Abstract

Anderson–Fabry disease (AFD) is a rare X-linked lysosomal storage disorder characterized by the accumulation of globotriaosylceramide, leading to multi-organ involvement and significant morbidity. Cardiovascular manifestations, particularly arrhythmias, are common and pose a considerable risk to affected individuals. This overview examines current approaches to arrhythmic risk stratification in AFD, focusing on the identification, assessment, and management of cardiac arrhythmias associated with the disease. We explore advancements in diagnostic techniques, including echocardiography, cardiac MRI, and ambulatory ECG monitoring, to enhance the detection of arrhythmogenic substrate. Furthermore, we discuss the role of genetic and biochemical markers in predicting arrhythmic risk and the implications for personalized treatment strategies. Current therapeutic interventions, including enzyme replacement therapy and antiarrhythmic medications, are reviewed in the context of their efficacy and limitations. Finally, we highlight ongoing research and future directions with the aim of improving arrhythmic risk assessment and management in AFD. Abstract

In recent years, new high spatial resolution observations of the Sun's atmosphere have revealed the presence of a plethora of small-scale magnetic elements down to the resolution limit of current cohort of solar telescopes ($\sim 100-120$ km on the solar photosphere). These small magnetic field concentrations, due to the granular buffeting, can support and guide several magneto-hydrodynamics (MHD) wave modes that would eventually contribute to the energy budget of the upper layers of the atmosphere.\\ In this work, exploiting the high spatial and temporal resolution chromospheric data acquired with the Swedish 1-meter Solar Telescope (SST), and applying the empirical mode decomposition (EMD) technique to the tracking of the solar magnetic features, we analyse the perturbations of the horizontal velocity vector of a set of chromospheric magnetic elements. Abstract

Acute fulminant myocarditis is a rare event in children, accounting for about 10% of all cases of acute myocarditis. Its lack of specific onset patterns and unpredictable evolution make diagnosis and prompt treatment challenging. We observed six cases of fulminant myocarditis admitted to our Pediatric Emergency Unit (Campania region, Sothern Italy) within a very short timeframe (50 days, from July to September 2024). Three of them died, and two are still under treatment in a Pediatric Cardiologic Intensive Care Unit in critical condition. In only one case, cardiac function improved. The described cases were not geographically linked, belonging to different areas of Southern Italy. No common etiological agent was found. Given the relatively low incidence of the condition, the occurrence of six pediatric myocarditis within approximately two months should be considered exceptional. Abstract

Abstract Background Septic shock is a severe form of sepsis marked by hypotension with an ominous outcome despite the introduction of modern intensive care. The aim of the present study is to obtain a panel with biomarkers, echocardiographic and vascular parameters to better risk stratify patients and identify those at higher risk of ominous outcome. Methods Between May 2013 and April 2016, 35 consecutive patients admitted at the Intensive Care Unit (ICU) of ASST Great Metropolitan Hospital Niguarda with the diagnosis of severe sepsis or septic shock were enrolled. All patients underwent rest echocardiography and several circulating biomarkers of myocardial damage or oxidative stress. Results The multivariate Cox’s proportional hazard model showed that the only independent prognostic predictor for 30-day mortality was the angiopoietin-2, (HR 1.017, 95% CI 1.000–1.034; P = 0.049). An angiopoietin-2 concentrations ≥ of 33,418 pg/mL was identified as the optimal threshold for the discrimination between survivors and non survivors at the time of admission in ICU, with a sensitivity of 80% and a specificity of 68%. Conclusions Septic shock has a poor in-hospital outcome even when the best of care is implemented. Among the biochemical parameters angiopoietin was able to identify patients at risk of death. The lowest the value at admission, the highest the risk of in-hospital death. No echocardiographic nor vascular parameter was able to predict outcome in this setting.

Abstract

This study aimed to examine the role of resilience resources in patients' lifestyle changes after the first Acute Coronary event. 275 Italian patients (84.0% men; mean age = 57.5, SD = 7.9) participated in a longitudinal study. Resilience resources (Self-esteem, Dispositional Optimism, Sense of Coherence – SOC, General and Disease-specific Self-efficacy), and lifestyles (diet, physical activity, and smoking) were assessed twice (at baseline and after 6 months). Path analysis using latent change models was performed to model the combined effect of levels and changes of the resilience resources over lifestyle changes. Patients with strong SOC at baseline were less prone to smoke and more prone to decrease smoking; enhancement in SOC was associated with a smoking decrease. High Disease-specific Self-efficacy at baseline was associated with an improvement in all lifestyles; enhancement in Disease-specific Self-efficacy predicted an increase in physical activity. Findings underline the need to design psychological interventions that promote patients' Disease-specific Self-efficacy and SOC.

