- Abstract
Background: For the prevention of attacks of hereditary angioedema (HAE), the efficacy and safety of subcutaneous human C1-esterase inhibitor (C1-INH[SC]; HAEGARDA®, CSL Behring) was established in the 16-week COMPACT trial. Objective: To assess the long-term safety, occurrence of angioedema attacks, and use of rescue medication with C1-INH(SC). Methods: Open-label, randomised, parallel-arm extension of COMPACT across 11 countries. Patients with frequent angioedema attacks, either study treatment-naïve or who had completed the COMPACT trial, were randomly assigned (1:1) to 40 IU/kg or 60 IU/kg C1-INH(SC) twice per week, with conditional up-titration to optimise prophylaxis. ClinicalTrials.gov registration: NCT02316353. Results: 126 patients with a monthly attack rate of 4.3 in 3 months prior to entry in the COMPACT program were enrolled and treated for a mean of 1·5 years; 44 patients (34·9%) had >2 years exposure. Median steady-state C1-INH functional activity increased to a maximum of 73.0% with 60 IU/kg. Incidence of adverse events was low and similar in both dose groups (11·3 and 8·5 events per patient-year for 40 IU/kg and 60 IU/kg, respectively). For 40 IU/kg and 60 IU/kg, median annualised attack rates were 1·3 and 1·0, respectively and median rescue medication use was 0.2 and 0.0 times per year, respectively. Of 23 patients receiving 60 IU/kg for >2 years, 19 (83%) were attack-free during months 25–30 of treatment. Conclusion: In patients with frequent HAE attacks, long-term replacement therapy with C1-INH(SC) is safe and exhibits a substantial and sustained prophylactic effect, with the vast majority of patients becoming free from debilitating disease symptoms.

- Abstract
Chronic urticaria is a disease characterized by the appearance of weals, angioedema or both longer than 6 weeks. Degranulation of cutaneous or submucosal mast cells leads to release of mediators including histamine resulting in redness, swelling and itch. Because mast cells are widely distributed throughout the body, the question is why they are not activated systemically or does systemic activation occur without overt end organ dysfunction? We have conducted an exploratory literature search for reports that have evidence of organ-specific dysfunction in chronic urticaria that might justify prospective observational studies. This search revealed some evidence of systemic effects of chronic urticaria in cardiac, respiratory, gastrointestinal, central nervous and musculo-skeletal systems. The relevance of these findings needs to be further determined. However, they justify prospective studies in larger numbers of patients and at different stages of disease activity., NA

Autoantibodies against the alpha subunit of the high affinity immunoglobulin E (IgE) receptor may cause histamine release in patients with chronic urticaria. Chronic urticaria is a skin disorder characterized by recurrent, itchy lesions. Histamine is a substance that functions as a mediator of hypersensitivity, or an exaggerated immune response. A study examined histamine release in four patients with chronic urticaria of unknown cause. Autoantibodies against the alpha subunit of the high affinity IgE receptor were found in the peripheral blood of all four patients. Histamine release was induced by purified immunoglobulin G (IgG) from all four patients' blood. The purified IgG reacted almost completely with the alpha subunit of the high affinity IgE receptor. The antibodies against the alpha subunit may be histamine-releasing IgG autoantibodies.

