The Internet of Things (IoT) is expected to bring about many new applications resulting in an exponentially increasing number of connected devices within the next decade. Two key technologies for the IoT era are wireless connectivity and world-wide positioning enabling applications such as asset tracking or sensor nodes for smart farming. With applications only being limited by cost, power consumption, and form factor unprecedented integration levels are required that cannot be met with off-the-rack cellular modems and Global-Navigation-Satellite-System (GNSS) receivers. This contribution embraces this challenge by integrating a low-cost Assisted-GNSS (A-GNSS) receiver supporting GPS and BeiDou into a dual-mode cellular IoT modem RF-System-on-Chip (RF-SoC). The GNSS-related overhead in terms of silicon area is limited to 2% by taking full advantage of synergies arising from this integration and by exploiting missing real-time requirements of many IoT applications while achieving 25-m position accuracy.

Introduction: Veno-arterial extracorporeal membrane oxygenation is well-established for pediatric patients with post-cardiotomy heart failure. However, extracorporeal membrane oxygenation support is associated with major complications, that is, hemorrhage and thromboembolism. We seek to report our experience with delayed systemic heparinization during neonatal cardiac extracorporeal membrane oxygenation and its impact on bleeding and thromboembolism. Methods: We retrospectively identified 15 consecutive neonates who were placed on extracorporeal membrane oxygenation after congenital heart surgery during a period of 3 years (2015-2017). Our anticoagulation protocol consisted of full heparin reversal by protamine after switching from cardiopulmonary bypass to extracorporeal membrane oxygenation (target activated clotting time: 120 ± 20 seconds). Administration of systemic heparinization was delayed until postoperative drainage volume declined to Results: Our cohort (mean age: 13 ± 2.6 days; mean weight: 3.1 ± 0.3 kg; 66.7% male) required post-cardiotomy extracorporeal membrane oxygenation with a mean support time of 4.5 ± 2.2 days. Systemic heparinization was delayed averagely for 18.1 ± 9.3 hours. No thromboembolic events were observed on extracorporeal membrane oxygenation or after weaning. Relevant surgical site bleeding occurred in two patients (13.3%) requiring re-thoracotomy on the first postoperative day. Analysis of transfusion volumes revealed 24.5 ± 21.9 mL/kg/d mean packed red blood cells, 9.6 ± 7.1 mL/kg/d mean fresh frozen plasma, and 7.5 ± 5.7 mL/kg/d mean platelets. In-hospital survival was 86.6% (n = 13). Conclusion: In this retrospective analysis, the results of delayed systemic heparinization in neonatal post-cardiotomy extracorporeal membrane oxygenation could lead one to conclude that this routine is safe and favorable with low risk for thromboembolic events, reduced postoperative hemorrhage, and reduced blood product utilization.

106. Plasma Density and Magnetic Field Fluctuations in the Ion Gyro-Frequency Range Near the Diamagnetic Cavity of Comet 67P

- Abstract
We report the detection of large-amplitude, quasi-harmonic density fluctuations with associated magnetic field oscillations in the region surrounding the diamagnetic cavity of comet 67P. Typical frequencies are similar to 0.1 Hz, corresponding to similar to 10 times the water and less than or similar to 0.5 times the proton gyro-frequencies, respectively. Magnetic field oscillations are not always clearly observed in association with these density fluctuations, but when they are, they consistently have wave vectors perpendicular to the background magnetic field, with the principal axis of polarization close to field-aligned and with a similar to 90 degrees phase shift with respect to the density fluctuations. The fluctuations are observed in association with asymmetric plasma density and magnetic field enhancements previously found in the region surrounding the diamagnetic cavity, occurring predominantly on their descending slopes. This is a new type of waves not previously observed at comets. They are likely ion Bernstein waves, and we propose that they are excited by unstable ring, ring-beam, or spherical shell distributions of cometary ions just outside the cavity boundary. These waves may play an important role in redistributing energy between different particle populations and reshape the plasma environment of the comet.

