108. Progenitor Cell Heterogeneity in the Developing Retina Is Revealed Through Distinct Requirements for LHX2 in the Regulation of Competence and Proliferation

Abstract

During development, all seven of the major retinal cell types are produced in a distinct yet overlapping order from a single pool of multipotent retinal progenitor cells (RPCs). In order to accomplish this task, it is thought that each individual RPC proceeds irreversibly through a series of intrinsically defined competence states, capable of producing only a subset of these cell types at any given time. To ensure that all early- and late-born cell types are produced in the correct number, RPCs must not only proceed through these competence states in a timely fashion, but also limit their rate of differentiation in order to prevent premature depletion. Lhx2 is a LIM-homeobox transcription factor expressed in many different tissues during development, known to regulate both proliferation and fate choice. It is also expressed in most if not all RPCs, and we assessed its contribution to their various properties by performing conditional inactivation at multiple time points during retinal neurogenesis. We find that Lhx2 is required within a limited temporal window to ensure the balanced production of earlyborn cell types, as retinal ganglion cells (RGCs) are selectively overproduced in the Lhx2 conditional knock-out (CKO) retina. LHX2 is also necessary for the normal cessation of RGC genesis, suggesting that in its absence, RPCs fail to undergo a normal transition in competence. We show that sustained Notch signaling, dependent on the transcription factor Rbpj, is likely responsible for maintaining a low level of both proliferation and neurogenesis in the absence of Lhx2, based on their successive requirements at distinct stages in the lineage progression of RPCs. We find further that LHX2 is required for the iv normal response of RPCs to Sonic Hedgehog (SHH), a negative-feedback signal secreted by RGCs and known to limit their further generation. In addition, we show that both LHX2 and SHH promote the expression of Ascl1, a gene expressed in RPCs and implicated as molecular readout of competence progression. Together, the results presented here demonstrate how intrinsic factors such as LHX2 may perform distinct functions at distinct phases of RPC lineage progression to orchestrate the process of retinal neurogenesis.

Abstract

Objectives: To describe the baseline characteristics of SLE patients requiring biologic therapy in the UK and to explore short term efficacy and infection rates associated with rituximab (RTX) use.\ud \ud \ud \ud Methods: Patients commencing biologic therapy for refractory SLE and who consented to join BILAG-BR were analysed. Baseline characteristics, disease activity (BILAG 2004/SLEDAI-2K) and rates of infection over follow-up were analysed. Response was defined as loss of all A and B BILAG scores to ⩽ 1 B score with no new A/B scores in other organ systems at 6 months.\ud \ud \ud \ud Results: Two hundred and seventy SLE patients commenced biologic therapy from September 2010 to September 2015, most commonly RTX (n = 261). Two hundred and fifty (93%) patients were taking glucocorticoids at baseline at a median [interquartile range (IQR)] oral dose of 10 mg (5–20 mg) daily. Response rates at 6 months were available for 68% of patients. The median (IQR) BILAG score was 15 (10–23) at baseline and 3 (2–12) at 6 months (P < 0.0001). The median (IQR) SLEDAI-2K reduced from 8 (5–12) to 4 (0–7) (P < 0.001). Response was achieved in 49% of patients. There was also a reduction in glucocorticoid use to a median (IQR) dose of 7.5 mg (5–12 mg) at 6 months (P < 0.001). Serious infections occurred in 26 (10%) patients, being more frequent in the first 3 months post-RTX therapy. A higher proportion of early infections were non-respiratory (odds ratio = 1.98, 95% CI: 0.99, 3.9; P = 0.049).\ud \ud \ud \ud Conclusion: RTX is safe and is associated with improvement in disease activity in refractory SLE patients with concomitant reductions in glucocorticoid use. Early vigilance for infection post-infusion is important to further improve treatment risks and benefits.

Permanent establishment status under the U.S.-Mexico Income Tax Treaty can be avoided by US concerns operating affiliates in Mexico if they plan properly. Permanent establishment findings result in taxation of net income attributable to the establishment when certain activities not considered temporary or auxiliary are performed at a Mexican site. Maquiladoras wishing to avoid permanent establishment status may wish to use arms-length valuation of all transfers between the US parent and the maquiladora. Abstract

The formation of specific protein‐protein interactions is often a key to a protein's function. During complex formation, each protein component will undergo a change in the conformational state, for some these changes are relatively small and reside primarily at the sidechain level; however, others may display notable backbone adjustments. One of the classic problems in the protein‐docking field is to be able to a priori predict the extent of such conformational changes. In this work, we investigated three protocols to find the most suitable input structure conformations for cross‐docking, including a robust sampling approach in normal mode space. Counterintuitively, knowledge of the theoretically best combination of normal modes for unbound‐bound transitions does not always lead to the best results. We used a novel spatial partitioning library, Aether Engine (see Supplementary Materials), to efficiently search the conformational states of 56 receptor/ligand pairs, including a recent CAPRI target, in a systematic manner and selected diverse conformations as input to our automated docking server, SwarmDock, a server that allows moderate conformational adjustments during the docking process. In essence, here we present a dynamic cross‐docking protocol, which when benchmarked against the simpler approach of just docking the unbound components shows a 10% uplift in the quality of the top docking pose. Abstract