Abstract

We studied the formation of a pore in AR NOAA 11462. We analysed data obtained with the IBIS at the DST on April 17, 2012, consisting of full Stokes measurements of the Fe I 617.3 nm lines. Furthermore, we analysed SDO/HMI observations in the continuum and vector magnetograms derived from the Fe I 617.3 nm line data taken from April 15 to 19, 2012. We estimated the magnetic field strength and vector components and the LOS and horizontal motions in the photospheric region hosting the pore formation. We discuss our results in light of other observational studies and recent advances of numerical simulations. The pore formation occurs in less than 1 hour in the leading region of the AR. The evolution of the flux patch in the leading part of the AR is faster (< 12 hour) than the evolution (20-30 hour) of the more diffuse and smaller scale flux patches in the trailing region. During the pore formation, the ratio between magnetic and dark area decreases from 5 to 2. We observe strong downflows at the forming pore boundary and diverging proper motions of plasma in the vicinity of the evolving feature that are directed towards the forming pore. The average values and trends of the various quantities estimated in the AR are in agreement with results of former observational studies of steady pores and with their modelled counterparts, as seen in recent numerical simulations of a rising-tube process. The agreement with the outcomes of the numerical studies holds for both the signatures of the flux emergence process (e.g. appearance of small-scale mixed polarity patterns and elongated granules) and the evolution of the region. The processes driving the formation of the pore are identified with the emergence of a magnetic flux concentration and the subsequent reorganization of the emerged flux, by the combined effect of velocity and magnetic field, in and around the evolving structure., Comment: Accepted for publication in Astronomy and Astrophysics

122. Polar Cap Patches Scaling Properties: Insights from Swarm Data

Abstract

Among the effects of space weather, the degradation of air traffic communications and satellite-based navigation systems are the most notable. For this reason, it is of uttermost importance to understand the nature and origin of ionospheric irregularities that are at the base of the observed communication outages. Here we focus on polar cap patches (PCPs) that constitute a special class of ionospheric irregularities observed at very high latitudes in the F region. To this purpose we use the so-called PCP flag, a Swarm Level 2 product, that allows for identifying PCPs. We relate the presence of PCPs to the values of the first- and second-order scaling exponents and intermittency estimated from Swarm A electron density fluctuations and to the values of the Rate Of change of electron Density Index (RODI) for two different levels of geomagnetic activity, over a time span of approximately 3.5 years starting on 16 July 2014. Our findings show that values of RODI, first- and second-order scaling exponents and intermittency corresponding to measurements taken inside PCPs differ from those corresponding to measurements taken outside PCPs. Additionally, the values of the first- and second-order scaling exponents and of intermittency indicate that PCPs are in a turbulent state. Investigation of the coincidence of loss of lock (LoL) events with PCPs displayed that approximately 57.4% of LoLs in the Northern hemisphere and 45.7% in the Southern hemisphere occur in coincidence of PCPs when disturbed geomagnetic activity is considered. During quiet geomagnetic conditions these percentages decrease to 51.4% in the Northern hemisphere and to 20.1% in the Southern hemisphere.

Abstract

Abstract Aims To evaluate endothelial function in subjects with left ventricular assist devices (LVADs), comparing them with subjects with chronic heart failure with reduced ejection fraction on the list for heart transplant (HT) and with HT patients with a normal systolic cardiac function to identify any differences. Methods We enrolled 28 subjects with LVAD, 55 subjects with HT, and 42 subjects with heart failure on the transplant list. The subjects underwent a general physical examination, assessment of laboratory blood parameters, and assessment of endothelial function through flow‐mediated dilation (FMD) of brachial artery. Results The three groups were homogeneous as regards age, gender, smoke abuse, C‐reactive protein (CRP) and FMD parameters (P = ns). In LVAD group percentage of FMD change showed an inverse correlation with CRP (rho: −0.5, P: 0.003), a well‐known marker of inflammation and tissue damage. Conclusions Continuous flow related to LVAD seems to not worsen endothelial function. Endothelial function was not affected by cardiovascular risk factors (hypertension, hypercholesterolaemia, diabetes, obesity, and tobacco habit), by the functional status expressed by New York Heart Association class, by the left ventricular systolic function and by the presence or absence of ischaemic heart disease in all the populations analysed. CRP was the only factor able to influence percentage of FMD change in patient with LVAD, reinforcing the hypothesis that inflammation is the main determinant of endothelial function.

