulting with data sharing linkage and early warning coordination. At the same time, around the construction of the “human-machine-environment” global AI visual information intelligent perception and holographic generalized scene platform, the technical processing process of visual perception and identification and early warning of mine safety hazards has been sorted out. What’s more, the characteristic of the processing-enhancement-reconstruction-detection-recognition method are also summarized, and the mainstream direction and trend of the key technology development of coal mine safety production video AI recognition are also pointed out. Secondly, based on the application cases of representative mines such as Wangjialing Coal Mine and Baodian Coal Mine, the author demonstrates the latest progress and application effects of typical application scenarios in the process of coal mine safety production. Finally, according to the key technology characteristics of coal mine safety production video AI recognition, it is concluded that the existing coal mine safety production video AI recognition system has weak technical theory, different specifications of intelligent terminals, confusing application scenarios, poor data compatibility and linkage closed-loop ability, weak database security, inconsistent evaluation mechanism as well as imperfect application standards, etc. Subsequently, this paper pointed out that the future development direction is to strengthen the research on key technologies and theories of video AI recognition, establish and improve intelligent terminal hardware specifications and applicable systems and build a new coal mine information multi-dimensional active perception model and industrial internet application platform with unified standards, perfect mechanism, real-time interconnection, dynamic prediction, collaborative control, safety and reliability, which gradually form a high-precision intelligent perception field of holographic generalization in the whole mine, so as to realize the precise perception of the underground "human-machine-environment" global video information and the coordinated control of danger sources.

Objective To investigate the clinical efficacy of combined gonadotropin-releasing hormone agonist(GnRH-a) therapy in patients with ovarian endometrioma after conservative surgery. Methods Using a retrospective cohort study analysis, 206 patients who underwent conservative surgery for ovarian endometriosis cysts at the Department of Gynecology, Affiliated Hospital of Nanjing University of Chinese Medicine from December 2014 to December 2018 were selected and divided into 138 cases in the observation group(postoperative combined GnRH-a use for 6 months) and 68 cases in the control group(postoperative with no drug). Recurrence between two groups and the scores of dysmenorrhea, pain during in tercouse（PDI）, and chronic pelvic pain were compared between two groups at different times in 2 years after surgery. Results In the 2 years follow-up, the observation group was superior with statistical significance in the comparison of chronic pelvic pain and dysmenorrhea scores between the two groups and time points(P＜0.05). There was no significant difference in intergroup effect and time effect of sexual intercourse pain (P>0.05). The recurrence rate of patients in the observation group was lower than that of the control group in the first year after surgery(3.62% vs 11.77%, P＜0.05), while no significant difference in recurrence rate of two groups was found in the second year after surgery (P>0.05), but the 2-year cumulative recurrence rate in the observation group was lower than that in the control group (13.24% vs 25.00%, PConclusion GnRH-a can be a choice for postoperative pain management and recurrence prevention in endometriosis.

In order to reveal the influence of loading rate on the deformation and failure of cemented waste rock filling body, uniaxial compression tests of cemented waste rock filling body under five groups of loading rates were carried out, and the changes of mechanical properties, failure mode and energy dissipation were analyzed. The results show that the peak strength and elastic modulus of cemented waste rock filling body have positive linear correlation and quadratic function growth with loading rate, respectively; with the increase of loading rate, the failure mode of filling body changes from tensile splitting failure to shear failure, and the greater the loading rate is, the greater the degree of damage is; combined with the energy evolution characteristics, the cemented waste rock filling body experiences five stages: compaction, linear elasticity, stable crack growth, accelerated crack growth and post-peak strain softening attenuation; with the increase of loading rate, the increase of total strain energy and elastic strain energy of cemented waste rock filling body is more and more, the increase of dissipation energy decreases, the proportion of elastic strain energy increases, and the pre-peak plasticity decreases.

