- Abstract
Hydrogeology is now taught in a broad spectrum of departments and institutions to students with diverse backgrounds. Successful instruction in hydrogeology thus requires a variety of pedagogical approaches depending on desired learning outcomes and the diverse background of students. We review the pedagogical literature in hydrogeology to highlight recent advances and analyze a 2005 survey of 68 hydrogeology instructors. The literature and survey results suggest there are ~15 topics that are considered crucial by most hydrogeologists and >100 other topics that are considered crucial by some hydrogeologists. The crucial topics focus on properties of aquifers and fundamentals of groundwater flow, and should likely be part of all undergraduate hydrogeology courses. Other topics can supplement and support these crucial topics, depending on desired learning outcomes. Classroom settings continue to provide a venue for emphasizing fundamental knowledge. However, recent pedagogical advances are biased towards field and laboratory instruction with a goal of bolstering experiential learning. Field methods build on the fundamentals taught in the classroom and emphasize the collection of data, data uncertainty, and the development of vocational skills. Laboratory and computer-based exercises similarly build on theory, and offer an opportunity for data analysis and integration. The literature suggests curricula at all levels should ideally balance field, laboratory, and classroom pedagogy into an iterative and integrative whole. An integrated approach leads to greater student motivation and advancement of theoretical and vocational knowledge. [ABSTRACT FROM AUTHOR]

We investigated the effect of acute and acclimatory temperature on the relative contribution of glucose and lactate to metabolism in resting sartorius muscle of the American bullfrog (Lithobates catesbelana). We examined the fate of these metabolites in vitro by supplying radiolabeled [14C]glucose, 14C]lactate and [14C]palmitate to isolated muscle bundles from frogs (1) acutely exposed to incubation conditions of 5, 15 or 25°C, (2) acclimated for 2-6 weeks to 5 or 25°C or (3) acclimated for 2-6 weeks to 5 or 25°C and the muscles incubated at 15°C. Under all three temperature conditions tested, net rate of lactate metabolism exceeded that of glucose. Acute exposure to 5°C reduced net rate of glucose metabolism by 15X and net lactate metabolism by 10X as compared with 25°C-exposed tissues. Acclimation to 5°C favored glucose storage as glycogen and increased the proportion of lactate oxidized (versus stored or converted to glucose) when compared with 25°C-acclimated tissues. Net rates of storage of lactate as glycogen (glyconeogenesis) were significantly higher in muscles from 5°C-acclimated frogs during incubation at a common temperature of 15°C. These data suggest that lactate is the predominant fuel for resting skeletal muscle over this temperature range, and particularly so under cold conditions. Ready use of lactate as a substrate, and enhancement of glyconeogenic pathways in response to cold acclimation, could play a role in the tolerance of this species to seasonal temperature changes by promoting sequestration and storage of available substrate under cold conditions. We characterized carbohydrate metabolism following activity in the American bullfrog, Rana catesbeiana, and compared whole body metabolic profiles between two seasons. Forty-eight adult male Rana catesbeiana were chronically cannulated and injected with [U-14C]L-lactic acid sodium salt in either summer (June) or winter (January) after acclimation for 2 weeks at 15°C with a 12 h:12 h L:D photoperiod. Following injection with [14C]lactate, frogs were either allowed to rest for 240 mm (REST), hopped for 2 min on a treadmill and immediately sacrificed (PE), or hopped for 2 mm on a treadmill and allowed to recover for 240 mm (REC 4). Exercise caused a significant increase in blood lactate level from 2.7±0.1 mmol l-1 at rest to 17.0±2.1 mmol l-1 immediately following exercise. This increase persisted throughout the recovery period, with average blood lactate level only reduced to 13.7±1.1 mmol l-1 after 240 min of recovery, despite complete recovery of intramuscular lactate levels. Lactate levels were not significantly different between seasons in any treatment (REST, PE, REC4), in either gastrodnemius muscle or blood. The vast majority of [14C]lactate was recovered in the muscle, in both winter (863%) and summer (875%). Season had no effect on total amount of 14C label recovered. [14C]Lactate was measured in the forms of lactate, glucose and glycogen, in the liver and the muscle sampled. The most robust difference found in seasonal metabolism was that both the liver and the gastrocnemius contained significantly higher levels of intracellular free glucose under all treatments in winter. These data suggest that, overall, bullfrogs accumulate and slowly clear lactate in a manner quite similar to findings in fish, other amphibians and lizards. Additionally, our findings indicate that lactate metabolism is not highly influenced by season alone, but that intracellular glucose levels may be sensitive to annual patterns. [ABSTRACT FROM AUTHOR]