We calculate the momentum flux and pressure of ions measured by the Ion Composition Analyzer (ICA) on the Rosetta mission at comet 67P/Churyumov-Gerasimenko. The total momentum flux stays roughly constant over the mission, but the contributions of different ion populations change depending on heliocentric distance. The magnetic pressure, calculated from Rosetta magnetometer measurements, roughly corresponds with the cometary ion momentum flux. When the spacecraft enters the solar wind ion cavity, the solar wind fluxes drop drastically, while the cometary momentum flux becomes roughly 10 times the solar wind fluxes outside of the ion cavity, indicating that pickup ions behave similarly to the solar wind ions in this region. We use electron density from the Langmuir probe to calculate the electron pressure, which is particularly important close to the comet nucleus where flow changes from antisunward to radially outward.

Aortic root dilatation and its complications are known to be the most important and life limiting features in patients with Marfan syndrome (MFS). Since monitoring of patients, preventive medical and surgical treatments are available nowadays, other MFS pathologies are becoming more relevant for the outcome of the disease. Main pulmonary artery (MPA) dilatation is a cardiac finding, which has not been fully investigated in children. Due to the similarities in tissue composition of the aortic and pulmonary root, MPA dilatation may cause complications and require treatment. In addition, it may be a predictor for severe connective tissue involvement. We retrospectively examined 135 pediatric patients with MFS. 8.1% showed MPA dilatation. - Abstract
Background: Several clinician, informant, and self-report instruments for tics and associated phenomena have been developed that differ in construct, comprehensiveness, and ease of administration. Objective: A Movement Disorders Society subcommittee aimed to rate psychometric quality of severity and screening instruments for tics and related sensory phenomena. Methods: Following the methodology adopted by previous Movement Disorders Society subcommittee papers, a review of severity and screening instruments for tics was completed, applying a classification as “recommended,” “suggested,” or “listed” to each instrument. Results: A total of 5 severity scales (Yale Global Tic Severity Scale, Tourette Syndrome Clinical Global Impression, Tourette's Disorder Scale, Shapiro Tourette syndrome Severity Scale, Premonitory Urges for Tics Scale) were “recommended,” and 6 (Rush Video-Based Tic Rating Scale, Motor tic, Obsessions and compulsions, Vocal tic Evaluation Survey, Tourette Syndrome Global Scale, Global Tic Rating Scale, Parent Tic Questionnaire, Tourette Syndrome Symptom List) were “suggested.” A total of 2 screening instruments (Motor tic, Obsession and compulsions, Vocal tic Evaluation Survey and Autism-Tics, Attention Deficit/Hyperactivity Disorder and Other Comorbidities Inventory) were “recommended,” whereas 2 others (Apter 4-questions screening and Proxy Report Questionnaire for Parents and Teachers) were “suggested.”. Conclusions: Our review does not support the need for developing new tic severity or screening instruments. Potential objectives of future research include developing a rating instrument targeting the full spectrum of tic-related abnormal behaviors, assessing/screening malignant forms of tic disorders, and developing patient-reported outcome measures.