Abstract

This report presents the first case of Brugada pattern complicated by a supraventricular arrhythmia in a child with SARS-CoV-2 related Multisystem Inflammatory Syndrome in Children (MIS-C). A 7-year-old boy came to our Emergency Department with 7 days of abdominal pain and fever. MIS-C was diagnosed on the basis of the clinical, laboratory and instrumental tests. On admission, ECG showed type 1 Brugada pattern in the right precordial leads. During hospitalization the onset of supraventricular arrhythmias complicated the clinical picture. This case underlines management complexity of supraventricular arrhythmic events, different from atrial fibrillation, in patients with Brugada pattern in the context of a systemic inflammatory condition with significant cardiac involvement. All potential therapeutic choices should be considered to ensure the best outcomes.

Abstract

The investigation of dynamics of the small scale magnetic field on the Sun photosphere is necessary to understand the physical processes occurring in the higher layers of solar atmosphere due to the magnetic coupling between the photosphere and the corona. We present a simulation able to address these phenomena investigating the statistics of magnetic loops reconnections. The simulation is based on N-body model approach and is divided in two computational layers. We simplify the convection problem, interpreting the larger convective scale, mesogranulation, as the result of the collective interaction of convective downflow of granular scale. The N-body advection model is the base to generate a synthetic time series of nanoflares produced by interacting magnetic loops. The reconnection of magnetic field lines is the result of the advection of the magnetic footpoints following the velocity field generated by the interacting downflows. The model gives a quantitative idea of how much energy is expected to be released by the reconfiguration of magnetic loops in the quiet Sun., Comment: 6 pages, 2 figures. Abstract

Multisystem inflammatory syndrome in children (MIS-C) is a rare, severe complication of COVID-19. A better knowledge of immunological, cellular, and genetic characteristics of MIS-C could help better understand the pathogenesis of the disease and contribute to identifying specific diagnostic biomarkers and develop targeted therapies. We studied 37 MIS-C children at hospital admission and 24 healthy controls analyzing serum cytokines (IFN-α, IFN-β, IFN-γ, IL-6, IL-10, IL-17A, IL-12p70 and TNF), lymphocyte populations by flow cytometry and 386 genes related to autoimmune diseases, autoinflammation and primary immunodeficiencies by NGS. MIS-C patients showed a significant increase of serum IFNγ (despite a significant reduction of activated Th1) and ILs, even if with a great heterogeneity among patients, revealing different pathways involved in MIS-C pathogenesis and suggesting that serum cytokines at admission may help to select the inflammatory pathways to target in each patient. Flow cytometry demonstrated a relevant reduction of T populations while the percentage of B cell was increased in agreement with an autoimmune pathogenesis of MIS-C. Genetic analysis identified variants in 34 genes and 83.3% of patients had at least one gene variant. Among these, 9 were mutated in more patients. Most genes are related to autoimmune diseases like ATM, NCF1, MCM4, FCN3, and DOCK8 or to autoinflammatory diseases associated to the release of IFNγ like PRF1, NOD2, and MEF. Thus, an incomplete clearance of the Sars-CoV2 during the acute phase may induce tissue damage and self-antigen exposure and genetic variants can predispose to hyper-reactive immune dysregulation events of MIS-C-syndrome. Type II IFN activation and cytokine responses (mainly IL-6 and IL-10) may cause a cytokine storm in some patients with a more severe acute phase of the disease, lymphopenia and multisystemic organ involvement. The timely identification of such patients with an immunocytometric panel might be critical for targeted therapeutic management.