Objectives It has been over 10 years since the first report of autoantibodies directed against phenylalanyl tRNA synthetase (anti-Zo) in a patient with features of the anti-synthetase syndrome. In that time no further cases have been published. Here we aim to characterize more fully the clinical phenotype of anti-Zo–associated myositis by describing the clinical features of nine patients. Methods Anti-Zo was identified by protein-immunoprecipitation in patients referred for extended spectrum myositis autoantibody testing at our laboratory. Results were confirmed by immunodepletion using a reference serum. Medical records were retrospectively reviewed to provide detailed information of the associated clinical phenotype for all identified patients. Where possible, HLA genotype was imputed using Illumina protocols. Results Nine patients with anti-Zo were identified. The median age at disease onset was 51 years, and six patients were female. Seven patients had evidence of inflammatory muscle disease, seven of interstitial lung disease and six of arthritis. The reported pattern of interstitial lung disease varied with usual interstitial pneumonia, non-specific interstitial pneumonia and organizing pneumonia all described. Other features of the anti-synthetase syndrome such as RP and mechanics hands were common. HLA data was available for three patients, all of whom had at least one copy of the HLA 8.1 ancestral haplotype. Conclusion Patients with anti-Zo presenting with features of the anti-synthetase syndrome and interstitial lung disease is a common finding. Like other myositis autoantibodies, there is likely to be a genetic association with the HLA 8.1 ancestral haplotype. [ABSTRACT FROM AUTHOR]

Abstract

Background: Rituximab (RTX) in combination with methotrexate (MTX) has been licensed since 2006 for the management of severe active rheumatoid arthritis (RA) in patients who have failed at least one anti-tumour necrosis factor (anti-TNF) therapy. Published clinical trials have demonstrated the efficacy of RTX in improving both clinical symptoms and patients' physical function. This study aimed to assess the effectiveness of RTX in RA patients treated in routine clinical practice by examining clinical and patient reported outcomes six months after receiving a first course of RTX. Methods: The analysis involved 550 RA patients registered with the BSRBR, who were starting RTX and were followed up for at least 6 months. Change in Disease Activity Score (DAS28) and European League Against Rheumatism (EULAR) response were used to assess the clinical response while change in Health Assessment Questionnaire (HAQ) score was used to assess the physical function of the patients 6 months after starting RTX. The change in DAS28 and HAQ was compared between seronegative and seropositive patients and anti-TNF naïve patients versus anti-TNF failures. The response was also compared between patients receiving RTX in combination with MTX, other non-biologic disease modifying anti-rheumatic drugs (nbDMARDs) or no nbDMARDs. Results: The mean (s.d.) age of the cohort was 59 (12) years and 78% of the patients were females. The patients had a mean (s.d.) of 15 (10) years of disease duration. 16% were biologic naïve while 84% were anti-TNF failures. 32% of the patients were seronegative and 68% were seropositive. The mean (95% CI) DAS28 at baseline was 6.2 (6.1, 6.3) which decreased to 4.8 (4.7, 4.9) at 6 months of follow up. 16% were EULAR good responders, 43% were moderate responders and 41% were non responders. Abstract

Paul Dukes writes: The publication of the Diary of Patrick Gordon has by now fully established itself as an invaluable source for Russian, British and European history in the second half of the seventeenth century. Volume V, thoroughly edited like its predecessors by Dmitry Fedosov, comprises a wide range of activities on land and sea from 1690 to 1695, many of them involving the young tsar Peter. Having helped the future Peter the Great to consolidate his hold on the throne, Gordon grew closer to him in ‘large discourse', being ‘entertained & detained all night', and so on. On the practice battlefield, the Scottish general played a leading part realistic enough for himself to be shot in the thigh and his son-in-law to be mortally wounded. As Fedosov says, Gordon along with the tsar proceeded from the pastimes of Mars to those of Neptune in naval exercises on the White Sea. Moving south to actual combat, the Russian forces re-engaged the Crimean Khanate and Ottoman Empire in further episodes in the long struggle for the fortress of Azov. Volume V, the longest of the six, contains many letters to and from Gordon and a wide range of his observations on political developments throughout Europe.

Abstract

Objective.Currently there are no evidence-based recommendations regarding fitness and strength tests for patients with childhood or adult idiopathic inflammatory myopathies (IIM). This hinders clinicians and researchers in choosing the appropriate fitness- or muscle strength-related outcome measures for these patients. Through a Delphi survey, we aimed to identify a candidate core set of fitness and strength tests for children and adults with IIM.Methods.Fifteen experts participated in a Delphi survey that consisted of 5 stages to achieve a consensus. Using an extensive search of published literature and through the work of experts, a candidate core set based on expert opinion and clinimetrics properties was developed. Members of the International Myositis Assessment and Clinical Studies Group were invited to review this candidate core set during the final stage, which led to a final candidate core set.Results.A core set of fitness- and strength-related outcome measures was identified for children and adults with IIM. For both children and adults, different tests were identified and selected for maximal aerobic fitness, submaximal aerobic fitness, anaerobic fitness, muscle strength tests, and muscle function tests.Conclusion.The core set of fitness- and strength-related outcome measures provided by this expert consensus process will assist practitioners and researchers in deciding which tests to use in patients with IIM. This will improve the uniformity of fitness and strength tests across studies, thereby facilitating the comparison of study results and therapeutic exercise program outcomes among patients with IIM.