Background: Ultra-high density mapping (HDM) is a promising tool in the treatment of patients with complex arrhythmias. In adults with congenital heart disease (CHD), rhythm disorders are among the most common complications but catheter ablation can be challenging due to heterogenous anatomy and complex arrhythmogenic substrates. Here, we describe our initial experience using HDM in conjunction with novel automated annotation algorithms in patients with moderate to great CHD complexity. Methods: We studied a series of consecutive adult patients with moderate to great CHD complexity and an indication for catheter ablation due to symptomatic arrhythmia. HDM was conducted using the Rhythmia™ mapping system and a 64-electrode mini-basket catheter for identification of anatomy, voltage, activation pattern and critical areas of arrhythmia for ablation guidance. To investigate novel advanced mapping strategies, postprocedural signal processing using the Lumipoint™ software was applied. Results: In 19 patients (53±3 years; 53% male), 21 consecutive ablation procedures were conducted. Procedures included ablation of atrial fibrillation (n=7; 33%), atrial tachycardia (n=11; 52%), atrioventricular accessory pathway (n=1; 5%), the atrioventricular node (n=1; 5%) and ventricular arrhythmias (n=4; 19%). A total of 23 supraventricular and 8 ventricular arrhythmias were studied with the generation of 56 complete high density maps (atrial n=43; ventricular n=11, coronary sinus n=2) and an average of 12,043±1,679 mapping points. Multiple arrhythmias were observed in n=7 procedures (33% of procedures; range of arrhythmias detected 2–4). A total range of 1–4 critical areas were defined per procedure and treated within a radiofrequency application time of 16 (interquartile range 12–45) minutes. Postprocedural signal processing using Lumipoint™ allowed rapid annotation of fractionated signals within specific windows of interest. This supported identification of a practical critical isthmus in 20 out of 27 completed atrial and ventricular tachycardia activation maps. Conclusions: Our findings suggest that HDM in conjunction with novel automated annotation algorithms provides detailed insights into arrhythmia mechanisms and might facilitate tailored catheter ablation in patients with moderate to great CHD complexity.

Intravitreal injections with anti-vascular endothelial growth factor (VEGF) drugs can slow progression in neovascular age-related macular degeneration (nAMD). Best spectacle-corrected visual acuity (BSCVA) and/or central retinal thickness (CRT) are common barometers of efficacy of this treatment. However, BSCVA does not accurately measure reading ability, which is often severely impacted by nAMD. Since most studies on the effect of intravitreal anti-VEGF injections on reading ability in nAMD have limitations, we conducted an open-label nonrandomized non-blinded self-controlled clinical trial using Radner Charts, a standardized, validated tool for measuring reading ability.

- Abstract
Using in situ measurements from different instruments on board the Rosetta spacecraft, we investigate the properties of the newly discovered low-frequency oscillations, known as singing comet waves, that sometimes dominate the close plasma environment of comet 67P/Churyumov-Gerasimenko. These waves are thought to be generated by a modified ion-Weibel instability that grows due to a beam of water ions created by water molecules that outgass from the comet. We take advantage of a cometary outburst event that occurred on 2016 February 19 to probe this generation mechanism. We analyze the 3D magnetic field waveforms to infer the properties of the magnetic oscillations of the cometary ion waves. They are observed in the typical frequency range (similar to 50 mHz) before the cometary outburst, but at similar to 20 mHz during the outburst. They are also observed to be elliptically right-hand polarized and to propagate rather closely (similar to 0-50 degrees) to the background magnetic field. We also construct a density dataset with a high enough time resolution that allows us to study the plasma contribution to the ion cometary waves. The correlation between plasma and magnetic field variations associated with the waves indicates that they are mostly in phase before and during the outburst, which means that they are compressional waves. We therefore show that the measurements from multiple instruments are consistent with the modified ion-Weibel instability as the source of the singing comet wave activity. We also argue that the observed frequency of the singing comet waves could be a way to indirectly probe the strength of neutral plasma coupling in the 67P environment.

- Abstract
Aims: On July 3, 2015, an unprecedented increase in the magnetic field magnitude was measured by the Rosetta spacecraft orbiting comet 67P/Churyumov-Gerasimenko (67P). This increase was accompanied by large variations in magnetic field and ion and electron density and energy. To our knowledge, this unusual event marks the highest magnetic field ever measured in the plasma environment of a comet. Our goal here is to examine possible physical causes for this event, and to explain this reaction of the cometary plasma and magnetic field and its trigger. Methods: We used observations from the entire Rosetta Plasma Consortium as well as energetic particle measurements from the Standard Radiation Monitor on board Rosetta to characterize the event. To provide context for the solar wind at the comet, observations at Earth were compared with simulations of the solar wind. Results: We find that the unusual behavior of the plasma around 67P is of solar wind origin and is caused by the impact of an interplanetary coronal mass ejection, combined with a corotating interaction region. This causes the magnetic field to pile up and increase by a factor of six to about 300 nT compared to normal values of the enhanced magnetic field at a comet. This increase is only partially accompanied by an increase in plasma density and energy, indicating that the magnetic field is connected to different regions of the coma.