Background Considering the importance of the inflammatory status of recipients on outcomes following liver transplantation (LT), we investigated the association between C-reactive protein–to–albumin ratio (CAR) and one-year mortality following LT and compared it with other parameters reflecting patients’ underlying inflammatory status. Methods A total of 3,614 consecutive adult LT recipients were retrospectively evaluated. Prognostic parameters were analyzed using area under the receiver operating characteristic curve (AUROC) analysis, and subsequent cutoffs were derived. For survival analysis, Cox proportional hazards and Kaplan–Meier analyses were performed. Results The AUROC for CAR to predict one-year mortality after LT was 0.68 (0.65–0.72), which was the highest compared with other inflammatory parameters, with the best cutoff of 0.34. A CAR ≥ 0.34 was associated with a significantly higher one-year mortality rate (13.3% vs. 5.8 %, log-rank P < 0.001) and overall mortality rate (24.5% vs. 12.9%, log-rank P = 0.039). A CAR ≥ 0.34 was an independent predictor of one-year mortality (hazard ratio, 1.40 [1.03–1.90], P = 0.031) and overall mortality (hazard ratio 1.39 [1.13–1.71], P = 0.002) after multivariable adjustment. Conclusions Preoperative CAR (≥ 0.34) was independently associated with a higher risk of one-year and overall mortality after LT. This may suggest that CAR, a simple and readily available biomarker, maybe a practical index that may assist in the risk stratification of liver transplantation outcomes.

The Monte Carlo particle transport code program MCNP is used to establish a cobalt-60 irradiation device model and simulate the peripheral spatial dose field distribution outside the irradiation station in the irradiation room. The Cartesian coordinate system is established with the center point of the single-rack source frame as the coordinate origin. Considering the γ-rays non-self-absorption and the γ-rays self-absorption of cobalt-60 source, the dose rate distribution of the air surface at intervals of 10 cm along the coordinate axis and the variation law of dose rate on the coordinate axis are studied. The results show that the dose rate of the peripheral spatial dose field is relatively small, when the irradiation products of the irradiation room occupy all the irradiation stations. The variation law of dose rate on the coordinate axis of the single-rack source frame is more consistent with binomial fitting function. In the case of γ-rays self-absorption of cobalt-60 source, the dose rate near the coordinate axis of the single-rack source frame is obviously low, and the high-dose-rate region of the air surface moves to both sides as air surface moves away from the single-rack source frame. In the case of γ-rays non-self-absorption of cobalt-60 source, the high-dose-rate region of the air surface at the end face of the single-rack source frame is always located near the coordinate axis. The theoretical simulation calculation and analysis of MCNP has important practical guiding significance for the peripheral spatial dose field outside the irradiation station in the irradiation room of the cobalt-60 irradiation facility.

(1) Background: Liver transplantation (LT) is associated with significant hemorrhage and massive transfusions. Fibrinogen replacement has a key role in treating massive bleeding during LT and hypofibrinogenemia is treated by fibrinogen concentrate or cryoprecipitate. However, these two products are known to be associated with major thromboembolism events (MTEs). We aimed to compare the effect of fibrinogen concentrate and cryoprecipitate on MTEs in living donor LT (LDLT) recipients. (2) Methods: We analyzed 206 patients who underwent LDLT between January 2021 and March 2022. The patients were divided into two groups according to fibrinogen concentrate or cryoprecipitate use. We compared the incidence of MTEs between the two groups. In addition, we performed multiple logistic regression analyses to identify the risk factors for MTEs. (3) Results: There was no significant difference in the incidence of MTEs (16 [14.7%] vs. 14 [14.4%], p = 1.000) between the cryoprecipitate group and fibrinogen concentrate group. In the multivariate analysis, cryoprecipitate (OR 2.09, 95%CI 0.85–5.11, p = 0.107) and fibrinogen concentrate (OR 2.05, 95%CI 0.82–5.12, p = 0.126) were not significantly associated with MTEs. (4) Conclusions: there was no significant difference in the incidence of MTEs between cryoprecipitate and fibrinogen concentrate in LDLT recipients.