Abstract In less than half a century, allergy, originally perceived as a rare disease, has become a major public health threat, today affecting the lives of more than 60 million people in Europe, and probably close to one billion worldwide, thereby heavily impacting the budgets of public health systems. More disturbingly, its prevalence and impact are on the rise, a development that has been associated with environmental and lifestyle changes accompanying the continuous process of urbanization and globalization. Therefore, there is an urgent need to prioritize and concert research efforts in the field of allergy, in order to achieve sustainable results on prevention, diagnosis and treatment of this most prevalent chronic disease of the 21st century. The European Academy of Allergy and Clinical Immunology (EAACI) is the leading professional organization in the field of allergy, promoting excellence in clinical care, education, training and basic and translational research, all with the ultimate goal of improving the health of allergic patients. The European Federation of Allergy and Airways Diseases Patients’ Associations (EFA) is a non-profit network of allergy, asthma and Chronic Obstructive Pulmonary Disorder (COPD) patients’ organizations. In support of their missions, the present EAACI Position Paper, in collaboration with EFA, highlights the most important research needs in the field of allergy to serve as key recommendations for future research funding at the national and European levels. Although allergies may involve almost every organ of the body and an array of diverse external factors act as triggers, there are several common themes that need to be prioritized in research efforts. As in many other chronic diseases, effective prevention, curative treatment and accurate, rapid diagnosis represent major unmet needs. Detailed phenotyping/endotyping stands out as widely required in order to arrange or re-categorize clinical syndromes into more coherent, uniform and treatment-responsive groups. Research efforts to unveil the basic pathophysiologic pathways and mechanisms, thus leading to the comprehension and resolution of the pathophysiologic complexity of allergies will allow for the design of novel patient-oriented diagnostic and treatment protocols. Several allergic diseases require well-controlled epidemiological description and surveillance, using disease registries, pharmacoeconomic evaluation, as well as large biobanks. Additionally, there is a need for extensive studies to bring promising new biotechnological innovations, such as biological agents, vaccines of modified allergen molecules and engineered components for allergy diagnosis, closer to clinical practice. Finally, particular attention should be paid to the difficult-to-manage, precarious and costly severe disease forms and/or exacerbations. Nonetheless, currently arising treatments, mainly in the fields of immunotherapy and biologicals, hold great promise for targeted and causal management of allergic conditions. Active involvement of all stakeholders, including Patient Organizations and policy makers are necessary to achieve the aims emphasized herein.

- Abstract
Seit vielen Jahren setzen Allergologen auf IgE-Tests wie Hautpricktests und die Messung der spezifischen IgE-Antikörper im Serum gegen das vollständige Allergen, um IgE-vermittelte, klinisch relevante Allergien zu diagnostizieren. Diese Tests haben einen wesentlichen Nachteil: Sie stellen zwar eine Sensibilisierung fest, diese hängt aber nur indirekt mit der klinisch relevanten Allergie zusammen. Mit vielen Patienten müssen daher Provokationstests durchgeführt werden, die häufig teuer und mit potenziell schwerwiegenden Reaktionen verbunden sind, um eine endgültige Diagnose zu stellen.Die Wahrscheinlichkeit für eine klinisch relevante Allergie kann durch die Ergebnisse eines IgE-Tests semi-quantitativ bestimmt werden. Wie belastbar diese Korrelation ist, variiert je nach Alter, ethnischer Herkunft und Art der vermeintlich allergischen Reaktion des Patienten, und wird durch den Grad der Pollenexposition, die Ernährung sowie bestehende klinische Erkrankungen, etwa Ekzeme, beeinflusst. Werden diese individuellen Faktoren bei der Auswertung eines IgE-Tests berücksichtigt, kann die Wahrscheinlichkeit für das Auftreten einer klinisch relevanten Allergie präziser abgeschätzt werden (Prätestwahrscheinlichkeit). Beruhend auf den Testergebnissen weist dann das Vorhandensein jeder dieser patientenspezifischen Charakteristika darauf hin, ob bei einem Patienten mit höherer oder geringerer Wahrscheinlichkeit eine klinisch relevante Allergie vorliegt (Posttestwahrscheinlichkeit). Die Einbeziehung von Ko-Faktoren, wie zum Beispiel sportliche Betätigung, könnten auch erforderlich sein, um sensitivere und genauere Testresultate zu erzielen.In dieser Arbeit werden zwei Ansätze vorgestellt, wie Prätestwahrscheinlichkeiten bei der Interpretation der IgE-Testergebnisse eingebunden werden können. Obwohl diese erhebliche Vorteile bieten, speziell in der Klinik, muss noch mehr in diesen Forschungsbereich investiert werden, um künftig die Prätestwahrscheinlichkeiten für verschiedene Bedingungen, Lebensregionen und Allergene ableiten zu können. Ebenfalls muss noch genauer geklärt werden, welchen Stellenwert die Testergebnisse in den verschiedenen Populationen haben. Kliniker müssen dafür sorgen, dass die Patienten die Testergebnisse klar verstehen, um ihnen Selbstvertrauen zum Selbstmanagement zu geben.