Cardiovascular biomarkers might help to identify fetuses or pregnancies at risk.To examine the umbilical cord neonatal and maternal levels of cardiovascular biomarkers at the time of delivery, and to correlate maternal and fetal biomarker levels to each other, to gestational age and to delivery mode.In a prospective, observational, cross-sectional, single-center study biomarkers were measured in paired maternal and umbilical venous cord blood samples.The sample cohort included 66 sets of fetal and maternal blood samples (11 after multiple gestation, 53 after cesarean section, 17 after exposure to labor).Midregional pro-adrenomedullin (MRproADM), midregional-pro atrial natriuretic peptide (MRproANP), brain natriuretic peptide (BNP), n-terminal-pro brain natriuretic peptide (NTproBNP), copeptin, and high sensitive troponin I (hsTnI) levels were measured.Mean ± SEM for biomarker levels in umbilical venous/maternal blood were: MRproADM [nmol/L] 1.02 ± 0.04/1.24 ± 0.08, MRproANP [pmol/L] 215.53 ± 12.96/54.65 ± 3.41, BNP [pg/mL] 32.02 ± 3.37/19.76 ± 3.29, NTproBNP [pg/mL] 1228.94 ± 91.73/71.48 ± 8.65, copeptin [pmol/L] 103.42 ± 22.89/10.41 ± 1.71, and hsTnI [pg/mL] 13.54 ± 5.17/4.91 ± 2.37. Fetal MRproANP, NTproBNP, and BNP were inversely correlated with gestational age. Maternal and fetal MRproANP (r=0.472, p=0.002) and copeptin (r=0.572, p0.001) levels were correlated, whereas there was no feto-maternal correlation for the other biomarkers. Fetal copeptin was elevated after exposure to labor.Biomarker levels appear to be regulated independently in mother and fetus. Fetal biomarkers are influenced by gestational age and delivery mode. In this study on term and near term pregnancies without specific fetal pathology, correlation between paired maternal and fetal biomarker levels was weak or not demonstrable.

Aim Marfan syndrome (MFS) is a progressive, life-threatening genetic disorder of the connective tissue, which causes impaired quality of life (QoL) in adults. This study investigated the quality of life in children and adolescents, taking into account their gender, age and how MFS affected their organs. Methods This prospective nonrandomised single-centre study included 46 patients with verified MFS with a mean age of 10.98 years (±3.72). QoL was measured using the self-reported, multidimensional KINDL-R questionnaire and compared with an age-matched control group of 174 children and adolescents. Results No significant overall reduction of QoL was found. Total QoL scores for patients diagnosed at four to seven years were the same as the control group (77.65 ± 9.37 versus 77.06 ± 11.72), but they were higher for patients aged eight to 16 years (75.15 ± 9.19 versus 70.46 ± 11.35, p = 0.025). No gender-specific differences or impairments in QoL during adolescence were observed (p > 0.05). Analysis of the effect of organ manifestation on QoL showed better or equal QoL scores (p > 0.05), despite distinctive phenotypes such as ectopia lentis. Conclusion QoL was fairly good in paediatric patients with MFS, and there was no impairment during adolescence. Despite the distinctive phenotype, quality of life was unimpaired in younger patients.