128. Use of Ultrasound to Determine Need for Laparotomy in Trauma Patients

- Abstract
Study objective: To determine whether abdominal ultrasound can be used routinely as the primary screening test to identify the need for laparotomy in trauma patients. Methods: Ultrasound was used at a Level II trauma center as a primary screening test for evaluation of intraabdominal injury. We reviewed the charts of all patients from trauma codes presenting between January 1, 1991, and December 31, 1993, to determine the results of abdominal ultrasound evaluation and to learn whether laparotomy was required. Results: A total of 2,013 trauma patients presented during the study interval. Ultrasound was performed in 1,631 patients as the primary screening test for abdominal injury (mean time after arrival, 22.9 minutes), abdominal computed tomography (CT) was performed as the primary screen on 8 (mean time, 68.6 minutes), and 93 patients underwent both ultrasound and CT of the abdomen. Of 86 patients requiring laparotomy who were screened by ultrasound, 80 had positive diagnostic findings, for a sensitivity of 93.0%. Of the 1,545 ultrasound-screened patients who did not require laparotomy, 1,390 had negative findings, for a specificity of 90.0%. None of the patients with negative ultrasound results died or sustained identifiable mortality as a consequence of their negative scans. Conclusion: Ultrasound is a sensitive and specific test with which to evaluate trauma patients for abdominal injury requiring surgery. Routine abdominal ultrasound can be performed at the bedside in the emergency department as a timely, noninvasive diagnostic test. This use of a screening abdominal ultrasound examination can improve clinical decisionmaking for the use of emergency laparotomy. Ultrasound may be a better alternative to CT or diagnostic peritoneal lavage for the initial screening evaluation of abdominal trauma. This paper is the outcome of a community initiative to identify major unsolved scientific problems in hydrology motivated by a need for stronger harmonisation of research efforts. The procedure involved a public consultation through online media, followed by two workshops through which a large number of potential science questions were collated, prioritised, and synthesised. In spite of the diversity of the participants (230 scientists in total), the process revealed much about community priorities and the state of our science: a preference for continuity in research questions rather than radical departures or redirections from past and current work. Questions remain focused on the process-based understanding of hydrological variability and causality at all space and time scales. Increased attention to environmental change drives a new emphasis on understanding how change propagates across interfaces within the hydrological system and across disciplinary boundaries. In particular, the expansion of the human footprint raises a new set of questions related to human interactions with nature and water cycle feedbacks in the context of complex water management problems. We hope that this reflection and synthesis of the 23 unsolved problems in hydrology will help guide research efforts for some years to come.

- Abstract
Water is a fundamental resource, yet its spatiotemporal availability in East Africa is poorly understood. This is the area where most hominin first occurrences are located, and consequently the potential role of water in hominin evolution and dispersal remains unresolved. Here, we show that hundreds of springs currently distributed across East Africa could function as persistent groundwater hydro-refugia through orbital-scale climate cycles. Groundwater buffers climate variability according to spatially variable groundwater response times determined by geology and topography. Using an agent-based model, grounded on the present day landscape, we show that groundwater availability would have been critical to supporting isolated networks of hydro-refugia during dry periods when potable surface water was scarce. This may have facilitated unexpected variations in isolation and dispersal of hominin populations in the past. Our results therefore provide a new environmental framework in which to understand how patterns of taxonomic diversity in hominins may have developed.

Publisher's version (útgefin grein), This paper is the outcome of a community initiative to identify major unsolved scientific problems in hydrology motivated by a need for stronger harmonisation of research efforts. The procedure involved a public consultation through online media, followed by two workshops through which a large number of potential science questions were collated, prioritised, and synthesised. In spite of the diversity of the participants (230 scientists in total), the process revealed much about community priorities and the state of our science: a preference for continuity in research questions rather than radical departures or redirections from past and current work. Questions remain focused on the process-based understanding of hydrological variability and causality at all space and time scales. Increased attention to environmental change drives a new emphasis on understanding how change propagates across interfaces within the hydrological system and across disciplinary boundaries. In particular, the expansion of the human footprint raises a new set of questions related to human interactions with nature and water cycle feedbacks in the context of complex water management problems. BACKGROUND: The extent to which health care systems have adapted to the COVID-19 pandemic to provide necessary cardiac diagnostic services is unknown.OBJECTIVES: The aim of this study was to determine the impact of the pandemic on cardiac testing practices, volumes and types of diagnostic services, and perceived psychological stress to health care providers worldwide.METHODS: The International Atomic Energy Agency conducted a worldwide survey assessing alterations from baseline in cardiovascular diagnostic care at the pandemic's onset and 1 year later. Multivariable regression was used to determine factors associated with procedure volume recovery.RESULTS: Surveys were submitted from 669 centers in 107 countries. Worldwide reduction in cardiac procedure volumes of 64% from March 2019 to April 2020 recovered by April 2021 in high- and upper middle-income countries (recovery rates of 108% and 99%) but remained depressed in lower middle- and low-income countries (46% and 30% recovery). Although stress testing was used 12% less frequently in 2021 than in 2019, coronary computed tomographic angiography was used 14% more, a trend also seen for other advanced cardiac imaging modalities (positron emission tomography and magnetic resonance; 22%-25% increases). Pandemic-related psychological stress was estimated to have affected nearly 40% of staff, impacting patient care at 78% of sites. In multivariable regression, only lower-income status and physicians' psychological stress were significant in predicting recovery of cardiac testing.CONCLUSIONS: Cardiac diagnostic testing has yet to recover to prepandemic levels in lower-income countries. Worldwide, the decrease in standard stress testing is offset by greater use of advanced cardiac imaging modalities. Pandemic-related psychological stress among providers is widespread and associated with poor recovery of cardiac testing.