Abstract

Abstract Background Acute hematogenous osteomyelitis (AHOM) is an insidious infection of the bone that more frequently affects young males. The etiology, mainly bacterial, is often related to the patient’s age, but it is frequently missed, owing to the low sensitivity of microbiological cultures. Thus, the evaluation of inflammatory biomarkers and imaging usually guide the diagnosis and follow-up of the infection. The antibiotic treatment of uncomplicated AHOM, on the other hand, heavily relies upon the clinician experience, given the current lack of national guidelines for the management of this infection. Methods A systematic review of the studies on the empirical treatment of uncomplicated AHOM in children published in English or Italian between January 1, 2009, and March 31, 2020, indexed on Pubmed or Embase search engines, was carried out. All guidelines and studies reporting on non-bacterial or complicated or post-traumatic osteomyelitis affecting newborns or children older than 18 years or with comorbidities were excluded from the review. All other works were included in this study. Results Out of 4576 articles, 53 were included in the study. Data on different topics was gathered and outlined: bone penetration of antibiotics; choice of intravenous antibiotic therapy according to the isolated or suspected pathogen; choice of oral antibiotic therapy; length of treatment and switch to oral therapy; surgical treatment. Conclusions The therapeutic management of osteomyelitis is still object of controversy. This study reports the first Italian consensus on the management of uncomplicated AHOM in children of pediatric osteomyelitis, based on expert opinions and a vast literature review.

Abstract COVID-19 has brought considerable changes and caused critical psychological responses, especially among frail populations. So far, researchers have explored the predictive effect of diverse factors on pandemic-related psychological distress, but none have focused on the impact of prior depression and anxiety symptomatology adopting an extended (10-year) longitudinal design. 105 patients aged over 60, affected by hypertension who participated in a previous longitudinal study were assessed through a follow-up telephone structured interview. The Hospital Anxiety and Depression Scale (HADS) and the Impact of Event Scale-Revised (IES-R) were used for assessing depression and anxiety symptoms and the psychological impact of COVID-19, respectively. Multiple linear regression analyses were conducted. At the assessment, participants did not report clinically relevant depression, anxiety, and psychological pandemic-related distress symptoms. Abstract

In this work, we aim to characterize the effective scale height at the ionosphere F2-layer peak (H0) by using in situ electron density (Ne) observations by Langmuir Probes (LPs) onboard the China Seismo-Electromagnetic Satellite (CSES—01). CSES—01 is a sun-synchronous satellite orbiting at an altitude of ~500 km, with descending and ascending nodes at ~14:00 local time (LT) and ~02:00 LT, respectively. Calibrated CSES—01 LPs Ne observations for the years 2019–2021 provide information in the topside ionosphere, whereas the International Reference Ionosphere model (IRI) provides Ne values at the F2-layer peak altitude for the same time and geographical coordinates as CSES—01. CSES—01 and IRI Ne datasets are used as anchor points to infer H0 by assuming a linear scale height in the topside representation given by the NeQuick model. COSMIC/FORMOSAT—3 (COSMIC—1) radio occultation (RO) data are used to constrain the vertical gradient of the effective scale height in the topside ionosphere in the linear approximation. With the CSES—01 dataset, we studied the global behavior of H0 for daytime (~14:00 LT) and nighttime (~02:00 LT) conditions, different seasons, and low solar activity. Results from CSES—01 observations are compared with those obtained through Swarm B satellite Ne-calibrated measurements and validated against those from COSMIC—1 RO for similar diurnal, seasonal, and solar activity conditions. H0 values modeled by using CSES—01 and Swarm B-calibrated observations during daytime both agree with corresponding values obtained directly from COSMIC—1 RO profiles. Differently, H0 modeling for nighttime conditions deserves further investigation because values obtained from both CSES—01 and Swarm B-calibrated observations show remarkable and spatially localized differences compared to those obtained through COSMIC—1. Most of the H0 mismodeling for nighttime conditions can probably to be attributed to a sub-optimal spatial representation of the F2-layer peak density made by the underlying IRI model. For comparison, H0 values obtained with non-calibrated CSES—01 and Swarm B Ne observations are also calculated and discussed. The methodology developed in this study for the topside effective scale height modeling turns out to be applicable not only to CSES—01 satellite data but to any in situ Ne observation by low-Earth-orbit satellites orbiting in the topside ionosphere.