Parkinson's disease is a neurological disease which involves the selective destruction of some brain cells preferentially over others. The brain is able to compensate for considerable damage, and the available evidence suggests that the symptoms of Parkinson's disease appear only after the destruction of neurons (nerve cells) surpasses the brain's ability for compensation. Parkinson's disease largely results from the destruction of special brain cells within a region called the substantia nigra; by some estimates as many as 90 percent of these neurons may be destroyed before the brain can no longer compensate and disease symptoms appear. Therefore, the disease is present, and destroying neurons in the substantia nigra, long before it is recognized by the patient and physician. Evidence is accumulating that the drug selegiline may significantly slow the progression of Parkinson's disease, and so the initiation of treatment prior to the onset of symptoms might provide important clinical benefit. Just how long is Parkinson's disease present before the symptoms actually appear? Since there is no reliable test for the presence of Parkinson's disease, there is no way of answering with certainty. However, estimates may be made on the basis of neuropathological observations of brain tissue specimens obtained at autopsy. The characteristic pathological observation in Parkinson's disease is the Lewy body, which is a structure visible in the cytoplasm of some brain cells under the microscope. Lewy bodies are also found in cells of the substantia nigra of about 10 percent of people over age 60 who do not have symptoms of Parkinson's disease. If the assumption is made that Lewy bodies represent an early stage in the long progression towards brain cell destruction, then calculations suggest that the delay between the appearance of Lewy bodies and the development of the symptoms of Parkinson's disease must be about 30 years. The cells of the substantia nigra may be examined, of course, only at autopsy, and so other means must be found to diagnose Parkinson's disease in its presymptomatic stages. The search is underway to evaluate many techniques of medical imaging, biochemical analysis, and genetic screening for the diagnosis of presymptomatic Parkinson's disease. Patients with Parkinson's disease (PD), a degenerative neurologic disorder, have a variety of clinical signs and symptoms. A better understanding of the different forms that PD can take could help in its diagnosis and treatment. As part of a study about the effects of two drugs (deprenyl and tocopherol) on PD, baseline data were gathered on 800 patients with early untreated PD before they were randomly assigned to the drug testing phase. The group as a whole is referred to as the DATATOP cohort (deprenyl and tocopherol antioxidative therapy of parkinsonism). An evaluation of the spectrum of signs and symptoms among these patients is presented. Early-onset patients (whose first PD symptoms appeared at the age of 40 or younger) and late-onset patients (first symptoms at 70 or older) were disabled to the same extent, but the early-onset group had a longer duration of symptoms (2.9 years versus 1.7 years). Patients with a benign disease course (slow rate of progression) had earlier onset than those with a malignant course (fast progression). Two forms of PD were identified: the postural instability and gait difficulty (PIGD)-dominant, and the tremor-dominant, forms. Those in the former group (233 patients) had more occupational, movement, and intellectual disabilities than the 441 patients in the latter group. Family histories of PD prevalence were not different between these two groups. Follow-up of these patients is ongoing. (Consumer Summary produced by Reliance Medical Information, Inc.)

150. First observations of magnetic holes deep within the coma of a comet

The Rosetta spacecraft of the European Space Agency made ground-breaking observations of comet 67P/Churyumov-Gerasimenko and of its cometary environment. We search for magnetic holes in that environment, i.e., significant depressions in the magnetic field strength, measured by the Rosetta fluxgate Magnetometer (MAG) in April and May 2015. In that time frame of two months, we identified 23 magnetic holes. The cometary activity was intermediate and increasing because Rosetta was on the inbound leg toward the Sun. While in April solar wind protons were still observed by Rosetta near the comet, in May these protons were already mostly replaced by heavy cometary ions. Magnetic holes have frequently been observed in the solar wind. We find, for the first time, that magnetic holes exist in the cometary environment even when solar wind protons are almost absent. Some of the properties of the magnetic holes are comparable to those of solar wind holes; they are associated with density enhancements, sometimes associated with co-located current sheets and fast solar wind streams, and are of similar scales. However, particularly in May, the magnetic holes near the comet appear to be more processed, featuring shifted density enhancements and, sometimes, bipolar signatures in magnetic field strength rather than simple depressions. The magnetic holes are of global size with respect to the coma. However, at the comet, they are compressed owing to magnetic field pile-up and draping so that they change in shape. There, the magnetic holes become of comparable size to heavy cometary ion gyroradii, potentially enabling kinetic interactions.