Liquid-liquid phase transfer catalysis (LLPTC) mechanisms are provided for a displacement reaction where [Q.sup.+] denotes the quaternary catalyst cation (tetraalkylammonium or phosphonium ion), [X.sup.-], the catalyst anion, [M.sup.+][Y.sup.-], the water soluble reagent and RX, the substrate soluble in the organic phase. These colorful and fairly safe experiments can be used to help students get acquainted with the feasibility of extracting some soluble anionic reagents from water into an organic layer.

Abstract

The great Russian historian, S.М. Soloviev, regarded Patrick Gordon as “one of the most remarkable men” ever employed by the tsars, and was grateful to him for “recording his adventures and existence day by day, leaving to us curious tidings of himself, of his brothers in arms, and of Russia before the age of transformation” – and much else besides. Passages from the Diary were published in 1859. Now, 150 years on, the appearance of the first of six volumes marks the beginning of the publication of the complete work. In Volume I, Gordon writes of his early life near Ellon, Aberdeenshire, and of his military apprenticeship in the armies of Sweden and Poland, with colourful descriptions of people (including many fellow Scots), places and events.

Abstract

Objective. Ciclosporin and MTX are used in idiopathic inflammatory myopathies (DM and PM) when patients incompletely respond to glucocorticoids. Their effectiveness is unproved in randomized controlled trials (RCTs). We evaluated their benefits in a placebo-controlled factorial RCT. Methods. A 56-week multicentre factorial-design double-blind placebo-controlled RCT compared steroids alone, MTX (15-25 mg weekly) plus steroids, ciclosporin (1-5 mg/kg/day) plus steroids and all three treatments. It enrolled adults with myositis (by Bohan and Peter criteria) with active disease receiving corticosteroids. Results. A total of 359 patients were screened and 58 randomized. Of the latter, 37 patients completed 12 months of treatment, 7 were lost to follow-up and 14 discontinued treatment. Patients completing 12 months of treatment showed significant improvement (P < 0.001 on paired t-tests) in manual muscle testing (14% change), walking time (22% change) and function (9% change). Intention to treat and completer analyses indicated that ciclosporin monotherapy, MTX monotherapy and ciclosporin/MTX combination therapy showed no significant treatment effects in comparison with placebo. Conclusion. Neither MTX nor ciclosporin (by themselves or in combination) improved clinical features in myositis patients who had incompletely responded to glucocorticoids. 148. Cost savings associated with an alternative payment model for integrating behavioral health in primary care.

Abstract

Financially supporting and sustaining behavioral health services integrated into primary care settings remains a major barrier to widespread implementation. Sustaining Healthcare Across Integrated Primary Care Efforts (SHAPE) was a demonstration project designed to prospectively examine the cost savings associated with utilizing an alternative payment methodology to support behavioral health services in primary care practices with integrated behavioral health services. Six primary care practices in Colorado participated in this project. Each practice had at least one on-site behavioral health clinician providing integrated behavioral health services. Three practices received non-fee-for-service payments (i.e., SHAPE payment) to support provision of behavioral health services for 18 months. Three practices did not receive the SHAPE payment and served as control practices for comparison purposes. Assignment to condition was nonrandom. Patient claims data were collected for 9 months before the start of the SHAPE demonstration project (pre-period) and for 18 months during the SHAPE project (post-period) to evaluate cost savings. During the 18-month post-period, analysis of the practices' claims data demonstrated that practices receiving the SHAPE payment generated approximately $1.08 million in net cost savings for their public payer population (i.e., Medicare, Medicaid, and Dual Eligible; N = 9,042). The cost savings were primarily achieved through reduction in downstream utilization (e.g., hospitalizations). The SHAPE demonstration project found that non-fee-for-service payments for behavioral health integrated into primary care may be associated with significant cost savings for public payers, which could have implications on future delivery and payment work in public programs (e.g., Medicaid). 149. Exploring Exponential Decay Using Limited Resources

Abstract

Science students need exposure to activities that will help them to become familiar with phenomena exhibiting exponential decay. This paper describes an experiment that allows students to determine the rate of thermal energy loss by a hot object to its surroundings. It requires limited equipment, is safe, and gives reasonable results. Students record the decreasing temperature values displayed by a thermometer, which was initially heated by body contact. The procedure is easy to repeat, taking only 3 min. The students then convert temperature changes into thermal energy losses and plot graphs. Data analysis can be used to teach graphing and ratio interpretation. This experiment can be used by high school or first-year college students in situations where resources or space may be limited or where large student populations dictate a need for uncomplicated experimental procedures, particularly those that minimize the production of chemical waste.