Abstract

Aims Estimation of left ventricular (LV) filling pressures in patients with a history of atrial fibrillation (AF) is challenging due to lack of reliable parameters. This study investigates the association between cardiac structure and function and invasive mean left atrial pressure (LAP). Methods and results This is a multi-centre prospective study enrolling patients undergoing transcatheter ablation for AF. The invasive measurement of LAP was performed at the time of the procedure while the echocardiography within the previous 24 h. A mean LAP ≥ 15 mmHg was considered as increased. Overall, 101 patients were included (mean age 65.8 ± 8.5 years, 68% male, mean LV ejection fraction 56.6 ± 8.0%). No significant differences regarding clinical characteristics were detected between the group of patients with normal (n = 47) or increased LAP (n = 54). The latter showed lower values of LV global longitudinal strain, larger left atrial volumes (LAVs) and worse right ventricular (RV) function. After multivariable adjustment, higher E/e′ ratio (P = 0.041) and minimal LAV index (LAVI min) (P = 0.031), lower peak atrial longitudinal strain (P = 0.030), and RV free wall longitudinal strain (P = 0.037), but not maximal LAV index (LAVI max) (P = 0.137), were significantly associated with mean LAP. The associations were not modified by cardiac rhythm. Overall, LAVI min showed the best diagnostic accuracy to predict elevated LAP (area under the curve 0.703). Conclusion LA structure and function assessment well correlates with mean LAP in patients with a history of AF. These measures may be used in the assessment of filling pressure in these patients. Abstract

The study of the physical properties of the topside ionosphere is fundamental to investigating the energy balance of the ionosphere and developing accurate models to predict relevant phenomena, which are often at the root of Space Weather effects in the near-Earth environment. One of the most important physical parameters characterising the ionospheric medium is electrical conductivity, which is crucial for the onset and amplification of ionospheric currents and for calculating the power density dissipated by such currents. We characterise, for the first time, electrical conductivity in the direction perpendicular to the geomagnetic field, namely Pedersen and Hall conductivities, in the topside ionosphere at an altitude of about 450 km. For this purpose, we use eight years of in situ simultaneous measurements of electron density, electron temperature and geomagnetic field strength acquired by the Swarm A satellite. We present global statistical maps of perpendicular electrical conductivity and study their variations depending on magnetic latitude and local time, seasons, and solar activity. Our findings indicate that the most prominent features of perpendicular electrical conductivity are located at low latitudes and are probably driven by the complex dynamics of the Equatorial Ionisation Anomaly. At higher latitudes, perpendicular conductivity is a few orders of magnitude lower than that at low latitudes. Abstract Background Substrate analysis of the left atrium in patients undergoing atrial fibrillation ablation has limitations when performed by means of simple bipolar acquisition. Objective To evaluate the incidence of low‐voltage zones (LVZs) through maps constructed by means of various catheters: multipolar (MC), omnipolar (OC), and circular catheters (CMCs) with the 3D electro‐anatomical systems (3d‐S) CARTO3 and EnSite Precision. Methods To assess LVZs, we acquired maps by means of CMC and MC in the voltage range 0.05‐0.5 mV in 70 consecutive patients in sinus rhythm. In the case of OC only, we made an intra‐patient comparison of bipolar maps constructed by means of the along and across, and HD‐Wave configurations of the EnSite 3d‐S in the ranges of 0.05‐0.5 and 0.5‐1.0 mV. On the basis of this comparison, we chose the range that best identified LVZs as a set of different colors (SDC) compatible with patchy fibrosis (qualitative analysis). Subsequently, we detected the voltage values corresponding to purple and gray points, close to SDC, and the value inside corresponding to blue, green, and red colors, and we evaluated the color change in other voltage ranges. Finally, we performed a quantitative analysis of LVZs by applying the qualitative characteristics described above. Abstract

Abstract The present work focuses on the analysis of the scaling features of electron density fluctuations in the mid- and high-latitude topside ionosphere under different conditions of geomagnetic activity. The aim is to understand whether it is possible to identify a proxy that may provide information on the properties of electron density fluctuations and on the possible physical mechanisms at their origin, as for instance, turbulence phenomena. So, we selected about 4 years (April 2014–February 2018) of 1 Hz electron density measurements recorded on-board ESA Swarm A satellite. Using the Auroral Electrojet (AE) index, we identified two different geomagnetic conditions: quiet (AE < 50 nT) and active (AE > 300 nT). For both datasets, we evaluated the first- and second-order scaling exponents and an intermittency coefficient associated with the electron density fluctuations. Then, the joint probability distribution between each of these quantities and the rate of change of electron density index was also evaluated. We identified two families of plasma density fluctuations characterized by different mean values of both the scaling exponents and the considered ionospheric index, suggesting that different mechanisms (instabilities/turbulent processes) can be responsible for the observed scaling features. Furthermore, a clear different localization of the two families in the magnetic latitude—magnetic local time plane is found and its dependence on geomagnetic activity levels is analyzed. These results may well have a bearing about the capability of recognizing the turbulent character of irregularities using a typical ionospheric plasma irregularity index as a proxy.
